Abstract
Description
本発明は、1以上の哺乳類ディフェンシンの投与により、同種造血幹細胞移植を受けたか、又は受けようとしている患者における急性及び慢性の移植片対宿主病(GVHD)及びGVHDと関連付けられる合併症の治療並びに/又は予防のための方法に関する。本方法は、腸と肺の微生物叢の正常化及びサイトカインストームのリスクを排除する低下されたサイトカイン生成による免疫系のバランスの再調整に基づいて、下痢及び敗血症などのGVHDの症状を治療できる。
移植片対宿主病は依然として、同種造血幹細胞移植後の罹患率と死亡率の主要な原因である。胃腸のGVHDが死亡の主な原因である(Teshima、2016)。急性GVHDの初期段階は、主に胃腸管での組織の損傷とそれに関連する粘膜バリア機能の喪失によって誘発される。これは、悪性疾患をその後の免疫制御に適する最小限の残存レベルにし既存の免疫機能を取り除き、未治療のドナーの接種材料の生着を可能にするのに必要とされる投与計画の調節によって引き起こされる。幹細胞移植は通常、これらの二重の目的を達成するために、全身照射、免疫抑制及び化学療法を用いる。しかしながら、それらはまた、GI管粘膜及び「サイトカインストーム」に寄与する他の細胞を損傷する。サイトカインストームは、炎症性サイトカイン;古典的には、Th1サイトカイン:IFNγ、IL−2及びTNF−αならびにTh2サイトカイン:IL−4、IL−5、IL−10及びIL−13の放出によって特徴付けられる(Henden、2015)。現在主流のGVHD療法は、広範囲の免疫抑制に焦点を当てており、それは生着に影響を及ぼし、場合によっては、望ましい移植片対腫瘍活性を低下させ得る(Nalle、2015)。移植後の期間に抗菌除染を使用することによる細菌負荷の低減は、GVHDの重症度を低下させることができる。肺及び皮膚は、線維症−閉塞性細気管支炎及び強皮症として現れる慢性GVHDの主な標的臓器である(Henden、2015年)。急性GVHDは、依然として治療の失敗の主な原因であり、その結果、20%の受容者が死亡し、生存者の60〜80%がある程度の慢性GVHDを経験する(Markey、2014)。
定義:
ディフェンシン:本明細書で使用される「ディフェンシン」という用語は、抗菌ペプチドのディフェンシンクラスに属するポリペプチドを指す。ディフェンシンは、健康な微生物叢を維持し潜在的な病原体を追い払うのに役立つ、主要な先天性宿主防御の1つを代表する(Wehkampら、2002及びSalzmanら、2007)。ディフェンシンは、グラム陽性菌及び陰性菌、真菌及び古細菌に対して抗菌作用を保有し、抗炎症活性を発揮するペプチドである。ヒトのディフェンシンは、それらの3つの分子内システインジスルフィド結合のトポロジーに基づいて、α−ディフェンシン及びβ−ディフェンシンに分けられる、小さなカチオン性ペプチドである(図6)。哺乳類ディフェンシンには明確な構造的特徴がある:成熟ペプチドは小さく(3〜6kDa、28〜42アミノ酸長)、カチオン性で(正味電荷+1〜+11)、両親媒性であり、3つの逆平行鎖βシートの高度に保存された3次構造を有する。ディフェンシン間の構造の保存は、アミノ酸配列の変動性を考えるといくぶん驚くべきことであり、ディフェンシンファミリー全体で6つの保存されたシステイン残基による。これらの6つのシステイン残基は3つのジスルフィド結合を形成し、それは3本鎖βシートのコア構造を安定化させる(Dorinら、Molecular Medical Microbiology(第2版)、2015の中の哺乳類の抗菌ペプチド;ディフェンシン及びカテリシジン)。α−ディフェンシンは、小腸の陰窩(HD5とHD6又はDEFA5とDEFA6)中でパネート細胞により発現されるものである。β−ディフェンシン(DEFBn)は主に、皮膚、目、中耳、口、気管、肺、消化管、泌尿生殖器系、腎臓、膣、肝臓、膵臓及び乳腺を含む様々な組織並びに臓器で上皮細胞によって生成される。ディフェンシンの例は、ヒト腸アルファディフェンシン5(HD5;配列番号5);ヒト腸アルファディフェンシン6(HD6;配列番号6);どちらもアルファディフェンシンクラスに属する;また、ヒトベータディフェンシン1(hBD1;配列番号1);ヒトβディフェンシン2(hBD2;配列番号2);ヒトβディフェンシン3(hBD3;配列番号3);ヒトβディフェンシン4(hBD4;配列番号4);短縮型ヒトβディフェンシン2(配列番号7)を含む。ディフェンシンは前駆体として発現され、シグナルペプチド及び場合によってはプロペプチドの切断によってプロセシングされた後、細胞外空間に分泌される。ヒトのディフェンシンのいくつか、例えばhBD1は、構成的に生成されるが、他のもの、例えば、hBD2、hBD3及びhBD4は、炎症性サイトカイン又は外因性微生物の生成物によって誘導される。上記の特定された配列は、予測される成熟生理活性ディフェンシンを表す。当業者なら、プロセシングは細胞ごとに異なる可能性があること、かつ生じる分泌成熟ペプチドは、C末端若しくはN末端の1個又は2個のアミノ酸が予測される配列と異なり、なお生物活性を維持し得ることを理解するであろう。
