本発明は、外傷性脳損傷(TBI)、軽度TBI(脳震盪)および外傷性脊髄損傷(SCI)ならびに慢性神経変性疾患からのこれらの区別を含む、神経外傷の重症度および転帰の検出、早期推定、進行および治療のモニタリングのための抗体、プローブ、キットおよび関連材料、ならびにこれらの使用を含む、本発明の内容の組成物に関する。

本発明は、対象における外傷性脳損傷(TBI)、軽度TBI、および/または脊髄損傷(SCI)の状態を検出またはモニターするための方法を提供する。当該方法は、対象におけるTBIまたはSCIの存在、進行、予測、および重症度の区別を判定するために使用することができる。一実施形態において、当該方法は、対象から得た体液の標本をアルドラーゼC(ALDOC)および脳脂質結合タンパク質(BLBP/FABP7)、またはALDOCもしくはBLBP/FABP7の外傷特異的な分解産物(BDP)から選択されるTBIのマーカーについてアッセイするための試薬と接触させることを含む。当該方法はさらに、対照試料と比較して、当該標本中に存在するマーカーの量を測定すること、およびマーカー量の増加が対照試料と比較して標本中に存在する時、TBIまたはSCIの存在を判定することを含む。一実施形態において、TBIのマーカーは、ALDOCおよび/またはそのBDP、ならびにBLBPおよび/またはそのBDPである。任意に、当該方法はさらに、グルタミンシンテターゼ(GS)、星状細胞リンタンパク質PEA−15(PEA15)、αB−クリスタリン(CRYAB/HSP27)、ALDOC、GS、PEA15、またはCRYABの外傷特異的タンパク質分解性切断産物、あるいはこれらのうちの2つ以上からなる任意の組み合わせの量を測定することを含む。一実施形態において、当該方法はさらに、グリア線維性酸性タンパク質(GFAP)またはGFAPの20〜30kDaのBDPの量を測定することを含む。

以下の実施例は、本発明を説明するために、ならびに本発明品を作製および使用する上で当業者を支援するために呈されている。実施例は、いかなる方法においても、さもなくば本発明の範囲を制限するよう意図するものではない。
本実施例は、異なる重症度を用いた急性圧力−パルス外傷性伸張の30分後および48時間後のヒト星状細胞の母集団スコアを示す(図1、PSIは比較的軽度な損傷については2.6〜4.0に、および重度な損傷については4.4〜5.3に及ぶ)。ヒト星状細胞を16〜18週の寄贈された胎児新皮質脳標本から分離し、精製し、次に変形可能な膜上で分化させた(Wanner,2012)。細胞膜の受傷/損壊、機械的穿孔(Barbee,2005)は、核形状評価(真ん中の図、染色された核、固定後にヘキスト染色、ややピンク色の点あり、ヨウ化プロピジウム(PI)で染色した核小体)を伴う、生培養物におけるPIの取り込みを用いて判定した。細胞死は、核濃縮(コンパクトになった染色質ならびに明るいヘキストおよびPI染色を有する核濃縮した核)を伴うPI取り込みによって判定した。細胞統合性損壊の有意な百分率は損傷30分後に示される(左のグラフ)のに対し、有意な細胞死は損傷2日後に存在した(右の棒)。
本実施例は、機械的に外傷を受けたヒト星状細胞が、受傷後の損壊した状態においてマウス星状細胞に対して長期の持久性を示すことを実証する(図2)。伸張中の色素の取り込み(0.05mMの10kDaデキストランローダミン)、ならびにアポトーシスによる細胞死(10μMのカスパーゼ3基質を用いたカスパーゼの活性化)を、温度および湿度を制御された回転ディスク共焦点顕微鏡上でのタイムラプス撮影を用いてモニターした(Levineら,2016)。ヒトの外傷を受けて損傷しおよび再密封したヒトアストログリアは、統合性の損壊後の長期の持久性を示す。
