Description

【発明の詳細な説明】 発明の技術分野 本発明は、皮膚科的疾患の処理のための組成物類、そ
して特に、正常な角質化、表皮形成又は毛包脂腺機能の
変化から発生する皮膚科的疾患、たとえば座瘡、乾癬、
脂漏、内成長毛（ingrown hair）及び偽毛のう炎性ひげ
（pseudofolliculitis）、並びに色素過剰皮膚の処理の
ための局所用組成物に関する。
発明の背景 座瘡、皮膚感染、乾癬、及び皮膚及び偽毛のう炎性単
位又は角質形成の過程の他の疾患は典型的には、肉眼で
見える皮膚科的病変、たとえば座瘡又は皮膚感染の発生
した閉鎖面皮包、しばしば着色した開放面皮包、赤又は
膿疱のように見える赤く隆起した丘疹、丘疹、小節及び
のう胞；偽毛のう炎性ひげの容易に肉眼で見える内成長
毛；又は脂漏、魚鱗癬及び乾癬の肉眼で見える鱗屑の存
在により特徴づけられる。そのような疾患のための可能
な処理の臨床的評価は典型的には、それらの肉眼で見え
る病変の数及び重症度を減じることへの処理の有効性に
基づいている。
皮膚の表面上の肉眼で見える病変の発生の前には、本
明細書において発生前病変として言及される肉眼で見え
ない病変が一般的に、皮膚内に存在する。出現前病変
は、従来の臨床的研究における分類のために肉眼で見る
には不十分であるが、皮膚内でのそれらの存在が、感触
及び又は皮膚内の痛み及び緊張により認識され得る。
出現前病変は、荒皮及び真皮内の、又は皮膚の表皮、
真皮、又は両者に位置する、毛包脂腺小胞内の及びその
周囲の出現前工程により引き起こされる。座瘡において
は、この出現前過程は通常、毛包脂腺小胞内で始まる。
毛包脂腺小胞が、皮脂腺、細菌（一次性コリネバクテリ
ウム・アクネス（Corynebacterium acnes）又はP.アク
ネス）、及び小胞の内壁から脱落するケラチン細胞によ
り小胞中に分泌される皮脂により満たされる。出現前過
程においては、小胞壁が炎症性物質、特に脂肪分解性酵
素、又はリパーゼにより皮脂に存在するトリグリセリド
の分解により生成される過剰の遊離酸、及びP.アクネス
から生成され、そして誘発される走化性物質及び炎症性
物質により攻撃される。正常な患者に比較される場合、
アクネ患者は高レベルの皮脂分泌、及びP.アクネスの高
められた存在及びその関連するリパーゼ活性を有し、そ
して遊離脂肪酸及び他の関連する炎症性物質のレベルの
上昇をもたらすことが研究において示されている。それ
らの上昇は、疾患の重症度に対しておおよそ比例するこ
とが示されている。
さらに、座瘡患者はしばしば、異常な小胞壁を有す
る。正常な皮膚においては、小胞壁は角質化の過程によ
り形成される、角質化された細胞から完全に構成され
る。この角質化された細胞壁は、脱落したケラチン細胞
と小胞及び小胞を取り囲む水性組織内の皮脂及び細菌成
分との間にバリヤーを形成する。座瘡患者においては、
誤った角質化が構造的に弱い皮脂含有細胞の小胞壁中へ
の挿入を可能にし、それにより壁を攻撃及び破壊に対し
てより傷つきやすくする。細胞壁が侵害された後すぐ
に、脂肪酸、皮脂及び炎症性成分、生存及び死亡細菌、
小胞壁からの脱落された細胞、及び他の小胞内容物が、
炎症過程を確立する、小胞を取り囲む水性組織中に開放
される（Webster,G.,Jnl.Am.Acad.Dermatol.,1955;33:2
47−253）。
上記出現前過程は、表面上では肉眼で見えないが、炎
症及び得られる内部損傷が感覚又は苦痛として皮膚内に
感じられ、そしてまた、隆起物として皮膚の表面に感じ
られ得る点に進行することができる。
いくつかの研究が、皮膚の表面上に肉眼で見える皮膚
科的問題、たとえば赤く隆起した鱗屑、赤く隆起した丘
疹及び膿疱、皮膚感染、内成長毛及び角化鱗屑に対する
剤、たとえば過酸化ベンゾイル、αヒドロキシ酸及び亜
鉛剤の側の効果を開示している（Handbook of Non−Pre

