Description
・MRDが検出限界以下の場合、定量化が不可能である。これは、多くの患者の場合である。初期治療に非常によく応答し、且つその後の治療の強度が減少させることができる患者を特定する試みには大きな関心が寄せられている。そのような患者は、非常に低レベルの、例えば、10−6未満のMRDを有することを特徴とすることになるが、検出限界が10−4である場合、10−4〜10−6のレベルを有する患者と区別することができない。
・アッセイの確率的変動のために、MRDのレベルが検出限界に近いが、依然としてそれを超える場合、測定の精度は不十分である。
・2つのプライマーが結合する配列は固有ではなく、通常はゲノムに存在する配列であり、増幅の特異性は、結合配列を互いに近づける再編成に起因してのみ生じる。
・ある程度の相同性は、V遺伝子ファミリーの異なるメンバー間、及びD遺伝子ファミリーの異なるメンバーの間でも存在する。これは、非白血病性再編成にハイブリダイズする上流プライマーの確率を増加させる。
・非白血病リンパ球の集団における再編成は非常に不均一であり、従って、正常集団の1つ以上の細胞の再編成が、白血病細胞の集団の普遍的な再編成に類似する可能性がある。
・再編成標的遺伝子の異なる領域を標的とする一連の上流プライマーを使用する2ラウンドまたは3ラウンドのネスティッドPCRの実施。これは、非特異性を排除し、高感度アッセイをもたらす(例えば、Morley et al、2009)。しかし、このアプローチは、より複雑であり、PCR産物による環境汚染のリスクがある。
・PCRを次世代配列決定に置き換えること。この手順は、可能性として、10−5までのMRDを、及びおそらく、追加ステップを用いて、10−6までのMRDを測定し得る。しかし、この手順は、特に、高感度が所望される場合、複雑で費用がかかる。
・約60℃のアニーリング温度と組み合わせて、上流のASOプライマーを最大のN領域に、下流のプライマーを生殖細胞系J配列に導くこと(例えば、Bruggemann et al,2004;Pongers−Willemse et al,1999;Nakao et al,2000;van der Velden et al,2002;van der Velden et al,2004;van der Velden et al,2007;van der Velden et al,2009;van der Velden et al,2014;Verhagen et al,2000)。

・Tmが最大約65℃のプライマーの使用及び/または最大69℃のアニーリング温度の使用(Bruggemann et al,2004、Pongers−Willemse et al,1999、Nakao et al,2000、van der Velden et al,2002、van der Velden et al,2007、van der Velden and van Dongen,2009、van der Velden et al,2014、Verhagen et al,2000)。
・短縮プライマーの使用。

・1.2Mから1.5mMまでの一価陽イオン(Na+)濃度、600mMから0.01mMまでの二価陽イオン(Mg++)濃度、及び二価陽イオン濃度の最大120%の三リン酸(dNTP)濃度を有する中性緩衝液(pH7〜8)中の長さが8〜60塩基のオリゴの予測は正確である。
・オリゴ濃度は、相補的な標的の濃度よりも大幅に高い(少なくとも6倍)とみなされ、これは、分子生物学実験の大部分に当てはまる。そうでない場合は、標的の濃度は、無視できず、ボックスに入力されるべきである。
・[鎖1]≧[鎖2]の場合、オリゴ濃度=[鎖1]−[鎖2]/2

・DNAテンプレートにハイブリダイズされた非連続LNA塩基は、McTigue’04のモデルを使用する。DNAテンプレート上の連続LNA塩基及びRNAテンプレート上の任意のLNA塩基は、RNAエネルギーパラメーターを仮定しており、それ故、予測の精度が低い。

