[0022] 同種培養生後胸腺組織由来産物は、先天性無胸腺症に起因するT細胞免疫不全(原発性免疫不全)の治療に有用であることが示されている。無胸腺症によるT細胞免疫不全は、機能的胸腺の発達を防止する先天性障害、例えば、22q11.2欠失に関連する完全型ディジョージ異常(cDGA)およびchd7(クロモドメイン−ヘリカーゼ−DNA結合タンパク質7)遺伝子における突然変異に関連するCHARGE(コロボーマ、心臓欠陥、後鼻孔閉鎖、成長または精神遅滞、性器発育不全、および耳異常または難聴)症候群、および無胸腺患者におけるフォークヘッドボックスタンパク質N1(FOXN1)欠損に関連する。先天性無胸腺症は、まれな致死的状態であり、現在、規制当局が承認した製剤を利用した薬物治療の選択肢を有さない。治療せずに放置し、小児の免疫系の治療的再構築が起こらない場合、先天性無胸腺症による一次免疫不全は、致死的であり、ほとんど全ての乳児が2歳前に、ほとんどの場合、重症感染によって死亡する。

一実施形態では、第1〜第4の態様の方法は、細胞寛容を示すための混合リンパ球反応における今後の使用のために、死亡ドナーからの末梢血単核細胞を凍結保存するステップをさらに含む。

全身麻酔の誘導後、大腿前部コンパートメントの1つにかけて縦皮膚切開を行う(典型的には長さ約5cm)。注記:切開のサイズおよび移植処置のための片方または両方の脚の使用は、患者のサイズ、計画された移植組織の量、および彼/彼女の筋肉量によって決定される。組織の全てまたは大部分を片脚に移植することができる場合、片脚のみが使用されるべきである。

Claims (124)
- 固形臓器移植を必要とするレシピエントにおいて、死亡ドナーから得られる同種固形臓器移植に対するドナー特異的寛容を促進するための方法であって、前記方法が、
(a)前記レシピエントの胸腺の除去ステップと、
(b)前記レシピエントを、1つ以上の免疫抑制剤を含む誘導免疫抑制レジメンで治療して、前記レシピエントのT細胞を枯渇させる、および/または前記レシピエントのT細胞が前記移植された固形臓器を拒絶するのを抑制するステップと、
(c)死亡ドナーからの適切な固形ヒト臓器および胸腺の両方を提供するステップと、
(d)前記固形ヒト臓器を前記レシピエントに移植するステップと、
(e)前記レシピエントを維持免疫抑制レジメンで治療するステップと、
(f)同種培養生後胸腺組織由来産物を提供するステップであって、前記同種培養生後胸腺組織由来産物が、前記固形臓器ドナーの適切な胸腺組織から得られ、前記ドナー胸腺組織が、同種培養生後胸腺組織由来産物を産生するために最大21日の期間コンディショニングレジメンに供され、さらに、前記ドナー胸腺組織のための前記コンディショニングレジメンが、胸腺臓器培地において前記ドナー胸腺組織を無菌で処理して、部分的にT細胞が枯渇したドナー胸腺組織片を産生することを含み、前記部分的にT細胞が枯渇したドナー胸腺組織片が、前記組織全体に散在したサイトケラチン(CK)(抗体AE1/AE3を使用)について陽性の領域、少なくとも1つのハッサル小体の存在、前記組織全体に散在したCK14染色、およびインタクトな核の存在を示す、ステップと、

