(非常に特異的なペプチドを用いる、PETでの迅速な腫瘍PD-L1検出)
1.1背景:腫瘍微小環境(TME)におけるPD-L1の増加は、能動免疫浸潤物によって発現されるプログラム細胞死タンパク質1(PD-1)受容体への結合を介した免疫浸潤物の不活性化による免疫抑制を引き起こす(Okazakiら、2007、およびTopalianら、2015)。腫瘍細胞上およびTME中のPD-L1発現は、患者の層別化および治療的モニタリングのための潜在的なバイオマーカーと考えられている(Herbstら、2014)。PD-L1 IHCに基づく補足的な診断試験が、米国食品医薬品局によって近年承認され、PD-L1がin vivoでのイメージングに適切な標的であり得ることを示唆している(Roachら、2016)。
1.2.1:WL12は、PD-1と同様の様式でPD-L1に結合する。WL12のPD-L1への結合様式を評価するために、PD-1の代わりにWL12をドッキングするための、PD-1に結合したヒトPD-L1の共結晶構造(PDB ID:4ZQK)(Zakら、2015)を使用した。大環状分子WL12の構造的複雑性を考慮し、本発明者らは、最初に立体配座検索を行い、Glideを用いてPD-L1上のPD-1結合部位に配座異性体をドッキングした(Friesnerら、2004;Halgrenら、2004)。WL12は、2つの大環状鎖の骨格間に2つの水素結合を持つベータシート様構造を形成する(図1B)。この立体配座は、円偏光二色性実験によって支持されている(図2)。PD-1の構造を、結合されたWL12と重ね合わせると、これら2つの間の結合モードの類似性が明らかになる。PD-L1との結合界面を形成するPD-1の2本のベータ鎖は、WL12の偽鎖と重なる(図1C)。WL12のL-ロイシンは、PD-1のIle134と同じ小さい疎水性ポケットに挿入され、2つのノルロイシン残基のうちの1つは、PD-1のIle126と整列する。これらの疎水性相互作用に加えて、多数の水素結合がWL12とPD-L1との間に存在する。WL12上のアスパラギンのカルボキサミドはTyr123と水素結合を形成し、グリシンアミドはGly120の骨格と水素結合を形成し、セリンヒドロキシルはGln66と相互作用する。オルニチン残基は露出しており、PD-L1との結合には関与しない。任意の1つの特定の理論に縛られることを望むものではないが、これは、アミンカップリング法による適切な標識のコンジュゲーションが、WL12のPD-L1への結合を妨害しないことを示唆する。
1.4.1材料:PD-L1結合ペプチド、WL12は、>95%の純度でCPC Scientific(カリフォルニア州サニーベール)によってカスタム合成された。別段に指定されない限り、他の全ての化学物質を、Sigma-AldrichまたはFisher Scientificから購入した。2,2´,2´´-(10-(2,6-ジオキソテトラヒドロ- 2H-ピラン-3-イル)-1,4,7,10-テトラアザシクロドデカン-1,4,7-トリイル)三酢酸(DOTAGA無水物)および[64Cu]Cl2を、それぞれCheMatech Macrocycle Design Technologies(カタログ番号C109;ディジョン、フランス)およびウィスコンシン大学から購入した。特に指定しない限り、全ての細胞培養関連試薬をInvitrogenから購入した。ポリクローナル抗ヒトIgG-Eu3+クリプテート(カタログ番号61HFCKLA)およびXL665コンジュゲートマウスモノクローナル抗6ヒスチジン抗体(カタログ番号61HISXLA)を、Cisbio Assays(ベッドフォード、マサチューセッツ州)から購入した。組み換えヒトPD-1 Fcキメラタンパク質(カタログ番号1086-PD-050)および組み換えヒトPD-L1(B7-H1)-His-タグタンパク質(カタログ番号9049-B7)を、R&D systems(ミネアポリス、ミネソタ州)から得た。
(PD-L1指向性PETからPD-L1標的薬開発)
