Description
本発明は外科的移植に関する。具体的には、プロテーゼ構成材の人体管腔への固定性および密封性の向上に関する。
脈管構造における、動脈瘤およびその他の問題に対処するための観血的処置においては、脈管内移植のように、手術による移植を用いることが一般的な方法である。このようなデバイスは、主に疾病の進行に従って生じる解剖学的変化、および、デバイスが配置される人体管腔の不規則形状によって生起する多くの固定に関する課題に直面している。このような課題を克服するため、数多くの種類および形状のステントがグラフト材の内側に配され、グラフトを所定の位置に固定する。だが、この技術には制限がある。例えば、この技術を用いた腹部大動脈瘤の治療では、動脈瘤と腎動脈との間に、最小で1センチメートルの距離がなければならない。さらに、大多数のステントの半径方向拡大断面形状は円形であるのに対し、それが配置される人体管腔の断面形状は非円形である。
本発明により、管腔の復元または治療を目的として人体の管腔内に移植可能なデバイスが供される。
図1を参照すれば、血管内インプラントまたはデバイス100が示されている。デバイス100は、プロテーゼ構成材115を有する。プロテーゼ構成材115はグラフト130を備え、また、グラフト130の内周部の内部に配されたステント140を備えてもよい。デバイスが経血管的(endovascularly)に配置される場合、プロテーゼ構成材はステントを有することが望ましい。他方、プロテーゼ構成材115はグラフトのみで構成してもよく、これはデバイスが腹腔鏡的にまたは観血的処置で配置される場合に望ましいものである。グラフト130は、ダクロン(Dacron)、ポリエステル、または、ポリテトラフルオロエチレンのような繊維の織物を有し、ステント140周囲を取り囲む円筒形壁を形成する。ステント140はワイヤメッシュであり、当業界において周知のあらゆる適切な種類、構成のものも可能である。例えば、ステント140は網目状、もしくは、糸状、または、レーザ切断もしくは食刻されていてもよく、また、自己膨張可能でも、バルーン式で膨張可能であってもよい。自己膨張式ステントの一形態例として、ニッケルおよびチタンを有する形状記憶合金がある。

Claims (9)
- 人体管腔内における移植のためのデバイスであって、
外周部を有するグラフトおよび内部空間を規定する裾を含んでいるプロテーゼ構成材、
前記外周部に取り付けられた第1部分、および、前記第1部分の半径方向外側に延びる第2部分を有し、前記プロテーゼ構成材を取り囲む人体隣接領域に取り付けるための、少なくとも1つのアタッチメントタブであって、前記の少なくとも1つのアタッチメントタブが複数のアタッチメントタブを含む、少なくとも1つのアタッチメントタブ;
前記グラフトの前記外周部に沿って配置され、前記タブを、前記人体の前記隣接領域と接触するよう、半径方向に伸長させるために、前記タブに取り付けるための少なくとも1つのタブ伸長リング;ならびに、
前記内部空間に配置された、前記プロテーゼ構成材を前記人体管腔に対して固定することを支援するため流体を吸収することで膨張する、コード、
を有するデバイス。 - 前記アタッチメントタブがポリエステル、ポリテトラフルオロエチレン、または、生体吸収性材料である、ことを特徴とする請求項1に記載のデバイス。
- さらに、前記アタッチメントタブを前記グラフトに繋ぐための、ワイヤを有する請求項1に記載のデバイス。
- 前記プロテーゼ構成材がさらに、前記グラフトの半径方向内側に配置された自己膨張性ス
テントを有する、ことを特徴とする請求項1に記載のデバイス。 - 前記プロテーゼ構成材がさらに、前記グラフトの半径方向内側に配置されたバルーン式膨張性ステントを有する、ことを特徴とする請求項1に記載のデバイス。
- 前記アタッチメントタブが正方形である、ことを特徴とする請求項1に記載のデバイス。
- 前記アタッチメントタブが山形である、ことを特徴とする請求項1に記載のデバイス。
- 前記アタッチメントタブが前記グラフトの一体部分である、ことを特徴とする請求項1に記載のデバイス。
- 人体管腔内における移植のためのデバイスであって、
グラフト上に形成された裾を備えた前記グラフトを有し、前記裾が内部空間を規定する、プロテーゼ構成材、
前記グラフトの半径方向内側に配置されたステント、
前記内部空間に配置された、前記プロテーゼ構成材の前記人体管腔に対する固定を支援するために流体を吸収することによって膨張する、コード、
前記グラフトの前記外周部に取り付けられた第1部分、および、前記プロテーゼ構成材の移植体を取り囲む前記人体の隣接領域に取り付けるため、前記第1部分の半径方向外側に伸長するように構成された第2部分、を有する少なくとも1つのアタッチメントタブ、ならびに、
前記グラフトの前記外周部に沿って、前記ステントの一部として、また、前記タブを前記人体の前記隣接領域と接触させるために半径方向に前記タブを伸長させるように少なくとも1つのタブに取り付けるように構成されている、少なくとも1つのタブ伸長リング、
を有するデバイス。
