Abstract
Description
基本的には、本発明は、被検者からの生体試料中におけるMG分子を検出及び定量することによって、代謝副産物であるメチルグリオキサール(MG)産生レベルを測定することにある。
上皮由来の癌は、腺癌及び扁平上皮癌のような全組織のタイプ;及び全ての部位、例えば頭及び首の癌(すなわち口腔、舌、中咽頭、咽頭、喉頭等)、気管支及び肺、胸部、胃、結腸直腸、膵臓、肝臓(及びその他の全ての消化器系)、子宮頸部及び子宮内膜、卵巣、尿生殖器(前立腺、膀胱、腎臓)等を含む。非上皮性の癌は、特に白血病、リンパ腫、黒色腫又は肉腫のいずれかのタイプからなる。
また、例えば精巣癌、未分化胚細胞腫、膠芽腫、星状細胞腫、中皮腫、ユーイング肉腫、小児癌及びHIVに関連付けられる腫瘍等のその他の癌も本発明によって特定できる。
癌の「スクリーニング」とは、症状のない個体の集団における代謝的に活性な癌又は前癌性の病変の体系的検出を意味するものと理解される。
その他の開裂した鎖状の異性体であるL-グルコースも同様に、4つの区別可能な環状のL-グルコースを生ずる。
検査される生体試料中においてMGレベル及びグルコースレベルを測定すれば、当該検査される生体試料中におけるMGレベルとグルコースレベルとの比である、いわゆるMG/G指数の算出が可能となる。次いで、μモル/gで表されるこの比と正常対照比とを比較し、患者が癌に罹患しているか否かを判定する。
本明細書で用いるように、用語「腫瘍応答」は、疾患が認知可能な癌患者を抗癌治療した後の腫瘍の進展についての種々の国際的に認められた様式をいう。すなわち、腫瘍応答は、腫瘍を臨床的に測定することによって直接的に、及び/又は利用可能なイメージング技術を用いて腫瘍を測定することによって間接的に評価することができる。応答のタイプは、その間に抗癌治療している間一定の時間間隔で測定する。評価は、治療前に行った測定と治療後に行った測定とを比較することにある。4つの応答カテゴリがある:(1)進行性の腫瘍(腫瘍の体積の増大が25%を超える)、(2)安定な腫瘍(腫瘍の体積の増大が25%未満であり、腫瘍の退縮が25%未満である)、(3)部分的な応答(腫瘍の退縮が25%を超えるが、100%未満である)、(4)完全な応答(測定された腫瘍の体積がゼロである、すなわち利用可能な技術によっては腫瘍の検出が不可能である)。
メチルグリオキサール(MG) -式:(CH3-CO-CH=O又はC3H4O2)のピルビン酸のアルデヒド型(別名ピルブアルデヒド又は2-オキソプロパナール)-は、全ての哺乳類細胞を含む殆どの生命体において存在する独特だが普遍的な分子である(Inoue, Adv Microb Physio 1995)。これは、呼吸をする生物において代謝の重要なステップである解糖中に主に産生される、反応性が高く、用量依存的な細胞毒性の代謝産物である。
本発明者らは、癌細胞が正常細胞より多量のMGを産生及び放出できること、腫瘍内で直接的に、次いで生体の細胞外区画において、より具体的には末梢血において、多量のMGを産生及び放出する一方、正常細胞(又は炎症細胞)が生体の組織及び細胞外区画において、より具体的には末梢血において、検出可能な量のMGを産生及び放出しないか、又は検出可能だが低量のMGしか産生及び放出しないことを見出した。
本発明の第2の主要な実施態様では、本発明は、生体の細胞外区画の生体試料中において、より好ましくは末梢血中においてMG産生レベルを測定し;測定されたMG産生レベルと正常対照値とを比較することによる、被検者における腫瘍の存在を判定するための方法を包含する。
a) 細胞外流体から被検者の生体試料中におけるMG産生レベルを測定する工程と、
