Abstract

Description
本開示は、レニン-アンジオテンシン系(RAS)モジュレーター、RASモジュレーターを含む医薬組成物、およびウイルス感染症の治療方法に関するものである。
ワクチンは特定のウイルス疾患を予防することができ、抗ウイルス剤はウイルスの繁殖を阻害したり、特定のウイルス感染に対する免疫反応を強化することができる。しかし、多くのウイルス感染症に対して有効な抗ウイルス剤はまだない。そのため、ほとんどのウイルス感染症では、免疫系がウイルスを撃退するのを待ちながら、症状を改善する治療法しかないのが現状である。重症急性呼吸器症候群コロナウイルス2型(SARS-CoV-2)の出現とそれに続くCOVID-19の大流行は、必要なワクチンと抗ウイルス薬がない場合にウイルスが引き起こす大惨事を浮き彫りにした。
少なくとも1つの実施形態は、ウイルス感染症を治療する方法に関する。例示的な実施形態では、方法は、レニン-アンジオテンシン系(RAS)モジュレーターを含む医薬組成物を、それを必要とする対象に投与して、ウイルス感染の細胞及び器官への影響を緩和することを含み得る。緩和は、活性酸素種の抑制、サイトカイン放出の抑制、アンジオテンシン変換酵素2(ACE2)のアップレギュレーション、及び/又はアンジオテンシンII受容体1型(AT1)のダウンレギュレーションを含んでもよい。

例えば、本明細書で開示されるのは、ウイルス感染の治療のために、抗酸化物質(例えば、Tempol(テンポール))に繋留されたアンジオテンシン受容体ブロッカーを使用する方法である。一例として、YK-4-250は、ウイルス感染症の合併症を予防または抑制するために有効な薬剤として使用され得る。別の例として、YK-4-250とHMG-CoA還元酵素阻害剤との組み合わせは、ウイルス感染症の合併症を予防または抑制するために有効な薬剤として使用され得る。また、アンジオテンシン受容体ブロッカーとHMG-CoA阻害剤をウイルス感染症の治療に用いる方法も開示されている。一例として、テルミサルタン及びロスバスタチンを、ウイルス感染症の合併症の予防又は抑制に有効な薬剤として使用することができる。また、ウイルス感染症の治療のために、抗酸化物質(例えば、テンポール)に繋留されたDIMを使用する方法も開示される。DIM(およびその前駆体であるI3C)は、VEGFおよび他の成長因子によって誘導されるRASシグナル伝達を阻害し、血管新生に必要なその下流の生物学的効果を妨害することが実証されている(*1)。さらに、ウイルス感染の治療のために、抗酸化物質(例えば、テンポール)に繋留されたピルフェニドン(PFD)を使用する方法が開示されている。PFDは、TGF-βプロモーター活性及びTGF-βタンパク質分泌の遮断、TGF-β誘導性Smad2-リン酸化、ECM刺激及びROS生成の阻害、並びにRNAプロセシングの調節を通じて、抗線維化作用を発揮する。TGF-β1は、RASおよびマイトジェン活性化タンパク質(MAP)キナーゼ、ホスホイノシチド3キナーゼ(PI3K)/Akt経路、Rho GTPasesを活性化し、細胞の成長、生存、移動、細胞骨格構成を制御している(*2)。

図1は、レニン-アンジオテンシン系(RAS)の活性化とCOVID-19感染の概略を示す図である。アンジオテンシン変換酵素2(ACE2)は、COVID-19感染の受容体であり、肺、胃腸(GI)管、および心臓血管系に発現している。ACE2はレニン-アンジオテンシン系(RAS)の重要な酵素であり、同系の負の調節因子であるアンジオテンシンII(Ang II)を不活性化する。ACE2は、COVID-19感染症、酸吸引、敗血症によって引き起こされる重度の急性肺障害からマウスを保護する。Ang IIがAT1受容体に結合すると、活性酸素種(ROS)と炎症が促進され、肺組織の損傷と機能低下が引き起こされる(図1)。ACE2は、重症急性呼吸器症候群コロナウイルス2型(SARS-CoV-2)感染による急性肺障害からマウスを保護することができる(文献1)。急性肺損傷および他の臓器への生命を脅かす可能性のある損傷におけるACE2受容体の重要な保護機能は、COVID-19感染に対する治療の可能性を指し示している(*3)。RASの活性化が進むと、活性酸素が急増し、炎症による組織障害がエスカレートする。COVID-19感染後のウイルスによるRASおよび活性酸素の活性化を抑制する治療薬があれば、罹患率と死亡率に大きな影響を与えることが期待される。

