Description
【発明の詳細な説明】 発明の背景 本発明は一般に患者の血液の化学的性質をモニタする
装置に関し、特に患者からの血液試料をセンサ組立体内
への吸引を制御する装置に関する。

本発明は、特に血液の化学的性質を測定するために周
期的に中断しながら概ね連続して患者への注入流体を注
入するシステムに使用するのに適している。そのような
システムは米国特許第4,573,968号に記載されている。
そのようなシステムにおいては、注入ポンプは注入チュ
ーブとカテーテルとを介して患者の体内中へ適当な注入
流体を注入するが、カテーテルを介して患者から血液試
料を注入チューブに接続されたセンサ組立体内へくみ出
すためにその方向を間欠的に反転させる。適当なセンサ
組立体の物理的形態は米国特許第5,165,406号に記載さ

よびその他の要求を満足させる。

発明の要約 本発明は血液の化学的性質を検出するためのセンサ組
立体内の所定位置までの患者の血液試料の吸引を正確、
かつ繰り返し制御するための改良された装置に関する。
本発明はセンサ組立体の個々のセンサ全てに血液試料が
確実に到達し、かつ隣接する注入流体の希釈作用を最小
にするように十分追加の血液がくみ出されるよう保証す
る。本発明は、化学的分析のために、間欠的に反転さ
れ、患者から血液試料を吸引している間に患者に注入流
体を概ね連続して注入する注入システムの一部として特
に使用される。

したことが検出された後単に約0.1から1.0ミリリットル
の追加の血液をくみ出すだけで許容しうる程度に小さ
く、かつ繰り返し可能レベルまで希釈作用を低減するこ
とが可能である。従って、制御装置23は、ポンプ13を切
り替える前にセンサに血液試料が到来したことを検出し
た後、どの程度の持続時間が必要であろうとも血液をく
み出し続けるようにプログラム化される。

れ、隣接の注入流体の希釈作用を最小にする。そのよう
な希釈作用を完全に排除するためには数ミリリットルが
必要であるが、血液がモニタされつつあるセンサに到来
したことが検出された後単に約0.1から1.0ミリリットル
の追加の血液をくみ出すだけで許容しうる程度に小さ
く、かつ繰り返し可能レベルまで希釈作用を低減するこ
とが可能である。従って、制御装置23は、ポンプ13を切
り替える前にセンサに血液試料が到来したことを検出し
た後、どの程度の持続時間が必要であろうとも血液をく
み出し続けるようにプログラム化される。
更に、制御装置23は、患者の血液試料の到来がポンプ
の逆転の開始後の所定の最大時間経過内に検出されなか
った場合、そしてまた血液試料の到来が所定の最小時間
の経過前に行われたことが検出された場合、アラームを
起動させ、注入ポンプ13のスイッチを切るようにプログ
ラム化されることが好ましい。このことによってセンサ
の故障あるいはその他のシステムの故障の場合に患者か
ら血液をくみ出すよう無限に作動しないように保証し、
また医療関係者にセンサの故障、注入チューブ17あるい
はカテーテル21の詰まり、あるいはその他のシステムの
故障を喚起させる。

図2を再度参照すると、本発明によるシステム内に組
み込み可能な血液の化学的性質を検出するためのセンサ
組立体19の第1の実施例が示されている。センサ組立体
は、二酸化炭素センサ27、酸素センサ29、カリウムセン
サ31、サーミスタ33、カリウムセンサ35、ナトリウムセ
ンサ37、およびpHセンサ39を含む多数の分析センサを含
むものとして示されている。各センサは特定の成分の濃
度、あるいは隣接して位置している別の何らかのパラメ
ータを指示する電気信号を発生させる。二酸化炭素セン
サおよび酸素センサ、並びに基準電極41はセンサ組立体
の第1の室43に隣接して位置しており、一方残りのセン
サは第2の室45に隣接して位置している。
み込み可能な血液の化学的性質を検出するためのセンサ
組立体19の第1の実施例が示されている。センサ組立体
は、二酸化炭素センサ27、酸素センサ29、カリウムセン
サ31、サーミスタ33、カリウムセンサ35、ナトリウムセ
ンサ37、およびpHセンサ39を含む多数の分析センサを含
むものとして示されている。各センサは特定の成分の濃
度、あるいは隣接して位置している別の何らかのパラメ
ータを指示する電気信号を発生させる。二酸化炭素セン
サおよび酸素センサ、並びに基準電極41はセンサ組立体
の第1の室43に隣接して位置しており、一方残りのセン
サは第2の室45に隣接して位置している。
3個のステンレス鋼製のスリーブ47a,47bおよび47cが
第1の室と第2の室43との間で第1の室への入口、かつ
第2の室の出口においてそれぞれ位置している。スリー
ブは隣接する流体と直接接触するように配置され、スリ
ーブ47aおよび47bは相互に短絡している。先ず、3個の
スリーブは、全てスリーブ47bとスリーブ47cとの間の流
体の電導性を測定することにより作動するヘマトクリッ
トセンサの一部を形成している。第2に、スリーブ47a
と47bとは遮断された接地に接続され患者11を電気ショ
ックから保護する。第3に3個のスリーブは全て、さも
なければ各種のセンサ27〜39によって発生する信号に干
渉する可能性のある、カテーテル21および注入チューブ
17に沿って進行する電流を排除しようとするノイズ低減
回路(図示せず)の一部を形成している。そのようなノ
イズ低減回路の一例が米国特許第5,220,920号に記載さ
れている。