b) 上記のMG産生レベルと、対照値、すなわち癌でない被検者におけるMGレベルとを比較する(ここで、生体試料中におけるMG産生レベルが上記の対照値より高い場合には、被検者は癌に罹患しているか、又は癌であるリスクが高い)工程と
を含む、糖尿病でない被検者における癌の早期検出、スクリーニング及び診断のためのインビトロ方法も記載する。
30人の治療されていない1型及び2型糖尿病患者において、癌の発生率は統計的に有意に高い。しかしながら、MG産生レベルは、高血糖状態、すなわち治療されていないか、又は正しく治療されていない糖尿病において増大することが知られている(McLellan, Clin Sci 1994)。したがって、本発明のMG癌バイオマーカーは、これらの患者においてまぎらわしいであろう。
a) 糖尿病の被検者の第1生体試料中におけるMG産生レベルを測定する工程と、
b) 被検者の第2生体試料中におけるグルコースレベルを測定する工程と、
c) これら2つのレベルのMG/G比(MG/G指数)と、健常な個体及び血糖正常治療を受けた糖尿病被検者において測定された対応する対照比とを比較する工程であって、ここで、工程(C)で得られたMG/G指数が対応する対照比より大きい場合、被検者は、癌に罹患しているか、又は癌リスクが増大していると考えられ;工程(C)で得られたMG/G指数が対応する対照比と同程度である場合、被検者は癌に罹患しておらず、かつ、癌リスクは増大していないと考えられる、糖尿病の被検者において癌を早期検出、スクリーニング及び診断するためのインビトロ方法を記載する。
好ましい実施態様では、健常な個体又は血糖正常治療を受けた糖尿病被検者の生体試料から測定及び決定されたMG/G指数は、好ましくは約0.01μモル/gの値であり、健常なドナーの血液から取得したMG/G指数の中央値と、血糖正常治療を受けた非癌性の糖尿病患者の血液から取得したMG/G指数の中央値との中間値であるMG/G指数値に対応する(図4参照)。
イメージング技術は、初期の癌性状態を検出すること及び国際的に認められた4つのカテゴリ(I〜IV)に癌を正しくステージングすることにおいて正確ではない。実際、臨床腫瘍専門医にとって重大な関心事は、亜臨床的状態にある生体において、癌の進行及び拡大を正しく評価することである。
・ステージング及び予後評価のため:
a) 患者から取得した治療前の第1生体試料中におけるMG産生レベルを測定する工程、
b) 治療前MGレベルと正常MG対照値とを比較する工程、
c) ステージ分類の4ステージのうち1つに従って治療前MGレベルを分類する工程、
・抗癌治療の効果をモニタリングするため:
a) 患者から取得した第1生体試料中における治療前の第1MG産生レベルを測定する工程、
b) 第1試料の取得後所定の時点において治療後に患者から取得した第2生体試料中におけるMG産生レベルを測定する工程、
c) 第1及び第2MG産生レベルを比較する工程であって、第2MG産生レベルが第1MG産生レベルより大きい場合、治療は患者に対して有効ではないと考えられ;一方、第2MG産生レベルが第1MG産生レベルより小さい場合、治療は患者に対して有効であり、優先して続けるべきと考えられる工程。
このモニタリング方法は、癌を呈しているヒト又は動物の被検者に用いることができる。
本発明のインビトロ方法は、認知可能な疾患を既に治療されており、好ましくは本発明の方法を用いて、残りの亜臨床的な疾患を治療するためにアジュバント抗癌治療を必要とする癌患者に施した予防的抗癌治療の治療効果をモニタリングするためにも用いることができる。
a) 患者から取得した第1生体試料中における第1MG産生レベルを測定する工程と、
b) 第1試料の取得後所定の時点で前記患者から取得した第2生体試料中における第2MG産生レベルを測定する工程と、
