Description

【発明の詳細な説明】 本発明は、ナットが螺合する突出ネジ付き頭部から上
端部で当接部を形成する断面多角形の中間ボディによっ
て隔てられた下部の錨止部を有する骨アンカー部材と、
接続ロッドを受け入れ可能な受入ロック構体とを備え、
前記錨止部がネジの場合に前記中間ボディによって錨止
部を骨にねじ込み可能とし、更に前記構体が接続部で連
結された二分岐をもつクランプ部材を備え、各分岐には
前記頭部を当接部まで挿入可能な整列した穴が設けら
れ、クランプ部材が両分岐の間に通路を画定している特
に調整可能な接続ロッドのための脊椎用装着具に関す
る。
本発明はまた、ロッドをアンカー部材に接続するのに
特に適した受入ロック構体にも関する。
フランス特許第2657775号公報には、脊椎の整復及び
拘束に適したロッド形脊椎用装着具が述べられており、
この脊椎用装着具は、ネジ付き円柱頭部を有する掘削ネ
ジを備え、その円柱頭部は、ネジ頭部に係合した部分を
受け入れて該部分のベアリング部とネジ頭部に係合した
ロックナットの円錐部との間で接続ロッドのロックを可
能としている。
この形式の装着具はロッドをネジに対してロックする
が、ネジに対するロッドの向き、即ちネジと直交するロ
ッドの向きは、全てのネジで同一であって変更不可能で
ある。
フランス特許第2693365号公報には、ロッドと掘削ネ
ジの各軸間の角度を特定の平面内で多少調整できるよう
にし（即ち、この平面をロッドの軸回りに枢動可能とす
ることなく）、ロッドをその位置でネジにロックするよ
うにしたロッド形脊椎用装着具が述べられている。
この装着具は、中間ボディを有するネジ付き頭部を持
った掘削ネジと、このネジ付き頭部に長穴で係合して、
ロッドをネジの側部で中間ボディの球面状の上面に当接
させて受け入れるためのハウジングを提供する受入ロッ
ク構体とを備えている。ネジ付き頭部に螺合したナッオ
の裁頭円錐状の下面は、ロッドとの当接部位の反対側で
受入ロック構体の対応する裁頭円錐状表面に接触状態と
なる。
この場合、ネジに対するロッドの角度を増加してロッ
ドの曲がりとそれに基づく多数の掘削ネジと結合した装
着具配列内の応力とを更に減少可能とする一方でロック
機能及びその結果として構造の剛性を顕著に改善しうる
受入ロック構体が望まれることは述べるまでもない。
米国特許第5222954号明細書には複数のロッドを結合
するための脊椎用装着具が述べられ、この装着具は、ナ
ットが螺合する突出ネジ付き頭部から上端部で当接部を
形成する断面多角形の中間ボディで隔てられた錨止部
と、突出ネジ付き頭部16と通路34との係合を可能とする
穴28の形式の受入ロック部26とを備え、通路34の軸心は
穴28の軸心と直交し、穴28が接続ロッド40を受け入れる
ように構成されている。この装置では接続ロッドを全方
向に向けることができず、これは本発明の装置で可能と
なることに注意すべきである。
同様に、ヨーロッパ特許出願公開第585518号公報及び
米国特許第5352226号明細書に開示されているものも接
続ロッドを全方向に向けることはできない。
米国特許第4771767号明細書に開示されたものではネ
ジ付きロッドを用いることが要求され、脊椎の奇形や解
剖処置に適合するように変形できないという欠点があ
る。
従って本発明の目的は、中立軸に対して或る角度内で
ロッドを全方向に向けることを可能としつつ接続ロッド
の改良されたロック方式を特徴とする脊椎用装着具を提
供することにある。
本発明は、前記通路がキャビティを画定している点
と、接続ロッドを受け入れる中央通路を有し、前記ネジ
付き頭部に螺合したナットによる両分岐の締め付けで前
記接続ロッドを所期の角度位置にロックする玉継手を構
成する圧縮可能なリング部材が前記キャビティ内に配置
されている点とを特徴とする。
本発明の脊椎用装着具は、例えばステンレス鋼やチタ
ン基合金のような外科移植用の生体適合材料製のものと
する。
骨アンカー部材は、それ自体公知の形式で掘削ネジま
