Abstract
Description

本技術は一般的に、浸潤性乳癌(invasive breast cancer)のリスクを有する対象が、様々な形態の癌治療に応答するか否かに関係する。
この方法は、対象からDCISサンプルを提供すること;少なくともPRのレベル、及び、a)少なくともHER2及びSIAH2又はb)少なくともFOXA1の少なくともいずれかについて、前記DCISサンプルを分析すること;少なくともPR、HER2及びSIAH2に基づいて、又は少なくともPR及びFOXA1に基づいて、予後を提供すること(ここでサンプルがPR陽性である場合、COX2のレベルについてサンプルをさらに分析することをさらに含み、少なくともFOXA1陽性を伴うCOX2陽性が、浸潤性乳癌のリスクが高いことを示す);前記対象がHER2陽性であるかどうかを決定すること;並びに、対象がHER2陽性である場合に積極的な療法を対象に施すこと;を含み、積極的な療法は放射線療法ではなく、積極的な療法はHER2に対する抗体又はトラスツズマブからなる群から選択される。

対象に対するリスク及び対象が治療に応答する可能性の両方について、様々な対象を適切な形態の療法を特定して治療するための方法が本明細書に提供される。全ての対象が、浸潤性乳癌のリスクが高い対象であっても、様々な形態の治療、特に放射線療法に反応するわけではないことが認識されている。従って、本明細書中に提供される種々の実施形態は、浸潤性乳癌のリスクが高まった対象が、放射線療法を受けるべきか、あるいは代わりに何らかの他の療法を受けるべきかどうかを決定することを可能にする。

正常な乳管及び小葉並びに乳管内上皮増殖は、2つの上皮層から構成される。外側の筋上皮層の消失は、乳房の浸潤癌の特徴である。外側の筋上皮層は非浸潤性乳管癌とすべての良性増殖過程で同様に保持されている。従って、筋上皮特異的タンパク質に対する抗体を用いて筋上皮の存在又は消失を特定することは、形態が不明確である状況において、in situ癌と浸潤癌を区別するのに有用であり得る(Kalof AN et al., Kalof AN, Tam D, Beatty B, Cooper K. いくつかの実施形態では、DCIS核グレードがヘマトキシリン・エオシン(H&E)で染色されたスライドマウント組織切片又は同等の組織学的染色を使用することによって決定され得る。いくつかの実施形態では、評価は米国病理学会(College of American Pathologists)「Protocol for Examination from Patients with Ductal Carcinoma in Situ(DCIS)of Breast」(Lester SC, Bose S, Chen YY et alArch Pathol Lab Med 2009; 133, 15-25)に沿って行われる。I(低)、II(中)、及び/又はIII(高)の核グレードは、表12に基づいて記載される:
いくつかの実施形態では、DCISにおける壊死の存在及び程度がヘマトキシリン・エオシン(H&E)又は同等の組織学的染色で染色されたスライド装着組織切片を使用して試験され得る。この評価は、College of American Pathologists “Protocol for the Examination of Specimens from Patients with Ductal Carcinoma in Situ (DCIS) of the Breast” (2012年6月)の参考文献に基づいて行われる。いくつかの実施形態において、壊死は以下のように分類され得る:A)特定されない:壊死の証拠はない、B)焦点状(点状):小さい病巣、低倍率で不明瞭、又は単一細胞壊死、又はC)中間(面皰状/広範囲):関与する管腔の中央部分は、低倍率で容易に検出される広範囲の壊死の領域によって置き換えられる。一般に、ゴースト細胞及び核破片(karyorrhectic debris)は存在する。中心部の壊死は一般的に高悪性度の核(comedo DCIS)と関連しているが、低又は中間の核グレードのDCISでも起こりうる。いくつかの実施形態では、本明細書で提供される方法のいずれかは壊死を決定することを含むことができる。

[HER2 IHC]

いくつかの実施形態において、HER2を評価するための代替方法は、ISHであり得る。Vysis PathVysion HER−2 DNA Probe Kit(Abbott Molecular)を使用する蛍光ISH(FISH)アッセイは、各工程の間に洗浄を伴う以下の工程を含み得る。脱蝋した組織切片を、0.2N HClで20分間、次いでVysis Pretreatment Solution(1Nイソチオシアン酸ナトリウム)で30〜60分間、80℃で処理し(前処理工程)、続いてVysis Protease Solutionで10〜60分間、37℃で処理し(プロテアーゼ工程)、続いて10%中性緩衝ホルマリンで処理し(固定工程)、続いて順次濃度増加するアルコール溶液で処理することができる(脱水工程)。

