Abstract
Description
癌免疫療法は、患者の免疫系による癌細胞の攻撃を含む。Tリンパ球の制御及び活性化は、活性化のために正または負のシグナルを伝達するT細胞受容体、及び、さらに共シグナル伝達受容体によるシグナル伝達に依存する。T細胞による免疫応答は、免疫チェックポイントと呼ばれる共刺激及び阻害シグナルのバランスにより制御される。
本発明は、癌免疫療法の好ましい奏効(response)の可能性が予測することができるという発見を包含する。本発明は特に、特定の癌の場合、変異バーデン(burden)が、特定の療法の奏効性と、相関する可能性があるという発見を含む。さらに、本発明は、特定の癌細胞が、患者の免疫系により非自己として認識可能であるネオエピトープをもたらす体細胞変異を含むことがあり、このようなネオエピトープの存在及び/または同一性が、特定の療法の奏効性と相関することがあるという知見を提供する。本明細書に記載されるような癌サンプルにおける複数の変異の同定は、どの癌患者に、免疫療法、特に、免疫チェックポイント調節因子を用いる処置が有利に奏効する(respond)可能性があるかを判定するのに有用である。
本発明がより容易に理解されるために、特定の用語が以下に定義される。特定の用語が、本明細書の他の箇所で提供されてもよく、かつ/または文脈から明らかになるであろうことを、当業者らは理解するであろう。

後天的な(または体細胞)変異は、個体の生存期間中のある時に、細胞のDNAに生じる可能性がある。これらの変化は、日光からの紫外線、化学物質または煙草の煙中の発癌物質などの環境要因により引き起こされる可能性があるか、または、DNA複製中にミスが起きた場合に生じる可能性がある。多くの場合環境因子または変異への慢性的な曝露の結果である癌由来の癌細胞、例えば、肺癌またはメラノーマ、は多くの場合、様々なタイプの複数の変異を有する。

一部の実施形態では、本発明は、免疫チェックポイント調節因子を用いる処置が有利に奏効する可能性がある癌患者を同定するための方法を提供する。一部の実施形態では、本発明は、免疫チェックポイント調節因子を用いる処置が有利に奏効する可能性がある癌患者を同定し、かつ免疫チェックポイント調節因子で患者を処置するための方法を提供する。一部の実施形態では、本発明は、免疫チェックポイント調節因子を用いる処置が有利に奏効する可能性があると以前に同定された癌患者を、免疫チェックポイント調節因子で処置する方法を提供する。一部の実施形態では、本発明は、免疫チェックポイント調節因子を用いる処置が有利に奏効する可能性がない癌患者を同定し、かつ免疫チェックポイント調節因子で患者を処置しないための方法を提供する。一部の実施形態では、本発明は、免疫チェックポイント調節因子を投与された場合に、1つ以上の自己免疫性合併症を患う可能性がある癌患者を同定するための方法を提供する。一部の実施形態では、本発明は、免疫チェックポイント調節因子で処置された場合に、1つ以上の自己免疫性合併症を患う可能性があると以前に同定された癌患者を免疫抑制剤で処置するための方法を提供する。一部の実施形態では、免疫抑制剤は、免疫チェックポイント調節因子の前またはこれと同時に患者に投与される。
本発明の特定の方法によれば、免疫チェックポイント調節因子は、個体に投与されるか、または投与されている。一部の実施形態では、免疫チェックポイント調節因子を用いる処置は単独療法として利用される。一部の実施形態では、免疫チェックポイント調節因子を用いる処置は、1つ以上の他の療法と組み合わせて使用される。
今日、免疫系がヒト癌を拒絶することができることの最初の観察以来の1世紀を超え(1)、免疫チェックポイント阻害剤は、適応免疫が、癌の処置のために利用することができることを実証している(2−5、60、61)。進行した非小細胞肺癌(NSCLC)では、抗PD−1療法は、17〜21%の奏効率を実証しており、いくつかの奏効は、非常に持続的である(3、6)。
本実施例は、癌の変異の全体像の解析を説明し、免疫チェックポイント調節因子が有利に、または不十分に奏効する患者の有用な特質を規定することにおいてその効果を実証する。実施例は特に、PD−1遮断(例えば、ペンブロリズマブ)で処置された肺癌患者の解析を例示し、このような患者の例示的変異特性を規定する。
本実施例は、特定の体細胞変異シグネチャーが、免疫チェックポイント調節因子を用いる処置の有効性と関連することを実証する。
