材料及び方法
患者試料
この研究には、マレーシアの4つの紹介病院(Penang General Hospital、ペナン;Tengku Ampuan Rahimah Hospital、セランゴール;University of Malaya Hospital、クアラルンプール;及びTung Shin Hospital、クアラルンプール)から、新たに診断されたHNSCC患者が登録された。本プロジェクトは、マラヤ大学(University of Malaya)歯学部の治験審査委員会[参考文献:DF OS0910/0049(L)]、及びマレーシア保健省(Ministry of Health、Malaysia)の医学研究及び倫理委員会によって承認された(NMRR−09−944−4848)。
口腔がん細胞株(ORL−48、ORL−115、ORL−136、ORL−150、ORL−153、ORL−156、ORL−166、ORL−174、ORL−188、ORL−195、ORL−196、ORL−204、ORL−207、ORL−214、ORL−215、及びORL−247)、鼻咽頭がん細胞株(NP69、NP640、HK1、TW01、C666.1、HeLa/S、及びHeLa/T)、乳がん細胞株(MCF−7及びSKBR−3)、結腸がん細胞株(HT−29及びHCT−116)、前立腺がん細胞株(PC−3)、肺がん細胞株(A549)、4つの正常口腔ケラチノサイト初代培養株(ORL−231、ORL−295、ORL−235、及びORL−232)、並びに2つの不死化鼻咽頭(NP)細胞株であるNP69及びNP460が本願発明者らによって使用された。HeLa/S及びHeLa/Tは、以前NPC細胞株と間違えられたHeLa細胞のバリアントであった(Ye et al. 2015)。これらの細胞株を購入し(Gibco、Thermo Fisher社、米国マサチューセッツ州)、2mMのL−グルタミン(Sigma Aldrich社、米国ミズーリ州)及び10%ウシ胎児血清(Gibco、Thermo Fisher社、米国マサチューセッツ州)を添加した推奨培地(口腔がん細胞株にはDMEM/F12、鼻咽頭がん細胞株、乳がん細胞株、結腸がん細胞株、及び肺がん細胞株にはRPMI−1640)で培養した。一方、ケラチノサイト−SFM(Gibco、Thermo Fisher社、米国マサチューセッツ州)を、正常な口腔ケラチノサイト及び非悪性鼻咽頭細胞株の培養に使用した。標的抗原であるORL195−MAGED4B(195Mと称する)及びHeLa/T−FJX1(HeLa/T−Fと称する)を過剰発現する細胞株を、ウエスタンブロット実験の陽性対照として使用した。FJX1を過剰発現するC666.1−A2細胞株(C666.1Fと称する)(イタリアがん研究センター(Centro di Riferimento Oncologico)のRicardo Dolcetti博士より提供)及びMAGED4Bを過剰発現するORL−195(195Mと称する)を、以下でさらに説明する細胞毒性ELISPOTアッセイの標的細胞株として使用した。本研究で使用される全ての細胞株は、本研究での使用に先立ち認証されている。
本実施例で使用されるペプチドは、FJX1(F1)及びMAGED4b(M6)に由来する2つのHLA−A2制限ペプチドで構成されている。HLA−A2制限FluM由来ペプチドを陽性対照として使用し、HIVペプチドを非関連ペプチド対照として使用した。上記ペプチドの配列は、表2〜表5に示すとおりである。本研究に使用された全てのペプチドは、JPT Peptide Technologies社(ドイツ、ベルリン)によって商業的に生産された。上記ペプチドの純度はHPLCを使用して決定及び保証され、ペプチドの純度結果は80%を超えていた。
