Description
本発明は、慢性の痛みなどの疾患を治療するための患者の脊椎への電極リードのインプランテーションに関する。
脊髄、脊椎神経根、その他の神経束を電気的に刺激することによって慢性の痛みを治療することが知られている。典型的な手順では、蛍光透視を行いながら、一またはそれ以上の刺激リードを患者の背中から硬膜外スペースへ導入する。現在、二つのタイプの商業的に入手可能な刺激リードがある。経皮的リードと、外科的リードである。経皮的リードは、リング状電極を有する筒状ボディを具え、トウーイ状針を介して痛みのある脊椎組織に接触させるよう導入できる。この針は、皮膚を通り、所望の椎骨間を通って、硬膜層の上の脊椎腔内に入る。片側だけの痛みに対しては、脊髄の対応する片側に経皮的リードを配置する。両側の痛みに対しては、脊髄の中線の下に経皮的リードを配置するか、2本の経皮リードを中線の両側に配置する。外科的リードは、多数の電極を個別コラムに配置したパドルを有し、特に椎弓切除術などの外科的手順を用いて痛みのある脊髄組織に接触させるように導入する。この椎弓切除術は、薄層でできた椎骨組織を除去して、硬膜層にアクセスし、リードを配置する手順を含む。
本発明の一の実施例では、第1及び第2の組織刺激リードを有する刺激キットが提供されている。第1の刺激リードは、第1の細長ボディと、第1の刺激エレメント(例えば、電極)と、前記第1の細長ボディの少なくとも一部に沿って縦方向に延在する第1の接続機構を具える。第2の刺激リードは、第2の細長ボディと、第2の刺激エレメント(例えば、電極)と、例えばレールや、スロット構成などで前記第1の接続機構を摺動可能に係合するように構成した第1の相補接続機構とを具える。刺激キットは、選択的に、第1及び第2の刺激リードに接続するように構成された刺激源を具える。選択的に、各刺激リードはより広い刺激カバレージを提供するために、複数の刺激エレメントを具える。
図1を参照すると、本発明の一実施例によって構成したモジュラ刺激リードキット100は、一般的に、患者の脊椎の硬膜外スペース中に経皮的に送出され、インプラントされるように構成した主刺激リード102と2本の二次刺激リード104と、前記刺激リード102/104に刺激エネルギィを送出するように構成されたインプラント可能な電気的刺激源106と、刺激リード102/104を遠隔にインプラントされた刺激源106に接続するように構成された追加エクステンションリード108と、を具える。以下に更に詳細に述べるように、二次刺激リード104は、図2に示すように、主刺激リード102に取り付けて、モジュール化した刺激リードアッセンブリ110を形成できる。
Claims (29)
- 第1の細長ボディと、第1の刺激エレメントと、前記第1の細長ボディの少なくとも一部に沿って縦方向に延在する第1の接続機構を具える第1の組織刺激リードと;
第2の細長ボディと、第2の刺激エレメントと、前記第1の接続機構と摺動可能に係合するように構成された第1の相補接続機構とを具える第2の組織刺激リードと;
を具えることを特徴とする刺激キット。 - 請求項1に記載の刺激キットにおいて、前記第1及び第2の細長ボディの少なくとも一方が円筒形状であることを特徴とする刺激キット。
- 請求項1又は2に記載の刺激キットにおいて、前記第1及び第2の細長ボディの少なくとも一方の最大断面寸法が5mm以下であることを特徴とする刺激キット。
- 請求項1ないし3のいずれかに記載の刺激キットにおいて、前記第1及び第2の刺激エレメントが電極であることを特徴とする刺激キット。
- 請求項1ないし4のいずれかに記載の刺激キットにおいて、前記第1及び第2の刺激エレメントが、前記第1及び第2の細長ボディ上にそれぞれ装着されていることを特徴とする刺激キット。
- 請求項1ないし5のいずれかに記載の刺激キットにおいて、前記相補接続機構が前記接続機構に摺動可能に係合するときに、前記第1及び第2の刺激エレメントが、単一の方向を向くように構成されていることを特徴とする刺激キット。
- 請求項1ないし6のいずれかに記載の刺激キットにおいて、前記第1及び第2の刺激リードの各々が、複数の刺激エレメントを具えることを特徴とする刺激キット。
- 請求項1ないし7のいずれかに記載の刺激キットにおいて、前記接続機構と前記相補接続機構が、レールまたはスロット構造において互いに摺動可能に係合するように構成されていることを特徴とする刺激キット。
- 請求項1ないし8のいずれかに記載の刺激キットにおいて、前記相補接続機構が、前記第2の細長ボディの遠位部分のみに沿って延在することを特徴とする刺激キット。
- 請求項1ないし9のいずれかに記載の刺激キットにおいて、前記第2の細長ボディが前記第1の細長ボディより短いことを特徴とする刺激キット。
- 請求項1に記載の刺激キットにおいて、前記第2の細長ボディが、前記接続機構から前記相補接続機構の少なくとも一部を摺動可能に係合を解くことによって、前記第1の細長ボディから展開するように構成されていることを特徴とする刺激キット。
- 請求項11の刺激キットにおいて、前記第2の細長ボディが予め曲がっていることを特徴とする刺激キット。
- 請求項11に記載の刺激キットにおいて、前記第2の細長ボディが、前記第1の細長ボディから展開した後、第1のジオメトリから第2のジオメトリへ能動的に変化するように構成されていることを特徴とする刺激キット。
