背景技术
世界上患病态肥胖症的人口比例与日俱增。严重肥胖者遭受心脏病、中风、糖尿病、肺病和意外事件的风险增大。因为病态肥胖症影响患者的寿命,所以人们正在研究治疗肥胖症的方法。
实际上病态肥胖症的许多非手术治疗都不能取得长期的效果。节食、动作矫正、用线缝合患者的下颌以及药理学方法都已被尝试,尽管暂时有效,但都不能治愈该疾病。还在胃里引进了一种物体,如食管-胃橡皮球,填充到胃部来治疗疾病;但是该方法易导致刺激胃,且不能长期有效。
对于病态肥胖症的外科治疗已经取得了巨大成功。这些方法归纳起来就是缩小胃的有效尺寸,限制食量,以及产生对所吃食物的吸收障碍。比如,病人可以使用可调束胃带(adjustable gastric band,简称AGB),它可以方便地借助腹腔镜置于胃附近,形成所需尺寸的口,使食物填充在胃的上部,并带来饱腹感。在埋入手术后,为了调节口的大小,在AGB的内部流体囊状物和置于患者腹板前面皮下的流体注入口之间有一个流体导管贯通。之后用灌肠针按需要注入或抽出流体以调节AGB。
尽管治疗肥胖症的方法对一些人来说有效,但是对于另一些患者来说会不尽人意地改变其生活方式,被迫限制其食量。另外患者的身体状况需要一种更能长期有效的解决方法。为了这一目的,就需要用外科手术的方法来改变用以消化食物的胃和/或小肠部分。目前进行胃绕道术的借助腹腔镜的吻合方法包括:钉皮术、缝合术和放置生物可碎环,每一步都具有挑战性。比如,缝合术消耗时间,且需要技巧和依靠灵活性。钉皮术需要放置压砧,该压砧为一较大的装置,所以不能通过套管针端口导入。只能通过剖腹手术来引入端口,该手术则存在伤口感染风险加大的问题,因为要将管腔容纳物拉到剖腹术的入口位置。
作为后一种方法的例子,美国专利US6543456所述的胃分流外科手术包括用夹钳穿过口腔将吻合部件(如压砧)的近端和远端插入。胃和小肠通过内窥镜被外科切割和装订工具横切,从而产生一个胃囊、一个引流回路引流回路和一个Roux分支。将一个通过内窥镜插入的环形装订器连接在吻合部件的远端,从而将引流回路连接在小肠的远端,而环形装订器连接在吻合部件的近端,以将Roux分支连接在胃囊上。其后,去掉吻合部件从而在胃的连接部分和小肠之间产生开口。该方法减少了剖腹开口的数量,避免了将吻合装置(如环形装订器)通过借助腹腔镜插入的较大的外科手术开口,且不需要切肠术和切肠后的缝合。
虽然上述方法相对于治疗病态肥胖症的已知胃绕道术和类似外科手术治疗取得了显著的进步,人们仍然希望以较少的步骤和较少的腹腔镜插入次数来完成胃绕道术。该方法在Park等人的美国专利申请US2003/0032967中有说明,其中通过插入穿过两组织如胃和小肠的通道壁的保护套来完成肠胃或肠(包括胆)的吻合。在保护套套管两侧的开口上具有一带热形状记忆效果(Shapememory effect,简称SME)的金属丝三维织管。在袖口状动脉分流术中灌肠器SME材料曾经被实践过,如美国专利US5676670、US5797920和US6007544中所述。使用织管导致管的外环和端部折回,保持吻合位置与管腔界面并置。因此就减少或避免了在传输系统中使用机械的压缩构件,减小了传输装置的尺寸和复杂性。
尽管该已知环装置在病态肥胖症的治疗中取得了显著的进步,但是需要在临床效果上取得还的提高。特别地,已知的环装置为声称是自激发吻合环的织管或帐状物。这样插入已知环装置的所述灌肠器只是将环装置置于进行吻合的位置,并且通过离开套管而展开该环装置,依靠环装置的SME的特性来进行激发。遗感地是已知环装置有时不能进行激发并从压缩圆柱状变到松开的夹子状,可能是因为其编织设计产生的摩擦波动的不规则性所致。已知SME吻合环的一个缺点是,它们设计成通过形成装置的金属丝间的交叉移动而从通常的圆柱状转换成空心铆钉状(“环状”)。特别地,金属丝必须在金属丝弯曲形成的节点(即凹槽或沟部)内移动,并且必须从凹槽攀升回来。在有些情况下,该装置因为缺少摩擦源而不能充分自激发。
