本発明は一般に、粘液又は喀痰の粘性を低下させ、且つ粘液又は喀痰の液状化を誘導する、向上させる及び/又は高めるための、還元状態でのチオレドキシン単一システイン活性部位を含有するタンパク質又はペプチドの使用に関する。

実施例1
この実施例は、野生型Trx(本明細書では「rhTrx」とも呼ばれる)及び単一システイン活性部位のrhTrx(本明細書では「r(Cys)hTrx」とも呼ばれる)のタンパク質の発現及び精製を実証する。rhTrx及びr(Cys)hTrxのタンパク質発現レベル及び翻訳後発現の忠実性は、その全体が参照によって本明細書に組み入れられるHarrisら,Biotechnol.Biogen.109:1987−97(2012)によって記載されたようなヒトTrx−1のDNA配列のコドン最適化によって最大化された。大腸菌での産生にための合成構築物の好適なベクターへのクローニングを行い、野生型及び単一システイン活性部位r(Cys)hTrx変異体のタンパク質がμg規模で産生されるのを可能にした。幾つかの発現タグ及び精製戦略を、内毒素及び内在性の宿主チオレドキシンからの精製を円滑にする親和性切断とともに実験室規模で評価した。
開始因子メチオニンを含む105アミノ酸の成熟rhTrxタンパク質をコードするrhTrx遺伝子を大腸菌での発現のために最適化し、合成した(DNA2.0、Inc)。誘導性T7プロモータ、カナマイシン耐性マーカー、Ni親和性精製のための6−ヒスチジンタグ及びタバコetchウイルスプロテアーゼ切断部位(TEV)を含有するpEVベクターに遺伝子をサブクローニングした。rhTrxを発現させるために、rhTrxpEVプラスミド(配列決定で検証した)でC43大腸菌(Lucigen)を形質転換し、約0.6(600nm)の光学密度に増殖させ、イソプロピルβ−D−1−チオガラクトピラノシド(IPTG)で誘導した。ホモジナイズ及び界面活性剤溶解とその後の遠心分離を用いてタンパク質を大腸菌から抽出した。Ni親和性クロマトグラフィを用いて大腸菌溶解物からrhTrxを精製した。Ni親和性精製したrhTrxをプロテアーゼ切断(TEV)してニッケル親和性タグを取り除き、イオン交換クロマトグラフィ及び逆相高速液体クロマトグラフィ(HPLC)を用いてさらに精製した。タンパク質の純度はSDS−PAGE及びイオンスプレー質量分光分析法(LC/MS/MS)によって評価し、タンパク質の配列はrhTrxのトリプシン切断とその後のイオントラップ質量分光分析法によって検証した。タンパク質の濃度及び内毒素の濃度はそれぞれ、ビシンコニン酸アッセイ(Pierce)及びLimulus Amoebacyteアッセイ(Charles River Lab.)によって決定した。r(Cys)hTrxタンパク質は類似の方法に従って調製したが、r(Cys)hTrx遺伝子はDNA2.0によって最適化し、合成した。その後透析と脱塩カラム処理によって取り除かれたDTTを用いてこれらrhTrx及びr(Cys)hTrxタンパク質を試験管内で還元した。
親和性結合法ではなく成熟タンパク質の直接発現を利用したネイティブのrhTrx及び単一システイン活性部位r(Cys)hTrxタンパク質のための好まれる発現、精製及び還元(活性化)の戦略をこの実施例で記載する。一般的な構成性又は誘導性のプロモータ系の範囲で細菌発酵の常法を用いて、大腸菌BL21及びK12に由来する宿主の振盪フラスコ回分培養及び流加発酵にてr(Cys)hTrx及びネイティブのTrx対照タンパク質を産生させたが、糖誘導性又はIPTG誘導性の系が、最高の発現株を生じるので好まれた。微量流体流動装置による細胞の粉砕に続いて、可溶性のTrxを含有する上清を遠心分離によって回収し、硫酸アンモニウム(AS)で処理して宿主細胞のタンパク質を優先的に沈殿させた。15mMのHEPES、10mMのβ−メルカプトエタノール(β−ME)、250mMのNaClへの緩衝液交換のために限外濾過/透析濾過(UF/DF)を用いて、得られたTrxを濃縮した上清を濾過した。この緩衝液では、TrxはQ-SepharoseHPアニオン交換クロマトグラフィ(AEC)樹脂には結合しないのに対してDNA及び他の不純物は高親和性で結合する。高塩AECに続いて、Trx濃縮の通過分画を、還元条件下(10mMのDTT)のHi−TrapQ−HPカラム(GE Healthcare)上での第2のAEC工程ために低塩緩衝液に交換し、その後、製剤化緩衝液(以下で記載する)に緩衝液交換した。或いは、AS沈殿の後、可溶性分画を30kDのカットオフで濾過し、疎水性相互作用クロマトグラフィ(HIC)カラムに負荷した。溶出した物質を濃縮し、1KDのUF/DFカセットを用いて緩衝液を交換し、SourceQ(アニオン交換)カラムに負荷した。塩勾配を用いてイオン交換カラムからチオレドキシンタンパク質を溶出し、次いで保存緩衝液に交換する追加のUF/DF工程を行った。
