Description
本発明は、体内の物質を係合する係合アセンブリを有する医療用具に関する。さらに特定すると、本発明は、体路(body tract)にある物質(例えば、結石)を砕石器(例えば、レーザー砕石器)によりその物質を処理するために、この物質を保持する回収アセンブリ(例えば、バスケット、グラスパ(graspers)、鉗子またはスクリーン)に関する。
医療用回収用具は、一般に、砕石器(例えば、レーザー砕石器)により断片化するために、体路から物質(例えば、結石)を回収するかまたは体路で物質を安定化するのに使用される。既存の医療用回収用具では、その回収アセンブリ(例えば、バスケット)は、複数のワイヤ脚部またはループから、または織物(例えば、金属製織物またはメッシュ)により形成される。この回収アセンブリは、物質(例えば、尿管、尿道、腎臓、胆管または胆嚢の結石)を捕獲するのに使用される。一旦、結石がバスケットに捕獲されると、結石が回収アセンブリで捕獲されている間に、この医療用回収用具を体内から引き出すことにより、結石を除去することが試みられている。

Claims (47)
- 体路にある物質を、該物質を処理するために保持する医療用具であって:
近位ハンドル;
該ハンドルから遠位に伸長しかつ管腔を有する、鞘;
該物質を係合する係合アセンブリ;および
該係合アセンブリに連結され、かつ該鞘の該管腔内で該係合アセブンブリから近位方向に伸長する細長部材を備え、
該係合アセンブリは、少なくとも、該係合アセンブリが該鞘の該管腔内にある位置、および該係合アセンブリが該鞘の遠位末端から該管腔の外に該係合アセンブリ内の物質を保持かつ安定化するように伸長する第2の位置を有し、
ここで、該係合アセンブリが複数の脚部を含み、各脚部が、該細長部材に接続される近位末端、およびレーザー耐性織物に接続される遠位末端を有し、そしてここで、該レーザー耐性織物が該複数の脚部の各々の遠位端にのみ接続される、医療用具。 - 前記鞘が、前記係合アセンブリの上を軸方向に移動可能であり、それにより、該係合アセンブリが、該鞘を軸方向に移動したとき、該鞘に対して移動する、請求項1に記載の医療用具。
- さらに、細長部材を含み、該細長部材が、前記鞘管腔内を伸長しており、かつ前記係合アンブリの近位末端に作動可能に装着され、それにより、該細長部材を該鞘内で軸方向に移動したとき、該係合アセンブリが移動する、請求項1に記載の医療用具。
- 前記係合アセンブリが、外科用回収バスケットを含む、請求項1に記載の医療用具。
- 前記複数の脚部の少なくとも1本が、個々に動作可能である、請求項1に記載の医療用具。
- 前記複数の脚部の少なくとも1本の断面が、D形、V形または長方形である、請求項1に記載の医療用具。
- 前記係合アセンブリが、体内の物質を係合可能にするように、閉鎖位置と開放位置との間
で移動可能である、請求項1に記載の医療用具。 - 前記係合アセンブリが、近位部分および遠位部分を含み、該遠位部分が、該遠位部分を前記鞘の遠位末端を超えて伸長し、かつ該係合アセンブリの該近位部分が該鞘内で折りたたまれた場合に、体内の物質を捕獲するためにあり、該係合アセンブリが、該係合アセンブリの該遠位および近位部分を該鞘の遠位末端から伸長した場合に、捕獲した物質を解放するように拡大する、請求項1に記載の医療用具。
- 前記係合アセンブリが、バスケットを含む、請求項8に記載の医療用具。
- 前記係合アセンブリが、外科用スクリーンを含む、請求項1に記載の医療用具。
- 前記レーザー耐性織物が、少なくとも1種のフルオロカーボンプラスチックを含む、請求項1に記載の医療用具。
- 前記フルオロカーボンプラスチックが、発泡ポリテトラフルオロエチレンを含む、請求項11に記載の医療用具。
- レーザーエネルギーによる損傷に耐性の材料が、前記係合アセンブリの少なくとも一部を覆う、請求項1に記載の医療用具。
- 前記鞘が、前記係合アセンブリの上を軸方向に移動可能であり、それにより、該係合アセンブリが、該鞘を軸方向に移動したとき、該鞘に対して移動する、請求項13に記載の医療用具。
- 前記係合アセンブリが、外科用回収バスケットを含む、請求項13に記載の医療用具。