定量的PCR(qPCR)を使用して、OSCC細胞株及びNPC細胞株における2つの標的抗原であるM6抗原及びF1抗原のmRNAレベルを決定した。RNA精製キット(Nucleospin RNA II Purification Kit)(Macherey−Nagel社、デューレン、ドイツ)を使用して、16個のOSCC細胞株及び7個NPC細胞株の全てから全RNAを抽出し、オリゴ(dT)プライマー及びSuperscript II(Invitrogen、Thermo Fisher社、米国マサチューセッツ州)を使用したcDNA合成に用いた。配列検出システム(ABI Prism7500(登録商標)Sequence Detection System)(Applied Biosystems社、ドイツ)を使用して、標準のSYBR Greenプロトコルで定量的PCRを実行した。グリセルアルデヒド−3−リン酸デヒドロゲナーゼ(GAPDH)を増幅し、内部対照として使用した。

1X HALTプロテアーゼ阻害剤カクテル(Pierce Biotechnology社、米国イリノイ州)を添加したRIPA溶解バッファー(5%NaDOC、1%SDS、25mMのHEPES、pH7.5、1MのHCl、1.5mMのMgCl2、1mMのEDTA、1%Triton X−100、1mMのDTT)で細胞を氷上で溶解した。4℃で20分間、10000gで遠心分離した後、細胞溶解物を収集した。総タンパク質濃度は、Bradfordタンパク質アッセイ(Pierce Biotechnology社、米国マサチューセッツ州)を使用して決定した。50μgの総タンパク質を12%及び10%SDS−ポリアクリルアミドゲル電気泳動(それぞれFJX1タンパク質及びMAGED4bタンパク質の検出用)にかけ、イモビロン(Immobilon)−Pメンブレン(Millipore社、米国マサチューセッツ州)に転写した。ブロットを5%スキムミルク(0.1%Tween−20を含むトリス緩衝生理食塩水(TBST))で1時間ブロックし、以下の一次抗体とさらにインキュベートした:抗MAGED4B(1:1000、Sigma Aldrich社、米国ミズーリ州)及び抗−FJX1(1:2000、Aviva Systems Biology社、米国カリフォルニア州)。ブロットを0.1%Tween−20(TBS−T)を含むTBSバッファーで洗浄(各5分間、3回)し、その後、ホースラディッシュペルオキシダーゼ(1:10000、Southern Biotech社、米国アラバマ州)と結合したそれぞれの二次抗体で1時間プローブした。洗浄(TBS−T)後、イメージングシステム(FluorChem HD2 Imaging System)(ProteinSimple社、米国カリフォルニア州)を使用して、高感度化学発光法により検出を行った。ブロットは、各実験のハウスキーピングコントロールとして、抗GAPDH抗体(1:1000、Trevigen社、米国メリーランド州)又は抗アクチン抗体(1:1000、Milipore社、米国カリフォルニア州)でプローブした。
本研究に含まれる、NPC(n=74)、OSCC(n=49)、及び179例の複数種のがん(乳がん:n=41、結腸:n=30、肺:n=48、前立腺:n=44、直腸:n=16)から構成される組織マイクロアレイ(TMA)スライドを、Biomax社(米国メリーランド州)から購入した。標的タンパク質の発現レベルは、Dakocytomation Envision+ Dual Link SystemHRP (DAB+)キット(Dako社、グロストルップ、デンマーク)を使用した検出システムに従って、抗MAGED4B抗体(1:100、カタログ番号HPA003554、Sigma Aldrich社、米国ミズーリ州)及び抗FJX1抗体(1:200、カタログ番号HPA059220、Sigma Aldrich社、米国ミズーリ州)を用いた免疫組織化学(IHC)により検出した。IHC分析は、PV1の有効性評価のために登録された患者由来のアーカイブされたFFPE OSCC組織切片及びNPC組織切片でも実施した。上記2つの抗体の免疫反応性は、陽性染色の腫瘍細胞の割合及び4点強度スコアリングシステムに基づいてスコア化した:0=陰性発現、1=弱陽性、2=中程度の陽性、3=強い陽性(Charafe−Jauffret et al.2004)。全てのIHC分析は、認定病理学者によって評価された。コアが欠落したか不完全な、又は評価するには腫瘍が少なすぎる組織マイクロアレイは、分析から除外した。
