IV-1. バッテン病の治療方法に関連するアッセイ
当業者は、本明細書に記載のアッセイ及び/または当技術分野で公知の技術(例えば、WO2018209205A1に記載のアッセイ)を使用して、本明細書に記載の組成物及び方法を試験する、例えば、バッテン病の治療方法において本明細書に提供するrAAVを試験し得る。アッセイの詳細は、実施例1及び2に提供されている。実施例1及び2はまた、そのようなアッセイをどのように使用して、本明細書で提供するrAAVを試験することができるかをより詳細に示している。
当業者は、本明細書に記載のアッセイ及び/または当技術分野で公知の技術を使用して、本明細書に記載の組成物及び方法を試験する、例えば、本明細書に提供する製剤を試験してもよい。アッセイの詳細を、実施例3及び4に提供する。実施例3及び4はまた、そのようなアッセイをどのように使用して、本明細書で提供する製剤を試験することができるかをより詳細に示している。
制御された凍結/解凍サイクルを、凍結乾燥機で実行することができる。バイアルは、シェルフ上で十分な間隔を空けることができ、緩衝液の4つのバイアルをサーモカップリングすることができる。
温度ストレス発生安定性試験を、1.0×1012GC/mLで、37℃、4日間にわたって実施して、本明細書で提供する製剤の相対的安定性を評価することができる。
長期の開発安定性試験を12か月間実施して、本明細書で提供する製剤における、-80℃(≦-60℃)及び-20℃(-25℃~-15℃)でのin vitro相対力価及び他の品質の維持を実証することができる。
ddPCR GC力価を遺伝子発現に関連付けるために、HEK293細胞を形質導入し、細胞培養上清を抗VEGF Fabタンパク質レベルについてアッセイすることにより、in vitroバイオアッセイを実施してもよい。HEK293細胞を3枚のポリ-D-リジンでコーティングした96ウェル組織培養プレートに一晩プレーティングする。次いで、細胞を野生型ヒトAd5ウイルスに事前感染させた後、構築物II参照標準と被験物質の3つの独立して調製した段階希釈液で形質導入し、各調製物を別々のプレートの異なる位置にプレーティングする。形質導入の3日後に、細胞培養培地をプレートから回収し、ELISAを介してVEGF結合Fabタンパク質レベルを測定する。ELISAの場合、VEGFでコーティングされた96ウェルELISAプレートをブロックし、次いで回収した細胞培養培地と共にインキュベートして、HEK293細胞によって産生された抗VEGF Fabを捕捉する。Fab特異的抗ヒトIgG抗体を使用して、VEGFで捕捉されたFabタンパク質を検出する。洗浄後、西洋ワサビペルオキシダーゼ(HRP)基質溶液を添加し、発色させ、停止緩衝液で停止させ、プレートリーダーでプレートを読み取る。HRP産物の吸光度またはODを対数希釈に対してプロットし、各被験物質の相対力価を、平行類似性試験に合格した後の4パラメーターロジスティック回帰モデルでフィッティングした同じプレート上の参照標準に対して、式:EC50参照÷EC50被験物質を使用して計算する。被験物質の力価は、3枚のプレートの加重平均から計算した、参照標準力価の割合として報告される。
ddPCRを使用してベクターゲノム濃度GCを評価することもできる。
DNAに結合したSYBR(登録商標)Gold核酸ゲル染色(「SYBR Gold色素」)の蛍光によって、遊離DNAを測定することができる。マイクロプレートリーダーを使用して蛍光を測定し、DNA標準で定量することができる。ng/μLでの結果を報告することができる。
SECは、Waters Acquity Arc機器ID0447(C3PO)で、パス長25mmのフローセルを用いて、Sepax SRT SEC-1000ピークカラム(PN215950P-4630、SN:8A11982、LN:BT090、5μm 1000A、4.6×300mm)を使用して実施することができる。移動相は、例えば、20mMリン酸ナトリウム、300mM NaCl、0.005%ポロキサマー188、pH6.5とすることができ、流速は0.35mL/分で20分間、カラムは周囲温度とする。データ収集は、2ポイント/秒のサンプリングレートと1.2nmの解像度で実施することができ、214、260、及び280nmで25ポイントの平均平滑化を行う。理想的な標的負荷は1.5E11GCであり得る。試料には、理想的な標的の約1/3である50μLを注入するか、5μLを注入することができる。
