Abstract
Description
本発明は、米国国立衛生研究所によって与えられたグラント番号CA120512及びARRA−CA120512により、政府の助成をうけ達成された。米国合衆国政府は、本発明に一定の権利を有する。
本発明は、レチノイドアゴニスト及び顆粒球コロニー刺激因子(G−CSFまたはGCSF)を用いる、疾患の治療用の、組成物、方法及びキットに関する。本発明はまた、細胞療法のための、生体外改変細胞(造血幹細胞[HSC]由来の顆粒球[例えば、好中球、好酸球、好塩基球]など)を用いる、疾患の治療用の、組成物、方法及びキットに関する。当該疾患は、好中球減少症の様々な形態を含むが、それらに限定はされない。

確立されたヒト及びマウスの好中球減少モデルにおいて(図4)、GCSFまたはAm80のいずれかでは、好中球減少症の初期発症段階における、化学療法薬により誘発された好中球減少を防ぐことができない。一方で、GCSFにより急激に増加した好中球は、好中球回復期にある、好中球減少症の期間を短縮できるが、これらの増加した好中球は、微生物感染との戦いには未成熟である。好中球減少症の異なる段階を占める一連の好中球減少マウスモデルの使用により、本発明者は、Am80単独、またはAm80とGCSFの組み合わせのどちらが、微生物感染症と効果的に対抗できるか、という疑問に対処した。好中球減少マウスモデルからの結果は(図6、7)、好中球減少症の初期発症段階のマウスにおいて、HSCからの好中球再生が、依然として化学療法薬により抑制されている場合、Am80単独は、既存の顆粒球前駆体の、感染に対抗する成熟好中球への分化において、GCSFまたはAm80−GCSFより優れていることを示した。しかしながら、それに続く好中球回復期においては、顆粒球形成が、好中球の再生に対して徐々に復活する場合、細菌感染に対応するために、相乗効果的に多量の成熟好中球を再生し及び分化するのは、Am80単独またはGCSF単独ではなく、Am80とGCSFの組み合わせである(図8)。さらに、継続的な菌血症の状態下での好中球減少マウスの異なる生存モデルの使用を通して、本発明者らは、Am80とGCSFの相乗効果が、好中球減少マウスの感染及び感染関連死亡率を低下させる成熟好中球の十分な量を再生するには十分である、ことを示す(図9)。これらの知見は、Am80は、HSCからの好中球再生よりは、好中球分化の促進においてより効果的であり、及びHSCの骨髄性増殖の促進におけるGCSFの機能と、好中球の分化を強化するAm80の影響との相乗効果により、Am80とGCSFの組み合わせが、当該好中球減少症疾患において微生物に対抗するための、多量の成熟好中球の要求を満たす、ことを定義する。したがって、本発明者らの研究は、Am80とGSCFの組み合わせが、好中球分化を伴う骨髄性増殖を調整でき、Am80とGCSFの組み合わせ治療により、効果的な、がん化学療法誘発性の好中球減少症(CCIN)療法を開発する機会を与えることを示している。
多様な実施形態において、本発明は、レチノイドアゴニスト及びG−CSFまたはその類似体を含む組成物を提供する。1つの実施形態において、当該レチノイドアゴニスト及びG−CSFは、同じ組成物中にある。別の実施形態において、当該レチノイドアゴニスト及びG−CSFは、別々の組成物中にある。本発明に従い、当該組成物は、対象の疾患を治療し、予防し、発症の可能性を減らし、その疾患の重症度を軽減し及び/またはその進行を遅らせるために使用しても良い。本発明に従い、当該疾患は、好中球減少症または微生物感染症である。なおも本発明に従い、当該組成物は、顆粒球、特に好中球を生成するために、細胞(非限定的に、HSC、骨髄顆粒球前駆細胞、及び造血CD34陽性細胞を含む)の刺激に対して使用しても良い。

多様な実施形態において、本発明は、対象の疾患を治療し、予防し、その重症度を軽減し及び/または進行を遅らせるためのキットを提供する。当該キットは、ある量のレチノイドアゴニスト、ある量のG−CSFまたはその類似体、及び当該レチノイドアゴニスト及び当該G−CSFまたはその類似体を、対象の疾患を治療し、予防し、その重症度を軽減し及び/または進行を遅らせるために使用するための説明書を含む。
