Abstract
Description
本出願は、以下の同時係属中の、2018年9月24日に出願された米国仮出願第62/735,492号、2018年12月26日に出願された米国仮出願第62/785,205号、及び2019年8月23日に出願された米国仮出願第62/891,203号に基づく優先権を主張する。前述の出願のそれぞれは、参照によりその全体が本明細書に組み込まれる。
本技術は、一般に、患者の疾患に対処するための耳介神経刺激、並びに関連するシステム及び方法に関する。特定の実施形態では、電気信号は、例えば関節リウマチ、片頭痛、及び喘息を含む様々な患者の障害のいずれかに対処するために、迷走神経の耳介枝に経皮的に送達される。これらの技術によって治療可能なさらなる障害は、本明細書の以下に説明される。電気信号は、一般に、約15kHzから約50kHzの範囲内の周波数で提供される。特定の実施形態では、信号の周波数は、患者の聴覚能力を介する潜在的に望ましくない副作用を誘発するのを回避するように、患者の聴覚限界を超えるように選択される。さらなる代表的な実施形態では、電気信号が送達される生理学的位置は、主に又は排他的に求心性の信号を生成するように意図的に選択される。したがって、刺激の効果は、脳の特定の領域を標的とする刺激によって、患者の障害の影響及び/又は根本的な原因を減少させることに限定することができ、信号が患者の他の(例えば、末梢)神経を不注意に刺激することはない。
特に明記しない限り、「約」及び「おおよそ」という用語は、記載された値の20%以内の値を指す。
本技術による代表的なシステムは、電気信号を経皮的に患者の迷走神経の耳介枝に送達する。この電気信号は、患者の片方又は両方の耳に位置決めされた、又はその内部に部分的に位置決めされた電極を介して送達される。図1は、人間の耳180の外側部分の一般的な生理機能を示しており、電気信号が印加される代表的な標的領域195を示している。外耳180は、三角窩182及び舟状窩184を部分的に取り囲み、耳垂又は耳たぶ190で終端する耳輪181を含む。耳輪181内には、対輪185、対輪脚183、対珠188、及び珠間切痕189がある。耳甲介191は、対輪185から内方にあり、耳甲介舟192及び耳甲介腔193を含み、耳甲介腔193は、耳珠187に接し、耳輪脚186によって耳甲介舟192から分離されている。外耳180の皮膚196は、鼓膜(図1では見えない)で終端する外耳道194の中へ延びている。迷走神経の耳介枝197は耳180を神経支配し、標的領域195は、一般に耳介枝197の上にある、及び/又は隣接している。
上記の代表的なシステムは、選択された信号送出パラメータに従って、患者に電気信号を送達する。信号送出パラメータには、信号を定義又は説明する特性、及び信号が送出される位置を含めることができる。一般に、信号は二相性であり、約15kHzから約50kHzの範囲の周波数で印加される。図12は、代表的な信号1260の概略図である。信号(例えば、信号波形)は、相間間隔1264によって分離された陽極パルス1261及び陰極パルス1262を含む。陽極パルス1261及び陰極パルス1262の個々の対は、パルス間間隔1265によって、隣接する対から分離することができる。各パルスは、パルス幅1263を有することができ、パルス幅1263は、実施形態に応じて、陽極パルス1261の幅と陰極パルス1262の幅とが同じであっても異なっていてもよい。信号の繰り返し周期1266は、陽極パルス1261、陰極パルス1262、相間間隔1264、及びパルス間間隔1265から構成される。周期1266の逆数は、信号の周波数に対応する。
・周波数:約15kHzから約50kHz、もしくは約20kHzから約50kHz、又は20kHz
・振幅:約0.1mAから約10mA、もしくは約1mAから約5mA、又は約2mAから約4mA
・パルス幅:約5マイクロ秒から約30マイクロ秒、例えば約20マイクロ秒

・パルス間間隔:約1〜約15マイクロ秒
・デューティサイクル:0.1秒〜15分のオン期間、0.1秒〜15分のオフ期間
本技術の実施形態は、様々な患者の適応症を治療するのに適している。代表的な適応症は次のとおりである。
(1)炎症性の適応症(例:関節炎、関節リウマチ、線維筋痛症、過敏性腸症候群、クローン病、喘息、乾癬、シェーグレン症候群、自己免疫性腎症(例:ベルジェIgA腎症)、敗血症、ループス)、
(2)神経性の適応症(例:うつ病、産後うつ病、アルツハイマー病、片頭痛、頭痛、群発頭痛、てんかん、気分障害)、
(3)睡眠関連の適応症(例:不眠症、熟睡できない、レム睡眠行動障害、睡眠時随伴症)、
(4)機能強化(例:記憶増強、連想学習)、及び/又は
(5)肺機能障害(喘息、アレルギー性鼻炎、アレルギー性気管支炎、運動誘発性気管支収縮)。
本出願の譲受人であるVorso Corp.は、現在、本技術に従って構成された装置の安全性、忍容性、及び有効性を研究するために、複数の見込みのある多施設パイロット試験を行っている。ある試験は、図13に示すように、中等度から重度の活動性関節リウマチの患者を対象とし、別の試験は、図14に示すように、反復性片頭痛の患者を対象としている。

