Description
本発明は、制御放出性の生物分解性微小球(microspheres)および極小球(nanospheres)配合物、および特に、シクロスポリンまたはシクロスポリン類似物含有生物分解性配合物に関する。
背景技術
シクロスポリンAは、とりわけTヘルパーリンパ球において、インターロイキン-2および幾つかの他のサイトカインの転写を阻害するカルシウム依存性、特異的および可逆的阻害を起こす真菌トリポコロジウム インフラタム ガスム(Tolypoclodium inflatum Gams)から単離される分子量1203の親油性環式ウンデカペプチドである。その免疫抑制性のために、これは移植拒絶および様々な自己免疫疾患の予防および処置における第一線の治療法として広く使用されている。標準的治療では治療しにくい深刻な疾患の患者の場合、経口シクロスポリンは急性の眼性ベーチェット症候群、内因性ブドウ膜炎、乾癬、アトピー性皮膚炎、リューマチ関節炎、活性クローン病(active Crohn's disease)およびネフローゼ症候群における有効な治療である。この薬剤はまた、骨髄移植ができない中程度および重度の再生不良性貧血の患者および原発性胆汁性肝硬変に罹っている患者の治療にも使用される。シクロスポリンはまた、治療が難しい壊疽性膿皮症、多発性筋炎/皮膚真菌症または重度のコルチコステロイド依存性喘息の患者にも有効である。シクロスポリンは、その正確なメカニズムは不明のままであるが、T-細胞増殖に非常に特異的効果を有することが知られている。これはまたヒトおよび囓歯目動物細胞における多剤耐性の有効な変性剤であることが示されている。シクロスポリンAの多数の非-免疫抑制性類似物質は、耐性変性活性を有することが示されており、その幾つかは親化合物よりも強力である。
多毛症、歯肉増殖症および神経系および胃腸管に対する作用は、シクロスポリン被投与者において最も一般的な副作用である。また、腎機能障害を示す検査項目値における変化もごく一般的に起きる。
シクロスポリンは、非常に親油性であり、水に溶けにくく、したがって一般には臨床用にオリーブ油またはピーナッツ油剤として供給される。しかしながら、このような油剤からのシクロスポリンの生物利用効率は非常に低く、大きな患間の変動が起こり、全身性の利用効率の報告値は4-25%の範囲である(ケイ.タカダ(Takada, K.)ら、J. Pharmacobio-Dyn., 11:80-7(1988))。シクロスポリンの生物利用効率は、食物、胆汁および他の相互作用因子に依存することが報告されている(ファー、エー.(Fahr, A.)、Clin.,Pharmacokinetics, 24:472-95(1993))。シクロスポリンの微小乳化製剤を小腸および大腸の異なった部分(十二指腸、空腸、回腸および下行結腸)へ局所投与した最近の研究では、シクロスポリンは主に小腸に吸収されることがわかった(ドレウェ、ジェイ.ら(Drewe, J. et al.)、Br.J.Clin. Pharmac., 33:39-43(1992))。
シクロスポリンAは、ポリ-D,L-ラクチド-コ-グリコリド微小球および極小球に封入された(アロンソ、ジェイ.(Alonso, J.)、Proceed. Intern. Symp. Control. Rel. Bioact. Mat., 20:109-10(1993))。しかしながら、これらの微小球および極小球は28日間内に捕捉したシクロスポリンの50%以上を放出することができなかった。
したがって、毒性および同一もしくは患者間での利用効率の変動を克服するために、生物利用効率が向上したシクロスポリン薬剤配合物についての必要性が存在する。さらに、シクロスポリンの吸収部位に対し有効にシクロスポリンを向かわせるシクロスポリン配合物の必要性が存在する。
発明の開示
本発明は制御放出性の薬剤配合物を提供するものであり、この薬剤配合物は、生物分解性ポリマーに捕捉されたシクロスポリンを含み微小球および極小球を形成し、その際シクロスポリンは実質的に非晶質状態であり、生物分解性ポリマーは、ポリ(ラクチド)を12.5%w/wより多く含むものである。
ここで使用される用語“シクロスポリン”はシクロスポリンAおよび同様の物理的特性を有するシクロスポリンAの類似物に関する。
