Abstract
Description
本発明は、多段階プロセスで生成されることが好ましい、活性物質を含有する浴用溶液、特にNOを含有する浴用溶液の、生成およびシャワー適用に関係した医療用入浴機器に関する。本発明はまた、このNO製造プロセスを用いて疾患、特に糖尿病に起因する循環障害および下肢の創傷の処置に使用されることが可能な、入浴機器に関する。
(a)1つ以上の身体四肢を収容するための処置チャンバ;
(b)活性物質を含む浴用溶液を調製するための反応容器;
(c)活性物質を含有する浴用溶液を圧送しかつ/または循環させるためのシステム;
(d)シャワー装置;および
(e)使用済みの浴用溶液を保持するための容器
ここで、反応容器内で生成した活性物質を含む浴用溶液は、循環/ポンプシステムによってシャワー装置へと輸送される。
(a)活性物質を含有する浴用溶液を生じさせるために水連結部に直接連結するかまたはパイプラインを介して連結することができる、反応容器;および
(b)活性物質を含有する浴用溶液を分注するための可搬式シャワーヘッド;を備え、
シャワーヘッドは、パイプラインを介して反応容器に連結されているかまたは反応容器を備えている、医療用入浴機器である。
1.反応容器を水連結部に直接取り付けることができる;
2.反応容器を、パイプラインを介して、好ましくはホースを介して、水連結部に連結することができる;または
3.反応容器をシャワーヘッドに組み込んでもよい。
本発明によれば、身体または身体部分を処置する入浴機器はシャワー装置を備える。
NOを含有する入浴機器の溶液は、酸化窒素(NO)を生成させるプロセスであって、下記ステップ:
(a)少なくとも1つのpH不安定性NO供与体を含むキャリア媒体を提供すること;
(b)キャリア媒体のpH値を、NOの形成時に少なくとも1つのpH不安定性NO供与体の分解を引き起こすpH値に調節すること;
(c)NOの形成を引き起こすpH値を、生理学的に妥当なNO量の形成を可能にする期間にわたって維持すること;
(d)キャリア媒体のpH値を上昇させること;
(e)場合により、ステップ(d)または次のステップ(e)においてさらなる少なくとも1つの追加の酸化防止剤を添加すること;
を含み、ステップ(a)においてキャリア媒体が少なくとも1つの酸化防止剤をさらに含有するか、または少なくとも1つの酸化防止剤をステップ(b)において添加する、プロセスを用いて、反応容器内で調製されることが好ましい。
浴用溶液としては、NOを吸収することができ、かつそれを再び放出することもできる、任意の液体を使用することができる。好ましくは、浴用溶液は水性液体である。
pH不安定性NO前駆体(NO供与体、NOD)は、現行技術において既知であり、当業者によく知られている。
NO発生時に生じる多重酸化された窒素酸化物、酸素ラジカルアニオン、またはヒドロキシルラジカルを除去するためには、浴用溶液は少なくとも1つの酸化防止剤を含む必要がある。
モル[NO供与体]<モル[酸化防止剤I]<モル[酸化防止剤II]
に従うモル比を有することが有益である。
pH不安定性NO供与体の開裂のためには液体を酸性pH値にする。本発明によれば、このpH値は、pH不安定性NO供与体の開裂を引き起こしてNOを形成する程に低い。具体的なpH値は、NO供与体のpH不安定性およびNO発生の所望の期間に依存する。pH値が低いほど、浴用溶液中にNOがより速く発生するであろう。
本発明の好ましい実施形態では、ステップ(b)でのNO発生は、光潜在性酸の活性化によって開始され、それは、電磁波の照射によって酸を遊離させ、したがって液体を酸性にする。これは、外部から酸を反応系に添加する必要がないが、浴用溶液中に存在している物質によって酸性化を引き起こせる、という点で好都合である。
