Abstract
Description
(1) 脊椎インプラントであって、
前面、後面、上面、及び下面を有する本体であって、椎弓切除術が実施された椎骨に対して、前記本体の第1の外側端部が前記椎骨の第1の切断された椎弓板端部の少なくとも一部を受承し、前記本体の第2の反対側の外側端部が前記椎骨の第2の反対側の切断された椎弓板端部の少なくとも一部を受承することにより、前記本体が前記椎骨の前記第1及び第2の切断された椎弓板端部にまたがるような設置位置に位置付け可能である、本体と、
前記本体の前記第1の外側端部に形成された第1の骨アンカー受容穴であって、前記本体が前記椎骨に対する前記設置位置に配置される時に、前記第1の骨アンカー受容穴を通して挿入される骨ねじが椎骨の第1の外側塊の内部に延びるような角度をなす、第1の骨アンカー受容穴と、
前記本体の前記第2の外側端部に形成された第2の骨アンカー受容穴であって、前記本体が前記椎骨に対する前記設置位置に配置される時に、前記第2の骨アンカー受容穴を通して挿入される骨ねじが椎骨の第2の反対側の外側塊の内部に延びるような角度をなす、第2の骨アンカー受容穴と、を備える、脊椎インプラント。
(2) 前記本体に形成された、受容ヘッドを選択的に連結することができる少なくとも1つの嵌合機構を更に備える、実施態様1に記載のインプラント。
(3) 前記少なくとも1つの嵌合機構が、前記本体の中心上下軸に沿って配置された中心嵌合機構を含むことにより、前記本体が椎骨に対する前記設置位置に配置される時に、前記中心嵌合機構は前記椎骨の正中線の上に位置付けられている、実施態様2に記載のインプラント。
(4) 前記少なくとも1つの嵌合機構が、前記本体の中心上下軸から外側にオフセットして位置付けられた第1及び第2の外側嵌合機構を含む、実施態様2に記載のインプラント。
(5) 前記少なくとも1つの嵌合機構が、前記本体に形成された螺刻穴を含む、実施態様2に記載のインプラント。
(7) 前記本体が、前記本体の中心上下軸を中心として湾曲していることにより、前記本体の前記前面は、前記本体が椎骨に対する前記設置位置に配置される時に、脊髄を保護するための湾曲した逃げ部を画定する、実施態様1に記載のインプラント。
(8) 前記本体に形成された前記少なくとも1つの嵌合機構に受容ヘッドを選択的に連結するように構成された嵌合機構を有する受容ヘッドを更に備える、実施態様2に記載のインプラント。
(9) 前記受容ヘッドが、多軸連結、一平面連結、及び単一軸連結の少なくとも1つを介して前記本体に連結されるように構成されている、実施態様8に記載のインプラント。
(10) 前記第1の骨アンカー受容穴は、前記本体の最も後方の区域が存在する平面に対して約120°〜約140°の範囲の角度で延びている、実施態様1に記載のインプラント。
患者の椎骨から第1及び第2の椎弓板を除去することにより、第1の切断された端部と第2の切断された端部とを形成することと、
椎弓板プレートの第1の終端部が前記椎骨の前記第1の切断された端部と接触し、前記椎弓板プレートの第2の終端部が前記椎骨の前記第2の切断された端部と接触するように、前記患者体内に前記椎弓板プレートを挿入することと、
前記椎弓板プレートの第1の骨ねじ穴を通して、前記椎骨の第1の外側塊の内部へと第1のねじを挿入することと、
前記椎弓板プレートの第2の骨ねじ穴を通して、前記椎骨の第2の外側塊の内部へと第2のねじを挿入して、前記椎弓板プレートを前記椎骨に連結することと、
前記患者体内に前記椎弓板プレートを挿入した後、前記椎弓板プレートに受容ヘッドを取り付けることと、を含む、方法。
(12) 前記第1及び第2の骨ねじが前記椎弓板プレートを貫通して挿入された後、前記受容ヘッドが前記椎弓板プレートに取り付けられている、実施態様11に記載の方法。
(13) 前記第1及び第2の骨ねじが前記椎弓板プレートを貫通して挿入された後、前記受容ヘッドが前記第1の骨ねじ及び前記第2の骨ねじの一方に取り付けられている、実施態様11に記載の方法。
(14) 脊椎の複数の連続した椎骨レベルに完全椎弓切除術を実施することと、前記連続した椎骨レベルのそれぞれに椎弓板プレートを連結することとを更に含む、実施態様11に記載の方法。
(15) 各椎弓板プレート内に受容ヘッドを取り付けることを更に含む、実施態様14に記載の方法。
