本治療法のための候補者である対象には、RDH12をコードする遺伝子における変異によって引き起こされるLCAの診断を受ける対象が含まれる。RDH12をコードする遺伝子における変異によって引き起こされる他の眼科的に臨床的に規定された状態、例えば、早期発症網膜色素変性症に苦しむ対象も、本明細書に記載の方法を使用して治療することができる。RDH12をコードする遺伝子における変異によって引き起こされるLCAまたは別の眼科的状態の診断は、当該技術分野で公知の方法を使用して行うことができる。
以下に示す実施例に開示されている実験では、以下の材料および方法を使用した。
RDH12−/−マウスの生成と分析については以前に記載されている(Kurth et al.2007)。この研究で使用したRdh12−/−マウスを、機関の動物施設で飼育しているヌル欠損の雄と雌との間の兄妹交配から育成した。この研究で使用したWTマウスは、Jackson Laboratory(マサチューセッツ州ウィルミントン)のC57BL/6であった。
以前に記載されているように、RDH12コード領域全体を包含するように設計されたプライマーを使用してPCRによりヒトRDH12cDNAをヒト網膜cDNAから増幅し、クローン化し、シーケンシングして、忠実度を検証した(Janecke et al.2004)。AAVベクターを構築するために、RDH12cDNAを親pAAV−hGRK1−hrGFPベクターのマルチクローニングサイトに挿入した。得られたpAAV−hGRK1−Rdh12ベクターをAAVにパッケージングした。AAV2/5およびAAV2/8偽型ベクターは、二部トランスフェクション、すなわち、(1)目的の遺伝子をコードするAAVベクタープラスミドと、(2)血清型2からのAAVRepタンパク質、および血清型5または血清型8からのCapタンパク質をコードするAAVヘルパープラスミド、ならびに293T細胞へのアデノウイルスヘルパー機能と、によって生成された。トランスフェクションおよび精製は、公表されているプロトコルを使用して実行された(Nishiguchi et al.2015)。トランスフェクション後の2日で、凍結および解凍のサイクルを繰り返して細胞を溶解した。細胞残屑を最初に除去した後、ウイルス産生細胞の核酸成分をベンゾナーゼ処理により除去した。組換えAAVベクター粒子は、AVBマトリックスを使用するアフィニティクロマトグラフィーで精製し、1×PBSで洗浄し、Vivaspin4(10kDa)コンセントレータを使用して100〜150mlの容量に濃縮した。ベクターは、qPCR増幅により力価測定された。
約4週齢のマウスのコホートをケタミン(90mg/kg)/キシラジン(9mg/kg)の腹腔内注入により全身麻酔下に置いた。局所麻酔薬として、0.5%プロパラカイン溶液を角膜に塗布した。瞳孔がトロピカミド(0.5%)の局所塗布で拡張された。眼科手術用顕微鏡下で、30ゲージの針を使用して、角膜輪部に隣接する角膜に小さな切開を入れた。ハミルトン注入器に取り付けられた34ゲージの鈍針をレンズの後ろの切開部から挿入して、網膜に押し込んだ。すべての注入を、網膜の鼻側四半部内の位置で網膜下に行った。各眼には、最大2μLの容量で最大2×109vgAAV2/5−(hGRK1)−hRDH12が投与された。RDH12をコードするベクターを、治療を受ける各マウスの片方の眼に別々に投与し、反対側の眼には注入しなかった。注入後の眼底検査では、殆どの場合、網膜の30%以上が剥離していることがわかり、網膜下送達の成功が確認された。
使用した一次抗体は、マウスタンパク質に特異的なウサギアンチRdh12ポリクローナル抗体(CSP)(252SPFFKSTSQGAQ263、配列番号4に対して)、およびヒトタンパク質に特異的なマウスアンチRDH12モノクローナル抗体(2C9)(C−284DCKRTWVSPRARNNKT299、配列番号5に対して)(Kurth et al.2007)、マウスアンチRHOモノクローナル抗体(1D4)(Mackenzie et al.1984)、変性タンパク質に対して生成されたウサギアンチRHOポリクローナル抗体、およびウサギアンチ赤/緑錐体オプシンポリクローナル抗体(Millipore cat#AB5405)であった。
