の実施形態では、IL−10発現構築物を微小粒子中に任意選択でカプセル化する。IL−10発現構築物をカプセル化する技術は、用いられる微小粒子のタイプに応じて異なり、このような技術については後に詳しく説明する。本発明の微小粒子は、任意の生分解性ポリマーから構成されるものでよい。本発明の制御薬剤送達製剤において生分解性ポリマーとしての使用を成功させるために、材料は、化学的に不活性で、かつ溶出性不純物を含まないものでなければならない。理想的には、ポリマーは、適切な物理的構造も有し、望ましくない経年変化が最小で、しかも、容易に加工可能である。材料のいくつかは、ポリ(2-ヒドロキシエチルメタクリレート)、ポリ(N−ビニルピロリドン)、ポリ(メタ
クリル酸メチル)、ポリ(ビニルアルコール)、ポリ(アクリル酸)、ポリアクリルアミド、ポリ(エチレン−コ−酢酸ビニル)、ポリ(エチレングリコール)、及びポリ(メタクリル酸)を含む。本発明において特に有用な生分解性ポリマーとしては、ポリラクチド(PLA)、ポリグリコリド(PGA)、ポリ(ラクチド−コ−グリコリド)(PLGA)、ポリ無水物、ポリカプロラクトン、ポリ−3−ヒドロキシ酪酸、及びポリオルトエステルが挙げられる。このような生分解性ポリマーは、広範囲にわたって特性決定されており、当分野で公知のように、要望される分解特性を発揮するように製剤化することができる(例えば、エドルンド(Edlund)及びアルバートソン(Albertsson)著、分解性脂肪族ポリエステル(Degradable Aliphatic polyesters)、p.67〜112(2002年)、バーマン(Barman)ら著、制御放出学誌(J.of Controlled Release)、第69巻、p.337〜344(2000年);コーヘン(Cohen)ら著、薬剤研究(Pharmaceutical Res.)、第8号、p.713〜720(1991年)を参照のこと)。
送達しようとする30%のpDNAを、例えば、3週間〜6週間の放出プロフィールを有する微小粒子中にカプセル化してもよく;送達しようとする30%のpDNAを、例えば、6週間〜10週間の放出プロフィールを有する微小粒子中にカプセル化してもよく;送達しようとする30%のpDNAを、例えば、8週間〜12週間の放出プロフィールを有する微小粒子中にカプセル化してもよい。このような実施形態では、単一の種類の生分解性ポリマーを用いるが、これを異なる放出プロフィールを有する製剤に使用してもよく;あるいは、様々な放出特性を有する異なる生分解性ポリマーを用いてもよい。また別の実施形態では、微小粒子の製剤は、例えば、必要に応じて、関節の滑液からの治療組成物の移動を増強するか、又は遅らせるために、微小粒子の表面を変化させるように改変してもよい。

以下の実施例は、本発明をどのように実現及び使用するかについて完全な開示及び説明を当業者に提供するために記載するものであり、本発明者らがその発明として考える範囲を制限する意図はなく、また、以下に示す実験が、実施された全て又は唯一の実験であることを表現又は意味することも意図されない。当業者であれば、具体的な実施形態に示すように、広範に記載する本発明の精神又は範囲を逸脱することなく、本発明に様々な変更及び/又は修正を加えられ得ることは理解されよう。従って、本実施形態は、あらゆる点で、例示的であり、限定的ではないものと考慮すべきである。
ラットインターロイキン−10(pDNA−IL−10F129S)をコードするプラスミド構築物は、既に、ミリガン(Milligan)ら著、疼痛(Pain)、第126巻、第1〜3号、p.294〜308(2006年)に詳しく記載されている。手短には、プラスミドは、点突然変異(F129S)及びウイルスSV40ポリアデニル化シグナルを含むラットIL−10遺伝子の発現を駆動するサイトメガロウイルスエンハンサー/ニワトリβ−アクチンプロモータ(CMV enh/CB pro)から構成される転写カセットを含む5.9キロベース(Kb)の環状プラスミドDNAからなる。転写カセットは、149bp逆方向末端反復配列によってフランキングされている。IL−10を欠いた同一のプラスミドをpDNA対照として用いた。いずれのプラスミドも、SURE
