Abstract
Description

本明細書で使用する時、用語「嚥下(swallow)」は、口腔から下咽頭を通り食道内へ入る、食物の安全な通過を指すと理解されてもよい。更に、嚥下には通常、保護された気道内への食物の進入が起きない無呼吸期間が伴う。対照的に、「誤嚥(aspiration)」は、吸息の間における気道内への異物の進入を意味してもよい。用語「嚥下活動」は、嚥下又は誤嚥、あるいは、どちらかの非存在を含んでもよい。

本方法は、第1の食感を有する第1の食物品目及び第2の食感を有する第2の食物品目について実行され、第1の食感は第2の食感と異なる。
第1の食物品目及び第2の食物品目は両方とも、増粘剤を用いて少なくとも1つの流体を増粘することによって作製される。
増粘剤は単位部分の形態で提供される、又は、提供可能であり、増粘剤の単位部分のうちの1つ以上を流体と組み合わせることによって、少なくとも1つの規定の食感が達成される。

本研究の目的は、非先天的、非外科的、及び非腫瘍起源の口腔咽頭嚥下障害のリスクがある成人被検者における障害性嚥下を検出するための嚥下加速度測定分類器の有用性を探求することである。
研究の終点は、異なる流動性の食塊に基づき障害性嚥下を識別するための加速度測定ベースの分類器のための教師あり調整誤り率である。
非先天的、非外科的、及び非腫瘍起源の口腔咽頭嚥下障害のリスクがある成人被検者を、彼らが嚥下障害を有するらしいと疑われるかどうかにかかわらず、登録のために評価することになる。被検者はまた、以下のグループのうちの少なくとも1つに属しなければならない:卒中、後天的脳損傷、又は除外基準によって網羅されない他の理由のために患者として登録されていること、並びに65歳を超えていること。およそ200名の患者が試験に登録される。