逆転可能な注入ポンプ13は患者11から血液試料をくみ
出すにつれて、血液は先ず二酸化炭素センサ27と検出接
触し、その後まもなくカルシウムセンサ35と検出接触す
るようになる。血液試料が二酸化炭素センサに到来する
と、血液は通常著しくより多くの分解した二酸化炭素を
塩水の注入流体よりも多く運んでいるために、発生した
信号は増加し始める。分析器25が二酸化炭素センサから
の信号が基本線レベル以上の5から15ミリボルト高いレ
ベルまで上昇したことを検出すると、タイマが作動す
る。
出すにつれて、血液は先ず二酸化炭素センサ27と検出接
触し、その後まもなくカルシウムセンサ35と検出接触す
るようになる。血液試料が二酸化炭素センサに到来する
と、血液は通常著しくより多くの分解した二酸化炭素を

タイマの作動の後、分析器25はカルシウムセンサ35に
よって発生した信号のモニタを開始する。この信号はタ
イマの作動後7から10秒以内に基準線レベル以上に同様
に上昇したことを示すはずである。これは、血液が通常
使用している注入流体以上に著しく多いイオン化したカ
ルシウムを運んでいるためである。カルシウムセンサの
信号の予期した上昇が実際にこの時間内に発生したとす
れば、血液試料は二酸化炭素センサ27とカルシウムセン
サ35との双方に到達しているものと考えられる。
よって発生した信号のモニタを開始する。この信号はタ
イマの作動後7から10秒以内に基準線レベル以上に同様
に上昇したことを示すはずである。これは、血液が通常
使用している注入流体以上に著しく多いイオン化したカ
ルシウムを運んでいるためである。カルシウムセンサの
信号の予期した上昇が実際にこの時間内に発生したとす
れば、血液試料は二酸化炭素センサ27とカルシウムセン
サ35との双方に到達しているものと考えられる。
分析器がカルシウムセンサ35からの信号のレベルの上
昇を検出すると、制御装置23は患者11から追加の0.1か
ら1.0ミリリットルの血液をくみ出すのに十分な時間逆
方向に注入ポンプ13を作動し続ける。センサ組立体19の
内部構造は、血液試料の先導縁部が組立体のハウジング
内に位置したチューブ49の長さに概ね全体に亘ってくみ
出されるようなものである。この位置において、血液試
料はセンサ組立体の残りの分析センサ27-39全てと検出
接触しているはずである。くみ出された血液の先導縁部
は調整のため、および血液を過剰にくみ出すことによっ
て生じる不当な遅れを避けるための双方の目的から組立
体ハウジング内に留まっていることが好ましい。

患者11から規定の追加の量の血液がくみ出された時点
において、分析センサ27〜39のいずれか、あるいは全て
の読み取りを行なうことが出来る。分析器25はこれらの
センサからの種々の信号を読み取り、それらを患者の血
液の化学的性質の状態の指示に変換する。次に、分析器
はプリント記録、光学ディスプレイ、デジタルデータ通