この開示は、同種造血幹細胞移植で治療されているか、若しくは治療されようとしている対象の治療又は予防における、ヒトアルファディフェンシン及びベータディフェンシン、より好ましくはヒト科などの哺乳類アルファディフェンシン及び/又はベータディフェンシンの使用に関する。
哺乳類のα−ディフェンシン又はβ−ディフェンシンの半減期は、α−ディフェンシン又はβ−ディフェンシンを別の部分と融合又はコンジュゲートすることによって、すなわち、α−ディフェンシン又はβ−ディフェンシンとして同じ方法で投与されるα−ディフェンシン又はβ−ディフェンシンのインビボ血漿半減期と比較して実質的に増加されるα−ディフェンシン又はβ−ディフェンシンのインビボ血漿半減期をもたらす医薬として許容される分子に結合された、長時間作用する生物活性のあるα−ディフェンシン又はβ−ディフェンシンを構築することによって、延長させることができる。
ヒトのベータディフェンシン2及びHD5を、重度の腸炎症及び腸内菌共生バランス失調(デキストラン硫酸ナトリウム、トリニトロベンゼンスルホン酸及びCD4+CD25+T細胞移植)の予防の動物モデルと治療の動物モデルの両方で試験した。α−ディフェンシン及びβ−ディフェンシンは経口投与又は皮下投与にかかわらず、腸の健康を改善することにより、これらの化学物質によって引き起こされる下痢及び体重減少を軽減する。重要なことに、ディフェンシンは、全てGVHDの治療に日常的に使用されている、プレドニゾロン/デキサメタゾン及びシクロスポリンなどの強力で標準的なGVHD免疫抑制剤並びに抗TNF−αと同等に疾患活動指数と炎症マーカーとしてのミエロペルオキシダーゼ活性を減少させる。そのため、ディフェンシンは、同種造血幹細胞移植を受けたか、若しくは受けようとしている患者の予防的及び/又は治療的処置として強力な活性を示す。
哺乳類アルファディフェンシン及び哺乳類ベータディフェンシンを含む哺乳類抗菌ペプチドは、当技術分野で知られている従来の方法を使用して、インビトロ合成によって調製されてよい。様々な市販の合成装置、例えば、Applied Biosystems社、Beckmanなどの自動合成装置が利用可能である。合成装置を使用することにより、天然アミノ酸を非天然アミノ酸、特にD異性体(又はD型)、例えば、D−アラニン及びD−イソロイシン、ジアステレオ異性体、及び異なる長さ又は機能性を有する側鎖などで置換してよい。特定の順序及び調製の方法は、利便性、経済性、及び必要な純度などによって決定される。化学的結合は、アミド又は置換アミン形成のためのアミノ基、例えば還元的アミノ化、チオエーテル又はジスルフィド形成のためのチオール基、及びアミド形成のためのカルボキシル基などの、結合に便利な機能を含む様々なペプチド又はタンパク質に提供されてよい。必要に応じて、様々な基を、合成中又は発現中にペプチドに導入してよく、それは他の分子又は表面への結合を可能にする。そのため、システインはチオエーテルを作製するために使用でき、ヒスチジンは金属イオン錯体に結合するため、カルボキシル基はアミド又はエステルを形成するため、及びアミノ基はアミドを形成するために使用できる、などである。
ヒトのアルファディフェンシン又はヒトのベータディフェンシンなどの哺乳類アルファディフェンシン又は哺乳類ベータディフェンシンは、好ましくは患者に対して許容可能な毒性を伴い同種造血幹細胞移植を受けた患者の移植片対宿主病を予防又は治療するのに有効な量にて医薬組成物中で好ましく使用される。ヒトアルファディフェンシン及びヒトベータディフェンシンなどの哺乳類アルファディフェンシン及び哺乳類ベータディフェンシンはまた、好ましくは治療を必要とする患者に対して許容できる毒性を伴い、肺及び/若しくは腸で正常な微生物叢構成を維持するか、又は肺及び/若しくは腸での腸内菌共生バランス失調の微生物叢を治療又は正常化するのに有効な量にて医薬組成物中で好ましくは使用される。
一実施形態では、哺乳類ディフェンシンは、患者が同種造血幹細胞移植を受ける前のある期間、患者に投与される。この場合、ディフェンシンの投与は、移植の間継続されてよく、移植が完了した後にさらに投与されてよい。一実施形態では、本開示による哺乳類ディフェンシンは、同種造血幹細胞移植を受けるときに患者に投与される。例えば、ディフェンシンは移植と同じ日に投与されてよい。ディフェンシン投与を移植後に継続してよい。
哺乳類アルファディフェンシン及びベータディフェンシンは、任意の従来の経路による投与のために製剤化された組成物中で治療的に使用できる。
本発明の組成物を投与するために気道投与を使用してもよい。肺内投与とは、肺への局所投与を意味する。本明細書で使用される場合、「気管内、気管支内又は肺胞内投与」という用語は、安定剤又は他の賦形剤の添加の有無にかかわらず、ディフェンシンの溶液の点滴による、粉末形態でディフェンシンを適用することによる、又はエアロゾル化若しくは噴霧化された溶液若しくは懸濁液又は吸入される粉末若しくはゲルとしてのディフェンシンの吸入による気道の関連部分にディフェンシンを到達させることによるかにかかわらず、ディフェンシンが気管、気管支又は肺胞にそれぞれ適用されるような投与の全ての形態を含む。

