背景
１．技術説明
本開示は、内視鏡外科用ステープル留めデバイスに関し、より具体的には、コンパクトな発射アセンブリを有する外科用ツールアセンブリを含む外科用ステープル留めデバイスに関する。
２．関連技術の背景
内視鏡用の外科用ステープル留めデバイスは、関節運動ツールアセンブリを含む。リニア内視鏡外科用ステープル留めデバイスは、一対のジョー、ツールアセンブリを通して可動なナイフを支持する駆動部材、及びジョーの１つに支持されたステープルカートリッジからステープルを射出するように構成された作動スレッドを有するツールアセンブリを含む。ツールアセンブリはまた、ステープルカートリッジのステープルの位置の近位にある位置のジョーの対によって画成される組織間隙内に組織が位置付けられることを防止する組織止めを含んでもよい。作動スレッドは、組織の切断に先立って組織内のステープルの形成を容易にするために駆動部材及びナイフの遠位に位置付けられる。

ここで、コンパクトな発射アセンブリを備えたツールアセンブリを含む本開示の外科用ステープリングデバイスが、図面を参照して詳細に記載され、同様の参照番号は、いくつかの図の各々において、同一または対応する要素を示す。この記載において、用語「近位」は、概して、臨床医に近いデバイスの部分を指すために使用され、一方、用語「遠位」は、概して、臨床医から遠いデバイスの部分を指すために使用される。加えて、「内視鏡の」という用語は、概して、内視鏡、腹腔鏡、関節鏡、及び小さな切開部または患者の体内に挿入されたカニューレを通して行われる任意の他の外科手術を指すために使用される。最後に、臨床医という用語は、概して、医師、看護師、及びサポート要員を含む医療関係者を指すために使用される。
本開示の外科用ステープル留めデバイスは、ハンドルアセンブリ、ハンドルアセンブリから遠位に延びる細長い本体、及び細長い本体の遠位部に支持され、既知の外科用ステープル留めデバイスと比較してコンパクトな発射アセンブリを有するツールアセンブリを含む。実施形態では、ツールアセンブリは、関節運動のために細長い本体の遠位部に取り付けられ、複数のステープル及びプッシャーを支持するステープルカートリッジ、ステープルカートリッジからステープルを射出するためにステープルカートリッジを通して可動な作動スレッド、及び駆動またはクランプ部材を含む。クランプ部材は、ツールアセンブリを開位置から近接位置に動かすために、及びステープルを射出し、組織を切断するためにステープルカートリッジを通して作動スレッドを前進させるために、ツールアセンブリを通して可動である。ツールアセンブリの近位部における発射アセンブリのための空間を最小にするために、作動スレッド及びクランプ部材は、ツールアセンブリの作動前状態において入れ子の関係で支持される。
図１は、ハンドルアセンブリ１２、細長い本体１４、及び本開示のツールアセンブリ１００の例示的実施形態を含む外科用ステープル留めデバイス１０を図示する。本明細書では詳細には説明しないが、ツールアセンブリ１００は、当該技術分野で知られているように、細長い本体１４から取り外し可能なリロードアセンブリ１６の一部を形成することができる。代わりに、ツールアセンブリ１００が、細長い本体１４の遠位部にしっかり固定することができる。ハンドルアセンブリ１２は、ハンドグリップ１８、複数のアクチュエータボタン２０、及び回転ノブ２２を含む。回転ノブ２２は、当該技術分野において知られているように、ハンドルアセンブリ１２に対する細長い本体１４及びリロード１００の回転を容易にする。アクチュエータボタン２０は、近接、発射及び切断を含むステープル留めデバイス１０の様々な機能の動作を制御する。リロードは、ロボット手術システムの使用のために構成することができる。ステープル留めデバイス１０は、電動ステープル留めデバイスとして図示されているが、本開示のツールアセンブリ１００はまた、手動で動く外科用ステープル留めデバイスで使用するのに適していることが想定される。参照によりその全体が本明細書に組み込まれる、米国特許第９，０５５，９４３号（「’９４３特許」）は、動力付きハンドピース、アダプタ、及びアダプタに取り外し可能に結合されたツールアセンブリを有する外科用ステープル留めデバイスを開示している。