2.1概要:がん免疫療法(CIT)は、さまざまな悪性腫瘍で持続的な反応をもたらすことによって患者の生存率を高めている。しかしながら、免疫チェックポイント標的療法で治療された患者のほぼ70%が単剤療法に反応しない(Lipsonら、2015;Topalianら、2015)。精密免疫療法に対する応答の決定要因を同定するという、満たされていない要求がある。チェックポイント併用療法は生存期間を延ばすが、多くの場合、免疫関連有害事象(irAE)の増加を犠牲にし、併用戦略に関する知識の増加が毒性を軽減するために必要であることを示唆している(Marroneら、2016)。免疫チェックポイント治療のための新しいバイオマーカーと、その幅広さおよび耐久性を高めてirAEを減らすそれらの組み合わせとを特定するための集中的研究が大いに求められている。したがって、本開示の主題の一態様は、PD-L1標的治療薬の開発および評価においてPD-L1ベースのPETイメージングを使用するための戦略を開発することである。進行期患者において侵襲性で非実用的である血漿または組織(生検)ベースのバイオマーカーに頼る現在の戦略とは異なり、本発明は、PD-L1標的治療薬(抗体、ペプチド、小分子)の用量対占有関係を、関連するin vivoモデルの腫瘍においてPD-L1 PETイメージングを使用して確立する。
(2.1.3.1論拠)
PD-L1およびPD-1を標的とする治療用抗体は、PD-L1陽性腫瘍を有する患者のごく一部で非常に優れた有効性を示している。現在使用されている用量では、レスポンダーおよび非レスポンダー集団は、PBMCにおける約65%のPD-L1占有率を示しているが、PBMCにおけるPD-L1占有率と腫瘍における占有率との関係は、動的であり、よくわかっていない(Brahmerら、2012)。また、腫瘍モデルにおける試験は、いくつかの腫瘍において、PD-L1抗体が腫瘍血管系に限定されることを見出した(Dengら、2016)。放射性標識AtzMabを用いた予備的結果は、NSCLC異種移植片におけるこれらの発見を総括した(Chatterjeeら、2016)。まとめると、これらの所見は、腫瘍におけるPD-L1占有率およびその用量依存性、ならびに腫瘍における抗PD-L1抗体の滞留時間の理解を向上させることが、より良い情報に基づくPD-L1指向治療に必要であることを示唆する。任意の1つの特定の理論に縛られることを望むものではないが、PD-L1 PETは、抗PD-L1抗体(またはペプチドおよび小分子)のそのようなPK測定を標的結合および滞留時間に関して評価するための貴重なツールを提供すると考えられる。また、PET情報に基づく投与は、PD-L1 PETによって定量化することができ、腫瘍のPD-L1発現および免疫細胞浸潤における治療誘導性変化と相関させることができる腫瘍内の免疫プロファイルの変化をもたらすと考えられる。
(セラノスティック抗体によるPD-L1エンゲージメントの非侵襲的定量化)
3.1概要:プログラム死リガンド-1(PD-L1)を標的とした抗体治療薬は、免疫チェックポイント阻害剤を含む臨床試験のほぼ4分の1に採用されている。総PD-L1レベル、それらのPD-L1治療薬による占有率、ならびに最適な免疫応答を確実にするための腫瘍内のターゲットエンゲージメントの程度と持続時間とに対する投与の関連性は、未知のままである。腫瘍内のPD-L1の占有率は、PD-L1の発現の動的変化、ならびに血漿および腫瘍抗体濃度を変化させる腫瘍の内因性および外因性パラメータによって影響を受ける可能性がある。しかしながら、そのような重要な変動は、末梢薬物動態学的および薬力学的評価によっては捉えられていない。PD-L1治療薬の用量-薬物ばく露関係におけるギャップに対処するために、PD-L1発現の動的変化の定量化を可能にする放射性標識PD-L1結合ペプチドを調べた。構造分析は、ペプチドおよび治療用モノクローナル抗体(mAb)のPD-L1との相互作用における重なりを示し、腫瘍中の治療用mAbの占有率を陽電子放出断層撮影(PET)を用いて測定することを可能にした。複数の異種移植片モデルにおいて、PETイメージングおよび生体内分布試験は、変動するPD-L1発現、およびPD-L1治療用抗体によるその飽和が定量化され得ることを示した。さらに、3つの異なる抗体による腫瘍におけるPD-L1占有率を測定し、腫瘍におけるPD-L1占有率に対する用量および時間の影響を定量した。ペプチドベースのPD-L1 PETは、免疫関連有害事象を減らすことを目的として、用量および治療計画を最適化するためのツールとして有望である。