本開示で使用するための医薬組成物又は製剤は、医薬として許容される担体、好ましくは水性担体若しくは希釈剤と組み合わされた、好ましくはそれらに溶解されたディフェンシン、又はペグ化調製物として、又は吸入によるエアロゾルとして投与されるリポソーム若しくはナノ粒子調製物として、又は気管支肺胞洗浄液として若しくはブラインド気管内洗液又は洗浄液として気管支鏡を介して投与される洗浄液体として下気道に運ばれるディフェンシンを含む。0.9%生理食塩水、緩衝生理食塩水、及び生理学的に適合性のある緩衝液などを含むがこれらに限定されない様々な水性担体を使用してよい。組成物は、当業者によく知られている従来の技術によって滅菌されてよい。得られた水溶液は、使用のために包装されるか、又は無菌条件下で濾過され凍結乾燥されてよく、この凍結乾燥調製物は、投与前に滅菌水溶液に溶解される。
実施例1
10日間の経口デキストラン硫酸ナトリウム誘発性大腸炎マウスの予防モデルにおける腹腔内マウス抗TNF−αに対するhBD2の皮下投与と静脈内投与の抗炎症効果
治療の投薬計画:
図1は、研究計画を示す。
1ケージあたり5匹のグループで飼育された70匹の雄のC57BL/6マウスを、7つの異なる治療グループに割り当てた。全ての動物にDSS 2%を補充した飲料水を7日間与えて大腸炎を誘発し結腸組織に顕著な炎症と損傷をもたらした。これらの動物にhBD2を尾静脈に静脈内投与するか又は背側皮膚に皮下投与し、マウス抗TNF−α又はビヒクルとしてPBSを10日間腹腔内投与した。
疾患活動指数(DAI)スコアリングシステムを臨床評価に適用した(図7)。10日目に動物を屠殺し、結腸を、組織学的スコアリングシステムを使用する組織学的評価のために摘出した(図8)。
最も重要な効果は、hBD2が1日1回又は2回のいずれかで皮下経路により投与されたときに得られたが、hBD2の静脈内投与もDAIの大幅な減少をもたらした。抗TNFαとhBD2の両方が、DAIに対して同様の統計的に有意な効果を示した。hBD2は、特に1日2回皮下投与した場合、組織学的スコアに対し非常に統計的に有意な効果を与えたが、抗TNF−αはこのスコアに対し有意な影響を示さなかった(図9)。

治療の投与計画:
ケージあたり5匹のグループで飼育された50匹の雄のC57BL/6マウスを、5つの異なる治療グループに割り当てた。全ての動物にDSS 2%を補充した飲料水を7日間与えて大腸炎を誘発し結腸組織に顕著な炎症と損傷をもたらした。これらの動物に、3つの異なる用量レベルでhBD2を1日2回経口投与するか、又はプレドニゾロンを1mg/kgで1日2回投与するか、又はビヒクルとしてPBSを10日間投与した。
疾患活動指数(DAI)スコアリングシステムを臨床評価に適用した(図7)。10日目に動物を屠殺し、結腸を、組織学的スコアリングシステムを使用する組織学的評価のために摘出した(図8)。
これらの動物がより多く摂食し、より良い健康状態を経験したことを示す体重減少の大幅な低下が、3つのhBD2用量レベル全てで見られたが、5mg/kgで1日2回投与した高用量グループで最も顕著であった(図10)。hBD2の2つの低用量レベルは、プレドニゾロンと同等にDAIに対し統計的に有意な影響を及ぼしたが、高用量hBD2(5mg/kgの1日2回)は、これらよりもDAIに対し有意に優れた影響を及ぼした(図11)。プレドニゾロンとhBD2の両方が組織学的スコアに対し統計的に有意な効果を示したが、全3つのhBD2用量グループの効果はプレドニゾロンよりも有意に優れていた(図12)。
8日間のトリニトロベンゼンスルホン酸誘発性大腸炎マウスの予防モデルにおけるプレドニゾロンの皮下投与に対するhBD2の皮下投与の抗炎症効果
治療の投与計画:
ケージあたり5匹のグループで飼育された105匹の雄のBALB/cByJマウスを、7つの異なる治療グループに割り当てた。軽い麻酔下で0日目にトリニトロベンゼンスルホン酸の結腸内投与によって大腸炎を誘発し、結腸組織の顕著な炎症と損傷をもたらした。これらの動物に、1日1回の4つの異なる投与レベル(1mg/kg、0.3mg/kg、0.1mg/kg、及び0.03mg/kg)のhBD2又は1日1回のプレドニゾロン10mg/kg又はビヒクルとしてのPBSを背側皮膚下に7日間皮下投与した。
疾患活動指数(DAI)スコアリングシステムを臨床評価に適用した(図7)。10日目に動物を屠殺し、結腸を、組織学的スコアリングシステムを使用する組織学的評価のために摘出した(図8)。

7日間のデキストラン硫酸ナトリウム誘発性大腸炎マウスの予防モデルにおける経口シクロスポリンに対するhBD2の経口投与と皮下投与の抗炎症効果
治療の投与計画:
ケージあたり5匹のグループで飼育された80匹の雄のC57BL/6マウスを、7つの異なる治療グループに割り当てた。全ての動物に、DSS 3%を補充した飲料水を7日間与えて、上皮の損傷、陰窩構造の破壊によって特徴付けられる大腸炎、並びに粘膜と粘膜下組織における単核及び好中球の蓄積によって特徴付けられる中程度の炎症を誘発した。これらの動物に、0.6mg/kgで1日2回又は0.4mg/kgで1日3回の用量でhBD2を経口投与し;0.15mg/kgで1日2回又は0.1mg/kgで1日3回の用量でhBD2を皮下投与するか、又は50mg/kgで1日1回のシクロスポリン又は水を7日間与えた。
