Abstract
Description
i)前記対象由来の遺伝物質を含んでなる試料において、rs1130214、rs456998、rs7211818およびrs1053639の一塩基多型(SNP)の配列を決定すること、および
ii)前記SNPの配列に基づいて前記対象がEPSを発症するリスクを予測すること
を含んでなる方法に関する。
i)前記対象由来の遺伝物質を含んでなる試料において、rs1130214、rs456998、rs7211818およびrs1053639のSNPの配列を決定すること、および
ii)前記SNPの配列に基づいてDRD2遮断効力が低い抗精神病薬に基づく療法を受ける対象を選択すること
を含んでなる方法に関する。
i)前記対象由来の遺伝物質を含んでなる試料において、rs1130214、rs456998、rs7211818およびrs1053639のSNPの配列を決定すること、および
ii)前記SNPの配列に基づいて適切な抗精神病薬に基づく療法を選択すること
を含んでなり、前記抗精神病薬が、DRD2遮断効力が低い抗精神病薬に基づく療法および任意の抗精神病薬に基づく療法からなる群から選択される方法に関する。
第一の面において、本発明は、対象において抗精神病薬に基づく処置により誘導されるEPSの発症を予測する方法(以下「本発明の第一の方法」という)であって、
i)前記対象由来の遺伝物質を含んでなる試料において、rs1130214、rs456998、rs7211818およびrs1053639の一塩基多型(SNP)の配列を決定すること、および
ii)前記SNPの配列に基づいて前記対象がEPSを発症するリスクを予測すること
を含んでなる方法に関する。

rs1130214、rs456998、rs7211818およびrs1053639のSNPの予測値はさらに、抗精神病薬により誘導されるEPSを発症するリスクが高い患者であって、EPSを誘導する可能性が低い療法、すなわち、DRD2遮断効力が低い抗精神病薬に基づく療法を受ける患者の選択を実現できることが当業者に理解されよう。よって、別の面において、本発明は、抗精神病薬により処置可能な疾患に罹患している対象であって、DRD2遮断効力が低い抗精神病薬に基づく療法を受ける対象を選択する方法(以下「本発明の第二の方法」という)であって、
i)前記対象由来の遺伝物質を含んでなる試料において、rs1130214、rs456998、rs7211818およびrs1053639のSNPの配列を決定すること、および
ii)前記SNPの配列に基づいてDRD2遮断効力が低い抗精神病薬に基づく療法を受ける前記対象を選択すること
を含んでなる方法に関する。
本発明はまた、処置すべき対象に存在するrs1130214、rs456998、rs7211818およびrs1053639のSNPの対立遺伝子の組み合わせに従って個別療法を選択することについても検討する。
i)前記対象由来の遺伝物質を含んでなる試料において、rs1130214、rs456998、rs7211818およびrs1053639のSNPの配列を決定すること、および
ii)前記SNPの配列に基づいて適切な抗精神病薬に基づく療法を選択すること
を含んでなり、前記抗精神病薬が、DRD2遮断効力が低い抗精神病薬に基づく療法および任意の抗精神病薬に基づく療法からなる群から選択される方法に関する。
本発明はまた本発明に従って用いるためのキットの製造も検討する。
上に定義された第二の方法は、また、抗精神病薬により処置可能な疾患に罹患している患者への個別療法の提供を可能にする。特には、EPSを発症する高いリスクを有すると考えらえる患者は、EPSを誘導する可能性がより低いことが知られている抗精神病薬に基づく療法からおそらく最も利益を受けるであろう。反対に、EPSを発症するリスクが低いことを示す患者は任意の抗精神病薬に基づく療法で処置され得る。
別の面において、本発明は、対象において抗精神病薬に基づく処置により誘導されるEPSの発症を予測するためのrs1130214、rs456998、rs7211818およびrs1053639のSNPの使用(以下「本発明の第一の使用」という)に関する。
対象
有意性の5%レベル、80%パワーおよび2倍のEPSリスク(オッズ比)を前提とするサンプルサイズ計算は、検討する対立遺伝子の担持と関連した(対立遺伝子頻度>0.1)。計算はQuanto1.2 software(http://hydra.usc.edu/gxe)を用いて行った。
