Abstract
Description
技術分野は血友病および他の凝固障害の治療に関する。
血液凝固は、血液が安定な血餅を形成して出血を停止させるプロセスである。このプロセスには、血中を循環している幾つかの酵素前駆体および補酵素前駆体(または「凝固因子」)が関与している。それらの酵素前駆体および補酵素前駆体は幾つかの経路で相互作用し、該経路により、それらは活性化形態へと連続的または同時に変換される。最終的に、該プロセスは、第Va因子、イオン性カルシウムおよび血小板の存在下、活性化第X因子(FXa)によりプロトロンビンをトロンビンへと活性化する。そして活性化トロンビンは血小板凝集を誘発し、フィブリノーゲンをフィブリンに変換し、ついでこれは活性化第XIII因子(FXIIIa)により架橋されて血餅を形成する。

本開示は、血友病治療および他の治療のための、組織因子経路インヒビター(TFPI)に対する安全な長時間作用性抗体を記載する。現在、抗TFPI抗体はそれぞれ前臨床および臨床開発中であるが、抗TFPI抗体のインビボ半減期は他のIgG抗体のものより比較的短い。これは恐らく、標的媒介性クリアランスによるものであろう。また、抗TFPI抗体は、炎症を有する又はFVIIaで治療された患者においては特に、副作用を引き起こしうるという懸念も生じている。
本明細書に開示されている抗体はTFPIに特異的に結合する。すなわち、それらは、無関係な抗原(例えば、BSA、カゼイン)に対するそれらの結合アフィニティより高い(例えば、少なくとも2倍高い)アフィニティでTFPIに結合する。本明細書中で用いる「組織因子経路インヒビター」または「TFPI」なる語は、細胞により天然で発現されるヒトTFPIの任意の変異体、アイソフォームおよび種ホモログを意味する。

抗TFPIプロドラッグ抗体は合成的または組換え的に製造されうる。抗体を製造するためには多数の技術が利用可能である。例えば、抗体を製造するためにファージ−抗体技術が用いられうる(Knappikら,J.Mol.Biol.296:57−86,2000(これを参照により本明細書に組み入れることとする))。抗体を得るためのもう1つのアプローチは、WO 91/17271およびWO 92/01047(それらを共に参照により本明細書に組み入れることとする)に記載されているとおり、B細胞からのDNAライブラリーをスクリーニングすることである。これらの方法においては、メンバーが種々の抗体をそれらの外表面上に提示する、ファージのライブラリーを作製する。抗体は、通常、FvまたはFabフラグメントとして提示される。選択されたタンパク質への結合に関するアフィニティ増強により、ファージ提示抗体を選択する。抗体は、トリオーマ(trioma)法(例えば、Oestbergら,Hybridoma 2:361−367,1983;米国特許第4,634,664号;米国特許第4,634,666号(それらの全てを参照により本明細書に組み入れることとする))を用いて製造されうる。
本開示は、プロドラッグ抗体をコードするポリヌクレオチドをも提供する。これらのポリヌクレオチドは、例えば、治療用の大量の抗体を製造するために使用されうる。

抗TFPIプロドラッグは、医薬上許容される担体を含む医薬組成物中で提供されうる。医薬上許容される担体は、好ましくは、非発熱性である。抗TFPIプロドラッグ抗体を含む医薬組成物は単独で又は少なくとも1つの他の物質、例えば安定化化合物と共に投与されることが可能であり、それは、塩類液(食塩水)、緩衝食塩水、デキストロースおよび水(これらに限定されるものではない)を含むいずれかの無菌生体適合性医薬担体中で投与されうる。種々の水性担体、例えば0.4% 塩類液、0.3% グリシンなどが使用されうる。これらの溶液は無菌であり、一般に、粒子状物を含有しない。これらの溶液は通常の良く知られた滅菌技術(例えば、濾過)により滅菌されうる。該組成物は、生理的条件に近づけるために、必要に応じて、医薬上許容される補助物質、例えばpH調節および緩衝化剤などを含有しうる。医薬組成物中の抗TFPIプロドラッグ抗体の濃度は広範に、すなわち、約0.5重量%未満、通常は少なくとも約1%から、15または20重量%まで変動可能であり、主に、選択される個々の投与様式に従い、流体体積、粘度などに基づいて選択される。例えば、米国特許第5,851,525号(これを参照により本明細書に組み入れることとする)を参照されたい。所望により、2以上の異なる抗TFPIプロドラッグ抗体が医薬組成物に含まれうる。

