(2)ジーン・ボープレ(Jean Beaupre)による米国特許出願第10/675,091号、名称「アンフォールディング吻合用リング装置(Unfolding Anastomosis Ring Device)」
(3)マーク・オルティス(Mark Ortiz)による米国特許出願第10/675,705号、名称「内腔間吻合のための単一内腔配置用リング(Single Lumen Access Deployable Ring for Intralumenal Anastomosis)」

図2において、アプライヤー10は、テーパ状先端部24を有するカニューレ22をなす成形作動部材20に対して概ね円筒状に維持された、外側チューブ18の先端側のリング装置12を有する。このテーパ状先端部24は、2つの胃腸管の並置された組織壁30及び32を通る吻合開口28を形成するための先端側刺入面26を有する。後述するように、テーパ状先端部24は、半透明な組織壁30及び32を介して基端方向から見た時に、吻合リング装置12の配置及び動作を確認できる照明要素を含むことができる。
図7及び図9において、点線で示された吻合リング装置12の先端側部分が作動状態を例示している。この点線はまた、基端側の視点から配置の状態を確認できる臨床的な利点を提案している。内視鏡では、通常は基端側の位置から吻合開口28を見る。図2−図7に戻ると、アプライヤー10に配置用の照明要素を設けることにより、半透明な組織壁を介して配置を確認することができる。具体的には、照明用の電源(例えば、バッテリ)150及び制御部(例えば、スイッチ)152がハンドル34内に設けられ、捩じられた一対の電線154として示されている導線が、内部ロッド48を通って、基端側を向いたエレクトロルミネセンス装置156を含むテーパ状先端部24まで延びている。別法では、導電インクトレースをアプライヤー10の長手方向下側部分に設けて、テーパ状先端部24まで電気回路を形成することもできる。外部からアクセス可能なボタン158を制御部152に向かって押して電源150を起動し、エレクトロルミネセンス装置156を含む照明回路を生成することができる。
連続的なワイヤループを備えた吻合リング装置の形成には、編みワイヤストランドの端部の接合や連続的なワイヤループから編み構造を形成することを含め製造上の問題がある。図12−図17に示されているように、吻合リング装置212を製造する有利な方法には、ワイヤストランド216のそれぞれにボール端部214を設けることが含まれる。例示的な実施形態では、各ボール端部214にレーザーで孔を開け、ボール端部214をワイヤストランド216にクリンプする。ボール端部214は、編みストランド216によって形成されたペタル218がほぐれるのを防止する役割を果たす。加えて、ボール端部214により、図17に示されているように外傷を与えないで組織壁220に接触できる。
図29−図30に示されているように、吻合リング装置512は、SME金属(例えば、ニチノール)またはばね鋼から形成された螺旋ワイヤ補助ばね530を含む。従って、吻合リング装置512のステント部分532の編み材料は、作動させるためにSME材料とする必要がない或いは少なくともSME特性に完全に依存する必要がない。螺旋ワイヤ補助ばね530により、ステント部分532の所望のワイヤ太さ及び所望の材料の選択が可能となる。例えば、ステント部分532は、自己作動能力を有していない連続的に編まれたワイヤブレードの長手方向にカットされ分離した部分またはプラスチックからも形成することができる。
一般的に知られているニチノールリング装置600は、徐々に閉じていくループペタル602を含む。ループペタル602の先端部は、図31及び図32に示されているように、拘束された概ね円筒状の状態では長軸に対して横方向に広がり、解放されて作動した状態では中に入り込む。このように湾曲したペタル602は、組織壁に係合する利点があると考えられている。しかしながら、それぞれのペタル602の外向きの傾斜が増大すると、従来のニチノールリング装置600の自己作動に対する抵抗が大きくなり、組織壁への係合の利点が打ち消され、ニチノールリング装置600の自己作動により大きな力が必要となる。
(1)前記各ペタルの先端部分が、径方向に狭まった先端部分及び径方向に広がった基端部分を含むことを特徴とする請求項10に記載の吻合装置。
(2)更に、前記編みチューブが前記作動していない位置から前記作動した位置にする作動の力を付与する、前記編みチューブに結合された螺旋ばねを含むことを特徴とする請求項10に記載の吻合装置。
(3)前記螺旋ばねが螺旋状に形成されたばね鋼ストランドを含むことを特徴とする実施態様(2)に記載の吻合装置。
(4)前記螺旋ばねが形状記憶効果材料を含み、前記少なくとも1本のワイヤストランドがそれぞれ、取り付けられていない端部を含むことを特徴とする実施態様(2)に記載の吻合装置。
(5)前記編みチューブが、取り付けられていない端部を有する少なくとも1本のストランドを含むことを特徴とする請求項10に記載の吻合装置。
(7)前記取り付けられていない端部がそれぞれループで終わっていることを特徴とする実施態様(5)に記載の吻合装置。
(8)前記少なくとも1本のワイヤストランドの少なくとも1つの取り付けられていない端部が、前記編みチューブが前記作動した位置で前記吻合外科部位の組織壁に係合するようにフックで終わっていることを特徴とする請求項10に記載の吻合装置。

Claims (1)
- 吻合装置であって、
少なくとも1本のワイヤストランドからなる編みチューブを含み、
前記編みチューブのそれぞれの長手方向の端部が外周ペタルで終わっており、
前記編みチューブが、挿入するための概ね円筒状の作動していない位置と、吻合外科部位に2つの近接した組織壁間に吻合結合を形成するための中空リベット形状の作動した位置とを含み、
前記少なくとも1本のワイヤストランドのそれぞれが、他のワイヤストランドの端部に取り付けられていない端部を含み、
前記少なくとも1本のワイヤストランドの前記取り付けられていない端部の少なくとも1つがボールで終わっていることを特徴とする吻合装置。