ステップ(c)による一次NO発生の下流プロセスであるステップ(d)では液体のpH値を上昇させる。このpH値上昇は、本発明に従って、塩基もしくは塩基性緩衝系の添加または光潜在性塩基の光活性化によってもたらすことができる。
(a)pH7.0以上へのpH値上昇;
(b)少なくとも1のpH段のpH値上昇;
(c)新たに形成したNOの量が浴用溶液中でのNO減少の程度に相当するようにNO発生の低減と関連付けられたpH値へのpH値上昇。
好ましい実施形態では、pH値上昇のために光潜在性塩基を使用し、それは電磁放射線による照射によって塩基を遊離させ、それによって浴用溶液(液体であることが好ましい)におけるpH上昇をもたらす。そのような光潜在性塩基は、ここでも外部から塩基を系に添加する必要はないが、本発明に従って場合によって使用する(UV)光源が外部からpH値偏移を誘発することができる、という利点を有している。
本発明の一実施形態では、浴用溶液、特に水性液体としてのその実施形態は、1つ以上の薬理活性物質を含有する。これらは、NOの薬理作用を支援することができ、またはそれぞれの疾患を考慮した治療上妥当な様式でNOとは無関係に作用することができる。
本発明の好ましい実施形態では、ステップ(d)またはそれに続くステップ(e)においてpH値上昇と共に少なくとも1つの酸化防止剤を添加する。
本発明のさらなる実施形態では、ステップ(d)または(e)によるプロセスにおいて、NO供与体を光分解させてNOを形成させるために浴用溶液を光で照射する。その後の光分解的NO発生は、(NOのさらなる反応/分解および浴用溶液からの放出が累加することによる)NO含有量の減少が光分解的NO発生によってもたらされ、引き起こされる再生が、浴用溶液へのさらなる物質の添加を必要としない滑らかな様式で補償されることができ、さらに、照射継続時間および/または照射強度に対してNO発生の程度が簡単に制御される、という利点を有する。
本発明によれば、この方法において光源を使用することができる。
したがって、特定の態様において、本発明は、疾患の処置または防止において使用するのに適した、患者の少なくとも1つの身体四肢をNO含有浴用溶液に曝露する入浴機器を提供する。
1.)開放創に対して、というのも、驚くべきことに本発明において規定されるその適用は皮膚刺激をもたらさないことが判明したからである;
2.)危険性のある患者のMRSAの予防のために;または
3.)従来の抗生物質との相乗的用途として、というのも、驚くべきことに従来の抗生物質はNO作用の結果として残留炎症と効果的に闘うことができることが示されたからである。
本発明によれば、「処置」という用語は、疾患の症候もしくは原因を緩和もしくは完全に抑制さえするかまたは疾患の発症の妨害、遅延もしくは延期をする役目を果たす本発明に係る機器の、個体に対するいかなる適用をも意味する、と理解されるべきである。
(a)1つ以上の身体四肢を受け入れるための処置チャンバ;
(b)活性物質を含有する浴用溶液を生成させるための反応容器;
(c)活性物質を含有する浴用溶液を圧送しかつ/または循環させるためのシステム;
(d)シャワー装置;および
(e)使用済みの浴用溶液を受け入れるための容器を備え;
反応容器内で生成した活性物質を含有する浴用溶液は、循環/圧送システムによってシャワー器械へと輸送される、医療用入浴機器に関する。
(a)活性物質を含有する浴用溶液を生成させるために水道栓に直接連結するかまたはラインを介して連結することができる反応容器;および
(b)活性物質を含有する浴用溶液を送達するための可搬式シャワーヘッド;を備え、
シャワーヘッドはラインを介して反応容器に連結されているかまたは反応容器を備えている、医療用入浴機器。
(a)少なくとも1つのpH不安定性NO供与体を含む浴用溶液を提供すること;
(b)浴用溶液のpH値を、少なくとも1つのpH不安定性NO供与体の分解を引き起こしてNOを形成するpH値に調節すること;
