Description
にきびは、命に係わるものではないが、罹患した患者にとって重大な問題となり得る。その深刻さおよび他の要因に従い、難治療のにきびは、患者に精神的な苦痛を与え、また患者に多大な費用および心理面での負担を課し得る。最近のにきび治療における進歩にもかかわらず、今尚、特に成人女性において、治療効果が現れないことは常である。多くの成人は、成人に達するとにきびを罹患しなくなるが、医学の継続的な進歩にもかかわらず、成人期間の多くをにきびに悩ませ続けられる人もいる。不運な事に、現在使用されている最も有効なにきび治療は、催奇性を有する治療薬の全身投与であり、これは多くの女性にとって重大な問題である。より局所的でありかつ効果的で、副作用が最小なにきび治療に対する必要性が存在している。

これらの互いに共存するにきび形成の道筋における最後の通常の経路は、濾胞の閉塞である。
現在使用されているにきびの治療法は、にきびの連鎖的な生成を様々な角度から検討するものである。最も普通に使用されている治療法は、抗菌効果を有し、かつ遊離脂肪酸を減少させることにより炎症および濾胞閉塞を減少させる、過酸化ベンゾイルの局所投与であると思われる。

ボツリヌス毒素およびその薬理学
嫌気性細菌であるクロストリディウム属ボツリヌス菌(Clostridium botulinum)は、

ツリヌス毒素の効果は、副交感神経のコリン作動性神経のシナプス前終末にて生じることが判明した。
シャー州メイデンヘッド所在]からDysport(登録商標)の名称により販売されて
いる)は、腺神経連絡および筋神経連絡の双方において、特定の治療目的のために使用されている。ボツリヌス毒素の一時的な筋神経妨害は、これまでも数多く使用されており、その中には、斜視、偏頭痛、弛緩、半側顔面痙攣の治療、しわ切除術、および痙攣性発声障害の治療が含まれる。この目的で使用されるボツリヌス毒素の注射は、その副作用が局所膨張や周辺筋肉の一時的な弱化などのみで非常に少なく、安全かつ有効であることが証明されている。ボツリヌス毒素AおよびBは、使用した患者の体内にて良好な耐性を有することが報告されている。

筋神経連絡の妨害
最初に報告されたボツリヌス毒素Aの臨床的使用は、1970年代における斜視治療である。眼球外の小筋肉に注射を行うと、ある場合には、筋肉が再編成されて、眼球が真直ぐになり、目の整合性が改良された。次いで、ボツリヌス毒素Aは、機能および美容面の双方において、他の数々の治療に使用された。この治療法は、局所ジストニア、痙性ジストニア、眼瞼痙攣、半側痙攣、斜頸、および頸部ジストニアの治療に効果がある事が証明されている。この治療法は最近、顔面形成外科手術、特にしわ切除術にも使用されている。
国立予防衛生研究所(National Institutes of Health)の全ては、様々な臨床的病状にお
けるボツリヌス毒素Aの治療的効果を支持するステートメントを発表した。ボツリヌス毒素Aが使用される病状には、眼瞼痙攣、斜視、頸部ジストニア、痙性斜頸、しわ、半側顔面痙攣、顔面痙攣、痙性ジストニア、手掌ジストニア(focal hand dystonia)、過度の顔面皺、Frey症候群、多汗症、成人の痙攣、大脳麻痺があり、大脳麻痺における痙攣、口下顎ジストニア、および他のジストニアの補助療法がある。
腺神経連絡の妨害
腺神経連絡におけるコリン作動抑制性物質としてのボツリヌス毒素Aの有効性は、筋神経連絡における有効性ほど広範囲には研究されていない。局所多汗症に対するボツリヌス毒素の皮内適用の有効性を調べる臨床試験において、注射領域内の発汗は、3〜7日以内に完全になくなった。有害作用は報告されず、5ヶ月間の追跡調査では多汗症の臨床的再発は起こらなかった。キンケリン(I.Kinkelin)らによる忍ffective Treatment of Frontal Hyperhidrosis with botulinum toxin A・ Brit. J. Dermatol., 第14