c) 前記第1及び第2産生レベルを比較する工程であって、第2MG産生レベルが第1MG産生レベルより大きい場合、患者の生存可能性は短期間であると予測され;一方、第2産生レベルが第1産生レベルより小さい場合、患者の生存可能性は延長されていると予測される工程と
を含む、患者の生体試料を用いて、癌患者の生存可能性を予測するためのインビトロ方法も記載する。
この治療効果予測方法は、癌を呈するヒト又は動物の被検者に用いることができる。
好ましくは、方法は、血液試料を用いて行う。
注目すべきは、第2試料中におけるMG産生レベルが第1試料中におけるMG産生レベルと同程度である場合;すなわち、これらの比が0.7〜1.3、より一層好ましくは0.9〜1.1である場合、第1及び第2試料が例えば1月間隔で採取されると、患者が増殖中の、安定な、又は崩壊中の癌を有するかについて正確に予測することはできないことである。よって、同じ治療を行って、数週又は数月後に測定を繰り返し、結果を確認することが必要である。
悪液質は、腫瘍量及び転移の有無に拘りなく、癌患者の大部分において(とりわけ膵臓癌、胃癌、大腸癌及び肺癌患者において)起こると推定され、クオリティ・オブ・ライフの乏しさ及び生存期間の短縮に関連付けられる。それは、臨床的には食物摂取量の減少及び体重減少によって、生物学的にはsの炎症、脂肪動員及び酸化の増大、全身のタンパク質分解及び代謝回転の増大、及び炭水化物の代謝障害によって特徴付けられる。悪液質患者において、炭水化物の代謝の変化は、耐糖能、全身のインスリン抵抗性、宿主のグルコース酸化の低減、グルコース新生の増大、及びグルコースの代謝回転及びリサイクリングの増大を含む;これら全てのプロセスにおいて、インスリンが重要な役割を担う(Tayek, J Am Coll Nutr 1992)。
a) 患者から取得した生体試料中におけるMG産生レベルを測定する工程と、
b) MG産生レベルと悪液質関連MG対照値とを比較する工程とを含み、ここで、生体試料中におけるMG産生レベルが悪液質関連MG対照値より大きい場合、患者は悪液質又は重篤な前悪液質になっており、したがって、有効な抗悪液質治療がない場合には生存は短期間になると予測され、
一方、生体試料中におけるMG産生レベルが悪液質関連MG対照値より小さい場合、患者は悪液質にも重篤な前悪液質にもなっておらず、したがって、生存はより延長されていると予測される、癌の被検者又は患者において、悪液質又は前悪液質を予測、検出及び診断するためのインビトロ方法にも関する。
これは、健常な被検者におけるMG正常対照値より約3倍大きい。
繰り返すが、この予測方法は、癌を呈するヒト又は動物の被検者に用いることができる。
直接的なインサイチュMG分析/検出は、固形組織、より具体的には腫瘍の試料中におけるマトリクス支援レーザー脱離イオン化(MALDI)と飛行時間質量分析計(TOF)とを組み合わせたMALDI-TOF/TOF質量分析を用いて行うことができる。
本明細書に提示するデータ(図3及び「実施例」を参照)から、本発明者らは、RP-HPLCを用いて癌患者の全血中におけるMGを測定するときに、そのときの結果の10〜15%が偽陰性の可能性があると推定した。このような場合、組織又は細胞中においてMGを直接測定する本発明の方法のような、その他の方法を用いなければならない。偽陽性の誤りは、慢性尿毒症(Nakayamaら, Am J Nephrol 2008)並びに1型及び2型糖尿病で起こり得るが、慢性尿毒症及び糖尿病は容易に認知及び診断することができ、本発明者らは、糖尿病患者において癌を検出するためにMG/G指数を用いることを提案している。上記のように、糖尿病に加えて、AGEは、老化及び動脈性高血圧、体重超過/肥満又はアルツハイマー病のような癌でないいくつかの年齢に関連する疾患に関連付けられる。