%膜透過性細胞(%受傷、赤色データ、左)に対する、ならびにPI色素取り込みアッセイおよび核の形態(図1および図2を参照されたい)を用いた外傷を受けたヒト死星状細胞の百分率と相関した、ALDOC、BLBP、PEA15およびCRYABについての外傷放出性アストログリアマーカーレベル(図3を参照されたい)の二重プロットを図4Aに示す。回帰線(R2値)およびp値は、相関有意性を示す。ALDOC、PEA15およびCRYABは、ヒトアストログリア細胞受傷と最も良い相関を示す。CRYAB、BLBPおよびALDOCはまた、外傷後の細胞死を伸ばすことと相関していた。
候補となる星状細胞損傷バイオマーカーを次の戦略によって選択した(図5)。TBIのCSFプロテオームを、19名の重度TBI患者からのCSF試料を用いたボトムアップ質量分析によって編集し、9名の対照被験者からのCSFプロテオーム(Crl)と比較した。このTBIのCSFプロテオーム(483のタンパク質)は、対照CSFのプロテオーム(402のタンパク質)由来の252のタンパク質と重複しており、残りは231の独特なTBIのCSFタンパク質であった。このTBIのCSFタンパク質の60%は、公開された血漿プロテオームにおいても存在し(Omennら,2005、Schenkら,2008)、多量な場合、TBIバイオマーカーとして適切ではないかもしれない。候補となる脳損傷マーカーを選択するために、本発明者らは、単純な外傷モデルにおけるプロテオームアプローチを用いた、外傷を受けた星状細胞から放出されるタンパク質を判定し、79の差次的に放出されるタンパク質を、圧力−パルス伸張後にマウス星状細胞を伸張して識別した(Levineら,2015、Sondejら,2011)。
本実施例は、アストログリア損傷マーカーが、損傷当日および損傷後5日間連続で、TBI患者対後向き観察コホートにおける対照のCSFにおいて上昇することを実証する。図6Aは、30μlの対照CSF(Crl)とともに54歳男性重度TBI患者(1a〜1f)の損傷当日(i)および損傷後の5日間(i+1〜i+5)からの30μlのCSF試料の縦断的セットのGFAP(BDP37、25、20および18kDaとともにある50kDa)およびS100□およびALDOC(40kDa)、GS(45kDa)、BLBP(15kDa)およびPEA15(15kDa)のイムノブロットを示す。BLBPおよびPEA15は、損傷当日に最強のシグナルを、および損傷3日後に第二の上昇を示し、このことは、患者の二次的な虚血性エピソードと関係していた。出血指標のAPOB(130および250kDa)は、損傷後経時的に変化する強度を有しており、健常CSFからはなく、CSFマーカーであるPTGDS(22kDa)は、CrlのCSFにおいて頑強なシグナルを有していたが、TBI後および損傷翌日の急性期にはなく、その損傷後の日々においてシグナルを徐々に回復した。
本実施例は、新たな、細胞死と関係する下位のGFAP分解産物のCSFレベルが、TBIの生き残った者に対して生き残らなかった者において2桁ほどより高かったことを実証する(図7)。PEA15レベルは、TBI後の生き残った者に対して、生き残らなかった者において2〜3桁高く、損傷3日後のp値は、p=0.07であった。損傷後に一過性に低下する健常マーカーPTGDSのレベルは、TBIの生き残った者において損傷3日後までに有意な回復を示したのに対し、生き残らなかった者においては、レベルは低いままであった。