から生じたことを明らかにした(図4C)。特に、このT細胞応答は、療法の開始時に、検出レベル以下であった(0.005%の検出レベル)が、療法の開始後3週間以内に容易に検出可能なレベルまで増大し(CD8+T細胞の0.040%)、44日目に維持された(Cd8+T細胞の0.044%)。T細胞反応性のこの迅速な誘導は、腫瘍退縮と相関し、腫瘍退縮がプラトーに達したので、続く数ヶ月でバックグラウンドをわずかに上回るレベルに戻った(図4D)。44日目に収集されたPBL由来のHERC1 P3278S多量体反応性T細胞を、CD45RA−CCR7−HLA−DR+LAG−3−表現型により特性決定し、活性化されたエフェクター集団と一致した。(図20)。これらのデータは、抗PD−1療法の臨床奏効に照らして、癌ネオアンチゲンに対する自己T細胞応答を明らかにする。本発明者らの知る限りでは、これは、抗PD−1療法の臨床奏効に照らして、癌ネオアンチゲンに対する自己T細胞応答を明らかにするための、癌のエクソームデータの最初の使用である。
この実施例は、免疫原性ペプチドのインビトロ検証を実証する。
のいずれかで再刺激し、細胞内サイトカイン染色で分析した。両方の時点で、(IFNγ、CD107a、MIP1β(ケモカインCCL4)、及びTNFα産生を特徴とする)多機能性CD8+T細胞の実質的な集団は、野生型ペプチドではない変異体に応答して検出された(図4E、13A−D)。
本実施例は、本明細書で実施例1〜3に提示した作業のための詳細な材料及び方法を提供する。
全ての患者は、ステージIVの小細胞肺癌(NSCLC)を有し、プロトコールNCT01295827で、Memorial Sloan Kettering Cancer Center(n=29)またはロサンゼルスのthe University of California(n=5)において処置された(図17)。全ての患者は、2012〜2013に療法を開始し、3週間毎に2mg/kgで処置された5人の患者を除いて、2〜3週間毎に10mg/kgで処置された。総合奏効率及び無増悪生存期間は、投与量及びスケジュールを通じて類似していると報告されている(6)。全ての患者は、組織収集及び配列決定を可能にするInstitutional Review Boardに承認されたプロトコールに同意していた。予め検証されたマウス抗ヒト抗PD−L1抗体(clone 22C3、Merck & Co., Inc.)を使用して、免疫組織化学により、将来を見通してPD−L1発現を評価した。腫瘍細胞及び腫瘍内浸潤免疫細胞上のPD−L1の膜発現をスコア化した。31人(91%)は、PD−L1発現に関して少なくとも1%の陽性を得;2人の患者は、PD−L1陰性であり;1人は、不明であった。MSKCCのケアの基準として実行される予め完了した自己申告の喫煙質問表、またはUCLAでの医療記録のレビューを使用して、喫煙状態を評価した。この分析に適格である患者は全て、研究療法の少なくとも2つの投与量を受け、ペンブロリズマブの奏効を評価可能であり、毒性または同意の撤回により、早期に中止しなかった。
処置後の組織が使用された1人の非奏効者除き、ペンブロリズマブを投与する前に、配列決定に使用される全ての腫瘍組織を得た(研究ID DM123062)。全エクソーム配列決定に使用される腫瘍サンプルをパラフィン包埋した(FFPE)。末梢血を収集し、DNAを全ての患者から単離した(Nucleospin Blood L, Machery−Nagel)。胸部病理学者による代表的なヘマトキシリン・エオシン染色されたスライドの検査により、配列決定されたサンプル中の腫瘍組織の存在を確定した(N.R.またはA.M)。DNEasy kit(Qiagen)を使用して、DNA抽出を実施した。
研究放射線科医による研究者が評価した免疫関連応答基準(irRC)(37)により、ペンブロリズマブの客観的奏効を評価した。プロトコール毎に、CTスキャンを9週間毎に実施した。奏効の初期同定の少なくとも4週間後に行われる反復イメージングにより部分奏効及び完全奏効を確定し;未確定の奏効は、2回目のCTスキャンの結果によって決まる安定または進行と考えられた。持続的臨床効果(DCB)を、6ヶ月超続く安定疾患または部分奏効と規定した(27週目、第3のプロトコールでスケジュールされた奏効評価時)。療法の開始から6ヶ月以内の持続的効果なし(NDB)を、進行と規定した。データロック時(2014年10月10日)に依然として進行中の研究療法であったが、まだ6ヶ月の経過観察に到達していなかった患者を、「未到達」(NR)とみなした。これらの患者は、DCB/NDBの分析に含まれなかったが、客観的奏効及び無増悪生存期間の評価に含まれた。研究療法に対する進行中の奏効がある患者の場合、無増悪生存期間を、最新の画像評価のデータで打ち切った。生存患者の場合、全生存期間を、最後の知られている接触の日で打ち切った。