MGレベル増大は、動脈壁において、及び高血圧のラットの血液において検出されているが(Wu及びJuurlink Hypertension 2002)、一般的な動脈性高血圧患者の血液中においては、MG産生レベルの増大はこれまで証明されていない。アルツハイマー病患者の髄液中におけるタンパク質の糖化及びMGレベルの増大は報告されているが、アルツハイマー病患者の末梢血中におけるMG増大は観察されていない。また、アルツハイマー病患者の末梢血中において検出される、終末糖化産物に関連付けられるパラメータは、認知症でない対照に比して、より低い値であることが見出された(Thorne Jら, Life Science 1996)。この発見は、このような患者において、血中MGレベルが増大され得ることを示唆するものではない。実際、慢性尿毒症並びに1型及び2型糖尿病を例外として、動脈性高血圧又はアルツハイマー病のような年齢に関連する疾患を患うヒトにおいて、遊離MGの血中レベルが高いことを支持するデータはない。また、正常で健常な被検者においては、老化が血中MG産生レベルに影響するとは考えられておらず、年齢に関連するMG血中レベルは正常な値の範囲内に含まれるので、老化それ自体は偽陽性を構成し得ない。加えて、幾人かの慢性炎症性疾患患者の血中MG産生レベルの増大は観察されていない。

腫瘍検体は、90匹の雄性及び雌性BD-IXラット(Charles River, France)に、PRO腫瘍形成性結腸癌細胞をグラフトし、6週間後に取得した(Charles Riverから提供された45匹のメス及び45匹のオス)。腫瘍を−80℃で凍結し、クライオスタットの手順中、超純水によって−20℃で固定し、12μmの厚みの切片に切断した。次いで、特定のMALDIプレート(Brukerから提供)に切片を置き、調製物をエタノールで処理し、次いでo-PD (0.01%) (Sigma Aldrich, France)で処理し、湿潤室中で、暗中室温で一晩インキュベートした。インキュベーション後、(デシケーターを用いて)切片を乾燥させ、α-シアノ-4-ヒドロキシケイ皮酸(HCCA)(Sigma Aldrichから提供)マトリクス溶液でコーティングした。MALDI-TOF/TOF質量分析計(Bruker UltraFlex III)を用いて2MQX (2-メチルキノキサリン)(Sigma Aldrichから提供)に対する効果を分析することによって、2つの2MQX分子フラグメント(一方は91Da及び他方は118Da)を選択し、腫瘍中におけるMGを検出し、MS/MSイメージング解析を行った。
食べ物及び飲み水にMGが存在し得るので、試料採取前8〜12時間被検体を絶食にしなければならない。血液試料を4℃で採取し、MGは赤血球細胞中で一定濃度であるため、全血について分析を行うことができる。このことは、おそらくは、赤血球細胞において、グリセロンホスフェート及びグリセルアルデヒド-3-ホスフェートからMGが非酵素的に一定速度で生成されるという事実に由来する(Thornalley, Biochem 1989)。
正常細胞と比較した癌細胞によるMG産生の測定は、インビトロ組織培養物を用いて行った。細胞培養物を用いる典型的な実験では、HCT116ヒト結腸直腸癌腫細胞株の馴化培地(CM)中における癌細胞によるMG産生は、LC-MS/MSを用いて行った。低グルコース条件(5.6 mM)下又は高グルコース条件(25 mM)下のいずれかで細胞を培養し、48時間後に回収した。
様々なタイプ及び位置の種々のステージの癌に罹患する101人の継続患者の組の血中MGの存在を分析し、癌患者において得られたレベルを年齢及び性別を合わせた36人の正常な対照及び(試験の陽性対照として用いた6人の治療していない2型糖尿病患者に加えて)12人の血糖正常治療を受けた2型糖尿病患者の組で得られたレベルと比較した。癌患者に包含する基準は、癌の病理学的診断、以前に治療していないこと、臨床的及び/又は生物学的に認知可能な疾患の存在、糖尿病、腎不全及びその他の慢性疾患がないこととした。
