Description
・MRDが検出限界未満の場合、定量化が不可能である。これは、多くの患者に当てはまる。初期治療に非常によく応答し、且つその後の治療の強度が減少させることができる患者を特定する試みには大きな関心が寄せられている。そのような患者は、非常に低レベルの、例えば、10−6未満のMRDを有することを特徴とすることになるが、検出限界が10−4である場合、10−4〜10−6のレベルを有する患者と区別することができない。
・アッセイの確率的変動のために、MRDのレベルが検出限界に近いが、依然としてそれを超える場合、測定の精度は不十分である。
・2つのプライマーが結合する配列は固有ではなく、通常はゲノムに存在する配列であり、増幅の特異性は、結合配列を互いに近づける再編成に起因してのみ生じる。
・ある程度の相同性は、V遺伝子ファミリーの異なるメンバー間、及びD遺伝子ファミリーの異なるメンバーの間でも存在する。これは、上流のプライマーが非白血病性の再編成にハイブリダイズする可能性を増加させる。
・非白血病リンパ球の集団における再編成は非常に不均一であり、従って、正常集団の1つ以上の細胞の再編成が、白血病細胞の集団の普遍的な再編成に類似する可能性がある。
・再編成標的遺伝子の異なる領域を標的とする一連の上流プライマーを使用する2ラウンドまたは3ラウンドのネスティッドPCRの実施。標的とされる領域は通常、2つのN領域を含む。これは、非特異性を排除し、高感度アッセイをもたらす(例えば、Morley et al、2009)。しかし、このアプローチは、より複雑であり、PCR産物による環境汚染のリスクがある。
・PCRを次世代配列決定に置き換えること。この手順は、可能性として、10−5までのMRDを、及びおそらく、追加ステップを用いて、10−6までのMRDを測定し得る。しかし、この手順は、特に、高感度が所望される場合、複雑で費用がかかる。
・改善されたフローサイトメトリーを使用すること。
・非特異性を最小限に抑える試みで、対立遺伝子特異的(ASO)プライマーの3’末端を設計すること。効率的な増幅を得るために、PCRプライマーの3’末端にGまたはC塩基を含むことが望ましいと広く信じられている。具体的には、効率的なプライマーのハイブリダイゼーション及び伸長、3’末端に1つ以上のGまたはC塩基が存在することにより支持される。それは、これらの塩基が、AまたはT塩基よりも相補的な塩基に対してより強い水素結合を形成するからである。