動的光散乱(DLS)は、試料量30μLのCorning3540 384ウェルプレートを使用してWyatt DynaProIIIで実施することができる。反復ごとに10秒間にそれぞれ10回の取得結果を収集することができ、試料ごとに3回の反復測定を実施した。溶媒は、試料に使用する溶媒に応じて、例えば、dPBS中の構築物IIの場合は「PBS」、スクロース試料を含む修飾dPBS中の構築物IIの場合は「4%スクロース」を設定することができる。データ品質基準(ベースライン、SOS、ノイズ、適合)を満たさない結果を「マーク」して、分析から除外することができる。スクロース試料を含む修飾dPBSの低遅延時間カットオフを1.4μsから10μsに変更して、約1nmでのスクロース賦形剤のピークが、人為的に低いキュムラント分析直径の結果をもたらすことに対して影響を及ぼすことを排除することができる。
低温示差走査熱量測定(低温DSC)は、TA Instruments DSC250を使用して実行することができる。約20μLの試料をTzero panにロードし、Tzero Hermetic lidで圧着することができる。試料を25℃で2分間平衡化し、次いで5℃/分で-60℃に冷却し、2分間平衡化してから、次いで5℃/分で25℃に加熱することができる。熱流データは、従来の様式で収集することができる。
様々な製剤緩衝液のpHを、-30℃までのpHを検出可能なINLAB COOLPRO-ISM低温pHプローブでモニタリングした。1ミリリットルの緩衝液を15mLのFalconチューブに入れ、次いでpHプローブを緩衝液に浸した。1枚のパラフィルムを使用して、ファルコンチューブとpHプローブの間のギャップを密閉し、汚染と蒸発を防いだ。ファルコンチューブと一緒にプローブを-20AD冷凍庫に入れた。緩衝液のpHと温度を、約20時間、またはpH対温度の挙動が繰り返しパターンを達成するまで、2.5分ごとに記録した。自動霜取りプロセスによって引き起こされる温度変化は、緩衝液のpH安定性のためのストレス条件を生成した。
浸透圧計は、凝固点降下の技術を使用して浸透圧を測定する。機器のキャリブレーションは、50mOsm/kg、850mOsm/kg、及び2000mOsm/kg NISTトレーサブル標準を使用して実施することができる。290mOsm/kgの参照溶液を使用して、浸透圧計のシステム適合性を判定することができる。
密度は、Anton Paar DMA500濃度計で、水を基準として測定することができる。試料間で、濃度計を、水、次いでメタノールで洗浄し、風乾することができる。
M. 粘度測定
粘度は、当技術分野で公知の方法、例えば、2019年に公開された米国薬局方(USP)及びそれ以前のバージョン(その全体が参照により本明細書に援用される)で提供されている方法を使用して測定することができる。
Francois,et al. Molecular Therapy Methods & Clinical Development(2018)Vol.10,pp.223-236(その全体が参照により本明細書に組み込まれる)に記載されているようにTCID50感染力価アッセイを使用することができる。2018年10月15日に提出された仮出願62/745859に記載されている相対感染力アッセイを使用することができる。

当業者は、本明細書に記載のアッセイ及び/または当技術分野で公知の技術(例えば、WO2018209205A1に記載のアッセイ)を使用して、本明細書に記載の組成物及び方法を試験する、例えば、本明細書に提供するrAAVを試験し得る。アッセイの詳細は、実施例5に提供されている。実施例5はまた、そのようなアッセイをどのように使用して、本明細書で提供するrAAVを試験することができるかをより詳細に示している。