この実施例は、r(Cys)hTrx及びネイティブのrhTrxを発現させるための構築及び大腸菌株を明らかにする。r(Cys)hTrx及びネイティブのrhTrx対照は、種々のベクター系及び大腸菌の実験室BL21株又はK12株で可溶性の成熟タンパク質として容易に発現可能である。大腸菌の高い収率(>1g/L)のTrx発現株を作出するために、コドンを最適化したr(Cys)hTrx及びネイティブのrhTrxの配列を合成し、栄養欠乏に基づく誘導系、糖誘導性の系(たとえば、米国特許第7,871,815号のような当該技術で既知のラムノース、キシロース、メリビオース等で誘導性のプロモータ)、又はバクテリオファージT3、T5若しくはT7に由来するプロモータに基づくIPTG誘導性の系を含む様々な細胞内プロモータ系を用いて様々な発現ベクターにクローニングした。DNA配列の確認に続いて、誘導に使用される分子の利用を欠如するように操作された株を含む、又はメチオニンアミノペプチダーゼの過剰発現を誘導して高力価での発現の忠実性を改善し得る様々な一般的に使用されるB21/K12大腸菌宿主株を発現構築物で形質転換した(Liu, M. et al., Protein Expr Purif 84(1):130-139, 2012)。最少限の培地にて30〜37℃で振盪しながら、得られた作出宿主/ベクターの組み合わせを高処理能力形式の深穴マイクロタイタープレートにおける培地にて増殖させ、たとえば、ゲル電気泳動又はキャピラリー電気泳動のような当該技術で一般的に使用される方法を用いて発現についてスクリーニングした。SDS−PAGE及びウエスタンブロット及び/又はELISA/ELISPOTを介して陽性のクローンを確認した。株の流加発酵を行い、タンパク質(r(CYs)hTrx)の収量をSDS−PAGE及びウエスタンブロット及び/又はELISA/ELISPOTによって評価した。タンパク質を精製し、上記実施例1及び2で記載された方法を用いて還元し、活性化されたタンパク質の還元状態をDTNBアッセイによって評価し、特定のジスルフィド結合の還元活性をインスリン還元アッセイによって評価した。翻訳忠実性及び適正な分子量を確認するために配列同一性を質量分光分析法(MALDI又はエレクトロスプレー)によって解析した。
この実施例は、野生型及び単一システイン活性部位のrhTrxの試験管内での活性及び機能を明らかにする。rhTrxの活性は、標準の比色DTNBアッセイにてその還元活性を測定することによって、及び好中球のエラスターゼ活性を阻害するその能力を判定することによって評価した。加えて、rhTrxと比べたr(Cys)hTrxのヒトCFの粘液を液状化する能力を、喀痰圧縮アッセイを用いて、及び任意で特殊化した流量計を用いてCF喀痰の粘弾性の低下を調べることによって判定した。rhTrx及びr(Cys)hTrxを看護CF粘液溶解Pulmozyme(rhDNaseI、ドルナーゼα)の標準と比較した。
予備還元したTrx製剤の一般的な還元活性を以前報告された(Rancourt, R. et al., Free Radic Biol Med, 42(9):1441-1453, 2007)ようなDTNB還元アッセイを用いて定量した。rhTrx及びr(Cys)hTrxの酵素的還元については、5又は50mMのrhTrx及び0.5mMの精製TrxRを含有する50μlのアッセイ緩衝液(100mMのリン酸カリウム、pH7.0、10mMのEDTA及び0.05mg/mlのウシ血清アルブミン)を96穴プレートに加えた。次に720mMのNADPHを含有する175μlの試料緩衝液を加え、その後、試料緩衝液中6mMのDTNBを25μl加えた。化学的に予備還元したrhTrxの還元活性を測定するために、2.5mMのrhTrx50μlを96穴プレートに加え、その後、175μlの試料緩衝液及び25μlの6mMのDTNBを加えた。DTNBの添加によって反応が開始された後、DTNBの還元による412nmでの速度論的吸光度における変化を30℃にて分光光度計でモニターした。同じプロトコールに従って、r(Cys)hTrxをこのアッセイと同様に以下で記載するものに供した。r(Cys)hTrxの総還元活性はrhTrxよりも低く、r(Cys)hTrxに存在する1つ少ない還元されたCysを反映する。しかしながら、双方のタンパク質は潜在的な還元状態の90〜95%超えて還元され、完全な活性化を反映した。