本技術の少なくともいくつかの実施形態では、前述の電気治療信号は、薬理学的/生物学的製剤を患者に投与することも含む全体的な治療レジメンの一部として提供することができる。薬理学的/生物学的サプリメントは、電気治療の有効性及び/又は持続期間を増加させること、及び/又は電気治療は、薬理学的治療を介して得られた結果を改善することが期待される。例えば、電気治療信号は、薬効と毒性との間の差に対応する、投薬のための治療「ウィンドウ」を改善することができる。これらの薬理学的/生物学的薬剤の中には、用量依存的効果が強いものがあり、電気治療は、患者が必要とする薬剤の量を減らし、事実上、副作用を抑えることができると期待されている。代表的な例では、治療レジメンは、以下のグループから選択されるがこれらに限定されない有効量の医薬品を投与することを含むことができる。
・csDMARD(従来型合成疾患修飾性抗リウマチ性関節炎薬)群。メトトレキサート、スルファサラジン、レフルノミド、ヒドロキシクロロキン、金塩を含むが、これらに限定されない。
・bDMARD(生物学的疾患修飾性抗リウマチ性関節炎薬)群。アバタセプト、アダリムマブ、アナキンラ、エタネルセプト、ゴリムマブ、インフリキシマブ、リツキシマブ、トシリズマブを含むが、これらに限定されない。
・tsDMARD(分子標的合成疾患修飾性抗リウマチ性関節炎薬)群。トファシチニブ、バリシチニブ、フィルゴチニブ、ペフィシチニブ、デセルノチニブ、ウパダシチニブを含むが、これらに限定されない。
及び/又は
・CGRP(カルシトニン遺伝子関連ペプチド)阻害薬群。エレヌマブ、フレマネズマブ、ガルカネズマブ、エプチネズマブを含むが、これらに限定されない。
・喘息の治療に有用な薬剤には、吸入コルチコステロイド、ロイコトリエン修飾剤、長時間作用性ベータ刺激薬(LABAs)、テオフィリン、短時間作用性ベータ刺激薬(例えばアルブテロール)、イプラトロピウム(Atrovent(登録商標))、静脈内コルチコステロイド(重篤な喘息発作用)、アレルギー注射(免疫療法)、及びオマリズマブ(Xolair(登録商標))などがある。
前述のことから、開示された技術の特定の実施形態は、例示の目的で本明細書に記載されているが、技術から逸脱することなく様々な修正を行うことができることが理解されよう。例えば、上記のイヤピースの実施形態は、バイポーラ信号を送達する電極の対を含む。他の実施形態では、個々のイヤピースは、イヤピースから離れて位置決めされた戻り電極を備える単一の単極電極を含むことができ、又はイヤピースは、3つ以上の電極を含むことができる。ネックピースは、前述の図に具体的に示されている構成以外の構成にすることができる。電気治療信号が送達されるときの振幅は、いくつかの実施形態では、治療の期間を通して一定のままであるステップ関数の形態で提供することができる。他の実施形態では、例えば、振幅が単一のステップで増加するときに、患者が不快感などの感覚的な副作用を経験する場合、信号の振幅は、徐々に(例えば、複数の増分ステップにわたって)増加させることができる。

