Abstract
암 치료를 위한 면역체크포인트 억제제의 병용 요법에 관한 것으로, 구체적으로, 화학식 1의 화합물 또는 이의 약학적으로 허용가능한 염을 2종 이상의 다른 항암제(예를 들면, 항 PD-1 항체, 항 PD-L1 항체 또는 항 CTLA4 항체)와 3중 병용 투여 시 항암에 대한 현저한 시너지 효과가 나타나, 암 치료에 유용하게 사용될 수 있는 효과가 있다.
Description
암 치료를 위한 면역체크포인트 억제제의 병용 요법에 관한 것이다.
인돌아민 2,3-다이옥시게나제(Indoleamine 2,3-dioxigenase; IDO)는 활성화된 마크로파지 및 다른 면역-조절 세포에 의해 생성된 면역-조절 효소이다(또한, 많은 종양 세포에 의해 면역 반응에서 벗어나기 위한 전략으로서 이용될 수 있다). 인간에서 유전자 IDO(또한 IDO1 및 INDO로서 공지됨)에 의해 코딩되고, 중추 신경계(CNS), 부고환, 장, 흉선, 기도, 비장, 췌장, 태반, 수정체 및 신장 뿐만 아니라 마이엘로이드 세포, 예컨대 마크로파지, 덴드리트 세포 및 마이크로글리알(microglial) 세포를 비롯한 많은 조직에서 발현된다. 이는 N-폼일-카이누레닌으로의 트립토판 이화작용에서의 제1 속도-제한 단계에 촉매 작용할 수 있다. 트립토판 및 이의 유도 대사산물의 결핍은 잠재적 면역 억제를 일으켜 T 세포 성장 및 활성화, T 세포 세포자멸의 유도 및 조절 T 세포의 증가를 일으킬 수 있다(문헌[FASEB J. 1991, 5, 2516-2522]). 트립토판-카이누레닌 대사 경로는 선천성 및 적응성 면역에 중요한 것으로 증명되었다. 트립토판 2,3-다이옥시게나제(Tryptophan 2,3-dioxygenase; TDO 또는 TDO2)는 또한 트립토판-카이누레닌 대사 경로를 따라 트립토판을 분해하는 비관련성 간 효소이다. 인간에서 이는 유전자 TDO에 의해 코팅되고, 간, 태반, 및 뇌에서 높은 수준으로 발현된다. 이는 트립토판- 카이누레닌 대사 경로를 따라 트립토판 분해의 제 1 속도-제한 단계를 촉매 작용할 수 있어서, 전신 트립토판 수준을 조절할 수 있고, 동일한 반응이 IDO에 의해서 촉매 작용된다.
많은 전임상적 연구에서 이런 면역 관용(immune tolerance) 경로가 암, 자가면역, 감염, 이식 거부 및 알러지에 관여되는 것을 밝혔다. 증가된 IDO 활성은 암 증식 및 전이에 중요한 역할을 한다. 종양 특이적 세포독성 T 림포사이트의 불활성화를 초래할 수 있고, 이는 이후 암 세포를 공격하는 이의 능력을 소실케 할 수 있음이 밝혀졌다. 실제로, IDO의 과-발현은, 전립선암, 결직장(colorectal)암, 자궁경부암, 위암, 난소암, 뇌암 및 폐암을 비롯한 많은 인간 암에서 발견된다. IDO의 억제는 종양에 의해 억제되는 면역 기능을 역전(reverse)시킬 수 있어서, 효과적인 항-종양 면역성을 생성할 수 있다. 도 6은 N'-(4-bromophenyl)isoxazole-4-sulfonohydrazide와 Epacadostat을 이용한 병합 면역 치료 효능 비교 실험 디자인을 나타낸 도이다.

이하 본 발명을 실시예를 통하여 보다 상세하게 설명한다. 그러나, 이들 실시예는 본 발명을 예시적으로 설명하기 위한 것으로 본 발명의 범위가 이들 실시예에 한정되는 것은 아니다. 또한, 본 명세서에서 N'-(4-bromophenyl)isoxazole-4-sulfonohydrazide는 시험물질, STB-C017 또는 STB로 지칭될 수 있다.