に対する認識される座瘡薬の効能に気づいていない。
発明の要約 本発明は、皮膚科的吸収性の局所用抗微生物剤、抗生
物質、抗菌剤又は抗カビ剤；皮膚科的吸収性のα又はβ
ヒドロキシ酸；及び皮膚科的吸収性の亜鉛化合物を皮膚
科的に許容できるキャリヤーに含んで成る組成物の局所
投与により、正常な角質化、表皮形成又毛包脂腺機能の
変化から発生する皮膚科的疾患、たとえば座瘡、乾癬、
脂漏、内成長毛及び偽毛のう炎性ひげ、並びに色素過剰
皮膚を処理するための方法を提供する。本発明の組成物
は、好ましくは、透明ゲル、不透明ゲル、ローション、
懸濁液、軟骨、クリーム、粉末及び同様のもの形で、毎
日、患者の皮膚の患部に適用される。
皮膚科的吸収性の局所用抗微生物剤、抗生物質、抗菌
剤又は抗カビ剤は好ましくは、アゾレアン酸（azoleaic
acid）、過酸化ベンゾイル、エリスロマイシン、バシ
トラシン、亜鉛バシトラシン、ポリミキシン、ネオマイ
シン、クロラムフェニコール、テトラサイクリン、ミノ
サイクリン、クリンダマイシン、ドキシサイクリン、ウ
ンデシレン酸及びその塩、プロピオン酸及びその塩、カ
プリル酸及びその塩、シプロフロキサシン、セファロス
ポリン、安息香酸、シクロピロックス・オラミン、クロ
トリマゾール、エコナゾール硝酸塩、メトロニダゾー
ル、ミコナゾール硝酸塩、ケタコナゾール、オキシコナ
ゾール、トルナフテート及びそれらの組合せから成る群
から選択され；そして最とも好ましくは、過酸化ベンゾ
イルである。抗微生物剤、抗生物質、抗菌剤又は抗カビ
剤は約0.1％〜約30％の量で存在することができるが、
また0.25％〜20％、好ましくは約0.5％〜約10％の量で
も使用され得る。本明細書に言及されるすべての％は、
重量によるものである。
α又はβヒドロキシ酸は、次の化合物の遊離酸、塩、
アミド、両性及びポリマー形から成る群から選択され
る：クエン酸、グリコール酸、グルクロン酸、ガラクツ
ロン酸、グルクロンラクトン、グルコノラクトン、α−
ヒドロキシ酪酸、α−ヒドロキシイソ酪酸、乳酸、リン

