」として知られるプロセスである。これらの抑制性受容体は、制御されない免疫応答を防止するため、免疫チェックポイントとしてはたらく。モノクローナル抗体(mAb)によるこれらの免疫チェックポイントの1又はいくつかの遮断は、遮断しない場合には疲弊してしまう抗腫瘍T細胞をレスキューする(rescue:機能回復させる)ことが示され、最も重要なことには、癌患者では客観的な臨床奏功と関係している。
ラスミドによってコードされる遺伝子又はcDNAの全身及び局所の発現は、in vivo電気穿孔の適用により得ることができる。in vivo電気穿孔の使用は、腫瘍組織におけるプラスミドDNA取り込みを増強して腫瘍内での発現をもたらし、また筋組織へプラスミドを送達して、サイトカイン等の或る特定の免疫調節分子の全身性の発現をもたらす。

本明細書で使用される「免疫チェックポイント」分子は、T細胞機能不全又はアポトーシスを誘導する一群の免疫細胞表面受容体/リガンドを指す。これらの免疫抑制性の標的は、過剰な免疫応答を減じ、自己寛容を確実にする。腫瘍細胞は、これらのチェックポイント分子の抑制効果を利用する。免疫チェックポイント標的分子として、限定されないが、表1に記載されるチェックポイント標的が挙げられる。

本発明は、個体において腫瘍のサイズを減少させる若しくは癌細胞の増殖を阻害する、又は癌を患う個体において転移性癌の発症を減少させる若しくは阻害する、免疫療法的アプローチを提供する。治療法は、タンパク質治療薬の全身送達、又は電気穿孔を使用する、チェックポイント阻害剤の様々な可溶性形態をコードするプラスミドの腫瘍内送達のい
ずれかによって達成される。チェックポイント阻害剤療法は、免疫賦活性サイトカイン、例えばIL−12の電気穿孔による腫瘍内送達の前であってもよく、その間であってもよく、又はその後であってもよい。

チェックポイント分子のアンタゴニスト/阻害剤は、少なくともPD−L1の細胞外ドメイン(ECD)を含む可溶性PD−L1等の表1の中のチェックポイント阻害剤の可溶性結合パートナーであってもよい。他の可溶性チェックポイント阻害剤は、同様に、膜貫通ドメイン及び細胞内ドメインを欠くが、それらの結合パートナーに結合して、生物学的作用を誘発することができる。電気穿孔による腫瘍内送達のため、ECDは発現ベクターにコードされ、腫瘍に送達された場合に発現される。

少なくとも1つのチェックポイント阻害剤治療薬の全身送達、又はチェックポイント阻害剤をコードするプラスミドDNAの腫瘍内電気穿孔を、他の治療薬の実体、特に、少なくとも1つの免疫賦活性サイトカインと共に(例えば電気穿孔によるIL−12の腫瘍内の送達)適用することができることが意図される。併用療法の適用は、電気穿孔単独、又は電気穿孔と全身送達との組み合わせによって達成され得る。

装置は、癌又は他の非癌性の(良性)増殖に悩む患者における使用が意図される。これらの増殖は病変、ポリープ、新生物(例えば乳頭状尿路上皮新生物)、乳頭腫、悪性腫瘍、腫瘍(例えばクラッキン腫瘍、肝門部腫瘍、非侵襲性乳頭状尿路上皮腫瘍、胚細胞腫瘍、ユーイング腫瘍、アスキン腫瘍、原始神経外胚葉性腫瘍、ライディッヒ細胞腫瘍、ウィルムス腫瘍、セルトリ細胞腫瘍)、肉腫、癌(例えば扁平上皮癌、総排泄腔癌、腺癌、腺扁平上皮癌、胆管細胞癌、肝細胞癌、侵襲性乳頭状尿路上皮癌、扁平尿路上皮癌)、瘤、又は他の型の癌性若しくは非癌性増殖のいずれかとして現れ得る。本実施形態の装置及び方法で処理される腫瘍は非侵襲性、侵襲性、表在性、乳頭状、扁平、転移性、限局性、単
中心性、多中心性、低悪性度、及び高悪性度のいずれかであってもよい。
アニオン、ペプチド、油エマルジョン等の界面活性物質、並びにBCG(カルメット・ゲラン桿菌(bacille Calmette-Guerin))及びコリネバクテリウム・パルブム(Corynebacterium parvum)等の潜在的に有用なヒトアジュバントを含む、様々なアジュバントを使