OSCC患者又はNPC患者からの血液35mLをCPT Vacutainer(バキュテナー)チューブ(Becton Dickinson社、米国)に採取し、製造元の説明書に従ってPBMCを分離した。次に、PBMCをハンクス平衡塩溶液(HBSS)(Gibco、Thermo Fisher社、米国マサチューセッツ州)で洗浄し、5%熱不活性化ヒトAB血清(Gemini Bio−Product社、米国カリフォルニア州)、1Xペニシリン/ストレプトマイシン(Gibco、Thermo Fisher社、米国マサチューセッツ州)、及び2mM L−グルタミンを添加したロズウェルパーク記念研究所(RPMI:Roswell Park Memorial Institute)培地(Gibco、Thermo Fisher社、米国マサチューセッツ州)を含む完全培地に再懸濁した。次いで、単離されたPBMCを、HLA−A2:Ig二量体アッセイ、並びにエクスビボ細胞傷害性ELISPOTアッセイの使用によるHLAタイピングに使用したが、これについては以下で詳細に説明する。
FJX1ペプチド及びMAGED4bペプチドの両方がHLA−A2特異的であることを考慮して、本発明者らは、一致するHLAサブタイプを有する血液試料のみを利用した。HLA−A2ステータスは、フィコエリトリン(PE)標識マウス抗ヒトHLA−A2抗体(クローンBB7.2、BD Pharmingen社、米国カリフォルニア州)を使用してPBMC試料を染色することにより決定した。PE標識マウス抗ヒトHLA IgG2b kアイソタイプ(クローンBB7.2、BD Pharmingen社、米国カリフォルニア州)及びマウス抗ヒトHLA−ABC(クローンDX17、BD Pharmingen社、米国カリフォルニア州)で染色したPBMCをそれぞれ陰性対照と陽性対照として使用した。染色用に、細胞を適切な抗体と共に4℃で1時間インキュベートし、洗浄し、血球計算器(FACSCanto 2 Cytometer)(BD Biosciences社、米国カリフォルニア州)で分析した。
HLA−A2陽性NPA患者及びOSCC患者のPBMCにおける内因性FJX1及びMAGED4b特異的CD8+T細胞の存在を、HLA−A2:Ig二量体アッセイを使用して評価した。1μgの可溶性組換え二量体であるヒトHLA−A2:Ig組換えタンパク質(BD Pharmingen社、米国カリフォルニア州)、0.25μgのβ2−ミクログロブリン(Sigma Aldrich社、米国ミズーリ州)、及び5μgのペプチド(F1、M6、又はPV1)を合わせてペプチド特異的二量体を調整し、37℃で一晩インキュベートした。完全培地(CM)及びβ2−ミクログロブリンを負荷した二量体をバックグラウンド対照として使用し、HIVペプチド及びβ2−ミクログロブリンを負荷した二量体を非関連ペプチド対照として使用し、β2−ミクログロブリンを有するFluM負荷二量体を陽性対照として使用した。翌日、新たに単離したPBMCを、別々の丸底ポリプロピレンチューブ(BD Falcon社、米国カリフォルニア州)の各ペプチド負荷二量体に加え、4℃で30分間インキュベートし、1mLのリン酸緩衝生理食塩水(PBS)で洗浄した。その後、細胞を100μLのPBSに再懸濁し、FITC結合ラット抗マウスIgG1(クローンA85−1、BD Biosciences社、米国カリフォルニア州)、PE結合マウス抗ヒトCD8(クローンHIT8a、BD Biosciences社、米国カリフォルニア州)、及びAPC結合マウス抗ヒトTCRα/β(クローンBW242/412、Miltenyi Biotech社、ドイツ)を用いて4℃で1時間染色した。次に、細胞を洗浄し、PBSに再懸濁し、血球計算器(FACSCanto 2 Cytometer)(BD Biosciences社、米国カリフォルニア州)を使用してフローサイトメトリー分析にかけた。ペプチド特異的CD8+T細胞の存在は、IgG1、CD8、及びAPCに対して陽性であると評価された細胞によって決定した。特定のペプチドに特異的なCD8+T細胞の割合を、ペプチド無しの染色で得られた値を差し引いた後の、ペプチド二量体陽性CD8+T細胞の値として計算した。
