Description

図2において、アプライヤー10は、外側チューブ(すなわちシース)18によって保護されたカニューレ13に概ね円筒状に保持された吻合リング装置12を有する。リング装置12は、外側チューブ18によって覆っているが、作動前にノブ19の回動により外側チューブ18が引き戻されて露出される。リング装置12は、中間点が固定チューブ22に結合された成形作動部材20に保持されている。成形作動部材20の先端側にテーパ状先端部24が設けられている。このテーパ状先端部24は、2つの胃腸管の並置された組織壁30及び32を通る吻合開口28を形成するための先端側刺入面26を有する。後述するように、テーパ状先端部24は、半透明な組織壁30及び32を介して基端方向から見た時に、吻合リング装置12の配置及び動作を確認できる照明要素を含むことができる。
図7及び図9において、点線で示された吻合リング装置12の先端側部分が作動状態を例示している。この点線はまた、基端側の視点から配置の状態を確認できる臨床的な利点を提案している。内視鏡では、通常は基端側の位置から吻合開口28を見る。図2−図7に戻ると、アプライヤー10に配置用の照明要素を設けることにより、半透明な組織壁を介して配置を確認することができる。具体的には、照明用の電源(例えば、バッテリ)150及び制御部(例えば、スイッチ)152がハンドル34内に設けられ、捩じられた一対の電線154として示されている導線が、内部チューブ48を通って、基端側を向いたエレクトロルミネセンス装置を含むテーパ状先端部24まで延びている。別法では、導電インクトレースをアプライヤー10の長手方向下側部分に設けて、テーパ状先端部24まで電気回路を形成することもできる。外部からアクセス可能なボタン158を制御部152に向かって押して電源150を起動し、エレクトロルミネセンス装置156を含む照明回路を生成することができる。
(A)吻合リング装置を移植するための外科器具であって、
作動圧縮力に応答して作動していない円筒状の位置と中空リベット形状との間で移行でき、吻合リング装置を保持するように構成された作動部材と、
前記作動圧縮力を生成する作動機構を含むハンドルと、
前記作動圧縮力を前記ハンドルから前記作動部材に伝達するように構成された、前記ハンドルを前記作動部材に連結する細長いカニューレと、
前記作動部材の先端側に結合された刺入先端部と、
を含むことを特徴とする外科器具。
(1)更に、体内の内腔を拡張するために前記先端部と前記ハンドルとを連通させる通気路を含むことを特徴とする実施態様(A)に記載の外科器具。
(2)外科器具であって、2つの内腔の壁部組織まで吻合リング装置を挿入するための手段と、前記吻合リング装置を概ね円筒状から中空リベット形状に変換するための手段とを含むことを特徴とする外科器具。
(3)外科器具であって、カニューレと、概ね円筒状の吻合リングを保持するために前記カニューレの先端部の外側に設けられた作動部材と、前記作動部材の長手方向の端部を前記作動部材の中心に向かって圧迫して保持された前記吻合リングの対応する部分を作動させることができる第1の制御部とを含むことを特徴とする外科器具。
(4)更に、前記作動部材の別の長手方向の端部を前記作動部材の中心に向かって圧迫して保持された前記吻合リングの対応する部分を作動させることができる第2の制御部を含むことを特徴とする実施態様(3)に記載の外科器具。
(5)更に、前記カニューレに対して前記作動部材の前記中心を機械的に固定するための固定部材を含むことを特徴とする実施態様(3)に記載の外科器具。
(7)前記腸切開形成先端具がベレス針を含むことを特徴とする実施態様(4)に記載の外科器具。
(8)更に、前記カニューレを介して先端側に連通した通気路を含むことを特徴とする実施態様(3)に記載の外科器具。
(9)更に、前記通気路に連通したベレス針を含むことを特徴とする実施態様(3)に記載の外科器具。
(10)更に、前記カニューレに連結された照明要素を含むことを特徴とする実施態様(3)に記載の外科器具。

Claims (10)
- 吻合リング装置を移植するための外科器具であって、
作動圧縮力に応答して作動していない円筒状の位置と中空リベット形状との間で移行でき、吻合リング装置を保持するように構成された作動部材と、
前記作動圧縮力を生成する作動機構を含むハンドルと、
前記作動圧縮力を前記ハンドルから前記作動部材に伝達するように構成された、前記ハンドルを前記作動部材に連結する細長いカニューレと、
前記作動部材の先端側に結合された刺入先端部と、
を含むことを特徴とする外科器具。 - 前記ハンドルが更に、長手方向基端側への動き及び長手方向先端側への動きを生成して前記作動圧縮力を生成するように機能的に構成されており、前記細長いカニューレが、前記作動部材の先端側部分及び基端側部分のそれぞれに対して前記基端側への動き及び前記先端側への動きを個別に伝達できるように機能的に構成されていることを特徴とする請求項1記載の外科器具。
- 前記細長いカニューレが、前記作動部材の前記基端側部分に連結された第1のチューブと、前記第1のチューブにスライド可能に受容され、前記作動部材の前記先端側部分に連結された第2のチューブとを含むことを特徴とする請求項2記載の外科器具。
- 前記細長いカニューレが更に、前記第1のチューブと前記第2のチューブとの間に配設された、先端側が前記作動部材の中心部分に結合された第3のチューブを含むことを特徴とする請求項3記載の外科器具。
- 前記刺入先端部が腸切開形成先端部を含むことを特徴とする請求項1記載の外科器具。
- 前記刺入先端部がベレス針を含むことを特徴とする請求項5記載の外科器具。
- 更に、前記作動部材の前記先端側部分に近接して連結された照明源を含むことを特徴とする請求項1記載の外科器具。
- 前記作動部材が光透過材料を含むことを特徴とする請求項7記載の外科器具。
- 前記作動部材がエレクトロルミネセント材料を含むことを特徴とする請求項7記載の外科器具。
- 更に、体内の内腔を拡張するために前記先端部と前記ハンドルとを連通させる通気路を含むことを特徴とする請求項1に記載の外科器具。