網膜ホモジネート中のタンパク質をSDS−PAGEで分離し、ニトロセルロース膜に転写し、その後ブロックし、一次抗体と共に一晩インキュベートし、洗浄し、アルカリホスファターゼ結合二次抗体と共にインキュベートし、5−ブロモ−4−クロロ−3’インドリルりん酸p−トルイジン塩およびニトロブルテトラゾリウムクロリドを使用して発現した。
マウスを安楽死させ、目を方向付けのために採点し、次に除核した。凍結切片の場合、レンズと眼球前区を取り除き、4%パラホルムアルデヒドで目を簡単に固定し、PBSで洗浄し、スクロース/OCTに移行させ、瞬間凍結し、厚さ10μmに切片化した。凍結置換の準備のために、目全体をドライアイスで冷却したイソペンタン中で30秒間瞬間凍結し、次に氷酢酸3%を含むドライアイスで冷却したメタノールに移した。眼を80℃で48時間、次に−20℃で一晩インキュベートし、パラフィンに包埋し、厚さ6μmに切片化した。次のように免疫標識化する前に、パラフィン切片を脱パラフィン化し、1mM EDTA、0.05%ツイン20、pH8.0で90℃において30分間インキュベートして抗原を回収した。簡潔に述べると、網膜断面をPBSで洗浄し、PBS−T(0.3%トリトンX−100)で透過化し、1%ウシ血清アルブミン、10%正常ヤギ血清、および0.3%トリトンX−100でブロックし、一次抗体と共に4℃で一晩インキュベートし、洗浄した後、フルオロフォア結合二次抗体と共に室温で1時間インキュベートした。4’,6−ジアミジノ−2−フェニルインドール(DAPI;インビトロジェン)を含むProLong Goldゲルマウントを使用して切片を封入処理し、Leica DM6000蛍光顕微鏡を使用して画像形成した。
ERGを、Espion e2記録システム(Diagnosys、マサチューセッツ州ローウェル)を使用して、以前に説明されているように(Thompson.2012)実行した。簡潔に述べると、マウスを一晩暗順応させ、ケタミン(93mg/kg)およびキシラジン(8mg/kg)の腹腔内注入で麻酔をかけた。瞳孔をトロピカミド(0.5%)の局所投与で拡張した。体温は、加熱パッドで37℃に維持した。角膜ERGは、0.5%テトラカイン局所麻酔および角膜水和のための一滴の2%メチルセルロースを含む金ワイヤループを使用して、両目から記録された。口内に置かれた金ワイヤループを基準として使用し、接地電極が尾にあった。ERGプロトコルは、短い白色フラッシュに対する暗順応(暗所)応答の記録で構成されていた(桿体分離B波の場合は、−2.31log cd.s.m−2、桿体錐体組み合わせA−およびB−波の場合は1.09log cd.s.m−2)。光順応(明所)ERGは、1.09log cd.s.m−2強度フラッシュ(錐体分離B波の場合)に応じて、白色の32cd.m−2桿体抑制バックグラウンドに対し10分間順応した後に記録された。応答は、1.25〜1000Hzで1,000ゲインで増幅され、2000Hzのレートでデジタル化された。ノッチフィルタを使用して、60Hzのラインノイズを除去した。応答はコンピュータで平均化され、刺激強度に応じて3〜60秒間隔で記録された。統計解析では、対応のあるt検定を使用して、治療した眼のERG振幅が未治療の眼と有意に異なるかどうかを判断した。
アルビノRdh12−/−マウスの片眼にAAV2/5−hGRK1p.hRDH12(1.3×109vg)、または等しい溶量のPBSを注入し、反対側の眼には注入しなかった。注入後6週間で、ERG分析を実行し、暗所応答を上記のように定量化した。1週間後、マウスを一晩暗順応させ、その瞳孔をトロピカミド(0.5%)で拡張し、個別の透明なトレイのライトボックス内に入れた。マウスを5,000ルクスに2時間曝した後、7日間飼育室に戻し(12時間暗/12時間明(<20ルクス))、その後ERG分析を繰り返した。光損傷後に残った元のERG応答の割合が各目に対して計算され、平均がプロットされ、標準誤差が示された。両側の対応のあるt検定を使用して、治療した眼のERG振幅が未治療の眼と有意に異なるかどうかを判定した。
