【発明の属する技術分野】
本発明は、移植組織または移植臓器(同種異系または異種)に含まれるT細胞にドナー抗原(異種抗原または同種異系抗原)に対する耐容性を与えるために、これらの臓器を生体外で処置する方法を提供する。処置した組織または臓器は、移植片対宿主病の危険性を低下して、レシピエントに移植できる。

(発明の背景)
抗原特異的T細胞の活性化およびクローン増殖を誘導するために、抗原提示細胞(APC)により提供される2つのシグナルは、休止Tリンパ球の表面に送達されなければならない(Jenkins, M. and Schwartz, R. (1987) J. Exp. Med. 165, 302-319; Mueller, D.L., et al. (1990) J. Immunol. 144, 3701-3709; Williams, I.R. and Unanue, E.R. (1990) J. (発明の詳細な説明)
移植片対宿主病(GVHD)は、ドナー同種異系T細胞のレシピエントへの注入で起きる多重臓器系の破壊プロセスである。急性の移植片対宿主病はHLA特異的同胞間ドナー移植片で20−40%のレシピエントに、非関連ドナー移植片で70−80%のレシピエントに生じるので、骨髄移植のこの合併症を防止する手段が必要である。現在のところ一般的に言えば2種の対応がなされている。第1は、メタトリシド、シクロスポリンA、ステロイドなどの免疫抑制剤のインビボ注入である。上記したように急性の移植片対宿主病がインビボ免疫抑制剤の注入において見られる。この防止効果が不完全なことに加えて、これらの免疫抑制剤は骨髄移植後に長期間の免疫不全を起こす。これによって、レシピエントは感染合併症の危険に曝され、骨髄移植後の再発の発生率が増加する。第2の対応は、ドナー細胞からT細胞の生体外除去である。これは、急性の移植片対宿主病を防止するのにかなり効果的であるが、移植片の失効、再発、感染合併症の発生率が高くなり、免疫再構築の時間を遅くする。

一方、本発明は、ドナーT細胞を生体外で処置する方法であって、宿主中への移植に際しこのT細胞を同種異系または異種の抗原に対する実質的な非応答性を与える。具体的には、本発明は、ドナーT細胞を、少なくとも1種のgp39(CD154)アンタゴニストおよび同種異系または異種の細胞または組織で処置する方法に関し、該ドナー細胞に、該同種異系または異種の細胞を含有する宿主中に移植した際に、ドナー抗原(同種抗原または異種抗原)に対する実質的な非応答性を与えることを目的とする。

本発明は、ドナー骨髄あるいは末梢血などを含有するドナーT細胞または組織または臓器をレシピエントに移植する際に、起こり得る移植片対宿主病を防止または抑制する効果的な手段を提供する。

好ましくは、ドナーT細胞を、十分な量の抗gp39抗体および移植レシピエント由来の細胞とともに生体外でインキュベートして、ドナーT細胞に、移植時にレシピエント細胞に対する実質的な非応答性を与える。

同種異系細胞なる用語は、レシピエントと同種の異なる個体から得た細胞を意味する。
同種抗原なる用語は、レシピエントと同種の異なる個体から得た細胞を意味する。
異種の細胞(異種の移植臓器)なる用語は、別種、通常、移植レシピエントに対して異種から得た細胞を意味する。(例えば、ヒヒT細胞をヒトレシピエントに移植する場合に、ヒヒT細胞が異種個体細胞を含む。)
異種抗原なる用語は、別種、通常、移植レシピエントに対して異種から得た細胞によって発現された抗原を意味する。
gp39アンタゴニストなる用語は、gp39(CD154)−CD40の相互作用を妨げる分子を意味する。gp39アンタゴニストは、gp39に対する抗体(例えば、ヒトgp39に対する特異的なモノクロール抗体)、その断片または誘導体(例えば、Fab、F(ab)'2断片、キメラ抗体、ヒト抗体またヒト適合抗体)であるのが好ましい。また、gp39アンタゴニストは、gp39リガンドの融合タンパク質の可溶性形態(例えば、可溶性CD40Ig)またはgp39−CD40相互作用に干渉する医薬を包含する。