図２及び図３を参照して、ツールアセンブリ１００は、アンビル１０２、及びカートリッジアセンブリ１０４を含む。カートリッジアセンブリ１０４は、複数のステープル１０８及びプッシャー１１０を支持するステープルカートリッジ１０６、底面１１２ａを画成するカートリッジチャネル１１２、ステープルカートリッジ支持板１１４、駆動ねじ１１６、クランプ部材１１８と、及び作動スレッド１２０を含む。支持板１１４は、弾性突起、例えば弾性ランス１１４ａ（図４）を含み、以下でさらに詳細に記載するように、作動スレッド１２０のための停止面を画成する。代わりに、他の突起構成も想定される。ステープルカートリッジ１０６は、中央ナイフスロット１４０を画成し、スナップフィットまたは他の接続によって支持板１１４内に取り付けられる。支持板１１４及びステープルカートリッジ１０６アセンブリは、やはりスナップフィット接続によってカートリッジチャネル１１２に取り付けられる。あるいは、これらの構成要素を共に固定するための他の技術を用いてもよい。ステープルカートリッジ１０６は、組織接触面１２４に穴として構成された、横方向に離間したステープル保持スロット１２４ａの複数の列を画成する組織接触面１２４を含む。各スロット１２４ａは、ステープル１０８の１つ、及びそれぞれのプッシャー１１０を受ける。ステープルカートリッジ１０６はまた、ステープルプッシャー１１０を収容する複数の長手方向のカムスロット（図示せず）を画成し、作動スレッド１２０の通過を容易にするために組織接触面１２４の反対側の端部に開口している。
アンビルアセンブリ１０２は、アンビルアセンブリ１０２の近位部に画成された開口１２５及びカートリッジアセンブリ１０６の近位部に画成された開口１２７を通って延びる枢軸部材１２２の周りで、カートリッジアセンブリ１０４に枢動可能に連結される。アンビルアセンブリ１０２は、開位置（図１）と近接位置（図２）の間でツールアセンブリ１００を移行させるためにカートリッジアセンブリ１０６に対して枢動する。アンビルアセンブリ１０２は、当該技術分野で知られているように、複数のステープル変形陥凹（図示せず）を画成する組織接触面１０２ａ（図９）を含む。アンビルアセンブリ１０２は、当接面１２８（図９）及び組織止め１３０を含む。当接面１２８は、クランプ部材１１８によって係合されるように位置付けられ、ツールアセンブリ１００の開位置から近接位置への動きを容易にする。実施形態において、組織止め１３０は、アンビルアセンブリ１０２の組織接触面１０２ａのそれぞれ反対側に位置付けられた一対の下方に延びる翼部１３０ａを含む。翼部１３０ａは、ツールアセンブリ１００が開位置にあり、遠位の組織係合面１３０ｂを有するとき、ステープルカートリッジ１０６の組織接触面１２４の下方に延在する。組織止め１３０は、ステープル留めできない組織が切断されるのを防止するために、組織止め１３０を越えてアンビル１０２とカートリッジアセンブリ１０４との間でツールアセンブリ１００に組織が進入することを防止する。
また図５及び図６を参照して、クランプ部材１１８は、基部１４２、垂直支柱１４４、上部梁１４６、及び下部梁１４８を含む。垂直支柱１４４は、基部１４２に固定された第１の端部と、上部梁１４６に固定された第２の端部とを有する。代わりに、基部、上部梁及び垂直支柱はユニットとして一体で形成することができる。実施形態では、基部１４２は、駆動ねじ１１６（図３）を回転可能に受け入れるねじ付穴１５０を画成する。駆動ねじ１１６は、駆動ねじ１１６の回転運動をクランプ部材１１８の長手方向の動きに変換するために、ねじ山を持ち、クランプ部材１１８のねじ付穴１５０内で回転可能である。