例54.前記周波数が、約15kHzから約50kHzの周波数範囲にあることを特徴とする例53の方法。

Claims (56)
- 電気治療信号を生成するための命令を有する信号生成器であって、前記電気治療信号の少なくとも一部が、患者の聴覚周波数限界以上の周波数と、約0.1mAから約10mAの振幅範囲内の振幅と、5マイクロ秒から30マイクロ秒のパルス幅範囲内のパルス幅と、を有することを特徴とする信号生成器と、
前記患者の外耳、外耳道、又はその両方の皮膚に適合するように形作られた輪郭の外面を有する少なくとも1つのイヤピースであって、患者の耳介神経と電気通信する状態に位置決めされた少なくとも2つの経皮電極を担持していることを特徴とする少なくとも1つのイヤピースと、
を備えることを特徴とする患者を治療するためのシステム。
- 前記電気治療信号の前記周波数が、約15kHzから約50kHzの周波数範囲内にあることを特徴とする請求項1に記載のシステム。
- 前記電気治療信号が、感覚異常が生じない電気治療信号であることを特徴とする請求項1に記載のシステム。
- 前記電気治療信号が、感覚反応を伴わない電気治療信号であることを特徴とする請求項1に記載のシステム。
- 前記少なくとも2つの経皮電極が、導電性ポリマー製の外面を含むことを特徴とする請求項1に記載のシステム。
- 前記信号生成器が、前記患者の首の周りで前記患者によって支持されるように位置決め可能なネックピースを含み、前記システムが、前記ネックピースと前記少なくとも1つのイヤピースとの間に結合されたイヤピースリンクをさらに備えることを特徴とする請求項1に記載のシステム。
- 前記イヤピースリンクが、少なくとも1つの細長い導体を含むことを特徴とする請求項6に記載のシステム。
- 前記少なくとも1つのイヤピースが、前記イヤピースリンクから取り外し可能であることを特徴とする請求項6に記載のシステム。
- 前記イヤピースリンク及び前記信号生成器が、一体型ハウジング内に含まれていることを特徴とする請求項6に記載のシステム。
- 前記少なくとも1つのイヤピースが、前記患者の右耳に適合するように形作られた第1のイヤピースと、前記患者の左耳に適合するように形作られた第2のイヤピースとを含むことを特徴とする請求項1に記載のシステム。
- 前記少なくとも1つのイヤピースが、前記患者の耳にカスタムフィットすることを特徴とする請求項1に記載のシステム。
- 前記患者の可聴周波数範囲内でのフィードバック信号を生成するために、前記少なくとも1つのイヤピースに結合された可聴フィードバック装置をさらに備えることを特徴とする請求項1に記載のシステム。
- 前記フィードバック信号の周波数が患者固有であることを特徴とする請求項12に記載のシステム。
- 前記患者の皮膚に対する前記少なくとも1つの電極の位置を検出するように位置決めされた近接センサをさらに備えることを特徴とする請求項1に記載のシステム。
- 前記信号生成器と無線通信するように構成された外部コントローラをさらに備えることを特徴とする請求項1に記載のシステム。
- 前記外部コントローラが、前記信号生成器を制御するためのアプリケーションを有するモバイル装置を含むことを特徴とする請求項15に記載のシステム。
- 電気治療信号を生成するための命令を有する信号生成器であって、前記電気治療信号の少なくとも一部が、
患者の聴覚周波数限界以上の平均周波数であって、前記平均周波数は、複数の周期にわたる前記信号の平均周期の逆数であり、個々の周期が、第1の極性における第1のパルスの第1のパルス幅と、前記第1の極性と反対の第2の極性における隣接する第2のパルスと、前記第1及び第2のパルス間の相間期間と、前記第2のパルスと前記第1の極性の次のパルスとの間のパルス間の期間との合計であることを特徴とする平均周波数と、
0.1mAから10mAの振幅範囲内の振幅と、
5マイクロ秒から30マイクロ秒のパルス幅範囲内のパルス幅と、
を有することを特徴とする電気治療信号を生成するための命令を有する信号生成器と、
患者の外耳、外耳道、又はその両方の皮膚に適合するように形作られた輪郭の外面を有する少なくとも1つのイヤピースであって、患者の耳介神経と電気通信するように位置決めされた少なくとも2つの経皮電極を担持している少なくとも1つのイヤピースと、
を備えることを特徴とする患者を治療するためのシステム。
- 前記電気治療信号の前記周波数が、約15kHzから約50kHzの周波数範囲にあることを特徴とする請求項17に記載のシステム。
- 前記電気治療信号が、感覚異常が生じない電気治療信号であることを特徴とする請求項17に記載のシステム。
- 前記電気治療信号が、感覚反応を伴わない電気治療信号であることを特徴とする請求項17に記載のシステム。
- 前記信号生成器が、前記患者の首の周りで前記患者によって支持されるように位置決め可能なネックピースを含み、前記システムが、前記ネックピースと前記少なくとも1つのイヤピースとの間に結合されたイヤピースリンクをさらに備えることを特徴とする請求項17に記載のシステム。
- 前記イヤピースリンクが、少なくとも1つの細長い導体を含むことを特徴とする請求項21に記載のシステム。
- 前記少なくとも1つのイヤピースが、前記イヤピースリンクから取り外し可能であることを特徴とする請求項21に記載のシステム。
- 患者の外耳、外耳道、又はその両方の皮膚に対して位置決めされたイヤピースによって担持される複数の経皮電極を介して、電気治療信号を前記患者の耳介神経に印加することを含む、患者を治療するための方法であって、

1mAから10mAの振幅範囲内の振幅と、
5マイクロ秒から30マイクロ秒のパルス幅範囲内のパルス幅と、
を有することを特徴とする前記患者を治療するための方法。
- 前記電気治療信号が前記患者に感覚異常を生じさせないことを特徴とする請求項24に記載の方法。
- 前記電気治療信号が、患者が気づく感覚反応を生じさせないことを特徴とする請求項24に記載の方法。
- 前記周波数が15kHzから50kHzの周波数範囲にあることを特徴とする請求項24に記載の方法。
- 前記電気治療信号を印加することにより、前記患者の迷走神経の耳介枝に求心性反応を生じさせることを特徴とする請求項24に記載の方法。
- 電気治療信号を印加することは、前記電気治療信号を前記患者の耳の片方だけに印加することを含むことを特徴とする請求項24に記載の方法。
- 電気治療信号を印加することは、少なくとも1つの電気治療信号を前記患者の両耳に印加することを含むことを特徴とする請求項24に記載の方法。
- 前記同じ電気治療信号が両耳に印加されることを特徴とする請求項30に記載の方法。
