Abstract
Description
本発明は、一実施例において、
活性成分を含有し、平均密度が1.1〜1.4g/mlである複合体を含む第1組成と;
平均分子量が0.5〜3.0MDaである第2組成と;を含み、
前記第2組成は、常温で24〜1500cpsの粘度を有することを特徴とする病変標識用注射剤組成物を提供する。
下記段階を含む、病変の位置に関する情報を提供する方法を提供する:
(a)前記注射剤組成物を個体で発生した病変に投与する段階;および
(b)前記個体から活性成分の信号が発生して病変の位置を確認する段階。
しかしながら、これは、本発明を特定の実施形態に対して限定しようとするものではなく、本発明の思想および技術範囲に含まれるすべての変更、均等物ないし代替物を含むことが理解されなければならない。
本発明は、一実施例において、
活性成分を含有し、平均密度が1.1〜1.4g/mlである複合体を含む第1組成と;
分子量が0.5〜3.0MDaである第2組成と;を含み、
前記第2組成は、常温で24〜1500cpsの粘度を有することを特徴とする病変標識用注射剤組成物を提供する。
本発明の一実施例において、第1組成は、大凝集アルブミン( macroaggregated albumin、MAA)に活性成分が結合された組成を含むことができる。
本発明の一実施例による第2組成は、分子量が0.5〜3.0MDaであり、生体適合性粘性物質を含むことを特徴とする。
|T2−T1|<15%
T2は、製造された注射剤組成物を1×1×3cm3サイズの透明容器に担持された状態で120分間放置した後、前記容器高さの1/4の地点で測定した550nmに対する透過度(%)である。
|F2−F1|<20gf
F2は、製造された注射剤組成物を26G針を含む注射器に担持された状態で120分間放置した後、注射するときの注射開始時に測定した滑走力である。
(a)本発明による注射剤組成物を個体で発生した病変に投与する段階;および
(b)前記個体から活性成分の信号が発生して病変の位置を確認する段階を含む。
製造例1:大凝集アルブミン(MAA)の製造
大凝集アルブミン(MAA)を製造するために、0.1Mのアセテート緩衝液(pH5.4、Sigma Aldrich、Korea)に希釈された2%ヒト血清アルブミン10ml(SKプラズマ)を塩化スズ50mg(Sigma Aldrich、Korea)と混合し、10分間室温で強く撹拌した後、70℃で20分間さらに撹拌して反応させた。
そして、反応が終了した後、更なる凝集を防止するために、前記反応物を氷に入れて急速冷却させた。
製造例2−1:ICGとMAAの混合比率の決定
近赤外線蛍光を示すMAA基盤の標識子を製造するために、実施例1で製造したMAAに近赤外線蛍光を示すインドシアニングリーン(Indocyanine green、ICG、第一薬品、ジアグノグリーン注射)を結合して、複合体(ICG−MAA)を製造した。
そして、製作されたそれぞれのICG−MAA複合体に示される近赤外線蛍光の信号強度を測定した(表1および図2)。
MAA濃度が1、2、4、8mg/mlであるICG−MAA複合体を製造した。ICG−MAA複合体製造反応は、常温で進めた。
実施例1:ICG−MAA−HAの製造
MMA濃度が1、2、4、8mg/mlであり、ICGが6.5μMであり、ヒアルロン酸(hyaluronic acid、HA、Shandon Focuschem Biotech Co.)の濃度が0.5%であるICG−MAA−HA注射剤組成物を製造した。
参考として、ICG−MAA−HAの製造工程は、常温で進めた。
実験例1.ICG−MAA複合体の密度測定
ICG−MAA複合体の密度を測定した。
ICG−MAA複合体は、生体物質などの形態を損傷せず、水溶液の状態で実際使用形態で密度を直接測定することは難しいため、間接的に濃度による密度の変化を参考にしてICG−MAA複合体の密度を測定した。
