Abstract
Description
本明細書で使用する用語「FGF−18化合物」又は「FGF−18」は、ヒトFGF−18タンパク質の生物学的活性を少なくとも1つを保持するタンパク質であるという意図である。FGF−18は、天然型、その成熟型、組換え型、又は切断型であってもよい。ヒトFGF−18タンパク質の生物学的活性としては、特に軟骨細胞または骨芽細胞の活性増加(WO98/16644)、あるいは軟骨形成の増強(WO2008/023063)が挙げられる。天然又は野生型のヒトFGF−18は、関節軟骨の軟骨細胞によって発現されるタンパク質である。ヒトFGF−18は、当初zFGF−5と命名されWO98/16644に詳述されている。配列番号1は、天然のヒトFGF−18のアミノ酸配列に相当し、アミノ酸残基1(Met)〜27(Ala)から成るシグナルペプチドを有する。ヒトFGF−18の成熟型は、配列番号1の残基28(Glu)〜残基207(Ala)のアミノ酸配列(180個のアミノ酸)に相当する。
本実施例の組み換え切断型FGF−18(rhrFGF18)は、WO2006/063362に記載される技術に従い大腸菌における発現により調製した。以下の実施例において、rhrFGF18、FGF−18、及びspriferminは、互換的に使用される。
方法:
新鮮な硝子軟骨を若いウシ膝(生後3〜6ヶ月)の滑車溝から採取した。8mmの円筒状外植片(図1A)を、生検パンチにより取り出し、完全培地(DMEM4.5g/L、D−グルコース及びL−グルタミン、10%FBS、1%PSF、1%ファンギゾン、1%MEM ビタミン、25mM HEPES及び50μg/mlのビタミンC)において一晩、培養した。試料は、骨をトリミングし、欠損部(4mmの直径)を作成し、コア及び環状修復構造体(図1B)を形成した。内部コア及び外環の両者を24時間、別々に培養し、その後、欠損部に、元のコアを充填した。次に、試料を完全培地において培養するか、又はSprifermin(rhFGF18、100ng/ml)により処理した。処理は、週に1回rhFGF18を24時間添加するか(1+6)(毎週繰り返す)、又は24時間の処理を1回行った後、完全培地において1ヶ月培養した(1+30日)。試料を、4週間の培養の後、採取した。押出し機械的試験(n=4〜6)を、カスタム試験リグ(図2E、[3])を用いて実施した(Instron 5848, Instron, Norwood, MA)。ピークの力をインテグレーション領域で除算することによりインテグレーション強度を計算した。3D可視化のために、試料(n=6)を、改変ルゴール溶液(蒸留水中、2.5%I2及び5%KI)に24時間浸し[4]、55kVのエネルギーレベル及び145μAの強度で、6μm及び10.5μmのボクセルサイズのμCT(μCT 35及びvivaCT 40、SCANCO Medical, Wayne, PA)によりスキャンした。スキャンを、製造業者のソフトウェアを用いて、分析し、再構成し、そして断面を用いて、欠損部のインテグレーションを評価した。追加試料(n=3)を、4%PFAにおいて一晩固定し、細胞及びその界面でのマトリクス沈着について、組織学的分析した。
対照試料のインテグレーション強度(図2D)は最も低く(2.5±1.4kPa)、1+30(毎月のサイクル)(5.0±2.4kPa)及び1+6(毎週のサイクル)(10.2±3.7kPa)処理の順で、次第に上昇した。その結果は、研究において可能な再生数で対照及び処理群を比較すると顕著であったが、統計学的有意性はなかった。対照構造体のμCT分析(図3、上部左側)は、明確な暗色の円状部を示し、外環と内部コア間の分離、つまり、インテグレーションが不良であることが示唆される。1+30処理(図3、中央左側)は、あまり明確でない円状部を示し、分離度が小さく、インテグレーションが高いことを示唆し、そして1+6処理(図3、底部左側)は、界面を通して非常に均質なμCTシグナルを示し、インテグレーションの度合いが最高であることが示す。インテグレーションがこのように向上するという証拠は、試料全体の垂直断面及び横断面の両者で明らかであった。