3’末端
−A/Tになる最後の塩基
−好ましくは、A/Tになる最後の2塩基
−最後の最大4塩基に対する塩基としてA/Tを有し得る。これ以上の場合、プライマーが効率的に機能しないリスクがある。
−最後の6塩基中の2つ以下のG/C。
−プライマーは、N1の一部または全て、Dの全て、及びN2の一部または全てを対象とする。
−N1の5’に最大4塩基、及び/またはN2の3’に1〜3塩基を含むことが、有利な可能性がある。これは、半固有V−N1及びN2−J接合部、ならびにJに提供される任意のA/T塩基を利用する。
−プライマーのTmが高すぎ、74℃より高い場合、1つ以上のN塩基を、実効Tmを下げるために、D領域に挿入することができる。
唯一のN領域が、V−N−Jまたは部分的なD−N−J再編成の存在下で利用可能であろう。場合により、2つのN領域が利用可能な場合であっても、1つだけを使用することが決定されてもよく、時に、それが長い場合、好ましい不均一な塩基配列を含み、良好な3’末端の設計が可能になる。
−N領域の後にD領域が続く場合、特に、これが3’末端を改善する場合、通常、D領域にプライマーを4〜6塩基を延長することが有利である。これは、半固有ND接合部を利用する。
−N領域の後にJ領域が続く場合、場合により、さらに3’末端でA/Tが調査される場合、通常は、プライマーを1〜3塩基伸長させてJ領域にすることが有利である。しかし、特に、この状況では、いくつかの異なるプライマーを試験することが重要である。
PCRで使用されるアニーリング温度は、プライマーのTmに依存する。ガイドラインは、以下のとおりである。
Tm=69〜72、アニーリング温度=72。
Tm=72〜74、アニーリング温度=75。
Tm>74、アニーリング温度=75であるが、N塩基の使用を検討する。
実際に最終的に使用される温度は、様々なアニーリング温度の試験中のプライマーの機能の仕方により決定される。
上記は単なるガイドラインであり、ほとんどの患者の場合、複数の候補プライマーを合成及び試験することが望ましい。
1.増幅効率及び特異性の決定
各患者に対し1つまたはいくつかのプライマーが合成される。アニーリング温度の勾配を使用して、各プライマーは、白血病DNAから増幅する能力及び非白血病DNAからの増幅の失敗について試験される。
一例が以下に示される。

Claims (35)

- 前記少なくとも2つのN遺伝子セグメントが、N1DN2、VN1DN2、またはN1DN2J再編成の前記N遺伝子セグメントである、請求項19に記載の方法。
- 前記V、DJ再編成が、完全なVDJ再編成である、請求項17に記載の方法。
- 前記少なくとも2つのN遺伝子セグメントが、前記N1DN2再編成の前記N遺伝子セグメントである、請求項22に記載の方法。
- 前記フォワード及び/または前記リバースプライマーが、3’末端に少なくとも1つのA及び/またはTヌクレオチドを含む、請求項1〜24のいずれか1項に記載の方法。
- 前記J遺伝子セグメントを対象とする前記プライマーが、3’末端に少なくとも1つのA及び/またはTヌクレオチドを含む、請求項1〜24のいずれか1項に記載の方法。
- プライマーサブ領域が、前記サブ領域の前記ヌクレオチドのうちの1つ以上をN混合物からのヌクレオチドで置換するように改変される、請求項1〜24のいずれか1項に記載の方法。
- 前記サブ領域の4ヌクレオチド毎が、置換される、請求項26に記載の方法。
- 前記サブ領域の3〜7つの隣接ヌクレオチドの配列が、置換される、請求項26に記載の方法。
- 哺乳動物のクローン細胞集団を検出及び/または監視する方法であって、クローン細胞が、2つ以上のV、D、またはJ遺伝子セグメントの再編成を特徴とするIgまたはTCR核酸領域を特徴とし、前記方法が、請求項1〜27のいずれか1項に記載の増幅方法を実施すること、及び増幅産物を検出することを含む、前記方法。
- 前記方法が、2つ以上のV、D、またはJ遺伝子セグメントの前記再編成を含むIgまたはTCR核酸領域を特徴とするクローン細胞集団を特徴とする病状を診断または監視するために使用される、請求項29に記載の方法。
- 前記クローン細胞が、クローンリンパ系細胞の集団である、請求項30に記載の方法。
- 前記病状が、新生物、好ましくは、リンパ系新生物である、請求項31に記載の方法。
- 前記リンパ系新生物が、白血病、リンパ腫、または骨髄腫である、請求項32に記載の方法。
- 前記方法が、最小残存疾患を検出するために使用される、請求項29〜33のいずれか1項に記載の方法。
- 前記哺乳動物が、ヒトである、請求項1〜34のいずれか1項に記載の方法。