たはフックとすることができる。他の形式のアンカー手
段も本発明の範疇で使用可能である。
前記「接続ロッド」は、本質的に滑らかな部材、即
ち、クランプ前においてリング部材を貫通して自由にス
ライドできる部材である。
ロッドは、脊椎の応力と荷重に抗するに充分な直径の
滑らかな接続部材とすることができ、その滑らかな表面
は、脊椎の奇形や解剖処置に適合する或る程度の曲がり
が得られるように変形できるという利点をもたらす。同
様に、該ロッドは編組可撓性ワイヤーからなっていても
よい。
外科医の仕事を容易にするために、キャビティの軸心
に対するリング部材の軸心の振れ角を制限することは好
ましいことである。これにより、前記通路が常に開口す
るようになるのでロッドの挿入が容易になる。このため
の有利な方策は、リング部材に回転規制用の一つ以上の
当接部を設けることである。
リング部材のロックを改良するために、キャビティに
よってリング部材上の幾つかの点に作用する押圧力を強
めるための適宜な追加手段を設けることは有利である。
この目的で、好ましくはキャビティにはリング部材を押
圧する少なくとも一つの尖鋭エッジが設けられる。これ
は、ロック位置においてリング部材で支承される二つの
尖鋭エッジを与えるキャビティ内の溝であってもよい。
クランプ特性は、キャビティ及びリング部材の表面をサ
ンドブラストまたは機械加工で少なくとも部分的に粗面
化することによっても改善可能である。
ロック機能を更に改良するために、前記ナットは球面
状に膨出した下面を有しており、これが対応する前記分
岐の穴を囲む球面状の窪み部に適合している。リング部
材は、有利には円環状、好ましくは球状リングで膨らん
だ外面を有し、その外径は、キャビティ内奥の中央環状
溝の内径とその両端に接するより小径の端側円筒内壁部
の内径との間である。
リング部材の圧縮機能は、好ましくは切割スロットに
よって保証する。リング部材の圧縮変形性能は、前記ス
ロットと対称的な反対側の溝によって更に改善できる。
この他の特徴と利点は、以下に図面を参照して述べる
本発明の装着具の実施形態の説明から明らかである。

図１を参照して、掘削ネジ１は、脊椎の骨部に貫入す
るに適した下部のネジ付き先端部２と、ナット５が螺合
可能な上部のネジ付き頭部３とを有している。先端部２
と頭部３とは、図示の実施例ではレンチを用いて先端部
２をねじ込み可能とする多角形断面形状の中間ボディ４
によって隔てられている。
ネジ付き頭部３は中間ボディ４から滑らかな部分６に
よって隔てられ、この部分６の直径は、例えば頭部３の
ネジのルート直径に対応している。多角形の中間ボディ
４は、当接部を形成する平らな上面７を有している。
図３、４および５はクランプを形成する部材９を示し
ており、これは本装着具の受入ロック構体８の一部であ
る。クランプ部材９は、下部分岐10と上部分岐12および
これら両分岐を連結する接続部12を有している。下部分
岐10の下面には、下向き外方凹部15を画定する直角段部
13が設けられている。上部分岐11の上面には、穴18と同
心の球面状の窪み16を画定する曲面エッジ14が形成され
ている。両分岐10および11は、同軸状に整列された適切
な直径の穴17および18を有し、これらの穴は、ネジ付き
頭部３が当接部７まで挿入できるように整列され、充分
に挿入された状態では中間ボディ４の縦面19が直角段部
13に支承されて掘削ネジ１とクランプ部材９との相対回
動が阻止されるようになっている。ナット５は、窪み部
16に適合する球面状の膨出した下面31を有している。
クランプ部材は、その両端面に開口する内部キャビテ
ィ20を画定している。このキャビティは内奥に中央環状
溝21を有し、該中央環状溝の両端には、それよりも小径
の側端円筒内壁22がそれぞれ接して形成されており、こ
れら環状溝と円筒内壁の三つの円筒部は互いに同軸であ
る。中央環状溝の内径が両側端円筒内壁の内径よりも大
きいことにより、境目の段部に尖鋭な全周エッジ23が形