全エクソームキャプチャライブラリーを、the Broad in−solution hybrid selection processを用いるAgilent Sure−Select Human All Exon v2.0, 44Mb baited targetにより作製した。豊富なエクソームライブラリを、HiSeq 2000 platform (Illumina)で配列決定して、150×の平均標的カバレッジの目標に対してペアードエンドリード(2×76bp)を生成した(Broad Institute、マサチューセッツ州ケンブリッジ)(図14A〜14Q、図18)。
HLAタイピングを、高解像度SeCore HLA配列ベースのタイピング法(HLA−SBT)(Invitrogen)による、MSKCC HLA typing lab New York Blood Centerで実施した。ATHLATES(http://www.broadinstitute.org/scientific−community/science/projects/viral−genomics/athlates)(38)も、HLAタイピング及び確定のためにも使用した。
Burrows−Wheeler Aligner(BWA)バージョン0.7.10を使用して、未加工の配列決定データをhg37ゲノムビルドにアラインした(39)(図6)。Genome Analysis Toolkit(GATK)バージョン3.2.2を使用して、さらなる挿入欠失再アライメント、ベース質スコア再キャリブレーション、及び重複リード除去を実施した(40)。SnpEffectバージョン3.5d(2014−03−05構築)を使用して、変異に注釈をつけた(41)。Somatic Sniperバージョン1.0.0(42)、VarScanバージョン2.2.3(43)、Strelkaバージョン1.0.13(44)、及びMuTectバージョン1.4(45)を使用して、デフォルトパラメータを用いて一塩基多様体(SNV)コールを生成した。VarScan及びStrelkaを使用して、挿入欠失コールを生成した。ベースラインフィルタ(腫瘍被検査物の7×カバレッジの深度、>97%の正常対立遺伝子画分、>10%の腫瘍対立遺伝子画分)を選択した。1000ゲノムプロジェクト、ESP6500(National Heart,Lung and Blood Institute[NHLBI]GOエクソーム配列決定プロジェクト)、及びdbSNP132と比較することにより、既知の一塩基多型(SNP)を排除した(46−48)。SNPデータベースではまれであり、かつ正常DNAにおける対立遺伝子画分がゼロである腫瘍に存在する一塩基多型(SNP)は、Integrative Genomics Viewer(IGV)を使用して手動でレビューされ、体細胞SNVとして含まれた(49)。IGVを使用した手動のレビューを、次のさらなる3つのカテゴリーのいずれかにおける全ての変異についても行った:(i)1つのコーラーによりコールされた(ii)7×〜35×のカバレッジ(iii)10%未満の腫瘍対立遺伝子であるが、2つ以上のコーラーによりコールされた。snpEff(高)、SIFT(50)(スコア<0.05)、またはPolyPhen−2(「D」若しくは「P」)によりそのように示される場合、コールされたSNVを有害と評価した(51)。SIFT及びPolyphen2予測スコア及びGERP++保存スコアを、dbNSFPバージョン2.2から解析した(52)。検出された変異の検証再配列は、97%を超える(53)。