gp39またはCD154、即ちCD40LCRまたはCD40CRなる用語は、分子CD40と相互作用するT細胞の表面で発現された分子を意味する。このCD40は、B細胞増殖を誘導することに関与する成熟B細胞のリンパ球と未熟B細胞のリンパ球表面で同定される。具体的にはT細胞上のgp39とB細胞上のCD40との相互作用が、抗原に対するB細胞の応答を活性化するのに中心的な役割をなす。また、gp39が、抗原、例えば、同種および異種抗原に対するT細胞の応答に非常に重要な役割を担うことが発見されている。
本発明におけるT細胞無応答性またはT細胞耐容性なる用語は、ドナー細胞を、gp39アンタゴニスト(抗gp39抗体)と同種または異種の抗体を保持する細胞とに生体外で接触させた後に、ドナーT細胞のレシピエントへの移植の際、同種または異種の抗体保持細胞に対するドナーT細胞誘導の免疫の応答(移植片対宿主応答)が低下されることを意味する。

説明したように、本発明は、外来ドナーT細胞含有組成物、例えば、同種異系または異種の骨髄または末梢血細胞の移植に際して、移植片対宿主病を防止する従来と異なる手段を提供する。

非常に少量のドナー細胞が宿主同種異系抗原を認識する能力を所有すること(0.0%以下とみられる)が知られている。本発明はこの応答を消失せしめようとするものであって(該細胞を同種異系抗原または異種抗原に対して非応答性または耐容性にする)、同種または異種抗原反応能を有するT細胞の集団を機能的に変えることによる。

これが強力に達成され得るのは、本発明による仮説では、ドナーT細胞と照射・切除T細胞宿主同種異系抗原または異種抗原を保持する刺激物との混合リンパ球反応を開始すること、およびこの混合リンパ球反応培養物にgp39アンタゴニスト、特に抗gp39抗体を加えることによる。この考えは、gp39−CD40のインビトロ相互反応(ドナーT細胞と、同種抗原または異種抗原を保持する細胞との間)に干渉し、移植レシピエント中に移植する際に、該細胞に同種異系抗原または異種抗原に対する耐容性または非応答性を与えようとするものである。理論的には、本発明者の発表を含む以前の発表を基にして可能である。これらの発表は、gp39アンタゴニスト(抗gp39抗体)のみで、または同種異系または異種の細胞と合わせて、異種または同種異系の組織または臓器の移植の前、同時あるいは後に、移植レシピエントを処置することにより、同種異系または異種の組織に対するT細胞の耐容性をインビボで誘発することに関する。このインビボ手段は、種々の組織や臓器、例えば、膀胱、皮膚、心臓組織などに対するT細胞の耐容性を起こすのに非常に効果的であるとされている。

しかし、この方法をインビトロでT細胞の耐容性または非応答性の誘発に拡張し得るかどうかは、予測できなかった。この結果が合理的に予測できなかったのは、文献に発表された以前の研究で、インビトロT細胞活性化の誘発にgp39の存在を必要条件としていないことによる。例えば、Flavellら(Nature, 378: 617-620 (1995))によると、gp39発現を遺伝子的に欠くT細胞レポーター形質転換T細胞が抗原提示細胞および抗原に正常に応答した。このことは、T細胞活性化はgp39を必要としないで、gp39なしでインビトロで正常に起こり得ることを示している。

具体的には、Grewal, J.S. et al, Nature, 378: 617-620 (1995), "Impairment of antigen-specific T-cell priming in mice lacking CD40 ligand"によると、T細胞の短期間インビトロ活性化はgp39の存在を必要としないで、gp39なしでインビトロで同種T細胞の耐容性が生じ得る。

従って、該細胞を、gp39アゴニストと、レシピエントT細胞を除去したレシピエントの同種異系または異種の細胞とともにインキュベートすることにより、T細胞耐容性すなわちドナーT細胞の非応答性がインビトロで効果的に誘発され得ることは、まったく予想外であった。この方法は、移植レシピエントの処置において非常に大きい可能性を与える。なぜなら、移植の組織または臓器に含有される移植T細胞に、レシピエント同種異系抗原または異種抗原に対する耐容性すなわち非応答性を与える、非常に効果的な非侵傷的手段をもたらすからである。従って、この移植の組織または臓器すなわち異種または同種異系の骨髄は、移植に際し有害な移植片対宿主応答を誘起しない。さらに、耐容性がインビトロで誘発されることはさらに優れた点であり、他の拒絶反応に対する対策、例えば、シクロスポリンなどの免疫抑制剤と合わせて、この処置をとり得る。また、移植の前、同時あるいは後に、抗gp39抗体(または他のリガンド)投与を併用し得る。