用してもよい。代替的には、キーホールリンペットヘモシニアン、破傷風トキソイド、ジフテリアトキソイド、オブアルブミン、コレラ毒素又はそれらのフラグメント等の分子との組み合わせ及び/又はカップリングによって免疫応答を増強してもよい。
ンスフェクションとも名付けられた血漿電気穿孔によって増強されてもよい。簡潔には、マイクロ秒放電は、電極表面でキャビテーションマイクロバブルを生じる。磁界と併せてマイクロバブル崩壊によって生じた機械力は、従来の電気穿孔に関連する拡散媒介輸送と比較して、細胞膜を横切る輸送効率を増加させる役目を果たす。血漿電気穿孔の技術は、2011年4月12日付発行のVankov, et al.の米国特許第7,923,251号、及び2012年10月9日付発行のVankov, et al.の米国特許第8,283,171号に記載される。また、この技術を細胞の形質転換のためin vivoで採用してもよい。Chalberg, et al (2006) Investigative Ophthalmology & Visual Science 47:4083-4090;2012年1月24日付発行のChalberg, et al.の米国特許第8,101169号。
TNMシステム(例えば、Edge SB et al. (2010) "Melanoma of the skin", AJCC Cancer Staging Manual. 7th ed. New York, NY: Springer-Verlagを参照されたい)を、病
変の特性を判断し、その疾患のステージを決定するために使用する。
黒色腫試験における臨床成果は、固形癌効果判定(RECIST:Response Evaluation Criteria in Solid Tumors)基準によって測定されてもよい。RECISTは、腫瘍が改善する(「奏功」)、同じままである(「安定化」)、又は悪化する(「増悪」)場合を定義するガイドラインを提供する。
a.完全奏功(CR)は全ての標的病変の消滅である;
b.部分奏功(PR)は、標的病変のLD合計の少なくとも30%の減少を示す;
c.安定状態(SD)は、PRの資格を得るのに十分な収縮も、進行性疾患(PD)の資格を得るのに十分な増加もない場合に起こる;
d.進行性疾患(PD):標的病変のLD合計の少なくとも20%の増加、又は1以上の新たな病変の出現。
皮膚病変の最大数は規定されない;
全ての測定可能な皮膚病変が客観的な腫瘍評価に含まれる;
全ての測定可能な皮膚病変の合計を使用して、治療法に対する奏功を特性評価する;
合計増加が天底から30%未満である場合に許容される皮膚上の新たな病変。
最大10個の内臓病変が、評価用の指標病変として認められる;
新たな病変は、自動的に完全奏功(CR)を排除しない;
新たな病変は、一旦それらが測定可能であるとされれば、直径の合計に追加される;
修正irRC腫瘍量=SPD指標+SPDNew measurable。SPDは、各病変の直交径の合計である。
指標は、治療に先立って観察される10個の内臓病変を表し、New measurableは、治療中に出現する新たな病変を表す。
最初の3回の免疫化が目的の抗原をコードするプラスミドの電気穿孔、続いて2回のタンパク質ブーストによって与えられるプライム/ブースト法に従ってラマ(Abcore)で抗体を作製した。DNA免疫化によるプライミングはヒトCTLA−4の細胞外ドメインを発現するプラスミド250μgのラマへの皮内注入、続く電気穿孔、及び隣接する領域での該処置の反復によって1日目、14日目及び28日目に行った。その後、56日目及び84日目に、2mLの不完全フロイントアジュバント(MP Biologicals)で乳化した50μgのヒトCTLA−4タンパク質(Sino Biological)の皮下注入による最後2回のブ
