Abstract
Description
-段階1は、面皰を伴うざ瘡に相当し、多数の開いたおよび/または閉じた面皰と小嚢腫によって特徴づけられる。
-段階2は、軽度から中度の丘疹膿疱性ざ瘡である。これは、開いたおよび/または閉じた面皰と小嚢腫だけでなく、紅色丘疹および膿疱が存在することにより特徴づけられる。主に顔面に影響を与え、ほとんど瘢痕を残さない。
-段階3は、より深刻な丘疹面皰性ざ瘡(papulocomedonienne acne)であり、背中、胸、および肩に広がる。より多くの瘢痕を伴う。
-初期血管/滲出フェーズ:血管の能動性うっ血、浮腫、炎症部位への白血球の遊走が含まれる;
-炎症性肉芽の形成フェーズ:形成された肉芽が再生芽に変換される。このフェーズは新鮮な肉芽形成としても知られる;
-炎症の清浄(即ち、壊死組織、微生物、存在し得る異物および浮腫液の除去)フェーズ、ならびに上皮形成(即ち、上皮細胞の増殖および治癒の終焉)フェーズ;
-治癒の適正フェーズ:新鮮に形成された肉芽から瘢痕性線維組織(または瘢痕)への変化を可能にする。
-潰瘍:この現象は、傷が中空となる治癒の異常であって、肉芽組織が再形成されない。肥厚性または萎縮性瘢痕は、特に外傷から生じるものであるが、尋常性ざ瘡もしくは水痘などの皮膚病理からも生じるものであり、中空領域またはアイスピック(ice-pick)状の瘢痕である。これらの形態も治癒の異常に基づくものである。
-肥厚性瘢痕、およびケロイド(または「ケロイド状瘢痕(keloidal scars)」):この現象は、肉芽組織が異常な形態で過剰増殖する過程である(Treatment of scars: a review、Alster et al.、Ann.Plast.Surg.、1997、Oct、39(4)、418-32頁)。
-肥厚性瘢痕は、長期間かけて自発的に改善する(平均して2または3年)。この瘢痕は、当初の部位に限定して残る。
-ケロイド状瘢痕の方は、自発的に改善する傾向を何ら示さず、永続的に留まり、実際には、時間と共にさらに悪化する。さらに、このタイプの瘢痕は、当初の瘢痕部位を越えて広がり、隣接する健常な組織に影響を与える。
-人種:黒色人種またはアジア系人種の人間は、白色系人種の人間よりも、ケロイドを生じ易い。
-年齢:子供で高頻度である。肥厚性瘢痕は高齢者では稀である。
-身体の場所:身体の一部の場所は、病理的瘢痕を発生し易い、例えば、胸骨、首、耳たぶまたは顔面下部などが挙げられる。
-「圧力療法(pressure therapy)」:特注の圧迫弾性衣料で、または圧迫を伴うシリコーン衣料を用いて実施される。およそ6ヶ月の間、継続的に(昼夜)適用される場合に非常に効果的であるが、常に達成されるものではない。
-「皮質療法(corticotherapy)」:長期的に有効なコルチゾン製品を瘢痕の内側に注入する。これらの瘢痕は通常非常に硬いために、製品を圧力下で瘢痕内に注入する最適な方法は、無針デバイス(「ダーモジェット(Dermo Jet)」)を使用することである。
(実施例1)
水溶性ゲルEpiduo(登録商標)の組成物
ゲルには、全重量に対する重量%として、以下が配合されている:
- 2.5%のBPO、
- 0.1%のアダパレン、
- 0.10%のEDTA二ナトリウム;
- 4.00%のグリセロール、
- 4.00%のプロピレングリコール、
およびまた、好ましくは、
- 0.05%のドキュセートナトリウム、
- 0.20%のポロキサマー124、
- 4.00%のナトリウムアクリロイルジメチルタウレート共重合体、イソヘキサデカン、およびポリソルベート80;
- NaOH、pHが5になるために十分な量。
アダパレン-過酸化ベンゾイルの組合せは、ビヒクルと比較して、ヒスパニック系人種におけるざ瘡の処置に迅速な効果を示す
この実施例の目的は、Epiduoが、炎症後色素沈着(PIH)とケロイド状瘢痕が高い割合で発生する皮膚の色が濃い(非白色系とも称する)患者の小グループにおいて、非常に効果的にざ瘡を処置することを実証することである。特に、ヒスパニック系患者は、丘疹病変、ざ瘡色素沈着過剰斑(AHM)および嚢胞性病変の頻度がより高い(それぞれ、74.5%、52.7%および25.5%)ことが分かっている。
炎症後色素沈着の評価
炎症後色素沈着を、この評価に適格な43人の対象(24人はアダパレン-BPOグループ、19人はビヒクルグループ)に対して評価した。ITT集団に対する各訪問時の炎症後色素沈着の解析を、以下の表Iおよび図1に示す。
異なる皮膚タイプ分類および人種もしくは民族間での刺激の発生および重症度の相違の可能性の調査
アダパレン-BPOゲルの皮膚刺激のメタ解析は、異なる皮膚タイプ分類および人種もしくは民族間での刺激の発生および重症度の相違可能性を調査するために行った。合計で3,855人の患者を含む3つの無作為抽出、二重盲検、ビヒクルおよびプラセボ対照臨床試験により、全ての皮膚タイプのざ瘡の処置におけるアダパレン-BPOゲルの安全性と有効性が確立された。この事後メタ解析は、3つの無作為抽出試験それぞれのアダパレン0.1%-BPO2.5%処置部門に割り当てられた患者からの忍容性データに基づいている。