前記処理ユニット(140)が、咽喉の振動を検出する能力を有するセンサ(104)からの信号(114)を供給されることと、

前記処理ユニット(140)は、前記センサ(104)から、被検者による標識食物の品目の摂取に関連付けられるセンサ信号(114)を受け取る能力を有し、

ii)前記嚥下過程の前記特性を評価する能力を有する基準技法によって、被検者による標識食物の品目の摂取の結果得られた被検者における嚥下過程の特性を評価するステップであって、前記基準技法は、イメージング技法、嚥下光ファイバ内視鏡評価(FEES)、官能試験を伴う嚥下光ファイバ内視鏡評価(FEESST)、及びパルス酸素測定又はその他の適切な技法からなる群から選択される、ステップ、
iii)前記処理ユニット(140)を調整するためにステップi)及びii)の結果を用いるステップ、
を少なくとも1回実行するステップと、
b)これにより、調整された前記処理ユニット(140)を得るステップと、を含む、方法。 - 前記イメージング技法がビデオ蛍光透視法(VF)である、請求項11に記載の方法。
- ステップiii)における前記調整することが、被検者(102)による標識食物の品目の摂取の結果生じた咽喉の振動の結果得られたセンサ信号(114)を判定すること、及び前記センサ信号(114)を、コンピュータ実行可能ソフトウェアによって動作させられる前記処理ユニット(140)へ転送し、それにより、嚥下過程の特性の評価を提供することを含み、
前記センサ信号(114)から得られた評価は、ステップii)の前記イメージング技法から得られた評価と相互に関連付けられる、請求項12又は13に記載の方法。 - 前記センサ信号から得られた被検者の嚥下過程の特性の評価と、前記イメージング技法から得られた嚥下過程の特性の評価との間の相互関連を改善するために、ステップi)〜iii)が少なくとも1回繰り返され、被検者は同じであるか、又は異なるものとすることができ、任意選択的に、前記異なる被検者は、1〜1,000名の異なる被検者であるとすることができる、請求項12〜14のいずれか一項に記載の方法。
- 前記方法がニューラルネットワークを実装するか、前記方法がコンピュータ実行可能ソフトウェアのパラメータを決定し、前記実行可能ソフトウェアは、前記調整されたプロセッサ上で作動し、前記プロセッサに命令する、請求項12〜15のいずれか一項に記載の方法。
- 前記調整段階が、
a)請求項12のステップb)の前記調整された処理ユニットを用意することと、
b)請求項12のステップa)において定義された前記方法を、前記調整された処理ユニット上で被検者のグループにおける被検者に対して実行することであって、前記グループは、10〜30名、好ましくは20名の被検者を含む、実行することと、
c)これにより、更に調整された処理ユニットを得ることと、
d)前記得られた、調整された処理ユニットをステップb)〜d)に1〜30回、再びかけることと、を含む、請求項12に記載の方法。 - 前記調整された処理ユニットが妥当性確認段階において妥当性を確認されるステップc)を更に含む、請求項12〜17のいずれか一項に記載の方法。
- 前記基準技法によって出力された嚥下不全の指示と、前記調整された処理ユニットによって出力された指示とを比較することによって、前記調整された処理ユニットが被検者の嚥下過程の特性について前記基準技法と同じ評価を提供することの妥当性が確認され、
これにより、調整され、妥当性を確認された処理ユニットを得る、更なるステップを含む、請求項18に記載の方法。 - 前記得られた、調整され、任意選択的に妥当性を確認された処理ユニットが、嚥下不全の指示を出力する能力を有し、誤嚥リスクを評価することが、50%〜100%、60〜90%、又は80%の感度をもって嚥下安全性を評価すること及び/又は嚥下効率を評価することを含み、且つ/あるいは、前記得られた、調整され、任意選択的に妥当性を確認された処理ユニットが、50%〜100%、60〜90%、又は80%の特異度をもって嚥下不全の指示を出力する能力を有し、任意選択的に、感度及び特異度は嚥下安全性及び嚥下効率の両方について判定されることができる、請求項12〜19のいずれか一項に記載の方法。
- 前記ステップi)〜iii)が、水、ネクター型粘度を有する食感を有する食物品目、ハチミツ型粘度を有する食物品目、及びプディング型粘度を有する食物品目からなる群から選択される1つの食物品目を用いて少なくとも2回実行され、もって、各ステップi)の前記実行を1つの特定の食物品目に関連付け、これにより、ステップa)において被検者に提供されるべきである食物品目の配列を提供するか、水、ネクター型粘度を有する食感を有する食物品目、ハチミツ型粘度を有する食物品目、及びプディング型粘度を有する食物品目のために異なるアルゴリズムが用いられる、請求項1〜20のいずれか一項に記載の方法。
- 水、ネクター型粘度を有する食感を有する食物品目、ハチミツ型粘度を有する食物品目、及びプディング型粘度を有する食物品目からなる群から選択される少なくとも2つ、3つ、又は4つの異なる食物品目が、ステップa)において被検者に提供される、請求項21に記載の方法。
- 提供されるべき食物品目の前記配列において、前記食物品目のうちの少なくとも2つが同じであるか、又は異なる、請求項22に記載の方法。
- ステップa)において被検者に提供されるべき食物品目の前記配列が、水、ネクター型粘度を有する食感を有する食物品目、ハチミツ型粘度を有する食物品目、及びプディング型粘度を有する食物品目からなる群から選択される前記少なくとも2つ、3つ、又は4つの異なる食物の任意の順列であることができる、請求項22又は23に記載の方法。
- 被検者に提供されるべき食物の各配列内で、前記食物がいずれも2回提供されない、請求項22〜24のいずれか一項に記載の方法。
- 請求項12のステップa)内のステップa)i)の前記少なくとも2つの食物(118)のうちの1つが水である、請求項22〜25のいずれか一項に記載の方法。
- 前記配列が、水、ネクター型粘度を有する食感を有する食物品目、ハチミツ型粘度を有する食物品目、及びプディング型粘度を有する食物品目である、請求項22〜26のいずれか一項に記載の方法。
- 請求項12〜27のいずれか一項に記載の方法によって得られる処理ユニット(140)。
- a)請求項28に記載の処理ユニットと、
b)被検者の咽喉上で外的又は内的に振動を検出するようになっているセンサ(104)と、を備えるデバイス。 - 被検者における嚥下の特性を評価するための、特に、誤嚥リスクを評価するための方法であって、前記誤嚥リスクを評価することは、嚥下安全性を評価すること及び/又は嚥下効率を評価することを含み、前記方法は、
a)食物(118)の少なくとも1つの品目を経口摂取のために被検者(102)に提供するステップと、
b)請求項29に記載のデバイスによって、被検者による食物(118)の品目の摂取に関連付けられる前記被検者(102)における嚥下の特性を評価するステップと、を含む、方法。 - 前記食物品目(118)が、食感が異なる少なくとも2つの規定の食物(118、120)の品目から選択される、請求項30に記載の方法。
- 前記少なくとも2つの規定の食物(118、130)の品目のうちの1つが水である、請求項30又は31に記載の方法。
- ステップa)及びb)が、食物の以前に用いられた品目と異なる食感を有する食物を用いて繰り返される、請求項30〜32のいずれか一項に記載の方法。
- 最初に提供される食物品目が水(130)である、請求項30〜33のいずれか一項に記載の方法。
- 被検者(102)における嚥下の特性を評価するための、特に、被検者における誤嚥リスクを評価するための請求項29に記載のデバイスの使用。
- 前記デバイス(108)が増粘剤(106)と組み合わせて用いられる、請求項35に記載の使用。
- 被検者における嚥下の特性を評価するための請求項29に記載のデバイスの使用であって、前記デバイスは、患者の経過を追跡し、且つ/又は提案される治療的介入を個人化するために、被検者についてのデータ読み取り値を経時的に記録することができる、使用。
- 食物(118)の少なくとも1つの品目を経口摂取のために提供するために好適な増粘剤(106)の使用であって、前記食物品目(118)は、請求項29に記載のデバイスによって、被検者(102)における嚥下過程の特性を評価するために、特に、被検者における誤嚥リスクを判定するために、食感が異なる少なくとも2つの規定の食物(118、120)から選択される、使用。