ーストをラマに与えた。力価レベルをモニターするため、免疫化処置の期間中に試験出血を採取した。
末梢血リンパ球(PBL)を、密度勾配分離によって免疫化サイクルの終わりにラマから採取した全血から単離する。密度勾配媒質であるLymphoprep(Stemcell)との遠心分離の前に、2%FBSを補足したPBSと全血を50:50で混合する。PBLの層を単離し、トータルRNAをRNeasy maxiキット(Qiagen)を使用して、PBLから抽出する。First−Strand cDNA合成キット(GE Healthcare
)を使用してcDNAを合成する。
ネステッドPCR戦略を使用し、免疫化したラマのPBLから抽出したRNAより合成したcDNAを使用して、VHH遺伝子を単離する。フォワードプライマーMJ1、MJ2、MJ3、VHBACKA6及びリバースプライマーCH2FORTA4を使用して一次PCRを行って(例えば、Baral et al., (2013) Curr. Protoc. Immunol. 103:2.17.1-2.17.57、及びSabir et al., (2014) C. R. Biol., 337: 244-249を参照されたい)、VHH及びVHの両方の可変ドメインを増幅する。VHHフラグメント(約550bp〜650bp)をゲル電気泳動法によってVHフラグメント(約900bp)から分離し、ゲル抽出キット(Qiagen)を使用して該VHHフラグメントを精製する。フォワードプライマーMJ7、VHBACKA5、及びリバースプライマーMJ8、隣接して付くpADL−20c(Antibody Design Labs)ファージミドベクターに対して特異的なSfiI制限酵素部位を有するF4revを使用して、二次PCR反応を行った。最終PCR産物を、SfiI制限酵素部位を使用してファージミドpADL−20cベクターへライゲートする。その後、ライゲートしたプラスミドを使用して、ファージディスプレー用のエレクトロコンピテントTG1 E.コリ(E. coli)細胞を形質転換する。
回バイオパニングすることにより、ヒトCTLA−4に特異的なVHHを濃縮する。結合したファージを0.1Mのトリエチルアミンの添加によって溶出する。該溶液を1MのTris−HCLによってpH7.4に中和する。
0μlをヒトCTLA−4で予め被覆したマイクロタイタープレートに移す。37℃で2時間のインキュベーションの後、マイクロタイタープレートを、PBS中0.1%のTween−20(PBST)で3回洗浄した後、抗M13 HRP抗体(Abcam)を添加す
る。ヒトCTLA−4に結合したVHHファージをTMB(Pierce)の添加によって検出する。発色を4N H2SO4の添加によって停止し、吸光度をOD450nmに設定したSpectramax(Molecular Devices)で読み取る。
CTLA−4との相互作用は、固定化ヒトCTLA−4組換えタンパク質(Sino Biological)に対する抗ヒトCTLA−4抗体又は抗体フラグメントの結合の検出によって測
定される。組み換えヒトCTLA−4タンパク質を、4℃で一晩、高結合EIAプレート(Corning)上に被覆する。PBST(PBS中0.1%のTween−20)でウェル
を洗浄した後、プレートを室温で1時間、Super Block(Scytek)によってブロッキングする。プレートをPBSTで洗浄し、抗ヒトCTLA−4の試料をウェルに添加して、1時間インキュベートする。ウェルをPBSTで再び洗浄し、2次ヤギ抗ヒトIgG(H+L)(Jackson Immunoresearch)を添加する。1時間のインキュベーションの後、2次抗体(the secondary)をPBSTで洗い流し、プレートをTMB(Pierce)で
発色させた。発色を4N H2SO4で停止し、ヒトCTLA−4組み換えタンパク質に対する抗ヒトCTLA−4の結合親和性を測定するため、450nmに設定した分光光度計で光学密度を読み取る。