ELISPOTアッセイを使用して、単一細胞レベルでT細胞を分泌するサイトカイン(IFNγ及びグランザイムB)の存在を評価した。前述のように、エクスビボELISPOTを使用して、上記ペプチドを認識する患者PBMCからの固有T細胞を決定した(Lim et al.2014)。まず、PBMCを50ng/mLのIL−7及び10ng/mLのIL−12を含む培地で37℃で2時間インキュベートした。2時間のインキュベーション後、細胞を4mLのHBSS(Gibco、Thermo Fisher社、米国マサチューセッツ州)で洗浄し、200μLの完全培地に再懸濁した。次いで、懸濁した細胞を50μg/mLのF1ペプチド、M6ペプチド、PV1ペプチド、FluMペプチド、及びHIVペプチドと混合した。各混合物100μLを、抗ヒトIFNγ/グランザイムBでコーティングしたELISPOTプレートで一晩インキュベートした(Mabtech社、スウェーデン)。一晩インキュベートした後、ELISPOTアッセイを、若干修正した製造元の説明書に従って実施した。IFNγ検出抗体又はグランザイムB検出抗体をそれぞれのプレートに添加し、37℃で2時間インキュベートした。これに続いて、洗浄及びストレプトアビジン標識二次抗体の添加を行い、プレートを37℃で45分間さらにインキュベートした。両方の抗体を、0.5%ヒトAB血清(Gemini Bio−Products社、米国カリフォルニア州)を含むPBSで1:500に希釈した。基質(BCIP/NBT−plus substrate)を加えて、サイトカイン分泌T細胞によって形成されたスポットを可視化した。検出されたスポットは、アナライザー(CTL ELISPOT Analyzer)(Cellular Technology Limited社、米国オハイオ州)を使用して定量し、ソフトウェア(ImmunoSpot Professional Software)(Cellular Technology Limited社、米国オハイオ州)を使用して分析した。ペプチドに曝露されていないバックグラウンド対照ウェルで形成されたスポットを差し引くことで、ペプチド特異的CTLを計算し、FluMペプチド及びHIVペプチドを、それぞれ陽性及び非関連ペプチド対照として使用した。
患者由来の樹状細胞を分離し、抗原提示細胞として使用した。簡潔には、PBMCをマクロファージ無血清培地(M−SFM)(Gibco、Thermo Fisher社、米国マサチューセッツ州)に再懸濁し、6ウェルプレートに播種し、37℃で1時間インキュベートした。インキュベーション後、PBMCから非接着細胞を回収し、HBSSでリンスし、IL−7を10ng/mL含む完全培地に再懸濁した。ウェルあたり約2〜5x105個の細胞を、丸底96ウェルプレートに播種した。その後、T細胞増殖のために、50μg/mLのペプチドを添加して37℃で2〜5日間細胞をインキュベートした。インキュベーションから生じた接着細胞を、樹状細胞(DC)の分化を誘導するために、100ng/mLのGM−CSF及び25ng/mLのIL−4の存在下で、M−SFMで2〜5日間37℃で培養した。細胞を培地を用いて除去することにより、分化したDCを培養皿から回収し、1.5mL遠心管に分注し、50μg/mLのペプチド(F1、M6、及びPV1)と共に37℃で2時間インキュベートした後、非接着PBMC由来の成熟T細胞に付与した。18〜24時間の共培養後、10ng/mLのIL−2をDC/T細胞培養に添加し、さらに24時間インキュベートした。インキュベーション後、IL−2を含む培地を新鮮な完全培地に交換し、さらに37℃で3〜5日間インキュベートして、細胞傷害性Tリンパ球(CTL)を活性化した。その後、FJX1又はMAGED4b発現標的細胞に対する潜在的な殺傷能力を決定するために、CTLを増殖後細胞毒性ELISPOTアッセイで標的細胞(195M及びC666.1F)と共にインキュベートした。