本明細書に記載のように、GCSFとの組み合わせにおいて、Am80の許容量よりも低用量の使用で、Am80またはGCSFの単独使用と比較して、好中球減少マウスの感染関連死亡率が際立って減少した。さらに、正常及び悪性造血前駆細胞の両方における、Am80のGCSFとの組み合わせ治療は、活性酸素(ROS)の生成、微生物感染に対抗する本質的な好中球機能を増加させた。細菌及び真菌感染症に対する好中球殺菌機能に基づいて、ROS生成の生物学的検定を、本明細書に記載の組み合わせ治療法の効果の評価に利用しても良い。現在の好中球減少症の治療に対する臨床の終点には、好中球数及び発熱性好中球減少症の発生率が含まれ、そのどちらも、微生物と戦う好中球の能力についての情報を与えない。当該ROS生成の生物学的検定は、細菌または真菌のいずれかの刺激への応答で、好中球のROS生成の基準値に基づいた好中球の殺菌活性のモニタリングを可能にする。そのような、好中球のROS生成の基準値は、GCSFへの追加療法としてのAm80の最適な投与量及びスケジュールの決定に重要な、迅速で機能的な臨床の終点を提供する。
細菌に曝露された好中球減少症マウスの死亡率を減らすための、成熟好中球の再生における、Am80−GCSFの相乗効果の基礎となる機構とは、何であろうか?細胞に特異的な転写パターンは、転写因子、細胞の運命の主制御因子を調節することにより変更できる。オールトランスレチノイン酸(ATRAまたはRA)は、転写因子の2つの分類、レチノイン酸受容体(RARα、−β、−γ)及びレチノイドX受容体(RXRα、−β、−γ)に結合し、かつ転写を活性化する高い親和力を有する。従来の研究では、RAが、RARαの活性化を通して確約された骨髄前駆細胞の分化を促進する一方で、RARγの活性化を通してHSC自己複製を強化することを示している。そのようなRAの広汎な作用(pan−action)は、多様な正常及び腫瘍細胞において、RARα、−β、−γサブタイプの活性化により分化を促進する一方で、増殖の抑制に強力な効果を発揮する。対照的に、Am80は、RARγではなく、したがってAPL患者の治療におけるRA活性化RARγにより誘発された副作用を避け、RARαの選択的結合のために設計されたレチノイドアゴニストである。本発明者らは、Am80が、細菌感染に対抗して好中球の分化を著しく強化する一方、骨髄性増殖を促進することを実証した。これはG−CFSとは対照的に、効果的な好中球分化を誘発できないが、骨髄性増殖を大いに促進する。組み合わせたAm80−GCSFによる治療が、着実な細菌感染という状況下における、好中球減少症マウスの死亡率を著しく減少させるために、Am80よりも著しく効果的である、という本発見は、Am80によるRARαの選択的活性化は、効果的な好中球分化に貢献できるが、Am80が媒介する量の少ない骨髄性増殖は、好中球減少症を伴う着実な細菌感染という状況下における、成熟好中球の適切な数の要求を満たさない、ということを示唆する。これらの知見は、骨髄性増殖と好中球分化の両方を十分に調和させるためには、組み合わせたAm80−GSCFの使用が必要であることを示し、それにより、がん化学療法誘発性の好中球減少症の治療のための、現在のG−CSF療法では効果がないことを克服する。
RARαは、CDK7、サイクリンH、及びMAT1タンパク質からなる、サイクリン依存性キナーゼ活性化キナーゼ(CAK)複合体の基質である。CAKは、異なるCDKのリン酸化による活性化により細胞周期の進行を調節するフリーCAKとして、またはRNAポリメラーゼIIC−末端ドメイン(RNA PII)のリン酸化を通して転写を調節する、TFIIH複合体のキナーゼサブユニットとのいずれかとして、細胞中に存在する。中でも注目すべきは、フリーCAKまたはTFIIH含有CAKのいずれかが、顆粒球の分化を抑制するためにRARαをリン酸化し(図1)、一方、本質的にプログラム化されたかまたはRA媒介性であるかのいずれかの、MAT1タンパク質の断片化により媒介されるRARαのCAKリン酸化における抑制が、RA標的遺伝子のRARα依存性転写を通した顆粒球分化を誘発する(図2)ことである。