ウイルスの有害作用は、COVID-19感染時の宿主の抗炎症、抗酸化、抗線維化反応を高めることによって媒介される可能性がある。例えば、アンジオテンシンII受容体ブロッカー(ARB)テルミサルタンを強力な抗酸化物質4-ヒドロキシTEMPO(Tempolと呼ばれる)につなぎ合わせた長時間作用型のYK-4-250は、COVID-19感染患者の副作用を軽減し生存率を向上させる。YK-4-250は、肺、消化管、内皮のAT1受容体に特異的に結合し、ウイルスが媒介するRASおよび活性酸素の有害な作用を逆転させる可能性がある。さらに、活性酸素の減少は、IL-6の放出を和らげ、ウイルスが誘発するサイトカインの嵐を鎮めるであろう。テルミサルタンとテンポールはともに、ヒトへの使用がFDAによって承認されている。しかし、テルミサルタン(半減期24時間)とTempol(半減期5分)は半減期が大きく異なるため、別々の薬剤として同時投与することはできない。そこで、テルミサルタンとテンポールを1剤で投与できるYK-4-250(半減期5.4時間)は、この新規適応の候補として望ましいと考えられる。
ACE2のアップレギュレーションは、COVID-19感染と戦うために重要である。ARBはACE2をアップレギュレートし、ARBがウイルス感染に対して患者を素因とするかもしれないという最近の指摘は、科学的な裏付けがない(*4)。さらに、複数の研究により、ACE2の増加は、SARS-CoV-2感染による肺損傷を細胞保護し、緩和することが証明されている(*1,5)。COVID-19ウイルスが細胞内に侵入すると、RASの制御に重要なACE2タンパク質の発現が直ちにダウンレギュレートされる。逆説的だが、肺におけるACE2発現を増加させると、RASを減衰させることにより、SARS-CoV-2スパイクタンパク質による肺損傷からマウスを保護することがわかった。ACE2はまた、腸の炎症を抑制し、抗酸化物質である(*6)。正常な患者とIBD患者の大腸細胞のシングルセルRNAシーケンスデータから、ACE2の発現は、ウイルス感染、自然免疫および適応免疫を制御する遺伝子と正の相関を示し、ウイルス転写、タンパク質翻訳、液性免疫とは負の相関を示すことが示された。これらのデータは、ACE2発現の増加がウイルス感染や免疫反応に複雑な役割を果たすことを強く示唆している。ロスバスタチンのようなHMG-CoA還元酵素阻害剤の添加は、ACE2の制御を高め、提案されているAT1阻害の治療と相乗的に作用することになるであろう。

COVID-19は、肺、消化管、循環器系を傷害する。急性呼吸窮迫症候群(ARDS)は、急性肺障害の中でも最も重症で、高い死亡率(30~60%)を示す破壊的な臨床症候群である(*1)。ARDSの素因は多様で、敗血症、誤嚥、肺炎、SARS-CoV-2による感染症などがある。現在のところ、ARDSの臨床転帰を改善する有効な薬剤はない。
局所組織ベースのAng IIは肺高血圧、急性肺損傷、肺線維症を悪化させるが(*7)、実験モデルでは、ACE2が肺胞上皮タイプII細胞へのウイルス侵入を仲介し、その欠損はRASを活性化することによって肺損傷を悪化させることが示された(*3)。Dclk1+タフト細胞上に存在するAT1受容体(未発表データ)をブロックすると、SARS-コロナウイルスのスパイクタンパク質を介した肺傷害が軽減され(*6)、肺高血圧を抑制した(*7)。スパイクタンパク質の関与は、ACE2の発現をダウンレギュレートし、RASを活性化する(*6)。重症のSARS-CoV-2肺炎では高血圧がよく見られることを考えると、重症肺炎患者の肺ではRASが活性化されている可能性が高い(*4)。SARS-CoV-2感染に伴う細胞内ACE2の発現低下により、RAS内でのACE2の機能が損なわれている。ACE2は急性肺傷害の顕著な抑制因子であるため、ACE2の発現低下はSARS-CoV-2感染時に観察される重篤な症状を引き起こすと考えられている。Ang IIがAT1受容体に結合すると、炎症、酸化ストレス、線維化、血管新生、血管収縮を促進し、組織損傷を引き起こすことが証明されている(*8)。ARBファミリーは、Ang IIのAT1受容体への結合を特異的に阻害するため、COVID-19感染による一連の副作用を逆転させる可能性がある。

中国湖北省でCOVID-19と診断された204人の患者を対象とした小規模な研究で、研究者は、これらの患者の約49%がGI症状を呈し、これらの患者はGI症状のない患者と比較して、有害な結果をもたらし生存期間が短くなることを指摘した(*2)。ACE2は、SARS-CoV-2が標的細胞にウイルスが侵入する際の受容体である。ACE2は、ウイルスのスパイクタンパク質(図1)と相互作用し、肺の肺胞上皮タイプII細胞や大腸上皮細胞のSARS-CoV-2感染を仲介する(*3)。また、肺の肺胞上皮II型細胞や消化管の大腸上皮細胞への感染を媒介する(*3)。
COVID-19感染者187例を対象とした検討では,27.8%に心筋障害が認められ,心機能障害や不整脈が発生した。心筋傷害はCOVID-19感染症の致死的転帰と関連している.炎症は心筋傷害の潜在的なメカニズムである可能性がある(*9)。心筋傷害のリスクの高い患者には、炎症と活性酸素を抑える積極的な治療を考慮する必要がある。YK-4-250のような活性酸素を抑制する薬剤は、このようなハイリスク患者にとって望ましい候補であろう。