スコアリングシステム(0=便に血液がない、1=便に血液が存在する証拠及び2=重度の出血)に従って臨床評価を実施した。7日目に動物を屠殺し、結腸を組織学的評価のために摘出した。
結腸の長さの維持(図14)と臨床スコア(図15)における非常に統計的に有意な改善が、4つのhBD2投与グループ全てとシクロスポリンについて観察された。hBD2の1日3回投与は、両方の投与経路での1日2回投与よりも効果的であり、皮下投与は経口投与よりも効果的であるように思われた(図15)。
14日間のデキストラン硫酸ナトリウム誘発性大腸炎マウスの治療モデルにおける腹腔内投与の抗TNF−αと腹腔内投与のデキサメタゾンに対するhBD2の皮下投与の抗炎症効果
治療の投与計画:
1ケージあたり5匹のグループで飼育された70匹の雄のC57BL/6マウスを、7つの異なる治療グループに割り当てた。全ての動物に、DSS 2%を補充した飲料水を14日間与えて、大腸炎を誘発し、結腸組織の顕著な炎症と損傷をもたらした。これらの動物に、hBD2を0.1mg/kgで1日2回皮下投与し;8日目〜14日目まで0.1mg/kgで1日1回皮下投与するか、又は1.2mg/kgで1日1回静脈内投与するか、又は8、10及び13日目にマウス抗TNFαを250μg/マウスで腹腔内投与するか、又はデキサメタゾン1mg/kgで1日1回腹腔内投与するか、又はビヒクルとしてPBSを皮下投与した。
疾患活動指数(DAI)スコアリングシステムを臨床評価に適用した(図7)。14日目に動物を屠殺し、結腸を、組織学的スコアリングシステムを使用する組織学的評価のために摘出した(図8)。
DAIに対する統計的に有意な効果が、hBD2の0.1mgで1日1回皮下投与及びデキサメタゾンの1mg/kgで1日1回腹腔内投与について観察された。同様に、組織学的スコアに対する統計的に有意な効果(図16)が、hBD2の0.1mg/kgで1日1回皮下投与とデキサメタゾンの両方で観察された。
CD4+CD25+T細胞移植誘発性大腸炎マウスモデルの14週間の治療での抗TNF−α(エンブレル)の皮下投与及びデキサメタゾンの腹腔内投与に対するhBD2の皮下投与の有効性
治療の投与計画:
1ケージあたり5匹のグループで飼育された70匹の雌のBALB/cマウスを、7つの異なる治療グループに割り当てた。BALB/cマウスからのCD4+CD25+T細胞の移植によってSCIDマウスで大腸炎を誘発した。脾臓から単離したリンパ球及びBALB/cマウスからのリンパ節を、CD4+T細胞のネガティブ選択に供した。その後、CD4+CD25+細胞をCD4+T細胞懸濁液からビーズへの結合によりポジティブに単離し、CD4+CD25−を上清から収集した。動物に、7日目から86日連続でhBD2を0.1mg/kgで1日1回皮下投与するか、1mg/kgで1日1回皮下投与するか、又は3mg/kgで1日1回皮下投与するか、又はマウス抗TNFαを100μg/マウスで週2回皮下投与するか、又はデキサメタゾンを0.3mg/kgで1日1回腹腔内投与するか、又はビヒクルとしてPBSを皮下投与した。
臨床評価は、体重減少、便の硬さ及び直腸周囲の血液の存在に基づいた。95日目に動物を屠殺し、結腸を、体重及びミエロペルオキシダーゼ活性を評価するために摘出した。
臨床スコアに対する統計的に有意で類似した効果が、hBD2の1mg/kgで1日1回皮下投与、デキサメタゾンの0.3mg/kgで1日1回腹腔内投与及びエンブレルについて観察された(図17)。同様に、これらの3つの投与計画は、結腸重量(図18)とミエロペルオキシダーゼ活性(図19)に対して統計的に有意で類似した効果を示した。
ディフェンシンによる予防的処置による腸内細菌叢の保護と維持
マウス:マウスをグループごとに3匹ずつ、4つのケージで飼育した。餌の摂取量を、午後6時に照明が消される直前に毎日記録した。個々のマウスを、グループとケージの両方の順序を変更して実験手順に供した。マウスを、SPF標準条件で12時間の明/暗サイクル下で、室温で維持した。
微生物分析を行って腸の微生物叢を研究した。長期的な16S特性評価を、60匹のマウスからの4つのペアの試料、合計で240の試料に対して行った。各マウスを、食事変更の前、食事変更の1週間後、食事変更の4週間後及び終了時にサンプリングし、従ってディフェンシン治療の結果としての糞便微生物叢の完全な特性評価を保証した。
体重変化
食物摂取量は3つの実験の食事グループ全てで同様であったが、両方の高脂肪食(HFD)グループは、10週間の研究期間にわたって低脂肪食(LFD)参照グループよりも有意に多く体重が増加した(*p<0.0001、二元配置分散分析、テューキー事後検定)。しかしながら、HFDプラスhBD2グループは、HFD参照グループよりも有意に少なく体重が増加した(*p=0.0028)。