なお、20分後、蒸留水にICG−MAA複合体を溶解した蒸留水を確認したところ、容器の下方の部分に白色の懸濁液が沈んでいることを確認することができた。
実験例2−1:注射剤組成物としてのICG−MAA−HAの有用性−1
注射剤組成物としてのICG−MAA−HAの有用性を判断するために、HAの濃度によるICG−MAA−HAの懸濁性維持時間を測定した。参考として、MAAは、2mg/mlの濃度を利用し、ICG−MAA−HA最終注射物は、MAAとICGが完全に溶解すると、淡緑色の懸濁液の性状を示す。
そして、その結果を表2に記述した。
すなわち、0.2%(w/v)から実際の注射用製剤として効果的であると判断された。
より具体的に、前記で製造したICG−MAA−HA注射剤を30分が経過した後に撮影した写真であり、図3(a)は、0.1%のHAが添加されたMAAの写真であり、図3(b)は、0.5%のHAが添加されたMAAの写真である。
実験例2−2では、ヒアルロン酸(HA)の濃度によって注射時に指にかかる力(gliding force)を様々な条件で測定し、注射剤組成物としてICG−MAA−HAの有用性を判断した。
そして、その結果を下記の表4に記述した。
実験例3−3:ICG−MAA−HAの沈降による注射器圧力の変化−1
実施例3−3では、ヒアルロン酸が添加される注射剤組成物をすぐに注射する場合の圧力(A)と前記注射剤組成物を120分間放置した後、組成物が沈降した後に注射器にかかる圧力(B)を測定して、下記の表に記述した。
なお、滑走力の単位は、gfであり、g重(1g重=0.0098N)を示す。
一般的に、ヒアルロン酸の濃度によって26Gの注射器にかかる圧力は、約20gf(0.1% HA)から70gf(1.0% HA)増加する。注射剤内にヒアルロン酸の他に注射剤組成物によって注射器にかかる圧力は増加し、一例として8mg/mlの場合、約15gfの力がさらに加えられる。
そして、組成物により詰まった針があけられながら120gfに圧力が減少した。
実施例1で製造された注射剤組成物を利用して、時間による吸光度の変化を測定した。参考として、MAAは、2mg/mlを含む注射剤組成物を利用して吸光度の変化を測定した。
図4は、0.5%(w/v)のHA、6.5μMのICG、2、4、8mg/mlのMAAが含まれたICG−MAA−HAと6.5μMのICG、2、4、8mg/mlのMAAが含まれたICG−MAA注射物の明視野イメージ(bright field image)を示す。MAA粒子分布は、血球計がある光学顕微鏡を使用して可視化した。各正四角形は0.05mm2を示す。上部の行(図4の1、2、3)は、HAが含有されていないICG−MAAを示し、下部の行(図4の4、5、6)は、0.5%HA(w/v)が含有されたICG−MAA−HAを示す。MAA濃度は、それぞれ下記の通りである:図4の1&4:2mg/ml;図4の2&5:4mg/ml;図4の3&6:8mg/ml。上部の行でMAA粒子は、全部沈んでいて、明視野で全部焦点が合って鮮明な映像を得たが、下部行では、MAA粒子が沈んでおり、注射物内で浮遊していて、一部の粒子は焦点が合って鮮明な映像を得、一部の粒子は、焦点が合わない様相を示した。
図5は、多様なMAAの濃度で、5μMのICGと0.5%HAが添加されたICG−MAA−HAからの蛍光信号を測定したものを示す。図5のAは、明視野イメージを示し、図5のBは、NIRFイメージを示す。図5のAおよび図5のBの左側から、MAA濃度は、0mg/ml、1mg/ml、2mg/ml、4mg/mlおよび8mg/mlである。NIRFイメージは、740nmのピーク波長を有する17個の2mAのNIR LEDを発光させることにより収得された。前記イメージは、下記カメラの設定によってAVT UniCam viewer software(Allied Vision Technologies)により撮影され、可視化した:露出時間:200ms;ゲイン:200;ターゲット階調値:125;明るさ:16.