細胞毒性ELISPOTアッセイを使用して、それぞれ標的抗原を過剰発現している標的細胞株に対する、患者由来のペプチドパルスCTLの応答を研究した。CTLを、エフェクター対標的比20:1で195M及びC666.1Fと共培養した。この培養物を20ng/mLのIL−7及びIL−2を添加した200μLの完全培地に再懸濁し、次に、IFNγ又はグランザイムBのELISPOTプレートの各ウェルに100μLを付与した。次に、細胞懸濁液を37℃で16時間さらにインキュベートした。その後、ELISPOTアッセイを上記と同様に実施した。
患者の人口統計
本研究で使用するために、41人のHNSCC(NPC及びOSCC)患者から末梢血を採取した。これらのうち、約50%(20/41人)がHLA−A2陽性であり、その後の分析に使用した。これらの患者の中で、16/20人はNPC患者であり、4/20人はOSCC患者であった。これらのHLA−A2陽性患者の80%(16/20人)が進行性の病期(ステージIII及びステージIV)にあった。これらの患者の人口統計を表8に示す。20人のHLA−A2患者のうち、15人の患者からの試料を二量体アッセイにかけ、16人の患者からの試料を、エクスビボ細胞傷害性ELISPOTアッセイにかけた。残りの試料は、PBMCが不十分であったか、試料の品質が最適ではなかった(血液溶解)ため、実験から除外した。
本発明者らは、MAGED4b及びFJX1がそれぞれOSCC及びNPCで過剰発現しているのに対し、それぞれの正常な対応物では発現が無視できる程度であることを既に示した(Chong et al.2012、Chong et al.2009、Ahmad Zabidi MM 2011、Bose et al.2009)。本研究では、HNSCC試料の拡張セットでこれら2つの抗原の過剰発現をスクリーニングした。得られたデータは、FJX1がNPCとOSCCの両方の細胞株で過剰発現していることを示した。図9(a)に見られるように、それぞれ正常な口腔ケラチノサイト及び正常な鼻咽頭組織と比較した場合、mRNAレベルで2倍以上の増加が5/16株のOSCC(ORL−150、ORL−153、ORL−156、ORL−174、及びORL−48)、6/7株のNPC(HK1、TW01、TW04、及びC666.1)、及び不死化NP(NP69及びNP460)で検出された。特に、全てのOSCC(ORL−136を除く)細胞株及びNP/NPC細胞株は、正常な口腔ケラチノサイト[図10(c)]と比較して、タンパク質レベルで顕著なレベルのFJX1を発現することが観察された[図10(a)]。同様に、MAGED4Bの場合、mRNAレベルはOSCC細胞株ORL−153、不死化NP69、及びNPC細胞株C666.1で2倍を超えることが判明した[図9(b)]。注目すべきことに、全てのOSCC細胞株(ORL−195及びORL−48を除く)及び全てのNPC細胞株は、陽性対照と比較してタンパク質レベルでMAGED4Bのわずかな発現を示した[MAGED4Bを異所的に過剰発現する細胞、図10(a)及び図10(b)]。正常な口腔ケラチノサイトの初代培養では、MAGED4Bの発現は検出されなかった[図10(c)]。また、本発明者らは、TCGAデータベース(Cancer Genome Atlas N.2015)の正常データと一致する43の頭頸部がんのRNAseqデータを調査し、腫瘍コホートの60%超でFJX1の有意な上昇レベルが認められ[図11(a)、p<0.01]、一致した正常データと比較した場合、コホートの約40%でMAGED4Bのレベルが上昇していた[図11(b)、p<0.01]。