血友病は、血管が破壊された際に出血を止めるために用いられる血液凝固または凝血を制御する身体の能力を損なう遺伝性遺伝障害の一群である。血友病A(凝固第VIII因子欠乏症)は、出生男性5,000〜10,000人に約1人の割合で存在する最もよく見られる形態の該障害である。血友病B(第IX因子欠乏症)は出生男性約20,000〜34,000人に約1人の割合で生じる。
1以上の抗TFPIプロドラッグ抗体を含む医薬組成物は単独で又は他の物質、薬物もしくは凝固因子と共に、血友病または他の凝固障害を治療するために患者に投与されうる。抗TFPIプロドラッグ抗体の「治療的有効量」は、凝固を促進する又は出血時間を低減する抗TFPIプロドラッグ抗体の量を意味する。治療的有効量の決定は当業者の技量の範囲に十分に含まれる。
以下の実施例は、本開示の種々の態様を更に例示するために記載されている。以下の技術は、具体的な実施形態の実施において十分に機能することが本発明者により見出された技術および/または組成物を表し、その実施のための好ましい様態を構成しうると、当業者に理解されるべきである。しかし、本開示の精神および範囲から逸脱することなく、これらの実施形態においては変更が施されることが可能であり、尚も同様または類似の結果が得られうる、と当業者は本開示を考慮して理解するであろう。
抗TFPI活性をマスク(遮蔽)するためには少なくとも3つの方法が想定されているが、追加的な方法も想定される。抗組織因子抗体ドメイン、抗赤血球抗体ドメインまたはアルブミン結合性ペプチドがマスキング(遮蔽)ドメインとして使用されうる。マスキング機能は、第1標的、例えば組織因子、赤血球またはアルブミンへのプロドラッグ抗体結合を含みうる。抗TFPIプロドラッグ抗体がその潜在性形態で存在する場合には、図1に示すとおり、凝固カスケードから産生されたプロテアーゼによりマスキング領域が切断されるまでは、TFPIへの結合は全く生じないか、または有意に低減されるであろう。種々の形態のプロドラッグ抗体が設計されている。それらの形態の全ては、FcRn結合を可能にして長い半減期をもたらすFc領域を含むが、可変領域は修飾されうる(縦列連結可変領域、scFv−可変領域融合、ペプチド−可変領域融合などを含む)。これらのプロドラッグ抗体の表示を図2に示す。現在想定されているプロドラッグ抗体の親抗体はヒト抗体ライブラリーから見出された。これらの抗体は、それらのアフィニティおよび機能性を改善するために、十分に最適化されている。親抗体gA200およびgB9.7はヒトTFPIに対する高いアフィニティおよび特異性を有していて、組織因子(TF)により開始される凝固を促進させる。
A.組織因子結合
組織因子(TF)は、酵素前駆体プロトロンビンからのトロンビン形成の開始に必要な、内皮細胞下組織および白血球に存在するタンパク質である。組織因子は、損傷が生じて初めて血流にさらされて、凝固を開始させる。したがって、TFの標的化は、抗TFPIプロドラッグ抗体が損傷部位上で活性化されることを可能にする。抗TFPIプロドラッグ抗体内に組込まれたTF結合のマスキングドメインは、TFの機能を阻止しないTF結合性抗体、ペプチドまたは代替的スカフォールドでありうるであろう。
RBC(赤血球)は薬物および酵素の運搬のための担体またはデポーとして使用されている。RBCは生体適合性、生分解性であり、長い循環半減期を有し、種々の生物活性物質がローディングされうる。抗体での表面修飾はそれらの標的特異性を改善し、それらの循環半減期を増加させることが示されている。本開示においては、抗TFPI抗体のN末端において融合するマスキングドメインとして抗RBC抗体を使用した。この抗TFPIプロドラッグ抗体は、未修飾親抗TFPI抗体より長い潜在的循環時間を有するプロドラッグを与える。RBC上の該プロドラッグの結合は、マスキングドメインが切断されるまでは、TFPIに結合するその能力を更に低減するであろう。