ボツリヌス毒素Aが筋神経連絡に作用する期間は、約3ヶ月間であると思われるが、腺レベルにおいては、より長い作用期間を有するものと想定される。ボツリヌス毒素Aは、味覚性多汗症の患者内にて、12ヶ月間アンヒドロシス(anhydrosis)を生成した。この作用期間の相違の理由は定かではない。筋繊維シナプス内にてSNAP−25(ボツリヌス毒素により裂かれたタンパク質)がより高速で再合成されること、腺組織と比較すると、筋繊維の軸索新芽形成および連続的な脈再形成(reinervation)の範囲は広いこと等の仮説がある。
思春期以来、過酷な嚢胞性にきびを罹患している20代後半の白人女性を「初発症例」とした。この女性は、ボツリヌス毒素Aの皮内注射を受けた後、にきびの完全な消散を体験した。その他の面では健康なこの女性は、それまでにスピロノラクトンおよび3工程のイソトレチノインの双方を含む、従来の数種のにきび治療を受けたが、にきびの消散は見られなかった。
この女性は、現在、額、顎、および頬/鼻ひだ領域に、ボツリヌス毒素A注射を、約6ヶ月に一度受けている。約6ヶ月後、嚢胞の再生が開始される。嚢胞は、注射後約1週間で消失する。注射は、約1.5cmの間隔を空け、注射部位当たりの投与量は、約2.5Uである。
実施例2
30代前半の白人女性は、思春期以来、過酷な嚢胞性にきびを罹患し、従来の治療法では対処できなかった。この女性には既に、にきびの結果としての過酷な傷跡が存在した。女性の背中に、ボツリヌス毒素Aを注射した。注射した領域内におけるにきびは、1週間以内にかなり減少し、丘疹および膿疹は、1ヶ月以内にかなり減少した。注射のなされた範囲から約1cmの箇所から始まる、縁部の周囲における活性病変の存在により、注射の終点にて識別可能な境界線を認めることができた。合計で86ユニットのボツリヌス毒素Aを使用して、互いに約2〜3cm離間された注射部位において、1回の注射につき2.5ユニットを使用した。
実施例3
20歳の白人男性は、思春期以来、顔面、首部、胸部、および背部において、過酷な嚢胞性にきびを罹患していた。この男性は以前、2工程のイソトレチノイン治療と、抗生物質全身投与および局所投与治療の多数の工程を受けたが、にきびは消失しなかった。男性の他の重大な治療上の唯一の問題は、結腸クローン病であった。男性は背部に、合計で162Uのボツリヌス毒素Aの注射を受けたが、にきびの存在した別の領域には注射を受けなかった。1週間後、治療領域において、にきびがかなり消失し、3週間後、ほぼ完全に消失した。それとは対照的に、未治療領域(即ち、顔面、首部、および胸部)では改善が見られなかった。
この最初の実験では、ボツリヌス毒素治療による試験を行った患者数は3人のみであったが、従来の治療法で対処できなかった難治療のにきびを罹患していた、最初の患者の100%において、劇的な改善が見られた。続いて、小規模の臨床試験も行った。結果を以下に報告する。
実施例4
成人のにきび病変の防止または減少に対するボツリヌス毒素A(Botox(登録商標))の皮内注射の臨床的な利益を検証するために、小規模臨床試験を行った。本願の出願日において、この試験は継続中であるが、いくつかの予備結果が得られている。この研究に使用されている手順は、以下のようなものである。
適格性の基準
適格性の基準を以下に記す。
適格者
1.男性または女性
2.最初の書類判定により、おおむね健康な者。
3.臨床実験と、顔および背部の写真撮影とに同意する者。
4.難治療と診断された者(抗生物質の全身投与を含む従来の治療で対処不能)。
5.妊娠検査にて陰性(女性)。
6.試験に参加した被験者のうち、少なくとも10〜15人が、額または背部に嚢胞性にきびを有すること(少なくとも半分が額に)
7.5人までの被験者が、額または背部に非嚢胞性の炎症にきびを有すること(少なくとも3人が額に)。
8.治療を受けない対照群となる事に同意すること。15人中、3人の被験者が治療を受けない群となる事に同意した(少なくとも、嚢胞性の群から2人、および非嚢胞性の群から1人)。
9.試験の要求事項を理解することができ、インフォームドコンセントの書式に署名すること。
10.各顔面試験の30分以上前に、化粧を取り去るのに同意すること。
適格でない者
1.ベースライン前に、12ヶ月以下の女性ホルモンまたはバースコントロールピルによる治療を受けた女性。
2.にきびの診断または評価を妨害し得る皮膚病を有する者。