실시예
1. IDO/TDO 억제제, PD-1 면역관문 억제제 및 CTLA-4 면역관문 억제제의 병합 면역 치료 효능의 확인
실험에 사용된 IDO/TDO 억제 화합물 N'-(4-bromophenyl)isoxazole-4-sulfonohydrazide(화학식 10) 및 화학식 11 내지 15는 ㈜신테카바이오(대한민국)으로부터 제공받아 사용하였다. 실험에 사용된 PD-1 억제제는 InVivo Mab anti-mouse PD-1 (CD279)를 사용하였고, CTLA-4 억제제는 InVivo Mab anti-mouse CTLA-4 (CD152)를 사용하였으며, 두 억제제 모두 Bio X Cell로부터 구입하였다.
아울러, 화학식 10 내지 15의 화합물의 IDO/TDO 억제 활성은 다음과 같다.

(1) 실험 목적
N'-(4-bromophenyl)isoxazole-4-sulfonohydrazide와 면역 체크포인트 억제제인 anti-PD-1 및 anti-CTLA-4의 병용 사용에 대한 효과를 검증하기 위해 본 실험을 진행하였다.
(2) 실험 방법
8주령 BALB/c 면역 보존 마우스에 2 x 105 CT26 (Colon cancer) 세포주를 오른쪽 측면에 피하주사 (subcutaneous injection) 후 종양 크기가 약 50 mm3에 도달하였을 때, 약물 투여를 진행하였다. 구체적인 실험 군 및 약물 투여 일정은 도 1과 같았다.
시험물질의 경우 앞의 실험을 바탕으로, 5일 투여 2일 휴식의 일정으로 수행하였으며, 모든 군에서 종양 크기 및 마우스 무게를 측정하며, 관찰 종료 시 CO2 가스를 사용하여 안락사를 시행하고 종양 조직을 채취하였다.
또한, 채취한 종양조직에 대해 유세포분석 (Flow cytometry, FACS)을 진행하였다.
(3) 실험 결과
1) 약물 처리에 따른 종양 억제 효과
Control군에 비해 αPD-1군은 25.1%, STB-C017 단독 치료군은 35.8%의 종양 성장 지연을 나타냈으며, 특히 STB-C017 및 αPD-1 병합 치료군에서 55.8%로 현저하게 종양 성장 지연이 증가하는 것을 확인하였다.
또한, 다른 실험군들과 달리 STB-C017 및 αPD-1 병합 치료군의 12.5% (1/8)에서 complete tumor regression(CR)이 관찰되었으며, 특히, STB-C016, anti-PD-1과 함께 anti-CTLA-4를 함께 처리해준 3중 병용처리 그룹에서는 대조군에 비해 종양크기가 88% 감소하였으며, 8마리 중 3마리에서 CR이 관찰되었다(하기 표 2 내지 7 및 도 2).

3) 약물 투여에 따른 종양 내 면역 세포의 변화
종양 내 침윤 CD8+ T세포 수의 경우, control군에 비해 STB-C017 단독 치료군, STB-C017 및 αPD-1 병합 치료군 및 3중 병용 치료군에서 모두 유의하게 증가하는 것을 관찰하였다.
특히, STB와 STB+P 그룹이나, 3중 처리 그룹에서는 T세포에서 CD8+ T 세포의 비율이 증가하는 것을 확인할 수 있었다.
또한, 종양 g당 CD8+ T 세포의 수를 보았을 때, STB 단독 그룹이 control 그룹에 비해 유의하게 증가하였으며, STB 단독군에 비해 3중 처리 그룹에서 CD8+ T cell이 유의하게 증가하는 것을 확인하였다.
또한, 종양 내 침윤 CD4+ T세포 수 및 종양 g당 CD4+ T세포 수의 경우에도 STB-C017의 단독 치료군 또는 병합 치료군에서 control군에 비해 증가하는 경향을 보였으며, 특히 3중 처리 그룹에서 유의하게 증가함을 확인하였다(도 4).
또한, CD4 세포 중 CD25FoxP3를 발현하는 Treg 세포를 분석하였을 때, control 그룹에 비해서 STB 단독 처리군에서 Treg 세포가 감소함을 확인하였으며, STB 단독 그룹보다는 3중 처리한 그룹에서 Treg 세포가 크게 감소함을 확인하였다.