実験動物、具体的には同系BDIXラットの1-2ジメチルヒドラジンで誘導された移植可能な結腸癌モデルを用いて、一連の実験を行った。これについて、2つの癌腫細胞クローン(DHD-K12/SRb及びDH-K12/JSb)を予めインビトロで選択し、ラットにグラフトしたときに、それぞれ進行性腫瘍(PROb)及び退縮性腫瘍(REGb)を形成した。これらの実験では、MG並びにグルコース及びインスリンのようなその他の分子を測定するための血液試料を第2週、第3週、第4週、第6週及び第9週に採取した。同時に、腫瘍を評価するために、腫瘍の質量(mass)を測定した。JMP 7(SAS Software, NC, USA)を用いて統計解析を行った。統計的有意性は、Fisher正確検定及び両側スチューデントt検定を用いて決定した。
表1の結果は、癌患者におけるMG血中レベルの平均値及び極値が、男女両方の正常な対照及び2型糖尿病を治療した血糖正常の患者よりも有意に高いことを示す。対照として用いる健常な被検者と血糖正常治療を受けた2型糖尿病患者との間では、有意差は見られなかった。
実際、正常な対照(及び血糖正常治療を受けた2型糖尿病患者)との比較において、MG血中レベルは頭頸部、肺、胸部、前立腺、直腸結腸、膵臓及び消化器系の癌の患者で有意に増大し、これらの患者において、種々のMG値は、腫瘍のタイプに依存して、正常対照値よりも1.5倍〜2倍大きいことを示す。注目すべきは、最も頻繁な癌である乳癌及び前立腺癌について得られた、正常な対照からのMGレベルの統計的に有意な差;肺、直腸結腸、膵臓、頭頸部の癌(これらについては、現在のところ、利用可能な早期検出バイオマーカーがない)についてのMGレベルの大いに統計的に有意な差がある。
腫瘍の体積が予後値であることが十分に実証されているので、ステージングにおけるMG血中レベルは予後指標と考えることができる。また、本発明者らは、MG血中レベルが腫瘍の体積をはっきりと反映することが示されているので、MG血中レベルはまた、後の、疾患の進展の間の予後の指標である。
図5は、PROb腫瘍形成性結腸癌細胞の移植後のBD-IXラットにおけるMG血中レベルの時間的変化を示し、図6は、REGb非腫瘍形成性結腸癌細胞の移植後のBD-IXラットにおけるMG血中レベルの時間的変化を開示する。これらのデータから実証されるように、PROb腫瘍細胞をグラフトしたラットにおいて、MG血中レベルと腫瘍の体積との間には、はっきりとした統計的に有意な正の相関がある。対照的に、REGb腫瘍細胞を移植したが、グラフトが定着できないラットにおいては、移植後4週目の一過的な増大後のMG血中レベルは更に検出することができなかった。このことは、腫瘍グラフトが定着しなかった動物においては、循環MG量の有意な増大は証明されないことを意味する。この実験は、増殖中の腫瘍が、増殖していない腫瘍よりも高いMG血中レベルに有意に関連付けられること、すなわち増殖性の癌細胞が非増殖性の癌細胞又は正常細胞よりも多い循環MG量を産生及び放出することを示す。このことは、MG血中レベルの増大が癌患者において検出可能である一方、癌でない被検者、より正確には活発に増殖する癌のない被検者においては、よりMG血中レベルが有意に低いか、又はMG血中レベルが検出されないことの理由さえ説明する。