in vitroでの抗ヒトCTLA−4活性を、ヒトCTLA−4タンパク質とそのリガンドであるB7.1及びB7.2との相互作用を妨害する能力により測定する。組み換えヒトB7.1又はヒトB7.2(R&D Systems)を、4℃にて一晩、高結合EIAプ
レート(Corning)上に被覆させる。PBST(PBS中0.1%のTween−20)
でウェルを洗浄した後、プレートを室温で1時間、Super Block(Scytek)によってブロッキングする。タイトレーションした抗ヒトCTLA−4を、一定濃度の組み換えヒト抗CTLA−4と混合する。Super Blockをウェルから除去した後、プレートをPBSTで洗浄し、抗ヒトCTLA−4/ヒトCTLA−4混合物をウェルに添加して、室温で1時間インキュベートする。ウェルをPBSTで洗浄し、ビオチン化抗ヒトCTLA−4(R&D Systems)を試料ウェルに添加し、1時間インキュベートする。
プレートをPBSTで再び洗浄し、ストレプトアビジン−HRP(Abcam)をウェルに添
加する。1時間のインキュベーションの後、結合していないストレプトアビジン−HRPをPBSTで洗い流し、プレートをTMB(Pierce)で発色させる。発色を4N H2SO4の添加によって停止し、抗ヒトCTLA−4試験試料によるhB7.1又はhB7.2へのヒトCTLA−4結合の妨害を測定するため、450nmに設定した分光光度計で光学密度を読み取る。阻害曲線をプロットし、IC50をPrism(Graphpad)等のソフトウェアを使用して計算する。

抗ヒトCTLA−4活性は、Jurkat細胞においてB7誘導性IL−2分泌のCTLA−4の阻害を妨げる能力によって細胞系で測定される。Jurkat細胞(ATCC)を、10%FBS及び2mM L−グルタミンを補足したRPMI−1640(Corning)
中で培養する。細胞を採取し、アッセイ培地(5%FBS及び2mM L−グルタミンを補足したRPMI−1640)で生育させ、96ウェル組織培養処理プレート(Greiner Bio-One)のウェルに蒔く。その後、Jurkat細胞をPHA−L(Sigma)及び組み換
えヒトB7.1タンパク質(R&D Systems)を用いて共刺激する。一定濃度の組み換えヒ
トCTLA−4タンパク質(R&D Systems)を、タイトレーションした抗ヒトCTLA−
4と混合した後、該溶液を刺激したJurkat細胞に添加し、37℃、5%CO2の加湿したインキュベーターで2日間〜3日間インキュベートする。上清を採取し、IL−2デュオセット検出キット(R&D systems)を使用してIL−2レベルを測定する。測定し
たIL−2をグラフで示し、抗ヒトCTLA−4のEC50をPrismで計算する。
B16.F10マウスメラノーマ細胞(CRL6475;メリーランド州ロックヴィルのAmerican Type Culture Collection)を10%FCS及び0.2%ゲンタマイシンを補足したダルベッコ最小イーグル培地(DMEM)で維持する。細胞をトリプシン処理し、注入前に無菌PBSで洗浄する。C57BL/6マウス(メリーランド州ベテスダのNational Cancer Institute)の左脇腹を剃毛し、50mlの無菌PBS中、1×106細胞
を皮下注入する。チャレンジの際、マウスに5×105のB16.F10細胞を右脇腹に注入する。ディジタルカリパスを使用して腫瘍を測定し、腫瘍が注入の7日後〜10日後に直径3mm〜5mmに達したら、治療を開始する。マウスをAALACガイドラインに従って収容する。
チェックポイント阻害剤(例えばアンタゴニスト抗体又はそのフラグメント)及びIL−12をコードするDNAを含むpUMVC3プラスミド(ALDEVERON)を作製する。チ