ことが検出される。これは、例えばラインにおける障害
あるいは1個あるいは双方のセンサの故障による可能性
がある。このようなことが発生すると、制御装置23はポ
ンプの作動を停止させ、アラーム50(図1)を起動さ
せ、医療関係者に注意を喚起する。更に、ポンプを逆転
させた後、二酸化炭素センサおよび(または)カルシウ
ムセンサからの信号において急激な顕著な変化が見られ
る場合、ラインに気泡が介在している可能性があると評
価される。このようなことが発生した場合も、制御装置
はポンプの作動を停止させ、アラームを起動させて医療
関係者に注意を喚起する。

図3を参照すると、本発明によるシステムに組み込む
のに適した血液の化学的性質を検出するセンサ組立体1
9′の第2の実施例が示されている。このセンサ組立体
は単に単一の分析センサ51、すなわちグルコーズセンサ
を含むものとして示されている。このセンサは隣接して
どのような流体が介在していようともグルコースの濃度
を指示する電気信号を発生させる。前記センサはセンサ
組立体の単一の室53に隣接して位置されている。くみ出
された血液試料の到来を検出するために使用するようス
テンレス鋼製のスリーブ55aと55bとが前記室の入口と出
口とにそれぞれ位置している。図2に示すセンサ組立体
19におけるステンレス鋼製スリーブ47a,47b,および47c
の場合と同様、これらのスリーブ55aおよび55bもまた、
ショックに対する保護のため遮断された接地を提供し、
かつノイズ低減回路で使用する電極を形成するために使
用することができる。そのようなノイズ低減回路は米国
特許第5,220,920号に記載されている。

た血液試料の到来を検出する。電導性が所定の限界値を
上回ることが検出されると、そのような到来が推測され
る。次に、約0.4ミリリットルである追加の血液の量が
隣接する注入流体の希釈作用を最小にする。血液試料の
先導縁部はセンサ組立体のハウジング内に位置したチュ
ーブ57の長さの略全体に亘ってくみ出される。一対の離
隔したスプール59aおよび59bの周りにチューブを巻くこ
とにより追加のチューブ長さが提供される。

前述の説明から、本発明は、各々が特定のパラメータ
に感応する多数のセンサを有した特殊なセンサ組立体内
へ患者から間欠的に血液試料をくみ出す、患者の血液の
化学的性質をモニタする改良されたシステムを提供する
ことが認識される。これらの種々のセンサによって発生
した信号が読み取られた後、本システムは血液試料を患
者に再注入させる。センサ組立体内の所望の最適位置に
順次血液試料をくみ出すことは1個以上の分析センサに
よって発生した信号をモニタすることにより達成され
る。このため、注入チューブおよびカテーテルが各種の
長さおよび内容積を持つようにすることが可能で、所望
位置に血液試料が到来したことを検出する個別のセンサ
を設ける必要性を排除している。
に感応する多数のセンサを有した特殊なセンサ組立体内
へ患者から間欠的に血液試料をくみ出す、患者の血液の
化学的性質をモニタする改良されたシステムを提供する
ことが認識される。これらの種々のセンサによって発生
した信号が読み取られた後、本システムは血液試料を患
者に再注入させる。センサ組立体内の所望の最適位置に
順次血液試料をくみ出すことは1個以上の分析センサに
よって発生した信号をモニタすることにより達成され
る。このため、注入チューブおよびカテーテルが各種の
長さおよび内容積を持つようにすることが可能で、所望
位置に血液試料が到来したことを検出する個別のセンサ
を設ける必要性を排除している。
本発明を組み込んだ流体注入および血液検査システム
の好適実施例をその理解と図示のために詳しく説明して
きた。例えば、使用されるセンサの配置、数量およびタ
イプは顕著に変動しうる。本発明の原理から逸脱するこ
となくその他の修正も十分可能である。従って、本発明
の範囲は、請求の範囲が法的に有効である均等物の全て
の範囲と共に、主として請求の範囲によって規定される
べきである。