本実施例は、損傷の範囲にわたるTBIの評価において有用な因子を創出するためのアストログリア外傷マーカーの群分けを実証する。マーカーパネルによって包含される多様なデータを組み合わせるために、スピアマン相関係数を基にした教師なし学習アルゴリズムを採用した多変量分散分析を使用した(因子分析、FabrigarおよびWegener,2012、Tucker,1997)。このアプローチは、神経外傷バイオマーカーの分野にとって新しく、当該マーカーのTBIのCSFシグナルを基にして本実施例においてマーカーを群分けすることによって潜在的な神経外傷容態を明らかにする。公知のアストログリアマーカーS100β、公知のおよび新たなBDPを有する細胞死マーカーGFAP、ならびに出血指標APOBをA因子へと群分けした(図8A、灰色、Cronbach係数α=0.921を用いて組み合わされた負荷を用いる)。新たな「細胞漏出性」の、BDPを有する細胞受傷マーカーALDOC、BLBP、GSおよびPEA15をB因子へと群分けした(α=0.879)。図8Bにおいて示されるように、A因子の一時的な特性は、損傷後の日々にわたって有意に低下したのに対し、B因子の軌跡は低下しなかった。A因子のデータは、損傷後の数日間における生き残った者と生き残らなかった者の間の有意差を示した(図8C)。B因子のデータは生き残りの差を示さなかった(図8D)。マーカー全部に対してシグナルが存在する12名のTBI患者からの標準化されたマーカー密度読み取りを、図8Aにおいて示される負荷を用いてA因子およびB因子へと変換した後、たがいに対してプロットした(図8E)。TBIの範囲における不均一性は、この二重プロットにおける組み合わされたパネルデータの広がりにおいて反映されている。分類ツリー分析(図8F)は、所与の因子閾値(Breiman,1984)によるTBIの対照、生き残った者および生き残らなかった者を区別する境界を決定した。複数の因子へと群分けしたバイオマーカーレベルを用いた患者区別閾値を図8Eにおける複数の線によって示す。
本実施例は、対象の標本におけるBLBPレベルとGFAPレベルの比が、外傷を受けたヒト星状細胞およびTBI患者における外傷重症度を区別するために使用することができることを実証する。「細胞死」マーカーGFAPにおける「細胞漏出性」マーカーBLBPの流体レベル比における有意差は、インビトロ(図9、CM、条件培地、左)でおよびTBI患者(CSF、図9の右側パネル)で示される。6名のドナー由来のヒト星状細胞は異なる重症度で、示されるPSI圧力−パルスによって外傷を受けた。中等度対重度TBI患者を、CSFにおける14倍の異なるBLBP/GFAP比によって識別する(n=9)。中等度のTBI患者を、8を超える蘇生後グラスゴー昏睡尺度(GCS)によって定義するのに対し、重度TBI患者は8未満のGCSを有していた。
図10に提示されるデータは、TBI6か月後の拡張グラスゴー転帰尺度(GOSe)を用いて評価される重度TBI患者の転帰に対してプロットされたALDOCおよびGFAPのレベルを示す。12(11)名のTBI患者からのデータは、ALDOCの上昇を早期に(白色、i、i+1、+2)、および好ましくない転帰を有する患者における損傷後後期に(黒色、i+3、+4および+5の損傷後の経過日)示すのに対し、GFAPレベルは、損傷後の経過日において高いレベルを同様に維持することはなかった。
本実施例は、ブタ動物モデルにおけるアストログリア外傷マーカーと脊髄損傷の重症度との相関を実証する。図11:急性UCLAアストログリアマーカーの放出は、ブタ脊髄損傷後の組織病理学的重症度の基準、組織喪失および出血と相関した。A)急性(損傷15〜30分後)UCLAアストログリア損傷マーカーALDOC、BLBPおよびグルタミンシンテターゼ(GS)ならびにGFAPのCSFシグナル密度を、確立した損傷重り落下モデル(Leeら,2013)を用いた脊髄挫傷損傷の1週間後のユカタンブタ脊髄における体軸方向の腔径に対してプロットする。スピアマン相関は、組織喪失の増大と有意に関係したこれらのアストログリアCSFマーカーレベルの急性上昇を示す。組織病理学:当該腔の体軸方向の伸展は、ズダンブラック染色した水平方向の脊髄切片における尺度である(所与のp値およびR2値、n=10個体)。B)ALDOCおよびGSの急性(損傷15〜30分後)のCSFシグナル密度は、損傷した水平方向の脊髄切片におけるブタ免疫グロブリン(IgG)蛍光を用いて測定され併行染色した非損傷の水平方向の脊髄切片によって標準化された、損傷1週間後の白質間質性組織出血と有意に関係していた。体軸方向の連続画像に関するこのような標準化された白質蛍光シグナルを各動物についてプロットし(n=8個体)、曲線下面積(AUC)を先の非損傷シグナルレベルとの距離にわたって測定した。