rhTrxのNE活性に対する効果を判定するためのインキュベート方式は以前記載されている(Lee, R., et al. (2005) Am J. Physiol. Lung Cell. Mol. Physiol. 289(5):L875-882)。手短には、Trxをリン酸緩衝化生理食塩水(PBS、pH7.2)で所望の濃度に希釈した。混合物を精製したヒトNE(PBS、pH7.2、0.01%TritonX−100のml当たり100μg)に加え、37℃にて1時間インキュベートした。インキュベートの間の最終容量は210μlであり、NEの濃度は1.6μg/mlだった。インキュベートの後、60μlのアリコートをエラスチン分解活性について3つ組で調べた。エラスターゼ活性は、実験試料にPBS(pH7.2)中の0.8mMのN−メトキシスクシニル−Ala−Ala−Pro−Val4−ニトロアニリドを120μl加えることによって測定し、速度論的吸光度を405nmにて37℃で4分間モニターした。これらの実験で使用したエラスターゼの濃度はアッセイの線形範囲内にあると判定された。パーセントNE活性阻害は、パーセント阻害=(1−実験群の吸光度の変化/PBS対照群の吸光度の変化)として判定した。r(Cys)hTrxのエラスターゼ活性はrhTrxに少なくとも匹敵することが見いだされた。
操作すべてについて、CF喀痰は微生物フードのもとで取り扱った。関連する濃度でのrhTrx、r(Cys)hTrx又は希釈液のみ(25μl)を1.5mlのエッペンドルフ円錐管におけるCF喀痰(275μl)に加え、非常に短時間のボルテックスで混合した。37℃で30分間インキュベートした後、試料を再び手短に混合し、ヘマトクリット管に負荷して両端を封止した。ヘマトクリット遠心機における5分間の遠心分離の後、直接的な線形測定によってパーセント固体(ゲル)とパーセント液体を判定し、以下:100×液体/(液体+固体)のようにパーセント液体として表した。各条件については、少なくとも5人の異なるCF患者に由来する喀痰を3つ組で測定した。r(Cys)hTrxの液状化能は、rhTrxよりもはるかに大きくなければrhTrxに少なくとも匹敵し、作用のr(Cys)hTrxのメカニズムの予想外の潜在能を反映した。
これらの測定は、円錐平板流量計を用いて行うと共にチオレドキシンによる処理に続いて喀痰の流れを直接観察することを介して質的に行う。上述と同じ喀痰の取り扱いプロトコールを用いてインキュベートを行う。37℃での1ラド/秒の角速度を持つ振動モードでAR−1000流量計(TA Instruments, New Castle, DE)を用いて粘弾性を評価する。Trxの各調製物については、喀痰の粘弾性を低下させる能力は少なくとも5人のCF患者から得た喀痰にて3つ組で評価する。r(Cys)hTrxの喀痰の粘弾性を低下させる能力は、rhTrxよりもはるかに大きくなければrhTrxに少なくとも匹敵し、作用のr(Cys)hTrxのメカニズムの予想外の潜在能を反映することが予想される。逆転させたエッペンドルフ管にてrhTrx又はr(Cys)hTrxと混合した喀痰の流れの速度を観察することによる喀痰の粘弾性の測定を行ったが、このアッセイでは、r(Cys)hTrxの流れはrhTrx、DTT及びPulmozymeよりも広範囲であり、本質的に喀痰の流れを示さない陰性(ビヒクル)対照よりもはるかに大きいことが見いだされた。
これらの試験では、上記のようにインキュベートすることによって関連する濃度のrhTrx、r(Cys)hTrx及びrhDNaseを比較した。30分後、喀痰の液状化(圧縮アッセイ)における変化及び粘弾性の変化について試料を評価した。加えて、相乗効果試験を行った。これらにおいて、喀痰試料をrhTrxに30分間、次いでrhDNaseに30分間暴露し、或いはrhDNaseに30分間、その後rhTrxに30分間に暴露した。これらの結果を各剤単独の効果と比較した。各条件について、少なくとも5人の異なるCF患者に由来する喀痰を3つ組で測定した。野生型及び変異体型のrhTrx変異体についての12試料(rhTrx×3、rhDNase×3、rhTrx+rhDNase×3、rhDNas+rhTrx×3)のそれぞれを調べるために、所与の日に個々のドナーに由来する十分な喀痰が必要だった。実際のところ、これは、単一ドナー/日に由来するよく混合した最少4.5〜5.0mlの喀痰である。r(Cys)hTrxの液状化能は、はるかに大きくなければ、rhDNase、rhDNase、rhTrx又はDTT陽性対照に少なくとも匹敵した。
医療提供者によって決定されたような成人及び小児のCF患者から喀痰を得た。患者は臨床症状を示し、2回の別々のピロカルピンイオン導入発汗試験にて60ミリモルを超える汗中塩化物値を有し、その後の遺伝的解析で2つの対立遺伝子CF生成突然変異を示すのであれば、CFであると診断された。試料はすべて自然発生的な吐出物又は高張食塩水誘導によって提供された。視覚的に検出可能な唾液を含有する喀痰試料は捨てた。喀痰は実験室に送達されるまで氷上に保持され、次いで乾燥を防ぐようにO−リング封止バイアルでの使用まで−80℃で保持した。
