Description
・H=IOP1−IOP2
を求めることによってヒステリシスHを計算することで、数値化される。
・IOPM=IOPC+IOPI
ここで、IOPMは、測定したIOPであり、IOPCは、角膜の影響によって生じた等価IOPオフセットであり、IOPIは、診断上重要な真の内向きの眼圧である。ヒステリシスHに関して前記した観測のように、IOPCはヒステリシスHの関数であると考えられる。これにより、次のようになる。
・IOPC=f1(H)
定義によれば、下記のようになる。
・IOPI=IOPN+EOP
ここで、IOPNは、14.7mmHgにおよそ等しい一定の標準的(平均的)な内向きの圧力であり、EOPは、(IOPNに対する)「過剰な」眼圧である。IOPNを定数K1に等しいとし、代入すると、以下のようになる。
・IOPM=f1(H)+K1+EOP
EOPがおおよそゼロであるN人の被検者の統計的な大規模集団を、臨床的に測定することにより、次の関係が示される。
・IOPMi=f1(Hi)+K1; i=1,N
IOPMi値は、r次の多項式に当てはめられ、例えば次のように示される。
ここで、a0=k1であり、「a」の値は、最小二乗差を最小限にする(例えば、Hiに対するIOPMiの値の、曲線の当てはめ)ことによって、決定することができる。IOPMiと、
との間の伝統的相関係数を計算して、データがr次の多項式にうまく当てはめられているかどうかを評価する。現在のデータは、約0.9の相関関係が生じている。曲線の当てはめについては、多項式に限られず、他の関数も用いられ得る。表にされた平滑化データセットは用いられ得るが、基本的な物理的過程では、第二の次元のデータHと第一の次元のデータIOPMの単純な関係が示される。
・I1=内向き圧平の圧力(mmHg)
・I2=外向き圧平の圧力(mmHg)
・H=角膜ヒステリシス(mmHg)
・R=勾配率([mu]sec/mmHg)
・I0=実際の内部眼圧(mmHg)
・H=I1−I2 (1)
・H=αR (2)
ここで、αは一定である。そして、
・I1=I0+H/2
・I2=I0−H/2 (3)
と仮定される。
I1の二つの測定値が用いられて、I1 0及びI1 1と示される。これは、それぞれR0及びR1の二つの異なる勾配率を用いる。二つの測定に対する二つの角膜ヒステリシス値は、それぞれH0及びH1と示される。また、

とする。ここでβは、スケーリング定数(scaling constant)である。式(2)から次が定義される。
・H0=αR0 (5)
・H1=αR1 (6)
そして、式(3)及び(4)から
・I1 0=I0+H0/2=I0+αR0/2=I0+αR0/2 (7)
・I1 1=I0+H1/2=I0+αR1/2=I0+αβR0/2 (8)
となる。式(7)から式(8)を引くことで、ヒステリシスH(=αR0)が得られる。
・H0=αR0=2×(I1 0−I1 1)/(1−β) (9)
I0を得るために(7)と(9)を加える。
・I0=I1 0−[(I1 0−I1 1)/(1−β)] (10)
これにより、異なる圧力勾配率を用いる「ツーショット」測定手法でも、2次元のデータI1及びHを得ることができる。尚、これらは、単一の内向き・外向き測定でも前もって導かれ得るものである。
Claims (1)
- 角膜ヒステリシスを検知するための装置であって、
一時的に角膜に圧力を付与して、凸状態の自然な状態から、圧平の第一状態を介して凹状態へ前記角膜を変形させ、その後、前記角膜を、前記凹状態から、圧平の第二状態を介して前記自然な凸状態へ戻るようにする手段と、
前記第一の圧平状態に関連付けられた第一圧力と、前記第二の圧平状態に関連付けられた第二の圧力とを検知する手段と、
前記第一及び第二圧力の圧力差を計算して、前記圧力差を角膜ヒステリシスとして示す処理手段と、
からなることを特徴とする装置。