本実施例は、定量的質量分析が損傷後のアストログリアのTBIマーカーのレベルの上昇を確認して、免疫学的方法が標準化されていない場合の免疫学的方法を用いるときに制限される客観的マーカー量比較を提供することを実証する。抗体非依存性の同時および定量的質量分析アプローチである多重反応モニタリングを、神経外傷バイオマーカー分野において最初に用いて、TBI患者のCSFにおける公知のおよび新たなアストログリアマーカーの量を比較した。マーカー特異的ペプチドを定義した量の添加した同位体標識ペプチドと併行して測定する(表6を参照されたい)。図13Aは、ALDOCおよびGFAPが損傷当日にTBIのCSFにおける最高濃度を有していたことを示し、両方とも示される他のマーカーのレベルとは有意に異なっていた。さらに、BLBPレベルは損傷当日、GSレベルよりも有意に高かった。図13Bに示されるように、TBIの3日後までに、ALDOCレベルは、GFAP濃度よりもある規模だけ異なって、有意に優れていた。
本実施例は、標準曲線を用いたTBI標本のCSFおよび血液におけるALDOCおよびBLBPのレベルの抗体を基にした定量的評価を提供する。図14における箱プロットは、ALDOC(左側)およびBLBP(右側)ならびにこれらのCSFおよび血液(血清および血漿)における量の中央値ならびに四分位間濃度範囲を示す。挿入図は、2つの濃度範囲およびイムノブロット検出条件を用いたアイソフォーム特異的組換えタンパク質ALDOCおよびBLBPの既知の量を用いた用量応答を示す(表7を参照されたい)。
本実施例は、アストログリア外傷マーカーが重度TBIに罹患している患者から得た試料を用いた血液検査と適合性があることを実証する。図15Aは、1つの対照血漿試料(Crl)とともに3名の異なる重度TBI患者からの縦断的血漿試料を示す。多量のタンパク質がイムノアフィニティアルブミンおよび免疫グロブリン枯渇カラム(Sigma,ProteoPrep)を用いて回収される。25kDaの新たなGFAP分解産物は、損傷当日一度も検出されなかった(i)が、損傷後のその後の日々において出現した。ALDOCは一貫して、3名の患者全員におけるすべての時点において存在していた。短命マーカーのPEA15およびBLBPは、損傷当日に頑強に存在しており、各患者におけるその後の損傷後の日々にわたって異なる時間特性を示した。最多11の対照血液試料(Crl)と比較される22名の重度TBI患者から得た26の血清試料および24の血漿試料由来のALDOC(B1)、BLBP(B2)、GFAP/25kDaのBDP(B3)およびPEA15(B4)についての目盛り付きデンシトメトリーシグナルを図15Bにプロットする。縦断的な同一患者のデータを灰色の線によって結ぶ。ALDOCは、損傷当日および損傷後の毎日で有意に上昇したのに対し、GFAPは損傷翌日に有意に高い開始となった。BLBPおよびPEA15は、対照レベルに対して損傷当日は有意に高かった。
本実施例は、縦断的な重度TBI血清試料を用いて、ALDOC対GFAPの拡張した検出ウィンドウを実証する。ALDOCは、GFAPの25kDaのBDPの最初の検出の19時間前に検出され、ALDOCシグナルは、37kDaの公知のGFAPのBDPである特異的GFAPシグナルの最後のものよりも2日間以上長く存在していた(図17)。