ェックポイント阻害剤を含むpUMVC3を、エンドトキシンフリーのキットで作製する。全てのプラスミドDNAを無菌注射用生理食塩水(0.9%)で希釈し、−20℃で保存する。
マウスを97%の酸素及び3%イソフルランを使用して麻酔する。25ゲージ針を備えるツベルクリン注射器を使用して、無菌の生理食塩水中のプラスミドDNA 50μL(1.0μg/ml)を腫瘍に注入する。6本の直径1cmの貫通電極を備えるアプリケータを腫瘍に挿入する。最大6回のパルスを、適切なパルス発生器を使用して1500V/cm(99its、1Hz)で送達する。
マウスをCO2窒息によって人道的に屠殺する。腫瘍を切除し、10%ホルマリン10mlを含む50mlの円錐管に入れる。固定後、組織をH&Eで次のように染色する:10%の中性緩衝ホルマリン中で6時間固定した後、代表的な組織試料をMiles VIP組織プロセッサ(インディアナ州ミシャウォーカのMiles Inc.)を使用して、パラフィンブロックへと加工する。簡潔には、組織を上昇グレードのエタノールで脱水し、キシレンで透徹し、パラフィン(Tissue Prep 2;Fisher Scientific)中で浸透
させる。包埋の後、組織を標準的な回転ミクロトームで切片にして、4mmの切片を水浴から回収し、スライドガラスに置く。1つの腫瘍当たり3枚の切片を調べる。切片を熱乾燥し、標準的な組織学の技術を使用して、H&E(ミシガン州カラマズーのRichard-Allen Scientific)で染色する。

en)を使用して、CD4+リンパ球、CD8+リンパ球及び血管の存在について腫瘍を調べるため免疫組織化学染色を行う。マウスをCO2窒息によって人道的に屠殺する。腫瘍をハサミで切除し、皮膚を除去してドライアイス及びエタノールの混合物で直ちに80℃で凍結保存する。5mの凍結切片を得る。免疫組織化学分析のため、ラット抗マウスCD4、ラット抗マウスCD8(Ly2)又はラット抗マウスCD31(PECAM−1)を、1:50の希釈で組織切片に塗布して30分間インキュベートした後、2倍濃度のVector EliteラットIgGペルオキシダーゼキットで検出する(ビオチン化抗ラットIgG及びABC複合体において各15分間)。免疫染色をDako自動染色機で行う。切片を400倍の倍率で分析する。

本研究は、直後に電気穿孔が続くpIL−12の腫瘍内注入の薬力学的効果を評価した。患者を米国の1つの研究センターから集めた。生検のアクセスが可能な三種陰性乳癌(TNBC)の最低10名の患者をこの研究で治療した。全ての患者が単回の28日間の治療サイクルを受けた。スクリーニング中、研究者は、治療していないままの(すなわち、注入されておらず、電気穿孔を受けていない)1つの対照病変を選択した。非治療対照病変は、最長径が少なくとも0.5cmであり、生検のアクセスが可能であった。治療を1日目、5日目及び8日目に適用した。スケジューリングの制約を融通するため、各予定された治療に対して±2日の期間を認めた。以下に定義されるように各予定された病変に対し、治療を適用した。治療は、直後に電気穿孔が続くpIL−12の腫瘍内注入(複数の場合もある)からなった。治療サイクルの終わりに、患者は、総括的な安全性評価及び腫瘍生検サンプリングのため再度病院を訪れた(最後の研究訪問)。最後の研究(EOS)訪問は、いかなる新たな抗癌療法/治療計画も始める前に、及び最後のIT−pIL−12−EP治療の20日間以内(±2日)に行われた。
はVGXI USAによって製造され、利用可能なバッチは0.5mg/mLの濃度で2.0mLバイアルとして1.5mLの充填容量で供給される。未開封のpIL−12のバイアルを、薬局若しくは他の適切な安全な場所の安全で継続的に温度監視がなされる警報を備える冷蔵庫において−20℃±5℃で保存した。温度の規定外変動が起こると、pIL−12プラスミドを研究治療のために利用する前に、治験依頼者に通知された。試験薬剤の受取及び調剤を、治験実施施設で認可された人によって記録した。臨床供給は、プロトコルで述べられた以外のいかなる目的にも使用されなかった。
