水化物欠乏トランスフェリン(CDT)の分離及び定量方
法に関する。
る多大の損害と、アルコールに関連した罹病率及び死亡
率による社会に対する重大な費用とを生じる。早期の認
識と治療とが個人にとって有利であり、社会にとっても
費用効果的であることが判明している。種々な集団にお
ける飲酒の併発症の危険にある個人を確認するための敏
感で、特異的な、迅速で、費用のかからない方法が必要
である。重度のドリンカーを検出するように設計され
た、多くの手段がこの30年間に開発されている。例えば
γ−グルタミルトランスフェラーゼ(γ−GT)、平均血
球容積(mean corpuscular volume)(MCV)、アスパル
テート又はアラニンアミノトランスフェラーゼ(AST又
はALT)、α−リポタンパク質及びフェリチンのような
慣習的な実験室試験が、アルコール乱用の生化学的マー
カーとして多年にわたって用いられているが、これらの
試験の診断的感度及び特異性は低い。アルコール関連小
脳性振せんを有する患者の脳脊髄液及び血清中のトラン
スフェリンのイソフォーム(isoform)における質的変
化が報告されている。アルコール利用に関連するイソフ
ォームは他のイソフォームよりもシアル酸含有量が少な
いので、含有量(charge)に応じて識別されることがで
きる。イソフォームを分離する幾つかの方法が紹介され
ているが、これらの方法は一般に実験室向きであり、非
定量的であり、費用がかかる。その後に免疫固定(immu
nofixation)、ウェスターンブロット法又はゾーン免疫
電気泳動を実施する、等電点クロマトグラフィー、RIA
(ラジオイムノアッセイ)と組み合わせた使い捨て式ミ
ニカラム及び等電点電気泳動が利用可能な方法である。

ラミン酸)を有する、2個のビー又はトリーアンテナリ
ー(bi−or tri−antennary)炭水化物鎖を有する糖タ
ンパク質である。等電点電気泳動はノルマルトランスフ
ェリンを高い分解能によって、鉄飽和度(ironsaturati
on)、シアル酸(SA)含量又はアミノ酸置換度(amino
acid substitution)に依存してイソフォームに分離す
る。完全な鉄飽和(2Fe原子/モル)後にトランスフェ
リンは通常、それらの大体の等電点(pI)に因んで、pI
5.2(5 SA)、pI 5.4(4 SA,主要画分)、pI
5.6(3 SA)及びpI 5.7(2 SA)と呼ばれる4イソ
フォームに分離する。通常の血清中にはpI 5.6及び5.7
を有する少量のトランスフェリンが存在する。アルコー
ル中毒者からの血清中で顕著に増加するイソフォームは
pI 5.7である。pI 5.7画分は通常、トランスフェリン
全体の0.8%未満であるが、重度のアルコール摂取後に1
0倍を越えて増加する可能性がある。過度の飲酒後に、
さらにpI 5.9画分(0 SA)が出現する可能性があ
る。pI5.7とpI 5.9画分は炭水化物欠乏トランスフェリ
ン(CDT)を表す。
症の高い危険性にある対象を確認するために適したHPLC
(高性能液体クロマトグラフィー)方法を開発し、特定
の集団における重度のアルコール摂取の検出に関するこ
の方法の感度と特異性とを評価することである。この目
的は、臨床的標準方法に適し、先立つ数週間中の重度の
アルコール摂取に関する、適当な生化学的マーカーであ
るCDTの濃度を特異的に測定する方法を提供することで
ある。この目的は下記主要請求項に記載の方法によって
満たされる。

Tレベルを示す。
血清1mlにつきNaHCO325μl(500mmol/l)とFeCl318μ
l(10mmol/l)とを添加することによって鉄で飽和させ
て、サンプルを用意した。混合し、+8℃において一晩
貯蔵した後に、血清1mlにつき硫酸デキストラン(10%
(w/v))10μlとCaCl2(1mol/l)50μlとを加えるこ
とによって、リポタンパク質を沈殿させた。この混合物
を+8℃に30〜60分間貯蔵し、次に10,000 x gにお
いて10分間遠心分離した。上澄み液を水によって5倍に
希釈して、HPLCオートインジェクター(autoinjector)
に移した。