3つの試験は全て、同様の目的と設計を有していた。試験は、多施設、無作為抽出、二重盲検、並行群間、活性-およびビヒクル-対照試験とした。試験1は、米国、プエルトリコおよびカナダにおける60施設で行った。試験2は、米国、カナダおよびヨーロッパにおける61施設で行い、試験3は米国における36施設で行った。アダパレン0.1%-BPO2.5%の組合せゲルの有効性と安全性を、アダパレン単独療法、BPO単独療法およびゲルビヒクルと比較した。参加者を、アダパレン-BPO組合せゲル、アダパレンゲル単独療法、BPOゲル単独療法またはゲルビヒクルに、無作為抽出した。有効性と安全性の評価は、ベースラインと1、2、4、8および12週目で行い、紅斑、剥れ、乾燥、および刺激感/灼熱感に関する0(無し)〜3(重度)の尺度での調査者による評価を含んでいた。
このメタ解析では、3つの試験それぞれにおいて、アダパレン-BPO処置グループに無作為抽出された対象を含めた。3つの試験の全てで最も深刻な刺激が現れたのは1週目と判断されたため、1週目の評価のみをこのメタ解析に含めた。各試験において、またメタ解析の組合せ試験において、以下の小グループの間で比較を行った:1)フィッツパトリック皮膚タイプI〜III型を有する対象とフィッツパトリック皮膚タイプIV〜VIを有する対象、2)白人系対象と非白人系対象、3)白人系対象と黒人系対象とヒスパニック対象(3元解析)。
合計で983人の患者は、3つの試験で、少なくとも1用量のアダパレン-BPOを受けた。メタ解析集団に含まれるのは909人の患者であった。その理由は、1週目に施設に来なかった74人の患者(7.5%)(例えば、連絡がとれなくなったもの、または個人的理由、処置の不具合、有害事象の発生のために試験から退いたもの)を解析に含めなかったためである。患者の大部分は白人(73%)、11%は黒人、11%はヒスパニック人であり、患者の73%がフィッツパトリック皮膚タイプのI〜IIIであり、37%がフィッツパトリック皮膚タイプのIV〜VIであった。
4つの皮膚刺激スコアおよびフィッツパトリック皮膚タイプのそれぞれの間の関係を、個々の試験および組合せ試験解析のそれぞれについて示す。4つのカテゴリーのそれぞれにおいて、45%以上の患者が刺激の徴候/症候を経験しなかった(図2)。刺激を経験した患者においては、その報告のほとんどが軽度であり、刺激は初期に生じ、処置の間に解決した(図示せず)。
4つの皮膚刺激スコアのそれぞれと人種もしくは民族との間の関係を、組合せ試験解析で示した。人種または民族グループの間での有意な差異が、4つのパラメータ全ての分布で見られた。
3つの無作為抽出臨床試験のメタ解析は、異なるフィッツパトリック皮膚タイプまたは人種の対象間で、アダパレン-BPOゲル処置の忍容性が異なっているかを判断するために行った。有色人種系対象が、白人系対象よりも、多くの局所的ざ瘡処置に対して感受性が高いと信じられていることに反して、このメタ解析の結果は、アダパレン-BPO処置における乾燥、剥れおよび刺激感/灼熱感に関して、フィッツパトリック皮膚タイプI〜IIIの対象と皮膚タイプIV〜VIの対象とを比較するときの統計学的有意差を示さなかった(P=NS)。紅斑の評価だけが、薄い皮膚タイプのフィッツパトリック皮膚タイプは濃い皮膚タイプより悪くなることについて統計学的な差異を示した(P<.001)。この紅斑についての知見は、濃い色の皮膚では軽度から中度の紅斑は見えにくいという事実によって一部説明し得るけれども、この結果は、アダパレン-BPO処方物と、高いフィッツパトリック皮膚タイプで高い発生割合の刺激の徴候または症候があることとは関係がないことを示唆している。
Claims (7)
- 非白人系人種におけるざ瘡の処置を目的とする医薬の調製のための、アダパレンと過酸化ベンゾイルとの組合せの使用。
- 処置が、炎症後色素沈着、ケロイド瘢痕、ざ瘡色素沈着過剰斑および嚢胞性病変から選択される病理または障害の少なくとも1種の徴候または症候を減少または防止させるものである、請求項1に記載のアダパレンと過酸化ベンゾイルとの組合せの使用。
- 医薬が、局所適用に適した形態の組成物であることを特徴とする、請求項1または2に記載の使用。
- 過酸化ベンゾイルが、医薬の全重量に対して、1〜10重量%、有利には2〜5重量%、より有利にはおよそ2.5重量%の濃度で存在することを特徴とする、請求項1または2に記載の使用。
- アダパレンが、医薬の全重量に対して、0.001〜20重量%、有利には0.005〜1重量%、より有利にはおよそ0.1〜0.3重量%の濃度で存在することを特徴とする、請求項1から3のいずれか一項に記載の使用。
- 医薬が、ゲル、エマルジョン、ローションの形態であることを特徴とする、請求項1から5のいずれか一項に記載の使用。
- 医薬が、全重量に対する重量百分率で以下の組成を有する水性ゲルを含む組成物の形態であることを特徴とする、請求項1から6のいずれか一項に記載の使用:
