この研究では、48匹のオスのSprague Dawleyラット(プラス56匹の合計8匹のエキストラ/交換品、Charles River Laboratories)、生後>8週、ベンダーによって移植された頸静脈血管カテーテルを備えた300-500gが使用された。ラットは、尾のマーキングまたは一意の英数字の表示を付けたイヤータグによって識別された。(SOP MRI-1504「動物およびケージの識別手順」を参照)。

動物の準備
研究1日目(GD暴露の日)の前またはその日に、ラットを希薄動物実験施設に移した。
研究1日目に、グループA~Eのすべての動物にコリンエステラーゼ反応促進剤を投与し、GDチャレンジの30分前に腹腔内(IP)経由でHI-6(125mg/kg)を投与した。グループA-Eのすべてのラットは、1回の皮下注射でGD(154μg/kgまたは~1.4xLD50)に暴露された。グループA-Eの約1分後のGD注射、アトロピン硝酸メチル(AMN)をIM経路(2.0mg/kg)で投与した。上記の表1に定義されるGD暴露後の発作活動開始の約20分後、グループAからEのすべての動物に、筋肉内に2mg/kgのミダゾラムを投与した。グループFは、HI-6、GD、AMN、ミダゾラム、およびRYANODEX(商標登録)を受け取らなかった。RYANODEX(商標登録)は、ハミルトンのマイクロシリンジを介して投与され、その後、カテーテルは、注射用の滅菌水0.2mLでフラッシュされ、静脈への完全な送達を確実にした。

ベンダーからの受領後最初の72時間以内に、各動物の体重を記録し、その後1、3、5、10、17、24、28日目に再び記録した。動物は攻撃後も体重が減少し続けていることを認識した。動物の体重が元の体重の10%以上減少した場合、体重が減少するのではなく、常に増加するまで毎日体重を測定した。すべてのグループの体重を図1に示す。RYANODEX(商標登録)で処理したすべてのソマン暴露グループ(グループA、B、C、D)の体重は、試験25日までに陰性対照(グループF)値に回復した。
一般的な健康状態の観察は、隔離期間と順応期間を通して毎日行われた。動物は、検疫と順応期間を通して、そして生涯を通して毎日2回取り扱われた。試験1日目に、暴露前にラットを観察した。GD曝露後、発作の初期出現を評価するためにラットを頻繁にモニターした。ミダゾラム、RYANODEX(商標登録)および/または滅菌水投与後、動物は最低でも1日2回、安楽死の日に少なくとも1回観察された。かがんだ姿勢、脱水、粗い被毛および食欲不振を含む臨床観察を評価し、記録した。該当する場合、修正されたRacineスコアが各観測ポイントで記録された。この時点で、SOP MRI-1528「動物の観察手順」に従って、他の異常な臨床観察も研究ノートまたは適切なデータキャプチャフォームに記録された。死亡時刻が記録された。死亡した動物は、発見された時点で記録される。
研究の1日目は、ソマン暴露とRYANODEX(商標登録)治療の日であった。死亡したすべてのラットは死んでいることが判明した(瀕死状態では安楽死されなかった)。プロトコルで指定されたラットの数は8匹の生存者/グループであり、これは高ソマン線量に関連した死亡率のために達成されなかった。全生存率は、ソマンにさらされた45匹のラットのうち27匹(60%)だった。表2を参照。
ソマン暴露により損傷を受けた脳領域には、海馬と嗅内、前頭、および頭頂皮質が含まれていた。これらの領域には、学習、記憶形成、情報処理、その他の認知プロセスのための構造と神経回路が含まれている。RYANODEX(商標登録)の潜在的な神経保護効果を評価するために、学習、記憶、感覚運動統合、および適応反応を必要とする一連の確立された行動テストを使用して、ラットを評価した。
スクロース嗜好試験
スクロース嗜好試験(SPT)は、ラットの自然な傾向を利用して、通常の水よりも砂糖の水を好む。快楽を求める行動(快感)またはそれの欠如(快感)を測定する確立されたテストであり、水道水と1%スクロース水を含むボトルの左右の配置の変化に動物が適応する必要がある。SPTは14日目と15日目に行われた。
強制水泳試験(FST)は、げっ歯動物における抗うつ薬の有効性を迅速にスクリーニングする方法として、1970年代後半にPorsoltによって開発された。未治療(「正常な」)げっ歯動物で5分間のFSTの終わり頃に生じる不動の増加は、「行動の絶望」を反映すると解釈された。ただし、このテストの構成概念の妥当性は、次のような多くの理由で疑問視される。2)FSTの従属変数は、試験に対する動物の急性反応であり、動物の特性ではない。3)浮遊行動の「行動的絶望」としての解釈は擬人化される。現在、未処理のラットで見られる進行性の不動は、逃げる可能性のない容器に入れられる急性ストレスに対する適応反応を反映すると考えられる。
神経行動学的検査の順序は、最もストレスの少ないものから最もストレスの多いものへと進んだ。同じ日に2つの試験は実行されなかった。すべての試験は午前中に行われた。試験は次のように実行された(チャレンジの日としての研究1日目以降)。
o研究8日目、スクロース嗜好試験(最長8日間)