事実、本方法は、他の抗拒絶反応剤の必要をなくする。抗拒絶反応剤は、免疫抑制作用からして、有害な副作用、例えば、感染や癌の危険性の増大をもたらすことがある。

好ましい実施態様において、ドナー、例えば同種異系または異種のドナーに由来のT細胞を、宿主T細胞の除去処置(例えば、照射)をしたレシピエントの同種異系または異種の組織とともにインビトロで培養する。この培養基に、有効量のgp39アンタゴニスト、典型的には抗gp抗体(例えば、米国特許No.GET313#に開示された24−31または89−76抗ヒトgp36抗体)を加える。この培養基をT細胞耐容性の誘発に十分な時間保持する。典型的には、この時間は、約1−2日から30日、さらに典型的には約5日から15日、最も典型的には約10日である。

さらに、対照または抗gp39(CD154)モノクロナール抗体処置ドナーT細胞の等量のインビボ投与では、移植片対宿主病の性質が非常に異なっていることが分かった。具体的には、対照における3倍高い数のドナーT細胞のレシピエントは、対照が0%であるのに比して50%の計算上の生存率を示した。他の実験で、抗gp39(CD154)モノクロナール抗体処置T細胞を用いると、移植片対宿主病の強度に30倍もの相違があった。このことからして、驚くべきことに、ドナーT細胞が混合リンパ球反応により生体外で効果的に耐容性が与えられると結論した。これは、宿主細胞および異種抗原に対するドナーT細胞の耐容性付与に重要な新しい手段を提供する。

この方法は、骨髄または末梢血細胞の移植療法の領域に意味のある可能性を提供する。骨髄および幹細胞の移植は、免疫細胞不全を含む白血球などの種々の疾患の処置のために、よく用いられている。さらに、骨髄移植は、自己免疫疾患の処置などの他の疾患の処置にも、利点をもたらす。しかし、通常の骨髄移植療法に関連してよく起きる危険は、GVHD応答を誘起する危険である。本主題の方法は、この危険を減少または消失せしめ、もって骨髄移植療法の適用を拡大する。

理想的には、本発明方法は、GVH応答を誘起しないで処置ドナーT細胞のレシピエントにおける免疫再構築を提供する。しかし、ある場合には、移植レシピエントにおいて移植組織を「受け」ないとき、この療法を繰り返す必要がある。あるいは、移植レシピエントのリンパ様システムが疾患または疾患処置の結果として、例えば、放射線処置の結果として障害を受けていると、繰り返しが必要となる。このような場合、適当なドナーT細胞を再度、抗gp39抗体およびT細胞除去同種または異種抗原保持レシピエント細胞に生体外で接触せしめ、T細胞耐容性を誘発し、ついで移植レシピエントに注入する。

ドナーのCD+リンパ節T細胞とMHCクラスII非対称同種抗原を保持する刺激性細胞との混合リンパ球反応(MLR)の結果を図1に示す。この実験において、C.H2bm12由来の高度に精製したCD4+リンパ節T細胞を0.5x106/mlの最終濃度で24ウエルプレートのミクロタイターウエルまたはバルク培養基中に入れた。刺激性細胞は、C57BL/6T細胞除去・照射脾臓細胞であって、1x106/mlの最終濃度で使用した。MLR培地は10%の子牛胎児血清、5%の付随物および2−MEから成る。抗gp39mAbを50μg/mlの最終濃度で加えた。図1で示すように、IL−2を最終濃度50単位/mlで加えた。1μCi/ウエルのトリチウム化チミジンを用い、採取前の18時間、ミクロタイターウエルを振動させた。平均ΔCPM(実験のCPM−応答体のCPMのみ)をY軸に、一次MLR培養日数をX軸に示す。このデータは、抗gp39mAb処置の培養基が強度の低反応性を示すことを証明する。この培養基に外因性IL−2を加えるとその低反応性を逆にし得る。