酔を得るために1%リドカインを病変の周囲に注入した。臨床上指定され、臨床上最適とされる代替的な局所麻酔薬を使用した。さらに、治療に先立って、又は治療中に必要に応じて、患者に鎮痛剤又は抗不安剤を与えた。不快を軽減するために病変に局所麻酔を直接注入した場合、プラスミドを注入する前に組織拡散を生じさせるため最低2分間おいた。大規模な疾患を有する患者に関する状況では、患者の不快をより良好に管理し最小限にするために、治療する研究者の判断で意識下鎮静を利用した。意識下鎮静を治験ガイドライン及び要件により行った。
IT−pIL12−EP治療がTILの増加を導くという仮説を検証するための腫瘍組織試料の詳細なフローサイトメトリー免疫表現型検査を行う。TILを治療前及び治療後の腫瘍試料から単離し、3種の免疫表現型検査抗体パネル(表2)で染色する。マルチレーザフローサイトメーターを使用して生存細胞を分析し、FLOWJO単一細胞分析ソフトウェア(FlowJo, LLC)を使用して分析する。
この第2相試験は、重度の黒色腫における腫瘍内IL−12プラスミド電気穿孔の安全性、抗腫瘍活性及び免疫学的効果を評価した。患者は、各90日の治療サイクルの1日目、5日目及び8日目に治療、すなわちアクセス可能な病変へのプラスミド注入/電気穿孔を受けた。各治療サイクルの後に奏功を評価した。安全性評価(臨床試験及び実験室での評価)を治療法の開始に先立って、及び一定間隔で、全ての治療した患者に対して行った。有害事象の重症度を、National Cancer Institute Common Terminology Criteria for Adverse Eventsのバージョン4.0に従って採点した。
サス州ザ・ウッドランズ)において、優良医薬品製造基準(GMP)の条件下で製造し、最終濃度2.1mg/mLで無菌のバイアルに供給し、−80℃で保存した。使用前に、プラスミドを4℃に解凍し、必要濃度である0.5mg/mLまで無菌の生理食塩水で希

患者は、最長5年間、増悪、粗生存率及び治療後療法について、研究停止による一定間隔でフォローを受けた。
上に記載される最初の第2相研究に対する追加プロトコルを開始して、より頻繁な治療計画で治療した患者におけるIT−IL12−EPの同様の安全性、抗腫瘍活性及び免疫学的効果を評価した。患者は、後のサイクルを含めて6週毎に1日目、8日目、及び15日目に治療を受けるか(治療計画A)、又は、最大で合計9治療サイクルの間6週毎に1日目、5日目及び8日目に治療を受けた(治療計画B)。研究集団を割り当てた治療計画に従って治療計画A、治療計画B、及び治療計画A/Bにグループ分けした。治療計画A/Bの患者は、治療計画Aを受ける研究を開始したが、その治療計画を研究に参加している間に治療計画Bに変更した患者を含んでいた。奏功を12週毎に評価し、安全性評価を治療法の開始に先立って、一定間隔で実施した。

これは、黒色腫を有する患者におけるペンブロリズマブと組み合わせた腫瘍内pIL−12 EPの多施設、第II相、非盲検、単一群試験である。評価可能な患者(n=42)は、腫瘍内pIL−12 EPの第1のサイクルの後にペンブロリズマブによる治療を開始する。ペンブロリズマブを、サイクル2から開始して、3週毎に200mgの一律の用量(flat dose)で投与する。患者がEPに対してアクセス可能な病変を有する限り、
