al.,Br J Cancer.2018,118(5):654-661)。多施設で二重盲検の第3相無作為化臨床試験では、アンロチニブが中国患者において全生存期間、無増悪生存期間を延長していることが示される。これは晩期NSCLC患者にアンロチニブは良好な忍容性があり、第三選択治療又は更なる治療として有望であることを示唆する(Han B,et al.,JAMA Oncol.2018 Nov,4(11):1569-1575)。
の提供により抗腫瘍免疫応答を増強させる。過去の20年、研究者ががん治療のための新たな免疫療法を生み出そうとして、特異的免疫チェックポイント阻害剤の開発に取り込む。晩期黒色腫患者の治療薬として、CTLA-4と結合してこれを抑制する抗体(Antibody)、イピリムマブ(Ipilimumab)(YERVOY(登録商標))の開発(Hodi,et al.,(2010)N Engl J Med 363:711-23)や、プログラム細胞死受容体-1(PD-1)と特異的に結合し阻害性PD-1/PD-1リガンド経路を遮断するニボルマブ(Nivolumab、Opdivo(登録商標))、ペンブロリズマブ(Pembrolizumab、Keytruda(登録商標))等抗体の開発(Topalian,et al.,(2012a)N Engl J Med 366:2443-54)などである。PD-1(programmed death-1、略称PD-1)は活性化されたTリンパ球及びBリンパ球によって発現された、免疫抑制を仲介する重要な免疫チェックポイント受容体であり、そのリガンドは少なくともPD-L1、PD-L2を含む。PD-L1(Programmed death-ligand 1)はCD274、B7-H1とも呼ばれ、CD274遺伝子によってコードされる40kDaの1型膜貫通タンパク質であり、PD-1のリガンドである。PD-L1もPD-1も免疫グロブリンスーパーファミリーに属し、いずれも2つの細胞外Igドメイン、即ちN末端Vドメイン及びC末端定常ドメインからなる。プログラム細胞死受容体-1(PD-1)及びB7-1(CD80)に対するPD-L1の結合インターフェースはIgV様ドメインにある(Lin,et al.,(2008)PNAS 105:3011-3016)。PD-L1は保存された短い細胞質尾部(約30のアミノ酸)を含み、PD-1は2つの細胞質チロシン依存性シグナルモチーフ(免疫受容体チロシン依存性抑制モチーフ(ITIM)、免疫受容体チロシン依存性スイッチモチーフ(ITSM))を含む。T細胞の刺激後、PD-1がチロシンホスファターゼSHP-2をその細胞質尾部内のITSMモチーフに動員することで、CD3+T細胞シグナル伝達カスケードに関与するエフェクター分子(例えば、CD3ζ、PKCθ、ZAP70)は脱リン酸化する(Freeman,et al.,(2000)J Exp Med
192:1027-34、Latchman,et al.,(2001)Nat Immunol 2:261-8、Carter,et al.,(2002)Eur J
Immunol32:634-43)。PD-L1はリンパ組織及び非リンパ組織内の白血球及び非造血細胞に幅広く分布しているだけでなく、様々ながん細胞にも幅広く分布しており、様々な腫瘍細胞の表面に高度に発現され、腫瘍の悪性度及び予後不良はPD-L1の発現レベルにも密接に関連する。腫瘍におけるPD-L1の高発現が腫瘍の攻撃性の増加や予後不良と関連があることは既に臨床データから判明する。PD-1/PD-L1複合体の形成により抑制性シグナルが伝達されT細胞の免疫応答が負に調節され、その結果、TCRが仲介するT細胞活性化、サイトカイン産生、T細胞増殖は阻害される(Fife,et al.,(2011)Nature Immunology 10:1185-1193)。相同抗原特異性T細胞の消耗又はアネルギーは誘導され(Hofmeyer,et al.,(2011)Journal of Biomedicine and Biotechnology 2011:1-9)、Th1細胞のFoxp3+制御性T細胞への分化は促進され(Armanath,et al.,(2011)Science TransMed 3:1-13、Francisco,et al.,(2009)J.Exp.Med.206:3015-3029)、エフェクターT細胞のアポトーシスは誘導される。PD-L1遺伝子の破壊により、T細胞応答がアップレギュレーションされ自己反応性T細胞が産生される(Latchman,et al.,(2004)PNAS 101:10691-10696)。PD-1又はPD-L1による抗体遮断は抗腫瘍免疫の増加につながる(Iwai,et al.,(2002)PNAS 99:12293-12297)。