マウスを麻酔し、瞳孔を0.5%トロピカミドで拡張した。ボリューム分析サイズが1.4×1.4mmのスペクトラルドメイン光干渉断層撮影(OCT)システム(Bioptigen Envisu R2200SD−OCT system(Durham,NC,USA))は、視神経頭を中心とした。Systane(Alcon)潤滑剤滴が、画像形成プロセス全体で使用された。
以前に記載された方法(Bligh and Dyer,1959)の修正を使用して、マウスの眼におけるオールトランス網膜および11−シス網膜を抽出した。注入後6週間のマウスを一晩暗順応させ、その後薄暗い赤色光の下で、CO2過剰摂取により安楽死させ、眼球を摘出し、液体N2で凍結した。薄赤色光の下で氷上で、各目を1mLのクロロホルム、すなわち、メタノール、ドロキシルアミン(2M)(3:6:1)で均質化し、室温で2分間インキュベートした。次に、200μLのクロロホルムおよび240μLの水を加え、各試料をボルテックスし、14,000rpmで5分間遠心分離した。下相を収集し、溶媒を窒素中で蒸発させ、試料をヘキサンに溶解した。キサン中の5%1,4−ジオキサンで発現されたSupelcosil LC−31カラム(25cm×4.6mm×3μm)を備えたWaters Alliance分離モジュールおよびフォトダイオードアレイ検出器を使用して、抽出物中のレチノイドを高速液体クロマトグラフィー(HPLC)分析によって識別および定量化した。標準化合物の保持時間との比較および最大波長の評価により、ピークの同定を行った。定量分析は、シン−およびアンチ−11−シスレチナールオキシムについて、それぞれ、347および351nm、ならびにシン−およびアンチ−オールトランスレチナールオキシムについて、それぞれ、357および361nmでのピーク面積を比較することによって行われた(Kurth et al.2007)。
マウス網膜ホモジネートを、注入後6週間で網膜還元酵素活性についてアッセイした。光に順応したマウスを安楽死させ、各網膜を、125μLの0.25Mスクロース、25mMTris−acetate、pH7、1mMジチオスレイトールで個別にホモジナイズした。ホモジネートを1000xgで5分間遠心分離して、破壊していない細胞を除去し、次に上清を氷上でマイクロチッププローブ(各1秒間に30回)で超音波処理した。タンパク質濃度は、ローリー法の修正(Peterson et al.1977)によって判定され、RDH12のレベルは、ウエスタンブロットによって評価された。同様の試料がプールされ、プールされた各ライセートの20μgが、HEPESバッファ(pH8)に200μMオールトランスレチナールおよび200μMNADPHを含むバッファで(3組で)アッセイされ、反応を37℃の水浴(レチノイド基質の熱異性化および酵素の不活性化を最小限に抑える反応温度)で0〜45分間インキュベートした。オールトランスレチノール形成は、既知の標準と比較して順相HPLC分析を使用して定量化された(Chrispell et al.2009)。
ヒトRDH12のAAVを介した発現
RDH12置換療法のための様々なベクターが発現され、試験された。最適なRDH12ベクター構築物を図2Aに示す。これは、ヒトロドプシンキナーゼ(GRK1)プロモーター断片の制御下でヒトRDH12cDNAを含む。構築物はAAV2/5血清型にパッケージングされる。AAV血清型5キャプシドは、視細胞の伝達を媒介することを示しているが、AAV8キャプシドと比較して反応速度は遅く、発現の堅牢性は低くなる(Yang et al.2002、Lotery et al.2003、Allocca et al.2007、Lebherz et al.2008)。AAV2/5−hGRK1p.hRDH12(1.3×109vg)の網膜下注入後6週間のマウス網膜におけるヒトRDH12タンパク質発現を、マウスRdh12またはヒトRDH12タンパク質に特異的な抗体を使用して評価した。ヒトRDH12のベクター送達レベルは、マウスRdh12の量にほぼ匹敵するように見える(図2B)。種特異的抗体を使用して、ネイティブマウスRdh12および組換えヒトRDH12を評価するために、網膜切片の間接免疫蛍光イメージングを使用した。内在性および組換えRDH12の局在は同一であるように見え、タンパク質が正常に処理されていることを示している(図2C)。