hBD2は、主に微生物の存在に影響を与えたが、HD5及びhBD2+HD5は主に微生物の存在量に影響を与えた(図20)。アロバキュラム(Allobaculum)の存在量の統計的に有意な増加が、HD5による予防後に小腸で観察された(p<0.02;図21)。アロバキュラムは、短鎖脂肪酸生成種である。短鎖脂肪酸は、GPCR43を介する結腸Treg細胞の恒常性の調節に重要な役割を果たす。T細胞の低形成とT細胞の恒常性の混乱は、急性GVHDと慢性GVHDの両方の中心的な部分である。図22は、異なる治療グループの種の相対的な存在量を示しhBD2とHD5の腸内細菌叢に対する効果を示す、属分析である。結腸におけるバルネシエラの存在量の統計的に有意な増加が、hBD2による予防的処置後に観察された(p<0.03;図24)。バルネシエラは、入院患者で観察され得る抗生物質耐性病原菌の腸内優勢を排除し、防ぐことができる細菌である。バルネシエラの存在量は、いくつかの免疫調節細胞の量に対応している。結腸でのバルネシエラのレベルが高いほど、脾臓と肝臓でより多くの辺縁帯B細胞とインバリアントナチュラルキラーT細胞が数えられた。IL−10−/−マウスでの大腸炎の発症において、バルネシエラファイロタイプのより高いレベルは、疾患のより低い活動レベルと相関した。乳酸桿菌の存在量が減少する傾向が、hBD2による予防的処置後に結腸で観察された(p=0.1;図23)。マウスのHFDグループは広範囲の脂肪症(肝内脂肪蓄積)を発症したが、hBD−2で予防的に治療したHFDグループのマウスは、最小限の脂肪症を発症した(図60)。
マウスと食事療法
この実験は、食餌誘発性肥満マウスの微生物叢に対するhBD2とHD5の効果を明らかにする。介入の前に、13週間の慣らし期間として、マウスに非常に高いHFD(脂肪からのエネルギーが60%)を与えた。慣らし期間中に最低12グラムの体重増加(初期体重の約50%)の基準を満たすマウスのみを最終分析に含めた。これらの基準を満たさなかったマウスは、階層「保持者」としてそれぞれのケージにとどめた。それらを、全ての実験的試験に供したが、分析から除外した。
介入前に、全てのマウスをMRスキャンした。マウスのケージを、脂肪量に基づいて実験グループに割り当てた。その後の全ての測定は、介入前の同じマウスからのデータとペアにした。LFD(低脂肪食)の参照グループの実験を並行して行った。介入の対照として、2つの追加グループを含めた:1つの非常に高いHFDと1つのLFD。実験マウスを、介入の間非常に高いHFDにとどめた(図3)。マウスは、10週間、実験食であった。それらを、実験を通して、ケージあたり4匹のマウス、グループあたり3つのケージで共飼育した。全ての試験を3日間にわたり、1日1グループあたり1ケージで行った。

体重変化−hBD2
標準の高脂肪食(HFD)を摂取したグループは、全研究期間を通じて等しい食物摂取量を有し、最初の13週間は脂肪と除脂肪量が等しく同じ体重増加を示したため、食事介入前の開始点は同じであった。体重増加は、低脂肪食(LFD)を摂取したグループよりも有意に大きかった(*p<0.05、二元配置分散分析)。食事療法の介入後に、HFDグループの体重は増加し続けたが、HFDプラスhBD2グループは食事介入後の最初の4週間により少なく体重が増加する傾向があったが、有意ではなかった(*p=0.07、二元配置分散分析)。4週目〜研究期間の終わりにHFDプラスhBD2グループは、標準のHFDグループと同様の体重を獲得した(*p=0.82、二元配置分散分析)。
全てのHFD摂取グループは、研究期間中は同じ食物摂取量であり、13週間の慣らし期間中の体重増加は等しかった。食事介入後にHFDプラスHD5を摂取したグループは、HFD対照よりも有意に少なく体重が増加した(*p<0.05、二元配置分散分析)。加えて、HFDプラスHD5グループで脂肪率が減少する傾向が観察され、HFDプラスHD5においてHFD対照と比較して有意に低い脂肪率が、食事の変更後4週目に測定された(*p=0.009、二元配置分散分析)。
両方のディフェンシンは、細菌の存在と細菌の不在に大きな影響を与えることが示された(図25)。HD5は、結腸においてアロプレボテラの存在量を統計的に有意に増加させたが(p<0.02)(図26)、hBD2はアロプレボテラの存在量に影響を与えなかった。hBD2は、小腸と結腸の両方でビフィズス菌の相対的存在量を劇的かつ統計的に有意に増加させた(それぞれp<0.0001及びp<0.04;図27)。HD5が小腸でビフィズス菌の存在量を増やす傾向があった(図27)。微生物叢は、特に幹細胞移植直後に行われる劇的な細菌除染後のGVHDにおいて重要な役割を果たすようである。糞便移植による実験的治療は現在、世界中のいくつかのセンターで進行中であり、例えば、ドイツのレーゲンスブルクのホラー教授は、細菌の除染後に通常の微生物叢を再設置しようとしている。この実施例8は、ディフェンシンが、存在する種の数と細菌の総数の点で微生物叢の構成に対し大きな影響を及ぼし、従って腸内菌共生バランス失調の状態を整え、例えば、結腸T reg細胞の恒常性を維持するのに重要である、正常微生物叢を再設置できるようであることを実証する。
イエダニによって促進されるアレルギー性喘息のマウスモデルにおける経口ディフェンシンに対する鼻腔内(IN)ディフェンシンによる予防的処置の有効性を決定及び評価すること
材料及び方法