垂直支柱１４４は、上部梁１４６と下部梁１４８との間に位置付けられるナイフ１５２を画成する遠位面を含む。クランプ部材１１８の後退位置からクランプ位置への動きが、ツールアセンブリ１００を開位置（図９）から近接位置 （図１１）に動かすように、上部梁１４６は、アンビルアセンブリ１０２の当接面１２８と係合するように位置付けられる。アンビルアセンブリ１０２とステープルカートリッジ１０６の組織接触面１０２ａ、１２４の間の最大組織間隙を画成するために、上部梁１４６はまた、アンビルアセンブリ１０２内に画成されたチャネル１６０（図９）を通して平行移動するように位置付けられ、下部梁１４８は、カートリッジチャネル１１２の底面１１２ａに沿って平行移動するように位置付けられる。
クランプ部材１１８はまた、下部フランジ１４８から離間した基部１４２の表面から遠位に延びる突出部１６４を含む。突出部１６４は、近位傾斜面１６６ａと、作動スレッド１２０と係合して前進するように構成された遠位面１６６ｂとを含む係合部材１６６を支持する。
作動スレッド１２０は、一対の離間したカム部材１７０ａ、１７０ｂを含む。各カム部材１７０ａ、１７０ｂは、ステープルカートリッジ１０６のそれぞれの長手方向カムスロット（図示せず）を通して平行移動し、当該技術分野で知られているように、ステープルカートリッジ１０６からステープル１０８を射出するために、プッシャー１１０（図３）と相互作用するように位置付けられる。実施形態では、カム部材１７０ａ、１７０ｂのそれぞれは、ステープルカートリッジ１０６のステープル保持スロット１２４ａ内のプッシャー１１０を持ち上げ、ステープルカートリッジ１０６のステープル保持スロット１２４ａからステープル１０８を射出するために、作動スレッド１２０がステープルカートリッジ１０６を通して平行移動するとき、プッシャー１１０（図３）に連続的に係合する２つの離間したカム面１７４、１７６を含む。カム面１７４、１７６の角度は、ステープル保持スロット１２４ａを通るプッシャー１０８の動きをより良好に制御し、より良好なステープル形成を提供するために、カム面１７４、１７６の長さに沿って変化してもよい。例えば、カム面１７４、１７６は、遠位部１８０及び近位部１８２を有してもよく、近位部１８２は遠位部１８０よりも急勾配である。
カム部材１７０ａ、１７０ｂは、カム部材１７０ａ、１７０ｂの間に開放近位端を有する長手方向チャネル１８６を画成するブリッジ１８４によって互いに接続される。ブリッジ１８４はまた、チャネル１８６の遠位端にクロス部材１８８を含む。以下にさらに詳細に論じるように、チャネル１８６は、クランプ部材１１８の突出部１６４の遠位端に形成された係合部材１６６を摺動可能に受け入れるように寸法決めされている。
図７〜１０を参照して、外科用ステープル留めデバイス１０が発射前の状態にあるとき、クランプ部材１１８及び作動スレッド１２０は入れ子の配置にある。入れ子の配置では、係合部材１６６が、作動スレッド１２０のクロス部材１８８の遠位に位置付けられるように、クランプ部材１１８の突出部１６４は、作動スレッド１２０のブリッジ１８４の下に位置付けられ、垂直支柱１４４は、作動スレッド１２０のチャネル１８６の近位部内に位置付けられる。加えて、クランプ部材１１８の上部梁１４６（図９）は、アンビルアセンブリ１０２の当接面１２８及びチャネル１６０の近位に位置付けられる。この位置では、ランス１１４ａを作動スレッド１２０の動きの経路から外に付勢するために、スレッド１２０は、支持板１１４のランス１１４ａ（図４）の上に位置付けられる。
図９及び図１０を参照して、駆動ねじ１１６は、クランプ部材１１８のねじ付穴１５０を通って延び、細長い本体１１４内に支持された駆動部材１９０に連結される近位部を含む。駆動ねじ１１６の近位部は、当該技術分野で知られているように、駆動ねじ１１６を回転可能に支持するように構成されたベアリング１９２上で支持される。参照により本明細書に組み込まれる、米国特許第８，５１２，３５９号は、駆動ねじ１１６と駆動部材１９０とを含む外科用ステープル留めデバイスを開示している。