成されている。各側端円筒内壁23の外方開口縁には、好
ましくは面取り部33を設けてロッドの挿入を容易にする
と共に、ロッドの角運動を自由に妨げないようにする。
図６〜８を参照して、前記キャビティ内には、好まし
くはキャビティよりも短い長さ寸法のリング部材24が配
置され、このリング部材は、直線状の切割スロット25を
備えている。リング部材24は、その内部の中央通路26内
に円筒又は円柱状の接続ロッド27を受け入れ可能であ
る。リング部材の外面は中央部に球面状膨出部28を含
み、この膨出部の頂部は、非クランプ状態でキャビティ
の中央環状溝21内に配置される。また、この膨出部の両
端には、それぞれ外方へ向けて円筒部29が延在してい
る。これら延在円筒部は環状当接部を構成し、それによ
り、非ロック位置において該環状当接部がキャビティの
側端円筒内壁22の内面で支承されてリング部材の軸心の
振れ角が制限されるようになっている。スロット22と対
称な位置で前記膨出部に設けられた溝30がリング部材の
撓み性能を改善している。
ここで、リング部材の球面状の膨出部28の外径は、キ
ャビティ内奥の中央環状溝の内径未満で且つ両側端円筒
内壁の内径よりも大きく選ばれている。このようにし
て、リング部材がキャビティ内にクリアランスを有して
安定に捕らえられている。
さらに、当接部29によるリング部材の角運動の規制に
より、リング部材の内部の中央通路26がクランプ部材の
キャビティ20の入口と正しく整列していることが保証さ
れる。
このようにして定められる円筒状キャビティの軸心に
対するロッドの回動可能な角度は好ましくは15度であ
る。ナット５をネジ付き頭部に螺合して締め込むことに
より、クランプ部材９の両分岐10,11がクランプ動作を
行ってリング部材24を圧縮する。スロット25と溝30で与
えられているリング部材の可撓性により、この圧縮され
たリング部材が接続ロッドをクランプし、これによりロ
ッドが専門家によって選ばれた位置にしっかりとロック
される。
ここで、クランプ部材９はリング部材24の上に全周尖
鋭エッジ23が当たるように被さっていることにより、極
めて高いロック性能が保証される。
このロック性能は、リング部材とそれが内挿された通
路の各表面、更には必要に応じて接続ロッドの表面を、
例えばサンドブラスト処理や機械加工によって少なくと
も部分的に粗面化しておくことにより更に改良可能であ
る。
図３〜５に示した部材の一つの変形態様では、図９及
び図10に示すように、両分岐10,11の穴17と18がクラン
プ部材の両分岐の長手方向に延在する長円形となってい
る。また、上部の分岐11の上面11は、凹部32を画定する
直角段部14'を備え、この凹部32に平らなナット（図示
しない）が部分的に収まるようになっている。直角段部
14'は、ナットをレンチで回せるようにナットから充分
に離れた位置に形成されている。
この変形態様によれば、接続ロッドとネジ付き頭部の
各軸間距離を変えることができ、遭遇する種々の形質タ
イプへの適合のみならず、配向を変えることができる点
と相埃ってロッドの曲がり度合いの制限をも果たすこと
ができる。
図示しないが、別の変形態様では、クランプ部材９が
部分的な長さに亙って下部および上部の中間スロットを
備え、それによってクランプ部材の弾性変形能を増加さ
せている。
尚、本発明は以上に述べた各実施例に限定されるもの
ではなく、その全ての変形態様を包含するものである。
特に、本発明による脊椎用装着具はフラスン特許第2657
775号に述べられているようなプレート形の装着具と共
用可能である。

    (57)【特許請求の範囲】
  1. 【請求項１】ナット（５）が螺合する突出ネジ付き頭部
    （３）から上端部で当接部（７）を形成する断面多角形
    （19）の中間ボディ（４）でよって隔てられた下部の錨
    止部（２）を有する骨アンカー部材（１）と、接続ロッ
    ドを受け入れ可能な受入ロック構体（８）とを備え、該
    構体が、接合部（12）に連結されて各々ネジ付き頭部を