本実施例は、CLN2疾患のモデルであるTPP1m1Jノックアウトマウスを使用して、本明細書で提供するrAAV、例えば、AAV9.CB7.hCLN2を評価するために使用し得る方法を提供する。

A. 非臨床薬理学
in vivoでの一連の非臨床薬理試験を、AAV9.CB7.hCLN2で実施した。これらの試験を、CLN2疾患の関連するマウスモデルで実施し、CSFへのAAV9.CB7.hCLN2の脳室内(ICV)投与後に、生存率の向上及び脳TPP1活性の増加が示された。神経病理学的分析により、脳、頸椎及び腰髄におけるニューロン喪失の減少、ならびに自己蛍光貯蔵物質のリソソーム内蓄積、アストロサイトーシス及びミクログリア活性化が示された。

a) TPP1m1Jノックアウトマウスの自然史研究
この研究の目的は、TPP1m1Jノックアウトマウスの表現型と組織病理学、及びヒトのCLN2疾患の重要な特徴を再現する動物モデルの能力を特徴づけることであった。この自然史研究には、20匹の動物からなる5群(10匹/性別/群)が含まれ、最長24週間観察した。また、動物の群を4週齢及び14週齢で安楽死させて疾患の進行を評価した。生存期間の中央値は、雄のノックアウト動物で16週間、雌のノックアウト動物で19.5週間であり、CLN2疾患の他のマウスモデルと同等であった(Sleat DE,et al. J Neurosci,2004;24(41):9117-26.;Geraets et al,2017.PLoS One;12(5):e0176526)。TPP1m1Jノックアウトマウスは、1か月齢と3か月齢の両方で、大脳と肝臓におけるTPP1酵素活性の欠損(検出不可能なレベル)を示した。CLN2の臨床表現型の発症は、歩行異常に基づいておよそ3か月齢であった。最も顕著な疾患の症状は、3.5か月での発作後の突然死であった。発作は雌よりも雄の方が早く観察され、ケージ内の仲間との争いに起因するストレスの増加と関連していた。TPP1m1Jノックアウトマウスの生後1週間(及びおそらくは出生前)から、TPP1酵素活性の欠損により、ニューロンの細胞質内にリポフスチン顆粒としても知られるリソソーム消化の脂質含有残基の蓄積が生じた。ニューロンの細胞質内でのリポフスチンの蓄積は、生後1か月のTPP1m1Jノックアウト動物のヘマトキシリン及びエオシン(H&E)染色によって明らかになり、アストロサイトの活性化またはアストロサイトーシス(神経炎症の指標)の増加と相関していた。その後、TPP1m1Jノックアウトマウスは、運動機能の進行性の悪化と歩行異常、振戦、発作、体重減少及び摂食不能、ならびに早期死亡を経験した。GFAP染色によって同定された神経炎症は、3か月齢でピークに達し、TPP1m1Jノックアウト動物の発症と相関していた。発症後に生存していたTPP1m1Jノックアウト動物は、CNSのニューロンに貯蔵物質の進行性の蓄積を示していた。
TPP1m1JノックアウトマウスにおけるAAV9.CB7.hCLN2の治療効果を試験するために、AAV9.CB7.hCLN2を1か月齢のC57Bl/6 TPP1m1Jノックアウトマウス(10匹/性別/群)に、0(PBS)、3×109GC/動物、または3×1011GC/動物の用量でICV投与した。エンドポイントには、臨床所見、運動協調性(ロッキングロータロッド試験)、学習能力(学習ロータロッド試験)、体重、TPP1活性、及び抗TPP1抗体が含まれていた。試験終了時に、ICV注射の26週間後、生存動物において、脳と肝臓におけるTPP1活性、ベクターの肝臓内分布、及び脳の組織病理学の修正を評価した。AAV9.CB7.hCLN2処理に関連する有害な臨床所見はなかった。CLN2関連エンドポイントのCNS及び末梢パラメーターの両方の改善に関連するTPP1レベルの用量依存的な増加が観察された。3×1011GC/動物で、TPP1m1Jノックアウトマウスにおける生存率は、26週間後に、雄では100%、雌では70%であった(図9)。