図１１及び図１２を参照して、駆動ねじ１１６が、外科用ステープル留めデバイス１０のクランプストロークを通してクランプ部材１１８を前進させるために、ハンドルアセンブリ１２及び細長い本体１４を通して作動されると、クランプ部材は、後退位置からクランプ位置に動く 。クランプ部材１１８がクランプ位置に向かって動くと、クランプ部材１１８は、作動スレッド１２０と無関係に駆動ねじ１１６に沿って、作動スレッド１２０のブリッジ１８４の長手方向チャネル１８６を通って遠位に動く。クランプ部材１１８がツールアセンブリ１００の近位部内で遠位に動くと、図１２の矢印「Ａ」で示される方向で、クランプ位置へのクランプ部材１１８の遠位方向への連続の動きが、ツールアセンブリ１００を開位置（図９）から近接位置（図１１）に付勢するために、アンビルアセンブリ１０２を図１１の矢印「Ｂ」で示す方向に枢動させるように、上部梁１４６はアンビルアセンブリ１０２の当接面１２８と係合する。アンビルアセンブリ１０２は静止していてもよく、カートリッジアセンブリ１０４が、アンビルアセンブリ１０２に対して開位置から近接位置に枢動してもよいことが予想される。
クランプ位置において、クランプ部材１１８の垂直支柱１４４は、チャネル１８６の遠位部の作動スレッド１２０のブリッジ１８４のクロス部材１８８の直ぐ近位に位置付けられ、ツールアセンブリ１００は、カートリッジアセンブリ１０６の組織接触面１２４がアンビルアセンブリ１０２の組織接触面１０２ａと並置された配置に位置付けられる近接位置にある。加えて、スレッド１２０は、支持板１１４のランス１１４ａの上にとどまっている。
ステープルをツールアセンブリ１００から射出するために、発射ストロークを通してクランプ部材１１８を動かすために駆動部材１１６が、ハンドルアセンブリ１２及び細長い本体１４を通して再び作動される。発射ストロークの間、クランプ部材１１８及び作動スレッド１２０は、図１３に示される第１の前進ステージを通して、図１４及び図１５に示される後退ステージを通して、及び図１６に示す第２の前進ステージを通して、動かされる。これらの各ステージについて以下に記載する。
図１３を参照して、発射ストロークの第１の前進ステージの間に、駆動ねじ１１６は、クランプ部材１１８をクランプ位置から遠位に矢印「Ａ」で示す方向に前進させる。垂直支柱１４４は、クロス部材１８８に近接して位置付けられているので、クランプ部材１１８の遠位方向の動きは、ツールアセンブリ１００を通して作動スレッド１２０を遠位に前進させるために、垂直支柱１４４を作動スレッド１２０の近位端に係合させる。作動スレッド１２０がランス１１４ａを通り過ぎて前進すると、ランス１１４ａは作動スレッド１２０の経路内に上方に撓み、第１の前進ステージが終了する。
図１４を参照して、発射ストロークの後退ステージの間に、クランプ部材１１８をツールアセンブリ１００内の矢印「Ｂ」によって示される方向に後退させるために、駆動ねじ１１６は反対方向に回転される。ランス１１４ａは、クランプ部材１１８が作動スレッド１１８とは独立して近位に動くように、作動スレッド１２０の近位方向の動きを防止する。クランプ部材１１８が近位に動くと、クランプ部材１１８の突出部１６４は、係合部材１６６の近位傾斜面１６６ａがブリッジ１８４のクロス部材１８８と係合するまで、ブリッジ１８４の下を摺動する。これが起こると、クランプ部材１１８の係合部材１６６の継続的な近位方向の動きは、クロス部材１８８を係合部材１６６の近位傾斜面１６６ａがステープルカートリッジ１０６の切欠き２００内に乗り上げさせる。これは、クランプ部材１１８の係合部材１６６が、作動部材１２０のブリッジ１８４のクロス部材１８８の下を動くことを許す。クランプ部材１１８を作動スレッド１２０のさらに近位に動かすことによって、ツールアセンブリ１００は、組織を切断する前に組織をステープル留めするように構成される。