トリヌクレオチド配列構成内の非同義エキソン一塩基置換を分析することにより、各サンプルの変異スペクトルを計算した。つまり、各サンプルに対し、フランキングヌクレオチドの16の可能な組み合わせのそれぞれのうちの、6つの可能な一塩基変化(Watson−Crick塩基対のピリミジンを最初に参照した、C>A:G>T、C>G:G>C、C>T:G>A、T>A:A>T、T>C:A>G、T>G:A>C)のそれぞれのパーセンテージを計算して、そのサンプルについての変異スペクトルを表すために使用される96特徴ベクトルを生成した。本発明者らは、トランスバージョン少ない(TL)の腫瘍及びトランスバージョン多い(TH)の腫瘍を区別する二値分類器を生成するために、Support Vector Machine(R package e1071)を利用した。以前に発表された分析(14)と同様に、生涯にわたる喫煙未経験者及び対応する対照としてのパックイヤー60以上の喫煙歴を有する患者を使用して、分類器をトレーニングした。TCGAからの公に利用可能なエクソーム配列決定及び喫煙歴データならびに以前に発表された結果(54)から、トレーニングセットを得た。全てのサンプルを、TLまたはTHカテゴリーのいずれかに属するものとして分類するために、この分類器を、全ての配列決定された患者に適応した。
NAseekと呼ばれる生物情報科学ツールを使用して、同定した全ての非同義点変異を、中央に変異体アミノ酸が位置するアミノ酸17個の部分列に翻訳した(22)。スライディングウィンドウ法を使用して、患者特異的HLA対立遺伝子の1つ(それ以上)に対する500nM以下のMHCクラスI結合親和性を有した変異体17mer内のアミノ酸9個の部分列を同定した。NetMHC v3.4ソフトウエアを使用して、変異体及び対応する野生型ノナマーに対する結合親和性を分析した(23、24、55〜57)。
HLA−A拘束性候補ネオアンチゲンを組織内(Netherlands Cancer Institute)で合成し、これらのペプチドを含有するHLA多量体を、以前に記載されているように、マイクロスケールのパラレルUV誘発ペプチド交換反応により作製した(26、58)。要約すると、UV感受性ペプチドを負荷したペプチドMHC複合体を、384ウェルプレート中、候補ネオアンチゲンペプチドの存在下で、4℃1時間、366nmの紫外線(CAMAG)に曝露させた。合計11の異なる蛍光ストレプトアビジン(SA)コンジュゲート(Invitrogen)を使用して、pMHC多量体を生成した。各pMHCモノマーに対し、コンジュゲーションを、これらの蛍光色素のうちの2つを用いて実施した。残りの結合部位をブロックするために、NaN3(0.02% w/v)及び過剰のD−ビオチン(26.4mM、Sigma)を加えた。T細胞染色のために、コンビナトリアルエンコーディングストラテジーを用いて、並行して最大47の異なるペプチドに対する反応性を分析することができた(27)。PBMCサンプルを解凍し、DNAseで1時間処理し、pMHC多量体パネルで37℃15分間染色した。続いて、抗CD8−AF700(Invitrogen)、抗CD4−FITC(Invitrogen)、抗CD14−FITC(Invitrogen)、抗CD16−FITC(Invitrogen)、抗CD19−FITC(Invitrogen)、及びLIVE/DEAD Fixable IR Dead Cell Stain Kit(Invitrogen)を、氷上でさらに20分間加えた。FACSDiva 6ソフトウエアを用いたLSR IIフローサイトメーター(Becton Dickinson)で、データの取得を実施した。奏効の規定に対するカットオフ値は、全CD8+細胞の0.005%以上及び10以上の事象であった。免疫表現型分析のために、44日目のPMLを、2色のHERC1 P3278S MHC多量体(qdot 625(Invitrogen)及びPerCPeFluor710(ebioscience))と抗CD45RA Ab(Invitrogen)、抗CCR7 Ab(BD Bioscience)、抗HLA−DR Ab(BD Bioscience)ならびに抗LAG−3 Ab(R&D systems)で染色した。HERC1 P3278S反応性及びバルクCD8+T細胞の免疫表現型を分析した。FASCDiva 6ソフトウエアを用いるLSR IIフローサイトメーター(Becton Dickson)を使用して、データを取得した。
HERC1 P>S変異体及び長さがアミノ酸9個の野生型ペプチド(変異体:ASNASSAAK、野生型:ASNAPSAAK)を合成した(GenScript Piscataway、NJ)。以前に記載された方法を使用して、IL−15(10ng/ml)及びIL−2(10IU/ml)が補充された、10%のプールされたヒト血清(PHS)、10mMのHEPES、2mMのL−グルタミン、及び50μMのβ−メルカプトエタノールを含有するRPMI培地中の、HERC1 P>S変異ペプチドでパルスした1.5×106個の自己PBMCで、1.5×106個の患者PBMCを培養した(59)。