全長の15kDaのBLBPに加えて、3kDaのBLBP特異的断片を、損傷当日および種々の損傷後の日々におけるTBIのCSFおよび血漿における2つの抗体を用いて検出した。結果を図17に示す。
図18は、損傷した血液脳関門を通る直接的な通過による新たなアストログリア損傷マーカーの急性循環様相についての証拠を提示する。図18のパネルAは、TBI直後(損傷3時間後)および損傷翌日の同じ重度TBI患者のCSFおよび血清中のALDOC、BLBPおよびPEA15とともにGFAPの25kDaのBDPについてのイムノブロットを示す。GFAPは、CSFにおいて最初に、および血清中で1日遅れて現れるのに対し、ALDOCは、両時点で両方の生体液中に存在していた。BLBPおよびPEA15は、血清中に最初に存在し、CSFに遅れて出現した。4つのアストログリアマーカー全部の対数空間軸におけるトレースは、CSFから血清へのGFAPの25kDaのBDPについての切り替え、生体液で安定したALDOCのより定常の存在、ならびに血清中での短命のBLBPおよびPEA15の存在の後のCSFにおける出現および遅延した上昇を提供する(図18B)。
本実施例は、軽度TBI患者の血清における最高レベルのアストログリア損傷マーカーの頑強なおよび早期の検出を実証する。損傷後早期の7名の軽度TBI(mTBI)患者由来の10個の血清試料を1個の対照血清(Crl)とともに図19に示す。試料を、GFAP、ALDOC、BLBPおよびPEA15についてプローブ付けした。特異的GFAPシグナルはかすかであり、1名の患者が損傷31時間後までにGFAP/25kDaのBDPを示す(患者番号II)4名のmTBI患者に制限された。ALDOCの38kDaのBDPは、対照に対してmTBI患者全員において一貫しておよび強く高かった。BLBPおよびPEA15は、変化し得る強度を示し、5名のmTBI患者において存在していた。ALDOC、BLBPおよびPEA15は、損傷1時間後にすでに検出された。脳震盪患者はコンピュータ断層撮影(CT)走査を受け、病変/出血に関して陽性の所見を有するものはCT+、おそらくmTBI合併症患者であり、可視的な創傷を有さない患者はCT陰性であり、または合併症がないものとして分類された(Bukiら,2015)。ALDOCの頑強な存在ならびにBLBPおよびPEA15についての差次的なシグナルは、脳震盪患者における危険の識別を補強するのに適している。
図20は、小児TBI患者の血清試料におけるALDOCの急性のおよび頑強な検出を示す。TBIに罹患している5名の乳児(1〜4か月齢)由来の損傷当日の血清試料を、22か月齢の子供由来の対照血清の隣に示す。頑強なALDOCシグナルは、対照症例に対して乳児TBI全員において検出されるが、2名の乳児のみが弱いGFAPシグナル(34kDaのBDP、未説明、症例番号I、III)を示した。
本実施例は、アルツハイマー病の慢性神経変性容態に罹患している患者に由来するCSF試料が全長(40kDa)および38kDaのBDPのALDOCの等しく異なるレベルを呈することを実証する。対照的に、急性TBI患者のCSFにおいて検出されたALDOCは、38kDaのBDPに対して全長(40kDa)の明確な優勢を示す。1名の重度TBI患者(症例番号I)および2名の齢を一致させた対照(症例番号VIおよびVII)および負荷のためのタンパク質染色(ポンソーS)と比較した5名のアルツハイマー病患者(AD、病期0.5、症例番号IIおよびIII;病期1、症例番号IVおよびV、Faganら,Science Transl.Med.2014を基にしている)由来の7個のCSF試料を図21に示す。TBI患者は、損傷4日後に主として全長のALDOCおよびわずかな38kDa断片を有していたのに対し、慢性変性AD試料は、全長および38kDaのALDOCのBDPの等しい存在を示していた。本データは、急性および慢性の脳損傷が、ALDOC/ALDOCのBDPの比を基にして識別することができることを示唆する。