図１５を参照して、発射ストロークの後退ステージの後、クランプ部材１１８の係合部材１６６の遠位面１６６ｂは、作動スレッド１２０のクロス部材１８８の近位側に近接して位置付けられ、クランプ部材１１８のナイフ１５２は、作動スレッド１２０のカム面１７４、１７６の近位端の近位に位置付けられる。上述のように、これは、組織が切断される前にステープル留めされることを確実にする。
図１６を参照して、発射ストロークの第２の前進ステージの間に、駆動ねじ１１６を回転させて、ツールアセンブリ１００を通してクランプ部材１１８を矢印「Ｃ」で示す方向に遠位に前進させる。クランプ部材１１８の係合部材１６６の遠位面１６６ｂは、クロス部材１８８の近位側に近接して位置付けられるので、クランプ部材１１８の遠位の動きは、ツールアセンブリ１００内の作動スレッド１２０の遠位運動をもたらす。作動スレッド１２０及びクランプ部材１１８がツールアセンブリ１００を通って前進すると、作動スレッド１２０のカム部材１７０ａ、１７０ｂは、ステープル１０８をステープル保持スロット１２４ａから順次駆動するために、プッシャー１１０と連続的に係合する。
図１７及び図１８は、本開示の代替実施形態のクランプ部材２１８及び作動スレッド２２０を示す。クランプ部材２１８は、基部２４２、垂直支柱２４４、上部梁２４６、及び下部梁２４８を含む。垂直支柱２４４は、基部２４２に固定された第１の端部と上部梁２４６に固定された第２の端部とを有する。実施形態では、基部１４２は、駆動ねじ１１６（図３）を回転可能に受け入れるねじ付穴２５０を画成する。上述のように、駆動ねじ１１６は、駆動ねじ１１６の回転運動をクランプ部材２１８の長手方向の動きに変換するために、ねじ山を持ち、クランプ部材２１８のねじ付穴２５０内で回転可能である。
クランプ部材２１８の垂直支柱２４４は、上部梁２４６と下部梁２４８との間に位置付けられるナイフ２５２を画成する遠位面を含む。上部梁２４６は、後退位置からクランプ位置（図１９）へのクランプ部材２１８の動きが、ツールアセンブリ２００を開位置から近接位置（図１９）に動かすように、アンビルアセンブリ２０２の当接面２２８（図１９）と係合するように位置付けられる。ツールアセンブリ１００に関して上述したように、アンビルアセンブリ２０２の組織接触面（図示せず）とステープルカートリッジ２０６との間の最大組織間隙を画成するために、ツールアセンブリ２００（図１９）の上部梁２４６もまた、アンビルアセンブリ２０２（図１９）内に画成されたチャネル（図示せず）を通して平行移動するように位置付けられ、下部梁２４８は、カートリッジチャネル２１２の底面２１２ａに沿って平行移動するように位置付けられる。
クランプ部材２１８はまた、基部２４２の対向する側壁から外向きに延びる半径方向延長部２６４を含む。半径方向延長部２６４はそれぞれ、以下でさらに詳細に記載するように、作動スレッド２２０と係合して前進するように構成される、近位傾斜面２６６ａ及び遠位面２６６ｂを含む。
また図１９を参照して、作動スレッド２２０は、作動スレッド１２０に関して上述した離間したカム部材１７０ａ、１７０ｂと実質的に同様である、一対の離間したカム部材２７０ａ、２７０ｂを含み、ここではさらに詳細には説明しない。カム部材２７０ａ、２７０ｂは、開放近位端及び遠位のクロス部材２８８を有するチャネル２７１を画成する。各カム部材２７０ａ、２７０ｂは、ステープルカートリッジ２０６のそれぞれの長手方向カムスロット（図示せず）を通して平行移動し、当該技術分野で知られているように、ステープルカートリッジ２０６からステープルを射出するために、プッシャー（図示せず）と相互作用するように位置付けられる。図示のように、作動スレッド２２０は、ツールアセンブリ２００の長手方向軸に垂直な遠位端壁２８４ａと、ツールアセンブリ２００の長手方向軸と鋭角をなす近位端壁２８４ｂとを有する凹部２８４を画成する。凹部２８４は、クランプ部材２１８の半径方向延長部２６４を受け入れるように寸法決めされ構成される。