12日目に細胞を採取し、3μLのPE−Cy5−CD107a(BD Pharmingen)で染色し、刺激しないままか、または(a)変異ペプチド若しくは(b)野生型ペプチドを2時間添加することにより刺激した。次に、細胞を1×のブレフェルジンA及びモネンシン(BioLegend)で4時間処置し、続いて1μLのAlexa Fluor 405−CD3(Invitrogen)、3μLのAPC−H7−CD8(BD Bioscience)、及び1μLのECD−CD4(Beckman Coulter)で染色した。後続の洗浄及び透過化処理の時、細胞を、細胞内サイトカインに対する次の抗体:3μLのAlexa Fluor 647−IFN−γ(Biolegend)、3μLのPE−MIP−1β、及び1μLのPE−Cy7−TNF−α(BD Pharmingen)で染色した。(CYANフローサイトメーター、Summit software、Dako Cytomation California Inc.、カルフォルニア州カーピンテリアを使用する)フローサイトメトリーにより、データを取得した。FlowJoバージョン10.1、TreeStar, Inc.により、解析を行い、CD3+単細胞リンパ球を、解析のためのゲーティングした(SS対FS[低、中]、FS対パルス幅[全、低]、及びCD3対「ダンプ」チャネル[高、低])。
Mann−Whitney検定を使用して、変異バーデン及びヌクレオチド変化の頻度の差を比較した。ログランク検定及びMantel−Haenszel検定を使用して、Kaplan−Meier生存曲線を比較した。Fisherの直接確率法を使用して、客観的奏効者/非奏効者またはDCB/NDBの割合を比較した。同じカットポイント(1−特異度)をも超えると考えられるNDB患者の割合に対する任意の所定のカットポイント(感度)を上回る変異バーデンを有する全てのDCB患者の割合をプロットすることにより、受信者操作特性(ROC)曲線を生成した。曲線下面積及び正確な95%の信頼区間が報告される。Spearman相関式を使用して、非同義的変異バーデン及びネオアンチゲンバーデンの間の相関、ネオアンチゲンバーデン及び最良総合効果の間の相関、ならびにネオアンチゲンバーデンの頻度/非同義変異及び最良総合効果の間の相関、を計算した。GraphPad Prism v.6(Graphpad Prism Software、カルフォルニア州サンディエゴ)を使用して、統計的解析を実施した。
本実施例は、転移性メラノーマの処置のために、米国食品医薬品局により承認された、抗体免疫療法(ペンブロリズマブ)を用いる癌(メラノーマ)の処置のための使用説明書を提供する。一部の実施形態では、長期臨床効果は、ペンブロリズマブ処置後に観察される。
KEYTRUDAは、イピリムマブ、及びBRAF V600変異陽性の場合はBRAF阻害剤の後に、切除不能または転移性メラノーマ及び疾患進行を有する患者の処置に適応されるヒトプログラム死受容体1(PD−1)遮断抗体である。この適応症は、腫瘍奏効率及び奏効の持続性に基づいた加速承認下で承認されている。生存または疾患関連症状の改善は、まだ確立されていない。この適応症に対する継続的な承認は、確定試験における臨床効果の検証及び記載に左右されることがある(1)。
3週間毎に30分間にわたって静脈内注入として2mg/kgを投与する(2.1)。
注射用:50mg、再構成のための単回使用バイアル中の凍結乾燥粉末(3)。

免疫介在性有害反応:反応の重症度に基づいてコルチコステロイドを投与する。(5.1、5.2、5.3、5.4、5.5、5.6)
(患者の20%以上で報告されている)最も一般的な有害反応としては、疲労、咳、悪心、かゆみ、発疹、食欲減退、便秘、関節痛、及び下痢が挙げられる。(6.1)
授乳中の母親:授乳を中止するか、またはKEYTRUDAを中止する。(8.3)
KEYTRUDA(登録商標)(ペンブロリズマブ)は、イピリムマブ、及びBRAF V600変異陽性の場合はBRAF阻害剤の後に、切除不能または転移性メラノーマ及び疾患進行を有する患者の処置に適応される[臨床研究(14)を参照のこと]。
2.1 推奨投与
KEYTRUDAの推奨投与量は、疾患進行または容認できない毒性まで、3週間毎に30分にわたり静脈内注入として2mg/kg投与される。
次のいずれかの場合に、KEYTRUDAを差し控える:
グレード2の肺炎[警告及び使用上の注意(5.1)を参照のこと]、
グレード2または3の大腸炎[警告及び使用上の注意(5.2)を参照のこと]、
