患者は、コペンハーゲン大学(Herlev、デンマーク)、Herlev病院の腫瘍学部門で、2014年3月〜2014年8月に実施されたファースト・イン・ヒューマンの第I相臨床試験に登録した。試験は、IDO1由来のペプチドワクチン接種をイピリムマブ又はベムラフェニブによる標準治療と組み合わせた2治療群第I相試験として当初デザインされた。ワクチンと組み合わせたイピリムマブにより治療された患者から得られたデータのみを報告する。対象は、組織学的に確認された切除不能な病期III又は病期IV悪性メラノーマを有し、年齢18歳以上、Eastern Cooperative Oncology Group(ECOG)パフォーマンスステータス≦2であり、メラノーマに対する最後の全身的治療から少なくとも21日経過し、その後十分に回復しており、そして血液機能、腎臓機能、及び肝臓機能が適切でなければならなかった。治療が無効であった、又は毒性起因で中止されたのでなければ、過去の抗CTLA-4治療の履歴は許容された。
ワクチンは、インドールアミン-2,3-ジオキシゲナーゼの残基194〜214に対応する、アミノ酸21個のペプチドからなる。ペプチドは、本明細書ではIO102と呼ばれる。ペプチドは、アミノ酸配列DTLLKALLEIASCLEKALQVF(配列番号3)を有する。ドイツ、ベルリンのJPT Peptides Technologies BmbH社が、GMP標準に準じて生産した。投与する場合、hospital pharmacy(デンマーク首都地域)が、2%のジメチルスルホキシド及び98%のPBS中にペプチドを溶解し、そしてモンタニドISA-51(Air Liquideグループヘルスケア部門Seppic社、パリ La Defence、フランス、の製品)と混合した。局所的5%イミキモドクリーム(Medea、Allerod、デンマーク)を、ワクチン部位に塗布し、そして注射の前6〜12時間、密封包帯で保護した。
試験治療の安全性を、有害事象の発生に関して評価し、有害事象共通用語基準(CTCAE)、第4.0版に基づき等級化した。抗腫瘍活性を、ベースライン時(治療開始前、最長28日)、12週間後、及びその後8〜12週間毎に進行するまで取得された陽電子放出型断層撮影法-コンピュータ断層撮影法(PET-CT)スキャンを用いて評価した。CTスキャンを、固形腫瘍における応答評価基準(RECIST)第1.1版に基づき評価した。応答の分類は、完全な応答(CR)、部分的な応答(PR)、進行性の疾患(PD)、及び安定な疾患(SD)である。少なくとも5回のワクチン投与を受けた患者を、本試験の臨床的及び免疫学的エンドポイントの評価について適格性を有するとみなした。
PBMCを、LeucoSep(商標)チューブ(Greiner Bio-One社)内で、lymphoprep(商標)(StemCell Technologies社)密度勾配遠心分離を使用して、ヘパリン処理された血液から精製した。処理後、細胞をNuncR 1.8ml CryoTube(thermo Scientific社)中で凍結し、そして90%のヒトAB血清(Sigma Aldrich社)及び10%のジメチルスルホキシド(Herlev Hospital Pharmacy)中、-150℃で保管した。患者-処理時間は、できる限り短時間に保たれるようにし、そして一般的に4時間以内に処理を開始した。血清収集用の血液を、凝固活性化剤を含有する8ml VacuetteRゲル-チューブ(Greiner Bio-One社)内に収集した。血清を、NuncR 1.8ml CryoTube(thermo Scientific社)内に一定量分取し、そして分析まで-80℃で保管した。PBMCを、1サンプリング毎に100mlのヘパリン処理血液から処理し、そして全血8mlから血清を処理した。血液サンプルをベースライン時、第4週目、第8週目、及び第12週目に取得した。非進行性の患者では、8〜12週間毎に追加の血液サンプルを取得し、同時に放射線評価を実施した。

PBMCを、24ウェルプレート(Nunc、Fischer Scientific社)内で、5%のヒトAB血清及び120U/mlのIL2(Novartis社、デンマーク)を含むX-vivo 15中のIO102ペプチドにより刺激した。最初に、20μMのペプチドを使用し、そして培養物を、7日間毎に、各週ペプチドの濃度をlog10減少させながら再刺激した。細胞培養物を、7〜10日毎に新しいIL2で補充した。細胞を3〜4×106/ウェルの濃度で保持した。IDO-特異的T細胞の富化及び増殖では、培養細胞を、培養4週間後に20μMのペプチドで再刺激し、そして製造業者の説明書に基づき、TNF-α分泌アッセイ(Miltenyi Biotec社)により富化させた。細胞を、2.5時間ペプチド刺激した後、一次抗体で染色した。富化した細胞を、改変された急速増殖プロトコールにおいて増殖させた(下記参照)。