本発明者らは、最近、Am80の効果は、RARα転写因子の選択的活性化から起こり、RARαのCAKリン酸化を抑制することにより、RA標的遺伝子の選択的発現を誘発することを発見した。Am80が誘発する骨髄性増殖は、細胞周期抑制因子p21Cip/Kipの低下レベルの一方、好中球エフェクターCD18、二次顆粒LL−37、及び骨髄特異的転写因子CCAAT/エンハンサー結合タンパク質イプシロン(C/EBPε)の発現の増加に示されるように、RA標的遺伝子の変化した転写発現の基礎を成す。このRA標的遺伝子のAm80が誘発する選択的発現は、RA刺激とは異なり、G−CSFまたはRAと比較した場合、好中球の殺菌機能の効果的な発達、著しいMAT1の断片化、低下したRARαのリン酸化、及び正常または悪性の造血前駆細胞における活性酸素(ROS)の際立って高い生成と関連している。これらのデータは、顆粒球形成に対する転写応答の制御において、レチノイドが媒介するCAK−RARαシグナル伝達経路を介して、Am80のRARαへの選択的結合が、量の少ない骨髄性増殖及び効果的な好中球分化を調節する、新規の転写応答を誘発することを示唆している(図3)。Am80−GCSFの組み合わせが、好中球減少症マウスの死亡率を際立って下げるという当発明者らの発見を伴うこれらの研究に基づき、特定の理論に帰結することを望まないが、当発明者らは、Am80−GCSFの組み合わせの相乗効果は、CAK−RARαの作用モードの介在に依存して良く、骨髄造血前駆細胞の最適な増殖及び好中球の効果的な最終分化の両方のクロス調節が、感染及び感染誘発性死亡率の低下に繋がると考える。本発明者らの研究は、がん化学療法誘発性の好中球減少症の患者の治療のための、Am80またはGCSFいずれかの単一の投薬計画の利用の欠点を克服する、効果的な治療法を提供する。
Ding et al.(Blood 2013 Vol 121 page996)、Trillet−Lenoir et al.(Eur J Cancer 1993; Vol 29A(3),page319)、Lalami et al.(Ann Oncol 2006;Vol 17(3),page507)、及びAbdul Rasool Hassan et al.(Asian Pac J Cancer Prev,2011;Vol 12(6),page1425)によって示されるように、がん化学療法の使用は、好中球減少症を誘発することがある。がん化学療法により誘発される重症の好中球減少症の期間の中央値は、ヒトで3〜6日間(ANC<0.5x109/L)及びマウスで2〜3日間であり、ここでGCSFは、好中球の数を増やすことにより、ヒト及びマウスの両方における好中球減少症の期間を短くすることができる(図4)。本開発者らの研究は、微生物感染のリスクが著しく高まる期間である、マウスの好中球減少症の3分の2の時期を、好中球減少段階(ヒトまたはマウスのいずれかにおいて、投薬計画で、好中球の減少を防ぐことができない)及び好中球回復段階(BM機能が、依然として抑制されている)に分けた(図4、右パネル)。好中球減少症の全周期は、好中球減少症の誘発、好中球減少症の減少、好中球減少症からの回復、及び好中球の復帰から正常段階を占める(図4、右パネル)。好中球減少症マウスの死亡率の低下におけるAm80−GCSF組み合わせの効果を、継続的な菌血症状態を維持しているマウスの、好中球減少症の全周期の期間で試験した。
Am80の生体内効果を評価するために、本発明者らは、好中球減少マウスにおいて、成熟好中球の再生におけるAm80及びGCSF(図5a)の異なる、高、中、及び低用量のヒト等価用量の半数効果濃度(EC50)を、最初に試験した。正常C57BL6/Jマウスに、マウス好中球を減らすために、シクロホスファミド(CPA)を単回投与で注射した。CPA注入の4時間後から、マウスを殺処分する前の連続3日間で、賦形剤、Am80、GCSF、またはAm80−GCSFで治療した(図5b)。ブランクマウスの末梢血(PB)のフィコール(Ficoll)分離で、成熟好中球が、その低層に分別されていることが示された(図5c)。その他の全ての治療との比較で、Am80またはAm80−GCSFの低用量及び中用量が、好中球レベルの維持において、良好なEC50効果を示し(図5d、右パネル)、一方Am80、及びAm80−GCSFの低用量は、その中用量よりも良好な顆粒球の形態分化を強化した(図5e)。