治療の投与計画:7〜10週齢の雌のBALB/cマウスを、研究開始の1日前に5つの研究グループに無作為に割り当て、イエダニ(200μLの生理食塩水中100μgのHDMプラス0.9%生理食塩水中のフロイント完全アジュバント)を皮下(SC)に感作させた。マウスを、hBD2の経口投与及び鼻腔内投与をそれぞれ1.2mg/kg/日(0.4mg/kg 1日3回)の用量で12日目の午前に開始し、約6時間間隔で1日3回を継続した。最後の用量を、14日目に、負荷の1時間前に投与した。総投与数は8回又は合計2mg/kgのhBD2であった。次に、マウスを、14日目にHDMで鼻腔内(IN)負荷した(50μLの生理食塩水中25μgのHDM)(図4)。
気道炎症:負荷の48時間後に、気管支肺胞洗浄を、冷PBSの3つの容量(0.4;0.3及び0.3mL、合計1mL)で肺を洗浄して行った。白血球数の合計と差を、自動血液分析装置Sysmex XT−2000iVで決定した。
生理食塩水負荷(非喘息)マウスと比較して、HDM負荷ビヒクル処置動物において、肺抵抗値の増加及び肺コンプライアンスの値の減少が観察された。両方のビヒクル処置(経口及び鼻腔内)マウスグループで炎症反応を、HDMによる感作の14日後に単回HDM負荷によって誘発した。それは、生理食塩水負荷対照と比較した場合、BALF中の総細胞数、好酸球、好中球、マクロファージ及びリンパ球数の統計的に有意な増加によって特徴付けられた(p<0.05)。また、肺組織ホモジネート中の7種のサイトカインTNF−α、IL−4、IL−5、IL−6、IL−9、IL−13及びIL−33の濃度の分析は、生理食塩水負荷対照と比較してHDM負荷動物での有意により高いレベルを明らかにした。

両方のビヒクル処置マウスグループ(経口及び鼻腔内)で炎症反応を、HDM及びアジュバントによる感作後14日目の単回HDM負荷によって誘発した。それは、生理食塩水負荷対照と比較して、肺組織ホモジネート中の5種のサイトカインIL−4、IL−5、IL−8、IL−9及びIL−13の濃度の統計的に有意な増加と、HDM負荷動物における肺組織の重度の組織学的炎症性変化によって特徴付けられた。
配列
幹細胞移植後の急性移植片対宿主病のマウスモデルにおける経口hBD−2による予防的処置の有効性を決定及び評価すること
材料及び方法
治療の投与計画:12匹のメスのBALB/cマウスに、0日目に少なくとも4時間の間隔で4.5Gy(2×498秒)を照射した。骨髄を採取した:後肢を2匹のメスのWT C57BL/6マウスから無菌的に採取した。骨格筋を摘出し、骨端を切除した。骨の内腔をPBSで洗い流し、細胞をペレット化して、取った。T細胞の採取:2匹のメスのWT C57BL/6マウスからの脾臓を、100μMのセルストレーナーを介してPBSで満たされた皿にふるい分けした。細胞をPBSに取り、50mLのファルコンチューブに移し、スピンダウンした。細胞を1mLのPBSに取り、脾臓あたり20μLのCD4マイクロビーズ+20μLのCD8マイクロビーズを加えて、4℃で20分間インキュベートした。細胞をPBSで洗浄し、CD4+細胞及びCD8+細胞をポジティブに選択するために、MACS分離に適用した。陽性選択されたT細胞を数え、取った。移植:第2回の照射の直後に、全てのWT BALB/cレシピエントマウスに、イソフルラン麻酔下で眼窩後静脈叢に5×1.000.000BM細胞(50μL)を静注した。15匹のBALB/cマウスを、0〜10日目に、強制経口投与により1日あたり100μLのPBS中1.2mgのhBD2/体重1kg/日で治療した。
マウスの体重を最初の7日間毎日測定し、生存を100日間監視した。
ビヒクルグループ中の15匹のマウス全てが35日までに死亡したのに対し、hBD2治療マウスのわずか4匹又は30%未満が35日までに死亡し、8匹のマウスは100日目まで生存していた(p<0.0001)(図47)。小腸、結腸及び肝臓の組織学的スコアは、PBSと比較して、hBD−2治療グループでは全て統計的に非常に低かった(図48)。hBD2治療マウスは、骨髄移植後の最初の7日間、統計的に有意に体重減少が少なく、腸の健康と腸の完全性が改善されたことを示唆する(図49)。hBD2での治療は、CD45+白血球の浸潤(図50のFACS分析);結腸の固有層における腸T細胞及び骨髄細胞の浸潤(図51a〜c)を低下させた。hBD2による予防的処置もまた、TNF−αとIL−6の濃度の低下及びIL−10の濃度の増加を示した(図52a〜c)。hBD−2治療は、骨髄細胞からのIL−1βの低下をさらに示した(図53a〜cの脾臓のFACS分析)。脾臓のFACS分析もまた、好中球の数の低下(図54a);特にTNF−α及びIFN−γのTh1サイトカイン生成の低下(図54b〜f)を示した。結腸試料のマイクロアレイ分析は、hBD−2治療グループ対PBSで炎症、白血球と骨髄細胞の遊走及び組織リモデリングの減少を示した(図55)。
この実施例13は、幹細胞移植の日から10日間のhBD−2による予防的処置が死亡率を低下させ、長期生存を劇的に増加させることを実証する。
幹細胞移植後の急性移植片対宿主病のマウスモデルにおけるシクロスポリンに対する経口hBD−2による予防的処置の有効性を決定及び評価すること

研究の間、マウスの体重を定期的に測定し、生存を90日間監視した。