多変量分類ツリー分析(Breiman,1984)を用いて、損傷に関する対照および生き残るまたは生き残らないTBI患者へと被験者コホートを最も正確に分割するマーカーを判定し、各々30μlのCSF試料のイムノブロット法によって分析した。AldoCおよびPTGDSの2つのマーカーのみを用いて100%の正確性を得る際に、(表3)、当該マーカーのみが将来の非侵襲性アッセイのために血中で検出可能であることを知るために本解析を反復した(表4)。表3に提示されるデータに使用するマーカーは、Aldo CおよびPTGDSであったのに対し、考慮されたが使用されなかったマーカーは、GFAP、PEA15、GFAP下位、S100B,BLBP、GS、APOB、PTGDS、およびAldoCの38kDであった。表3はしたがって、TBIの検出および生き残る者の転帰の予測を、分類ツリー解析におけるマーカー全部を用いて要約する。この解析は、ALDOCおよびPTGDSを最良の区別マーカーとして選択した。検査されるマーカー全部のうち、当該マーカーを算術的な教師なし学習アプローチによって利用した。複数の群、n=21、対照全部、および30TBI試料の損傷全日のTBIとした。示される7ng/30μlのCSFは、イムノブロット法を用いると、233ng/mlのCSFの濃度と等しい。群分けおよび予測された転帰は100%正確に符合するので、正確性は優れていた。図22は、表3の閾値の区別の説明を提供する。
二次的なTBI進行は、二次損傷としばしば関係する頭蓋内圧(ICP)の上昇を生じる可能性がある。細胞死滅マーカーGFAP下位BDP(19、20、25kDa二重線)のレベルとICPとの有意な相関が観察される。出血マーカーAPOBは、実質外+実質内病変体積および正中線偏位と有意に相関する。これらのCT所見は両方とも、硬膜外血腫、硬膜下血腫、くも膜下出血および実質内病変を含む脳の出血と関係する。グリア損傷マーカーBLBPおよびPEA15は、細胞死グリアマーカーGFAP下位BDPと同様に、脳組織挫傷、頭蓋内出血およびびまん性軸索損傷を含む実質内病変と相関する。
図24は、反復性軽度損傷についてのモデルを用いて得られた結果を示す。ヒト星状細胞は、30分間隔(30’)または1日間隔(1D)の単回(●)または二重の(●−●)軽度圧力パルスを受けた。急性漏出性および遅延性の死滅した細胞集団は、反復性外傷によってほとんど変化しなかった(図24A)。また、外傷放出マーカーGFAPおよびAldoCの条件培地(CM)流体レベルは、単回軽度伸張と比較して、直ちに追随する反復性軽度損傷の後に上昇し、2回の損傷が1日間隔である時、わずかに上昇するのみであった。AldoCレベルは、直ちに追随する軽度伸張の後に単回の重度損傷(●)のものにほぼ到達し、反復性損傷に対するAldoCレベルの感度を示した。
DAKO抗GFAPポリクローナル抗体を用いた条件培地試料における損傷48時間後のGFAP、上位および下位分解産物の2回のフィルム曝露を図25に示す。重度に伸張したおよび未伸張の培養物は、薬剤、カルパイン阻害薬PD150606(100μM、「Cali」)またはパンカスパーゼ阻害薬Z−VAD FMK(8.8μM、「Casi」)のいずれも受容しなかった。外傷放出性GFAP上位および下位BDPは、カルパインおよびカスパーゼの両方の阻害によって減少し、損傷後のGFAPを分解する両酵素の外傷誘発性活性化を示唆した。同じ実験の細胞可溶化物画分の類似の分析によって示唆されるように、この酵素分解の一部は細胞において生じ、一部は細胞外で生じる。未伸張培養物における下位GFAPのBDPの微量放出は、少数の非外傷性細胞死によるものであった。