さらに、骨髄(BM)細胞の複数の密度によるパーコール(Percoll)分離は、ブランクマウスの成熟BM好中球が、第3層に分別されたことを示した(図5f)。特に、GCSFの中用量が、第3層BM細胞の最も高い回復を示したが(図5g、右パネル)、これらの細胞は、Am80−GCSFの低用量によって誘発されたものに比べて、顆粒球の形態解析によって示されるように、より少ない分化であった(図5h、上下パネル)。同時に、これらの結果は、Am80、及びAm80−GCSFの低用量及び中用量は、マウス好中球減少症の早期段階における、PB及びBMの両好中球の中程度のレベルを維持できることを示唆している。*:P<0.05、**:P<0.01、***:P<0.005。データは、同様の結果を伴う2つの独立した実験を表している。
図6Aは実験デザインである。C57BL/6Jマウス(n=19)を、無差別に5グループに分けた(n=3のグループをコントロールに、n=4のグループを各実験グループに)。マウス好中球減少症を、0日目にCPAの腹腔内注射により誘発した。CPA注射の4時間後に、経口によるAm80若しくは皮下注射によるGCSF、または経口による賦形剤を伴う異なる投薬計画を、マウスに一日一回与えた。マウスに、2日目に尾静脈注射を通して、黄色ブドウ球菌の6.3x106CFUにより負荷を与えた。注入の3時間後に、好中球の初期の殺菌活性の分析のために、尾部から各100μlの血液を集めた。当該マウスは、PB及び心臓の採取のために、注射の16時間後に屠殺した(図6B)。PB中のWBC及び好中球のベトスキャン解析を実施した。ブランクマウスを除き、他の全てのグループに、好中球誘発は、観察されなかった(図6C〜E)。PB及び心臓(パネルE)における、好中球の殺菌活性を、生存している細胞外細菌の血液寒天培養分析を使用して、注入の3時間(図6C)及び16時間後(図6D)に決定した。GCSFまたはAm80またはAm80−GCSFマウスにおいて、好中球誘発が、細菌感染により引き起こされなかったので、Am80マウスにおける著しく増加した好中球による殺菌活性が、マウス好中球減少症の早期発達段階での、既存の顆粒球前駆細胞から成熟好中球への、Am80で促進された分化をもたらすことを示している。*:P<0.05、**:P<0.01、***:P<0.005。A+G:Am80−GCSF。

C57BL6/Jマウスを、無差別に5グループに分けた(n=3のグループをコントロールに、n=4のグループを各実験グループに。n=19)。(図8a)に示すように、マウスに、CPA注射(200mg/kg)の2日後に、投薬計画を与えた。治療後3日連続で、マウスに、安楽死の16時間前まで、黄色ブドウ球菌による負荷を与えた。本発明者らは、細菌感染後の全てのグループに、末梢血(PB)好中球の大幅な増加を見出した(図8b)。注入の3時間後及び16時間後に採取したPBを、好中球の殺菌活性の解析に使用した。独立した死滅アッセイ(killing assay)は、Am80単独ではなく、Am80−GCSFの組み合わせにより誘発された好中球が、著しく多数の細菌を殺傷したことを示した(図8c)。好中球生成の評価により、本発明者らは、Am80が、BM及びPBの両方において、GCSFまたはAm80−GCSFに比べて、最低限の好中球を誘発したことを見出した(図8d、8e)。GCSFは、Am80−GCSFと比べて、BMにおける著しく多量の、PBにおいても同じく多量の好中球を誘発したが(図8d、8e、下部パネル)、GCSF誘発の好中球は、Am80−GCSFに誘発されたものに比べ、著しく低い殺菌活性を見せた(図8c)。さらに、Am80−GCSF組み合わせにより誘発された好中球は、より良好な顆粒球形態分化を見せた(図8d、8e、左パネル)。これらの結果は、好中球回復の間、BM機能が抑制されている場合、GCSFに誘発された多数の好中球が、細菌殺傷に関して欠陥があるのに対して、ごく少ない数の顆粒球前駆細胞が、Am80により成熟好中球に分化できることを示している。しかしながら、低用量のAm80−GCSF治療で、Am80は、微生物感染に対抗して、GCSFが再生した顆粒球前駆細胞を、効果的に成熟好中球に分化させることができる。P<0.05、**:P<0.01、***:P<0.005。