PBS処置マウスの1匹を除く全てが90日目までに死亡したが、hBD−2治療動物の1匹だけが90日目までに死亡し(p=0.03、図56)、シクロスポリン治療動物の3匹が死亡した(p=0.16、図56)。hBD2及びシクロスポリン治療マウスは、PBS処置マウスと比較して、骨髄移植後に統計的に有意に体重減少が少なく、腸の健康と腸の完全性が改善されたことを示唆する(図57)。
この実施例14は、幹細胞移植の日から10日間のhBD−2による予防的処置が、シクロスポリンによる標準的な抗GVHD治療と少なくとも同等に死亡率を低下させることを実証するのに役立つ。

生存を60日間監視した。
PBS処置マウスの8匹中6匹は60日までに死亡したが、HD5で治療した動物の8匹のうち2匹は死亡し(p=0.06、図58)、hBD−2で治療した動物はどれも60日までに死亡しなかった(p=0.008、図58)。
この実施例15は、幹細胞移植の日から10日間のHD5又はhBD−2のいずれかによる予防的処置が死亡率を劇的に低下させることを実証するのに役立つ。
経口投与されたディフェンシンの抗菌効果(HD5及びhBD−2)
方法:
FISHによるムチンの免疫染色と細菌の局在
粘液免疫染色を、腸粘膜の表面での細菌の局在を分析するために以前に記載されているように、蛍光インサイツハイブリダイゼーション(FISH)とペアにした。簡単に説明すると、糞便物質を含まない結腸組織(近位結腸、盲腸から2cm)を、メタノール−カルノワ固定液(60%メタノール、30%クロロホルム、10%氷酢酸)中に室温で最低3時間置いた。組織を次いで、メタノールで2×30分、エタノールで2×15分、エタノール/キシレン(1:1)、15分、及びキシレンで2×15分洗浄した後、垂直方向にパラフィンに包埋した。5μmの切片を作成し、60℃で10分間予熱した後、キシレン60℃で10分間、キシレンで10分間、99.5%エタノールで10分間脱脂した。ハイブリダイゼーション工程を、ハイブリダイゼーション緩衝液(20mMのTris−HCl、pH7.4、0.9MのNaCl、0.1%のSDS、20%のホルムアミド)中10μg/mLの最終濃度に希釈したEUB338プローブ(アレクサ647を使用して5’標識した5’−GCTGCCTCCCGTAGGAGT−3’)により50℃で行った。洗浄緩衝液(20mMのTris−HCl、pH7.4、0.9MのNaCl)で10分間、PBSで3×10分間洗浄した後、PAPペン(Sigma−Aldrich)を使用して切片の周りに印をつけ、ブロッキング溶液(PBS中の5%ウシ胎児血清)を4℃で30分間加えた。ムチン−2一次抗体(ウサギH−300、Santa Cruz Biotechnology,Dallas,TX,USA)をブロッキング溶液で1:1500に希釈し、4℃で一晩適用した。PBSで3×10分間洗浄した後、1:1500に希釈した抗ウサギアレクサ488二次抗体を含むブロッキング溶液、1μg/mLのファロイジン−テトラメチルローダミンBイソチオシアネート(Sigma−Aldrich)及び10μg/mLのヘキスト33258(Sigma−Aldrich)を切片に2時間適用した。PBSで3×10分間洗浄した後、Prolong退色防止マウンティングメディア(Life Technologies,Carlsbad,CA,USA)を使用してスライドをマウントした。観察を、ソフトウェアZen 2011バージョン7.1を備えたZeiss LSM 700共焦点顕微鏡を用いて行った。このソフトウェアを使用して、細菌と上皮細胞単層の間の距離、及び粘液の厚さを決定した。
腸壁と細菌集団の間の溶解ゾーンに非常に統計的に有意な差が観察された。距離は、低脂肪と西洋食の両方を摂取したマウスについて小さかったのに対し、HD5、特にhBD−2は距離に大きな影響を及ぼした(図59)。
この実験は、マウスに経口投与したヒトHD5及びhBD−2が、経口投与後に期待される主に腸の内腔でではなく、マウス自体の上皮細胞によって生成されたかのように、マウスの上皮表面で微生物叢調節効果を発揮することを実証する。

Claims (33)
- 同種造血幹細胞移植を受けたか又は受けようとしている患者における急性移植片対宿主病の予防又は治療のための方法であって、前記患者へのβ−ディフェンシン及びα−ディフェンシンからなる群からの少なくとも1種の哺乳類ディフェンシンの投与を含む、方法。
- 前記ディフェンシンが、配列番号2、配列番号5、配列番号1、配列番号3、配列番号4、配列番号6、及び配列番号7並びに前記配列番号と5個未満、4個未満など、例えば3個未満、2個未満などのアミノ酸が異なる配列変異体から選択される、請求項1に記載の方法。
- 前記哺乳類ディフェンシンが、hBD2、HD5、HD6、hBD1、短縮型hBD2、hBD3及びhBD4からなる群から選択される、請求項1〜2のいずれか一項に記載の方法。
- 前記ディフェンシンが、hBD2、短縮型hBD2、又はHD5である、請求項13に記載の方法。
- 前記ディフェンシンが、組成物に含まれ、前記組成物が、2以上のディフェンシン、2種のディフェンシンなど、3種のディフェンシンなど、4種のディフェンシンなど、5種のディフェンシンなどを含む、請求項1〜4のいずれか一項に記載の方法。
- 前記2種のディフェンシンが、hBD2及びHD5である、請求項15に記載の方法。