研究29日目の各グループについて、生存しているすべてのラットに、200mg/kg未満のケタミンと20mg/kg未満のキシラジンの目標用量のIP注射で麻酔をかけた。ラットを0.4%パラホルムアルデヒドに続いてヘパリン添加生理食塩水で経心的に灌流した(灌流プロトコルはRao et al(2006)。各ラットの心臓を採取し、評価のために固定液で保存した。
各ラットの7つの脳切片を、6点半定量スコアリングシステムを使用して顕微鏡で評価しました。顕微鏡的病変は6点スケールで等級付けされた:

全体的に、RYANODEX(商標登録)投与動物は、非投与動物と比較して、神経行動学的試験で優れたパフォーマンスを示した。RYANODEX(商標登録)処理動物は、非処理動物と比較して、脳皮質の細胞壊死レベルが低かった。RYANODEX(商標登録)処理動物は、細胞死に影響を与える用量依存的および時間依存的な治療効果を示した。これらの神経行動学的検査と神経病理学的所見は一貫しており、直接相関しているようだ。RYANODEX(商標登録)はすべてのグループで忍容性が良好であり、新しい安全性シグナルは観察されなかった。
この研究の目的は、ラットの確立されたGD(ソマン)生存モデルにおいてRYANODEX(商標登録)が神経保護作用を有するかどうかを判断することである。ソマン誘発性発作の発症後20または60分に、ボーラスIV注射により10または30mg/kgRYANODEX(商標登録)の単回投与が行われる。生存は、皮下(SQ)ソマン注射の30分前の塩化アソキシム(HI-6)、SQソマン注射の1分後のアトロピン硝酸メチル、硫酸アトロピン、塩化プラルドキシム(2-PAM-Cl)、および少なくとも3のRacineスコアを達成するソマン誘発性発作の発症から20分後のミダゾラムでの治療によって促進される。コントロールには、2つの別個のグループが含まれ、未処理(ナイーブ)ラットのグループ(ネガティブコントロール)の1グループ、及び滅菌水または滅菌水とマンニトールビークル、ソマン誘発性発作の発症後60分のいずれかを投与する別のグループである。

HI-6、ソマン、硫酸アトロピン、硝酸アトロピンメチル、2-PAM Cl、およびミダゾラム:HI-6の単回投与(IP、125mg/kg);ソマン(SC、154μg/kg、1.4xLD50)、アトロピン硝酸メチル(IM、2mg/kg);硫酸アトロピン(0.45mg/kg)、2-PAM Cl(25mg/kg)、およびミダゾラム(IM、1.8mg/kg)は、モデルの以前の経験に基づいて選択される。適切な生存を確保するには、HI-6、硝酸アトロピン、およびミダゾラムを含める必要がある。このレジメンは、少なくとも3のRacineスコアと追跡調査のための容認可能な数の生存者を達成する痙攣をもたらすと予想される。
下の表に示すように、合計120匹に加えて、ベンダーによって移植された頸静脈カテーテルを備えた最大20匹の追加のオスSprague-Dawleyラットが6つのグループで使用される。動物は20の分隊で実行される。すべてのグループ割り当ては、各分隊が各分隊の少なくとも1つの動物によって表されるという規定でランダムに割り当てられる。

GDは、例えばSOP MRI-5821「研究開発および試験評価(RDTE)希釈溶液からの標準およびサンプルの調製」に従って、氷冷0.9%塩化ナトリウムで調製される。
動物の準備
試験1日目(GD曝露の日)の前またはその日に、ラットを試験場所に移す。
研究1日目に、すべての動物は最も近いグラムに重み付けされ、すべての治療用量はそれらの体重に基づいて計算される。グループA~Eは、GDチャレンジの30分前に腹腔内(IP)経路を介してコリンエステラーゼ活性化剤HI-6(125mg/kg)を受け取る。グループA~Eのすべてのラットは、1回の皮下注射でGD(154μg/kg(308ug/mL)~1.4xLD50)に曝露される。グループA-Eの約1分間のGD注射後、硝酸アトロピン(AMN)がIM経路(2.0mg/kg)で投与される。上記の表に定義されているGD暴露後の発作活動の開始から約20分後、グループAEのすべての動物は、2-PAM-Cl(25mg/kg)を筋肉内に混合した硫酸アトロピン(0.45mg/kg)を投与し、1.8mg/kgの用量でミダゾラムを筋肉内投与する。グループFは、HI-6、GD、AMN、ミダゾラム、およびRYANODEX(商標登録)を投与しない。RYANODEX(商標登録)は、投与される投与量に応じて、100uL Hamilton microsyringeまたはLuer lock syrine(1cc)を介して投与される。その後、カテーテルを少なくとも0.2mLの滅菌水でフラッシュし、静脈への完全な送達を確保する。