    当接部（７）まで通過させ得る整列した穴（17,18）を
    備えた二つの分岐（10,11）を有し、両分岐間に通路（2
    0）を画定したクランプ部材（９）を備えた、特に調整
    可能な接続ロッドのための脊椎用装着具において、 前記通路がキャビティを画定していることと、接続ロッ
    ドを受け入れる中央通路（25）を有し、前記ネジ付き頭
    部に螺合したナットによる両分岐の締め付けで前記接続
    ロッドを所期の角度位置にロックする玉継手を構成する
    圧縮可能なリング部材（24）が前記キャビティ内に配置
    されていることを特徴とする脊椎用装着具。
  2. 【請求項２】キャビティ（20）が、クランプ中にリング
    部材（24）を押圧する少なくとも一つの尖鋭エッジ（2
    3）を有することを特徴とする請求項１に記載の脊椎用
    装着具。
  3. 【請求項３】キャビティ（20）が内奥に中央環状溝（2
    1）を有し、該中央環状溝の両端にはそれよりも小径の
    側端円筒内壁（22）がそれぞれ接して同軸に形成され、
    中央環状溝と両側端円筒内壁との境目の段部に尖鋭なエ
    ッジ（23）が形成されていることを特徴とする請求項１
    または２に記載の脊椎用装着具。
  4. 【請求項４】両側端円筒内壁（23）の外方開口縁に面取
    り部（33）が形成されていることを特徴とする請求項３
    に記載の脊椎用装着具。
  5. 【請求項５】リング部材が円環状で且つ膨出した外面を
    有し、この膨出部の外径がキャビティ（20）の中央環状
    溝（21）の内径と側端円筒内壁（22）の内径の間にある
    ことを特徴とする請求項３または４に記載の脊椎用装着
    具。
  6. 【請求項６】リング部材（20）の外面が球面状であるこ
    とを特徴とする請求項５に記載の脊椎用装着具。
  7. 【請求項７】リング部材が切割スロット（25）を有する
    ことを特徴とする請求項１、５、６のいずれか１項に記
    載の脊椎用装着具。
  8. 【請求項８】キャビティの軸心に対するリング部材（2
    4）の軸心の振れ角が制限されていることを特徴とする
    請求項１に記載の脊椎用装着具。
  9. 【請求項９】リング部材（20）の外面が両端に延在する
    円筒部（29）を備え、該延長部が角運動規制用当接部を
    形成していることを特徴とする請求項８に記載の脊椎用
    装着具。
  10. 【請求項１０】リング部材が、膨出した外面に軸心に関
    して対称的に切割スロット（25）と溝（30）とを有する
    ことを特徴とする請求項５〜９のいずれか１項に記載の
    脊椎用装着具。
  11. 【請求項１１】リング部材に対して下面側に位置する分
    岐（10）が段部（13）を有し、該段部によって中間ボデ
    ィ（４）の側面を支承してアンカー部材とクランプ部材
    との相対回動を阻止可能としたことを特徴とする請求項
    １〜10のいずれか１項に記載の脊椎用装着具。
  12. 【請求項１２】リング部材に対して上面側に位置する分
    岐（11）が穴（18）の周縁に球面状の窪み部（16）を有
    し、該窪み部にナット（５）の球面状の膨出下面（31）
    が収まるようになっていることを特徴とする請求項１〜
    11のいずれか１項に記載の脊椎用装着具。
  13. 【請求項１３】リング部材に対して上面側に位置する分
    岐（11）が、ナットを少なくとも部分的に収容するため
    の凹部（32）を形成する段部（14'）を有することを特
    徴とする請求項１〜11のいずれか１項に記載の脊椎用装
    着具。
  14. 【請求項１４】キャビティおよび／またはリング部材が
    少なくとも局部的に粗面を有することを特徴とする請求
    項１〜13のいずれか１項に記載の脊椎用装着具。