- 前記係合アセンブリが、近位部分および遠位部分を含み、該遠位部分が、前記鞘の遠位末端および該係合アセンブリの近位部分を該鞘内で崩壊したとき、体内の物質を捕獲するためにあり、そして該係合アセンブリが、該係合アセンブリの該遠位および近位部分を該鞘の該遠位末端から伸長したとき、捕獲した物質を解放するように拡大する、請求項13に記載の医療用具。
- 前記係合アセンブリが、外科用スクリーンを含む、請求項13に記載の医療用具。
- 前記材料が、少なくとも1種のフルオロカーボンプラスチックを含む、請求項13に記載の医療用具。
- 前記フルオロカーボンプラスチックが、発泡ポリテトラフルオロエチレンを含む、請求項19に記載の医療用具。
- 体内の物質を処理するシステムであって:
体内に医療用具を挿入する手段であって、該医療用具が、以下の部分:近位ハンドル;鞘であって、該ハンドルから遠位に伸長し、かつ管腔を有する鞘;体内の物質を係合する係合アセンブリ;および該係合アセンブリに連結され、かつ該鞘の該管腔内で該係合アセンブリから近位方向に伸長する細長部材を備え、
該係合アセンブリが、少なくとも、該係合アセンブリが該鞘の該管腔内にある位置および該係合アセンブリが該鞘の遠位末端から該管腔の外に該係合アセンブリ内の物質を保持かつ安定化するように伸長する第2の位置を有し、
ここで、該係合アセンブリが複数の脚部を含み、各脚部が、該細長部材に接続される近位末端、およびレーザー耐性織物に接続される遠位末端を有する、医療用具、を含む、手段;
レーザー処理するために、該物質を該係合アセンブリで保持する手段;
該物質をレーザーで処理する手段;および
該医療用具を体内から除去する手段を備え、そしてここで、該レーザー耐性織物が該複数の脚部の各々の遠位端にのみ接続される、システム。 - 前記複数の脚部の少なくとも1つの長さの少なくとも一部分が、レーザーエネルギーからの損傷に耐性である少なくとも1つの物質を含む、請求項1に記載の医療用具。
- 前記複数の脚部が、内表面および外表面を備え、ここで、少なくとも1つの脚部の内表面または外表面のいずれかがレーザーエネルギーからの損傷に耐性である物質により被覆されている、請求項1に記載の医療用具。
- 前記フルオロカーボンプラスチックがポリテトラフルオロエチレンを含む、請求項11に記載の医療用具。

- 前記フルオロカーボンプラスチックがポリフッ化ビニリデンを含む、請求項11に記載の医療用具。
- 前記材料が、セラミックス、金、銀、またはニッケルを含む、請求項20に記載の医療用具。
- 前記複数の脚部の各々が内表面および外表面を備え、そして少なくとも1つの脚部の外表面の少なくとも一部分が、該少なくとも1つの脚部の内表面の少なくとも一部分を被覆する材料より大きな、レーザーエネルギーからの損傷に対する耐性を有する材料によって被覆されている、請求項1に記載の医療用具。
- 前記複数の脚部の各々が内表面および外表面を備え、そして少なくとも1つの脚部の内表面または外表面のいずれか一方のみの少なくとも一部分が、該少なくとも1つの脚部の他方の内表面または外表面の少なくとも一部分を被覆する材料より大きな、レーザーエネルギーからの損傷に対する耐性を有する材料によって被覆されている、請求項1に記載の医療用具。
- 前記複数の脚部の各々が内表面および外表面を備え、そして少なくとも1つの脚部の内表面のみがレーザーエネルギーからの損傷に対して耐性の材料によって被覆される、請求項1に記載の医療用具。
- 前記レーザー耐性織物が、実質的に三角形形状を有する、請求項1に記載の医療用具。
- 前記織物が、その中に開口部を有するメッシュ材料である、請求項1に記載の医療用具。
- 前記開口部が、約3.0mm以下の直径を有する、請求項35に記載のデバイス。
- 前記係合アセンブリの脚部が、形状記憶合金から形成される、請求項1に記載の医療用具。
- 前記鞘、前記管腔、および前記係合アセンブリが、共通の長軸方向軸を規定し、そして前記係合アセンブリの各脚部が、近位部分および遠位部分を含み、該係合アセンブリが前記第2の位置にあるとき、該遠位部分が、該近位部分および該長軸方向軸から外側に曲がる、請求項1に記載の医療用具。
- 前記各脚部の遠位部分が、前記係合アセンブリが前記第2の位置にあるとき、前記長軸方向軸から外側に約35゜から約60゜の角度で曲がる、請求項38に記載の医療用具。
- 前記レーザー耐性織物が、前記係合アセンブリが前記第2の状態にあるとき実質的に張りつめた平坦な表面を形成し、そして前記係合アセンブリの複数の脚部の近位部分が、前記鞘の遠位末端を超えて伸長する、請求項38に記載の医療用具。
- 前記レーザー耐性織物が、前記複数の脚部の遠位部分が前記鞘の遠位末端を超えて伸長し、そして該複数の脚部の近位部分が該鞘内に残るとき、凸状形状をとる、請求項38に記載の医療用具。
- 前記凸状形状の頂点が、前記複数の脚の各々の遠位末端にあるより、前記鞘の遠位末端からより遠くに配置される、請求項41に記載の医療用具。
- 前記レーザー耐性織物が、前記複数の脚部の遠位部分が前記鞘の遠位末端を超えて伸長し、そして該複数の脚部の近位部分が該鞘内の残るとき、凹状形状をとる、請求項38に記載の医療用具。
- 前記凹状形状の頂点が、前記複数の脚部の各々の遠位末端にあるより、前記鞘の遠位末端により近く配置される、請求項43に記載の医療用具。
- 前記レーザー耐性織物が、実質的に矩形形状を有する、請求項1に記載の医療用具。
- 前記レーザー耐性織物が、膨張したポリテトラフルオロエチレン(EPTFE)、ポリテトラフルオロエチレン(PTFE)、テトラフルオロエチレン(TFE)、フッ素化エチレンプロピレン(FEP)、パーフルオロアルコイ(PFA)、エチレンテトラ−フルオロエチレン(ETFE)、およびポリビニリデンフルオライド(PVDF)から選択される、請求項1に記載の医療用具。