(17) 前記安定化要素が、前記脊椎の正中線の上に位置付けられている、実施態様16に記載の方法。
(18) 前記安定化要素が、前記脊椎の正中線から外側方向にオフセットして位置付けられている、実施態様16に記載の方法。
(19) 前記第1の切断された端部と前記第2の切断された端部との間の距離が、約15〜約30mmの範囲である、実施態様11に記載の方法。
(20) 前記方法を実施する際に前記椎骨のわずかに約15〜約40mmが前記椎骨の正中線に対して外側方向に露出させられている、実施態様11に記載の方法。
第1のねじを椎骨の第1の椎弓板の内部に挿入することと、
第2のねじを前記椎骨の第2の椎弓板の内部に挿入することと、
前記第1及び第2のねじを挿入した後、前記第1及び第2の椎弓板の少なくとも一部を除去することにより、第1の切断された端部と第2の切断された端部とを形成することと、
前記第1及び第2のねじが前記第1及び第2の切断された端部からそれぞれ突出するように、前記椎骨から前記第1及び第2のねじを部分的に引き抜くことと、
椎弓板プレートの第1の終端部が前記椎骨の前記第1の切断された端部と接触し、前記椎弓板プレートの第2の終端部が前記椎骨の前記第2の切断された端部と接触するように、前記第1及び第2のねじに前記椎弓板プレートを取り付けることと、を含む、方法。
(22) 前記第1及び第2のねじに前記椎弓板プレートを取り付けた後、前記椎弓板プレートに少なくとも1つの受容ヘッドを取り付けることを更に含む、実施態様21に記載の方法。
(23) 前記第1及び第2のねじに前記椎弓板プレートを取り付けることは、前記椎弓板プレートを第1の圧縮位置から、前記プレートが前記ねじと係合する第2の拡張位置へと動かすことを含む、実施態様21に記載の方法。
Claims (10)
- 脊椎インプラントであって、
前面、後面、上面、及び下面を有する本体であって、椎弓切除術が実施された椎骨に対して、前記本体の第1の外側端部が前記椎骨の第1の切断された椎弓板端部の少なくとも一部を受承し、前記本体の第2の反対側の外側端部が前記椎骨の第2の反対側の切断された椎弓板端部の少なくとも一部を受承することにより、前記本体が前記椎骨の前記第1及び第2の切断された椎弓板端部にまたがるような設置位置に位置付け可能である、本体と、
前記本体の前記第1の外側端部に形成された第1の骨アンカー受容穴であって、前記本体が前記椎骨に対する前記設置位置に配置される時に、前記第1の骨アンカー受容穴を通して挿入される骨ねじが椎骨の第1の外側塊の内部に延びるような角度をなす、第1の骨アンカー受容穴と、
前記本体の前記第2の外側端部に形成された第2の骨アンカー受容穴であって、前記本体が前記椎骨に対する前記設置位置に配置される時に、前記第2の骨アンカー受容穴を通して挿入される骨ねじが椎骨の第2の反対側の外側塊の内部に延びるような角度をなす、第2の骨アンカー受容穴と、を備える、脊椎インプラント。 - 前記本体に形成された、受容ヘッドを選択的に連結することができる少なくとも1つの嵌合機構を更に備える、請求項1に記載のインプラント。
- 前記少なくとも1つの嵌合機構が、前記本体の中心上下軸に沿って配置された中心嵌合機構を含むことにより、前記本体が椎骨に対する前記設置位置に配置される時に、前記中心嵌合機構は前記椎骨の正中線の上に位置付けられている、請求項2に記載のインプラント。
- 前記少なくとも1つの嵌合機構が、前記本体の中心上下軸から外側にオフセットして位置付けられた第1及び第2の外側嵌合機構を含む、請求項2に記載のインプラント。
- 前記少なくとも1つの嵌合機構が、前記本体に形成された螺刻穴を含む、請求項2に記載のインプラント。
- 前記本体の前記上面は、前記本体が隣接する下位椎骨に対する前記設置位置に配置される時に、上位椎骨の棘突起を受容するように構成された、湾曲した逃げ部を画定する、請求項1に記載のインプラント。
- 前記本体が、前記本体の中心上下軸を中心として湾曲していることにより、前記本体の前記前面は、前記本体が椎骨に対する前記設置位置に配置される時に、脊髄を保護するための湾曲した逃げ部を画定する、請求項1に記載のインプラント。
- 前記本体に形成された前記少なくとも1つの嵌合機構に受容ヘッドを選択的に連結するように構成された嵌合機構を有する受容ヘッドを更に備える、請求項2に記載のインプラント。