(c)NO誘導pH値を、生理学的に妥当なNO量の形成を可能にする期間にわたって維持すること;
(d)浴用溶液のpH値を上昇させること;
(e)少なくとも1つのさらなる酸化防止剤を添加すること;
を含み、ステップ(a)において浴用溶液が少なくとも1つの酸化防止剤をさらに含有するか、または少なくとも1つの酸化防止剤をステップ(b)において添加する、医療用入浴機器。
(a)少なくとも1つのpH不安定性NO供与体を含む浴用溶液を提供すること;
(b)浴用溶液のpH値を、少なくとも1つのpH不安定性NO供与体の分解を引き起こしてNOを形成するpH値に調節すること;
(c)NO誘導pH値を、生理学的に妥当なNO量の形成を可能にする期間にわたって維持すること;
(d)浴用溶液のpH値を上昇させること;
を含む、医療用入浴機器の使用。
1.1 材料:
− Eco physics CLD 822:NOの定量
− 反応チャンバ:石英ガラス、約100x100x10mm(容量約100ml)
− 緩衝溶液 蒸留水中に150mMの酢酸、150mMのNaOH
− 塩基:1MのNaOH
− L−アスコルビン酸ナトリウム
− 1MのNaNO2
0.56gのL−アスコルビン酸ナトリウムを98.6mlの緩衝溶液に溶解し、反応チャンバに移し、1.4mlのNaNO2(1M)を添加した。したがって、亜硝酸ナトリウム濃度は14mMとなり、アスコルビン酸塩濃度は28.3mMとなった。最終溶液はpH値5.0と測定された。
反応時間の関数としてのNO測定の結果を図1に示す。NO濃度の連続的増加を認めることができ、60分後には液体中での1.11mMの濃度に相当する値に達した。
この実験の目的は、pH値を能動的に変化させることにより、長期間にわたって治療上妥当な最終濃度を達成することであった。
実施例1と同じ材料を使用した。
最初に、実験1と同様に、0.56gのL−アスコルビン酸ナトリウムを98.6mlの緩衝溶液に溶かし、これを反応チャンバに移し、1.4mlのNaNO2(1M)を添加した。したがって、亜硝酸ナトリウム濃度は14mMであり、アスコルビン酸塩濃度は28.3mMであった。最終溶液はpH値5.0と測定された。
初めに、NO濃度の連続的な増加が観察され、ここでは10〜15分の期間にわたって1.5mlのNaOH(1M)を反応チャンバに徐々に加え、それによって最終的にpH5.6のpH値に変化した。NO濃度はまず最初に低下し、t=20分には250±50μMの値に落ち着く。

Claims (23)
- 活性物質を含有する液体浴用溶液で身体抽出物を処置するための医療用入浴機器であって、
(a)1つ以上の身体四肢が入るのを可能にするために上部が開放された処置チャンバ;
(b)活性物質を含有する該浴用溶液を生成させるための反応容器;
(c)該活性物質を含有する該浴用溶液を圧送しかつ/または循環させるためのシステム;
(d)シャワー装置;および
(e)該処置チャンバに液体連通様式で連結されている、使用済みの該浴用溶液を受け入れるための容器または装置を備え、
該反応容器内で生成した該活性物質を含有する該浴用溶液は、循環/圧送システムの中を通ってホースライン(5)を介して該シャワー装置へと輸送される、医療用入浴機器。 - 前記使用済みの浴用溶液を収容するための容器が前記反応容器に相当する、請求項1に記載の医療用入浴機器。
- 前記反応容器と前記処置チャンバとが、独立した容器として液体ラインを介して互いに連結されている、請求項1または2に記載の医療用入浴機器。
- 前記反応容器が、前記浴用溶液のための少なくとも1つの入口と1つの出口とを有する密閉容器として構成されている、請求項1〜3のうちの1項に記載の医療用入浴機器。
- 前記反応容器が、UV光源であることの好ましい1つ以上の光源をさらに備える、請求項1〜4のうちの1項に記載の医療用入浴機器。