表3に示すように、癌を治療した複数の患者の長期的研究は、抗癌治療後の完全な応答について臨床的に評価された患者が正常なMG血中レベルと関連付けられた一方、治療に応答しなかった患者又は治療後に部分的な応答又は安定な疾患を有していた患者は、MG血中レベルの高止まりを示した。よって、癌患者では、MGは、疾患の進展及び治療応答のマーカーである。しかしながら、現在入手可能なバイオマーカー及び応答を評価するためのイメージング技術を用いることによって、治療に完全に応答すると考えられた複数の患者では、なお検出可能な血中MGレベルの増大が見られ、更に、それらのレベルが早期の腫瘍再発に関連付けられた。この発見は、治療した癌患者におけるMG検出が、腫瘍治療に対する応答の評価のための、古典的なバイオマーカー及び/又はイメージング技術に基づく現在利用可能な臨床的アプローチと比べてより良好なツールであり得ることを強く示唆している。
図4で示すように、血中において測定したMG/G指数は、健常な被検者及び血糖正常治療を受けた2型糖尿病患者と比べて、癌患者において殆ど2倍有意に増大する。この結果は、糖尿病患者が潜在的にMGをまぎらわしいものとするグルコース調節異常であるにも拘らず、(MG/G指数の低い)癌でない糖尿病患者と区別して、(MG/G指数の高い)癌を患う糖尿病患者を認識することが可能であることを強く示唆する。
体重超過/肥満の患者は、癌発生率の有意な増大に関連することが示されているので、癌患者 対 正常な対照において、MG血中レベルとBMIとの相関研究を行った。
表4に示すように、体重超過/肥満(BMI>25)の癌患者は、正常な体重(18<BMI<25)の癌患者と比べて、より低い(がなお高い)MG血中レベルに関連付けられる。しかしながら、健常な被検者とは異なり、癌患者では、BMIとMG血中レベルとの統計的に有意な逆相関があり(図7)、このことは、体重超過/肥満の癌患者における0.1μMより高いMG血中レベルの検出が癌に起因している蓋然性が高いことを意味する。したがって、体重超過/肥満の患者におけるMG測定は正当である。
表4に示すように、18未満のBMI (すなわち、低体重又は悪液質)の癌患者は、正常なBMI (18<BMI<25)の患者よりも有意に高いMG血中レベルである。また、MG血中レベルは、アルブミン血中レベルに有意に逆相関することも見出された(データ示さず)。低アルブミン血症が悪液質に関連付けられることが示されているので、このことは、癌患者において、高いMG血中レベルが悪液質に関連付けられることを間接的に確認する。この結果は、上記したように、癌患者におけるMG血中レベルがBMIと有意に逆相関することを示す一方(図7B)、健常な被検者においては、MG血中レベル及びBMIは相関しない(図7A)ことを示す図7に示されている。
悪液質において、I/G指数は、悪液質の進行度に依存して、25%の症例でそれぞれ増大されているか又は正常であり、50%の症例で低減されており、指数が低ければ悪液質の重篤度はより低い。実際、I/G指数の増大がインスリン抵抗性に関連する一方、I/G指数の低減がβ膵臓細胞によるインスリン分泌不全に関連することは周知である。図8に示すように、健常な被検者においては、MG血中レベルの値が何であれ、I/G指数は一定であるが、癌患者においては、それはMG血中レベルに有意に逆相関する。上記の考察に基づき(上記参照)、2つの曲線の交点を、癌患者のI/G指数が健常な被検者よりも低くなる臨界点と定義する。この交点は、MG臨界値(いわゆる「悪液質関連MG対照値」)といい、これを超えると、癌患者において、健常な被検者よりも少ないインスリン分泌が起こる。このことは、グラフ上で決定された0.2μMの血中MG対照値がMG臨界値に対応し、これを超えると、癌患者は、悪液質又は重篤な前悪液質になる(図8)。
膵臓におけるインスリン分泌との比較においてインスリン抵抗性レベルを測定するための、及び、これらの患者における悪液質又は重篤な前悪液質の状態になっていることを客観的に認識するための、癌患者における血中MGの測定は正当なようである。