本実施例は、塩酸アンロチニブの併用による晩期非小細胞肺がんの治療の研究結果を示す。主な対象集団はドライバー遺伝子陰性(即ち、EGFR、ALK、ROS1変異陰性
)で、手術治療を実施できず且つ根治的な同時化学放射線療法を受けられないと組織学的に又は細胞学的に判断された局所晩期(IIIB)の転移性又は再発性(IV期)NSCLC患者である。
抗PD-1抗体:各治療期間(合計21日)の初日(D1)にシンチリマブ注射剤200mgを静脈内注入で投与し、21日に1回投与する。
1.薬物基本情報
塩酸アンロチニブ:淡黄色の結晶性粉末、含有量98.9%、バッチ番号17316007、有効期限は2018年12月前、2~8℃遮光保存。
塩酸アンロチニブ、メシル酸レンバチニブは正大天晴薬業集団股フン有限公司が提供する。
塩酸アンロチニブ、メシル酸レンバチニブは蒸留水で所定の濃度に希釈し、mPD-1は生理食塩水で所定の濃度に希釈し、使用前に調製する。
10-cmシャーレでH22細胞を培養する。RPMI 1640培地に10%胎児ウシ血清と、ペニシリン、ストレプトマイシンとを加え、37℃で5%CO2を含むインキュベータにおいて培養する。週に2回継代し、指数関数的に成長すると、細胞を収集し、カウントし、接種する。
6~7週齢の雌性のKMマウスを使用し、上海傑思捷実験動物有限公司から購入する。承認番号はSCXK(上海)2013-0006であり、動物合格証明書番号はNo.311620400013790である。飼育基準はSPFである。
実験指標は腫瘍増殖に対する薬物の影響であり、具体的にはT/C(%)又は腫瘍増殖抑制率TGI(%)である。

腫瘍増殖抑制率(TGI)(%)=100-(T-T0)/T0×100
腫瘍が初期体積より縮小したこと、即ちT<T0又はC<C0であることを、部分寛解(PR)と定義し、腫瘍が完全に消失することを、完全寛解(CR)と定義する。
各マウスにH22細胞を皮下接種し、腫瘍が100~200mm3になると、マウスをランダムに群分けして投与する。胃内(i.g.)又は腹腔内注射(IP)投与である。溶媒群は同じ体積の蒸留水を胃内投与し、投与体積は0.1mL/10g体重である。投与用量及び投与計画の詳細を表4に示す。
塩酸アンロチニブ(1mg/kg、i.g.、QD×17)はマウスにおける肝がんH22皮下移植腫瘍の増殖を抑制でき、腫瘍増殖抑制率は61.7%である。メシル酸レンバチニブ(5mg/kg、i.g.、QD×17)のH22皮下移植腫瘍に対する腫瘍増殖抑制率は79%である。mPD-1(1mg/kg、IP、週に2回、合計5回)のH22に対する腫瘍増殖抑制率は29.3%であり、2/10の腫瘍が完全寛解している。塩酸アンロチニブとmPD-1の併用で腫瘍増殖抑制率は79.5%に上げられ、1/10の腫瘍が部分寛解し、1/10の腫瘍が完全寛解している。メシル酸レンバチニブとmPD-1の併用はH22に対する腫瘍増殖抑制率は63.4%であり、1/10の腫瘍が部分寛解している。塩酸アンロチニブとmPD-1の併用に担がんマウスは忍容性が高く、明らかな体重軽減等症状はない。塩酸アンロチニブとmPD-1の併用は塩酸アンロチニブ又はmPD-1単独より肝がんH22皮下移植腫瘍に対する効果が優れている。結果は表5を参照する。
PD-1ハイブリドーマ細胞株の作成
1.分子生物学的手法で所定のアミノ酸配列を有するPD-1融合タンパク質を調製し、前記方法で調製したPD-1融合タンパク質を抗原とし、フロイントアジュバントで乳化することでBALB/Cマウスに免疫を付与する。マウスに免疫応答が現れるとその脾細胞とマウス骨髄腫細胞の細胞融合を行ってハイブリドーマ細胞を得る。96ウェルプレートで前記ハイブリドーマ細胞を培養する。
ヒト化抗体を構築するために、マウス抗体の可変領域のアミノ酸配列とヒト可変領域の遺伝子配列を比較する。選択的な変異によりマウスアミノ酸配列の一部をヒトアミノ酸配列に導入することで、様々なヒト化抗体を得る。