AAV血清型8に由来するキャプシドは、視細胞の効率的かつ強力な形質導入を媒介することが示された(Allocca et al.2007、Natkunarajah et al.2008、Vandenberghe et al.2011、Vandenberghe et al.2013)。最初の研究は、上記のベクター構築物を担持するAAV2/8血清型を用いて行われた。108〜109のウイルスゲノム(vg)の用量でAAV2/8−hGRK1p.hRDH12の網膜下注入によって処置したRDH12−/−マウスは、組換えヒトRDH12タンパク質の強い発現をもたらした。しかしながら、注入された眼は、注入後3週間という早い時期から始まり、殆どすべての場合に注入後6週間までに、重大な網膜損傷を発症した。マクロファージ浸潤は、CD68+細胞の存在によって大部分の場合に観察された(図9)。離乳時から屠殺時まで継続的にRdh12−/−マウスにシクロスポリン(Borel et al.1976)の全身投与で、網膜の損傷は軽減されず、ベクターまたは導入遺伝子に対する免疫応答が損傷の原因ではなかったことが示された。このビューは、有意な網膜の薄化およびマクロファージ浸潤も注入後6週間までに発生した、ヒトRDH12(AAV2/8−hGRK1p.hRDH12)またはマウスRdh12(AAV2/8−hGRK1p.mRDH12)のいずれかをコードする構築物を注入された野生型C57BL/6Jマウスで得られた結果と合致した。

Claims (22)
- レチノール脱水素酵素12(RDH12)タンパク質をコードする遺伝子における1つ以上の機能喪失変異により眼科的状態を有するヒト対象を治療する方法であって、前記対象の少なくとも片方の眼に核酸を含むアデノ随伴ウイルスベクターを投与することを含み、前記核酸が、ヒトRDH12DNAを含み、前記ヒトRDH12DNAが、配列番号2の全長と少なくとも70%、80%、90%、95%、または99%同一であるタンパク質をコードする、方法。
- 前記眼科的状態が、レーバー先天性黒内障(LCA)である、請求項1に記載の方法。
- 前記RDH12DNAが、ヒトロドプシンキナーゼ1(hGRK1)プロモーターの発現制御下にある、請求項1に記載の方法。
- 前記hGRK1プロモーターが、配列番号3を含む、請求項3に記載の方法。

- マイクロ注入カニューレが、前記網膜下腔内に挿入される、請求項8に記載の方法。
- ヒトRDH12DNAをコードする核酸であって、前記ヒトRDH12DNAが、配列番号2の全長と少なくとも70%、80%、90%、95%、または99%同一であるタンパク質をコードし、前記RDH12DNAが、ヒトロドプシンキナーゼ1(hGRK1)プロモーターの制御下にある、核酸。
- 前記hGRK1プロモーターが、配列番号3を含む、請求項10に記載の核酸。

- 前記ヒトRDH12DNAが、配列番号1の全長と少なくとも60%または70%同一である、請求項10に記載の核酸。
- レチノール脱水素酵素12(RDH12)タンパク質をコードする遺伝子における1つ以上の機能喪失変異により眼科的状態を有するヒト対象の治療に使用するための、請求項10〜13に記載の核酸。
- 前記眼科的状態が、レーバー先天性黒内障(LCA)である、請求項14に記載の核酸。
- 請求項10〜15のいずれか一項に記載の核酸を含むウイルスベクター。
- アデノ随伴ウイルスベクターである、請求項16に記載のウイルスベクター。
- 前記アデノ随伴ウイルスベクターが、AAV−2、血清型−5(AAV2/5)、またはAAV−5である、請求項17に記載のウイルスベクター。
- レチノール脱水素酵素12(RDH12)タンパク質をコードする遺伝子における1つ以上の機能喪失変異による眼科的状態を有するヒト対象の治療に使用するための、請求項16〜18のいずれか一項に記載のウイルスベクター。
- 前記眼科的状態が、レーバー先天性黒内障(LCA)である、請求項19に記載のウイルスベクター。
- 単離された宿主細胞であって、請求項16〜20のいずれか一項に記載のウイルスベクター、または請求項10〜15のいずれか一項に記載の核酸を含む、単離された宿主細胞。
- 前記細胞が、ヒトRDH12タンパク質を発現する、請求項21に記載の単離された宿主細胞。