・短縮プライマーの使用。

プライマー設計のガイドライン
3’末端
−最後の3’塩基がA/Tとなる
−好ましくは、最後の2または3塩基がA/Tとなる
−最後の最大4つの最も3’の塩基に対する塩基としてA/Tを有し得る。これより多い場合、プライマーは効率的に機能しないリスクがある。
−最後の7塩基における少なくとも2つのG/C。
−プライマーは、N1の一部または全て、Dの全て、及びN2の一部または全てを対象とする。
−N1の5’に最大4塩基、及び/またはN2の3’に1〜3塩基を含むことが、有利な可能性がある。これは、半固有V−N1及びN2−J接合部、ならびにJに提供される任意のA/T塩基を利用する。
−プライマーのTmが高すぎ、74℃より高い場合、1つ以上のN塩基を、実効Tmを下げるために、D領域に挿入することができる。
唯一のN領域が、V−N−Jまたは部分的なD−N−J再編成の存在下で利用可能であろう。場合により、2つのN領域が利用可能な場合であっても、特に、長く、好ましい不均一な塩基配列を含み、良好な3’末端の設計が可能になる場合、1つだけを使用することが決定されてもよい。N領域が短い場合、及び/または良好な3’末端を有するプライマーを設計することが難しい場合は、
−N領域の後に、D領域が続く場合、特に、これが3’末端を改善する場合、通常、D領域にプライマーを4〜6塩基を延長することが有利である。これは、半固有N−D接合部を利用する。
−N領域の後にJ領域が続く場合、場合により、さらに3’末端でA/Tが調査される場合、通常は、プライマーを1〜3塩基伸長させてJ領域にすることが有利である。しかし、特に、この状況では、いくつかの異なるプライマーを試験することが重要である。
−N領域の前にV領域の3’末端がある場合、プライマーが長くなりすぎない限り、5’末端にVの3〜5塩基を組み込むことが有利であり得る。これは、半固有V−N接合部を利用する。
ASOプライマーの長さ及びTmは、再編成の性質に依存するが、多くの場合、正確な長さ及びTmを決定する際にある程度の柔軟性がある。その場合、TmがリバースプライマーのTmとほぼ同じか、または1℃低くなるように、プライマーを設計することが推奨される。
PCRで使用されるアニーリング温度は、プライマーのTmに依存する。ガイドラインは、
Tm=59〜61:アニーリング温度=63〜64、
Tm=69〜71:アニーリング温度=72、
Tm=71〜74:アニーリング温度=75、
Tm>74:アニーリング温度=75であるが、Tmを低下させるためにN塩基の使用を考察する。
これらのガイドラインは通常、アニーリング温度がTc値の3℃以内になる。しかし、特に、N領域が5以下の塩基を含有する場合、唯一のN領域を用いる再編成用のプライマーに対し、多少の困難が予想され得る。そのような再編成では、アニーリング温度は、最小限の非特異性を生じるか、または非特異性を全く生じないように、Tc値付近か、またはTc値でなければならない。いくつかの候補プライマーを合成及び試験することも賢明である。
上記は、ガイドラインのみであり、温度勾配結果及びプライマー配列に依存するべきである。一部の患者では、複数の候補プライマーを合成及び試験することが望ましい。
プライマーの試験
1.増幅効率、Tc、及び特異性の決定
各患者に対し1つまたはいくつかのプライマーが合成される。アニーリング温度の勾配を使用して、各プライマーは、白血病DNAから増幅する能力及び非白血病DNAからの増幅の失敗について試験される。例は、以下である。
これは、プライマーの後処理の最後に、またはMRDを測定する最初の最終的な実験中に実施することができる。プライマーは、20ウェルに対して試験され、ウェルそれぞれは、5人の非白血病者から得られた1μgのプールされたDNAを含有する。
患者ASOプライマーの配列と、Tm及びTc値
VまたはV、N、D、N、Jは、プライマーに標的とされる再構成領域を示す。プライマーは、急性リンパ芽球性または慢性リンパ性白血病患者由来の試料用であった。
勾配−プライマーの試験用
各プライマーは、6つの異なる温度で2回ずつ調査される。
正常DNAは、非特異性についてプライマーをチェックするために、2つの低温で実行される。
200pg/μlに診断試料を希釈する。2μl/チューブを添加し、28μl/プライマーセットを必要とする
250ng/μlに正常DNAを希釈する
A列、B列、C列、D列、E列、及びF列にD0 DNAを添加する
G列及びH列にPBL DNAを添加する
表は、6つのプライマー用であるが、プレートまたはストリップでより少ないプライマーを実行するために減少させることができる。
各プライマーセットに対し、13×18μl=234μlを除去し、200pg/μlで26μlのD0 DNAを追加する
残りの混合物に、正常DNA8.5μlを250ng/μlで添加する(500ng/ウェル)。
A列、B列、C列、D列、E列、及びF列に20μlのD0混合物を、G列及びH列に20μlの正常DNA混合物を分注し、密封し、次のプロトコールを実行する。
PCRマシンCFX上、91℃で3分×1実行する:
97℃、15秒、68〜75℃、30秒、×5サイクル
96℃、15秒、68〜75℃、30秒、×5サイクル
94℃、15秒、68〜75℃、30秒、×35サイクル

この方法は、様々なアニーリング温度でプライマーの増幅効率及び特異性を試験するためのものである。

融解曲線分析
PCRは、0.5μMの濃度の蛍光色素Syto82を含有する。予備試験は、この濃度が、レポータープローブで測定して、Ctを延長しなかったことを示した。温度が徐々に上昇するにつれて蛍光が測定され、その結果は、適切なプログラムを使用して分析した。蛍光対温度曲線の1次導関数は、非特異的増幅及び特異的増幅のピークを示す。