3.研究上の薬剤試験に現在参加しているか、またはベースライン訪問の30日以内に参加した者。
4.健康質問表により、糖尿病、高血圧、甲状腺機能亢進症、または甲状腺機能低下症などの、制御不能の代謝病を罹患していると判定された者。
5.最初の書類判定により、妊娠中、育児中、または6ヶ月以内に妊娠を計画している女性。
6.ボツリヌス毒素Aに対するアレルギーまたは過敏症を有する者。
7.試験に協力的でない者、治療法を遵守しない者、以前の臨床試験で信用性を欠くとされた者。
8.重症筋無力症、またはイートンランバート病を罹患した経験のある者。
9.抗生物質の使用後、少なくとも1.5ヶ月以内に変化のあった者。
10.アミノグリコシド、抗コリンエステラーゼ、硫酸マグネシウム、および神経筋伝達を妨害する他の薬品または製品を使用している者。
11.ボツリヌス毒素Aによる治療を受けたことがあるか、またはボツリヌス毒素Aの中毒にかかったことのある者。
試験の実施
方法
訪問1−スクリーニング(審査)
被験者となり得る者は、健康/適格質問表、写真公開フォーム、秘密保持契約、およびインフォームドコンセント同意書に記入する。有資格者は、顔面および背部に嚢胞性または炎症性にきびを有することを試験される。
被験者は、にきびの位置(額または背部)を基に、各々の治療群に分けられる。各被験者は、額および背部のどちらかの治療を受ける。(全被験者がスクリーニングされて、全被験者が参加した後)訪問1と訪問2の間に、治療群のランダム化が起こる。
訪問2−ベースライン
資格者判定訪問の1〜3日後、被験者はクリニックに戻る。治療群および対象群の双方の被験者が、注射前質問表に記入した。
第42巻、973〜975ページ(1997年)に別様に説明されているように、約2.
5〜3マウスユニットのBotox(登録商標)を各領域に注射する。希釈は、0.05ccにつき3ユニットであり、これはボツリヌス毒素Aの100ユニットにつき1.66ccに等しい。投与量の上限は、額に対して80ユニット、背部に対して125ユニットである。被験者は、注射を受けた後、クリニックに30分間残留する。被験者のにきび治療は、現在も継続している。
訪問3〜7、さらなる評価
被験者は、治療の約1,2,4,8、12週間後、クリニックに戻る(各々、訪問3,4,5,6,7)。ベースラインで説明したように、各訪問毎に評価が行われるが、さらなる注射は行わない。被験者は、各訪問毎に質問表に記入し、訓練された臨床評価技師が、にきび病変部の計数、紅斑および隆起の段階評価、および周辺皮膚の乾燥性および油性の段階評価を行う。単一交差偏光写真の撮影を各試験部位について繰り返す。
被験者が予定の検査時に戻らなかった場合、職員は被験者との接触を試み、被験者が試験指示書に従っていたか、試験に参加し続ける意志があるか否かを判断する。被験者は可能な限り早く対処されるか、または検査官の判断により試験訪問から除外される。試験から除外された被験者は、記録される。
実施例5〜9
予備結果
前述したように、この試験は、本願の出願時の時点にて継続中である。完了した試験の一部による予備結果を、以下に報告する。
度、ベースラインからの隆起病変の程度において、統計的にかなりの減少が見られた。他の3つの測定値において、大きな数値の差異が見られた。この差異は、サンプルサイズが小さいため、統計的に重大ではないが、追加のデータ、即ち丘疹数、開放面皰数、および嚢胞数のデータが入手されるにつれ、統計的に重要なものになることが十分予想される。サンプルサイズが小さいことを念頭に置き、これらの予備結果は、この特定の治療法にて、閉鎖面皰、乾燥性発生、および油性発生の数の差異は最小であるか、または全くないことを示唆する。有害作用、毒性、副作用は報告されなかった。
実施例10 投薬量および毒性
適切な投薬量の選択は考慮すべき重要な点である。適切な投薬量は、他の用途にて標準的である用量反応試験および毒性試験により確定されるであろう。この治療の対象領域は、多汗症の対象領域とほぼ同一であるため、最初の試験においてほぼ同じ投薬量が選択された。ノーマン(Naumann)らによる"Focal Hyperhidrosis: Effective Treatment with Intracutaneous Botulism Toxin," Arch. Dermatol., 第134巻、301〜3