この実施例は、野生型チオレドキシン活性部位を含有するタンパク質又はペプチドと比較したときのチオレドキシン単一システイン活性部位を含有するタンパク質又はペプチドの酵素活性(図1b)及び非酵素活性(図1a)を明らかにする。図1aで説明するように、非特異的な5,5’−ジチオビス−(2−ニトロ安息香酸(DTNB又はEllmanの試薬)の還元は野生型に比べてチオレドキシン単一システイン活性部位における還元可能なシステイン1つの喪失を反映する。図1b及び図2で説明するように、チオレドキシン単一システイン活性部位を含有するタンパク質又はペプチドは驚くべきことに、ヒト喀痰圧縮アッセイにて野生型チオレドキシン活性部位を含有するタンパク質又はペプチドと比較して高い潜在能を示したと共に、DNase又はNACに対比して等モル基準で高い潜在能を示した(図2)。この結果は予想外であり、Cys35の突然変異による活性部位での実質的な修飾が得られ、総還元力は以前のDTNBアッセイにおける還元可能なCys残基1つの喪失のお蔭でネイティブのチオレドキシンの4/5だったという所見が得られた。チオレドキシン単一システイン活性部位の潜在能の驚くべき上昇は、ムチンタンパク質のCys残基への共有結合のために実現されたが、それは、これらのCysが新しいジスルフィド結合を再形成するのを妨げる予想外の結果を有した。
この実施例は、ネイティブのrhTrxに比べて、初代培養のヒト気管支上皮細胞(HBE)による炎症誘発性サイトカインの放出を刺激するr(Cys)hTrx(還元された、活性状態での)低下した傾向を明らかにする。ドナーの組織及び細胞は、ヒト対象の権利を保護する認可されたプロトコールの庇護にもとで提供される。正常な(すなわち、病んでいない)肺からのHBE細胞を以前記載された(Fulcher, M.L., Methods Mol Med 107:183-206, 2005)ように酵素消化によって採取する。明確に定義された気道細胞培地(Fulcher, M.L., 2005、同書)にて2.5×105個/cm2の密度でほぐしたHBE細胞を直径12mmのTranswellClear支持体(Corning)に播く。使用する前に完全に分化するまで(約4〜6週)、培養物を気液界面で維持する。
この試験では、ナノリットル容量の試験剤(又は対照)をHBE培養物の表面に送達することが可能である用具が利用される。この系は、霧状にした薬物の超微細な噴霧を気道上皮細胞の表面に生体内で送達することを模倣する試験管内モデル系であるように設計され、たとえば、r(Cys)hTrxのような治療剤を加えることの効果を調べる理想的な方法を表す。気道の最初の20生成にわたるPariLCStarネブライザの平均堆積速度を明らかにする多重経路粒子線量測定モデル化(Anjilvel, S., et al. Fundam Appl Toxicol 28(2):41-50, 1995)の結果に基づいて、粒子の大半が送達されると予想される気道の最初の6生成にわたる平均堆積は15分間の経過にわたって約50nl/分/cm2であると概算される。これらの試験では、合計5の異なる実験群に対する750nl(15分間にわたって)の総容量での噴霧化が行われる。これらには、(1)ビヒクル対照(等張生理食塩水)、(2)ネイティブの組換えTrx(500μM)及び(3)3用量のr(Cys)hTrx(10μM、250μM及び1000μM)が含まれる。これらの濃度は「最終」気道表面濃度を表す。各試験試薬(又はビヒクル対照)の噴霧化に続いて、培養物を組織培養インキュベータに戻し、サイトカインの分析の前に24時間インキュベートする。
この試験では、IL−6、IL−8、TNF−α及びIL−1βを含む、HBE細胞によって放出される4つの主要なサイトカインの刺激に対するネイティブTrx及びr(Cys)hTrxの効果を評価する。市販の酵素結合免疫吸着アッセイ(ELISA)キット(R&D systems)を用いて無細胞の非濃縮培養上清におけるサイトカインを測定する。5群すべての噴霧化の24時間後に基底外側ALI培地の試料(Fulcher, M.L., 2005,同書)を得て、分析前に凍結する。バックグランド対照として、きれいな(未使用の)ALI培地を値すべてから差し引く。各サイトカインの分析については、濃度の30年にわたる陽性対照を用いて適当な検量線を生成する。さらに、各「未知」の試料は2つ組で分析する。測定されるサイトカインは:TNF−α(0.3pg/mlの検出下限)、IL−6(0.35pg/mlの検出下限)、IL−8(2.4pg/mlの検出下限)、及びIL−1β(1.5pg/mlの検出下限)である。これらのアッセイの互いとの及び他の組換えヒトサイトカイン(IL−la、IL−2、IL−3、IL−4、IL−7、腫瘍壊死因子−S、顆粒球コロニー刺激因子、及び形質転換増殖因子−III)との交差反応性はすべて検出限界を下回ることが以前示されている。
合計9の個々のHBE培養物で5つの条件のそれぞれを評価する。これは3人の異なる患者(n=3)に由来する3つの培養物(n=3)を表す。そのようなアプローチは十分に統計的な検出力を持って各条件の試料間及び試料内の変動の理解を提供する。データ解析については、個々の試料/群間の比較は、両側スチューデントのt検定を用いて行う。複数の群間の比較については、一元配置分散分析(ANOVA)を使用する。解析すべてについて有意性はp<0.05に設定する。