조절 T세포에 대한 CD8+ T세포의 비율을 비교한 결과, control 그룹보다 STB와 병용 처리한 그룹이 높았으며, 특히 2중 병용 처리보다 STB-C017, αPD-1 및 αCTLA-4의 3중 병용 처리 군에서 가장 높은 것을 확인하였으며, 이는 T세포의 cytotoxic effector function이 전반적으로 증가했다는 것을 나타낸다(하기 표 14 내지 38 및 도 5).

(4) 소결
시험물질인 N'-(4-bromophenyl)isoxazole-4-sulfonohydrazide와 αPD-1의 2중 병합 치료는 단독 치료에 비해 종양 내 침윤 CD8+ T세포의 effector function을 향상시켜 항종양 면역 반응 효과를 증가시킴을 확인하였으며, 시험물질과 αPD-1 및 αCTLA-4의 3중 병합치료는 2중 병합 치료에 비해 종양 내 침윤 CD8+ T세포의 effector function을 향상시켜 항종양 면역 반응 효과를 증가시킴을 확인하였다.
특히, STB-C017과 αPD-1의 병합 치료에서 complete tumor regression(CR)이 관찰되었으며, STB-C017, αPD-1 및 αCTLA-4의 3중 병합 치료에서 CR이 3마리 관찰되었는 바, 이는 강력한 항암 면역 반응이 유도되었음을 시사한다.
2. N'-(4-bromophenyl)isoxazole-4-sulfonohydrazide와 Epacadostat을 이용한 병합 면역 치료 효능 비교
(1) 실험 방법
8주령 BALB/c 면역 보존 마우스의 오른쪽 측면에 2 x 105 CT26 대장암 세포주를 피하주사(subcutaneous injection)하였고, 종양 크기가 약 50 mm3에 도달하였을 때, 약물 투여를 진행하였다.
구체적인 실험군 및 약물 투여 일정은 도 41과 같았으며, 모든 군에서 종양 크기 및 마우스 무게를 측정하며, 생존 분석(survival analysis)를 진행하였다.
(2) 실험 결과
1)
약물 처리에 따른 종양 억제 효과
Control군에 비해 STB-C017 또는 Epacadostat 단독 치료군에서 종양 성장이 지연되며, αPD-1과의 병합 치료에 의해 그 효과가 증가하는 것을 확인하였으며, 특히 αPD-1 및 αCTLA-4와의 3중 병합 치료에 의해 그 효과가 현저하게 증가하는 것을 확인하였다.
STB-C017 및 αPD-1 병합 치료군의 12.5% (1/8), αPD-1 및 αCTLA-4 병합 치료군의 25% (2/8), Epacadostat 및 αPD-1 병합 치료군의 25% (2/8), Epacadostat, αPD-1 및 αCTLA-4 병합 치료군의 37.5% (3/8)에서 complete tumor regression(CR)이 관찰되었으며, 특히 STB-C017, αPD-1 및 αCTLA-4 병합 치료군의 100% (8/8)에서 CR이 관찰되었는바, 완전히 암을 치료하였음을 확인할 수 있었다(도 7 및 8).
2) 생존 분석(survival analysis) 결과
Control군에 비해 STB-C017, αPD-1 또는 Epacadostat 단독 치료군에서는 전체 생존율에 차이를 보이지 않는 것을 확인하였다.
반면, STB-C017 및 αPD-1 병합 치료군, αPD-1 및 αCTLA-4 병합 치료군, Epacadostat 및 αPD-1 병합 치료군 또는 Epacadostat, PD-1 및 αCTLA-4 병합 치료군에서 전체 생존율이 증가됨을 확인하였다.
특히, STB-C017, αPD-1 및 αCTLA-4 병합 치료군의 100% (8/8)가 생존하였음을 확인할 수 있었다(도 9).
3) 소결
Epacadostat와 달리, STB-C017는 αPD-1 및 αCTLA-4와의 3중 병합 치료에 의해 암을 완치시킬 수 있으며, 치료군 전원 생존할 수 있었는 바, 3중 병합 치료제로서 사용할 경우 시너지 효과에 의해 매우 우수한 효능을 나타냄을 알 수 있었다.