TPP1m1JノックアウトマウスにおけるAAV9.CB7.hCLN2の有効性を試験するために、digital vivarium(Vium,Inc.,USA)でバイアスのない非侵襲的フルタイムモニタリングを使用して、AAV9.CB7.hCLN2を、3×1011GC/動物で、7週齢のTPP1m1Jノックアウトマウス(7匹の雄と6匹の雌)にICV投与した。処理した動物を、年齢を一致させたビヒクル処理したTPP1m1Jノックアウトマウス(5匹の雄と5匹の雌)及びWTマウス(5匹の雄と8匹の雌)と比較した。以下のエンドポイントを継続的に記録したが、これには、夜間の動き、日中の動き、呼吸数、及び概日リズムが含まれていた。AAV9.CB7.hCLN2 ICV注射の2週間後(9週齢)に記録を開始し、予定された殺処分(注射後16週目、23週齢)または予定外の死亡の場合はそれ以前まで続けた。試験の終わりに、脳と肝臓の秤量、及び脳と肝臓の組織病理学の評価を、生存していた動物で実施した。未処理のTPP1m1Jノックアウトマウスは、振戦、猫背の姿勢、異常歩行、発作を有し、病態の悪化により安楽死させたか、または死亡が判明した。臨床徴候は14.9週目から観察されたが;一方、AAV9.CB7.hCLN2処理したTPP1m1Jノックアウトマウスの54%(13匹中7匹)は、試験全体を通して臨床的に注目すべき点はなかった。最初の臨床事象での平均年齢は、TPP1m1Jノックアウトビヒクル処理マウスで16.1週、6匹のAAV9.CB7.HCLN2処理TPP1m1Jノックアウトマウスで18.6週であった。7匹のAAV9.CB7.hCLN2処理TPP1m1Jノックアウトマウスは、臨床徴候を示さなかった。
この試験の目的は、寿命を延ばし、リソソーム蓄積物質を減少させ、TPP1酵素を欠損するマウスの脳の病理を軽減する、AAV9.CB7.hCLN2の最小有効量を確立することであった。TPP1m1Jノックアウトマウスの群に、ビヒクルまたはAAV9.CB7.hCLN2の単回ICV投与を行い、生存率をモニタリングした。野生型C57Bl/6マウスの追加的な群を対照として含めた。この試験は2つの部分に分けられ、第1の部分は9週間後の薬力学(TPP1活性)と神経病理学を評価する部分であり、第2の部分はこれらのマウスの寿命を評価する部分であった。TPP1m1Jノックアウトマウスの群(n=30/性別/群)に、0(ビヒクル)、1.25×1010、5×1010、2×1011、8.5×1011GC/動物の用量でAAV9.CB7.hCLN2の単回ICV注射(5μL)を投与した。動物(5匹/性別/群)を、投与の9週間後(13週齢)に安楽死させた。寿命に対するAAV9.CB7.hCLN2の効果を評価するために試験の第2部分に割り当てた動物は進行中であり、以下に記載する。この試験では、以下のパラメーターとエンドポイントを評価した:死亡率、臨床所見、体重、神経行動学的観察(投与前及び8週目)、TPP1活性、抗TPP1抗体、肉眼的剖検所見、臓器重量及び神経病理(9週目)。神経病理学的検査では、脳、頸椎及び腰髄のニューロン喪失、ならびに自家蛍光貯蔵物質のリソソーム内蓄積、アストロサイトーシス(GFAP)及びミクログリア活性化(CD68)を評価した。
中枢神経系:神経行動学的エンドポイントは、マウスでの3か月毒性試験、及びカニクイザルでの4週間薬力学的試験に含まれた。
LINCLバッテン病(CLN2)のための疾患モデルであるTPP1m1Jノックアウトマウスを用いて、一連のin vivo薬理学試験を実施した。TPP1m1Jノックアウトマウスは、CLN2遺伝子のエクソン8の下流のスプライスドナー部位に一塩基変異を有し、これは可溶性リソソーム酵素TPP1をコードしている。これらのマウスを用いた自然史研究は、CLN2疾患の他のマウスモデルと同様の、早期月齢での臨床症状の発症、異常な表現型の急速な進行、及び寿命の短縮、ならびに同様の病態生理学的、生化学的、及び機能的変化を含む、ヒトにおけるCLN2疾患の特徴を示していた(Sleat DE,et al. J Neurosci,2004;24(41):9117-26.;Geraets et al,2017. PLoS One;12(5):e0176526)。AAV9.CB7.hCLN2を、ICV TPP1m1Jノックアウトマウスに投与した場合、生存期間中央値が18週間(未処理)から40週間(試験継続中)に向上した。