(A)はBis Tris 20mmol/l(pH6.2)であった。バッ
ファーBはバッファーA+同じpHのNaCl 350mmol/lで
あった。溶液C(NaCl 1mol/l)は再生のために用い
た。使用前に、全ての溶液を脱気し、0.45μm孔度フィ
ルターに通して濾過した。200μmのサンプルを注入
し、流量を1ml/分に維持し、図3の表に示した傾斜プロ
フィルを用いた。
No.2941(Pharmacia Biotechnology,スウェーデン)、
460nmフィルターを備えたJasco870 UV検出器及び10mm
フローセル(flow cell)から成るものであった。この
系は96サンプルに対する冷却系と共に、オートインジェ
クター、Waters WISP715を含有した。谷−谷モードに
応じてピーク面積を算出するためにShimazuCR5A積分計
を用いた。

t cathodal)イソフォームの最適分離はpH6.2において
得られた。それにも拘わらず、6.0〜6.4のpH値を有する
バッファー溶液によっても許容される結果が得られてい
る。しかし、最良の結果を得るためには、pHは6.1〜6.3
であるべきであり、最も好ましくは、pHは6.2であるべ
きである。NaHCO3をFeCl3と共に加えると、最高に安定
な鉄飽和が得られる。リポタンパク質の沈殿は、一部の
患者において、重いβ−リポタンパク質画分の下に隠さ
れる可能性があるpI 5.9イソフォームの分離を改良す
る。低いレベルのリポタンパク質を有する絶食(faste
d)サンプルは通院規模でのアルコール中毒者の研究に
用いることができない。図1は重度、中程度及び正常の
飲酒個人からのHPLCによるクロマトグラムを示す、図に
おいてpI 5.9イソフォームも第1パターンに出現す
る。460nmにおけるFe−トランスフフェリン錯体の吸光
度は280nm吸光度の約1/10であるが、トランスフェリン
画分に高度に特異的である。CD−トランスフェリン(pI
5.7)の量は絶対禁酒者及び偶発的ドリンカーにおけ
るトランスフェリン総量の僅か0.2〜0.8%(平均値±2
S.D.)に過ぎない。この値はクロマトグラフィープロフ
ィルの積分方法にやや依存した。基底ライン積分を用い
て、やや高い値が検出されたが、谷−谷方法(velley−
valley method)の方が再現可能であった。

はCDT、すなわちpI 5.7(*)とpI 5.9(**)を有
するトランスフェリンイソフォームを示す。クロマトグ
ラムの他のピークは右から、それぞれ、pI 5.2、5.4及
び5.6を表す。このクロマトグラムから、主要な画分がp
I 5.4であることが明らかである。さらに、CDTの値が
重度なアルコール摂取後に上昇することが明らかであ
る。Fe−トランスフェリン錯体の吸光度は460nmにおい
て測定された。
間の入院期間連続して測定することによって、CD−トラ
ンスフェリンの半減期を研究した、この時点においてア
ルコール乱用の再発は生じなかった。各サンプル中の総
トランスフェリン濃度(g/l)を測定し、CDTを算出し、
半対数ダイアグラム(図2)にmg/lとして示した。一部
の患者では入院期間中に総トランスフェリン濃度が時間
と共に上昇した。各曲線の半減期をその直線勾配から読
み取った。個々の患者間には小さな差異があり、平均T
1/2は9.5±1日間であると算出された。
からのサンプル中に検出されたCDT値は正常分布を示
し、谷−谷積分(図4)を用いると、一貫して1%未満
であった。高γ−GT値の前歴を有する、一般都市人口か
らの284人男性中で20%が40〜70g/24時間のアルコール
摂取を認めた。これらの男性では、CDTの感度(sensiti
vity)は55%であり、0.8%のカットオフレベルを用い
て、特異性は91%であった。重度アルコール中毒ドリン
カー(70〜500g/24時間)では、感度はほぼ100%であっ
た。嫌忌療法中に84%においてCDTの正常化が見られ
た。かれらの一部はかれらの基底CDTレベルにまだ達し
ていなかった。我々は、炭化カルシウム錠剤による治療
中の適度の飲酒を排除することはできない。
リーズに関しては自動化することができる。40人の患者
サンプルを24時間分析することができる。HPLCのための
投資を含めた試薬の費用は、ラジオイムノアッセイと組
合せた使い捨て式ミニカラム(Pharmacia Diagnostic
s)のための試薬費用の約30%である。この方法は2通
りにランしなければならない。HPLCの他の利点は、トラ
ンスフェリンの遺伝的変化において重要である特異的46
0nm吸光度の目視可能な資料であることである。アルコ
ール中毒者からの血清はしばしば高脂肪血症であり、一
部のリポタンパク質並びに他の血清タンパク質はpH6.2
において沈殿して、カラム圧力を徐々に上昇させる。そ
れ故、1つのカラムの再生中に分析を実施することがで
きるように、2個のカラムを用いることが便利である。
このアプローチは我々に、問題なく、1年間の経験(>
1000サンプル)を与えている。トランスフェリンの遺伝
的変異体(genetic variant)は人口の約29%に存在す
ると推定される。これらの大部分は主要Tfc表現型のサ
ブタイプ(subtype)であり、pI:sの変化は最小であ
る。これらはクロマトグラフィーパターンに干渉しな
い。白人の(caucasian)人口において出現頻度(frequ
ency)1〜2%の、TfBCヘテロ接合体とTfCDヘテロ接合
体のみがクロマトグラフィープロフィルに干渉する。こ
れらの場合に、専門実験室において等電点電気泳動によ
って結果を確認することが重要である。
(登録商標)による前処理によって血清からアルブミン
を除去した。使い捨て式カラム(ポリプロピレン5ml,Pi
erce)に、0.2g(乾重量)に相当する膨潤Blue−Sephar
ose(登録商標)Cl−6B(Pharmacia Biotechnology,ス
ウェーデン)を充填した。製造者の指示に従ってすすぎ
洗いした後に、小型カラムをグリシン(100mmol/l)(p
H7.2)と平衡化させた。鉄飽和した、リポタンパク質を
含まない上澄み液(100μl)にβ−メルカプトエタノ
ール10%(v/v)(2μl)を混合し、室温における1
時間後にミニカラムに供給した。血清タンパク質をグリ
シン(100mmol/l)(pH7.2)によって溶出した。