症状がある下垂体炎[警告及び使用上の注意(5.4)を参照のこと]、
グレード2の肺炎[警告及び使用上の注意(5.5)を参照のこと]、
グレード3の甲状腺機能亢進症[警告及び使用上の注意(5.6)を参照のこと]、
正常上限(ULN)の3倍を超え、かつ5倍までのアスパラギン酸アミノトランスフェラーゼ(AST)若しくはアラニンアミノトランスフェラーゼ(ALT)、またはULNの1.5倍を超え、かつ3倍までの総ビルリビン、
他の任意の重症またはグレード3の処置関連有害反応[警告及び使用上の注意(5.7)を参照のこと]。
有害反応がグレード0〜1に回復した患者において、KEYTRUDAを再開する。
次のいずれかの場合に、KEYTRUDAを永続的に中止する:
任意の生命に危険を及ぼす有害反応、
グレード3または4の肺炎[警告及び使用上の注意(5.1)を参照のこと]、
グレード3または4の腎炎[警告及び使用上の注意(5.5)を参照のこと]、
ULNの5倍を超えるAST若しくはALT、またはULNの3倍を超える総ビリルビン、
グレード2のASTまたはALTで処置を開始する肝転移を有する患者に対し、ASTまたはALTがベースラインと比較して50%以上増加し、少なくとも1週間持続する場合、
グレード3または4の注入に関連する反応、
コルチコステロイド投与量を、12週間以内に1日当たりのプレドニゾンまたは同等物10mg以下に低減させることができないこと、
KEYTRUDAの最後の投与後12週間以内に、グレード0〜1に回復しない持続性のグレード2または3の有害反応。
調製
バイアルの壁に沿って、かつ凍結乾燥した粉末に直接ではなく、水を注入することにより、2.3mLの注射用滅菌水、USPを加える(得られる濃度25mg/mL)。
製品は、防腐剤を含有しない。以下のいずれかでKEYTRUDAの再構成及び希釈溶液を貯蔵する。
滅菌非発熱性低タンパク質結合性の0.2マイクロメートル〜5マイクロメートルのインラインまたはアドオンフィルタを含有する点滴ラインを通して、30分間にわたり静脈内に輸液を投与する。
注射用:再構成のための単回使用バイアル中の50mgの凍結乾燥粉末。
なし。
5.1 免疫介在性肺炎
肺炎は、それぞれ治験1でKEYTRUDAを受けた8人(1.9%)及び1人(0.2%)の患者のグレード2または3の症例を含む、メラノーマ患者411人中12人(2.9%)に発生した。肺炎の発症までの期間中央値は、5ヶ月(0.3週〜9.9ヶ月の範囲)であった。期間中央値は4.9ヶ月(1週間〜14.4ヶ月の範囲)であった。グレード2の患者8人中5人及びグレード3の肺炎の患者1人は、高投与量の全身性コルチコステロイド(1日当たりプレドニゾンまたは同等物40mg以上)を用いる初期処置を必要とし、コルチコステロイド漸減が続いた。高投与量コルチコステロイド処置の初期投与量中央値は、プレドニゾンまたは同等物63.4mg/日であり、処置期間中央値は3日(1〜34の範囲)であり、コルチコステロイド漸減が続いた。肺炎は、3人(0.7%)の患者においてKEYTRUDAの中止に至った。肺炎により、グレード2〜3の肺炎の患者9人中7人において完全に治癒した。患者を、肺炎の徴候及び症状について監視する。放射線イメージングで肺炎が疑われる患者を評価し、Grade2以上の肺炎の場合に、コルチコステロイドを投与する。中等度(グレード2)の肺炎の場合に、KEYTRUDAを差し控え、重症(グレード3)または生命に危険を及ぼす(グレード4)肺炎の場合に、KEYTRUDAを永続的に中止する[投薬量及び投与(2.2)ならびに有害反応(6.1)を参照のこと]。
(顕微鏡的大腸炎を含む)大腸炎は、それぞれ治験1でKEYTRUDAを受けた1人(0.2%)及び2人(0.5%)の患者のグレード2または3の症例を含む、患者411人中4人(1%)に発生した。大腸炎の発症までの時間中央値は、6.5ヶ月(2.3〜9.8の範囲)であった。期間中央値は、2.6ヶ月(0.6週〜3.6ヶ月の範囲)であった。グレード2または3の大腸炎の3人の患者全員は、高投与量のコルチコステロイド(1日当たりプレドニゾンまたは同等物40mg以上)で処置され、プレドニゾンまたは同等物の初期投与量中央値は70mg/日であり;初期処置の期間中央値は、7日間(4〜41の範囲)であり、コルチコステロイド漸減が続いた。1人(0.2%)の患者は、大腸炎に起因するKEYTRUDAの永続的中止を必要とした。大腸炎の4人の患者全員が、事象の完全な治癒を呈した。患者を、大腸炎の徴候及び症状について監視する。グレード2以上の大腸炎の場合に、コルチコステロイドを投与する。中等度(グレード2)または重症(グレード3)の大腸炎の場合に、KEYTRUDAを差し控え、生命に危険を及ぼす(グレード4)大腸炎の場合に、KEYTRUDAを永続的に中止する[投薬量及び投与(2.2)ならびに有害反応(6.1)を参照のこと]。