この研究は、Spriferminが、軟骨修復モデルにおいて軟骨表面のインテグレーションを改善可能であることを実証する。この発見は、軟骨様の生体材料が、臨床学的な好結果を達成するために、天然の軟骨と正常にインテグレーションする必要があるような例えばOATSといった手術手順、及び組織工学アプローチにおいて、有用性がある可能性を示す。
方法:
初代変形性関節症の軟骨細胞を、膝関節全置換術を受けた患者の軟骨から単離した。処置前に、細胞を、まず、単層培養下で数日間培養し、そして次に、足場のない3D培養下で1週間培養した。処理は、合計4週間にわたり、永続的に又は1日/週、rhFGF18[100ng/ml]を用いたインキュベーションをおこなった。結果を、Spriferminを含まない対照培地と比較した。生化学的アッセイ、定量的PCR(qPCR)及び組織学的アッセイを用いて、3D構造体を特徴づけた。
確実に表現型を維持するために、3D足場のない培養を用いて、hCA軟骨細胞に対する効果を試験した。この設定においては、rhFGF18[1日/週]が、細胞含有量に対して有益な効果を有し、そして3D構造体のサイズ及びマトリクス含有量(GAG及びHPro含有量)を非常に高めることが見出された。また、rhFGF18は、未処理の細胞に比べて、I型コラーゲンの発現が低減することも見出された。
ウシ及びブタ軟骨細胞に関してのこれまでの研究において見られるように、spriferminがhOA軟骨細胞において同化活性を有することが見出された。この発見は、軟骨様の生体材料が、臨床学的な好結果を達成するために、天然の軟骨と正常にインテグレーションする必要がある組織工学アプローチにおいて有用性がある可能性を示す。
方法:
ブタ軟骨細胞を、ブタ股関節の大腿骨頭の軟骨から単離した。関節の切断後、軟骨を採取し、0.25%のコラゲナーゼ(2.5%のコラゲナーゼNBG4を含むHAM’s F12の1/10希釈液)を用いて室温で45分消化した。分離した細胞を廃棄し、軟骨を0.1%のコラゲナーゼを用いて一晩更に消化し、軟骨細胞を抽出した。ブタ軟骨細胞を、CTA(軟骨組織類似体)として懸濁液内で培養した。培養は、第1週は何れの処理もなしに、続いて次の処理の1つを行った:1)10又は100ng/mlのrhFGF18の永続的な存在下での4週の培養、2)10又は100ng/mlのrhFGF18の存在下での1週間の培養に続いて、rhFGF18なしでの3週間の培養、3)1週間に1日、10又は100ng/mlのrhFGF18の添加(すなわち24時間の暴露に続きrhFGF18なしでの6日の培養)による3週間の培養に続いて、rhFGF18なしでの1週間の培養、又は4)対照としてのrhFGF18の非存在下での4週間の培養(図4)。培養期間の最後にCTAを採取し、それらのGAG、ヒドロキシプロリン及び細胞含有量について分析した。I型及びII型コラーゲン及びSox9についての遺伝子発現を評価し、そしてサフラニンO、I型及びII型コラーゲンについての組織学的検査も実施した。