図２２を簡単に参照して、ツールアセンブリ２００は、ツールアセンブリ１００に関して上述した支持板１１４と同様の支持板２１４を含む。支持板２１４は、支持板２１４がランス１１４ａ（図４）の代わりに少なくとも１つの内側に延びる弾性タブ２１４ａを有する点で、支持板１１４とは異なる。１つのタブ２１４ａのみが示されているが、支持板２１４ａは、支持２１４のそれぞれの側にタブ２１４ａを含むことができる。タブ２１４ａは、支持板２１４内の作動スレッド２２０の近位方向の動きを防止する停止面を提供する点で、ランス１１４ａ（図４）と同様に機能する。作動スレッド２２０が後退位置（図１９及び図２０）にあるとき、スレッド２２０の側面は、弾性タブ２１４ａを作動スレッド２２０の動きの経路から内向きに付勢するように位置付けられる。
図１９を参照して、ツールアセンブリ２００を作動させる前に、クランプ部材２１８（破線で示す）及び作動スレッド２２０は、クランプ部材２１８の垂直支柱２４４が作動スレッド２２０のチャネル２７１内に位置付けられ、クランプ部材２１８の半径方向延長部２６４が凹部２８４を画成する近位壁２８４ｂに近接して位置付けられて、ツールアセンブリ２００の近位部内で入れ子の関係にある。上述したように、作動スレッド２２０の後退位置では、支持板２１４の弾性タブ２１４ａは、支持板２１４の側壁と係合し、作動スレッド２２０の動きの経路の外に付勢される。
駆動ねじ１１６（図３）が、外科用ステープル留めデバイス１０のクランプストロークを通してクランプ部材２１８を前進させるために、ハンドルアセンブリ１２（図１）及び細長い本体１４（図１）を介して作動されるとき、クランプ部材２１８は、ツールアセンブリ２００内の遠位方向に、後退位置（破線で示す）からクランプ位置まで動く。クランプ部材２１８が矢印「Ｄ」によって示される方向にクランプ位置に向かって動くと、クランプ部材２１８の垂直支柱２４４は、作動スレッド２２０のチャネル２７１内で前進し、クランプ部材２１８が駆動ねじ１１６に沿って作動スレッド２２０から独立して動くように、クランプ部材２１８の半径方向延長部２６４は、作動スレッド２２０の凹部２８４を通って前進する。クランプ部材２１８がクランプ位置に向かって動くと、アンビルアセンブリ２０２を矢印「Ｅ」で示す方向に近接位置に枢動させるために、クランプ部材２１８の上部梁２４６は、アンビルアセンブリ２０２の当接面２２８と係合する。クランプ部材２１８がクランプ位置にあるとき、クランプ部材２１８の半径方向延長部２６４は、凹部２８４を画成する遠位端壁２８４ａに近接する作動スレッド２２０の凹部２８４の遠位端に位置付けられる。作動スレッド２２０は、その完全に後退した位置にとどまる。
ツールアセンブリ２００からステープルを射出するために、駆動部材１１６（図３）は、発射ストロークを通してクランプ部材２１８を動かすために、ハンドルアセンブリ１２及び細長い本体１４を介して再び作動される。発射ストロークの間、クランプ部材２１８及び作動スレッド２２０は、図１９及び図２０に示す第１の前進ステージを通して、図２１に示す後退ステージを通して、及び図２３に示す第２の前進ステージを通して、動かされる。これらの各ステージは以下に記載される。
図２０を参照して、発射ストロークの第１の前進ステージの間に、駆動ねじ１１６はクランプ部材２１８をクランプ位置から遠位に矢印「Ｆ」で示す方向に前進させる。クランプ位置では、クランプ部材２１８の垂直支柱２４４は、クロス部材２８８に近接する作動スレッド２２０のチャネル２７１の遠位端に位置付けられ、クランプ部材２１８の半径方向延長部２６４は、作動スレッド２２０の凹部２８４を画成する遠位端壁２８４ａに近接して位置付けられる。そのようにして、クランプ部材２１８が駆動ねじ１１６（図３）に沿って前進すると、クランプ部材２１８の垂直支柱２４４がクロス部材２８８（図１７）と係合し、半径方向延長部２６４が凹部２８４を画成する遠位端壁２８４ａと係合し、作動スレッド２２０をツールアセンブリ２００内で遠位に押す。作動スレッド２２０が弾性タブ２１４ａ（図２２）を通り過ぎて前進すると、タブ２１４ａは、作動スレッド２２０の近位方向の動きを防止するために、作動スレッド２２０の動きの経路内へ内方向に撓む。この時点で、第１の前進ステージが終了する。