ヒトCFの初代気道上皮細胞培養:粘膜繊毛の分化を伴って気液界面でヒト初代気道上皮細胞を増殖させ、培養する最近の技法をSchlegelとその共同研究者ら(Liu, M. et al., Protein Expr Purif. 84(1):130-139, 2012; Suprynowicz, F.A., et al. Proc. Natl. Acad. Sci. U.S.A.. 109(49):20035-20040, 2012)によって報告された方法に基づいて採用する。これらの方法は遺伝子操作することなく、ALIで最終分化が依然として可能である初代気道上皮細胞の実際に無制限の供給を可能にする。F508delについてホモ接合体である3人の独特のCFドナーのそれぞれから30日間にわたってALIでの分化まで30ウェルを培養する。3つの濃度(10μM、250μM及び1000μM)のrhTrx(野生型)及びr(Cys)hTrxに先端表面で培養物を暴露し、その後、暴露負荷の4時間後及び24時間後に先端と基底側部の培地試料を回収する(表1を参照)。使用した培地は無血清であり、遠心分離して残渣を取り除き、使用まで−80℃で保存する。先端と基底側部の培地の双方から採取した試料すべてについてヒトの炎症性サイトカインのELISAアッセイを行う。先端の回収物は、先端表面に200μlのPBSを置き、15分間インキュベートした後、回収することによって生じる。IL−8及びIL−6についてのELISAを各試料で2つ組にて行う(Becker, M.N. et al., Am J Resp Crit Care Med 169(5):645-653, 2004)。30のALI培養物を含む上記の計画を3人のCFドナーのそれぞれについて繰り返す。ネイティブrhTrxとr(Cys)hTrxの結果の比較は、CF患者の分化した気道上皮細胞からの炎症誘発性サイトカインの放出を誘導するチオレドキシンの傾向は、調べた濃度ではネイティブに対比して単一システインのTrxで減衰することを明らかにする。
この実施例は、試験管内のHBE細胞培養から回収された均一な粘液に対するネイティブrhTrxに対比したr(Cys)hTrxの効果の生物物理学的及び生化学的な性状分析を明らかにする。
ヒトの気管支上皮細胞を記載されたように増殖させ、維持する(Matsui, H., et al., J Clin Invest 102(6):1125-1131, 1998)。手短には、培養物から回収された粘液をプールし、4℃で保存する。試料を透析チューブ(MWCO=3,500)に負荷し、4℃にて1〜5日間、ポリマー吸収剤(Spectra/Gel)によって濃縮する。次いで、濃縮された粘液を500μMのMgCl2と800μMのCaCl2を含有するPBSに対して4℃で透析し、適正な塩均衡を確立する(Matsui, H. et al., Proc Natl Acad Sci USA 103(48):18131-18136, 2006)。
円錐平板構造及び並行構造の双方にてBohlin Gemini流量計を用いて濃度及び時間経過のアッセイを行い、試験管内のHBE粘液特性に対する還元されたr(Cys)hTrx及びネイティブのチオレドキシンの粗大な肉眼的な生物物理学的効果を評価する。既知の応力(0.05〜約100Paの間)を処理した又は対照の粘液に10秒間適用し、流体のレオロジー回復をさらに50秒間記録するクリープ回復実験を行う。連続的な実行では、適用される応力は、流体の降伏応力に達する(すなわち、流体の粘性が突然且つ劇的に低下する応力)まで対数的に上昇させる。測定されるパラメータから、流体の粘性及び弾性を適用される応力の関数として決定する。一定応力と一定ひずみの双方で周波数掃引を行い、それを用いて流体の粘性及び剪断菲薄化挙動と同様に粘液のベースライン物性(それぞれG’及びG”)を決定する。実験はすべて23℃で行う。
処理した及び対照の粘液におけるムチンの濃度及び分子質量を示差屈折率測定によって評価し、ムチンの構造に対するネイティブのチオレドキシン及びr(Cys)hTrxの効果を判定する。試料(500μl)をSepharoseS1000カラム(Amersham Pharmacia)に負荷し、0.5ml/分の流速にて200mMの塩化ナトリウム/10mMのEDTAで溶出する。Wyatt/Optilab DSP干渉式屈折計に結合させたインラインDawnEOSレーザー光度計を用いて、それぞれ光散乱及び試料の濃度を測定する(Wyatt Technology Corporation)。