図6は、HA0.5%(w/v)の添加による差異を示す実験結果である。また、一般的な組織強度を示す鶏ささみおよび緻密な組織強度を示す鶏砂嚢を利用して組織強度による差異を確認した。ICG−MAA−HAとICG−MAA 50μlを鶏ささみおよび鶏砂嚢に5mmの深さでそれぞれ5回ずつゆっくり注射した。NIRFイメージは、740nmのピーク波長を有する17個の2mAのNIR LEDを発光させることにより収得された。前記イメージは、下記カメラの設定によってAVT UniCam viewer software(Allied Vision Technologies)により撮影され、可視化した:露出時間:195ms;ゲイン:170;ターゲット階調値:125;明るさ:16。注射を準備し注射するまで約2〜3分の時間が必要とされた。ICG−MAAは、鶏ささみ5回のうち2回、鶏砂嚢5回のうち3回でICG−MAA粒子が沈んでいて、注射針が詰まって、注射が困難であった。注射が困難な場合、さらに注射器を振った後、短い時間にさらに注射した。これに対し、ICG−MAA−HAは、10回の試みのうち注射が良好に行われない場合はなかった。
図8は、内視鏡用注射器(endoscopic catheter)を利用して胃癌手術におけるICG−MAA−HA混合物の適用性を実験したものを示す。図8の1〜図8の3および図8の5は、明視野イメージを示し、図8の4および図8の6は、NIRFイメージを示す;

図8の4:胃壁の内側にあるような注射部位のNIRFイメージを示す;
図8の5:注射された鶏ささみをひっくり返したイメージを示す;
図8の6:ひっくり返った鶏ささみ上の注射部位のNIRFイメージを示し、前記注射部位は、外部NIR供給源(sources)を利用して示すことができる。
沈降速度の増加によって注射時に一定量が注射される効果を調べるために、ヒアルロン酸が添加されていないICG−MAAと0.1%(w/v)のHAが添加されたICG−MAA−HA注射剤組成物の注射量を100μlずつ連続的にチューブに注射して、蛍光信号を測定した。
一方、蛍光信号は、チューブに注射し、5分間沈降させた後に測定した。

- 活性成分を含有し、平均密度が1.1〜1.4g/mlである複合体を含む第1組成と;
平均分子量が0.5〜3.0MDaである第2組成と;を含み、
前記第2組成は、常温で24〜1500cpsの粘度を有することを特徴とする病変標識用注射剤組成物。 - 前記注射剤組成物は、
常温で測定時に、下記数式1を満たすことを特徴とする請求項1に記載の病変標識用注射剤組成物:

大凝集アルブミン(macroaggregated albumin、MAA)に生体組織染色用色素、放射性同位元素またはこれらの組合せが結合された複合体を含むことを特徴とする請求項1に記載の病変標識用注射剤組成物。 - 前記生体組織染色用色素は、目視で確認可能な色素または蛍光色素であり、
前記放射性同位元素は、H−3、C−14、P−32、S−35、Cl−36、Cr−51、Co−57、Co−58、Cu−64、Fe−59、Y−90、I−124、I−125、Re−186、I−131、Tc−99m、Mo−99、P−32、CR−51、Ca−45、Ca−68およびこれらの組合せよりなる群から選択されることを特徴とする請求項6に記載の注射剤組成物。 - 前記第2組成は、ヒアルロン酸(Hyaluronic acid;HA)またはコラーゲンであることを特徴とする請求項1に記載の注射剤組成物。
- 第2組成は、全体注射剤組成物に対して0.2〜1%(w/v)含まれることを特徴とする請求項1に記載の病変標識用注射剤組成物。
- (a)請求項1〜9のいずれか一項に記載の注射剤組成物を個体で発生した病変に投与する段階と;
(b)前記個体から活性成分の信号が発生して病変の位置を確認する段階と;を含む病変の位置に関する情報を提供する方法。