図２１及び図２２を参照して、発射ストロークの後退ステージの間に、クランプ部材２１８をツールアセンブリ１００内の矢印「Ｇ」によって示される方向に後退させるために、駆動ねじ１１６は反対方向に回転される。タブ２１４ａは、クランプ部材１１８が作動スレッド２１８とは独立して近位に動くように、作動スレッド２２０の近位方向の動きを防止する。クランプ部材２１８がツールアセンブリ２００内で近位に動くと、半径方向延長部２６４は、半径方向延長部２６４の近位傾斜面２６６ａが凹部２８４を画成する近位端壁２８４ｂと係合するまで、作動スレッド２２０の凹部２８４内で摺動する。半径方向延長部２６４の近位傾斜面２６６ａが凹部２８４を画成する近位端壁２８４ｂと係合すると、タブ２１４ａ（図２２）と係合し、かつ近位に動くことができない作動スレッド２２０は、半径方向延長部２６４が凹部２８４から出て、作動スレッド２２０（図２３）の近位の位置に動くことを可能にするために、図２１の矢印「Ｈ」によって示される方向に上向きにカムされる。クランプ部材２１８を作動スレッド２２０のさらに近位に動かすことによって、ツールアセンブリ２００は、組織を切断する前に組織をステープル留めするように構成される。
発射ストロークの後退ステージの後、クランプ部材２１８の半径方向延長部２６４の遠位面２６６ｂは、作動スレッド２２０の近位側に近接して位置付けられ、クランプ部材２１８のナイフ２５２（図１８）は、作動スレッド２２０のカム部材２７０ａ、２７０ｂの近位端の近位に位置付けられる。上述のように、これは、組織が切断される前にステープル留めされることを確実にする。
図２３を参照して、発射ストロークの第２の前進ステージの間に、ツールアセンブリ２００を通してクランプ部材２１８を矢印「Ｉ」によって示される方向に遠位に前進させるために、駆動ねじ１１６は回転される。クランプ部材２１８の半径方向延長部２６４の遠位面２６６ｂは、作動スレッド２２０の近位側に近接して位置付けられるので、クランプ部材１１８の遠位の動きは、ツールアセンブリ２００内の作動スレッド２２０の遠位運動をもたらす。作動スレッド２２０及びクランプ部材２１８がツールアセンブリ２００を通って前進すると、作動スレッド２２０のカム部材２７０ａ、２７０ｂ（図１８）は、ステープル保持スロット１２４ａからステープル１０８を順次駆動するために、プッシャー１１０（図３）と順次係合する。

Claims (20)

  1. 外科用ステープル留めデバイスであって、
    細長い本体と、
    前記細長い本体の遠位部に支持されたツールアセンブリと、を備え、前記ツールアセンブリは、カートリッジアセンブリ及びアンビルアセンブリを含み、前記カートリッジアセンブリ及びアンビルアセンブリは、離間位置と近接位置との間で互いに対して可動であり、前記カートリッジアセンブリは、複数のステープルを支持するステープルカートリッジ、ナイフを支持する本体を有するクランプ部材、及びカム部材を含む作動スレッドを含み、作動前状態では、前記クランプ部材及び前記作動スレッドは、前記クランプ部材の前記ナイフが前記作動スレッドの前記カム部材の近位端の遠位に位置付けられる、入れ子の関係に位置付けられ、前記外科用ステープル留めデバイスの発射ストロークの少なくとも一部の間、前記クランプ部材は、前記ナイフを前記カム部材の近位に位置付けるために可動である、外科用ステープル留めデバイス。
  2. 前記作動スレッドの前記カム部材は、ブリッジによって相互接続された第１及び第２のカム部材を含み、前記ブリッジは、開放近位端を有するチャネルを画成する、請求項１に記載の外科用ステープル留めデバイス。