虽然该环装置还需要改进,但能够缓解已知环装置该缺点的是一个外科手术工具或者是一个灌肠器,它可以确定地并快速地在吻合位置形成连接,而不需要等到SME的缓慢激发形成连接。该灌肠器可以方便地形成一个单管腔通道,而不象先前已知的吻合需要插入压砧和环形装订器。
另外,必须依靠SME的激励足够大以便从压缩的、未激发状态转换到松开的、激发状态,这就限制了所选择材料的性质和尺寸范围。比如,较细的金属丝束可以方便地提供足够的保持力,直到吻合连接痊愈,而且还需要很容易地将当前已不需要的环形装置从患者身上取下。然而,该细金属丝束将不能把足够的SME力用于克服内部摩擦,并在激发的过程中将适当的组织壁拉在一起。
再者,当前使用的单管腔吻合的问题在于,临床效果要好并缩短患者的康复时间,使吻合的位置接近两个相邻组织通道(比如胃和小肠)的适当组织壁对中的一侧。然而当用内诊镜和类似光学成像装置观察时,只有组织壁中的一个和展开的环装置的近端在该优势点可以看到。可以确保有非常好的吻合连接。
因此,需要一种在缝合很少的情况下使用现有套针端口(例如,12mm大小)进行吻合的方法。另外,该方法还可以用在内窥镜外科手术中。为此,迫切需要吻合装置的可靠有效展开和激发,从而在进行吻合中不再需要钉皮和缝合。
发明内容
本发明克服了已有技术的上述和其它不足之处。
一方面,本发明提供了一种外科手术器械,它包括一个激发件,该激发件可以在一个圆柱状未激发状态和一个空心铆钉状状态之间活动,从而插入一个吻合环装置。该器械的把手具有激发机构,该激发机构产生一个加压的激发力,该力被一个长的套管向下传递来激发激发件,而套管在远端支撑着激发件。因此,该吻合环装置可以确保放对位置,还不需要单独依赖于一个环装置的自激发能力。
本发明的这些和其它目的与优点通过附图和说明就会清楚明了。

具体实施方式
回到附图,其中在几个附图中,相同的附图标记表示相同的部件,图1描述了一种灌肠器10,其方便地通过腹腔镜或内诊镜展开并且激发吻合环装置12,使其从常见圆柱状转换到中空铆钉状或环状,以在吻合目标位置处形成吻合连接,如在肥胖患者16的治疗肥胖的胃绕道术中。在说明方案中,吻合环装置12包括形状记忆(SME)材料,如镍钛合金,其还能对激发进行辅助作用以形成能啮合的空心铆钉状。
应该理解的是在这里所使用的特征“近端的”和“远端的”是参照临床医生抓紧的灌肠器10的把手而言。还应该理解,为了更方便和清楚,在这里所使用的空间词汇“右”、“左”、“垂直”、“水平”均相对于附图而言。但是,在许多方向和位置中使用外科器械,这些词汇并不是限制性和绝对的。另外,本发明的特征可以应用于借助内诊镜和腹腔镜的外科手术过程和通肠手术过程。不能将在这里使用的这些或相似词汇中的一种解释为限制本发明只能在外科手术的某一种中使用。
吻合环装置灌肠器:
在图2中,灌肠器10具有吻合环装置12,其在套管13上最好形成大致的圆柱形状,套管13由一个外管(或保护套)18保护,该外管覆盖环装置12,直到在激发之前转动旋钮19、缩回外管18以便暴露出环装置12。环装置12位于一个模制激发件20上,该模制激发件20在中点处连接在一固定管22上。模制激发件20的远端是一个锥形尖端24。该锥形尖端24具有帮助在胃和肠的两个通道的适当组织壁30、32形成吻合开口28的一个远端穿刺面26。如下所述,锥形尖端24可具有照明部件,用以通过半透明的组织壁30、32从近端的方向观察时,确保吻合环装置12的放置和激发。
模制激发件20可以形成常见的矩形件,矩形件包围一个轴柄。之后将到一起的矩形件的纵向边粘合或熔接在一起。或者,可以在每个纵向端(即近端的和远端)和中点上连接扣环,从而将模制激发件20固定在一起。
参照附图2-5,近端套管13的把手34包括一对纵列的触发器36、38。近端的触发器36通过套管13的中间套管42与模制激发件20的近端的叶片配合,其中所示的该近端的触发器36是处于近端的、未触发的位置。这样,近端触发器36向远端的移动就会引起中间套管42和近端叶片40沿纵向向远端运动,通过与模制激发件20的中心部分44的悬臂、铰接关系,叶片40象伞一样向外动作,模制激发件20反过来连接到从中间套管42中伸出的固定管22的端部。相似的,远端触发器38通过内管48与模制激发件20的远端叶片46配合,其中所示的该远端触发器38是处于最远端的、未触发的位置,该内管48在固定管22中配合移动并可从固定管22远端移出。远端触发器38向近端的移动就会引起模制激发件20的内管48和远端叶片50沿纵向向近端运动,该叶片通过与中心部分44的铰接关系而向外激发。