Am80が、感染に対抗して、GCSFが再生した多量の顆粒球前駆体を成熟好中球に実際に分化できる場合、Am80−GCSF組み合わせは、がん化学療法誘発性の好中球減少症において、感染関連死亡率を効果的に減らせるはずである。本発明者らは、好中球減少の全期間を通して菌血症を経験している好中球減少マウスを使用して、この仮説を試験した。(図9A)に示すように、マウスに、8日目まで、CPA注射4時間後の異なる投薬計画を与えた。3日目に、黄色ブドウ球菌感染を実施し、及び9日目に、生存率を計算した。3つの独立した実験により、Am80−GCSFは、GCSFまたはAm80単独の場合と比べて、その生存率を著しく高めた、と判断した(図9B、9D)。生存マウスにおける再生好中球の殺菌活性を評価するために、11日目の生存したブランク及びAm80−GCSFマウスを、安楽死の15分前に、さらに黄色ブドウ球菌に感染させた。ギムザ染色及びグラム染色の両方による新鮮な単離PBの並行分析で、感染マウスにおける好中球貪食対非感染マウスにおける無菌血症を決定した(図9E)。GCSFは通常、CCIN≧24時間後の抗がん剤に指定されているので、発明者らはさらに、(図9F、9J)に示すように、各投与に対して2回の独立した実験を伴い、CPAの24時間後に、Am80−GCSFの低・中用量を試験した。低用量試験からの結果は、GCSFグループは、たった12.5%の生存率であった一方で、ブランクまたはAm80−GCSFグループのいずれにおいても、死亡率は観察されなかった(図9g〜i)ことを示した。しかしながら、中用量の試験においては、Am80−GCF(55.6%)及びGCSF(11.1%)のグループ間で、当該生存率に統計的な有意差はなかった(図9K〜M)。全体として、これらの生存マウスモデルは、低用量のAm80−GCSFが、好中球減少マウスの感染関連死亡率を減らすことを実証した。
ウエスタンブロット法は、Am80による、CD34陽性細胞におけるMAT1フラグメント(顆粒球の分化を示している)(図10A)及びレチノイン酸受容体(RARα)の低リン酸化(すなわち、顆粒球分化の促進を示すRARαの低下したリン酸化)(図10B)を描いている。新規の遺伝子転写パターンが、オールトランスレチノイン酸(ATRAまたはRA)とは対照的に、Am80によって誘発されている。mRNAレベルにおける、C/EBPε、CD18、及び顆粒LL−37の著しい増加調節、及びp21Cip/Kipの減少が、後続するAm80投与によって観察された(図10C)。
Am80−GCSFは、NB4細胞の増殖を抑制する一方で(図11A)、ホルミル−メチオニル−ロイシル−フェニルアラニン(f−Met−Leu−Phe;fMLP)(図11C)またはPMA(図11D)のどちらかの存在下における、6日目の顆粒球分化(図11B)及びROS生成を、著しく促進する。*:少なくともP<0.04。G−CSFは、fMLPまたはPMAのどちらかの存在下における、NB4細胞でのROS生成の誘発に失敗している。
急性骨髄性白血病患者からのPB初代芽球の増殖を試験し(図13A)、P<0.05と定量した(図13B)。AMLのPB初代芽球の顆粒球形態を分析し(図13C)、Am80対G−CSFはP<4.0x10−8、Am80対コントロールは、P<6.02x10−7であると定量した(図13D)。
3人の正常ヒトドナーのPBから、単核細胞の上層を単離し、低用量のAm80、GCSF、またはAm80−GCSFと共に、顆粒球系統培地で、4日間まで培養した(図14A、B)。各ドナーにおいて、Am80−GCSFに誘発された好中球は、Am80またはGCSF単独との比較において、fMLP(図14C)またはPMA(図14D)のどちらかの存在下で、著しく高いレベルのROS生成を示した。Am80−GCSFは、正常ヒトドナーから単離された末梢血単核細胞でのROS生成を著しく促進する。

- 対象の疾患を治療し、予防し、発達の可能性を抑え、その重症度を軽減し、その予防を促進し及び/またはその進行を遅らせる方法であって、