3.3.1:WL12およびPD-L1 mAbとのPD-L1相互作用の構造分析およびin vitro検証。WL12は、PD-L1:PD-1相互作用を高親和性(IC50:20nM)で阻害する14アミノ酸ペプチドである(Chatterjeeら、2017)。初期の分子モデリング解析は、重要な分子相互作用に寄与する、PD-L1:WL12およびPD-L1:PD-1の、PD-L1の4つのアミノ酸(Y56、E58、D61およびA113)での相互作用面における重なりを示唆した(Chatterjeeら、2017)。治療用抗体アテゾリズマブ(AtzMab)と複合体を形成しているPD-L1の埋没面(2,106Å2)は、PD-1のそれ(1,970Å2)よりも大きい(Leeら、2017)。任意の1つの特定の理論に縛られることを望むものではないが、PD-L1上のWL12相互作用面も臨床的に入手可能な治療用mAbのそれと重なると考えられ、その理由はPD-L1:PD-1相互作用を阻害するようにそれらが同様に設計されているためである。それを試験するために、WL12の予測される結合立体配座をPD-L1 mAbのそれと比較した。全てのmAb、ならびにPD-1およびWL12でのAA接触の重なりは、PD-L1残基Y56、E58、A113、M115およびY123からなる共通の結合ドメインを明らかにする。PD-L1分子表面の視覚化(図33A、図34A)において明らかにされているように、重複領域(シアン)は深いポケットを形成し、全ての相互作用点に対するアンカー点として作用する。表面積に関して、AtzMab(赤)は、PD-1(紫色)、WL12(緑)、アベルマブ(AveMab、オレンジ)、およびデュルバルマブ(DurMab、青)からの相互作用表面と重なり合う共通結合コアの全ての側面上の残基との分子接触を生成する抗体からのループを持つPD-L1表面とより相互作用する。
免疫チェックポイント治療薬は何百もの臨床試験で試験されており、そのうちの約25%がPD-L1を標的としている。PD-L1治療を受けている患者のわずか30%のみが治療に反応するため、反応の分子的および細胞的根拠ならびにこれらの療法に対する耐性は、転写、遺伝、およびエピジェネティック試験を用いて調査されている。腫瘍における有効性に関連する薬物蓄積および標的飽和に対する用量の関連性は知られていない。加えて、大きなサイズの抗体治療薬は腫瘍浸透を制限し、作用部位での薬力学的評価に特有の課題を提起する。腫瘍内因性および腫瘍外因性の両方のパラメータを考慮し、腫瘍においてリアルタイムでPD-L1飽和/占有データを提供し、広く適用することができる有効な方法は不足していた。この知識の欠如は、用量選択、用量最適化、治療開発、および毒性を低減するための治療最適化を妨げる。本発明者らの現在の研究では、放射性標識PD-L1結合ペプチドは、可変で動的なPD-L1発現レベルを非侵襲的に検出することができ、腫瘍内因性パラメータ(PD-L1発現、再利用、間質圧)および外因性パラメータ(抗体アイソタイプ、動態、ATA、異化)を考慮しながら、腫瘍の占有率を測定するために使用でき、したがって、腫瘍におけるPD-L1:PD-1相互作用阻害PD-L1抗体の治療活性をモニターするための普遍的な手段を提供することが示された。

- プログラム死リガンド1(PD-L1)に対して結合特異性を有するペプチドとレポーティング部位とのコンジュゲート、および任意にリンカーを含む造影剤であって、前記リンカーが、存在する場合には、前記ペプチドと前記レポーティング部位とを連結し、前記リンカーが存在しない場合には、前記レポーティング部位が前記ペプチドのアミノ酸の第一級アミンを介して前記ペプチドに直接結合している、造影剤。
- PD-L1に対して結合特異性を有する前記ペプチドが、PD-L1のアミノ酸Y56、E58、A113、M115、およびY123と相互作用する、請求項1に記載の造影剤。