(自己免疫の肝炎を含む)肝炎は、治験1でKEYTRUDAを受けた1人(0.2%)の患者のグレード4の症例を含む、患者411人中2人(0.5%)に発生した。発症までの時間は、1.1ヶ月間続いたグレード4の肝炎の症例の場合に、22日であった。グレード4の肝炎の患者は、KEYTRUDAを永続的に中止し、高投与量(1日当たりプレドニゾンまたは同等物40mg以上)の全身性コルチコステロイドで処置され、コルチコステロイド漸減が続いた。肝炎の患者の双方が、事象の完全な治癒を呈した。患者を、肝機能の変化について監視する。グレード2以上の肝炎の場合に、コルチコステロイドを投与し、肝酵素上昇の重症度に基づいて、KEYTRUDAを差し控えるかまたは中止する[投薬量及び投与(2.2)ならびに有害反応(6.1)を参照のこと]。
下垂体炎は、治験1でKEYTRUDAを受けた患者において、グレード2の1症例及びグレード4の1症例(それぞれ0.2%)からなる、患者411人中2人(0.5%)に発生した。発症までの時間は、グレード4の下垂体炎の患者の場合に、1.7ヶ月、グレード2の下垂体炎の場合に、1.3ヶ月であった。両方の患者は、高投与量(1日当たりプレドニゾンまたは同等物40mg以上)のコルチコステロイドで処置され、コルチコステロイド漸減が続き、生理学的補充投与量に留まった。下垂体炎の徴候及び症状について監視する。グレード2以上の下垂体炎の場合に、コルチコステロイドを投与する。中等度(グレード2)の下垂体炎の場合に、KEYTRUDAを差し控え、重症(グレード3)の下垂体炎の場合に、KEYTRUDAを差し控えるかまたは中止し、生命に危険を及ぼす(グレード4)下垂体炎の場合に、KEYTRUDAを永続的に中止する[投薬量及び投与(2.2)ならびに有害反応(6.1)を参照のこと]。
腎炎は、グレード2の自己免疫性腎炎の1症例(0.2%)ならびにグレード3の1症例及びグレード4の1症例である、腎不全を伴う間質性腎炎の2症例(0.5%)からなる3人(0.7%)の患者に発生した。自己免疫性腎炎の発症までの時間は、KEYTRUDAの最初の投与の11.6ヶ月後(最終投与の5ヶ月後)であり、3.2ヶ月続いた;この患者は、生検を受けていなかった。急性間質性腎炎は、グレード3〜4の腎不全の2人の患者における腎生検により確定された。3人の患者全員は、高投与量のコルチコステロイド(1日当たりプレドニゾンまたは同等物40mg以上)を用いる処置で腎機能を回復し、コルチコステロイド漸減が続いた。患者を、腎機能の変化について監視する。グレード2以上の腎炎の場合に、コルチコステロイドを投与する。中等度(グレード2)の腎炎の場合に、KEYTRUDAを差し控え、重症(グレード3)のまたは生命に危険を及ぼす(グレード4)腎炎の場合に、KEYTRUDAを永続的に中止する[投薬量及び投与(2.2)ならびに有害反応(6.1)を参照のこと]。
甲状腺機能亢進症は、それぞれ治験1でKEYTRUDAを受けた2人(0.5%)及び1人(0.2%)の患者のグレード2または3の症例を含む、患者411人中5人(1.2%)に発生した。発症までの時間中央値は、1.5ヶ月(0.5〜2.1の範囲)であった。期間中央値は、2.8ヶ月(0.9〜6.1の範囲)であった。グレード2の甲状腺機能亢進症の患者2人中1人及びグレード3の甲状腺機能亢進症の患者1人は、高投与量のコルチコステロイド(1日当たりプレドニゾンまたは同等物40mg以上)を用いる初期処置を必要とし、コルチコステロイド漸減が続いた。1人(0.2%)の患者は、甲状腺機能亢進症に起因するKEYTRUDAの永続的な中止を必要とした。甲状腺機能亢進症の5人の患者全員が、事象の完全な治癒を呈した。甲状腺機能低下症は、治験1でKEYTRUDAを受けた1人(0.2%)の患者のグレード3の症例を含む、患者411人中34人(8.3%)に発生した。甲状腺機能低下症の発症の期間中央値は、3.5ヶ月(0.7週〜19ヶ月の範囲)であった。甲状腺機能低下症の患者の2人を除く全員を、長期間の甲状腺ホルモン補充療法で処置した。残りの2人の患者だけが、短期間甲状腺ホルモン補充療法を必要とした。甲状腺機能低下症の管理のために、コルチコステロイドを受け、またはKEYTRUDAを中止した患者はいなかった。