る再生後に数回使用可能である。

ォームを検出することができる。HPLCからの全ての病的
結果(CDT>0.8%)は今までは等電点電気泳動によって
確認されていた。

ン総量中のCDT割合を用いる動機を与える。pI 5.7イソ
フォームの約9.5日間の半減期は過去1〜3週間中のア
ルコール摂取の評価を可能にし、治療の成功を実証す
る。この結果はCDTに関して最近推定された15日間の半
減期ではなくノルマルトランスフェリンに関する8〜10
日間の発表半減期と一致する。

にCDTのために血液サンプルを採取した。40g/日より多
いアルコール摂取を報告した群において55%の感度が認
められた。しかし、70g/日より多くを摂取したアルコー
ル中毒個人の研究では、感度はほぼ100%であった。絶
対禁酒者及び偶発的ドリンカーにおける高い特異性、ほ
ぼ100%はCDTが利用可能な生物学的マーカーの中で最高
の特異性を有するという結論を可能にする。
Claims (10)
- 【請求項1】血液血清中のCD−トランスフェリンの分離
及び定量方法であって、血液血清を鉄によって飽和し、
トランスフェリンをイオン交換カラムにおいて塩傾斜を
用いて他の血清タンパク質から分離し、高性能液体クロ
マトグラフィーを用いて、異なるpI値を有するトランス
フェリンイソフォーム間の関係を示すクロマトグラムを
展開させることを特徴とする方法。 - 【請求項2】6.0〜6.4のpH、好ましくは6.1〜6.3のpHを
有するバッファー溶液中で分離を実施することを特徴と
する請求項1記載の方法。 - 【請求項3】6.2のpHを有するバッファー溶液中で分離
を実施することを特徴とする請求項1記載の方法。 - 【請求項4】460nmにおけるFe−トランスフェリン錯体
の選択的吸光度を特徴とする請求項1記載の方法。 - 【請求項5】クロマトグラフィープロフィルを谷−谷方
法を用いて積分することを特徴とする請求項4記載の方
法。 - 【請求項6】血清1mlにつきNaHCO3(500mmol/l)25μl
とFeCl3(10mmol/l)18μlとを加えることによって、
鉄飽和を実施することを特徴とする請求項1記載の方
法。 - 【請求項7】血清1mlにつき硫酸デキストラン(10%,w/
v)10μlとCaCl2(1mol/l)50μlとを加えることによ
って、混合及び+8℃における一晩の貯蔵後に、リポタ
ンパク質を沈殿させることを特徴とする請求項6記載の
方法。 - 【請求項8】得られた混合物を+8℃において30〜60分
間貯蔵してから、約10000 x gにおいて約10分間遠
心分離することを特徴とする請求項7記載の方法。 - 【請求項9】遠心分離した混合物の上澄み液を水によっ
て5倍に希釈することを特徴とする請求項8記載の方
法。 - 【請求項10】希釈した上澄み液をHPLCに注入すること
を特徴とする請求項9記載の方法。