を参照されたい。多汗症の一般的な臨床ガイドラインが作成されているが、多汗症の正確な用量反応曲線を定義する研究は存在しないものと思われる。表2を参照されたい。米国でFDAの厳しい監督下に作成された、ボツリヌス毒素Aの臨床製剤は、マウスユニットにて評価される。1マウスユニット、即ちIUは、非口径注射によって、18〜20グラムの雌のSwiss−Weberマウスのグループを50%(LD50)殺傷するボツリヌス毒素Aの量により定義される。ボツリヌス毒素Aの使用に関する禁忌は、殆ど存在しない。(注:Botox(登録商標)ユニットは、Dysport(登録商標)ユニットの3〜5倍の効力を有するとの示唆が報告されている。本願で報告する実験に使用される
ボツリヌス毒素Aは、Botox(登録商標)として得られたものであり、これら2種の商業的に入手可能なボツリヌス毒素Aの製剤は、その効力に差異があると仮定する限り、本明細書および特許請求の範囲に使用される「ユニット」は、Botox(登録商標)ユニットを指すものとみなされ得る)。

2人のにきび患者について、ボツリヌス毒素Aを局所適用する予備治験を行った。局所治療によく反応するにきび型を確認するために、さらなる大規模局所適用治験を行った。この局所治療では、ボツリヌス毒素Aを、溶媒0.5mLにつき100ユニットの濃度に再構成した。背部中央の治療では、溶媒は、中性点眼薬としてのポリビニルアルコールの1.4%水溶液を使用し、背部左側の治療では、溶媒は、保存料を含有しない整理食塩水を使用した。溶液の0.2mL、即ち40U部分を、6cm×4cmの範囲に塗布し、障害を避けるためにTegaderm(登録商標)を4時間貼付した。結果を以下に示す。
実施例13
ボツリヌス毒素Aの皮内注射または局部注射により、成人および青年の難治療にきびの発生を、安全かつ効果的に防止または減少し得ることを確認するために、第二相臨床治験を行う。該当する法律および規則に従って、第二相治験の後、第三相治験を実施する。
Claims (5)
- ボツリヌス毒素を含有し、にきびを発症し易い被験者のにきび形成を抑制するのに用いられる組成物。
- 前記ボツリヌス毒素はボツリヌス毒素Aである請求項1に記載の組成物。
- 前記ボツリヌス毒素は、ボツリヌス毒素B、ボツリヌス毒素C、ボツリヌス毒素D、ボツリヌス毒素E、ボツリヌス毒素F、又はボツリヌス毒素Gである請求項1に記載の組成物。
- 前記ボツリヌス毒素の含有量が5〜200U/mlである請求項1から請求項3のいずれか一項に記載の組成物。
- 前記組成物は、経皮投与、皮内注射、皮下注射、及び筋肉注射用である請求項1から請求項4のいずれか一項に記載の組成物。