各培養条件で、CTAを溶解し、そしてDNA含有量を評価し、CTA当たりの細胞数を計算した(図5)。対照培養(rhFGF18なし)においては、細胞数(120万個)は摂取密度(100万個の細胞/CTA)と同様で、増殖は観察されなかった。しかしながら、予想通り、rhFGF18の永続的な存在により、軟骨細胞の増殖が増加した(それぞれ10及び100ng/mlのrhFGF18によりそれぞれ220万個の細胞/CTA及び249万個の細胞/CTA)。rhFGFが1週のみ添加され、軟骨細胞がさらにrhFGF18なしで3週間培養される場合(1w)、対照に比べて増殖の増加は、観察されなかった。逆に、rhFGF18が1週間に1日添加される場合(1d/w)、rhFGF18は、対照と比べて増殖を刺激し、永続的な暴露に比べても増殖を刺激した。CTA当たりの細胞含有量は、100ng/mlのrhFGF18の1日/週添加により、400万個の細胞/CTAに達した。これに比べて、rhFGF18の不在下では120万個の細胞/CTA、又は100ng/mlのrhFGF18の永続的な存在下では249万個の細胞/CTAであった。
各培養条件で、CTAをプロティナーゼKにより消化し、そしてGAG及びヒドロキシプロリン含有量を評価した(図6)。GAGはプロテオグリカン含有量を反映し、ヒドロキシプロリンはCTAのコラーゲン含有量を反映する。前に観察されたように、rhFGF18の永続的な存在は、GAG含有量/CTAを低減し(対照に比較して、2.6倍低い)、またヒドロキシプロリン含有量/CTAも低減した(対照に比較して、2.1倍低い)。逆に、rhFGF18を間欠的に(1/週または1日/週)添加する場合、GAG及びヒドロキシプロリン含有量は増加した。例えば、100ng/mlのrhFGF18を1週間あたり1日添加する場合、対照に比較して、GAG含有率は2.67倍高く、ヒドロキシプロリン含有量は2.13倍高かった。
各培養条件で、RNAをCTAから単離し、そしてI型、II型、X型コラーゲン、及びSox9発現を、定量的PCRにより分析した(図7)。Sox9及びII型コラーゲンの高発現は、軟骨細胞表現型のマーカーであり、I型コラーゲンは、脱分化のマーカーであり、そしてX型コラーゲンは、軟骨細胞肥大のマーカーである。コラーゲンII/Iの比率も計算した。この比率は通常、培養における軟骨細胞の表現型の維持(高い比率)又は表現型の損失(低い比率)を示すために使用される。rhFGF18を用いたすべての条件において、10又は100ng/mlでの永続的な又は間欠的な暴露により、I型コラーゲンは低減した。この低減は、rhFGF18が100ng/mlの濃度で、1週間に1日添加される場合に最も強かった。例として、I型コラーゲンの発現は、対照に比較して、100ng/mlのrhFGF18を永続的に添加する場合は4倍、100ng/mlのrhFGF18を1週間に1日添加する場合は123倍低減した。II型コラーゲンは、rhFGF18の永続的な存在下で低減することが発見されたが、1週(1w)又は1週間に1日(1d/w)添加されるrhFGF18の存在下ではほとんど不変であった。Sox9発現においては重大な変化は観察されなかった。Sox9発現は、10ng/mlのrhFGf18を1週間(1w)添加する場合のみ、有意に増加した(2.2倍)。最終的に、rhFGF18の永続的な投与は、コラーゲンII/Iの比率に対しては効果がないことが見出されたが、しかしrhFGF18が1週間に1日添加される場合、この比率は、対照に比較して、それぞれ10及び100ng/mlのrhFGF18により19倍及び138倍増加した。また、X型コラーゲンも軟骨細胞肥大のマーカーとして評価され、本培養条件下でrhFGF18により影響されないことが見出された。
様々なrhFGF18暴露による4週間の処理の後、CTAの組織学的分析により、rhFGF18が永続的に存在すると、他の条件に比較して、CTAはより薄くなり、サフラニンO染色は薄くなることが示された。さらに、rhFGF18の永続的な存在下で、細胞密度が高く細胞外マトリクスがない増殖ゾーンが、その構造体の周囲で観察されることがある。一方、rhFGF18を間欠的に暴露すると、対照に比較して、構造体の厚さが増すことも観察され得る。すべての条件下で、I型コラーゲンは検出できなかったが(示さず)、すべてのCTAでII型コラーゲンの染色が強かった。