カラムの空隙容量で溶出される物質に関連する屈折率ピークを統合し、以前650nmで測定し、5%の範囲内で再現可能であることが見いだされている0.165ml/gの屈折率増分(dn/dc)についての値を採用することによってムチンの濃度を算出する。所与のムチン試料の総タンパク質含量(ムチンと低分子タンパク質)を類似の方法だが、G−25カラム(dn/dc=0.170)によって測定する。試料の非ムチン(又は低分子タンパク質)の含量は、ムチンとタンパク質含量の総質量(G−25カラムを介して試料を溶出することによって測定される)からムチン含量の質量(S−1000カラムを介して試料を溶出することによって測定される)を差し引くことによって決定される。ゲルに基づいた方法、たとえば、還元型又は非還元型のSDS−PAGEとの比較では、示差屈折率測定は、質的比較ではなく正確な分子量及び分子量分布が利用しやすいという点で有利である。これらの技法の定量的な利点は、サイズの基準が存在せず、分子量が1MDaを超えることが多いムチンのような大きな糖タンパク質の研究には特に重要である(Gillis, R.B., et al., Carbohydr Polym 93(1):178-183, 2013)。
ムチンの微細構造に対するチオレドキシン処理の潜在的な効果をインタクトなHBE粘液におけるタンパク質ブロッティングアッセイを行うことによって評価する。試料の50〜200μlアリコートをニトロセルロース膜に負荷し、真空を5分間適用して負荷した試料全体を膜の中に入れた。蒸留水で試料を2回洗浄した。炭水化物含量の過ヨウ素酸シッフ(PAS)アッセイについては、負荷し、洗浄した試料を0.25%過ヨウ素酸+3%酢酸の水溶液にて30分間インキュベートする。蒸留水での2回の洗浄に続いて、試料をNaMBS溶液(0.1%メタ重硫酸ナトリウム、1%HCl)にて5分間2回インキュベートする。次に、試料をシッフ試薬と共に5〜15分間インキュベートし、NaMBSで2回、蒸留水で1回洗浄し、素早く真空乾燥する。負荷及び最初の蒸留水での洗浄に続いて、TBST緩衝液(1.21%トリスHCl、8.76%NaCl及び0.5%Tween、pH8)中の1%ミルクでタンパク質特異的な抗体のブロットをブロックする。TBSTによる5分間の2回の洗浄の後、試料を一次抗体(MUC5AC、MUC5BIIIにはMAN−5ACI及びMUC5BにはK5B)と共に30分間インキュベートする。さらにTBSTによる5分間の2回の洗浄の後、試料を二次抗体と共に2時間インキュベートする。Li−CorOdyssey赤外線検出器を用いて膜ブロットを展開する。さらに、抗チオレドキシン抗体を用いてムチンに結合したネイティブのチオレドキシンに対比する単一システインのチオレドキシンを検出し得る。PAGEゲル上での分離に続く標準のウエスタン免疫ブロットに比べて、この直接的なブロッティング法はムチン含量のさらに迅速で正確な決定を可能にし、単一システイン活性部位のチオレドキシン(r(Cys)hTrx)とムチンのジスルフィド結合との間での共有結合の相互作用を視覚化し、定量するのを可能にする。
この実施例は、ラット及びマウスへの気管内送達に続いて肺にて炎症誘発性及び病態生理学的な効果を誘導するネイティブのrhTrxの傾向を減衰させる単一システイン活性部位のTrx(r(Cys)hTrx)の能力を明らかにする。
ネイティブのrhTrxに比べて炎症誘発性のシグナル伝達を誘導するr(Cys)hTrxの能力を測定するために、ビヒクル対照と比較した漸増する濃度のチオレドキシンタンパク質の気管内(IT)送達を利用した比較生体内試験を行う。2つの試験成分は、(1)精製したネイティブのrhTrxで得られた以前の知見(Rancourt, R. et al., Free Radic Biol Med, 42(9):1441-1453, 2007)を再現するように設計される当初の試験と、(2)r(Cys)hTrxの炎症誘発性効果をネイティブのrhTrxと比較する主要な試験である。試験はすべて、DTT又は他の好適な還元剤で処理してTrxの活性部位のCys残基を還元している精製された内毒素を含まないタンパク質を利用する。還元に続いてサイズ排除クロマトグラフィを介してDTT又は還元剤を取り除く。TrxのCys残基の完全な還元を試験管内のアッセイ(DTNB還元)によって検証し、インスリン還元アッセイ又はHPLC標的/結合アッセイを用いてr(Cys)hTrxの触媒的なジスルフィド結合還元活性をアッセイする。
大げさな投薬条件下でのr(Cys)hTrxをの肺への効果を評価するために、0.5〜20mg/kgのr(Cys)hTrxの様々な単回用量をIT経路で投与し、投薬後2日及び14日で病理変化が生じるかどうかの評価を行う。スプラーグ・ドーリー系のオス及びメスのラットを群当たり20匹(10/性別)の3つの実験群に無作為化する。動物はすべて試験前身体検査を受ける。ビヒクル(生理食塩水製剤)を陰性対照として使用する。この試験の評価項目には、(1)肉眼的な有害変化を探すための臨床検査及び組織病理検査、並びに(2)被験物質及び被験物質に対する抗体の存在を検出するための血清の性状分析が含まれる。単一システインr(Cys)hTrxはネイティブのrhTrxに対比して匹敵する投与量で比較的軽度の効果を生じる。