いくつかの実施形態において、HER2状態は、FISH染色スライドから、最低20個のDCIS細胞核中の橙色HER2シグナル及び緑色CEP17シグナルを計数し、次いで比を計算することによって決定される。DCISは同数のオレンジ及び緑のシグナルが存在するか、又はオレンジ対緑の比が2.0未満であり、細胞あたりの平均HER2コピー数が4.0未満である場合、HER2非増幅(陰性)とみなされる。DCISは、オレンジ対緑シグナルの比が2.0より大きい場合、HER2増幅(陽性)とみなされる。細胞はまた、オレンジ対緑シグナルの比が2.0未満であり、細胞当たりの平均HER2コピー数が6.0以上である場合、増幅(陽性)とみなされる。オレンジ対緑シグナルの比が2.0未満であり、細胞当たりの平均HER2コピー数が4.0以上6.0未満である場合、さらに20個の細胞をカウントし、40個の細胞すべてについてこの比を再計算し、閾値<2.0(陰性)対≧2.0(陽性)を適用する。

Kaplan−Meier生存分析を用いて、初回DCIS診断/手術後に最初の事象(DCIS又は浸潤性再発)を経験した患者の割合を推定できる。ハザード比(HR)は、Cox比例ハザード分析を用いて決定することができる。最初の事象(DCIS又は浸潤性)の時点で、患者は、他の事象タイプについて打ち切られ得る。また、術後6ヵ月以内又は術後の最初のマンモグラフィで事象が検出された場合も、再発ではなく持続性疾患とみなすことができるため、患者を打ち切りとすることができる。いくつかの実施形態において、持続性疾患を有する対象は、本明細書中に提供される種々の方法から除外される。
上記のアッセイシステム及びスコアリングシステムが提示されているが、いくつかの実施形態では代替技術及び/又はスコアリング方法を検出のために使用することができ、これは高リスク及び/又は低リスクについて、対応するが異なるカットオフ範囲をもたらすことができる。このような状況では、本技法について本明細書で提供される範囲が2つの異なる技法によって同じサンプル(又は同じDCISサンプルからの2つのサンプル)を分析し、それらを互いに一致又は等しいものとして識別することによって、他の技法(COX−2、Ki67、p16、SIAH2、FOXA1、PR、及び/又はHER2のうちの1つ又は複数の「対応する値」について)と相関させることができる。いくつかの実施形態では、Allredスコアリングシステム、免疫スコア、及びパーセンテージ及び強度スコアリング要素を組み合わせた他のものなどの代替スコアリング方法もまた、有効性を示す。いくつかの実施形態では、COX−2、Ki67、p16、SIAH2、FOXA1、PR、及び/又はHER2のうちの1つ以上について、対応する技術及び/又はスコアが存在し得る。いくつかの実施形態では、COX−2、Ki67、p16、SIAH2、FOXA1、PR、及び/又はHER2のうちの1つ以上に対応する技術が存在し得る。いくつかの実施形態では、COX−2、Ki67、p16、SIAH2、FOXA1、PR、及び/又はHER2のうちの1つ以上についての対応するスコアが存在し得る。当業者には理解されるように、これらの対応するスコア及び/又は技法は本明細書で提供される実施形態のいずれかに使用することができ、記載されたマーカーに関する各開示及びすべての開示の代替として明確に企図される。
その後の浸潤性同側乳房事象のリスクがある対象については、少なくともBCS+放射線療法(RT)で治療し、少なくともホルモン療法(タモキシフェン又はアロマ―ぜ阻害剤など)、及び/又はHER2療法(トラスツズマブなど)のさらなる補助療法を実施してもよい。どのオプションを選択するかは、本開示に基づいて、対象が放射線療法に応答するか否かに依存する(例えば、k−rasメンバーの上昇及び/又はHER2+及びSIAH+)。いくつかの実施形態において、対象が浸潤性同側乳房事象及びDCIS同側乳房事象の両方の可能性が高い場合、対象は乳房切除術で治療することができ、対象が放射線療法に応答しないと思われる場合(例えば、k−ras経路及び/又はHER2+及びSIAH+の上昇)、少なくともホルモン療法及び/又はHER2療法のさらなる補助療法を受けることができる。
Claims (25)
- 対象を治療する方法であって、
k−ras経路において高い活性レベルを有するDCISを有する対象を特定すること、及び
前記対象に積極的な乳癌治療を施すことを含み、
前記積極的な乳癌治療は、放射線ではない、方法。 - K−ras、RAF、MAPK、MEK、ETS又はSIAHの少なくとも1つのレベルが高い場合に前記k−ras経路が高い、請求項1に記載の方法。
- 対象を治療する方法であって、
HER2陽性かつSIAH2陽性のDCISを有する対象を特定すること、及び
前記対象に積極的な乳癌治療を施すこと
を含む、方法。 - 放射線療法に応答しない対象を特定する方法であって:
浸潤性乳癌のリスクが高いDCISを有する対象を特定すること、及び
前記対象がHER2(又はEGFR)陽性かつSIAH2陽性であるかどうかを決定すること、
を含み、
前記対象がHER2陽性かつSIAH2陽性である場合、前記対象に積極的な療法を施し、
前記積極的な療法が放射線療法ではなく、
前記積極的な療法がHER2に対する抗体又はトラスツズマブからなる群より選択される、方法。 - 積極的な癌治療を行う対象を特定する方法であって:
浸潤性乳癌のリスクが高いDCISを有する対象を特定すること、及び
前記対象がHER2陽性かつSIAH2陽性であるかどうかを決定すること;
を含み、
前記対象がHER2陽性かつSIAH2陽性である場合、前記対象に積極的な療法を施し、
前記積極的な療法が放射線療法ではなく、
前記積極的な療法がHER2に対する又はトラスツズマブ抗体からなる群より選択される、方法。 - 対象に推奨する治療方法を決定する方法であって、
浸潤性乳癌のリスクが高いDCISを有する対象を特定すること;及び
前記対象がHER2陽性かつSIAH2陽性であるかどうかを決定すること;
を含み、
前記対象がHER2及びSIAH2陽性である場合、前記対象に積極的な療法を推奨し、
前記積極的な療法が放射線療法ではなく、前記積極的な療法がHER2に対する又はトラスツズマブ抗体からなる群より選択される、方法 - 対象を治療するための方法であって:
対象からDCISサンプルを提供すること;
少なくともPRのレベル、及び
a)少なくともHER2及びSIAH2、又は
b)少なくともFOXA1
の少なくともいずれかについて、前記DCISサンプルを分析すること;
少なくともPR、HER2及びSIAH2に基づいて、又はPR及びFOXA1に基づいて、予後を提供すること(ここで、前記サンプルがPR陽性である場合、COX2のレベルについてサンプルを分析することをさらに含み、少なくともFOXA1陽性を伴うCOX2陽性が、浸潤性乳癌のリスクが高いことを示す);
前記対象がHER2陽性であるかどうかを決定すること;並びに、
前記対象がHER2陽性である場合、対象に積極的な療法を施すこと;を含み、
前記積極的な療法は放射線療法ではなく、前記積極的な療法はHER2に対する抗体又はトラスツズマブからなる群から選択される、方法。 - 対象における浸潤性乳癌事象のリスクを減少させるための方法であって:
対象からDCISサンプルを提供すること;
少なくともPRのレベルについて前記DCISサンプルを分析すること、及び
a)少なくともHER2及びSIAH2について前記サンプル分析すること、又は
b)少なくともFOXA1について前記サンプルを分析すること
の少なくともいずれか;
少なくともPR、HER2及びSIAH2に基づいて、又は少なくともPR及びFOXA1に基づいて、予後を提供すること;を含み、
前記サンプルがPR陽性かつFOXA1陰性である場合は、Ki67のレベル、サイズ、又は、Ki67のレベル及びサイズについてサンプルをさらに分析すること(ここで、前記サンプルがKi67陽性である場合、DCISのサイズが5mmを超える場合、又はそれらの両方である場合は、浸潤性乳癌のリスクが高いことを示す);並びに、
前記対象が
a)HER2陽性の場合、