本明細書では、げっ歯類におけるYK-4-250の最大耐容量(MTD)に関する研究を開示する。YK-4-250の単回投与急性毒性は、急性経口毒性(AOT)アップ・アンド・ダウン手順に従い、雄および雌のSDラットで測定された(文献23)。SDラットはNational Cancer Institute(NCI)から購入した。YK-4-250原液は水で調製し、濃度は200 mg/mLとした。YK-4-250(100、200、300、400、600 mg/kg)を経口投与し、最初の4時間は観察を中断せず、急性毒性を14日間毎日観察した。動物を犠牲にし、すべての病理学的所見を記録した。SDラットにおけるYK-4-250の単回投与MTDは550 mg/kg以上であった。
YK-4-250は強力な抗酸化物質であり、組織における活性酸素を抑制する。糸球体前部血管平滑筋細胞を10-6 M Ang IIで刺激し、O2
・―を発生させた。YK-4-250はO2
-の発生を抑制し、強力な抗酸化作用を示す。一方、テルミサルタンのようなAng II遮断薬には抗酸化作用がない(図2)。
ARBはSARS-CoV感染による重症急性肺不全を生体内で軽減する。Ang II ATは、Ang IIによる血管透過性と重症急性肺障害を媒介する重要な受容体である(*1,3)。AT1を阻害すると、Spike-Fc投与マウスの急性重症肺障害が実に減弱された。AT1Rの阻害もまた肺水腫を抑制した。以上のことから、SARS-CoV Spikeは、レニン-アンジオテンシン系の調節を低下させることにより、急性肺不全を拡大させることができることが示唆された。さらに、SARS-CoV Spikeが介在する肺不全は、Ang II AT1受容体の阻害によって救済されることがわかった。Ang II AT1受容体は、急性肺障害の重症度と肺血管の透過性を制御している。
本明細書で開示されるのは、YK-4-250の製造可能性である。候補治療薬であるYK-4-250に関して、優れた製造可能性が達成され得る。例示的な実施形態において、ワンポット合成でテンポールをテルミサルタンに添加し、0.81グラムのYK-4-250(図3)をピンク色の軟質固体として78%の収率で得た。化合物の分子量を確認し、飛行時間型高分解能質量分析計により純度を決定した。この反応は、キログラム単位のGMP生産に拡張可能である。
RASモジュレーター、医薬組成物、およびウイルス感染症の治療方法の例としては、少なくとも以下のものが挙げられる。
RASモジュレーターは、ARB-抗酸化物質結合体を含んでもよいが、例示的な実施形態はこれに限定されず、本明細書に開示するような他の種類の結合体を含んでもよい。

前記レニン-アンジオテンシン系モジュレーターが、アンジオテンシン受容体ブロッカー(ARB)、アンジオテンシン(1-7)、HMG-CoA還元酵素阻害剤、アンジオテンシン変換酵素(ACE)阻害剤、3,3´-ジインドリルメタン(DIM)、インドール-3-カルビノール(I3C)又はピルフェニドン(PFD)から選ばれる少なくとも1つを含む、方法。 - 請求項3に記載の方法において、
前記アンジオテンシン(1-7)が環状Ang1-7を含む、方法。 - 請求項3に記載の方法において、

前記レニン-アンジオテンシン系モジュレーターが、アンジオテンシン受容体ブロッカー(ARB)、アンジオテンシン(1-7)、HMG-CoA還元酵素阻害剤及びアンジオテンシン変換酵素(ACE)阻害剤のうち少なくとも1つをさらに含む、医薬組成物。 - それを必要とする対象におけるウイルス感染を治療するための医薬組成物であって、
抗酸化物質と結合したインドール-3-カルビノール(I3C)を含むレニン-アンジオテンシン系(RAS)モジュレーター;及び

前記レニン-アンジオテンシン系モジュレーターが、アンジオテンシン受容体ブロッカー(ARB)、アンジオテンシン(1-7)、HMG-CoA還元酵素阻害剤及びアンジオテンシン変換酵素(ACE)阻害剤のうち少なくとも1つをさらに含む、医薬組成物。 - それを必要とする対象におけるウイルス感染を治療するための医薬組成物であって、
抗酸化物質に連結したピルフェニドン(PFD)を含むレニン-アンジオテンシン系(RAS)モジュレーター;及び