アルブミンは、治療用および診断用物質のための、主として糖尿病、癌、関節リウマチおよび感染症の診断および治療のための汎用担体として登場した。ヒト血清アルブミンは、約40mg/mLの循環濃度を有する、体内で最も豊富なタンパク質である。アルブミンは67kDaの分子量を有する。アルブミン結合性部分は、抗TFPI抗体のN末端に融合するマスキングドメインとして使用可能であり、それにより、親抗TFPI抗体の場合より潜在的に長い循環時間を有する抗TFPIプロドラッグ抗体が得られる。該プロドラッグ構築物においてはアルブミン結合性ペプチドを使用したが、アルブミン結合性部分はペプチド、天然アルブミン結合性ドメイン、スカフォールド、抗体または抗体フラグメント、例えばFab、scFv、ドメイン抗体および他の誘導体でありうる。
損傷が生じると、組織因子(TF)が血流にさらされ、第VII因子を活性化してTF/FVIIa複合体を形成する。ついでTF/FVIIa複合体は第X因子を活性化し、FXaはプロトロンビンをトロンビンへと活性化して、フィブリン形成および血液凝固を引き起こす。組織因子経路の主要役割は、「トロンビン・バースト(burst)」を引き起こすことであり、このプロセスにより、フィードバック活性化の役割に関して凝固カスケードの最も重要な構成成分であるトロンビンが瞬時に遊離される。また、凝固カスケードにおいて、一連の他の凝固因子が活性化される。
9個のgA200−プロドラッグ重鎖(HC)および6個のgA200軽鎖変異体(LC)を、発現ベクターpTTF5内へのインフュージョン(Infusion)クローニング(Clontech)を用いて構築した。全ての変異体はFXaの6アミノ酸切断部位EGRTATを含有していた。該突然変異タンパク質は、該切断部位に隣接する種々のN末端およびC末端欠失を含有していた。HC1突然変異タンパク質およびLC1突然変異タンパク質の代表的プラスミド地図を図3A〜Bに示す。15個の軽鎖および重鎖変異体のDNA配列を図12に示す。該HC突然変異タンパク質をHC1〜HC9と命名し、該LC突然変異タンパク質をLC1〜LC6と命名した。抗TFPIと融合したscFv(抗RBCまたは抗TF)の代表的プラスミドベクター地図を図4A〜4Cに示し、抗TFPIと融合したアルブミン結合性ペプチドの代表的プラスミドベクター地図を図5A〜Bに示す。図13〜14は、設けられた切断部位を有する種々の軽鎖および重鎖の組合せを含む15個の構築物の配列を示す。
チャイニーズハムスター卵巣細胞を宿主細胞で使用した場合、アマクサ(Amaxa)のニュークレオフェクション(Neucleofection)技術を用いて、トランスフェクションを行った。簡潔に説明すると、反応当たり2×106個の細胞を1000rpmで5分間ペレット化した。ペレットを反応当たり100μlのヌクレオフェクター(Nucleofector)溶液Vに再懸濁させた。2μgのpQM1−3E10sc−gA200HC(pQM1−gA200LCを伴う)、pQM1−Ter119sc−gA200HC(pQM1−gA200LCを伴う)またはpQM1−56E4−gA200HC(pQM1−56E4−gA200LCを伴う)をそれぞれ、該細胞に加えた。ついで溶液V中のDNA/細胞をヌクレオキュベット(Nucleocuvette)容器に移した。プログラムU024を使用して、ヌクレオフェクター(Nucleofector)(登録商標)においてエレクトロポレーションを行った。エレクトロポレーション後、0.5mLの加温培地を該細胞に直ちに加え、ついで4.5mL/ウェルの予め加温されたQmix1培地(抗体を含有しない)と共に6ウェルプレートに移し、シェーカー上の37℃のインキュベーターに戻した。トランスフェクションの3〜4日後、プロドラッグ抗体の発現を測定した。陽性発現細胞に関して、安定プールを作製した。該細胞を0.5×106/mLに希釈し、G418を0.7mg/mLに加えた。細胞密度が3〜4×106/mLに達したら、該細胞を0.4×106/mLに再び希釈し、0.7mg/mLのG418を含有するQmix1中で常に維持した。該選択は約2週間を要し、ついで生産段階に入った。細胞生存度が>95%に回復し、細胞密度が3.5〜4×106/mLに達したら、培養温度を30℃に切り換えた。温度切り換えの4〜7日後、馴らし培地を回収した。該細胞を5000rpmで30分間の遠心分離により除去した。該馴らし培地をミリポア(Millipore)濃縮器を使用して5倍濃縮し、ついで9000rpmで40分間の追加的遠心分離を行った。