MAGED4BとFJX1の両方がHNSCC患者に存在することが示されたため、MAGED4B及び/又はFJX1特異的CD8+T細胞も存在すると推定される。本発明者らは、PV1ペプチド(MAGED4Bペプチド及びFJX1ペプチド)を認識可能なCD8+T細胞(CD8+、TCR+、IgG1+)の割合を二量体アッセイを使用して決定した。本発明者らが、分析した全てHNSCC試料(15/15)におけるPV1特異的CD8+T細胞の存在を実証したところ、平均は非ペプチド負荷二量体対照で正規化した後に2.6%〜7.1%の範囲内であった(図2)。検出された平均内因性PV1特異的T細胞集団は、単一のF1(2.6%、p<0.001)又はM6特異的T細胞(4.6%、p=0.177)集団と比較して高く(7.1%)、両方のペプチドを組み合わせて使用する場合に、より強い免疫原性が得られることを示した。非負荷二量体対照と比較した場合、HIVペプチド負荷二量体を認識するT細胞集団間に有意差は観察されなかった(p=0.125)。T細胞集団は陽性対照を認識しているが、FluMペプチド負荷二量体は、非負荷二量体対照と比較して有意に高かった(p<0.001)。
次に、本発明者らは、PV1ペプチドの使用により細胞傷害性T細胞のレベルを増加させることが可能かどうか判定を試みた。HNSCC患者(n=16、HLA−A2:Ig二量体アッセイで使用される試料と重複する15個の試料)における細胞傷害活性の誘導におけるPV1の有効性を決定するため、本発明者らは、M6、F1、又はPV1(F1+M6)のいずれかで、インビトロで患者のT細胞を刺激し、このT細胞をMAGED4B(195M)又はFJX1(C666.1F)を発現する標的細胞に曝露した。刺激の前に、エクスビボELISPOTアッセイにより、HLA−A2:Ig二量体アッセイでペプチド特異的T細胞が検出されたが、刺激前には、IFNγ及びグランザイムBサイトカイン分泌細胞(CSC)の集団は少なかった[図3(a)及び図3(b)]。図3(a)及び図3(b)に見られるように、M6、F1、又はPV1のいずれかでのインビトロ刺激後、細胞傷害性ELISPOT(CTX ELISPOT)アッセイにより、細胞溶解活性が増加し、エクスビボELISPOTでのサイトカイン分泌細胞集団と比較した場合、サイトカイン分泌細胞数がIFNγとグランザイムBの両方で増加したことが示された。全体として、M6ペプチド、F1ペプチド、又はPV1ペプチドに対するHNSCC患者T細胞の反応は、以前のペプチド刺激と比較した場合、細胞傷害性ELISPOTアッセイでサイトカイン分泌の2.4倍〜39.7倍の増加を示している。ただし、HIVペプチドは刺激後のIFNγ及びグランザイムB分泌のレベルの増加も引き起こしている。
ELISPOTアッセイで使用された16の患者試料のうち、発明者らは12/16人の患者からアーカイブされたFFPE腫瘍塊を取得できた。次に、IHCを実施して、これらの腫瘍におけるMAGED4B及びFJX1の発現を測定した。次に、発明者らは、IHC染色強度に基づいて患者を4つの群(低FJX1、高FJX1、低MAGED4B、及び高MAGED4B)に分け[図4(a)]、ペプチドによる刺激後の細胞毒性ELISPOTアッセイにおける患者のT細胞応答を比較した。染色強度0は陰性、強度1は低発現、強度2〜3は高発現と分類される。FJX1又はMAGED4Bの発現が高い患者は、FJX1又はMAGED4Bの発現が低い患者と比較して、サイトカイン分泌細胞(IFNγ又はグランザイムBのいずれか)の数が多いことが示された[図4(b)]。また、本発明者らは、組織上でFJX1及びMAGED4Bの両方の発現レベルが低い患者は、腫瘍上でFJX1又はMAGED4Bのいずれかを高レベルで発現する患者と比較して、PV1刺激に対する反応が低いことを見出した[図4(c)]。このデータは、高レベルのFJX1又はMAGED4Bを発現しているHNSCC患者の免疫系がPV1ペプチドワクチンで刺激されやすいことを示唆している。
