Description
本開示は、一般に、心臓外科バイパスシステムに関し、より詳細には、低侵襲的直接冠動脈バイパス外科手術(MIDCAB:minimally invasive direct coronary artery bypass surgery)を計画するための心臓撮像システム及び方法に関する。
米国心臓協会(American Heart Association)の統計によれば、米国だけで毎年500,000件を超える冠動脈バイパス移植(CABG:coronary artery bypass graft)が実施される。冠動脈の疾患では、心筋に血液を運ぶ動脈(すなわち、冠動脈)が、脂肪の蓄積であるプラークによって塞がった状態となる。CABG手術の期間、心臓への血流及び酸素を改善するために、血液は、塞がった動脈の周りで経路変更される。左内胸動脈などの健康な血管が胸壁から切り離され、その後、閉塞した領域を迂回するために使用される。あるいは、また、脚部の静脈の一部をCABGに使用することもできる。血管/静脈の一端は、大動脈(心臓から出ていく大きな動脈)上に縫い付けられ、他端は、閉塞した領域を越えたところで(又は過ぎたところに、又は遠位において)冠動脈に取り付けられ、又は「移植」される。患者は、同時に複数のバイパスを受けることができる。
前述の及びその他の従来技術の欠点ならびに欠陥は、患者のために低侵襲的直接冠動脈バイパス外科手術(MIDCAB)を計画する方法によって克服又は軽減される。例示的な一つの実施の形態では、該方法には、医用撮像システムから獲得データを取得するステップと、関心のある冠動脈及び1つ又は複数の心腔の3Dモデルを生成するステップとが含まれる。3Dモデル上で1つ又は複数の解剖学的ランドマークが識別され、セーブされた3Dモデルのビューが介入システム上で登録される。登録された1つ又は複数のセーブされたビューが介入システムによって視覚化される。
本明細書では、医療従事者(例えば、電気生理学者、心臓病学者、外科医)が処置のために取るべきアプローチを予め計画できるように、低侵襲的直接冠動脈バイパス外科手術(MIDCAB)を計画するための心臓撮像システム及び方法が開示される。より詳細な3D及びナビゲータ(内部)ビューにより、CT、磁気共鳴撮像、超音波などの撮像手法によって、冠動脈及び左心室(LV)の幾何学的表示が得られる。これにより、医療従事者は、MIDCABの標的とすべき冠動脈内の閉塞の向き、サイズ、異常及び範囲を識別することができる。この情報を使用すると、切開創が最も適切な部位に作られた、より多くの領域を標的とすることができ、また同時に切開創をより小さくできる、MIDCABのための一層正確なアプローチを取ることができる。
Claims (8)
- 患者のために低侵襲的直接冠動脈バイパス(MIDCAB)を計画するシステムであって、
獲得データを生成する医用撮像システムと、
前記獲得データを受け取って、前記患者の冠動脈及び心室の3Dモデル(130)を生成し、前記3Dモデル(130)を使用して、前記冠動脈内の病変の数とサイズを自動的に測定する画像生成サブシステム(110)と、
前記3Dモデル(130)上の1つ又は複数の解剖学的ランドマークを識別するユーザ入力を受け、その場所に対応する幾何学的マーカを挿入し、前記幾何学的マーカが挿入された前記3Dモデル(130)の複数のビューをデータベースに格納するオペレータ・コンソール(116)と、
前記データベースから前記幾何学的マーカが挿入された前記3Dモデル(130)の前記複数のビューを受ける介入システムのワークステーション(124)と、
を備え、
前記ワークステーションが、前記幾何学的マーカが挿入された前記3Dモデル(130)の前記複数のビューを前記介入システム上に登録する事後処理ソフトウェアを含み、
前記介入システムが、
前記幾何学的マーカが挿入された前記3Dモデル(130)の登録された複数のビューをインポートし、前記登録された複数のビューを視覚化し、関心のある心臓点に関する距離及び位置情報を定量化し、かつ関心のある前記心臓点に関する前記定量化された距離及び位置情報に基づいてMIDCABのための切開位置及び経路を識別するために前記登録された複数のビューを用いるように構成されるシステム。 - 前記事後処理ソフトウェアが、さらに、前記獲得データを処理して前記冠動脈及び心室の内部ビューを生成するように構成される、請求項1に記載のシステム。
- 前記介入システムに関連する表示画面(120、132)をさらに含んでおり、前記表示画面(120、132)が前記3Dモデル(130)及び前記内部ビューを視覚化するためのものである、請求項2に記載のシステム。
- 前記介入システムが、MIDCAB器具を登録するように構成される、請求項1に記載のシステム。
- 前記画像生成サブシステム(110)がEKGゲート制御される、請求項1に記載のシステム。
- 前記3Dモデル(130)から、前記冠動脈内の閉塞の向き及び寸法が識別される、請求項1に記載のシステム。
- MIDCABを必要とする冠動脈内の病変のサイズ、範囲及び数を測定する、請求項1に記載のシステム。
- 前記医用撮像システムが、コンピュータ断層撮影システム(100)、磁気共鳴撮像システム及び超音波システムのうちの1つである、請求項1乃至7のいずれかに記載のシステム。