- プログラム死リガンド1(PD-L1)に対して結合特異性を有する前記ペプチドが、ペプチドWL12、DK-A-221、またはDK-A-222に対して少なくとも80%の配列同一性を有する、請求項1または2に記載の造影剤。

- 前記1つもしくは複数の細胞または組織の前記造影剤との接触が、被験体において行われる、請求項33に記載のイメージング方法。
- 前記被験体が、ヒト、ラット、マウス、ネコ、イヌ、ウマ、ヒツジ、ウシ、サル、トリ、または両生類である、請求項34に記載のイメージング方法。
- 前記PD-L1の検出が、前記被験体への前記造影剤の投与後約60〜120分以内に起こる、請求項32に記載のイメージング方法。
- 前記イメージング方法が、がんを検出するために使用される、請求項32に記載のイメージング方法。
- 前記がんが、以下からなる群より選択される、請求項37に記載の造影剤:芽腫、癌腫、神経膠腫、白血病、リンパ腫、黒色腫、骨髄腫、肉腫、頭部がん、頸部がん、頭頸部がん、肺がん、乳がん、トリプルネガティブ乳がん、前立腺がん、大腸がん、食道がん、胃がん、白血病/リンパ腫、子宮がん、皮膚がん、内分泌がん、泌尿器がん、膵臓がん、消化管がん、卵巣がん、子宮頸がん、腎がん、膀胱がん、脳腫瘍、腺腫、および転移性がん。
- 前記イメージング方法が固形腫瘍を検出するために使用される、請求項32に記載のイメージング方法。
- 前記固形腫瘍が、脳、結腸、乳房、前立腺、肝臓、腎臓、肺、食道、頭頸部、卵巣、子宮頸部、胃、直腸、膀胱、子宮、精巣、および膵臓からなる群より選択される臓器にある、請求項39に記載のイメージング方法。
- 前記イメージング方法が感染症を検出するために使用される、請求項32に記載のイメージング方法。
- 前記感染症が微生物感染症である、請求項41に記載のイメージング方法。
- 前記微生物感染症が、ヒト型結核菌、大腸菌、クレブシエラ属、エンテロバクター属、プロテウス属、セラチア・マルセッセンス、緑膿菌、黄色ブドウ球菌およびコアグラーゼ陰性ブドウ球菌を含む黄色ブドウ球菌属、エンテロコッカス属、肺炎連鎖球菌、インフルエンザ菌、バクテロイデス属、アシネトバクター属、ヘリコバクター属、カンジダ属、メチシリン耐性黄色ブドウ球菌(MRSA)およびバンコマイシン耐性エンテロコッカス・フェシウム(VRE)からなる群より選択される1つまたは複数の微生物による感染症からなる群より選択される、請求項42に記載のイメージング方法。
- 前記イメージング方法が炎症を検出するために使用される、請求項32に記載のイメージング方法。
- 前記炎症が、喘息、自己免疫疾患、自己炎症性疾患、セリアック病、憩室炎、糸球体腎炎、化膿性汗腺炎、過敏症、炎症性腸疾患、間質性膀胱炎、中耳炎、骨盤内炎症性疾患、再かん流傷害、リウマチ熱、関節リウマチ、サルコイドーシス、移植拒絶反応、狼瘡、全身性エリテマトーデス、および血管炎からなる群より選択される障害に関連する、請求項44に記載の造影剤。
- 前記炎症が、関節リウマチまたは全身性エリテマトーデスによって引き起こされる、請求項45に記載のイメージング方法。
- 前記イメージング方法が、腫瘍中の1つまたは複数の免疫細胞を検出するために使用される、請求項32に記載のイメージング方法。
- 前記イメージング方法が、腫瘍内または被験体内の免疫細胞の全身分布を検出するために使用される、請求項32に記載のイメージング方法。
- 前記イメージング方法が、感染症に対する免疫細胞応答を検出するために使用される、請求項32に記載のイメージング方法。
- 前記イメージング方法が、炎症性疾患に対する免疫細胞の腫瘍内応答または正常組織内応答を検出するために使用される、請求項32に記載のイメージング方法。
- 前記イメージング方法が、前記被験体におけるPD-L1発現レベルを検出する、請求項32に記載のイメージング方法。
- 前記イメージング方法が、抗体またはペプチドまたは低分子量剤により、前記被験体の腫瘍部位または正常組織におけるPD-L1の占有またはターゲットエンゲージメントを測定する、請求項32に記載のイメージング方法。
- 請求項1〜27のいずれか一項に記載の造影剤を含む、プログラム死リガンド1(PD-L1)を検出するためのキット。