イソフルランでマウスを軽く麻酔し、傾斜した齧歯類用ワークステーションに載せる。拡大ルーペを装着した齧歯類用の咽頭鏡を用いて、咽頭を直接可視化し、微量噴霧器(PennCentury)を遠位気管に通す。固定した容量(25〜50μl)の被験物質溶液を気道に投与し、マウスを回復させる。気道導入及び安楽死の時点での体重を記録する。実験動物委員会(IACUC)が認可したプロトコールに従って、IT導入の6時間後に条件当たり5匹のマウスを、24時間後に別の5匹のマウスを安楽死させる。プロテアーゼ阻害剤(Pierce)を伴った合計1.5mlの冷却した無菌の生理食塩水で気道を洗浄する。肺全体を回収し、検体はすべてさらに処理されるまで氷上に保存される。気管支肺胞洗浄液(BALF)を遠心分離して白血球及び他の細胞残渣を沈殿させる。無細胞BALFはELISA試験が行われるまで−80で凍結する。BAL細胞ペレットを1mlの無菌生理食塩水に再浮遊させ、Coulterのカウンタ及び染色されたサイトスピン調製物からの300細胞の手動カウントを用いて白血球百分率の計数を行う。試料がすべて採取されている場合、BAL液を氷上で融解し、KC(IL−8の類似体)、TNFα、IL−6及びIL−1β(ElisaTech, Denver, CO)についてELISAを行う。BALFの細胞性を%及び合計の白血球(好中球、マクロファージ、リンパ球)の双方によって特徴付ける。
Rancourt及び共同研究者ら(Rancourt, R. et al., Free Radic Biol Med, 42(9):1441-1453, 2007)による以前の研究は、気道における追加の(外来性の)粘液の存在はネイティブのTrxによるサイトカイン放出の刺激を有意に低下させることを明らかにした。このことは、高い粘液負荷を持つCF患者がTrxの細胞外の還元のために低下した炎症反応を示し得る一方で、(1)内在性粘液の高い産生及び(2)前から存在する炎症反応(CFにおけるような)伴った気道モデルではチオレドキシンの効果に関する情報は現在のところない。この試験の目標は、慢性の粘液性の閉塞/炎症のマウスモデルを用いてネイティブのTrx及びr(Cys)hTrxの効果を調べることである。これらの試験については、ENaCチャンネルのβサブユニットを過剰発現し、肺において水の過剰吸収を示すβENaCマウスモデルが使用される(Mall, M., et al., Nat Med. 10(5):487-493, 2004)。βENaCマウスは過剰な粘液を産生し、粘液による気道の塞栓及び炎症を伴ったCF/COPD様の肺の表現型を発症する。これらのマウスは多数の前臨床試験にてCF肺疾患のモデルとして以前使用されていた(たとえば、Graeber, S.Y., et al., Am J Respir Cell Mol Biol 49(3):410-417, 2013)。本試験では、(1)気道/肺の組織病理及び炎症の状態に対するr(Cys)hTrxに対比したネイティブのrhTrxの効果、及び(2)βENaCマウスにおける粘液負荷に対するこれらの剤の効果を測定して送達用量の範囲にわたるネイティブTrx及び単一システインTrx(r(Cys)hTrx)の相対的な効果を調べ、慢性の粘液過剰産生と前から存在する炎症がこれらの化合物の潜在的な肺への毒性をどのように調節するのかを理解する。
この実施例は、r(Cys)hTrxが野生型rhTrxに対比して粘液を正常化する活性を改善することを明らかにする。喀痰圧縮アッセイにて患者の喀痰試料(治療当たりn=6)の正常化に対する野生型rhTrx、r(Cys)hTrx及び2つの濃度でのジチオスレイトール(DTT)、等モル濃度でのN−アセチルシステイン(NAC)及び組換えヒトDNase(rhDNase)を含む種々の対照の効果を測定した。喀痰試験は、各実験について3人までの個々のCF患者からの4〜6の自然に吐き出された(誘導ではない)試料を用いて実施された。喀痰の「好まれる」部分(すなわち、粘液)を採取し、各試料から合わせ、合計した試料を撹拌又は穏やかなボルテックスによって均質化し、その後、試験管に分け、希釈液(すなわち、トリス緩衝液)、希釈液中のDTT(0.58mM及び1.5mM)、希釈液中のNAC(0.58mM)、希釈液中のrhDNase(0.58mM)、希釈液中のrhTrx(0.58mM)又は希釈液中のr(Cys)hTrx(0.58mM)のいずれかに暴露する。DTTは陽性対照として各実験で新しく作った。