PBMCサンプルを、バッファー100ml当たり2.5mlのDNAse含有パルモザイム(Roche社)、及び0.26mmolのMgCl(Herlev Hospital Pharmacy)が補充された、37℃のRPMI 1640培地(Lonza社)中で解凍した。すべての染色を、0.5%のウシ血清アルブミン(Sigma-Aldrich社)を含有するリン酸緩衝生理食塩水(PBS)(Lonza社)内で実施した。下記の抗体を使用した:FoxP3-PE、HLA-DR-HV500、CD3-PE-Cy7、CD19-PE-Cy7、CD56-PE-Cy7、CD4-HV500、CD11b-APC、CD3-APC(BD Bioscience社から購入)、CD33-FITC、CD124-PE(BD Pharmigenから購入)、HELIOS-PerCP-Cy5.5、CD14-BV421、CD25-BV421(すべてBiolegend社から購入)、CD127-FITC、CD39-PE-Cy7(eBioscience社から購入)。更に、すべてのサンプルを、LIVE/DEADR Fixable Near-IR Dead Cell Stain Kit(Life Technologies社)で染色した。転写因子の細胞内染色では、Foxp3/Transcription Factor Staining Buffer Set(eBioscience社)を、製造業者の説明書に基づき使用した。FACSDivaソフトウェア、バージョン6.1.3(BD Biosciences社)を使用して、FACSCanto II上で細胞を取得した。フローデータの分析を、FlowJoソフトウェア、バージョン10 OSX(TreeStar inc.社、Ashland、OR)を使用して実施した。死細胞及びダブレット/トリプレットのフィルタリング後に、データ分析を実施した。
治療前及び期間中に患者から得た血清中のインターロイキン(IL) 2、IL4、IL6、IL10、IL17A、TNF-α、及びIFN-γの濃度を、BD(商標) Human Th1/Th2/Th17 CBA Kit(BD Biosciences社)を使用して、製造業者の説明書に従い測定した。サイトカイン濃度を、解凍した未希釈の血清サンプルにおいて測定した。サイトカインの濃度を、FCAP Array(商標)ソフトウェア、バージョン3.0(BD SoftFlow社)を使用して計算した。
臨床及び免疫学データの比較では、当施設においてイピリムマブで治療された患者コホートを使用した(患者データ等をマニュスクリプトIIに提示)。この患者は、the local Ethics Committee for The Capital Region of Denmarkにより承認されたバイオマーカー試験(承認番号H-2-2012-058)に参加し、そして全患者が同意書を提出した。このプロトコールの組み入れ基準は、ワクチン試験の組み入れ基準と類似したが、但し、ワクチンアジュバントのモンタニド又はイミキモドに対する不耐性既知については評価しなかった。患者を、年齢(+/-5歳)及びM-期に基づき、対照として選択した。イピリムマブ療法期間中のIO102に対するT細胞反応性の比較では、IFN-γ分泌を、上記で説明したように、ELISpotアッセイにおいて測定した。反応性を、治療開始前及び3シリーズの治療後に取得されたPBMCサンプルにおいて評価した。
すべての統計計算を、GraphPad Prism(GraphPad Software社、La Jolla, CA)を使用して実施した。散布図をメジアン(水平のライン)と共に提示する。すべての検定を両側について実施し、またp値≦0.05を有意とみなした。該当する場合には、経時的な差異を、Wilcoxonの対応した符号付き順位検定を使用して評価した。
人口統計学
患者13例を試験に組み入れるためにスクリーニングし、12例を組み入れたが、1例は参加する意志を有さなかった。患者2例が、最初の治療を受け取る前に臨床的に有意な脳転移と診断され、試験から除外した。患者10例が、プロトコール内の治療を受け、そして全員が、試験内の臨床的及び免疫学的エンドポイントの評価について適格性を有するとみなされた。これらの患者10例の患者特性を表1に提示する。患者8例が、プロトコールに規定する4シリーズのイピリムマブ、及び7回のペプチドワクチンの投与を受けた。患者1例が、3シリーズのイピリムマブ及び5回のワクチンの投与を受けた後に症候性の脳転移を発症し、高用量コルチコステロイド治療を必要とするのでプロトコールから除外した。すべての患者10例において、イピリムマブを第一選択療法として投与した。患者1例が、試験に参加する前に、補助療法としてインターフェロン-αを投与された。患者のいずれも、転移性疾患について何らかの事前の全身療法は受けなかった。