実施例2
図1で示す実験から得た普通に培養した細胞の上澄み液を、インターロイキン−2(IL−2)濃度について分析した。この結果を図1Aに示す。上澄み液の分析にはELISA(R&D Systems,Minneapolis, MN)を用いた。上澄み液濃度(pg/ml)をY軸に、MLR培養日数をX軸に示す。抗gp39mAbの追加は、一次MLR培養基中のドナーT細胞由来のIL−2産生を阻害した。

図2Aに示す実験に使用した同じ培養基中で、ELISAを用いてインターフェロンガンマの上澄み液濃度を分析した。この結果を図2Bに示す。抗gp39mAbの追加は、一次MLR培養基中のインターフェロンガンマを強度に低下せしめることがわかる。

実施例4
10日間の細胞培養の終わりにおいて、2色フローサイトメトリーで細胞の表現型を調べた。表1(実施例の後)から、高レベルのCD25、OX40、CTLA−4、B7−1、B7−2により立証されるように、抗pg39mAbの追加はT細胞の活性を予防しなかったことがわかる。しかしながら、高レベルのL−セレクチン、ICAM−1および低レベルのCD45が証明するように、抗pg39mAbの追加は、天然T細胞のエフェクターT細胞への変換を阻害した。処置培養基中の細胞がアポトーシスを受けなかったことは、7−AADについての陽性が比較的低いことで立証される。

実施例5
一次MLR培養の終わりに、細胞を洗い、96のウエルプレートを3x104濃度にして培養基を作り直した。それぞれのウエルに、C57BL6マウスの照射脾臓細胞を105の濃度にして加えた。この結果を図3に示す。表示のようにIL−2を最終濃度50単位/mlで加えた。培地は10%の子牛胎児の血清、5%の付随物および2−MEから成る。表示のように採取に先立つ18時間、各ミクロタイターウエルをそれぞれ1μCiでラベルした。平均増殖値(ΔCPM)をY軸に、二次MLR培養時間をX軸にとる。図3Aの結果からわかるように、一次培養において抗gp39mAbに露出され、二次培養においては露出されなかったドナーT細胞は、二次培養において同種抗原特異的応答性を保持した。この結果は、二次培養基のみに外因性IL−2を加えると、逆になる。

実施例6
別個の培養基において、対照処置培養基または抗gp39mAb処置一次MLRから得たドナーT細胞を、50単位/mlの最終濃度で外因性IL−2に露出した。この結果を図3Bに示す。二次条件下で検査して、対照処置から得たドナーT細胞が、抗gp39mAb処置の一次MLR培養の場合と等しい反応を示すことがわかる。

実施例7
二次MLRバルク培養基から得た上澄み液を、IL−2(図4)あるいはインターフェロンガンマ(図4B)の産生に関して、二次MLR培養で測定したようにELISAを用いて試験した。その結果、抗gp39mAbに一次MLR培養基で露出し二次では露出しなかったドナーT細胞は、IL−2(図4A)およびインターフェロンガンマ(図4B)の上澄み液の濃度が極めて低い状態を継続することがわかる。

実施例8
MLR培養の終わりにおいて、致死量以下の照射(身体照射総量600cGray)を受けたC57BL/6レシピエントに対してドナーT細胞を投与した。二種類の細胞投与量(105または3x105)で試験した。この結果を図5に示す。図5から、どちらの細胞投与量の対照培養細胞レシピエントも、移植後4週間になる前に、一様にGVHD致死となったことがわかる。それに反して、抗CD40L mAbに生体外で露出した105ドナーT細胞のレシピエントは88%の生存率を有した。抗CD40L mAbに露出した3x105ドナーT細胞のレシピエントは、移植後2ヶ月以上の期間において50%の生存率を有した。対照培養基から得たドナーT細胞のレシピエントと比較して、抗CD40L mAb処置に露出した同数のドナーT細胞のレシピエントは、その推計生存率が両方の投与量の場合とも有意に高かった(p<0.001)。