他のがんにおけるFJX1及びMAGED4Bの発現を調査し、PV1療法の恩恵を受ける可能性のある患者コホートを特定するために、本発明者らは5つの最も一般的ながん(乳がん、肺がん、大腸がん、前立腺がん、及び直腸がん)を含むTMAのセットを使用してMAGED4B及びFJX1の発現を調査した。その結果、MAGED4Bの発現は5つのがんにおける全ての患者の80%超で検出され、同様の観察がFJX1発現で見られたが、大腸がん患者については約50%のみがFJX1陽性であったことが判明した。MAGED4B及びFJX1の両方の発現をまとめて分析したところ、本発明者は、乳房、結腸、肺、前立腺、及び直腸の腫瘍組織の90%超がMAGED4B又はFJX1の発現を有することを示した[表7(a)]。全体として、MAGED4B及びFJX1は、病期に関係なく、乳がん、結腸がん、肺がん、前立腺がん、及び直腸がんの腫瘍で過剰発現することが示された[表7(b)]。これら5種類のがんの細胞株(A549、肺がん;PC3、前立腺がん;HT−29、結腸直腸がん;HCT−116、結腸直腸がん;SKBR−3、乳がん;MCF−7、乳がん)のウエスタンブロット分析の結果、MAGED4Bの発現はA549細胞株及びPC3細胞株で高く、発現レベルはHCT−116、SKBR−3、及びMCF−7で弱いことが示された。一方、FJX1は、PC3を除く6つの細胞株のうち5つ(A549、HT−29、HCT−116、SKBR−3、及びMCF−7)で発現することが示され(図12)、PV1は、MAGED4B及び/又はFJX1を発現するこれらのがんを標的にする可能性があることが示唆された。
多くの腫瘍は、精巣などの免疫特権部位(Inaoka et al.2011、Gjerstorff et al.2015)及び胚発生時(Lawrence et al.2011、Becker et al.2012)にのみ通常発現するタンパク質の調節解除された発現を有する傾向がある。これらの抗原の特有な発現により、抗原ががん治療の良い標的となる。本発明者らの以前の研究では、そのような過程において2つの腫瘍抗原であるFJX1及びMAGED4Bを同定した。これらは正常細胞では無視可能なレベルであるがNPC及びOSCCではそれぞれ過剰発現する(Chai et al. 2015、Chong et al.2012)。本研究では、本発明者らは、コホートをより多くの細胞株及び腫瘍試料に費やした。IHCデータは、ほとんどのOSCC腫瘍及びNPC腫瘍がMAGED4B又はFJX1のいずれかの過剰発現を有することを実証した。FJX1はHNSCC細胞株及び腫瘍組織の両方で過剰発現することが示されている。しかし、MAGED4Bについては、IHC染色が多数のHNSCC腫瘍組織で陽性染色を示したにもかかわらず、検出されたMAGED4Bの発現レベルはHNSCCがん細胞株で低かった。これは、長期培養環境での抗原の損失によるものである可能性があった。さらなる相関研究により、MAGED4Bの発現は腫瘍分化の程度と相関していることが明らかになったが、本発明者らは上記2つの抗原の発現と患者の病期分類との間に有意な相関を見出せなかった。OSCC及びNPCが頭頸部がん(HNSCC)のほとんどの症例を占めたため、本発明者らはMAGED4BとFJX1の両方がHNSCCの重要な標的になり得るという仮説を立てた。要約すると、本発明者らは、これらの2つの抗原がHNSCC患者の〜90%で過剰発現し、HNSCC患者の良好な治療標的として機能することを確認した。これに加えて、本発明者らは、MAGED4B及びFJX1が、乳がん、結腸がん、肺がん、前立腺がん、及び直腸がんを含む他のがん種の腫瘍に存在することも明らかにし、この事はこれらの2つの抗原を標的とすることは、HNSCC患者に恩恵をもたらすだけでなく、乳がん、結腸がん、肺がん、前立腺がん、及び直腸がんを患う患者にも利益をもたらす可能性があることを示した。
動物モデルでのPV1の評価
PV1は望ましくない炎症反応を増加させない
動物モデルを使用してPV1免疫原性を評価する前に、炎症反応を測定するGriessアッセイを使用して、M6及びF1ペプチドの毒性を評価した。マウスマクロファージ細胞株RAW264.7を、様々な濃度のM6ペプチド、F1ペプチド、又はPV1ペプチド(25μg/mL、50μg/mL、及び100μg/mL)で処理し、10%DMSO含有PBSを陰性対照として使用し、1ng/mLのリポ多糖類(LPS)を陽性対照として使用した。炎症の程度は、RAW264.7細胞によって産生される誘導型一酸化窒素合成酵素(iNOS)のレベルに基づいて間接的に測定した。