結果を表2に提示する。ワクチンと関連しCTCAEグレードIII又はIV有害反応は認められなかった。一般的に、治療は良好な忍容性を示し、限定的で管理可能な毒性と関連した。
IO102(配列番号3)、及びネスト化されたHLA-A2エピトープIDO5(配列番号2、IO101とも呼ばれる)に対する自発的T細胞応答がこれまでに報告されており、そしてワクチンエピトープとしてのIDO5の臨床的有効性及び免疫原性の両方が、これまでの肺がんの第I相臨床試験において実証された。
IDOは、免疫抑制の重要なメディエーターであり、また制御性の免疫細胞の動力学に対して重要な影響を有し得る。マウスでは、IDOの発現及びIDOに触媒されたトリプトファンの分解により産生された代謝物が、Tregの生成を誘導することが明らかにされている。更に、IDOは、MDSC(骨髄由来のサプレッサー細胞)内で発現され得るが、これにより、この細胞型に関するいくつかのエフェクター機構のうちの1つを代表する。末梢血液中のTreg及びMDSCのレベルを、療法の前、期間中、及び後に測定した。
組み込まれた患者12例のうち10例が、プロトコールの一環として治療を受け、また全員が、評価可能患者について規定されたプロトコールに従い、少なくとも5回のワクチン投与を受けた。このため、患者10例のうち7例が治療終了から10ヶ月後もなお生存した。
イピリムマブは、免疫阻害性分子CTLA-4を標的とし、転移性メラノーマを有する患者の全生存を延長することが明らかにされている。この治療は、腫瘍負荷の劇的な低下、また患者によっては完全な応答を伴う、永続性の応答を誘導する可能性がある。このように楽天的であるにもかかわらず、療法に実際に応答するのは患者のうちでもなおも小さい割合であり、改善の余地は大いにある。これを実現する試みにおいて、非常に多くの試験では、1回に2つ以上の標的に打撃を与える手段としての組合せ療法に重点が置かれた。いくつかの試験では、イピリムマブとその他の介入との組合せについて試験され、これまで、最も有望なデータは、PD-1標的抗体ニボルマブとの組合せに基づいた。しかし、ここで報告する試験以前において、IDO1に対するワクチンと組み合わせる試みは見当たらない。
IDOペプチドワクチン構成要素の更なる特徴づけ(1)
T細胞におけるCMVペプチドに誘導された刺激に対するIDO5及びIO102のブースト効果の比較
方法

第1日目:

IO102+抗PD1は、対照ペプチド+抗PD-1と比較して、THP-1細胞においてより良好なPBMC媒介型の細胞毒性を誘導する。

IDOペプチドワクチンに関するマウスモデル試験
TC-1腫瘍を担持するC57BL/6マウスを、TC-1特異的E7-ペプチド(RAHYNIVTF、配列番号35)又はIDO-Pep1(MTYENMDIL、配列番号33)で治療し、そしてペプチドワクチンの有効性を、腫瘍負荷及び生存率により評価した。マウスIDO1の配列はヒトIDO1の配列とは異なるので、マウスIDOペプチド配列(IDO-Pep1)を選択して、上記実施例において試験したヒトIDOペプチドに対応するマウス類似体を提供した。
4〜6週齢のC57BL/6マウスに、細胞70,000個/マウスのTC-1細胞を注射した。腫瘍が約0.075cm3の平均サイズに達したとき(腫瘍イノキュレーション後、約10日)、それぞれの群内の治療を開始した。ワクチン接種で使用されるペプチドには、TC1-特異的E7-ペプチド(RAHYNIVTF、配列番号35)及びIDO-Pep1(MTYENMDIL、配列番号33)が含まれた。マウスIDOペプチドを、MHCクラスI及びII結合アルゴリズムに基づきデザインした。Pan-HLADR-結合エピトープ(PADRE、aK-Cha-VAAWTLKAAa、20μg/マウス)とQuilA(10μg/マウス)を組み合わせて、100μg/マウスの投薬量で投与して、それぞれのペプチドをマウスの皮下にワクチン接種した。1週間の間隔を置きながら2〜3回マウスにワクチン接種を行い、そしてペプチドワクチンの有効性を、腫瘍容積及び/又は全生存を測定して評価した。
マウスIDOペプチドを用いたワクチン接種は、TC-1腫瘍モデルにおいて、抗腫瘍保護効果を実証し、腫瘍特異的ペプチド抗原であるE7ペプチドを用いてワクチン接種した場合よりも優れた保護を提供する。図10を参照。
腫瘍抗原ワクチンと組み合わせた、マウスモデルにおけるIDOペプチドワクチン試験