Claims (27)
- 化学的又は免疫学的なメチルグリオキサール(MG)のインビトロ測定方法を用いて、細胞及び/又は組織試料中における代謝的に活性な癌細胞によるインサイチュMG産生を測定及び分析することによって癌を早期検出及び診断するための方法。
- MALDI-TOF/TOF質量分析又は類似の手法を用いることを含む請求項1に記載の方法。
- a) 糖尿病でない被検者の細胞外流体からの生体試料中におけるメチルグリオキサール(MG)産生レベルを測定する工程と;
b) 前記産生レベルを対照値、すなわち癌でない被検者におけるMGレベルと比較する工程であって、前記生体試料中におけるMG産生レベルが前記対照値より大きい場合には、その被検者は癌に罹患していると考えられる工程と
を含む、糖尿病でない被検者における細胞外流体の生体試料中における癌の早期検出及び診断のためのインビトロ方法。 - 前記対照値が、健常な個体の生体試料中において測定されたMG産生レベルであり、好ましくは血中で約0.06μMの値である請求項3に記載のインビトロ方法。
- a) 糖尿病被検者の第1生体試料中におけるMG産生レベルを測定する工程と、
b) 前記被検者の第2生体試料中におけるグルコースレベルを測定する工程と、
c) これら2つのレベルの[メチルグリオキサール/グルコース]比(MG/G指数)と、健常な個体及び血糖正常治療を受けた糖尿病被検者において測定された対応する対照比とを比較する工程とを含み、
工程c)で得られたMG/G指数が前記対応する対照比より大きい場合には、前記被検者は癌に罹患しているか、又は、癌リスクが増大していると考えられ、
工程c)で得られたMG/G指数が前記対応する対照比と同程度である場合には、前記被検者は癌に罹患しておらず、また、癌リスクも増大していないと考えられる、糖尿病患者において癌を早期検出及び診断するためのインビトロ方法。 - 前記第1及び第2試料を同時に採取し、好ましくは単一の試料から取得する請求項5に記載のインビトロ方法。
- 前記対照比が、健常な個体又は血糖正常治療を受けた糖尿病被検者の生体試料から測定された[メチルグリオキサール/グルコース]比(MG/G指数)であり、好ましくは約0.01μモル/gの値である請求項5又は6に記載のインビトロ方法。
- 前記生体試料が血液試料である請求項3〜7のいずれか1つに記載のインビトロ方法。
- 前記腫瘍が、頭頸部、肺、胸部、前立腺、結腸直腸、膵臓又はその他の消化器系の腫瘍である請求項1〜8のいずれか1つに記載のインビトロ方法。
- 前記癌が、白血病、リンパ腫、黒色腫、肉腫、小児癌、又は脳、泌尿生殖器、子宮若しくは卵巣の癌又はその他の癌である請求項1〜8のいずれか1つに記載のインビトロ方法。
- 前記方法が、腫瘍又は炎症のプロセスに用いられることによって、悪性腫瘍と良性腫瘍との区別及び癌と炎症のプロセスとの区別を可能にする請求項1〜8のいずれか1つに記載のインビトロ方法。
- 前記方法が、症状のない被検者における癌のスクリーニングに用いられる請求項1〜11のいずれか1つに記載のインビトロ方法。
- 前記被検者が、ヒト又は動物である請求項1〜12のいずれか1つに記載のインビトロ方法。
- ヒト又は動物のいずれかである患者から取得した生体試料、好ましくは血液中におけるMG産生レベルを測定することによって、ヒトであるか又は動物であるかに拘りなく、癌患者の疾患をステージング及び予後評価するためのインビトロ方法。
- a) 癌患者から取得した第1生体試料中における治療前の第1MG産生レベルを測定する工程と、
b) 治療後であって、第1試料の取得後所定の時点で前記患者から取得した第2生体試料中における第2MG産生レベルを測定する工程と、