- 前記シャワー装置が、前記処置チャンバとは別に構成されていることの好ましい可搬式シャワーヘッドである、請求項1〜5のうちの1項に記載の医療用入浴機器。

- 前記浴用溶液が、前記処置チャンバから、前記使用済みの浴用溶液を回収するための容器または器械の中へと重力によって流出する、請求項1〜9のうちの1項に記載の医療用入浴機器。
- 前記処置チャンバと、前記使用済みの浴用溶液を回収するための容器または装置との間の前記液体ラインが、圧送装置を備える、請求項1〜9のうちの1項に記載の医療用入浴機器。
- 前記処置チャンバと、前記使用済みの浴用溶液を受け入れるための容器または装置との間の前記液体ラインが、該使用済みの浴用溶液を純化するためのフィルタ装置および/または吸収装置を備える、請求項1〜11のうちの1項に記載の医療用入浴機器。
- 前記使用済みの浴用溶液を吸収するための容器または装置が、超吸収性材料を含むことを特徴とする、請求項1〜12のうちの1項に記載の医療用入浴機器。
- 前記使用済みの浴用溶液を吸収するための装置が、前記入浴機器とは別の処分ユニットへの移送のための液体ラインである、請求項1〜13のうちの1項に記載の医療用入浴機器。
- 前記入浴機器が、床側にローラーまたは車輪を有する、請求項1〜14のうちの1項に記載の医療用入浴機器。
- 活性物質を含有する浴用溶液を含む、身体四肢の処置のための医療用入浴機器であって、
(a)活性物質を含有する水溶液を生成させるために水道栓に直接連結するかまたはラインを介して連結することができる反応容器;および
(b)該活性物質を含有する該浴用溶液を吐出するための可搬式シャワーヘッド;を備え、
該シャワーヘッドはラインを介して該反応容器に連結されているかまたは反応容器を備えている、医療用入浴機器。 - 活性物質を含有する前記浴用溶液が、酸化窒素(NO)を含有する浴用溶液である、請求項1〜16のうちの1項に記載の医療用入浴機器。
- NO含有浴用溶液を生成させるための、請求項1〜17のうちの1項に記載の医療用入浴機器の使用であって、該NO含有浴用溶液を生成させる方法は、下記ステップ:
(a)少なくとも1つのpH不安定性NO供与体を含む浴用溶液を提供すること;
(b)該浴用溶液のpH値を、少なくとも1つのpH不安定性NO供与体の分解を引き起こしてNOを形成するpH値に調節すること;
(c)NO誘導pH値を、生理学的に妥当なNO量の形成を可能にする期間にわたって維持すること;
(d)該浴用溶液のpH値を上昇させること;
を含む、医療用入浴機器の使用。 - ステップ(d)または(e)の後に、前記NO供与体の光分解のための浴用溶液に光を照射してNOを形成する、請求項18に記載の医療用入浴機器の使用。
- 患者の少なくとも1つの身体四肢を、前記入浴機器から放出されるNOに曝露する、疾患の処置または防止において使用するための、請求項1〜17のうちの1項に記載の医療用入浴機器。
- 前記疾患が、神経因性疼痛、拡張蛇行静脈、虚血および血小板障害、アレルギー、皮膚感染、皮膚感染、アトピー性皮膚炎、特に神経皮膚炎、皮膚筋炎および尋常性天疱瘡;慢性糖尿病性神経障害性潰瘍、下腿部潰瘍、褥創などの創傷欠損;(PAD)、末梢動脈閉塞性疾患(PAD)、末梢動脈疾患(PAD)、二次治癒感染、皮膚移植での合併症、勃起不全、化膿性汗腺炎(acne inversa)、疣贅、おむつかぶれ、皮膚の炎症性および自己免疫性疾患(乾癬、皮膚炎、神経皮膚炎)、皮膚の皮膚感染、皮膚の細菌性、微生物性および寄生虫性の疾患(例えばリーシュマニア症)、股部白癬および鼠径部白癬、筋ジストロフィー、鎌状赤血球貧血、ならびに脱毛症を含む群から選択される、請求項20に記載の医療用入浴機器。
- 糖尿病患者の下肢の慢性創傷を処置するために使用される、請求項21に記載の医療用入浴機器。