- 前記抗体がトラスツズマブである、請求項9に記載の方法。
- 放射線療法の利益を提供する方法であって:
浸潤性乳癌のリスクが高いDCISを有する対象を特定すること;及び
前記対象がHER2陰性である場合は前記対象に放射線療法を施し、前記対象がHER2陽性である場合は前記対象に放射線療法を施さないこと;を含む、方法。 - 対象におけるステージ1A浸潤性乳癌事象のリスクを低減するための方法であって:
対象からDCISサンプルを提供すること;
少なくともPRのレベルについて前記DCISサンプルを分析すること、及び
a)少なくともHER2及びSIAH2について前記サンプル分析すること、又は
b)少なくともFOXA1について前記サンプルを分析すること
の少なくともいずれか;並びに、
少なくともPR、HER2及びSIAH2に基づいて、又は、少なくともPR及びFOXA1に基づいて、予後を提供すること;を含み、
前記サンプルがPR陽性である場合、COX2のレベルについて前記サンプルをさらに分析すること、
FOXA1が少なくとも陽性でCOX2陽性である場合、浸潤性乳癌のリスクが高いことを示し、
浸潤性乳癌のリスクが高い場合、標準療法よりも積極的な療法を前記対象に提供することを含む、方法。 - 保険が放射線療法の費用をカバーするかどうかを決定する方法であって:
浸潤性乳癌のリスクが高いDCISを有する対象を特定すること;
前記対象がHER2陽性であるかどうかを決定すること;及び
前記対象がHER2陽性である場合には、前記対象に対する放射線療法の費用を負担せず、前記対象がHER2陰性である場合には前記対象に対する放射線療法の費用を負担すること、を含む、方法。 - 放射線療法に対する償還を提供する方法であって:
DCISを有し、さらに浸潤性乳癌のリスクが高い対象を特定すること;
前記対象がHER2陽性かつSIAH2陽性であるかどうかを決定すること;及び
前記対象がHER2陰性又はSIAH2陰性である場合、前記対象に放射線療法の費用の償還を提供すること、を含む、方法。 - 現在の標準療法の下では治療されない対象に治療を提供する方法であって:
