Description
にドナー抗原(異種抗原または同種異系抗原)に対する耐容性を与えるために、
これらの臓器を生体外で処置する方法を提供する。処置した組織または臓器は、
移植片対宿主病の危険性を低下して、レシピエントに移植できる。

入で起きる多重臓器系の破壊プロセスである。急性の移植片対宿主病はHLA特
異的同胞間ドナー移植片で20−40%のレシピエントに、非関連ドナー移植片
で70−80%のレシピエントに生じるので、骨髄移植のこの合併症を防止する
手段が必要である。現在のところ一般的に言えば2種の対応がなされている。第
1は、メタトリシド、シクロスポリンA、ステロイドなどの免疫抑制剤のインビ
ボ注入である。上記したように急性の移植片対宿主病がインビボ免疫抑制剤の注
入において見られる。この防止効果が不完全なことに加えて、これらの免疫抑制
剤は骨髄移植後に長期間の免疫不全を起こす。これによって、レシピエントは感
染合併症の危険に曝され、骨髄移植後の再発の発生率が増加する。第2の対応は
、ドナー細胞からT細胞の生体外除去である。これは、急性の移植片対宿主病を
防止するのにかなり効果的であるが、移植片の失効、再発、感染合併症の発生率
が高くなり、免疫再構築の時間を遅くする。
移植に際しこのT細胞を同種異系または異種の抗原に対する実質的な非応答性を
与える。具体的には、本発明は、ドナーT細胞を、少なくとも1種のgp39(
CD154)アンタゴニストおよび同種異系または異種の細胞または組織で処置
する方法に関し、該ドナー細胞に、該同種異系または異種の細胞を含有する宿主
中に移植した際に、ドナー抗原(同種抗原または異種抗原)に対する実質的な非
応答性を与えることを目的とする。

片対宿主応答)が低下されることを意味する。

理論的には、本発明者の発表を含む以前の発表を基にして可能である。これらの
発表は、gp39アンタゴニスト(抗gp39抗体)のみで、または同種異系ま
たは異種の細胞と合わせて、異種または同種異系の組織または臓器の移植の前、
同時あるいは後に、移植レシピエントを処置することにより、同種異系または異
種の組織に対するT細胞の耐容性をインビボで誘発することに関する。このイン
ビボ手段は、種々の組織や臓器、例えば、膀胱、皮膚、心臓組織などに対するT
細胞の耐容性を起こすのに非常に効果的であるとされている。
し得るかどうかは、予測できなかった。この結果が合理的に予測できなかったの
は、文献に発表された以前の研究で、インビトロT細胞活性化の誘発にgp39
の存在を必要条件としていないことによる。例えば、Flavellら(Nature, 378: 6
17-620 (1995))によると、gp39発現を遺伝子的に欠くT細胞レポーター形質
転換T細胞が抗原提示細胞および抗原に正常に応答した。このことは、T細胞活
性化はgp39を必要としないで、gp39なしでインビトロで正常に起こり得
ることを示している。

される移植T細胞に、レシピエント同種異系抗原または異種抗原に対する耐容性
すなわち非応答性を与える、非常に効果的な非侵傷的手段をもたらすからである
。従って、この移植の組織または臓器すなわち異種または同種異系の骨髄は、移
植に際し有害な移植片対宿主応答を誘起しない。さらに、耐容性がインビトロで
誘発されることはさらに優れた点であり、他の拒絶反応に対する対策、例えば、
シクロスポリンなどの免疫抑制剤と合わせて、この処置をとり得る。また、移植
の前、同時あるいは後に、抗gp39抗体(または他のリガンド)投与を併用し
得る。
抑制作用からして、有害な副作用、例えば、感染や癌の危険性の増大をもたらす
ことがある。

移植片対宿主病の強度に30倍もの相違があった。このことからして、驚くべき
ことに、ドナーT細胞が混合リンパ球反応により生体外で効果的に耐容性が与え
られると結論した。これは、宿主細胞および異種抗原に対するドナーT細胞の耐
容性付与に重要な新しい手段を提供する。

gp39mAb処置の培養基が強度の低反応性を示すことを証明する。この培養
基に外因性IL−2を加えるとその低反応性を逆にし得る。

析にはELISA(R&D Systems,Minneapolis, MN)を用いた。上澄み液濃度(p
g/ml)をY軸に、MLR培養日数をX軸に示す。抗gp39mAbの追加は
、一次MLR培養基中のドナーT細胞由来のIL−2産生を阻害した。