TBI損傷バイオマーカータンパク質の生体液濃度を、標的指向化多重反応モニタリング(MRM)質量分析によって測定した。生体液試料をまずエンドプロテイナーゼであるトリプシンを用いて消化し、タンパク質全部をそれらの個々のトリプシン処理ペプチドへと切断した。タンパク質特異的ペプチドシグナルを、それらの個々のタンパク質に対する代用基準として使用した。MRM−MSにおいて、ペプチドシグナルを、以下の表7に示すような前駆体→生成物イオン遷移として公知のものによって測定する(例えば、554.821(2+)→924.514(1+,y8))。関心対象の特異的前駆体イオンの選択は、増感を可能にする。選択された前駆体由来の特異的生成物イオンからシグナルを測定することによって、MRMは、高度の分析物特異性を可能にする。定量化のため、重リジン[K(標識:13C(6)15N(2))]または重アルギニン[R(標識:13C(6)15N(4))]のいずれかを含有する定義した量の安定同位体標識した標準物質(SIS)ペプチドを生体液試料中へとスパイクする。これらの重標準物質ペプチドは、それらの内在性(軽)対応物と化学的に同一であるが、内在性生体液ペプチドとの識別のために、+8および+10Da(それぞれKおよびR)の質量移行を示す。軽および重MRM移行間のピーク面積比の比較は、バイオマーカー濃度の絶対的な定量化を可能にする。本発明者らのアッセイのために、トリプシン消化した生体液をまず、0.1%ギ酸水溶液および0.1%ギ酸アセトニトリル溶液の溶出系を用いる逆相液体クロマトグラフィーによって分離し、試料の複雑性をさらに低下させ、シグナル感度を改善した。

Claims (29)
- (a)対象から得た体液の標本を、アルドラーゼC(ALDOC)および脳脂質結合タンパク質(BLBP/FABP7)、またはALDOCもしくはBLBP/FABP7の外傷特異的分解産物(BDP)から選択されるTBIのマーカーについてアッセイするための試薬と接触させること、
(b)対照試料と比較して前記標本中に存在するマーカーの量を測定すること、ならびに
(c)前記対照試料と比較したマーカーの量の増加が前記標本中に存在する時のTBIまたはSCIの存在を判定すること
を含む、前記対象における外傷性脳損傷(TBI)および/または脊髄損傷(SCI)の状態を検出またはモニターするための方法。 - 前記TBIのマーカーは、ALDOCまたはそのBDP、およびBLBPまたはそのBDPである、請求項1に記載の方法。
- グルタミンシンテターゼ(GS)、星状細胞リンタンパク質PEA−15(PEA15)、αB−クリスタリン(CRYAB/HSP27)、ALDOC、GS、PEA15、またはCRYABの外傷特異的タンパク質分解性切断産物、あるいはこれらのうちの2つ以上からなる任意の組み合わせの量を測定することをさらに含む、請求項1に記載の方法。
- ALDOCの前記外傷特異的タンパク質分解性切断産物が、38kDa断片、35kDa断片、30kDa断片、および23kDa断片からなる群から選択される、請求項1に記載の方法。
- GSの前記外傷特異的タンパク質分解性切断産物は、37+35kDa二重線、32kDa断片、23kDa断片、20kDa断片、および18kDa断片からなる群から選択される、請求項3に記載の方法。
- PEA15の前記外傷特異的タンパク質分解性切断産物は、12+13kDa二重線および8kDa断片からなる群から選択される、請求項3に記載の方法。
- αB−クリスタリンの前記外傷特異的タンパク質分解性切断産物は、18+19kDa二重線、17kDa断片、15+14kDa二重線および8kDa断片からなる群から選択される、請求項3に記載の方法。
- 前記対象から得た脳脊髄液(CSF)試料中の血液特異的タンパク質の量を測定することをさらに含む、請求項1に記載の方法。