rhFGF18の永続的な暴露は軟骨細胞の増殖を刺激したが、CTA中のマトリクス含有量は低減した(GAG及びヒドロキシプロリンの低減)。同様に、I及びII型コラーゲンの両者の発現も対照に比較して低減した。10又は100ng/mlのrhFGF1への永続的な暴露の有意な効果は、4週の処理の後のSox9では観察されなかった。この組織学的分析により、CTAが小さいことが明らかになり、そしてCTAの周囲でECMを欠いた増殖ゾーンが示された。すべてのそれらの結果をまとめると、rhFGF18が永続的に存在すると、マトリクス産生よりも増殖のようが有利になることが示唆される。
方法:
ウシ軟骨細胞を、実施例2及び3に記載するように入手した。それらを、永続的に存在する、100ng/mlのrhFGF18と共に(FGF18)、又は対照としてFGF18の不在下(CTR)で、1又は2週間、培養した。培養後、細胞を採取し、計数し、溶解してRNA単離及び遺伝子発現を解析した。Sox9、I型、II型コラーゲン発現を、定量的PCRにより評価した。
永続的なFGF18を用いた2週間の培養の後、細胞濃度は対照群よりも高かった。I型コラーゲンの発現はrhFGF18の存在下で強く抑制されたが、II型コラーゲン及びSox9の発現は増加した(図8)。
軟骨細胞が単層で培養される場合、100ng/mlのrhFGF18の永続的な暴露により、細胞増殖の増加が可能になり、良好な表現型の維持も可能になる(II型コラーゲン及びSox9の発現が高まり、そしてI型コラーゲンの発現は低下した)。
方法:
膝関節全置換術を受けた2人のOA患者から得た軟骨を使用した。軟骨細胞を、実施例3に記載のようにして単離し、そしてまず、単層下で高密度で3〜4日間、培養した。続いて、軟骨細胞を採取し、そして96ウェルプレートに、1×106個の細胞/200μlで接種し、何れの処理もせず1週間、凝集させCTAを形成させた。続いて、100ng/mlのrhFGF18の不在又は存在下の以下の処理の1つを行い、さらに4週間培養した:1)rhFGF18の不在下での4週間の培養(対照)、2)rhFGF18の永続的な存在下での4週間の培養(perm)、及び3)1週間に1日rhFGF18の添加(すなわち、24時間の暴露に続きrhFGF18なしでの6日の培養)による4週間の培養(1d/w)(図4)。培養期間の後にCTAを採取し、それらのGAG、ヒドロキシプロリン及び細胞含有量について分析した。患者2から入手されたCTAに関しては、I型及びII型コラーゲン及びSox9についての遺伝子発現を評価し、そしてサフラニンO、I型及びII型コラーゲンについての組織学的検査も実施した。
細胞増殖:
100ng/mlのrhFGF18により、3D培養でヒト変形性関節症の軟骨細胞の増殖が増大した(図9を参照)。患者1から得た軟骨細胞については、対照のCTA当たりの細胞数は、初期細胞数よりも少なく(接種密度は100万個の細胞/CTAであった)、このことは、多くの細胞が死滅したことを示唆する。しかしながら、永続的又は1日/週のrhFGF18の存在下では、150万個の細胞/CTAが見られ、このことは、細胞が死滅しないで増殖したことを示唆する。患者2から得た軟骨細胞においては、細胞数のわずかな増加(CTA当たり100万〜130万個の細胞)が、未処理のCTAにおいて観察され得る。さらに、永続的なrhFGF18の存在下で細胞数が増加した(CTA当たり、100万〜190万個の細胞)。
マトリクス産生:
100ng/mlのrhFGF18により、3D培養でヒト変形性関節症の軟骨細胞によるGAG産生が増大した(図10を参照)。永続的な及び1日/週のrhFGF18を受けた患者1、ならびに永続的なFGF18を受けた患者2においては、有意に多くのGAGがCTAに存在した。
遺伝子発現:
軟骨細胞表現型は、I型コラーゲン発現の低減又は不存在、そしてSox9及びII型コラーゲン発現の増加という特徴を有する。この発現パターンは、変形性関節症の軟骨細胞において異なっていた(図11を参照)。実際、未処理のCTAにおいて、I型コラーゲンの発現はII型コラーゲンの発現よりも高かった(それぞれ、0.67及び0.04の相対存在量)。rhFGF18はI型コラーゲンの発現を低減し、II型コラーゲンの発現を増加できた。結果として、コラーゲンII/Iの比率は、rhFGF18の存在下で、11〜13倍に上昇した。さらに、rhFGF18は、Sox9、すなわち軟骨細胞表現型のマーカーの発現を増加した。
組織学的検査(データは示さず):
対照と比較して、1日/週又は永続的なrhFGF18を用いて培養したCTAは、サフランO染色の増強を示し、このことは、それらがよりGAGを含むことを示唆する。これは、図10に提供される結果と一致する。I型コラーゲン染色はrhFGF18処理された細胞において低減し、これは図11の遺伝子発現結果に良好に一致する。II型コラーゲン染色は対照培養物において見られず、このことはヒト変形性関節症(hOA)軟骨細胞が軟骨様マトリクスを生成できないことを示唆する。しかしながら、1日/週及び永続的なrhFGF18は両者とも、II型コラーゲンを産生するhOA軟骨細胞の能力を回復できた。
ヒト変形性関節症の軟骨から単離された軟骨細胞により得られた結果により、rhFGF18が細胞増殖を促進でき、硝子様軟骨マトリクス産生を増強でき、そして軟骨細胞の表現型に好適であることが示された。この実験において、永続的及び1日/週でのrhFGF18は、いくつかのパラメーターに関しては同等であった。しかし、マトリクス産生に関して、1日/週でのrhFGF18のほうがやや良好であった(患者1においてはGAG蓄積が増大し、患者2においては、コラーゲンIIの発現が増大した)。

Claims (17)
- 組織工学用の移植可能な軟骨材料を製造する方法であって、
FGF−18化合物を含有する培養培地内で、移植可能な軟骨材料を形成するのに十分な時間、単層培養または3D培養のいずれかで、軟骨形成細胞を培養する工程を含む前記方法。 - 軟骨障害による関節軟骨欠損の領域における哺乳動物の軟骨を再生する方法であって、
(a)FGF−18化合物を含有する培養培地内で、単層培養または3D培養のいずれかで、軟骨形成細胞を培養する工程;
(b)工程(a)から得られた培養軟骨形成細胞をそれを必要とする哺乳動物に投与する工程;
を含む前記方法。 - 哺乳動物の軟骨組織における欠損を治療する方法に使用するためのFGF−18化合物であって、
ここで、前記軟骨の欠損は軟骨障害によるものであり、
該方法は、
(a)軟骨形成細胞をインビトロで培養する工程、ここで該培養は、FGF−18化合物を含む細胞培養培地内で行う;
(b)場合により、工程(a)を繰り返して、培養軟骨形成細胞を含む移植材料を得る工程;並びに、
(c)工程(b)の移植材料を、前記治療を必要とする哺乳動物の欠損部へ移植する工程;を含み、
ここで、工程(a)および工程(b)の間、軟骨形成細胞は、単層培養または3D培養のいずれで培養してもよい、前記FGF−18化合物。 - 前記FGF−18化合物は培養培地に1週間あたり約1、2、または3日、間欠的に添加され、前記約1、2、または3日の添加は、少なくとも2週間の培養、少なくとも3週間の培養または少なくとも4週間の培養の間、各週ごとに繰り返される、請求項1または2に記載の方法あるいは請求項3に記載の使用のためのFGF−18化合物。