行動エンドポイントの評価は、AAV9.CB7.hCLN2で処理したTPP1m1Jノックアウトマウスにおいて、運動協調と学習の改善、及び明確な二相性概日性運動特性の回復を示した。標準的な臨床観察に関して、未処理動物とAAV9.CB7.hCLN2処理動物の間で発症及び臨床徴候(振戦及び異常歩行)に明確な差異はなかったが、これらの臨床所見の進行はなく、試験を継続するにつれて、動物は体重を増加し続けている。すべての薬理学試験において、AAV9.CB7.hCLN2は、脳と脊髄のTPP1活性を増加させた。高用量において観察されたTPP1活性の変動性は、生存率と神経病理学的エンドポイントの減少に明確な用量反応性があったことから、妥当性は不明であった。AAV9.CB7.hCLN2を使用したすべての試験において、リソソーム蓄積物質の明らかな減少、正常な神経細胞の形態、及びアストロサイトーシスとミクログリアの活性化の減少が認められた。TPP1の発現を介してCLN2の神経病理学的表現型を弱めるという点で、AAV9.CB7.hCLN2の効果は、rhTPP1をCLN2-/-マウス及びTPP1-nullダックスフントに投与した後に記載された効果と同等である(Chang et al,2008;Vuillemenot et al,2015)。脳活性の増加に加えて、AAVrh10.hCLN2で認められるように肝TPP1活性が増加した(Sondhi et al 2007)。血液脳関門はTPP1がCNSを離れるのを防ぐ可能性が高いため、全身的に検出される活性は、投与直後の末梢循環へのAAV9.CB7.hCLN2の漏出に関連している可能性がある。全身性TPP1は、CNS外の組織と臓器におけるリソソーム貯蔵物質の蓄積に関連する機能障害を減弱し得るため、これはCLN2疾患の治療に潜在的に有益である(Katz,et al,Gene therapy 2017 Feb 24(4):215-223)。肝臓の形質導入効率(TPP1活性)におけるげっ歯類の薬理学試験で観察された性別関連の差異は、血清型、プロモーター、または導入遺伝子とは無関係に、AAVの取り込みに対するアンドロゲン依存性の影響であると考えられる(Lonning et al,2002.Molecular Therapy Vol.5,No.5;Davidoff et al,2003. Blood.15;102(2):480-8)。AAV9.CB7.HCLN2を用いたカニクイザルの4週間薬力学試験では、肝臓への形質導入において雄と雌の間に明確な差異はなかった。
薬力学(導入遺伝子産物)及び免疫原性の評価を、マウスでの3か月毒性試験及びカニクイザルでの4週間薬力学試験で評価した。
C57Bl/6マウスの群(n=30/性別/群)に、0(ビヒクル)、1.25×1010、5×1010、2×1011、8.5×1011GC/動物の用量でAAV9.CB7.hCLN2の単回ICV注射(5μL)を投与した。正確な投与装置を用いた互換性試験により、低用量でのいくらかのベクター損失が示され、したがって、1.25×1010、5×1010、2×1011、8.5×1011GC/動物に対して、投与する実際の用量は、それぞれ、0.9×109(70%回収)、3.9×1010(77%回収)、1.8×1011GC/動物(90%回収)及び8.5×1011GC/動物(100%回収)であった。
カニクイザルの群(雄1匹及び雌2匹/群)に、0、3.4×1011、3.2×1012または2.9×1013ゲノムコピー(GC)/動物(1mL/動物)の用量で、大槽穿刺(CM)を介した髄腔内注射により、AAV9.CB7.hCLN2を投与した。さらなる動物の群(雄1匹と雌2匹)に、腰部髄腔内穿刺(IT-L;1mL/動物)を介して3.2×1012GC/動物を投与した。血清及びCSFの試料を、試験中に回収した。TPP1活性とTPP1濃度を、以下に対して評価した:血清とCSF(投与前[-1日目または1日目]、4、8、11、15、18、22、25及び29日目)、肝臓、脊髄(頸部、胸部及び腰部)及び脳。脳については、前頭皮質、後頭皮質、小脳、線条体、延髄、中脳及び視床由来の表層または深層試料を分析した。