チル、ピルビン酸エチル、β−フェニル乳酸、β−フェ
ニルピルビン酸、キナ酸、サッカリン酸、酒石酸、タル
トロン酸及びβ−ヒドロキシ酪酸。好ましい態様におい
ては、α又はβヒドロキシ酸は、グリコール酸、乳酸、
マンデル酸、クエン酸、酒石酸、リンゴ酸、サリチル酸
及びアセチルサリチル酸から成る群から選択され、そし
て最とも好ましくは、グリコール酸又は乳酸である。α
又はβヒドロキシ酸は0.10％〜約70％の量で存在し、そ
して皮膚上へ短時間の適用のための好ましい濃度は約５
％〜約70％であり、そして適用の後、皮膚上に残存する
毎日の使用のための好ましい濃度は約0.15％〜約15％で
ある。
亜鉛化合物は、水溶性、低い水溶性及び水不溶性亜鉛
塩化合物及び複合体、たとえば酢酸亜鉛、亜鉛バシトラ
シン、臭化亜鉛、システイン酸亜鉛、カプリル酸亜鉛、
塩化亜鉛、クエン酸亜鉛、弗化亜鉛、蟻酸亜鉛、グリシ
ン酸亜鉛、ヨウ化亜鉛、乳酸亜鉛、硝酸亜鉛、亜硝酸亜
鉛、オレイン酸亜鉛、シュウ酸亜鉛、酸化亜鉛、過マン
ガン酸亜鉛、過酸化亜鉛、フェノールスルホン酸亜鉛、
リン酸亜鉛、プロピオン酸亜鉛、ピロリン酸亜鉛、リシ
ノール酸亜鉛、サリチル酸亜鉛、セレン酸亜鉛、珪酸亜
鉛、亜鉛セレン化物、硫酸亜鉛、ステアリン酸亜鉛、硫
化亜鉛、タンニン酸亜鉛、酒石酸亜鉛、吉草酸亜鉛、亜
鉛ペプチド及び亜鉛タンパク質複合体から成る群から選
択される。本発明の好ましい態様において、亜鉛化合物
は、乳酸亜鉛又は酢酸亜鉛であり、そして約0.001％〜
約30％、最とも好ましくは約0.1％〜約10％の量で存在
する。
他の治療剤は、本発明の組成物に有用に添加され得
る。そのような剤は、レチノイド、たとえばトランスレ
チン酸、13−シスレチン酸及びそれらの誘導体；抗−ア
ンドロゲン、たとえばスピロノラクトン；メトロニダゾ
ール；及び抗−炎症剤、たとえばヒドロコルチゾン、β
−メタゾン、クロペタゾール、フルオシノニド、トリア
ムシノロン、デソニド及びハルシノニドを包含する。そ
れらの治療剤は約0.001％〜約10.0％、好ましくは約0.0
1％〜5.0％の量で存在することができる。
本発明の活性成分は、非面皮包形成剤及び低アレルゲ
ン性剤、たとえば水、C12-15アルキルベンゾエート、グ
リセリン、セチルステアリルアルコール、ポリアクリル
アミド、C13-14イソパラフィン、ラウレス−７、PEG−1
000ステアレート、ステアレスＳ−２、ステアレスＳ−2
0、水酸化ナトリウム、ジメチゾン、及びジナトリウムE
DTAから構成される皮膚科的に許容できるキャリヤーに
組合される。特定のワセリン画分、すなわち高い割合の
長鎖分子を有する、より粘性の形のワセリンU.S.Pが特
に有用である。本発明において有用に用いられ得る他の
キャリヤーは、当業界において良く知られている。
発明の詳細な記載 例１ 本発明の組成物の２種の配合物、すなわち表Ｉに示さ
れる試薬から構成される配合物Ｉ及びIIを、次の手法を
用いて調製した。
ジナトリウムEDTA及び乳酸亜鉛を、60℃の温水に溶解
し（30％のバッチ重量）、そしてグリセリン添加する。
次に、ポリアクリルアミド−C13-14イソパラフィン−La
ureth−７を添加し、そして均等になるまで、混合し、
パートＡを供給する。別々に、等部の水にグリコール酸
を溶解し、そしてゆっくり、水酸化ナトリウムを添加す
る。パートＡを添加し、そして60℃で維持する。別々
に、ステアレスＳ−２及びＳ−20、グリセリルモノステ
アレートSE及びセチルステアリルアルコールを、流体及
び均等になるまで加熱する。これをパートＡに添加し、
滑らかになるまで混合する。ジメチコーンを添加し;40
℃への冷却を開始する。別々に、C12-15アルキルベンゾ
エートに過酸化ベンゾイルを分散し、そして冷却された
パートＡに添加する。最後に、水により100％にし、そ
して必要なら、均質化する。
本発明の組成物の第三配合物、特に処理のために皮膚
上にクレンガーとして使用され、又は皮膚上に残される
ゲル（この後、配合物IIIと称せられる）（表IIに示さ
れる試薬から構成される）を次の通りに調製した。
ワセリン、コシルイセチオン酸ナトリウム、メタリン
酸カリウム、二酸化チタン及び乳酸亜鉛を混合し、そし
て混合物Ｉを形成するために滑らかになるまで均質化し
た。カルボマーを一部のグリセリンに分散し、75℃に加
熱し、そして混合物Ｉに添加し、必要なら、クリーム状
になるまで均質化し、混合物IIを形成した。グリコール
酸を残るグリセリンに50℃で溶解した。これに、水５部
に前もって溶解された水酸化ナトリウムを添加した。そ
の得られる混合物を混合物IIに添加し、そして次に、45
℃に冷却し、混合物IIIを供給した。過酸化ベンゾイル
をC12-15アルキルベンゾエートに分散し、そして混合物
IIIに添加した。得られる生成物を必要に応じ微粉砕し
た。

２人の患者に対して試験し；前記２人の患者の病変はほ
とんど、顔及び首上に存在した。
両患者は、赤く隆起した丘疹、非炎症性内成長毛、炎
症性内成長毛、及び出現前病変を連続的に有していた。
本発明の配合物IIIによる処理の前、個々の患者は広範
囲の処理、たとえば局部用過酸化ベンゾイル、局部用エ
リトロマイシン、局部用エリトロマイシン＋亜鉛、局部
用クリンダマイシン、αヒドロキシ酸ローション、及び
局部用エリトロマイシン＋局部用過酸化ベンゾイル処理
を使用していた。それらのそれぞれの薬物は、少なくと
も２か月間、毎日、個々に使用された。個々の薬物は初
期に、皮膚科的病変の数及び重症度の低下を引き起こし
たが、しかしこの効果は、結果的には、低くなった。ど
の薬物も、病変、特に出現前病変の実質的且つ一貫した
低下においては、好結果を生むものではなかった。
本発明の配合物IIIによる処理の前、患者は表IIIに示

回、局部的に適用された。１か月及び２か月間の処理の
後に存在する皮膚科的病変の範囲が、それぞれ表IV及び

たとえば丘疹、及び眼に見えない出現前病変の両者の数
及び重症度を減じることにおいて効果的であることを示
す。
本発明は特定の態様で記載されて来たが、変更及び修
飾が、本発明の範囲内で実施され得る。

    (57)【特許請求の範囲】
  1. 【請求項１】座瘡の処理のための組成物であって、 （ａ）過酸化ベンゾイル； （ｂ）グリコール酸； （ｃ）乳酸亜鉛；並びに （ｄ）皮膚科的に許容できるキャリヤー を含んで成る組成物。