ここで使用されるポリマーへ適用される用語“生物分解性”は、酵素的または非-酵素的のいずれかによりインビボ分解し、さらに通常の生理学的経路を介して代謝または排泄されうる生物適合性または非毒性副産物を生ずるポリマーを意味する。生物分解性の合成ポリマーの例は、ポリ(ラクチド);ポリ(グリコリド)およびポリ(ラクチド-コ-グリコリド)、それらの立体異性体(すなわち、D,L)、ラセミ混合物、およびポリマー混合物を含む。
生物分解性ポリマーは好適にはポリ-D,L-ラクチドまたはポリ-D,L-ラクチドおよびポリ-D,L-ラクチド-コ-グリコリドのブレンドであり、ただしブレンドは捕捉されたシクロスポリンが実質的に非晶質状態であるように十分なポリ(ラクチド)を含む。
驚くべきことに、本発明は、ポリ(ラクチド)微小球または極小球からのシクロスポリンの放出が相当するポリ(ラクチド-コ-グリコリド)微小球または極小球から放出されるものよりかなり高いことを示している。放出とこれに続く生物利用効率におけるこの増加は、ポリ(ラクチド-コ-グリコリド)配合物における結晶性シクロスポリンの存在と反対に、シクロスポリンがポリ(ラクチド)配合物において実質的に非晶質形で存在することと関係する。
本発明の制御放出性の薬剤配合物は、好適には37℃で浸漬条件(sink condition)下に測定した時、シクロスポリンに対する溶解プロフィールが実質的に以下のようである;
a) 1時間以内に40-80%放出;
b) 4時間以内に75-95%放出;
c) 8時間以内に80%≧放出;
すなわち、本発明は、制御されたやり方で8時間以内に捕捉された薬剤を80%以上、好ましくは90%以上を放出することができるシクロスポリン含有微小球または極小球を提供するものである。これらの微小球または極小球を被検者特にヒトへ経口投与した場合、胃におけるシクロスポリンの放出は、食物、胆汁または他の因子の存在による生物利用効率の変動を避けるために、最少限にされ、しかしながら、シクロスポリンの放出は小腸を標的にし、この部位においてシクロスポリンが主に吸収される。
生物分解性微小-および極小球は、シクロスポリンを好ましくは20-80%w/w、より好ましくは45-55%w/w含む。
特に好ましい配合物は、シクロスポリン50%w/wを充填したポリ-D,L-ラクチド-コ-グリコリドとポリ-D,L-ラクチド微小および/または極小球の80:20ブレンドである。この配合物は、8時間以内にシクロスポリンをほぼ完全であるが比較的ゆっくり放出するという組み合わせた特性を有し、そして経口投与されるとシクロスポリンが小腸を標的にするのに有用である。
本発明による微小-および極小球は、経口投与形たとえばカプセル、錠剤、水を添加すると起泡しうる粉末を含む粉末、または懸濁剤へ混入されることが好ましい。さらに、腸溶性コーティングを微小球もしくは極小球または経口投与形態へ施して胃を通る間配合物を保護し、シクロスポリンを小腸へ標的放出させるようにすることができる。これに代わって、本発明による微小-および極小球は、非経口投与され、捕捉されたシクロスポリンの80%以上好ましくは90%以上を8時間かけて調節された方法で放出することができる。
したがって、便利で効果的な経口投与のためには、本発明の微小および/または極小球の有効量を一つ以上の賦形剤とともに錠剤化するか、カプセルたとえばゲルカプセル封入するか、水を添加した場合起泡溶液を提供する成分と配合するか、または液剤に懸濁する。微小-および極小球は、非経口投与に対しては生理食塩溶液等に懸濁されうる。
本発明による薬剤配合物は、中でも器官移植患者における免疫抑制をもたらし、そして自己免疫疾患を治療するために使用されることが理解されるであろう。
好適には、本発明の配合物は、シクロスポリンAの全血レベルが200〜500ng/mlの間に維持されるようにヒトへ投与され、このレベルでは深刻な腎臓の毒性が稀であると一般には思われる。したがって、本発明の配合物の好適な投与量は微小-または極小球に捕捉されたシクロスポリン5mg/Kg/日未満である。
シクロスポリン(ライラス(Leiras))20-80%w/wを含む生物分解性微小-および極小球は、乳化システムからの溶媒蒸発/抽出法により(ラムトーラ、ゼットら(Ramtoola, Z. et.al.,)、J.Microencapsulation,9:415-23(1992))好適に調製される。使用されるポリマーは、Mw16,000を有しそしてi.v. 0.2dl/gのポリ-D,L-ラクチド(R-203;ベーリンガーインゲルハイム)およびi.v. 0.5dl/gのポリ-D,L-ラクチド-コ-グリコリド50:50(RG-504 ;ベーリンガーインゲルハイム)である。