需要理解的是这样触发器36、38任一个都可以独自移动,来激发模制激发件20的一个近端或远端部分。这样近端叶片40可以在一个近端管腔中激发,从而在将锥形尖端24插入远端管腔之前,利用套管13将近端管腔定位在远端管腔上。或者,可以将套管13插入远端管腔中,远端叶片46被激发,而远端管腔则缩回并与近端管腔接触。
通过将锥形尖端24和远端穿刺面26以一个veress针的形式结合,就可以将套管13插入远端管腔并随后将套管13从管腔的远端和近端取出更加方便,就避免了因疏忽对组织造成伤害,并可以方便地膨胀管腔,也使。vress针有一个注射器刀尖,一个球体在注射器刀尖中平移。当veress针26压在组织壁30、32上时,球体就会弹性地缩入veress针26,暴露出穿刺面。一旦穿透,组织远端通常不能在张力作用下定位,并且不能通过扩张球体来移动,而是引起创伤地碰到刀尖。在扩张时,veress针26可以方便地通过一个外侧的开口同中空的内管48空气相通。这样,气压就被导入管腔的近端和远端,从而在不受塌陷组织的阻挡的情况下激发激发件,且激发件的缩回也不存在障碍。在把手34上与veress针26连通的空气开口提供了用于吹入空气的开口。对于相对小的开口(如1/16英寸),该开口可以在不用时保持打开,而不会有大量的空气通过器械10泄漏。
如图3所示的剖视图,锥形尖端24还还包括一个远端保护套51,在将激发件20插入和取出患者的过程中,它与外管18一起隔离了激发件20。
最好参照图4-5,腔体52包括近端和远端的孔54、56,以分别允许近端和远端的触发器36、38的纵向移动。每个触发器36、38均包括右开孔58,其可以沿向左伸出的轨道60纵向移动,该轨道形成于把手34的右半壳的腔体52中。
从最远端移动到最近端,横贯腔体52底部的第一、第二和第三侧面突起物62、64、66就分别限定了第一、第二、第三和第四腔体段68、70、72、74。配置第一块体76以使其在第一腔体段68中移动,该块体由左右两半78、80形成。纵向中心孔82与中间套管42的终止近端相配合并与其一起移动,该纵向中心孔82位于两半78、80之间。
固定管22穿过中间套管42进入第二腔体段70。第二间隔块体90具有纵向中心孔92,该间隔块体固定在第二腔体段70中,该孔位于与固定管22配合的左右两半94、95之间,相对把手34锁定固定管22。
内管48从固定管22的近端穿出,穿过第三和第四腔体段72-74,与孔86滑动接触,孔86贯穿把手34的近端88。第三块体96具有限定在它的上下半100、102之间的一个纵向中心孔98,上下半100、102与内管48配合并一起移动。远端触发器38的下部104附着在第三滑动块体96的远端表面上。位于第四腔体段74中的第四滑动块体106具有一个纵向中心孔108,该孔滑动接触内管48。近端触发器34的下部114附着在第四滑动块106的近端表面上。从第一滑动块76的左侧到第四滑动块106的左侧连接有一个联杆116。

在使用中,将灌肠器10的锥形尖端24嵌入并穿过套针管端口,进入组织通道,将该组织通道放置在接近另一组织通道的位置,二者吻合连接(见图1-2)。将锥形尖端24和模制激发件20的远端部分以及吻合环装置12插入到吻合开口28中,该开口形成于它们之间,然后激发该灌肠器,将在图7-8中描述部分激发的灌肠器10。为了更方便地定位远端和近端管腔,分开激发激发件20的两个部分,并且通过在器械10中通入压缩空气来使管腔膨胀。特别参照图8,示出近端和远端叶片40、50具有夹紧槽118,特别是在其未激发的、大致为圆柱状状态下,分别夹紧瓣状物120。在夹紧槽118中的向内的保持尖端121或其它夹紧部件相互结合以提高保持力。这些夹紧槽118有助于防止在激发时吻合环装置12从灌肠器10中滑出或是与在灌肠器上的位置不合适。在图9-10中,已经将灌肠器10完全激发,将吻合环装置12形成为空心铆钉状,以在组织壁30、32之间形成吻合连接。完全激发的近端和远端叶片40、50会使瓣状物120与夹紧槽118之间的配合断开。其后,灌肠器10就回复到未激发的状态,而已激发的吻合环装置12就随着锥形尖端24从吻合开口28和环装置12上抽出而展开,如图11所示。