- 前記血液特異的タンパク質は、アポリポタンパク質B(APOB)である、請求項8に記載の方法。
- 前記対象から得た脳脊髄液(CSF)試料中のプロスタグランジンシンターゼ(PTGDS)の量を測定することをさらに含み、およびPTGDSの量が減少した時、TBIの存在が判定される、請求項1に記載の方法。
- グリア線維性酸性タンパク質(GFAP)の20〜30kDaのBDPの量を測定することをさらに含む、請求項1に記載の方法。
- ステップ(a)の前記試薬は、前記TBIのマーカーを特異的に結合する抗体を含み、および前記測定することはイムノアッセイを含む、請求項1に記載の方法。
- 前記イムノアッセイは、ウェスタンブロット法、またはELISAを含む、請求項12に記載の方法。
- 前記対照試料は、前記対象から得た損傷前試料である、請求項1に記載の方法。
- 前記対照試料は、健常対象者の対照コホートから得た平均値である、請求項1に記載の方法。
- 追加のマーカーはアッセイされない、請求項1〜15のいずれか一項に記載の方法。
- 4以下のマーカーがアッセイされる、請求項1〜16のいずれか一項に記載の方法。
- 前記体液の標本は、血漿、血清、脳脊髄液(CSF)、鼻水、耳垢、尿、唾液、涙、または脳微小透析液を含む、請求項1〜17のいずれか一項に記載の方法。
- 前記試薬は、タンパク質配列およびタンパク質配列断片特異的ペプチドを含み、ならびに前記測定することにおいて標的指向化定量的質量分析を含む、請求項1に記載の方法。
- 前記測定することは、多重または併行反応モニタリング質量分析を含む、請求項1に記載の方法。
- バイオマーカーのセットを特異的に結合する試薬を含むキットであって、前記バイオマーカーは、
(a)アルドラーゼC(ALDOC)、および
(b)脳脂質結合タンパク質(BLBP)
を含み、この中で前記試薬はポリヌクレオチドまたは抗体であり、前記試薬は、検出可能なマーカーで任意に標識され、およびこの中で、前記キットは任意に、前記試薬を収容するための少なくとも1つの容器および/または検査試料における外傷性の脳もしくは脊髄損傷の状態を判定するための前記試薬の使用のための説明書からさらになる、前記キット。 - (c)星状細胞リンタンパク質PEA−15(PEA15)、および/または
(d)グリア線維性酸性タンパク質の20〜30kダルトン断片(GFAP−BDP)
を特異的に結合する試薬をさらに含む、請求項21に記載のキット。 - 前記抗体は、モノクローナル抗体である、請求項21に記載のキット。
- 前記バイオマーカーのセットは、最多4のバイオマーカーからなる、請求項21に記載のキット。
- (a)対象から得た血清試料を請求項21に記載のキットと接触させること、および
(b)前記バイオマーカーに対する前記試薬の結合を測定すること
を含む、前記血清試料におけるバイオマーカーALDOCおよびBLBPの発現を判定する方法。 - (a)対象から得た血清試料を請求項21に記載のキットと接触させること、および
(b)前記バイオマーカーに対する前記試薬の結合を測定すること、
(c)前記結合を対照試料と比較すること、ならびに
(d)ALDOCおよびBLBPに対する前記試薬の結合が、前記対照試料に対して前記対象由来の前記血清試料において亢進する場合に存在していることになっているTBIを判定すること
を含む、前記血清試料における外傷性脳損傷の状態を判定する方法。 - 対象由来の体液の標本を、対照試料と比較したALDOCおよびBLBPの量の増加についてアッセイすることを含む、前記対象におけるTBIを検出する方法であって、ALDOCおよび/またはBLBPの量の増加はTBIを示す、前記方法。
- 前記アッセイすることは、推定損傷の24時間以内に実施される、請求項27に記載の方法。
- 前記対象は、乳幼児または児童である、請求項27または28に記載の方法。