本研究は14C12H1L1と塩酸アンロチニブの組み合わせによる切除不能な肝細胞がんの第一選択治療に関する多施設オープンラベル第Ib/II相臨床試験の初期結果を開示する。
14C12H1L1注射液、100mg/10mL溶液、康方天成(広東)制薬有限公司提供、2~8℃遮光保存。
投与計画及び用量:
14C12H1L1とアンロチニブを併用する。14C12H1L1は200mgのQ3W静脈注射である。アンロチニブは1日1回で8mgを投与し、投与14日と休薬7日の21日を一期間とする。
治療効果:
2019年7月15日現在、合計で13人の被験者は少なくとも1回の腫瘍評価を受けており、平均治療期間は3回である。前記患者で3例は部分寛解(PR)であり、客観的奏効率(ORR)は23.1%(3/13)であり、疾患制御率(DCR)は69.2%(9/13)である(図6、図7を参照)。
2019年6月27日現在、試験治療下で発現した有害事象(TRAE)の発生率は75%であり、3級以上TRAEは12.5%である。14C12H1L1と塩酸アンロチニブの組み合わせによる切除不能な肝細胞がんの第一選択治療はアンロチニブ単独治療(データ未記載)又は14C12H1L1単独治療(データ未記載)よりも有害事象発生率(表6を参照)が顕著に低下していることは結果から示される。

PD-1受容体及びそのリガンドPD-L1の間の相互作用の阻害剤と、
式Iの化合物又は薬学的に許容されるその塩であるチロシンキナーゼ阻害剤とを含む治療有効量の薬物の組み合わせを対象に投与することを含む対象におけるがんの治療方法。

Claims (28)
- 式Iの化合物の薬学的に許容される塩は1-[[[4-(4-フルオロ-2-メチル-1H-インドール-5-イル)オキシ-6-メトキシキノリン-7-イル]オキシ]メチル]シクロプロピルアミンの塩酸塩である請求項1に記載の薬物の組み合わせ。
- 前記PD-1及びそのリガンドPD-L1の間の相互作用の阻害剤は抗PD-1もしくは抗PD-L1抗体、又はその抗原結合フラグメントである請求項1又は2に記載の薬物の組み合わせ。
- 前記抗PD-1抗体はニボルマブ、ペンブロリズマブ、トリパリマブ、シンチリマブ、カムレリズマブ、チスレリズマブ、14C12H1L1、ゲノリムズマブ、リズマブ、HLX-10、BAT-1306、AK103、AK104、CS1003、SCT-I10A、F520、SG001、GLS-010のいずれか1種又は2種以上から選ばれる請求項3に記載の薬物の組み合わせ。
- 前記抗PD-L1抗体はアテゾリズマブ(Atezolizumab)、アベルマブ(Avelumab)、デュルバルマブ(Durvalumab)、KL-A167、SHR-1316、BGB-333、JS003、STI-A1014(ZKAB0011)、KN035、MSB2311、HLX-20、CS-1001のいずれか1種又は2種以上から選ばれる請求項3に記載の薬物の組み合わせ。
- 前記抗PD-1抗体は、
配列番号1、配列番号2、配列番号3、配列番号7、配列番号8、配列番号11、配列番号12、配列番号13、配列番号31、配列番号32、または配列番号33から選ばれるアミノ酸配列又はそれと少なくとも80%の同一性を有するアミノ酸配列を含む重鎖可変領域と、

- 前記抗PD-1もしくは抗PD-L1抗体又はその抗原結合フラグメント及びチロシンキナーゼ阻害剤はそれぞれ医薬組成物として提供され、同時に投与され、連続して投与され、又は間隔投与される請求項8~21のいずれか1項に記載の方法。
- 前記式Iの化合物又は薬学的に許容されるその塩は投与2週(14日)と休薬1週(7日)の治療期間で投与する請求項8~22のいずれか1項に記載の方法。
- 前記各治療期間に投与する薬物の組み合わせは約84~168mgの式Iの化合物又は薬学的に許容されるその塩を含む請求項8~23のいずれか1項に記載の方法。
- 前記薬物の組み合わせは約84mg、約112mg、約140mg、約168mg、又は任意の前記値からなる範囲から選ばれる量の式Iの化合物又は薬学的に許容されるその塩を含む請求項8~24のいずれか1項に記載の方法。
- 毎日に約8mg、約10mg又は約12mgの式Iの化合物又は薬学的に許容されるその塩を投与し、投与2週後、1週休薬する請求項25に記載の用途又は方法。
- 約2週に1回(q2w)又は約3週に1回(q3w)の頻度で100~600mgの抗PD-1抗体を投与する請求項8~26のいずれか1項に記載の用途又は方法。
- 約2週に1回(q2w)又は約3週に1回(q3w)の頻度で約200mgの抗PD-1抗体を投与する請求項8~27のいずれか1項に記載の用途又は方法。