図2はr(Cys)hTrxが野生型チオレドキシン(図2では「rhTrx」と呼ばれる)に比べて患者の喀痰の液状化を顕著に高めることができたことを示すので、ネイティブのTrx及び看護用ヒトDNaseである「Pulmozyme」(商標)(「rhDNase」)の標準に対比して改善された粘液正常化活性を示す。チオレドキシン及びDNaseの双方は同様に、米国の外では一般に去痰剤として使用される等モル量のNACよりもさらに強力だった。ヒトCF患者の喀痰を用いたこれらの結果は、ムチンCysへの予想された共有結合にもかかわらず、単一システインのr(Cys)hTrxはCF喀痰の粘弾性の正常化について完全に有能なままである(及び未修飾のネイティブrhTrxよりも一層さらに強力である)ことを明らかにしている。しかしながら、肺上皮を交差する小分子12kDのr(Cys)hTrxの潜在力はムチンへの結合によって大部分最少限に抑えられるはずである。次いでその後、細胞の核に入り、たとえば、NFκBのようなレドックス調節性の標的タンパク質と相互作用する活性のあるr(Cys)hTrxの能力は、CD30TNF受容体のTrxが介在する活性化について理論化されているように、免疫細胞上の細胞外の膜貫通ドメインとの相互作用によるシグナル伝達のように、一層さらに減衰される(Schwertassek, U., et al., EMBO J 26(13):3086-3097, 2007)。r(Cys)hTrxについて観察された高い潜在能(図1a、1b及び2)は、ネイティブrhTrx(又は実際、小分子チオール剤)による処理とは異なって、結合したCysがジスルフィド結合の再形成から遮断されるので、分子によるムチンCysの共有結合に一致する。
本発明の種々の実施形態を詳細に記載してきたが、当業者がこれらの実施形態の改変及び改作を思いつくのは明らかである。しかしながら、そのような改変及び改作は、以下のクレームで言及されるような本発明の範囲内にあることが明白に理解されるべきである。

- 過剰に粘性又は粘着性の粘液又は喀痰を有する患者にて粘液又は喀痰の粘性を低下させる方法であって、接触させる工程の前に比べて粘液又は喀痰の粘性を低下させるのに有効な、還元状態でのチオレドキシン単一システイン活性部位を含有するタンパク質又はペプチドを含む組成物に患者の粘液又は喀痰を接触させることを含む方法。
- 患者が、粘液又は喀痰の異常な又は過剰な粘性又は粘着性が疾患の症状又は原因である肺疾患を有する請求項1に記載の方法。
- 患者が、粘液又は喀痰の異常な又は過剰な粘性又は粘着性が生物学的還元剤活性の欠如に関連する肺疾患を有する請求項1に記載の方法。

- 過剰に粘性又は粘着性の粘液又は喀痰を有する患者にて粘液又は喀痰の粘性を低下させる方法であって、ジスルフィド結合還元剤及びシステイン遮断剤を含む組成物に患者の粘液又は喀痰を接触させることを含む方法。
- ジスルフィド結合還元剤及びシステイン遮断剤が同一分子である請求項37に記載の方法。
- 分子がチオレドキシン単一システイン活性部位を含有するタンパク質又はペプチドである請求項38に記載の方法。
- ジスルフィド結合還元剤及びシステイン遮断剤が異なる分子である請求項37に記載の方法。
- ジスルフィド結合還元剤が、ジチオスレイトール(DTT)、エチレンジアミン四酢酸(EDTA)、グルタチオン、ジチオグリコール酸、2−メルカプトエタノール、N−アセチルシステイン及びトリス−(2−カルボキシエチル)ホスフェンから成る群から選択される請求項37に記載の方法。
- システイン遮断剤が、ヨードアセトアミド、ヨード酢酸又は他のアルキル化剤から成る群から選択される請求項37に記載の方法。

- 標的部位への共有結合を形成する薬剤を投与することによって、肺送達、経口送達及び局所送達から成る群から選択される経路による送達に続く患者における薬剤物質への全身性の暴露を防ぐ方法。

- 患者が、哺乳類及び鳥類から成る群から選択される脊椎動物である上記請求項のいずれか1項に記載の方法。
- 患者がヒトである請求項50に記載の方法。
- 患者がニワトリ又はシチメンチョウである請求項50に記載の方法。
- 過剰に粘性又は粘着性の粘液又は喀痰を有する患者にて粘液又は喀痰の粘性を低下させるための、還元状態でのチオレドキシン単一システイン活性部位を含有するタンパク質又はペプチドを含む組成物の使用であって、患者の粘液又は喀痰を組成物に接触させることが、接触させる工程の前に比べて粘液又は喀痰の粘性を低下させる使用。
- チオレドキシン単一システイン活性部位が、C−X−X−S(配列番号24)、C−X−X−X(配列番号17)、X−C−X−X−X−X(配列番号19)、X−C−G−P−X−X(配列番号21)、W−C−G−P−X−K(配列番号23)、X−C−X−X−S−X(配列番号25)、X−C−G−P−S−X(配列番号26)、及びW−C−G−P−S−K(配列番号27)から成る群から選択されるアミノ酸配列を含み、式中、C残基が還元状態にあり、X残基がシステイン以外の任意のアミノ酸残基である請求項54に記載の使用。