3か月マウス毒性試験におけるTPP1(導入遺伝子産物)の評価により、脳、肝臓、及び血清におけるTPP1活性の用量に関連した増加が示された。薬理学試験と同様に、肝TPP1活性は雌よりも雄の方が大きかったが、これはカニクイザルでは明確ではなかった。1か月と3か月の間に明確な差異はなく、4週間目までに導入遺伝子の発現がピークに達したことが示された。この試験では、薬理学試験で認められたように、抗TPP1抗体の発生率が高く、高用量で最も低い値が観察されたが、これはアッセイの干渉に起因するか、または免疫寛容の誘導を反映したものである可能性がある。
1. 単回投与試験
a) AAV9.CB7.hCLN2:C57Bl/6マウスでの3か月間毒性試験
この試験の目的は、単回ICV投与後のC57Bl/6マウスにおけるAAV9.CB7.hCLN2の薬力学、毒性、及び免疫原性を評価することであった。マウスの群(n=30/性別/群)に、0(ビヒクル)、1.25×1010、5×1010、2×1011、8.5×1011GC/動物の用量でAAV9.CB7.hCLN2の単回ICV注射(5μL)を投与した。投与後4週間(10匹/性別/群)または13週間後(10匹/性別/群)に動物を安楽死させた。追加的なサテライト動物の群(n=5/性別/群)を各時点で安楽死させて、脳及び肝臓における導入遺伝子産物(TPP1活性)を評価した。正確な投与装置を用いた互換性試験により、より低用量でのいくらかのベクター損失が示され、したがって、1.25×1010、5×1010、2×1011、8.5×1011GC/動物に対して、投与する実際の用量は、それぞれ、0.9×109(70%回収)、3.9×1010(77%回収)、1.8×1011GC/動物(90%回収)及び8.5×1011GC/動物(100%回収)であった。
この試験の目的は、4週間後のカニクイザルにおけるAAV9.CB7.hCLN2(AAV9.hCLN2)の薬力学と毒性を評価することであった。カニクイザルの群(雄1匹及び雌2匹/群)に、0、3.4×1011、3.2×1012または2.9×1013ゲノムコピー(GC)/動物(1mL/動物)の用量で、大槽穿刺(CM)を介した髄腔内注射により、AAV9.CB7.hCLN2を投与した。さらなる動物の群(雄1匹と雌2匹)に、腰部髄腔内穿刺(IT-L;1mL/動物)を介して3.2×1012GC/動物を投与した。試験の終わりに、動物を29日目に安楽死させた。含まれるエンドポイント:臨床所見、体重、食物消費(定性的)、神経学的検査(一般的な感覚及び運動機能、脳反射及び脊髄反射)薬力学(TPP1活性及び濃度)、CSF総細胞数、臓器重量、肉眼検査及び顕微鏡検査(脳[前脳から脳幹にまたがる]、脊髄神経根と神経節が付着した脊髄[頸部、胸部、腰部及び腰仙部]、坐骨神経及び三叉神経節)。薬力学的評価のために、血清、CSF、肝臓、脊髄(頸部、胸部及び腰部領域)及び脳(前頭皮質、後頭皮質、小脳、線条体、延髄、中脳及び視床由来の表層または深層の試料を分析した)の試料を評価した。
AAV9.CB7.hCLN2は、WT AAVの組み込み機構を含まない組換えAAVである。AAVが宿主細胞に内在化されると、AAVは核への輸送と脱コーティングを経て、核内に存続する環状エピソームを形成する。AAV9.CB7.hCLN2は、産物の生産に使用されるソースヘルパープラスミドに、複製にとって重要な必須のシスエレメントが含まれていないため、形質導入された細胞では複製することができない。これは、repエレメントの存在に加えて、再生産するために活性ヘルパーウイルスも必要とするWT AAVとは対照的である。
AAV9.CB7.hCLN2を用いた3か月マウス毒性試験では、前腫瘍性または腫瘍性病変は観察されなかった。
a) 免疫毒性
マウス及びサルにおけるAAV9.CB7.hCLN2を用いた毒性試験のデータから、白血球に対するAAV9.CB7.hCLN2に関連する影響、または免疫毒性を示す脾臓、リンパ節もしくは胸腺における形態学的変化がないことが示された(ICH S8による)。マウス毒性試験で観察された脾臓の変化は、ヒト導入遺伝子産物に対して観察された免疫原性に対する適応反応であると考えられ得る。