実施例9
図5Aに含まれる実験結果を、実験動物の平均体重曲線でGVHDのためにモニターした。この結果を図5Bに示す。それによると、対照細胞のレシピエントが平均体重カーブの顕著な減少を示し(Y軸)、その減少が移植後2.5週間で始まり、移植後4週間の前にCVHD致死となったことがわかる。これに反して、抗gp39mAb処置細胞のレシピエントは、移植前の平均体重を超える体重曲線を有した。同様に、対照培養基から得た105または3x105細胞のレシピエントは、GVH反応に一致して、著しい平均体重の減少を示した。これは、GVHD致死を抗gp39mAbによる処置で阻害できることを証明する。

別の実験において、抗gp39mAb処置培養のレシピエントが、対照培養のレシピエントに比べて、GVHD致死率が30倍まで減少した結果が観察された。

実施例10
ドナー同種活性T細胞の生体内増殖を検査した。図1〜5および表1〜2で示した実験から得たドナーT細胞を、重い複合免疫不全症のマウスに注入した。このレシピエントは、MHCクラスI+クラスII遺伝子座のドナーとは非対称である。移植後6日目に、1時間に1ml(1時間当たり、実験動物の総体水分のおよそ1/4〜1/3を意味する)の液体を連続的にマウスの静脈内に注入した。胸管リンパ本幹にカニューレを挿入して、胸管リンパ球を一夜かけて収集器で集めた。実験動物が死ぬまでに、1匹につき1時間に約1mlずつ集めた。次に、CD4+T細胞の1日当たりの産生数を定量した。対照培養細胞のレシピエントが、胸管流出液1mlにつき平均2x106のCD4+T細胞を産生したことがわかる。これに反して、抗gp39mAb処置培養のレシピエントは、1mlにつきわずか0.3x106のCD4+T細胞を産生したにすぎず、これは、生体内同種活性T細胞の発生がおよそ7倍減少したことを意味する。これらのデータによると、抗gp39mAbが生体内ドナーT細胞のGVHD致死仲介能力を減退させることもさらにわかる。

要約すると、上記の実験は、抗gp39mAbが、生体内におけるGVHDの能力を著しく減退させる明確な証拠を提供する。これは、生体内におけるGVHD致死予防の手段として、宿主抗原あるいは同種抗原に対する耐容性をドナーT細胞に与えるための新しい方法を意味する。

Claims (12)
- 所望の同種異系抗原または異種抗原を保持する細胞に対するドナーT細胞のT細胞耐容性または非応答性を体外で誘発する方法であって、下記:
(i)ドナーT細胞含有のドナー組織を含有する培養を準備し;
(ii)該ドナーT細胞培養に同種異系抗原または異種抗原を保持する細胞を加えることにより混合リンパ球反応培養をつくり;
(iii)得た混合リンパ球培養に、
a)gp39に対する抗体;b)gp39に対する抗体の断片または誘導体;c)可溶性CD40Ig
からなる群から選択される、gp39アンタゴニストを加え;
(iv)これらの細胞を培養中に1から30日間保持し、ドナーT細胞に、該同種異系抗原または異種抗原を保持する細胞に対する実質的な非応答性を与える;
ことを含む方法。 - ドナーT細胞を含有する組織がドナー骨髄または末梢血細胞である、請求項1の方法。
- gp39アンタゴニストが抗gp39抗体および可溶性CD40Igよりなる群から選ばれる、請求項1の方法。
- gp39アンタゴニストが抗gp39抗体である、請求項3の方法。
- 該抗gp39抗体が抗ヒトgp39モノクローナル抗体である、請求項4の方法。
- ドナーT細胞を該gp39アンタゴニストとともに、1日から30日の範囲の時間、培養する、請求項1の方法。
- 該時間の範囲が5日から15日である、請求項6の方法。
- 同種異系抗原または異種抗原保持細胞が、処置しレシピエントT細胞を除去した移植予定のレシピエントから得られた細胞または組織を含む、請求項1の方法。
- T細胞の除去が放射線処置によりなされる、請求項8の方法。
- 移植の必要なレシピエントにおいて移植処置のための、請求項1のドナーT細胞を含む医薬。
- 免疫再構築を必要とする疾患の処置における、免疫再構築のための請求項10の医薬。
- 癌または自己免疫疾患の処置のための、請求項11の医薬。