MAGED4Bペプチド6(M6)及びFJX1ペプチド1(F1)で構成される二重抗原PV1は、HNSCC患者から抽出した末梢血単核細胞(PBMC)を使用した試験により、免疫原性があることが示された。本研究は、トランスジェニックマウスモデルB6.Cg−Tg(HLA−A/H2−D)2Enge/Jを使用してPV1の免疫原性と有効性を評価することを目的としており、HLA−A2提示抗原に対するヒトT細胞免疫応答のモデリングを可能にする。
ワクチン接種期間中、動物の体重を週に2回測定した。本発明者らは、全ての治療群において、4週間の免疫期間中に体重減少又は有害事象を観察しなかった[図7(a)]。
以前の免疫原性研究で、本発明者らは、PV1が500μgという低濃度で抗原特異的T細胞応答を誘導できることを示した。したがって、マウスの全身腫瘍組織量の減少におけるPV1の有効性を決定するために、1000μgのPV1を後続のアッセイで使用した。

Claims (17)
- Four−jointed Box 1(FJX1)ペプチドと、メラノーマ抗原ファミリーD4b(MAGED4b)ペプチドと、を少なくとも含み、対象における抗がん免疫応答を誘導するための主要組織適合性複合体(MHC)クラスI分子と結合することを特徴とするペプチド組成物。
- 前記MHCクラスI分子が、ヒト白血球抗原A2(HLA−A2)分子である、請求項1に記載のペプチド組成物。
- 前記FJX1ペプチドが、配列番号1のペプチド配列を含む、請求項1に記載のペプチド組成物。
- 前記MAGED4bペプチドが、配列番号2の配列を含む、請求項1に記載のペプチド組成物。
- 前記ペプチド配列中の少なくとも1つのアミノ酸が欠失している、請求項3又は請求項4に記載のペプチド組成物。
- 前記ペプチド配列中の少なくとも1つのアミノ酸が置換されている、請求項3又は請求項4に記載のペプチド組成物。
- 前記ペプチド配列中に少なくとも1つのアミノ酸が挿入されている、請求項3又は請求項4に記載のペプチド組成物。
- 前記ペプチド組成物が、前記MHCクラスI分子と結合して、前記対象における前記抗がん応答を誘導するために、前記対象のサイトカイン分泌細胞を増加させる、請求項1に記載のペプチド組成物。
- 前記サイトカインが、インターフェロンガンマ(IFNγ)及びグランザイムB(Granzyme B)を含む、請求項8に記載のペプチド組成物。
- 前記抗がん免疫応答が、FJX1及び/又はMAGED4bががん細胞によって発現される場合に対象において誘導される、請求項1〜請求項9のいずれか一項に記載のペプチド組成物。
- 前記がん細胞が、頭頸部扁平上皮がん(HNSCC)細胞、乳がん細胞、結腸がん細胞、直腸がん細胞、肺がん細胞、及び前立腺がん細胞のうちいずれか1つであるか、又は2つ以上の組み合わせである、請求項10に記載のペプチド組成物。
- 請求項1〜請求項11のいずれか一項に記載のFJX1ペプチド及びMAGED4bペプチドを少なくとも含むワクチン。
- 前記ワクチンが、がん治療法と組み合わせて使用され得る、請求項12に記載のワクチン。
- 免疫チェックポイント阻害剤を含む、請求項13に記載のワクチン。
- 前記免疫チェックポイント阻害剤が、抗PD−1抗体を含む、請求項14に記載のワクチン。
- 前記抗がん免疫応答が、頭頸部扁平上皮がん(HNSCC)、乳がん、結腸がん、直腸がん、肺がん、及び前立腺がんのうちいずれか1つ又は2つ以上の組み合わせに由来するがん細胞が前記対象において検出される場合に、誘導される、請求項13に記載のワクチン。
- 治療を必要とする対象における、頭頸部扁平上皮がん(HNSCC)、乳がん、大腸がん、直腸がん、肺がん、又は前立腺がんのいずれか1つ又はその組み合わせを治療するための薬剤の製造における有効量のFour−jointed Box 1(FJX1)ペプチド及びメラノーマ抗原ファミリーD4b(MAGED4b)の使用であって、前記薬剤は主要組織適合性複合体(MHC)クラスI分子と結合して、前記対象において抗がん免疫応答を誘導することが可能である、前記使用。
