Claims (21)
- 軟骨病変の修復及び再生のための軟骨修復インプラントであって、

前記第1の方向に直交する第2の方向に配向される横糸ファイバと、
前記第1及び第2の方向に直交する第3の方向に配向される拘束ファイバと、を含み、
前記横糸ファイバ及び前記拘束ファイバは、前記面内縦糸ファイバ間に配置される、請求項1に記載の軟骨修復インプラント。 - 前記3次元足場材は、3次元縦糸インターロックファブリックである、請求項1に記載の軟骨修復インプラント。
- 前記織成ファイバの少なくとも3つの層は、
第1の方向に配向される面内縦糸ファイバを含む縦糸層と、
前記第1の方向に直交する第2の方向に配向される横糸ファイバを含む横糸層と、

前記少なくとも1つの無機マトリックスは、ハイドロキシアパタイト、燐酸カルシウム、炭酸カルシウム、アルミナ、ジルコニア、イットリア安定化ジルコニア、窒化珪素ベースの材料、生体活性ガラス、及び/又は、ガラスセラミックス、から構成される群から選択される、請求項1に記載の軟骨修復インプラント。 - 前記3次元足場材の少なくとも一部分は、生体材料ゲルで少なくとも部分的に充填され、
前記生体材料ゲルは、コラーゲン、ヒアルロン酸、アルギナート、アガロース、キトサン、ゼラチン、ラミニン、フィブロネクチン、完全に生物材料を、完全に合成物質を、若しくは、それらの混合物を、包含する相互侵入網目構造、及び/若しくは、フィブリン又はそれらの組合せ、から構成される群から選択される、請求項1に記載の軟骨修復インプラント。 - 前記織成ファイバの少なくとも一部分は、軟骨及び/又は骨の誘導のために、前記インプラントの内部で、トランスフェクト細胞又は形質導入細胞に適合された、ウイルス、プラスミド、又は、DNA、で被覆される、請求項1に記載の軟骨修復インプラント。
- 前記3次元足場材及び前記3次元多孔性基材の少なくとも一方の内部に埋め込まれた少なくとも1つの細胞を更に含み、
前記少なくとも1つの細胞は、1次細胞、未分化前駆細胞、幹細胞、iPS細胞、及び、それらの組合せ、から構成される群から選択され、
前記未分化前駆細胞及び幹細胞は、脂肪組織に由来する幹細胞又は前駆細胞、骨髄、滑膜、筋肉、骨、臍帯血、骨膜、及び、それらの組合せ、から構成される群から選択され、
前記1次細胞は、軟骨細胞、骨芽細胞、線維芽細胞、フィブロ軟骨細胞、及び、それらの組合せ、から構成される群から選択される、請求項1に記載の軟骨修復インプラント。 - 前記織成ファイバ及び前記3次元多孔性基材の個々は、生体適合性材料で形成され、
前記生体適合性材料は、吸収性材料、非吸収性材料、及び、それらの組合せ、から構成される群から選択され、
前記非吸収性材料は、ポリテトラフルオロエチレン(PTFE)、延伸ポリテトラフルオロエチレン(ePTFE)、ポリアミド、ナイロン、ポリスルホン、セルロース、アクリル、ポリビニルアルコール、炭素、セラミック、金属、アクリル、ポリカーボネート、ポリエステル、ポリエーテル、ポリ(エーテルケトン)、ポリ(エーテルエーテルケトン)、ポリ(エチレンテレフタル酸塩)、ポリ(メチル(メタ)アクリラート)、ポリオレフィン、ポリスルホン、ポリウレタン、から構成される群から選択され、
前記吸収性材料は、ポリグリコール酸(PGA)、ポリ乳酸(PLA)、ポリグリコライドラクチド、ポリカプロラクトン、ポリジオキサノン、ポリオキサレート、ポリ酸無水物、ポリ(燐酸エステル)、腸線縫合糸、コラーゲン、絹、アルギナート、アガロース、キチン、キトサン、ハイドロキシアパタイト、生体吸収性カルシウム燐酸塩、ヒアルロン酸、エラスチン、ポリオルトエステル、ポリ(アミノ酸)、プルロニック/F−12、ポリ(エチレンオキシド)/ポリ(エチレングリコール)(PEO/PEG)、コラーゲン、ゼラチン、フィブリン、ヒアルロン酸、プロテオグリカン、エラスチン、及び、それらの組合せ、から構成される群から選択される、請求項1に記載の軟骨修復インプラント。 - 前記3次元足場材の内部には、複数の隙間が形成され、
前記隙間の個々は、約50μm〜約1,000μmのポア寸法を画定する、請求項1に記載の軟骨修復インプラント。 - 前記隙間の個々は、約100μm〜約500μmのポア寸法を画定する、請求項15に記載の軟骨修復インプラント。
- 前記3次元足場材及び前記3次元多孔性基材の少なくとも一方は、複数の要素によって形成され、前記複数の要素の各要素は、約25μm〜約300μmの直径を有する、請求項1に記載の軟骨修復インプラント。
- 前記複数の要素の各要素は、約50μm〜約200μmの直径を有する、請求項17に記載の軟骨修復インプラント。
- 軟骨修復インプラントを軟骨病変に導入するための方法であって、
関節の軟骨病変を位置させるステップと、
前記軟骨病変を拡大するステップと、
前記軟骨修復インプラントの3次元多孔性基材を受容するために、前記軟骨病変の周辺を取り囲む骨層を準備するステップと、
前記軟骨修復インプラントの前記3次元多孔性基材及び3次元足場材が、拡大した前記軟骨病変を完全に満たすように、前記軟骨修復インプラントの前記3次元多孔性基材を、前記準備した骨層の中に、挿入するステップと、を含む方法。 - 前記軟骨修復インプラントは、前記拡大した軟骨病変に嵌合するように構成され且つ前記準備した骨層に嵌合するために寸法決めされた形状に成型される、請求項19の軟骨修復インプラントの導入のための方法。