TC-1腫瘍を担持するC57BL/6マウスを、TC-1特異的E7-ペプチド(RAHYNIVTF、配列番号35)、又はIDO-Pep1(MTYENMDIL、配列番号33)、又はE7及びIDO-Pep1の両方の組合せを用いて治療した。ワクチンの有効性を、全生存により評価した。方法は、ペプチドの組合せを含めたことを除き、実施例7と同一であった。
E7ペプチドでワクチン接種を受けたマウスは、TC-1モデルにおいて、治療有効性を期待通り有する(及び図10に示す)。但し、E7ペプチドを、IDO-Pep1と共に投与したとき、E7単独又はIDO-ペプチド単独よりも優れた保護を提供する。図11を参照。
追加のチェックポイント阻害剤と組み合わせた、マウスモデルにおけるIDOペプチドワクチン試験
TC-1腫瘍を担持するC57BL/6マウスを、IDO-Pep1(MTYENMDIL、配列番号33)単独、1-メチルトリプトファン(1-MT)単独、又はIDO-Pep1及び1-MTの両方の組合せを用いて治療した。ワクチンの有効性を全生存により評価した。
4〜6週齢のC57BL/6マウスに、細胞70,000個/マウスのTC-1細胞を注射した。腫瘍が約0.075cm3の平均サイズに達したとき(腫瘍イノキュレーション後、約10日)、それぞれの群内の治療を開始した。マウスのワクチン接種では、IDOエピトープ、IDO-Pep1を使用した。Pan-HLADR-結合エピトープ(PADRE、aK-Cha-VAAWTLKAAa、20μg/マウス)とQuilA(10μg/マウス)を組み合わせて、IDO Pep-1を100μg/マウスの投薬量でマウスの皮下にワクチン接種した。1週間の間隔を置きながら2〜3回マウスにワクチン接種を行い、そしてペプチドワクチンの有効性を、腫瘍容積及び/又は生存率を測定して評価した。マウスの1つの群を、試験期間、飲料水に溶解した1-メチルトリプトファン(1-MT)(2mg/ml)で治療し、またマウスの追加の群をIDO-Pep1及び1-MTの両方で治療した。
TC-1を担持するマウスを経口a-MTで治療したとき、実現する治療有効性はそれほど高くない。このようなマウスにIDO Pep-1を同時投与すると、生存が延長することから実証されるように、有効性が強化された。図12を参照。
IDOペプチドワクチンの代替的マウスモデル試験
マウスIDOペプチドのIDO-EP2(LPTLSTDGL、配列番号34)を、BALB/cマウスに予防的にワクチン接種し、そして7日後にCT26腫瘍を用いて誘発試験を行った。ペプチドワクチンの有効性を、腫瘍負荷により評価した。マウスIDO1の配列はヒトIDO1の配列とは異なるので、マウスIDOペプチド配列(IDO-EP2)を選択して、上記実施例において試験したヒトIDOペプチドに対応するマウス類似体を提供した。
6〜10週齢のBALB/cマウスに、1:1の乳化として調製した、100μgのペプチドIDO-EP2+30μgのCpGを含む100μLのモンタニドアジュバント[モンタニドISA 51 VG(Seppic社)]を尾部基底部の皮下注射により免疫した。モンタニド単独(媒体)を対照として使用した。マウス数匹を、更なる対照として未治療のままとした。免疫化を、腫瘍誘発試験の7日前に実施した。CT26腫瘍細胞移植では、各マウスは、1×105個のCT26を含む無血清DMEM、100μLの皮下注射を2回(各側腹部に1回)受けた。腫瘍長さ及び幅のカリパス測定値を、6日目から開始して3〜4日毎に記録した。腫瘍容積を式0.52(L×W2)を使用して計算した。合計した腫瘍容積が、約2,000mm3の定義されたエンドポイントに到達したとき、マウスを安楽死させた。
マウスIDOペプチドを用いた予防的ワクチン接種は、CT26腫瘍モデルにおいて抗腫瘍保護効果を実証した。図13を参照。
抗PD1抗体と組み合わせたPD-L1/IDOペプチドワクチンの臨床試験
第1の目的は、免疫チェックポイントブロッキング抗体のニボルマブ(ヒトPD1に対して特異的)と組み合わせて、転移性の悪性メラノーマ(MM)を有する患者に投与したときに、アジュバントとしてモンタニドISA 51を含む、ペプチドIDO long(DTLLKALLEIASCLEKALQVF、配列番号3)、及びPD-L1 long1(FMTYWHLLNAFTVTVPKDL、配列番号32)を含有するペプチドワクチンの忍容性及び安全性を評価することである。エンドポイントは、CTCAE 4.0により評価する有害事象(AE)である。

- 対象内のがんを予防又は治療する方法であって、
(i)免疫系チェックポイントの構成要素又は前記構成要素の免疫原性断片を含む免疫療法組成物、及び
(ii)(i)の組成物がその構成要素を含むチェックポイントと同一であり得る、又は異なり得る免疫系チェックポイントをブロック又は阻害する免疫調節剤