- 前記患者への前記ディフェンシンの投与が、同種造血幹細胞移植の日に開始される、請求項1〜6のいずれかに記載の方法。
- 前記患者への前記ディフェンシンの投与が、同種造血幹細胞移植後にGVDHの症状が現れたときに開始される、請求項1〜6のいずれかに記載の方法。
- ディフェンシンの投与が、少なくとも1週間など、例えば少なくとも2週間、少なくとも3週間など、例えば少なくとも4週間、少なくとも2ヶ月など、例えば少なくとも3ヶ月、少なくとも4ヶ月、少なくとも6ヶ月の間、前記患者に同種造血幹細胞移植によって引き起こされる症状がなくなるまで継続される、請求項7又は8に記載の方法。
- 前記組成物が、医薬組成物である、請求項13〜16のいずれかに記載の方法。
- 前記哺乳類ディフェンシンが、細胞透過性ペプチド(CPP)、アルブミン結合部分(ABM)、検出可能部分(Z)、及び半減期延長ペプチドからなる群から選択される少なくとも1つの追加部分をさらに含む、請求項1〜10のいずれか一項に記載の方法。
- 前記追加部分が、半減期延長ペプチドである、請求項18に記載の方法。
- 前記半減期延長ペプチドが、新生児Fc受容体(FcRn)、トランスフェリン、アルブミン(HAS)、XTEN(登録商標)若しくはPEGに結合できる分子、ホモアミノ酸ポリマー(HAP)、プロリンアラニンセリンポリマー(PAS)、又はエラスチン様ペプチド(ELP)、ヒアルロン酸、絨毛性ゴナドトロピン(CG)β鎖のカルボキシ末端ペプチド(CTP)などの負に帯電した高度にシアル化されたペプチド、ヒトIgG、及びCH3(CH2)nCO−(nは8〜22である)からなる群から選択される、請求項18に記載の方法。
- 前記哺乳類ディフェンシンが、プレバイオティクス、プロバイオティクス、トリプトファン、短鎖脂肪酸、グルココルチコイド、シクロスポリン、抗TNF−α、インテグリン、抗生物質、免疫抑制剤、糞便移植、照射又はこれらの組み合わせと組み合わせて投与される、請求項1〜13のいずれか一項に記載の方法。
- 前記1日用量が、1日あたり0.1〜10mgのディフェンシン/kg、0.5〜5mgのディフェンシン/kgなど、1〜2mgのディフェンシン/kgなど、1.2mgのディフェンシン/kgなどである、請求項1〜14のいずれか一項に記載の方法。
- 前記ディフェンシンが、少なくとも1日1回、少なくとも1日2回など、少なくとも1日3回など、少なくとも1日4回など、少なくとも1日5回などで又は継続的に投与される、請求項1〜15のいずれかに記載の方法。
- 前記投与が、経口、口腔、舌下、経腸、経膣、気管内、肺内、鼻腔内、頭蓋内、皮下、静脈内、皮膚又は経皮投与である、請求項1〜16のいずれかに記載の方法。
- 前記投与が、経口、皮下、肺内又は皮膚/経皮投与である、請求項1〜17のいずれかに記載の方法。
- 前記肺内、気管内又は鼻腔内投与が、吸入器、ネブライザー、又は気化器による、請求項1〜18のいずれかに記載の方法。
- 前記患者が、重度の腸及び肝臓の炎症(例えば腹痛、吐き気、嘔吐及び黄疸によって特徴付けられる)並びに粘膜防御の喪失、サイトカインストーム、体重減少、敗血症、皮膚の発疹、かゆみ、及び発赤から選択される1以上の症状を有する、請求項1〜19のいずれかに記載の方法。
- 前記症状の1以上が、治療後に改善される、請求項20に記載の方法。
- 前記治療が、死亡率を減少させかつ長期生存を増加させる、請求項1〜21のいずれかに記載の方法。
- 前記治療が、免疫系のバランスを再調整しかつ前記組織サイトカイン生成の正常化を通じて前記サイトカインストームを予防する又は治療する、請求項1〜22のいずれかに記載の方法。
- 腸透過性及び敗血症が、前記腸における粘膜防御又はミエロペルオキシダーゼ活性の正常化を通じて治療される、請求項1〜23のいずれかに記載の方法。
- 呼吸器合併症、肺容量、肺炎症及び敗血症が、治療される、請求項1〜24のいずれかに記載の方法。
- 組織学的肺炎症、気管支周囲及び血管周囲の炎症並びに気管支肺胞洗浄液への炎症細胞の移動が、低下される、請求項1〜25のいずれかに記載の方法。
- 閉塞性細気管支炎及び強皮症などの、肺及び皮膚の炎症並びに線維症が、治療される又は予防される、請求項1〜26のいずれかに記載の方法。
- 遺伝子の豊富さ、門の数、細菌の存在、細菌存在量及び/又は短鎖脂肪酸の生成、及び/又は酪酸生成が、増加されかつ/又は前記患者の腸若しくは肺の微生物叢からの酢酸生成が、減少される、請求項1〜27のいずれかに記載の方法。
- 前記患者の腸若しくは肺若しくは皮膚における正常な微生物叢が、成熟され、維持され、又は安定化される、請求項1〜28のいずれかに記載の方法。
- 前記患者の腸又は肺におけるアロバキュラム、アロプレボテラ、アッカーマンシア、バルネシエラ、ビフィズス菌、フェカリバクテリウム、ラクノスピラ、ロチア及びベイロネラの存在量が、増加される、請求項1〜29のいずれかに記載の方法。
- 前記患者における食物摂取及び体重が、増加される、請求項1〜30のいずれかに記載の方法。
- 請求項1〜31のいずれかに記載の治療方法での使用のためのディフェンシンポリペプチド。
- 請求項1〜32のいずれかで定義される障害の治療のための医薬品の調製のためのディフェンシンポリペプチドの使用。