薬力学(導入遺伝子産物)及び免疫原性(抗TPP1抗体)を含むAAV9.CB7.hCLN2の非臨床的安全性を、カニクイザル及びマウスを対象とした単回投与毒性試験で、それぞれ最長1か月及び最長3か月間評価した。これらの試験の両方で、AAV9.CB7.hCLN2に関連した生命に有害な所見はなかった。マウスとカニクイザルの両方での主な所見は、脊髄、背側神経節及び坐骨神経の軸索変性であった。
I. 序文
凍結速度及び解凍速度は、生物製剤の安定性に影響を与え得る(Cao et al.,2003,Biotechnol.Bioeng.82(6):684-90)。ゆっくりと凍結している間に水の結晶化は、生物製剤の安定性に影響を与え得る賦形剤の濃度をもたらし得る。生物製剤の安定性への影響と共に、相分離またはpHシフトもまた生じ得る。急速に凍結すると、氷の結晶が小さくなり、氷と水の界面領域が大きくなり、界面応力が生じ得る。急速な凍結はまた、氷の中に気泡を閉じ込め、解凍中に気液界面応力を引き起こし得る。ゆっくりと解凍すると、氷の再結晶化が生じ得、界面応力のために溶液中の生物製剤の安定性に影響を与え得る。
全体として、データは、0.12℃/分(または約11時間超)の低速~1℃/分(または約1時間超)の高速の範囲の速度で、5回の凍結/解凍サイクルの髄腔内製剤におけるAAV9.CB7.hCLN2の品質属性への影響が最小限であることを示している。

AAV9.CB7.hCLN2のIVRPを実施した。ddPCR GC力価を機能性遺伝子発現に関連付けるために、HEK293細胞を形質導入し、トリペプチジルペプチダーゼI(TPP1)酵素活性をアッセイすることにより、in vitroバイオアッセイを実施した。HEK293細胞を、3枚の96ウェル組織培養プレートに一晩プレーティングした。次いで、細胞を野生型ヒトアデノウイルス血清型5型ウイルスに事前感染させた後、AAV9.CB7.hCLN2参照標準と被験物質の3つの独立して調製した段階希釈液で形質導入し、各調製物を別々のプレートの異なる位置にプレーティングした。形質導入の2日後に、細胞を溶解し、低pHで処理してTPP1酵素を活性化し、TPP1による切断時に蛍光シグナルを増加させるペプチド基質を使用してTPP1酵素活性をアッセイした。RFUの蛍光を対数希釈に対してプロットし、各被験物質の相対力価を、平行類似性試験に合格した後の4パラメーターロジスティック回帰モデルでフィッティングした同じプレート上の参照標準に対して、式:EC50参照÷EC50被験物質を使用して計算した。被験物質の力価は、3枚のプレートの加重平均から計算した、参照標準力価の割合として報告された。
DNAに結合したSYBR(登録商標)Gold核酸ゲル染色(「SYBR Gold色素」)の蛍光によって、遊離DNAを測定した。マイクロプレートリーダーを使用して蛍光を測定し、DNA標準で定量した。ng/μLでの結果を報告した。
SECは、Waters Acquity Arc機器ID0447(C3PO)で、パス長25mmのフローセルを用いて、Sepax SRT SEC-1000ピークカラム(PN215950P-4630、SN:8A11982、LN:BT090、5μm 1000A、4.6×300mm)を使用して実施した。移動相は(20mMリン酸ナトリウム、300mM NaCl、0.005%Pluronic F-68、pH6.5-VA 15Apr19)であり、流速は0.35mL/分で20分間、カラムは周囲温度とした。データ収集は、2ポイント/秒のサンプリングレートと1.2nmの解像度で実施し、214、260、及び280nmで25ポイントの平均平滑化を行った。理想的な標的負荷は1.5E11GCであった。AAV9.CB7.hCLN2を5μL注入した。
動的光散乱(DLS)を、試料量30μLのCorning3540 384ウェルプレートを使用してWyatt DynaProIIIで実施した。反復ごとに10秒間にそれぞれ10回の取得結果を収集し、試料ごとに3回の反復測定を実施した。AAV9.CB7.hCLN2の溶媒を「PBS」に設定した。データ品質基準(ベースライン、SOS、ノイズ、適合)を満たさない結果を「マーク」して、分析から除外した。スクロース試料を含む修飾dPBSの低遅延時間カットオフを1.4μsから10μsに変更して、約1nmでのスクロース賦形剤のピークが、人為的に低いキュムラント分析直径の結果をもたらすことに対して影響を及ぼすことを排除した。