- 前記FGF−18化合物は培養培地に1週間あたり1、2、または3日、間欠的に添加され、前記1、2、または3日の添加は、2週間の培養、3週間の培養または4週間の培養の間、各週ごとに繰り返される、請求項1または2に記載の方法あるいは請求項3に記載の使用のためのFGF−18化合物。
- 前記FGF−18化合物は培養培地に1月あたり約1、2、または3日、間欠的に添加され、前記約1、2、または3日の添加は、少なくとも2月間の培養、少なくとも3月間の培養、または少なくとも4月間の培養の間、各月ごとに繰り返される、請求項1または2に記載の方法あるいは請求項3に記載の使用のためのFGF−18化合物。
- 前記FGF−18化合物は培養培地に1月あたり1、2、または3日、間欠的に添加され、前記1、2、または3日の添加は、2月間の培養、3月間の培養、または4月間の培養の間、各月ごとに繰り返される、請求項1または2に記載の方法あるいは請求項3に記載の使用のためのFGF−18化合物。
- FGF−18化合物を含む培地中で培養された哺乳動物の軟骨形成細胞を含む、軟骨障害を有する哺乳動物における組織工学における使用のための組成物。
- 前記軟骨障害は、変形性関節症、軟骨損傷、または骨軟骨欠損である、請求項2、4〜7のいずれか1項に記載の方法、請求項3〜7のいずれか1項に記載の使用のためのFGF−18化合物、あるいは請求項8に記載の組成物。
- 前記軟骨形成細胞は、成熟組織に由来する軟骨細胞または間葉系幹細胞である、請求項1、2、4〜7、9のいずれか1項に記載の方法、請求項3〜7、9のいずれか1項に記載の使用のためのFGF−18化合物、あるいは請求項8または9に記載の組成物。
- 前記FGF−18化合物は、
a)配列番号1の第28〜207位の残基を含むヒトFGF−18成熟型を含むまたは該成熟型から成るポリペプチド;
b)配列番号1の第28〜196位の残基を含むかまたは該残基から成るポリペプチド;および
c)配列番号2を含むかまたは配列番号2から成るポリペプチド;
から成る群より選択される、
請求項1、2、4〜7、9、10のいずれか1項に記載の方法、請求項3〜7、9、10のいずれか1項に記載の使用のためのFGF−18化合物、あるいは請求項8〜10のいずれか1項に記載の組成物。 - 前記軟骨形成細胞は、拡大または培養前に哺乳動物から採取または単離される、請求項1、2、4〜7、9〜11のいずれか1項に記載の方法、請求項3〜7、9〜11のいずれか1項に記載の使用のためのFGF−18化合物、あるいは請求項8〜11のいずれか1項に記載の組成物。
- 前記軟骨形成細胞は、治療対象の哺乳動物または異なる哺乳動物から採取または単離される、請求項1、2、4〜7、9〜12のいずれか1項に記載の方法、請求項3〜7、9〜12のいずれか1項に記載の使用のためのFGF−18化合物、あるいは請求項8〜12のいずれか1項に記載の組成物。
- 哺乳動物はヒトである、請求項1、2、4〜7、9〜13のいずれか1項に記載の方法、請求項3〜7、9〜13のいずれか1項に記載の使用のためのFGF−18化合物、あるいは請求項9〜13のいずれか1項に記載の組成物。
- 軟骨障害の治療において使用するための、請求項1に記載の方法により得られる移植可能な軟骨材料または請求項2に記載の方法により得られる再生軟骨。
- 前記軟骨障害は、変形性関節症、軟骨損傷または骨軟骨欠損である、請求項15に記載の移植可能な軟骨材料または再生軟骨。
- 前記FGF−18化合物は、培養培地内に永続的に存在する、請求項1または2に記載の方法、あるいは請求項3に記載の使用のためのFGF−18化合物。