Claims (35)
- 2つ以上のV、D、またはJ遺伝子セグメントの再編成を特徴とするIgまたはTCR核酸領域を増幅する方法であって、前記方法が、
前記再編成IgまたはTCR核酸領域を対象とするフォワード及びリバースプライマーと、目的の核酸試料を接触させることと、
(i)前記主題の増幅反応のTc及び(Tc−3℃)の間の範囲内のアニーリング温度、及び/または
(ii)3’末端ヌクレオチド位置に、少なくとも1つのA及び/またはTヌクレオチドを含む、少なくとも1つのプライマー、
ならびに、任意に、融解曲線分析を行う手段
を使用して、前記核酸試料を増幅させること
とを含む、前記方法。 - 哺乳動物のクローン細胞集団を検出及び/または監視する方法であって、そのクローン細胞が、2つ以上のV、D、またはJ遺伝子セグメントの再編成を特徴とするIgまたはTCR核酸領域を特徴とし、前記方法が、
(i)前記再編成IgまたはTCRに核酸領域を対象とする前記フォワード及びリバースプライマーと、哺乳動物に由来する生物学的試料のDNA物質を、前記標的核酸分子との前記プライマーの相互作用を促進するのに十分な条件下で接触させること、
(ii)(a)前記主題の増幅反応のTc及び(Tc−3℃)間の範囲内のアニーリング温度、及び/または、(b)少なくとも1つのプライマーであって、前記プライマーの3’末端ヌクレオチド位置に、少なくとも1つのA及び/もしくはTヌクレオチドを含む、前記プライマー、ならびに、任意に、融解曲線分析を実施する手段、を使用して前記核酸標的を増幅すること、ならびに
(iii)前記増幅産物を検出すること
を含む、前記方法。 - 前記方法が、
前記主題の増幅反応のTc及び(Tc−3℃)間の範囲内のアニーリング温度と、
1つ以上のプライマーであって、前記プライマーの3’末端ヌクレオチドの位置に、AまたはTヌクレオチドを含む、前記プライマー
との両方を使用することを含む、請求項1または2に記載の方法。 - 前記核酸領域が、DNAである、請求項1〜3のいずれか1項に記載の方法。
- 前記プライマーの2つの3’最末端ヌクレオチド位置のそれぞれに、A及び/またはTヌクレオチドが存在する、請求項1〜5のいずれか1項に記載の方法。
- 前記プライマーの3つの3’最末端ヌクレオチド位置のそれぞれに、A及び/またはTヌクレオチドが存在する、請求項1〜5のいずれか1項に記載の方法。
- 前記プライマーの4つの3’最末端ヌクレオチド位置のそれぞれに、A及び/またはTヌクレオチドが存在する、請求項1〜5のいずれか1項に記載の方法。
- 前記プライマーの5つの3’最末端ヌクレオチド位置のそれぞれに、A及び/またはTヌクレオチドが存在する、請求項1〜5のいずれか1項に記載の方法。
- 前記プライマーの6つの3’最末端ヌクレオチド位置のそれぞれに、A及び/またはTヌクレオチドが存在する、請求項1〜5のいずれか1項に記載の方法。
- 前記フォワードまたはリバースプライマーのいずれかまたは両方が、前記A及び/またはTヌクレオチドを含むように設計される、請求項1〜10のいずれか1項以上に記載の方法。
- 前記リバースプライマーのみが、前記A及び/またはTヌクレオチドを含むように設計される、請求項1〜10のいずれか1項以上に記載の方法。
- 前記VDJ再編成が、部分的再編成であり、前記再編成V及びD遺伝子セグメント、前記再編成D及びJ遺伝子セグメント、または前記再編成V及びJ遺伝子セグメントのうちの1つ以上の間の前記接合部に位置するN領域が存在する、請求項1〜11のいずれか1項以上に記載の方法。
- 前記VDJ再編成が、完全再編成であり、前記再編成V及びD遺伝子セグメント、前記再編成D及びJ遺伝子セグメント、または前記再編成V及びJ遺伝子セグメントのうちの1つ以上の間の前記接合部に位置するN領域が存在する、請求項1〜11のいずれか1項以上に記載の方法。
- 前記VDJ再編成が、部分的再編成であり、前記V遺伝子セグメント、D遺伝子セグメント、またはV遺伝子セグメントのうちの1つ以上の中に位置するN領域が存在する、請求項1〜12のいずれか1項以上に記載の方法。
- 前記VDJ再編成が、完全な再編成であり、前記V遺伝子セグメント、D遺伝子セグメント、またはV遺伝子セグメントのうちの1つ以上の中に位置するN領域が存在する、請求項1〜11または13のいずれか1項以上に記載の方法。
- 前記プライマーの少なくとも1つが、前記再編成V、D、及びJ遺伝子セグメントの1つのN遺伝子セグメントを対象とするハイブリダイゼーションサブ領域を含む、請求項1〜14のいずれか1項以上に記載の方法。
- 前記プライマーの少なくとも1つが、前記再編成V、D、及びJ遺伝子セグメントの少なくとも2つのN遺伝子セグメントを対象とするハイブリダイゼーションサブ領域を含む、請求項1〜14のいずれか1項以上に記載の方法。
- 前記J遺伝子セグメントを対象とする前記プライマーが、3’末端に、少なくとも1つのA及び/またはTヌクレオチドを含む、請求項1〜17のいずれか1項に記載の方法。
- プライマーサブ領域が、N混合物由来のヌクレオチドと、前記サブ領域のヌクレオチドのうちの1つ以上を置換するように改変される、請求項1〜18のいずれか1項に記載の方法。
- 前記サブ領域の4つおきのヌクレオチドが置換される、請求項19に記載の方法。
- 前記サブ領域の3〜7つの隣接ヌクレオチドの配列が置換される、請求項20に記載の方法。
- 融解曲線分析が実施される、請求項1〜21のいずれか1項に記載の方法。
- 前記融解曲線分析が、染料を使用して実施される、請求項22に記載の方法。
- 前記染料が、フルオロフォアである、請求項23に記載の方法。
- 前記方法が、Ct決定ステップを含む、請求項1〜26のいずれか1項に記載の方法。
- 前記Ct決定が、前記再編成IgまたはTCR領域を対象とするプローブを使用して実施され、このプローブが、検出手段に結合している、請求項25に記載の方法。
- 前記検出手段が、蛍光分子である、請求項26に記載の方法。
- 前記少なくとも1つのプライマーが、前記ASOプライマーである、請求項1〜27のいずれか1項に記載の方法。
- 下流の遺伝子セグメントを対象とする前記プライマーが、前記Jセグメントを対象とする、請求項1〜28のいずれか1項に記載の方法。