2スーパーコンピテント(Supercompetent)大腸菌(E.coli)細胞(アジレント・テクノロジー(Agilent Technologies)、米国(USA))において増幅し、製造者の指示に従い、無内毒素ギガ(Giga)プラスミド精製キット(キアゲン(Qiagen)、バレンシア(Valencia)、カリフォルニア州(CA)、米国(USA))を用いて単離した。精製済の無内毒素プラスミドを、3%スクロースを含む滅菌ダルベッコPBS(Dulbecco’s PBS)(DBPS、1、0.1ミクロン孔径でろ過済、pH7.2、カタログ#14190〜144、ギブコ(Gibco)、インビトロジェン社(Invitrogen Corp)、グランドアイランド(Grand Island)、ニューヨーク州(NY)、米国(USA))(DPBS−3%)中に再懸濁させた。DPBS−3%のビヒクルは、DPBS中の分子生物学グレードD(+)−スクロース(b−D−フルクトフラノシル−a−D−グルコ
ピラノシド;シグマ−アルドリッチ(Sigma−Aldrich)、セントルイス(St.Louis)、ミズーリ州(MO)、米国(USA))を用いて調製し、0.2um滅菌ろ過(パイロジェンフリーシリンジフィルタユニット、カタログ#25AS020AS、ライフ・サイエンス・プロダクツ社(Life Science Products,Inc.,CO)、米国(USA))した後、使用時まで、4℃で15ml円錐チューブ中に滅菌状態で保存した。
微小粒子は、改変型二相エマルジョン/溶媒蒸発プロトコル(A.M.ティンスレイ−ブラウン(A.M.Tinsley−Brown)ら著、制御放出学会誌(J.of Controlled Release)、第66巻、第2〜3号、p.229〜41(2006年))を用いて調製した。手短には、50:50PLGAコポリマー(MW75,000、ラクテル吸収性ポリマー(Lactel Absorbable Polymers))を酢酸エチル(シグマ(Sigma))中に溶解させた。ビヒクル単独(リン酸緩衝食塩水(PBS)+3%(w/v)スクロース(シグマ(Sigma)))又はビヒクル中のpDNAをPLGA溶液中に乳濁させた後、5%(w/v)ポリビニルアルコール、28%塩化カルシウム、3%スクロース(シグマ(Sigma))及び7%(v/v)酢酸エチル溶液中に再度乳濁させた。洗浄液中での4時間の硬化後、得られた微小粒子を回収し、凍結乾燥した後、4℃で保存した。走査型電子顕微鏡検査(SEM)を用いて、微小粒子の形態を調べた。NIHイメージJ(NIH ImageJ)ソフトウェアにより、10の異なる画像中に存在する粒径>1000の微小粒子が測定され、ビン化粒径を用いて、正規化度数分布を作成した。ナイコンプ380ZLSゼータ電位計(Nicomp 380 ZLS Zeta Potential Analyzer)により微小粒子のゼータ電位を測定した後、阻害の対照として階段希釈物を用い、LALアッセイにより、得られた微小粒子の内毒素レベルを試験した。使用した微小粒子は、SEMの下で球状の滑らかな形態を示し、ゼータ電位は、−28.04±2.12mVであった。微小粒子は、全体中央粒径が4.67±0.26μmの不均質な粒度分布を示したが、これは、類似する微粒子製造方法と一致しており、粒子のpDNAカプセル化効率は、55.1%であった。
pDNA放出分析により、pDNAの30%が3日後に放出され、75日を超える期間にわたって、安定した放出が達成されることが証明された。この2段階放出プロフィールは、エマルジョンベースのPLGA微小粒子からの高分子放出の共通する特徴であり、初期pDNA放出の増強段階は、微小粒子の表面上又はその付近のpDNA含量の増加によるものであり、その後、微小粒子内部からのpDNAの持続的放出及び拡散が続く(イエオ(Yeo)著、内毒素研究のアーカイブ(Archive of Endotoxin