以下の方法に従って、mTOR経路遺伝子の制御に関与している異なる9つの遺伝子のSNPを選択した;第一に、精神障害に関連する遺伝子におけるSNPを見つけるために、数種のデータベース(PubMed、Ensembl、Genetic Association Database、SZGene、PDGene、AlzGene、MSGene)を用いて文献調査を行った;第二に、SNPが見つからなかった場合は、他の疾患に関連するSNPを探した。というのもこの関連がタンパク質における機能変化を意味することもあるからである;第三に、いくつかの遺伝子について、前工程でSNPを選択できなかった場合は、PupaSuitデータベースを用いて検索を行い、予測された機能性を有するSNPを検出した;最後に、該SNPのタイプ(同義をコードする、非同義をコードする、イントロン、mRNA utr)および頻度をNCBIのSNPデータベース(http://www.ncbi.nlm.nih.gov/snp)で確認した。10%より低い頻度は排除する。選択されたSNPを表3に特定する。
EPSのリスクに対する各SNPの独立した寄与を評価するために、SNPassoc Rパッケージを用いてロジスティック回帰分析に基づく多変数方法により遺伝子型頻度を評価した。また、症例および対照の両集団においてハーディ・ワインベルグ平衡からの任意の逸脱に関してSNPデータを確認した。次に、オープンソースのMDRプロジェクト (www.epistasis.org/software.html)で入手可能なMDR2.0ソフトウェアを用いて、他の部分に記載されるような多因子次元削減(MDR)により遺伝子−遺伝子相互作用を分析した。第一に、コホート1を用いて、徹底的な調査における10倍交差検証を使用して可能なすべての2〜4座位の相互作用を試験することにより可能なすべてのSNPの組み合わせを構築した(試料はデータを訓練する9つの部分と試験する1つの部分を用いて10の部分に分けた。プロセスを10回繰り返し、各回で試験するための異なる部分を用いた。)。転帰パラメーターとして、本発明者らは、交差検証の一致性(cross-validation consistency)(10倍交差検証の間に計算に適用された最も良好なモデルに基づいて選択された多様性の同定の一致性を測定する)、試験均衡正確度(testing balanced accuracy)(相互作用が正確に症例−対照状態を予測する程度を測定するものであって、10倍交差検証において行われた試験の平均である(ラベル1は該モデルによる良好な予測を示す;ラベル0.5は該モデルが対照から症例を選択する可能性に過ぎないことを示す))および統計的有意性(最も良好なモデルのp−値はMDR並べ替え試験モジュール1.0を用いる10,000の並べ替えによる多重試験について補正した)を検討した。第二に、得られた最も良好なモデルにより新たな多座位特性を作り出した。これが上記の最も良好な転帰パラメーターを有するモデルである。新たな特性を構築および再分析して、コホート1全データセットにおいて統計値を計算し、オッズ比およびその信頼区間、p−値、感度(TP/TP+FN、EPS症例を正確に予測する能力を測定する)、特異度(TN/TN+FP、非EPS対象を正確に却下する能力を測定する)、正確度(TP+TN/TP+TN+FP+FN、EPSおよび非EPS患者を正確に予測する能力を測定する)ならびに精度(TP/TP+FP、EPS症例を正しく予測する能力を測定する)を得た。第三に、構築された特性をコホート2およびコホート3で検証し、コホート1全データセットについてと同様の統計値;オッズ比およびその信頼区間、p−値、感度、特異度、正確度ならびに精度を得た。
コホート1において試験した際には、検討した9つのSNPはいずれも単独ではEPSのリスクに有意に寄与しなかった。表3に症例および対照における対立遺伝子の頻度、性別および年齢により調整したロジスティック回帰分析ならびにハーディ・ワインベルグ平衡に関するp−値を要約する。
Claims (14)
- 対象において抗精神病薬に基づく処置により誘導される錐体外路症状(EPS)の発症を予測する方法であって、
i)前記対象由来の遺伝物質を含んでなる試料において、rs1130214、rs456998、rs7211818およびrs1053639の一塩基多型(SNP)の配列を決定すること、および
ii)前記SNPの配列に基づいて前記対象がEPSを発症するリスクを予測すること
を含んでなる、方法。 - 表1による対立遺伝子の組み合わせの1つの存在が、対象がEPSを発症するリスクが高いことを示し、あるいは、表2による対立遺伝子の組み合わせの1つの存在が、対象がEPSを発症するリスクが低いことを示す、請求項1に記載の方法。