プロドラッグタンパク質を、MabSelectプロテインAカラム(5mL HiTrap,GE HealthCare,#28−4082−55)を使用して、CHO細胞馴らし培地から精製した。培地を限外濾過により5〜10倍濃縮し、または濃縮することなく使用した。「平衡化バッファー」(50mM Tris−HCl,150mM NaCl,pH7.0)中でカラムを平衡化した後、1〜1.5mL/分の流速でカラム全体に培地をポンピングした。ローディング後、カラムを4mL/分の流速で5〜10カラム体積(CV)の平衡化バッファーで洗浄した。ついでカラムを「酢酸洗浄バッファー」(50mM 酢酸ナトリウム,150mM NaCl,pH5.4)で再平衡化した。
Maxisorb 96ウェルプレート(Nunc)をPBS中の1μg/mLのTFPIで4℃でコートした。該プレートを5% 脱脂乾燥乳PBS/0.5% Tween−20中で室温で1時間ブロッキングした。未消化および消化抗体の系列3倍希釈液を該ウェルに加え(100μL/ウェル)、室温で1時間インキュベートした。該プレートをPBS−Tで5回洗浄した。アンプレックスレッド(Amplex Red)(Invitrogen)溶液での検出のために、二次抗Fab−HRP結合抗体を加えた(100μLの1:10,000希釈液)を加えた。図7において認められうるとおり、HSA結合性プロドラッグ抗体は、その親抗TFPI抗体gA200より若干低減した、TFPIへの結合を示す。
赤血球へのプロドラッグ結合を試験するために、ELISAを用いた。透明な96ウェルMaxisorpマイクロタイタープレートのウェルを、107/mlの濃度で(CaもMgも含有しない)DPBS中に再懸濁された100μLのマウスゴーストRBCでコートした。プレートをテープで密封し、4℃で一晩インキュベートした。ウェルをDPBST(DPBS+0.05% Tween20)で1回洗浄し、ついで室温で1時間、5% 乳/DPBSTでブロッキングした。ブロッキングバッファーを廃棄し、50μLの希釈サンプルをウェルごとに加えた。サンプルをPBS中で1:3で系列希釈した。該プレートを室温で1時間インキュベートし、ついでDPBSTで素早く5回洗浄した。PBST(HRPヤギ抗hFAB,Jackson ImmunoResearch,cat#109−035−097)中で1:5000希釈された100μLの二次抗体をウェルごとに加えた。プレートを室温で1時間インキュベートし、ついでDPBSTで5回洗浄した。HRP基質(Amplex Red,Invitrogen A22177)を加え、485nmの励起波長および595nmの発光波長での蛍光測定をSpectraMax M2e(Molecular Devices)で行った。図8において認められうるとおり、該プロドラッグ抗体は、10nMを超える濃度でRBCに結合した。
方法
アミンカップリングキットを製造業者の説明に従い使用して、ヒトTFPIをCM4またはCM5チップ上に固定化した。抗TFPIプロドラッグ抗体または親抗TFPI抗体を、15μg/mL ヒト血清アルブミン(HSA)の存在下または非存在下の10μg/mL 抗体で、該系に流動させた。各注入の完了の2秒後に結合レベルを測定した。動力学アッセイにおいては、一連の濃度の抗体を注入し、30分間の解離時間を設けた。BiaEvaluationソフトウェアを使用して、該抗体の解離および会合速度をモデル化した。
図9はヒトTFPIへの種々のプロドラッグ抗体の結合を示す。図9に示されているとおり、HSAの非存在下ではABP−gA200プロドラッグ抗体は179反射単位(RU)でTFPIに結合するが、HSAの存在下ではシグナルは36RUへと80%減少した。逆に、ヒトアルブミンはTFPIへの親抗体gA200の結合に有意な影響を及ぼさなかった。