体重は、各動物について、ベンダーからの受領後最初の72時間以内に研究で記録され、その後再び研究1および2日に記録される。受領時の最初の体重測定から研究1日目の体重測定までの間に体重が増加しない動物はすべて交換される。
一般的な健康状態の観察は、検疫と順応期間を通して毎日行われる。動物は、検疫と順応期間中、および生涯を通して毎日2回扱われる。研究1日目に、ラットを観察し、暴露前に体重を測定する。GD暴露後、発作の発症を記録するためにラットを継続的に監視する。ミダゾラム、RYANODEX(商標登録)および/またはコントロール溶液の投与後、動物は発作発症後1、2、4、および6時間で、安楽死の日に少なくとも1回観察される。かがんだ姿勢、脱水、粗い被毛、食欲不振などの臨床所見が評価され、記録される。該当する場合、Racineスコアは各観測ポイントで記録される。他の異常な臨床的観察も、この時点で、例えばSOP MRI-1528「動物観察の手順」に従って、研究ノートまたは適切なデータ収集フォームに記録される。死亡時刻が記録され、死亡した動物は、発見された時点で記録される。
研究2日目の各グループについて、生存しているすべてのラットに、75mg/kg以上のペントバルビタール溶液をIP注射することにより、深く麻酔をかける。ラットは、ヘパリン生理食塩水に続いて0.4%パラホルムアルデヒドまたは10%中性緩衝ホルマリンで経心的に灌流される(灌流プロトコルはRao et al(2006)に従う。各ラットの心臓は、評価のために固定液で収集および保存される。
各ラットの7つの脳切片は、6点の半定量スコアリングシステムを使用して顕微鏡で評価される。微視的病変は6点スケールで等級付けされる。

Claims (28)
- 対象が神経剤に曝露された後に神経壊死から対象を保護する方法であって、治療有効量のダントロレンまたはその薬学的に許容される塩を含む医薬組成物を対象に投与する工程を含む方法。
- 請求項1記載の方法において、前頭頭頂皮質、海馬、および/または視床は神経壊死から保護される、方法。
- 神経剤に曝露された対象を治療する方法であって、治療有効量のダントロレンまたはその薬学的に許容される塩を含む医薬組成物を対象に投与する工程を含む、方法。
- 神経剤への曝露に起因する中枢神経系機能の低下から対象を保護する方法であって、ダントロレンまたはその薬学的に許容される塩の治療有効量を含む医薬組成物を対象に投与する工程を含む、方法。
- 神経剤誘発性発作、特にてんかん重積状態を治療する方法であって、治療有効量のダントロレンまたはその薬学的に許容される塩を含む医薬組成物を対象に投与する工程を含む、方法。
- 神経剤への曝露から生じる中枢神経系機能障害から対象を保護する方法であって、治療有効量のダントロレンまたはその薬学的に許容される塩を含む医薬組成物を対象に投与する工程を含む、方法。
- 神経剤への曝露から生じる行動変化を治療する方法であって、ダントロレンまたはその薬学的に許容される塩の治療有効量を含む医薬組成物を対象に投与する工程を含む、方法。
- 請求項1~7のいずれか1項記載の方法において、前記神経剤がアセチルコリンエステラーゼ阻害剤である、方法。
- 請求項1~8のいずれか1項記載の方法において、前記神経剤が有機リン酸塩である、方法。
- 請求項1~9のいずれか1項記載の方法において、前記神経剤が、O-ピナコリルメチルホスホノフルオリデート(ソマン)、エチルN、N-ジメチルホスホラミドシアニデート(タブン)、プロパン-2-イルメチルホスホノフルオリデート(サリン)、シクロヘキシルメチルホスホノフルオリデート(シクロサリン)、または2-(ジメチルアミノ)エチル(GV)である、方法。
- 請求項1~9のいずれか1項記載の方法において、前記神経剤が、O-シクロペンチルS-(2-ジエチルアミノエチル)メチルホスホノチオレート(EA-3148)、(S)-(エチル{[2-(ジエチルアミノ)エチル]スルファニル}(エチル)ホスフィン酸塩)(VE)、O、O-ジエチルS-[2-(ジエチルアミノ)エチル]ホスホロチオエート(VG)、S-[2-(ジエチルアミノ)エチル]O-エチルメチルホスホノチオエート(VM)、N、N-ジエチル-2-(メチル-(2-メチルプロポキシ)ホスホリル)スルファニルエタンアミン(VR)、またはエチル({2-[ビス(プロパン-2-イル)アミノ]エチル}スルファニル)(メチル)ホスフィネート(VX)である、方法。
- 請求項1~11のいずれか1項記載の方法において、前記対象が哺乳動物である、方法。