- 抗精神病薬により処置可能な疾患に罹患している対象であって、DRD2遮断効力が低い抗精神病薬に基づく療法を受ける対象を選択する方法であって、
i)前記対象由来の遺伝物質を含んでなる試料において、rs1130214、rs456998、rs7211818およびrs1053639のSNPの配列を決定すること、および
ii)前記SNPの配列に基づいてDRD2遮断効力が低い抗精神病薬に基づく療法を受ける前記対象を選択すること
を含んでなる、方法。 - 表1による対立遺伝子の組み合わせの1つの存在が試料において検出される場合に、DRD2遮断効力が低い抗精神病薬に基づく療法を受ける対象として選択される、請求項3に記載の方法。
- 抗精神病薬により処置可能な疾患に罹患している対象を処置するために適切な抗精神病薬に基づく療法を選択する方法であって、
i)前記対象由来の遺伝物質を含んでなる試料において、rs1130214、rs456998、rs7211818およびrs1053639のSNPの配列を決定すること、および
ii)前記SNPの配列に基づいて適切な抗精神病薬に基づく療法を選択すること
を含んでなり、前記抗精神病薬が、DRD2遮断効力が低い抗精神病薬に基づく療法および任意の抗精神病薬に基づく療法からなる群から選択される、方法。 - 表1による対立遺伝子の組み合わせの1つの存在が、DRD2遮断効力が低い抗精神病薬に基づく療法を受けるものとして選択される対象であることを示し、あるいは、表2による対立遺伝子の組み合わせの1つの存在が、任意の抗精神病薬に基づく療法を受けるものとして選択される対象であることを示す、請求項5に記載の方法。
- 任意の抗精神病薬に基づく療法が、DRD2遮断効力が低い抗精神病薬、DRD2遮断効力が中程度の抗精神病薬およびDRD2遮断効力が高い抗精神病薬からなる群から選択される、請求項5または6に記載の方法。
- DRD2遮断効力が低い抗精神病薬に基づく療法が、さらに補助的抗パーキンソン病薬、好ましくは、抗コリン薬を含んでなる、請求項3〜7のいずれか一項に記載の方法。
- rs1130214、rs456998、rs7211818およびrs1053639のSNPの配列を決定するのに適切な試薬を含んでなるキットであって、前記試薬が、DNAまたはRNAプローブを含んでなる、キット。
- rs1130214、rs456998、rs7211818およびrs1053639のSNPの配列に基づいて、対象において抗精神病薬に基づく処置により誘導されるEPSの発症を予測するための、請求項9に記載のキットの使用。
- 対象において抗精神病薬により処置可能な疾患の処置に用いるためのDRD2遮断効力が低い抗精神病薬であって、前記対象が、請求項3または4に記載の方法によって選択されるか、あるいは、前記対象が、該対象から単離された遺伝物質を含有する試料がrs1130214、rs456998、rs7211818およびrs1053639の一塩基多型(SNP)において表1による対立遺伝子の組み合わせの1つを示すことにより特徴づけられる、抗精神病薬。
- 前記処置が、さらに補助的抗パーキンソン病薬、好ましくは、抗コリン薬を含んでなる、請求項11に記載の抗精神病薬により処置可能な疾患の処置に用いるためのDRD2遮断効力が低い抗精神病薬。
- 対象において抗精神病薬に基づく処置により誘導されるEPSの発症を予測するための、抗精神病薬により処置可能な疾患に罹患している対象であって、DRD2遮断効力が低い抗精神病薬に基づく療法を受ける対象を選択するための、または抗精神病薬により処置可能な疾患に罹患している対象を処置するための適切な抗精神病薬に基づく療法を選択するための、rs1130214、rs456998、rs7211818およびrs1053639のSNPの使用。
- 表1による対立遺伝子の組み合わせの1つの存在が、対象がEPSを発症するリスクが高いことを示し、あるいは、表2による対立遺伝子の組み合わせの1つの存在が、対象がEPSを発症するリスクが低いことを示し、あるいは、表1による対立遺伝子の組み合わせの1つの存在が試料において検出された場合に、DRD2遮断効力が低い抗精神病薬に基づく療法を受ける対象として選択され、あるいは、表1による対立遺伝子の組み合わせの1つの存在が、DRD2遮断効力が低い抗精神病薬に基づく療法を受けるものとして選択される対象であることを示し、あるいは、表2による対立遺伝子の組み合わせの1つの存在が、任意の抗精神病薬に基づく療法を受けるものとして選択される対象であることを示す、請求項13に記載の使用。