甲状腺障害は、処置中のいつでも発生する可能性がある。患者を、(処置の開始時の、処置中に定期的な、かつ臨床評価に基づいて示されるような)甲状腺機能の変化ならびに甲状腺疾患の臨床徴候及び症状について監視する。グレード3以上の甲状腺機能亢進症の場合に、コルチコステロイドを投与し、重症(グレード3)の甲状腺機能亢進症の場合に、KEYTRUDAを差し控え、生命に危険を及ぼす(グレード4)甲状腺機能亢進症の場合に、KEYTRUDAを永続的に中止する。単独の甲状腺機能低下症は、処置の中断なしの、かつコルチコステロイドなしの補充療法で管理されてもよい[投薬量及び投与(2.2)ならびに有害反応(6.1)を参照のこと]。
他の臨床的に重要な免疫介在性有害反応が、発生する可能性がある。次の臨床的に重要な免疫介在性有害反応:剥脱性皮膚炎、ぶどう膜炎、関節炎、筋炎、膵炎、溶血性貧血、脳実質に炎症性病巣を有する患者に生じる部分発作、及び副腎不全は、治験1においてKEYTRUDAで処置された1%未満の患者に発生した。
作用機序に基づいて、KEYTRUDAは、妊婦に投与される時に、胎児に害を引き起こすことがある。動物モデルは、PD−1/PDL−1シグナル伝達経路を、胎児の組織に対する母胎の免疫寛容の誘導を介する妊娠の維持と関連づける。この薬物が、妊娠中に使用される場合、または患者がこの薬物の服用時に妊娠した場合に、胎児に対する潜在的な危険を、患者に知らせる。生殖能力のある女性に、KEYTRUDAを用いる処置中及びKEYTRUDAの最終投与後4ヶ月以内は、非常に効果的な避妊法を使用するように助言する[特定の集団における使用(8.1、8.8)を参照のこと]。

- 免疫チェックポイント調節因子を用いる療法の奏効性(responsiveness)と相関する変異シグネチャーを規定する方法であって、
免疫チェックポイント調節因子療法の奏効(response)特性を共有する腫瘍の複数のサンプルにおける1つ以上の変異特性を決定する工程;
決定された1つ以上の前記変異特性を、前記奏効特性を共有しない腫瘍の複数のサンプルのものと比較する工程;及び
その存在が奏効特性と相関する1セットの変異特性を同定する工程
を含む、前記方法。 - 前記1つ以上の変異特性が、変異バーデン(burden)、非同義変異バーデン、ネオアンチゲンバーデン、トランスバージョンバーデン、トランジションバーデン、相対的トランスバージョン対トランジションバーデン、DNA修復と関連する遺伝子における変異バーデン、DNA修復と関連する1つ以上の特定の遺伝子における変異の存在、DNA修復と関連する1つ以上の特定の遺伝子における変異の同一性、及びそれらの組み合わせからなる群から選択される変異特性を含む、請求項110に記載の方法。
- 測定された前記バーデンが、比率若しくは数であるかまたはこれを含む、請求項111に記載の方法。
- DNA修復と関連する前記遺伝子が、POLD1、PRKDC、DNA−PK、RAD17、POLE、及びMSH2からなる群から選択される遺伝子であるかまたはこれを含む、請求項111に記載の方法。
- 前記奏効特性が、6ヶ月超続く部分奏効若しくは安定した奏効(「持続的臨床効果」;「DCB」)、4週間超の腫瘍サイズの低減(「客観的奏効率」;「ORR」)、9週間超の疾患進行なし(「無増悪生存期間」;「PFS」)、及びそれらの組み合わせからなる群から選択される特性であるかまたはこれを含む、請求項111〜113のいずれか1項に記載の方法。
- 免疫チェックポイント調節因子療法の奏効特性と相関する1セットの変異特性の存在を判定する工程により腫瘍サンプルを特性決定する方法。
- 前記1セットの変異特性が、変異バーデン、非同義変異バーデン、ネオアンチゲンバーデン、トランスバージョンバーデン、トランジションバーデン、相対的トランスバージョン対トランジションバーデン、DNA修復と関連する遺伝子における変異バーデン、DNA修復と関連する1つ以上の特定の遺伝子における変異の存在、DNA修復と関連する1つ以上の特定の遺伝子における変異の同一性、及びそれらの組み合わせからなる群から選択される変異特性を含む、請求項115に記載の方法。
- 測定された前記バーデンが、比率若しくは数であるかまたはこれを含む、請求項116に記載の方法。
- DNA修復と関連する前記遺伝子が、POLD1、PRKDC、DNA−PK、RAD17、POLE、及びMSH2からなる群から選択される遺伝子であるかまたはこれを含む、請求項116に記載の方法。