たとえば、シクロスポリンおよび封入用ポリマーを塩化メチレンに溶解する。好適な塩化メチレンのポリマーに対する比は、1-2ml塩化メチレン/1g R-203および3ml塩化メチレン/1g RG-504である。次いでこの薬剤/ポリマー溶液を、水性PVA溶液(好適には0.27%)中において、該PVA溶液100mlに対し薬剤/ポリマー溶液約10mlの比で好適に乳化し、そして高速(好適には20,000-24,000rpm)で2分間混合し、続いて1000rpmで2時間攪拌する。遠心分離により粒子を回収し、減圧炉において一晩乾燥する。
以下の実施例において、製造された粒子は走査電子鏡検法(S360,ライカ、ケンブリッジ)により特性決定され、マルベルン(Malvern)2600レーザーサイザー(Laser Sizer)を用いてサイズ決定された。粒子はまたX線回折(Daco-MP 500,シーメンス)によっても特性決定された。微粒子の薬剤含量率は、HPLCにより、ノバパックC8カラムを用いて70℃にて、アセトニトリル:水:メタノール:リン酸(900:525:75:0.075)の移動相を用い、流速2ml/分で210nmのUVを用いて検出することにより検定された。
シクロスポリンの溶解度をラウリル硫酸ナトリウム(SLS)溶液の濃度を増加しながら37℃で測定し、溶解研究のための浸漬条件を決定した。予期したように、シクロスポリンの溶解度(S)は、以下の式に従って、SLS濃度(M)の増加に連れて直線状に増加することがわかった;
S=S0+K.M
(式中、S0はシクロスポリンの固有溶解度であり、Kは可溶化能力である)。
相関係数0.997で0.284の値がKに対し与えられ、SLS濃度≧0.3%(w/v)が浸漬条件を提供するために見出された。シクロスポリンのインビトロ放出研究を、浸漬条件下で、バンケル(VanKel)溶解浴を用いて行い、そして溶解サンプルをHPLCにより分析した。
【図面の簡単な説明】
図1は、薬剤充填率が25%から30%まで40%まで50%まで70%まで80%(w/w)まで増加するにつれてシクロスポリン充填ポリ-D,L-ラクチド微小球からシクロスポリンの放出が増加することを示し、そして
図2は、シクロスポリンが主に非晶質状態にあるシクロスポリン充填ポリ-D,L-ラクチド微小球またはポリ-D,L-ラクチド:ポリ-D,L-ラクチド-コ-グリコリド微小球(すなわち、ポリ-D,L-ラクチド-コ-グリコリド:ポリ-D,L-ラクチド比0:100,50:50,75:25および80:20を有する微小球)からのシクロスポリンの予期せぬ大きな放出と、これと対照的に結晶状態のシクロスポリンが存在するシクロスポリン充填微小球(すなわち、ポリ-D,L-ラクチド-コ-グリコリド:ポリ-D,L-ラクチド比87.5:12.5および100:0を有する微小球)からのゆっくりした放出を示す。
発明を実施するための方法
本発明をさらに以下の実施例により説明する。
実施例1
シクロスポリン充填ポリ-D,L-ラクチド微小球
封入用ポリマーとしてR-203を使用し前記の方法で調製したシクロスポリン充填微小球(20,500rpmで2分間混合したエマルジョン)の特性を、表1に示す。製造された粒子の薬剤捕捉効率は薬剤充填率に依存せず、すべての場合93%より高かった。顕微鏡写真は製造された粒子の表面が滑らかでありこの範囲の薬剤充填率で薬剤が表面に存在しないことを示す。走査電子鏡検法では直径5μ以下であることが示された。粒子サイズの分析では、粒子がより大きいD50%値を与え、これは多分この測定の間に粒子が凝集した結果であろう。微小球のX線回折は、シクロスポリンがこれらの薬剤充填率の全てに対して非晶質状態で存在することを示した。
微粒子からのシクロスポリンの放出は薬剤充填率が高いほど迅速であることが見出され、これは図1に示される。初期の爆発的放出が研究された全ての系について観察され、これは薬剤充填率が増えるほど大きかった。この爆発的放出は通常は微小球の表面またはその近くに位置する薬剤と関係するので、微小球の薬剤充填率が増加するにしたがって増加することが予想された。シクロスポリン放出は高い薬剤充填率(>40%)においては拡散制御メカニズムに従った。低い薬剤充填率では、初期放出(最初の24時間にわたって)は拡散により行われた。続く微小球からのシクロスポリンの放出はよりゆっくりであり、多分ポリマー分解により制御されたのであろう。