Claims (10)
- 【請求項1】患者に注入流体を注入する間に患者の血液
の所定のパラメータをモニタする装置において、 患者の血管へ挿入するように形成された注入ラインとカ
テーテルと、 注入流体源と注入ラインとカテーテルとの間に接続され
た逆転可能な注入ポンプと、 注入ラインに存在する流体の第1の所定のパラメータを
示す信号を提供する第1のセンサを含み、前記注入ライ
ンに接続された、血液の化学的性質のセンサ組立体と、 注入ラインとカテーテルとを通して注入流体をポンプで
くみ患者へ注入するため、注入ポンプを順方向に作動さ
せ、そしてカテーテルと注入ラインとを通して患者から
血液試料をくみ出して該血液の化学的性質のセンサ組立
体の前記第1のセンサに接触させるため、順方向の注入
ポンプの作動を間欠的に中断して注入ポンプを逆方向に
作動させる制御装置とを含み、 前記制御装置が更に該血液の化学的性質のセンサ組立体
の前記第1のセンサによって提供される信号をモニタ
し、前記第1のセンサへの血液試料の到着を示す信号の
変化を検出し、 前記制御装置が、前記第1のセンサへの血液試料の到着
を示す前記第1のセンサにより提供される信号の変化の
検出に応答して、逆方向の注入ポンプの作動を停止さ
せ、 前記第1のセンサによって提供された信号が、血液試料
が前記第1のセンサと接触している際に患者の血液の第
1の所定のパラメータを示すことを特徴とする患者の血
液の所定のパラメータをモニタする装置。 - 【請求項2】血液の化学的性質のセンサ組立体の第1の
センサが、二酸化炭素センサ、酸素センサ、カリウムセ
ンサ、カルシウムセンサ、ヘマトクリットセンサおよび
ナトリウムセンサとからなる群から選択されることを特
徴とする請求の範囲第1項に記載の装置。 - 【請求項3】血液の化学的性質のセンサ組立体の第1の
センサが二酸化炭素センサであり、 血液の化学的性質のセンサ組立体が更にカルシウムセン
サを含み、 前記制御装置が二酸化炭素センサによって発生した第1
の信号とカルシウムセンサによって発生された第2の信
号との所定の組み合わせに応答して第1のセンサへの血
液試料の到着を検出するように構成されていることを特
徴とする請求の範囲第1項に記載の装置。 - 【請求項4】血液の化学的性質のセンサ組立体の第1の
センサがヘマトクリットセンサ、温度センサおよびpHセ
ンサとからなる群から選択されることを特徴とする請求
の範囲第1項に記載の装置。 - 【請求項5】第1のセンサからの所定の信号に応答して
作動可能なアラームを更に含むことを特徴とする請求の
範囲第1項に記載の装置。 - 【請求項6】前記制御装置が逆方向の注入ポンプの作動
を開始させた後所定時間内に血液の化学的性質のセンサ
組立体の第1のセンサからの所定の信号が不在であるこ
とに応答して作動可能なアラームを更に含むことを特徴
とする請求の範囲第1項に記載の装置。 - 【請求項7】前記制御装置が血液の化学的性質のセンサ
組立体の第1のセンサへの血液試料の到着を検出した後
所定時間に逆方向の注入ポンプの作動を停止させるよう
に構成されていることを特徴とする請求の範囲第1項に
記載の装置。 - 【請求項8】前記制御装置が、血液の化学的性質のセン
サ組立体の第1のセンサへの血液試料の到着を検出した
ことに続いて患者から所定の追加量の血液がくみ出され
た後、逆方向の注入ポンプの作動を停止させるように構
成されていることを特徴とする請求の範囲第1項に記載
の装置。 - 【請求項9】前記血液の化学的性質のセンサ組立体が第
1のセンサの他に、隣接する流体の異なる所定のパラメ
ータを示す信号を各々が発生する複数のセンサを更に含
み、 前記装置が、血液試料がセンサと接触している間に前記
センサ組立体の複数のセンサによって発生した信号をモ
ニタして患者の血液の複数の異なる所定のパラメータを
測定するモニタを更に含むことを特徴とする請求の範囲
第1項に記載の装置。 - 【請求項10】前記血液の化学的性質のセンサ組立体が
更に第2のセンサを含み、 前記制御装置が、患者の血液試料をくみ出して前記血液
の化学的性質のセンサ組立体の第2のセンサと接触さ
せ、第2のセンサによって提供された信号をモニタし、
第2のセンサへの血液試料の到着を示す信号の変化を検
出するように構成されており、 前記制御装置が更に第1のセンサと第2のセンサとの双
方への血液試料の到着の検出に応答して逆方向における
注入ポンプの作動を停止させるように構成され、 第2のセンサによって提供された信号が、血液試料が第
2のセンサと接触している際に患者の血液の第2の所定
のパラメータを示すことを特徴とする請求の範囲第1項
に記載の装置。