低温示差走査熱量測定(低温DSC)を、TA Instruments DSC250を使用して実行した。約20μLの試料をTzero panにロードし、Tzero Hermetic lidで圧着した。試料を25℃で2分間平衡化し、次いで5℃/分で-60℃に冷却し、2分間平衡化してから、次いで5℃/分で25℃に加熱した。熱流データは、従来の様式で収集した。
A. 凍結融解試験の温度特性
製品の温度は、熱伝達の制限と凍結及び融解中の緩衝液の相転移のために、シェルフと正確に一致しなかった。全体の平均速度を計算するために、サイクルの代表的な部分について、製品が25℃付近~-60℃であった期間を使用して決定した平均速度を表7に要約する。

図29は、FF/FTのシェルフとプローブの温度特性を示す。実験室のスケジューリングのために、いくつかのサイクルでより長い凍結保持を行った。
図31は、FF/STのシェルフとプローブの温度特性を示す。コンデンサーの負荷を減らすために、FF/STの第1のサイクルを25℃から-55℃まで実行し、再び25℃に戻した。その後の4サイクルを-60℃に設定した。凍結及び解凍の時間を更新しなかったため、目標速度を1.6℃/分(1.5℃/分から)及び0.13℃/分(0.125℃/分から)に増加させた。この差は、当初の目標速度から10%未満で無視することができる。第3のサイクルでの室温付近のスパイクは、サイクルを継続するためにシステムを手動リセットし、それに伴ってシェルフ温度設定が10℃付近のデフォルトへ一時的に(数分間)低下したことによるものであった。
図32は、SF/FTのシェルフとプローブの温度特性を示す。実験室のスケジューリングのために、最終サイクルでより長い凍結保持を行った。
図33は、FF/FTのシェルフとプローブの温度特性を示す。実験室のスケジューリングのために、第3のサイクルでより長い凍結保持を行った。第3のサイクルでの室温付近のスパイクは、サイクルを継続するためにシステムを手動リセットし、それに伴ってシェルフ温度設定が10℃付近のデフォルトへ一時的に(数分間)低下したことによるものであった。
in-vitro相対効力(IVRP)の結果は、髄腔内製剤中のAAV9.CB7.HCLN2の凍結及び解凍の高速及び低速のすべての順列について、対照と同様であり、方法の変動性の範囲内であった(表8)。

遊離DNAの全体的な結果の要約を表9に示す。SYBR Gold結合による遊離DNAについての範囲を提供し、これは100%に対するGC/mL(OD)値または100%ベースに対する熱ストレス結果のいずれかに基づく割合を表す。

図37に示す改質型エリオットB製剤緩衝液の低温DSCサーモグラムは、-22.4℃にピークを有する共晶溶融と、それに続く氷の融解による大きな吸熱ピークを有する。他の遷移は観察されなかった。共晶溶融物は、塩化ナトリウム及び水共晶と一致している。
この試験のこれらの結果は、髄腔内製剤中のAAV9.CB7.hCLN2について、低速(0.12℃/分または約11時間超)及び/または高速(1℃/分または約1時間超)のいずれかで最大5回の凍結/解凍サイクルが許容可能な可動域であることを示した。
この実施例は、本明細書で提供するrAAVを含む医薬組成物の製剤を評価するために使用し得る方法、特に、製剤を参照組成物、例えば脳脊髄液(CSF)と比較する方法を提供する。

簡潔に述べると、ヒト胎児腎臓(HEK)293浮遊細胞株を使用してAAV9.CB7.hCLN2ベクターを作製し、剪断ヒトアデノウイルス5型DNAによって形質転換された永久株である(Graham et al.,1977)。マスターセルバンク(MCB)由来のHEK293細胞に3つのプラスミドをトランスフェクトして、パッケージ化されたベクターゲノムを生成する:CLN2発現カセットを含むプラスミド(cisプラスミド)とAAV9キャプシドタンパク質及び複製エレメントをコードする2つのヘルパープラスミド(transプラスミド及びアデノウイルスヘルパープラスミド)。