現型を調べた。表1(実施例の後)から、高レベルのCD25、OX40、CTL
A−4、B7−1、B7−2により立証されるように、抗pg39mAbの追加
はT細胞の活性を予防しなかったことがわかる。しかしながら、高レベルのL−
セレクチン、ICAM−1および低レベルのCD45が証明するように、抗pg
39mAbの追加は、天然T細胞のエフェクターT細胞への変換を阻害した。処
置培養基中の細胞がアポトーシスを受けなかったことは、7−AADについての
陽性が比較的低いことで立証される。
射脾臓細胞を105の濃度にして加えた。この結果を図3に示す。表示のように
IL−2を最終濃度50単位/mlで加えた。培地は10%の子牛胎児の血清、
5%の付随物および2−MEから成る。表示のように採取に先立つ18時間、各
ミクロタイターウエルをそれぞれ1μCiでラベルした。平均増殖値(ΔCPM)
をY軸に、二次MLR培養時間をX軸にとる。図3Aの結果からわかるように、
一次培養において抗gp39mAbに露出され、二次培養においては露出されな
かったドナーT細胞は、二次培養において同種抗原特異的応答性を保持した。こ
の結果は、二次培養基のみに外因性IL−2を加えると、逆になる。

した。この結果を図3Bに示す。二次条件下で検査して、対照処置から得たドナ
ーT細胞が、抗gp36mAb処置の一次MLR培養の場合と等しい反応を示す
ことがわかる。
ーフェロンガンマ(図4B)の産生に関して、二次MLR培養で測定したようにE
LISAを用いて試験した。その結果、抗gp39mAbに一次MLR培養基で
露出し二次では露出しなかったドナーT細胞は、IL−2(図4A)およびインタ
ーフェロンガンマ(図4B)の上澄み液の濃度が極めて低い状態を継続することが
わかる。

%の生存率を有した。抗CD40L mAbに露出した3x105ドナーT細胞
のレシピエントは、移植後2ヶ月以上の期間において50%の生存率を有した。
対照培養基から得たドナーT細胞のレシピエントと比較して、抗CD40L m
Ab処置に露出した同数のドナーT細胞のレシピエントは、その推計生存率が両
方の投与量の場合とも有意に高かった(p<0.001)。

VHD致死を抗gp39mAbによる処置で阻害できることを証明する。
レシピエントに比べて、GVHD致死率が30倍まで減少した結果が観察された

した実験から得たドナーT細胞を、重い複合免疫不全症のマウスに注入した。こ
のレシピエントは、MHCクラスI+クラスII遺伝子座のドナーとは非対称であ
る。移植後6日目に、1時間に1ml(1時間当たり、実験動物の総体水分のお
よそ1/4〜1/3を意味する)の液体を連続的にマウスの静脈内に注入した。胸
管リンパ本幹にカニューレを挿入して、胸管リンパ球を一夜かけて収集器で集め
た。実験動物が死ぬまでに、1匹につき1時間に約1mlずつ集めた。次に、C
D4+T細胞の1日当たりの産生数を定量した。対照培養細胞のレシピエントが
、胸管流出液1mlにつき平均2x106のCD4+T細胞を産生したことがわ
かる。これに反して、抗gp39mAb処置培養のレシピエントは、1mlにつ
きわずか0.3x106のCD4+T細胞を産生したにすぎず、これは、生体内
同種活性T細胞の発生がおよそ7倍減少したことを意味する。これらのデータに
よると、抗gp39mAbが生体内ドナーT細胞のGVHD致死仲介能力を減退
させることもさらにわかる。

D致死予防の手段として、宿主抗原あるいは同種抗原に対する耐容性をドナーT
細胞に与えるための新しい方法を意味する。

Claims (12)
- 【請求項1】 所望の同種異系抗原または異種抗原を保持する細胞に対する
ドナーT細胞のT細胞耐容性または非応答性を誘発する方法であって、下記: (i)ドナーT細胞含有のドナー組織を含有する培養を準備し; (ii)該ドナーT細胞培養に同種異系抗原または異種抗原を保持する細胞を加え
ることにより混合リンパ球反応培養をつくり; (iii)得た混合リンパ球培養にgp39アンタゴニストを加え; (iv)これらの細胞を培養中に十分な時間保持し、ドナーT細胞に、該同種異系
抗原または異種抗原を保持する細胞に対する実質的な非応答性を与える; ことを含む方法。 - 【請求項2】 ドナーT細胞を含有する組織がドナー骨髄または末梢血細胞
である、請求項1の方法。 - 【請求項3】 gp39アンタゴニストが抗gp39抗体、溶性CD40、
溶性CD40融合タンパク質よりなる群から選ばれる、請求項1の方法。 - 【請求項4】 gp39アンタゴニストが抗gp39抗体である、請求項3
の方法。 - 【請求項5】 該抗gp39抗体が抗ヒトgp39モノクローナル抗体であ