展开照明:

作为实现本发明的等效结构的例子,可以使用液压、电力或者气动来相对把手34移动套管13。具有电子设备和反馈回路的计算机控制可以用来移动管14或者是基于所施加组织力的大小选择性地张紧受力件。作为实现本发明的等效结构的另一个例子,可以使用机器人技术,将吻合装置连接在受控制的机械臂上,机械臂移动吻合装置10的器械来影响吻合术。

作为实现本发明的等效结构和方法的另一个例子,灌肠器10的几何大小足以穿过一个人工(hand)开口来进行放置,该指针开口用于人工辅助腹腔镜外科手术,比如由俄亥俄州的辛辛那提的Ethicon Endo-Surgery出售的Lap-Disk牌人工开口。为了看得清楚,一个穿过人工开口使用灌肠器10的外科医生可能会穿过一个第二开口来使用一个内诊镜,也可能保持腹腔积气。在进行外科手术如吻合术的过程中,外科医生还可能使用套管针、抓具、刀具和其它内腔镜器械来帮助抓住管腔或者在管腔中生成otomies。
作为实现本发明的等效结构和方法的再一个例子,当外科医生用手来贯穿一个人工开口放置灌肠器10时,一个长的、硬的灌肠器10,或者是一个长的、柔性的灌肠器10可以穿过一个附加开口。
作为等效结构的其它例子,锥形尖端24的表面可以具有有利于不同组织手术的许多形状,比如圆锥形尖鼻状结构,它对小的组织外伤来说很钝,且对于远端来说具有较好的可见性。作为另一个例子,可以在较短的空间内使用一个otomy可迅速扩张的具有凹曲线面的一个鼻状结构,或者是在脆性组织中使用可以缓慢扩张的具有凹面的鼻状结构。可以使用一种偏移弯曲鼻状结构,因为它的不对称可以在一侧更好地看到,还可用于辅助操作,通过其不对称性可以最低限度地抓住组织。还可使用一种球形鼻状结构,可以在有限的空间内进行短距离操作,且减少组织受到创伤的机会。这些表面的结合也是比较有利的,比如具有凹面的鼻状结构也可能具有凹槽。其它的结合对本领域普通技术人员来说也是存在的。
Claims (10)
1.一种插入吻合环装置的外科手术器械,包括:
一个激发件,该激发件用来容纳一个吻合环,该激发件还可以在一个圆柱状未激发位置和一个空心铆钉状位置之间活动;
一个把手,具有激发机构,该激发机构产生一个加压的激发力;
一个长的套管,该套管将手柄连接在激发件上,并将加压的激发力从手柄传递到激发件。
2.如权利要求1所述的外科手术器械,其特征在于:所述把手还用以用过纵向向近端的运动和纵向向远端的运动来产生所述加压的激发力,该长的套管可以将向近端和远端的运动分别传递到激发件的远端部分和近端部分。
3.如权利要求2所述的外科手术器械,其特征在于:所述长的套管具有连接在激发件近端部分的一个第一管和一个可滑动容纳在第一管中并连接在激发件远端部分的第二管。
4.如权利要求3所述的外科手术器械,其特征在于:所述长的套管还具有插入第一和第二管之间的一个第三管,并在远端与激发件的中心部分配合。
5.如权利要求1所述的外科手术器械,其特征在于:还包括一个在远端连接在激发件上的一个穿刺尖端。
6.如权利要求5所述的外科手术器械,其特征在于:所述穿刺尖端包括一个切肠尖端。
7.如权利要求6所述的外科手术器械,其特征在于:所述穿刺尖端包括一个veress针。
8.如权利要求1所述的外科手术器械,其特征在于:还包括一个连接在接近激发件远端的一个照明光源。
9.如权利要求8所述的外科手术器械,其特征在于:所述激发件具有一个光传导材料。
10.如权利要求8所述的外科手术器械,其特征在于:所述激发件具有一个电发光材料。