腫瘍内pIL−12 EP(各サイクルとも1日目、5日目及び8日目の治療からなる)のサイクルを6週毎に行う。12週毎に奏功について患者を評価する。患者が、疾患評価の際に研究者による評価のもと定義される安定状態又は改善(better)を有する場合、患者は治療法を継続する。疾患が増悪するか又は毒性が許容できなくなるまで治療法を施す。研究者の評価により、RECIST v1.1又は修正RECISTを使用して、pIL−12 EP及びペンブロリズマブによる第1の治療の24週間以内の最良総合効果(Best overall response rate)(BORR)、すなわちCR+PRを決定し、同様の集団における単独療法としてのペンブロリズマブに対する既存効果(historical rates)と比較する。さらに、安全性及び認容性として定義される有害事象を、NCI CTCAEのバージョン4.0を使用して評価する。この試験で対処された他のエンドポイントは、CR又はPR、24週間の画期的な無増悪生存率(治療開始日から疾患増悪又は24における死亡のいずれかの最初の日までの期間によって定義されるPFS)、全生存期間(OS)及び最良総合効果(CR+PR)を経験する者に対する奏功期間(DOR)を含む。
キサス州ザ・ウッドランズ)において優良医薬品製造基準(GMP)の条件下で製造し、直接腫瘍内注入に続くin vivo EPに対してリン酸緩衝生理食塩水(PBS)中に配合する。プラスミドを、0.5mg/mLの濃度、1.5mLの充填容量で2.0mLバイアルとして供給する。
cur39への電気穿孔に最適な膜電位差を生じる。電気穿孔パルスは、6本の六角形に向かい合った針電極間で行われる。第1のパルスの後、向かい合った針電極対の間の極性を逆にし、針対に再びパルスが送られる。最初の一対のパルスの後、合計6回のパルスを送達して電気穿孔シーケンスを完了するまで、パルス送達を次の向かい合った針対まで時計回りに回転させた。同じ日に複数の腫瘍に注入される場合、各腫瘍のプラスミド注入直後にEPを行わなくてはならない。一旦腫瘍を完全に治療すると、次の腫瘍にプラスミドを注入し、直ちに電気穿孔を施すことができる。
サイクルで投与する。ASLは次の基準、(1)最長の直交径が少なくとも0.3cm×0.3cmであること、(2)電気穿孔の適用に適した位置にあることを満たすものとして定義される。被験者が複数のASLを有し得る場合、(1)患者の認容性、及び(2)20mLの最大一日投与量を超過しないことに留意して、最大数の病変を各サイクルで治療すべきである。pIL−12 EPの新たな治療サイクルの開始に先立って、研究者は、そのサイクル中の治療に対してASLを決定する。サイクルの各日(すなわち1日目、5日目及び8日目)において同じASLを治療すべきである。1日当たり患者一人当たりの最大のプラスミド注入量が20mLを超過しない限り、ベースラインに存在する以前に治療されて以前に同定された、治療されていない病変、及び研究期間中に出現する、ASLの定義に合致する新たな病変を治療してもよい。ASLが後のサイクルで存在しない場合、被験者は、pIL−12のそのサイクルを飛ばして、研究日程を継続してもよい。実行可能であれば、治療していない病変の生検を可能とするため、1つの病変をサイクル1の間、治療せずに維持する(6.1.2.3セクションを参照されたい)。
であるPD−L1及びPD−L2との間の相互作用を直接遮断するように設計されたIgG4/カッパアイソタイプの強力で非常に選択的なヒト化mAbである。この抗体は、各用量200mgで患者に静脈内投与される、100mg/4mLの注射用に溶液で供給される。ペンブロリズマブは、全ての処置/評価が完了した後のサイクルの1日目(±3日)に、全てのサイクルの開始時(IT−PIL−12のみのサイクル1の後)に投与されなければならない。ペンブロリズマブは、30分間の静脈内点滴として投与される(治療サイクルの間隔は、5.2セクションに記載されるように毒性により増加される場合がある)。