  3. 前記クランプ部材は、垂直支柱、上部梁、及び下部梁を含み、前記作動前状態では、前記垂直支柱は前記チャネルの近位部内に位置付けられる、請求項２に記載の外科用ステープル留めデバイス。
  4. 前記クランプ部材は突出部を含み、前記ブリッジは、前記チャネルの遠位端を横切って延びるクロス部材を含む、請求項３に記載の外科用ステープル留めデバイス。
  5. 前記突出部は、係合部材を含み、前記係合部材は、前記作動前状態で前記クロス部材の遠位に位置付けられる、請求項４に記載の外科用ステープル留めデバイス。
  6. 前記発射ストロークの間、前記係合部材は、前記クロス部材の近位位置に動かされる、
    請求項５に記載の外科用ステープル留めデバイス。
  7. 前記係合部材は、前記ブリッジの前記クロス部材の下を通って前記チャネルに入るように構成された近位傾斜面を含む、請求項６に記載の外科用ステープル留めデバイス。
  8. 前記係合部材は、前記係合部材が前記クロス部材の近位に位置付けられるとき、前記クランプ部材の遠位方向の動きを前記作動スレッドの遠位方向の動きに変換するために、前記クロス部材と係合するように構成された遠位面を含む、請求項７に記載の外科用ステープル留めデバイス。
  9. 前記クランプ部材は、少なくとも１つの半径方向延長部を含み、前記作動スレッドは凹部を画成し、前記少なくとも１つの半径方向延長部は、前記作動前状態では前記凹部内に位置付けられる、請求項１に記載の外科用ステープル留めデバイス。

  13. 前記クランプ部材のクランプストロークの間、前記クランプ部材は、前記少なくとも１つの半径方向延長部が前記凹部の近位部から前記凹部の遠位部に動くように、前記作動スレッドとは独立して動く、請求項１２に記載の外科用ステープル留めデバイス。
  14. 前記発射ストロークの間に、前記少なくとも１つの半径方向延長部は、前記凹部内の位置から前記作動スレッドの近位位置に動かされる、請求項１３に記載の外科用ステープル留めデバイス。
  15. 前記凹部は、角度を有する近位端壁によって画成される、請求項１４に記載の外科用ステープル留めデバイス。
  16. 前記少なくとも１つの半径方向延長部は、近位傾斜面を有する、請求項１５に記載の外科用ステープル留めデバイス。
  17. 前記少なくとも１つの半径方向延長部は、前記クランプ部材のそれぞれ反対側に位置付けられた２つの半径方向延長部を含む、請求項１６に記載の外科用ステープル留めデバイス。
  18. 外科用ステープル留めデバイスであって、
    クランプ部材および作動スレッドを含むツールアセンブリを含み、
    前記クランプ部材は、前記ツールアセンブリを開位置から近接位置に動かすために、クランプ位置に作動スレッドとは独立して前進させられるように構成されており、
    前記クランプ部材は、発射ストロークを通して前進させられるように構成され、
    前記クランプ部材および前記作動スレッドは、前記発射ストロークの第１の前進ステージを通して遠位に移動されるように構成され、
    前記クランプ部材は、前記発射ストロークの後退ステージを通して前記作動スレッドとは独立して近位に移動させられるように構成され、
    前記クランプ部材および前記作動スレッドは、ステープルを前記ツールアセンブリから射出するために、前記発射ストロークの第２の前進ステージを通して遠位に動かされるように構成される、外科用ステープル留めデバイス。
  19. 外科用ステープル留めデバイスであって、停止部材を含み、前記作動スレッドは、前記第１の前進ステージにおいて、前記作動スレッドの近位方向の動きを防止するために位置付けられた前記停止部材を過ぎて遠位に動くように構成されている、請求項１８に記載の外科用ステープル留めデバイス。
  20. 前記クランプ部材は、ナイフを含み、前記ナイフは、前記後退ステージにおいて、前記作動スレッドのカム面の近位に動かされるように構成される、請求項１８に記載の外科用ステープル留めデバイス。