Res.)、第27巻、第1号、p.1〜12(2004年))。カプセル化pDNAを含む、又は含まない微小粒子からの内毒素レベルは、10mg/mlの微小粒子濃度(1mgの微小粒子/ウェル)以下で、LALアッセイの場合検出限界に満たなかった。
D−マンノースなどのアジュバントを使用する実施形態では、D−マンノース(カタログ#M6020)をシグマ−アルドリッチ(Sigma−Aldrich)(セントルイス(St.Louis)、ミズーリ州(MO))からキャリアフリーで購入することができる。D−マンノースを滅菌食塩水中の0.1%BSAと合わせて、IL−10発現構築物と同時に、又はIL−10発現構築物の投与の1〜10日前のいずれかに関節内注射により投与する。D−マンノースは、典型的に、イヌにおいて関節当たり2.5μg〜500μgの用量で送達する。
「ネイキッド」pDNAとしてのIL−10発現構築物(pDNA−IL−10F129S)は、一群のイヌの罹患関節に急性関節内注射によって投与した。静脈内鎮静法及び麻酔モニタリングの下で、イヌ患者の罹患関節を、外科的に剃毛することにより、罹患関節を覆う全ての毛を除去し、クロルヘキシジン(Chlorahexidine)2%などの外科用スクラブを使用して殺菌した。患者の心拍数、血圧、酸素飽和度、換気及び心リズムを連続的にモニターした。22又は20ゲージの皮下注射針を滑膜腔に挿入した。針の適正な配置を確実にするために、pDNAの投与前に滑液を吸引した。関節内での針の配置は維持しながら、滑液吸引物を含有する注射器を取り換えた。滑液を吸引したら、同じ関節内針を使用して、関節中に治療用抗炎症組成物を投与した。最大1mgのプラスミドDNA当量を注射したが、700μgという少量で有効であることが判明した。投与に際し、関節注射当たりの量が、1mlを超えた場合には、釣り合わせるために関節滑液を吸引した。関節腔内に治療用抗炎症組成物を首尾よく導入した後、患者を鎮静作用から逆転させ、臨床的にモニターした。処置中の血圧、心拍数、酸素投与、換気又は心リズムに対するあらゆる変化を適正な医療措置で補正した。鎮静法の予想される作用には、徐脈、低血圧、低換気などがある。酸素療法は、酸素飽和レベルを最大限にするために、処置全体を通じて継続した。以下に示す個々の患者に合わせた鎮静薬並びに単剤又は併用:
A:デクスメデトミジン0.5mg/m2、逆転(アチパマゾール0.05〜0.2mg/lb)
B:オピオイド−ブトルファノール(0.05〜0.1mg/lb)、
C:プロポフォール−i.v.で実施
D:ベンゾジアゼピン(ジアゼパム):0.1〜0.2mg/lb
対象は、当日(12時間未満)院内施設に残り、その後帰宅が許可された。
Claims (32)
- 対象における炎症性関節疾患を治療する方法であって、
炎症を起こした前記関節に、治療に有効な量の細菌又はウイルスIL−10発現構築物を含む治療用抗炎症組成物を注射するステップを含み、
前記IL−10発現構築物が、細菌骨格又はウイルス骨格と、インターロイキン−10をコード化する核酸配列とを含む方法。 - 前記IL−10発現構築物が、アジュバントと一緒に投与される、請求項1に記載の方法。
- 前記アジュバントが、D−マンノース、スクロース、グルコース、リン酸カルシウム、デンドリマー、オリゴヌクレオチド、高分子量ヒアルロン酸、又はリポソームから選択される、請求項2に記載の方法。
- インターロイキン−10をコード化する前記核酸配列が、アミノ酸129位の野生型フェニルアラニンのアミノ酸置換を有する、請求項1に記載の方法。
- 前記アミノ酸置換が、セリン、アラニン、トレオニン又はシステインの群から選択される、請求項4に記載の方法。
- インターロイキン10をコード化する前記核酸配列が、IL−10F129Sをコード化する、請求項5に記載の方法。
- プラスミドDNAが、前記IL−10コード配列の5’側に少なくとも1つの核ターゲティング配列を含む、請求項1に記載の方法。
- プラスミドDNAが、前記IL−10コード配列の3’側に少なくとも1つの核ターゲティング配列を含む、請求項1に記載の方法。
- 希釈剤をさらに含む、請求項1に記載の方法。