2面における直交径で正確に測定することができる新生物性腫瘤。その最長径及びその最長の直交線の両方が10mm以上でなくてはならないか、又は軸方向のスライス厚が2倍でなくてはならない。リンパ節は、15mm以上の短軸線長を有していなければならない。悪性リンパ節は、2つの直交径において測定可能でなければならない。その最長径及びその最長の直交線の両方が15mm以上でなくてはならないか、又は軸方向のスライス厚が2倍でなくてはならない。量的エンドポイントは、最長径とその最長の直交線との積
として定義される。
測定不可能な病変は、経時的な量的評価に適していないものである。これらは以下を含む:
な(ill defined)腹部腫瘤等を含む。
Claims (28)
- 癌性腫瘍を有する被験体を治療する方法であって、
a)有効量の、少なくとも1つの免疫賦活性サイトカインをコードする少なくとも1つのプラスミドを前記癌性腫瘍に注入することと、
b)前記腫瘍に対して電気穿孔療法を適用することと、
c)前記被験体に対して有効量のチェックポイント阻害剤を投与することと、
を含む、方法。 - 前記電気穿孔療法が、約100マイクロ秒〜約1ミリ秒の持続時間に亘る少なくとも1回の電圧パルスの適用を含む、請求項1に記載の方法。
- 前記腫瘍に送達された前記少なくとも1回の電圧パルスが約20V/cm〜約1500V/cmの電場強度を有する、請求項1に記載の方法。
- 前記チェックポイント阻害剤が全身投与される、請求項1に記載の方法。
- 前記チェックポイント阻害剤がプラスミド上にコードされ、電気穿孔療法によって前記癌性腫瘍に送達される、請求項1に記載の方法。
- 前記チェックポイント阻害剤が、前記免疫賦活性サイトカインをコードする前記プラスミド上にコードされ、電気穿孔療法によって前記癌性腫瘍に送達される、請求項1に記載の方法。
- 前記チェックポイント阻害剤が、表1の少なくとも1つのチェックポイント標的のアンタゴニストである、請求項1に記載の方法。
- 前記チェックポイント阻害剤が、ニボルマブ(ONO−4538/BMS−936558、MDX1106、OPDIVO)、ペンブロリズマブ(MK−3475、KEYTRUDA)、ピディリズマブ(CT−011)、及びMPDL328OA(ROCHE)からな

- 癌性腫瘍を有する被験体を治療する方法であって、
a)時間T1に第1の電気穿孔療法を適用することと、
ここで、前記第1の治療が、第1の有効量の、少なくとも1つの免疫賦活性サイトカインをコードする少なくとも1つのプラスミドを前記癌性腫瘍に注入することを更に含む;
b)時間T1に前記腫瘍に対して第1の電気穿孔療法を適用することと、
ここで、前記第1の電気穿孔療法が、約100マイクロ秒〜約20ミリ秒の持続時間を有する少なくとも1回の電圧パルスの適用を更に含む;
c)時間T2に第2の治療を適用することと、
ここで、時間T2は時間T1より後であり、
前記第2の治療が、第2の有効量の、少なくとも1つの免疫賦活性サイトカインをコードする少なくとも1つのプラスミドを前記癌性腫瘍に注入することを更に含む;
d)時間T2に前記腫瘍に対して第2の電気穿孔療法を適用することと、
ここで、前記第2の電気穿孔療法が、約100マイクロ秒〜約20ミリ秒の持続時間を有する少なくとも1回の電圧パルスの適用を更に含む;
e)時間T3に第3の治療を適用することと、
ここで、時間T3は時間T2より後であり、
前記第3の治療が、第3の有効量の、治療用タンパク質をコードするプラスミドを前記癌性腫瘍に注入することを更に含む;

g)時間T3の後にチェックポイント阻害剤を投与することと、

c)少なくとも1つのチェックポイント阻害剤、又は少なくとも1つのチェックポイント阻害剤と、電気穿孔による少なくとも1つの免疫賦活性サイトカインをコードする少なくとも1つのプラスミドの腫瘍内送達の少なくとも1回の追加のサイクルとの組み合わせの
前記患者への投与と、
を含む、治療計画。 - 前記チェックポイント阻害剤が、表1から選択される少なくとも1つのチェックポイント阻害剤である、請求項23に記載の方法。