ヒトHemA血漿を使用して、TFPIプロドラッグ抗体のトロンビン産生アッセイ(TGA)を行った。血小板欠乏血漿(PPP)試薬および較正物質を1mLの蒸留水で再構成させた。100nMから出発する1.56nMの最終濃度の抗TFPIプロドラッグ抗体の1:2系列希釈液をHemAヒト血漿に加えた。血漿のみのサンプルを対照として使用した。96ウェルTGAプレートにおいて、20μLのPPP試薬または較正物質を各ウェルに加え、種々の濃度の抗TFPIプロドラッグ抗体を含有する80μLの血漿サンプルを加えた。該プレートをTGA装置内に配置した。ついで該装置は20μLのFluCa(Flu基質+CaCl2)を各ウェル内に自動的に分注した。トロンビン産生を60分間測定した。Ter119scFv−gA200をTGAにおいて試験した際、マウスRBCゴーストにHemA血漿を添加した。Ter119scFv−gA200をマウスRBC−GOLDと共に室温で15分間インキュベートした。
材料および方法
以下の試薬をFXaアッセイに使用した。
該プロドラッグのプロテアーゼ切断およびプロテアーゼ枯渇のためにNovagen Thrombin切断捕捉キット(69022−3)およびNovagen第Xa因子キット(69037−3)を使用した。
抗TFPIプロドラッグ抗体TPP−2652およびTPP−2654のLC−MS分析を無傷または還元条件下で行った。無傷タンパク質の場合には、2μgをPLRPカラムに直接ローディングした。還元サンプルの場合には、PLRPカラムへのローディングの前に、試験サンプルを10mM DTTで37℃で30分間処理する。
Claims (35)
- (a)第1軽鎖および第1重鎖可変領域を含み、組織因子経路インヒビター(TFPI)に結合する第1可変ドメイン、
(b)第1軽鎖および/または第1重鎖可変領域のアミノ末端に連結されたマスキングドメイン、ならびに
(c)第1軽鎖および/または第1重鎖可変領域とマスキングドメインとの間に挟まれたプロテアーゼ切断可能リンカー
を含む抗体。 - 該プロテアーゼ切断可能ドメインがトロンビン、プラスミン、第VIIa因子または第Xa因子切断部位である、請求項1記載の抗体。
- 該マスキングドメインが、第2軽鎖および第2重鎖可変領域を含む第2可変ドメインを含む、請求項1記載の抗体。
- 該抗体がIgG1、IgG2、IgG3、IgG4、IgM、IgA1、IgA2、分泌性IgA、IgDおよびIgE抗体である、請求項1記載の抗体。
- 該抗体がヒト抗体またはヒト化抗体である、請求項1記載の抗体。
- 該抗体が一本鎖抗体である、請求項1記載の抗体。
- 該抗体が2価であり、2つのマスキングドメインを含み、1つが各第1軽鎖可変領域のアミノ末端に連結されている、請求項1記載の抗体。
- 該抗体が2価であり、2つのマスキングドメインを含み、1つが各第1重鎖可変領域のアミノ末端に連結されている、請求項1記載の抗体。
- 該抗体が2価であり、4つのマスキングドメインを含み、1つが各第1軽鎖可変領域および各第1重鎖可変領域のアミノ末端に連結されている、請求項1記載の抗体。
- 該マスキングドメインのうちの2つが第2軽鎖可変領域であり、該マスキングドメインのうちの2つが第2重鎖可変領域であり、ここで、第2軽鎖および重鎖可変領域が第2可変ドメインを形成している、請求項9記載の抗体。
- 第2可変ドメインが組織因子(TF)、赤血球またはアルブミンに結合する、請求項3または10記載の抗体。
- 該マスキングドメインがアルブミン結合性ペプチドまたはタンパク質である、請求項1、7または8記載の抗体。
- プロモーターの制御下の請求項1〜12のいずれか1項記載の抗体のコード領域を含む発現ベクター。
- 請求項13記載の発現ベクターを含む細胞。
- 医薬上許容されるバッファー、担体または希釈剤と共に製剤化された請求項1〜12のいずれか1項記載の抗体を含む医薬製剤。
- (a)第1軽鎖および第1重鎖可変領域を含み、組織因子経路インヒビター(TFPI)に免疫学的に結合する第1可変ドメイン、
(b)第1軽鎖および/または第1重鎖可変領域のアミノ末端に連結されたマスキングドメイン、ならびに
(c)第1軽鎖および/または第1重鎖可変領域とマスキングドメインとの間に挟まれたプロテアーゼ切断可能リンカー
を含む抗体を、対象における凝固を促進させるのに有効な量で投与することを含む、対象における凝固障害の治療方法。 - 該プロテアーゼ切断可能ドメインがトロンビン、プラスミン、第VIIa因子または第Xa因子切断部位である、請求項16記載の方法。
- 該マスキングドメインが、第2軽鎖および第2重鎖可変領域を含む第2可変ドメインを含む、請求項16記載の方法。
