本発明は、概して、医学的な装置および方法に関する。より具体的には、本発明は、再狭窄を阻害するための血管用ステントおよび移植片などの管腔状補綴物を提供する。
患者の血管系の狭窄したアテローム性硬化部位を治療して十分な血流を再開させることを目的として、数多くの経皮的血管内手法が開発されている。これら治療法の中で最も成功しているのが経皮経管血管形成術(PTA)である。PTAにおいては、拡張可能な遠位端であり通常は膨張可能なバルーンの形式である遠位端を有するカテーテルが、血管の狭窄部位に配置される。この拡張可能な遠位端を拡張させて血管を拡張させ、これにより、疾患部位を越えて十分な血流を再開させる。狭窄部位を広げるためのその他の手法としては、方向性アテレクトミー、回転式アテレクトミー、レーザー血管形成術、ステント留置術などがある。これらの手法は、(単独または組合せとして、特にステント留置と組み合わせたPTAとして)広く受け入れられているものの、依然として重大な欠点を有する。PTAおよび狭窄部位を広げるためのその他の既知の手法において特に多い欠点の1つは、再狭窄が頻繁に生じることである。

Claims (17)
- 患者の体内で使用するための冠動脈用ステントであって、
体内に挿入される場合の挿入元側となる端部と挿入先側となる端部とその間に位置する中間部とにより規定される縦の寸法を有するとともに放射状に拡張可能に形成された構造と、
該構造に結合し、治療用薬剤を放出するよう構成された、少なくとも1つの治療用薬剤の少なくとも1つの供給源と、
を含み、
該供給源は、前記治療用薬剤又は当該治療用薬剤を含む材料が、前記拡張可能な構造の表面に層をなすようにしてコーティング、噴霧、浸漬、堆積又は塗布されていることで設けられているとともに、
前記挿入元側の端部に付与された前記治療用薬剤の方が前記挿入先側の端部に付与された前記治療用薬剤より親水性が高いことにより、前記治療用薬剤の放出プロフィールが前記挿入先側の端部より前記挿入元側の端部において大きいことを特徴とする冠動脈用ステント。 - 前記放出プロフィールが、さらに中間部より端部において大きいことを特徴とする請求項1に記載の冠動脈用ステント。
- 中間部における供給源の構成が、端部における供給源とは異なることを特徴とする請求項2に記載の冠動脈用ステント。

- 供給源の少なくとも一部に隣接して配置された速度制御要素をさらに含み、患者の体内において治療用薬剤の放出を制御するよう構成されたことを特徴とする請求項1乃至9のいずれか1に記載の冠動脈用ステント。
- 少なくとも1つの供給源が、構造の縦方向に隣接する領域において、新生内膜過形成を阻害するのに有効量の治療用薬剤を提供するよう構成されていることを特徴とする請求項1乃至10のいずれか1に記載の冠動脈用ステント。
- 隣接する領域が、構造の近位または遠位で隣接する領域のうち少なくとも1つを含むか、または構造の一方もしくは各端から5ミリメートル以内であることを特徴とする請求項11に記載の冠動脈用ステント。
- 前記構造は、放射状に拡張可能であって移植可能な構造であるとともに、拡張中に異なる機械的プロフィールを示す複数の領域を縦の寸法に沿って有し、且つ比較的低い機械的プロフィールと比較的高い機械的プロフィールとを含む構造であり、
前記供給源は、前記構造の端部で比較的高い機械的プロフィール領域と、前記構造の中間部で比較的低い機械的プロフィール領域とのそれぞれに結合させて設けられていることを特徴とする請求項1乃至12のいずれか1に記載の冠動脈用ステント。 - 前記比較的低い機械的プロフィール領域が、構造の拡張時に実質的なたわみ、屈曲、伸張、または圧縮を受けないことを特徴とする請求項13に記載の冠動脈用ステント。
- 構造の少なくとも一部に隣接して配置された速度制御要素をさらに含み、速度制御要素の厚みが構造の縦の寸法にわたって変化することを特徴とする請求項13又は14に記載の冠動脈用ステント。
- 構造の中間部の少なくとも一部に唯一隣接して配置された速度制御要素をさらに含むことを特徴とする請求項13又は14に記載の冠動脈用ステント。
- エッジ効果またはキャンディ包装効果を阻害することを特徴とする請求項1乃至16のいずれか1に記載の冠動脈用ステント。