(f)前記凍結保存された同種培養生後胸腺組織由来産物を凍結保存同種培養生後胸腺組織由来産物バンクにおいて液体窒素中で維持するステップと、を含む、方法によって調製される、凍結保存同種培養生後胸腺組織由来産物。 - 前記胸腺が、採取日に、領域の50%超がレース状染色パターンでケラチンについて陽性であること、ハッサル小体が存在すること、CK14がレース状パターンで染色すること、および核の90%超がインタクトであることを示す、請求項66に記載の凍結保存同種培養生後胸腺組織由来産物。
- 固形臓器移植を必要とするヒトレシピエントにおいて、生体ヒトドナーから得られる同種固形臓器移植に対するドナー特異的寛容を促進するための方法であって、前記方法が、
(a)前記レシピエントの胸腺の除去ステップと、
(b)前記レシピエントを、1つ以上の免疫抑制剤を含む誘導免疫抑制レジメンで治療して、前記レシピエントのT細胞を枯渇させる、および/または前記レシピエントのT細胞が前記移植された固形臓器を拒絶するのを抑制するステップと、
(c)生体ヒトドナーからの適切な固形臓器を提供するステップと、
(d)前記固形臓器を前記レシピエントに移植するステップと、
(e)前記レシピエントを維持免疫抑制レジメンで治療するステップと、
(f)凍結保存同種培養生後胸腺組織由来産物バンクに維持された凍結保存同種培養生後胸腺組織由来産物を提供するステップであって、前記凍結保存同種培養生後胸腺組織由来産物が、前記固形臓器移植に存在しない前記レシピエントにおけるHLA対立遺伝子に適合したHLA対立遺伝子を発現する胸腺ドナーからの胸腺組織から処理され、前記ドナー胸腺組織が、最大12日の期間コンディショニングレジメンに供され、さらに、前記ドナー胸腺組織のための前記コンディショニングレジメンが、胸腺臓器培地において前記ドナー胸腺組織を無菌で処理して、部分的にT細胞が枯渇した胸腺組織片を産生することを含み、前記胸腺組織片が、組織全体に散在したケラチンAE1/AE3について陽性の領域、少なくとも1つのハッサル小体の存在、組織全体に散在したCK14染色、および前記コンディショニングレジメンの完了時にインタクトな核の存在を示す、ステップと、
(g)前記凍結保存同種培養生後胸腺組織由来産物を解凍するステップと、
(h)前記解凍された凍結保存同種培養生後胸腺組織由来産物を前記レシピエントに移植するステップであって、前記凍結保存同種培養生後胸腺組織由来産物の投与量が、約1,000〜20,000mm2の胸腺組織表面積/1m2のレシピエント体表面積であり、さらに、前記移植された同種培養生後胸腺組織由来産物が、前記レシピエントにおいて胸腺リンパ球新生および寛容を誘導する、ステップと、を含む、方法。 - HLA適合が、第2のフィールドにおける対立遺伝子のアミノ酸配列の小さな変動を許容する、請求項68に記載の方法。
- HLA適合が、第2のフィールドにおけるHLA−DP対立遺伝子の不適合を許容する、請求項68に記載の方法。
- 前記解凍された凍結保存同種培養生後胸腺組織由来産物の約半分が、前記レシピエントに移植され、残部が、今後の使用のために凍結保存される、請求項68に記載の方法。
- ステップ(h)が、前記固形臓器の移植の約1か月以上後に行われる、請求項68に記載の方法。
- 固形臓器移植を必要とするヒトレシピエントにおいて、死亡ヒトドナーから得られる同種固形臓器移植に対するドナー特異的寛容を促進するための方法であって、前記方法が、
(a)前記レシピエントの胸腺の除去ステップと、
(b)前記レシピエントを、1つ以上の免疫抑制剤を含む誘導免疫抑制レジメンで治療して、前記レシピエントのT細胞を枯渇させる、および/または前記レシピエントのT細胞が前記移植された固形臓器を拒絶するのを抑制するステップと、
(c)生体ヒトドナーからの適切な固形臓器を提供するステップと、
(d)前記固形臓器を前記レシピエントに移植するステップと、
(e)前記レシピエントを維持免疫抑制レジメンで治療するステップと、
(f)凍結保存同種培養生後胸腺組織由来産物バンクに維持された凍結保存同種培養生後胸腺組織由来産物を提供するステップであって、凍結保存同種培養生後胸腺組織由来産物が、固形臓器移植片中に存在しないレシピエントにおけるHLA対立遺伝子に適合したHLA対立遺伝子を発現する胸腺ドナーからの胸腺組織から処理され、ドナー胸腺組織が、最大12日の期間コンディショニングレジメンに供され、さらに、ドナー胸腺組織のためのコンディショニングレジメンが、胸腺臓器培地においてドナー胸腺組織を無菌で処理して、部分的にT細胞が枯渇した胸腺組織片を産生することを含み、胸腺組織片が、コンディショニングレジメンの完了時に、組織全体に散在したケラチンAE1/AE3について陽性の領域、少なくとも1つのハッサル小体の存在、組織全体に散在したCK14染色、およびインタクトな核の存在を示す、ステップと、
(g)前記凍結保存同種培養生後胸腺組織由来産物を解凍するステップと、
(h)前記解凍された凍結保存同種培養生後胸腺組織由来産物を前記レシピエントに移植するステップであって、前記凍結保存同種培養生後胸腺組織由来産物の投与量が、約1,000〜20,000mm2の胸腺組織表面積/1m2のレシピエント体表面積であり、さらに、前記移植された同種培養生後胸腺組織由来産物が、前記レシピエントにおいて胸腺リンパ球新生および寛容を誘導する、ステップと、を含む、方法。 - 前記胸腺が、採取日に、領域の50%超がレース状染色パターンでケラチンについて陽性であること、ハッサル小体が存在すること、CK14がレース状パターンで染色すること、および核の90%超がインタクトであることを示す、請求項68または73に記載の方法。
- 前記レシピエントの胸腺の前記除去が、外科手術によるものである、請求項68または73に記載の方法。