実施例2
シクロスポリン充填ポリ-D,L-ラクチド-コ-グリコリド:ポリ-D,L-ラクチド微小球
50%w/w薬剤充填率のシクロスポリン充填微小球は、表2に示すようにより親水性のRG-504ポリマーおよびRG-504およびR-203の様々なブレンド(24,000rpmにて2分間混合したエマルジョン)を用いて前記方法にしたがって調製された。ポリ-D,L-ラクチド微小球と同様に、顕微鏡写真は、製造された粒子の全ての表面が滑らかで薬剤が存在せず、5μ以下の直径であることを示した。薬剤の捕捉効率は使用するポリマーブレンドに依存せず、そして79.5%以上であることがわかった。ポリ-D,L-ラクチド微小球と対照的に、100%ポリ-D,L-ラクチド-コ-グリコリド微小球およびポリ-D,L-ラクチド12.5%までを有するもののX線回折は結晶性シクロスポリンの存在を示した。
より親水性のポリ-D,L-ラクチド-コ-グリコリド微小球からのシクロスポリンの放出は、図2に示すようにポリ-D,L-ラクチドから調製された相当する微小球についてよりもゆっくりであった。ブレンドポリマーの有する初期爆発的放出は、ポリ-D,L-ラクチドだけのもので観察されたものより低く、そしてポリ-D,L-ラクチド含有率が増加すると増加することが見出された。より疎水性のR-203の比がポリマーブレンドにおいて増加するので、シクロスポリンの放出は、ポリ-D,L-ラクチド含有率が12.5%から20%w/wまで増加すると溶解速度における著しい増加とともに増加することが見出された。
ポリ-D,L-ラクチド-コ-グリコリド微小球からのシクロスポリンのゆっくりした放出は、ポリ-D,L-ラクチド-コ-グリコリドの方がより親水性で、したがってポリ-D,L-ラクチドより簡単に湿潤されるので予期されなかった。しかしながら、シクロスポリンが疎水性薬剤であるので、これが疎水性ポリ-D,L-ラクチドマトリックスにおいて分子分散を形成し、より高い初期爆発的放出効果が起こり迅速な放出曲線が観察されたと考えられる。より親水性の微小球(すなわち、ポリ-D,L-ラクチド12.5%w/wまで含む微小球)における結晶性シクロスポリンの存在と比べて封入されたシクロスポリンが非晶質状態であることを示すポリ-D,L-ラクチド粒子についてのX線回折データにより、これらのより親水性微小球から観察された予期せぬゆっくりした薬剤放出を説明することができる。
Claims (9)
- 微小球または極小球を形成する生物分解性ポリマーに充填されたシクロスポリンを含んでなり、シクロスポリンが実質的に非晶質状態であり、かつ、生物分解性ポリマーは100%がポリ(ラクチド)からなるか又は12.5%w/wを越えるポリ(ラクチド)を含むポリマーブレンドからなる、制御放出性の薬剤配合物。
- 生物分解性ポリマーがポリ−D,L−ラクチドである請求項1の制御放出性の薬剤配合物。
- 生物分解性ポリマーがポリ−D,L−ラクチドおよびポリ−D,L−ラクチド−コ−グリコリドのブレンドである請求項1の制御放出性の薬剤配合物。
- シクロスポリンについて37℃で浸漬条件で測定された溶解プロフィールが実質的に以下のようである;
a) 1時間以内に40〜80%放出;
b) 4時間以内に75〜95%放出;
c) 8時間以内に80%≧放出;
請求項1〜3のいずれか1の制御放出性の薬剤配合物。 - さらに、小腸に対してシクロスポリンを標的放出するために微小球または極小球上に腸溶性コーティングを含む請求項1〜4のいずれか1の制御放出性の薬剤配合物。
- 微小球または極小球の薬剤充填率が約20%から80%までの範囲である請求項1〜5のいずれか1の制御放出性の薬剤配合物。
- 微小球または極小球がカプセル、錠剤、粉末、水を添加すると気泡しうる粉末、または懸濁剤として配合される請求項1〜6のいずれか1の制御放出性の薬剤配合物。
- 微小球または極小球が錠剤として配合され、さらにシクロスポリンの小腸に対する標的放出を行わせるために錠剤上に腸溶性皮膜を含む請求項7の制御放出性の薬剤配合物。
- 微小球または極小球の薬剤充填率が45〜55%の範囲であり、生物分解性ポリマーがポリ−D,L−ラクチド:ポリ−D,L−ラクチド−コ−グリコリド20:80ブレンドである請求項1〜8のいずれか1の制御放出性の薬剤配合物。