- 請求項13に記載の刺激キットが更に、前記第2の細長ボディを介して挿入して、前記第2の細長ボディを前記第1のジオメトリから前記第2のジオメトリに変えるように構成したスタイレットを具えることを特徴とする刺激キット。
- 請求項13に記載の刺激キットにおいて、前記第2の刺激リードが、前記第2の細長ボディを前記第1のジオメトリから前記第2のジオメトリに変えるために引っ張られるように構成したプルワイヤを具えることを特徴とする刺激キット。
- 請求項1に記載の刺激キットにおいて、前記相補接続機構は、前記第2の細長ボディの一部に沿って延在しておらず、前記第2の細長ボディが、前記第1の細長ボディから前記第2の細長ボディの一部を曲げることによって、前記第1の細長ボディから展開するように構成されていることを特徴とする刺激キット。
- 請求項1に記載の刺激キットにおいて、前記第2の刺激リードが、各刺激エレメントを配置したフラップを具え、当該フラップが前記相補接続機構の一部に沿って延在しており、前記相補接続機構の部分が前記接続機構に摺動可能に係合するときに、前記接続機構によって固定され、前記相補接続機構の部分が前記接続機構を摺動可能に係合を解くときに、前記接続機構によって開放されるように構成されていることを特徴とする刺激キット。
- 請求項1に記載の刺激キットにおいて、前記第1の刺激リードが、前記第1の細長ボディの少なくとも一部に沿って縦方向に延在する別の接続機構を具え、前記刺激キットが更に、第3の細長ボディを具える第3の刺激リードと、第3の刺激ボディに装着した刺激エレメントと、前記別の接続機構を摺動可能に係合するように構成した別の相補接続機構と、を具えることを特徴とする刺激キット。
- 請求項1ないし18のいずれかに記載の刺激キットが更に、前記第1及び第2の刺激リードに接続するように構成された刺激源を具えることを特徴とする刺激キット。
- 請求項18に記載の刺激キットにおいて、前記相補接続機構が前記接続機構に摺動可能に係合し、前記別の相補接続機構が前記別の接続機構に摺動可能に係合するときに、前記第1、第2、及び第3の刺激エレメントが、単一の方向に向くように構成されていることを特徴とする刺激キット。
- 請求項18に記載の刺激キットにおいて、前記接続機構と前記相補接続機構が、レールまたはスロット構造において互いに摺動可能に係合するように構成され、前記別の接続機構と前記別の相補接続機構が、レールまたはスロット構造において互いに摺動可能に係合するように構成されていることを特徴とする刺激キット。
- 請求項18に記載の刺激キットにおいて、前記相補接続機構が、前記第2の細長ボディの遠位部分のみに沿って延在し、前記別の相補接続機構が、前記第3の細長ボディの遠位部分のみに沿って延在することを特徴とする刺激キット。
- 請求項18に記載の刺激キットにおいて、前記第2の細長ボディが前記第1の細長ボディより短く、前記第3の細長ボディが前記第1の細長ボディより短いことを特徴とする刺激キット。
- 請求項18に記載の刺激キットにおいて、前記第2の細長ボディが、前記接続機構から前記相補接続機構の少なくとも一部を摺動可能に係合を解くことによって、前記第1の細長ボディから展開するように構成され、前記第3の細長ボディが、前記別の接続機構から前記別の相補接続機構の少なくとも一部を摺動可能に係合を解くことによって、前記第1の細長ボディから展開するように構成されていることを特徴とする刺激キット。
- 請求項24の刺激キットにおいて、前記第2の細長ボディが予め曲がっており、前記第3の細長ボディが予め曲がっていることを特徴とする刺激キット。
- 請求項24に記載の刺激キットにおいて、前記第2の細長ボディが、前記第1の細長ボディから展開した後、第1のジオメトリから第2のジオメトリへ能動的に変化するように構成され、前記第3の細長ボディが、前記第1の細長ボディから展開した後、第1のジオメトリから第2のジオメトリに能動的に変化するように構成されていることを特徴とする刺激キット。
- 請求項26に記載の刺激キットが更に、前記第2の細長ボディ及び前記第3の細長ボディを介して挿入して、前記第2の細長ボディ及び前記第3の細長ボディを前記第1のジオメトリから前記第2のジオメトリに変えるように構成したスタイレットを具えることを特徴とする刺激キット。
- 請求項26に記載の刺激キットにおいて、前記第2の刺激リードが、前記第2の細長ボディを前記第1のジオメトリから前記第2のジオメトリに変えるために引っ張られるように構成したプルワイヤを具え、前記第3の刺激リードが、前記第3の細長ボディを前記第1のジオメトリから前記第2のジオメトリに変えるために引っ張られるように構成したプルワイヤを具えることを特徴とする刺激キット。
- 請求項18に記載の刺激キットにおいて、前記相補接続機構は、前記第2の細長ボディの一部に沿って延在しておらず、前記第2の細長ボディは、前記第1の細長ボディから前記第2の細長ボディの一部を曲げることによって、前記第1の細長ボディから展開するように構成され、前記別の相補接続機構は、前記第3の細長ボディの一部に沿って延在しておらず、前記第3の細長ボディは、前記第1の細長ボディから前記第3の細長ボディの一部を曲げることによって、前記第1の細長ボディから展開するように構成されていることを特徴とする刺激キット。