c) 前記第1及び第2MG産生レベルを比較する工程とを含み、
前記第2MG産生レベルが前記第1MG産生レベルより大きい場合、治療は前記患者に対して有効でないと考えられ;
前記第2MG産生レベルが前記第1MG産生レベルより小さい場合、治療は前記患者に対して有効であると考えられる、
癌患者に施された抗癌治療の治療効果をモニタリングするためのインビトロ方法。 - 請求項15に記載の方法を用いて、亜臨床的な癌が検出された症状のない被検者に施された予防的抗癌治療の治療効果をモニタリングするためのインビトロ方法。
- 認知可能な疾患について既に治療されており、残りの亜臨床的な疾患を治療するためにアジュバント抗癌治療を必要とする癌患者に施された予防的抗癌治療の治療効果を、請求項15に記載の方法を用いてモニタリングするためのインビトロ方法。
- a) 癌患者から取得した生体試料中におけるMG産生レベルを測定する工程と、
b) 前記MG産生レベルと、悪液質関連MG対照値とを比較する工程とを含み、
前記生体試料中におけるMG産生レベルが悪液質関連MG対照値より大きい場合、前記患者は、悪液質又は重篤な前悪液質になっており、
前記生体試料中におけるMG産生レベルが悪液質関連MG対照値より小さい場合、前記患者は、悪液質にも重篤な前悪液質にもなっていない、癌の被検者又は患者における悪液質又は前悪液質を予測、検出及び診断するためのインビトロ方法。 - 癌患者における[インスリン/グルコース]比(I/G指数)の時間的変化と健常者におけるI/G指数の時間的変化とを比較して前記MG対照値を決定することにより、血中MGが約0.2μMと推定され、これを超えるとβ膵臓細胞によるインスリン分泌レベルに欠陥があり、癌患者が悪液質又は重篤な前悪液質になっている「悪液質関連対照値」と称されるMG産生レベルの臨界点の特徴決定を可能にするインビトロ方法。
- a) 癌に罹患する患者から取得した第1生体試料中における第1MG産生レベルを測定する工程と、
b) 第1試料の取得後所定の時点で前記患者から取得した第2生体試料中における第2MG産生レベルを測定する工程と、
c) 前記第1及び第2産生レベルを比較する工程とを含み、
前記第2MG産生レベルが前記第1MG産生レベルより大きい場合、前記患者の生存可能性は短期間であると予測し、
前記第2MG産生レベルが前記第1MG産生レベルより小さい場合、前記患者の生存可能性はより長期間であると予測する、
癌に罹患する患者又は被検者の生存可能性を、前記患者又は被検者の生体試料を用いて予測するためのインビトロ方法。 - 前記生体試料が血液試料である請求項12〜20のいずれか1つに記載のインビトロ法。
- - 生体試料を採取するための手段と、
- MG産生レベルを測定するための手段と、
- キットを使用するための説明書と、
- 任意に、対照試料と
を含む、癌の早期検出及び診断のための、癌のステージングのための、癌患者の生存可能性の予測のための、抗癌治療に対する応答のモニタリングのための及び悪液質の予測及び早期検出のためのキット。 - 請求項22に記載の手段及び説明書を含む癌のスクリーニングのためのキット。
- MALDI-TOF/TOF質量分析又は類似の手法によって細胞塗抹標本又は組織においてMGをインサイチュで検出及び測定するための手段が提供され、
細胞外流体中においてMGを測定するための手段が、o-PD又はDMB、2MQX又はDMQ、MQX又はDDQを含むRP-HPLC分析(化学的試験)のための化学的試薬及び任意に「サンドイッチ」ELISA試験においてMGを特異的に認識するモノクローナル又はポリクローナル抗体からなる化学的及び免疫酵素学的試験のキット群から選択される請求項22又は23に記載のキット。 - グルコース産生レベルを検出するための手段及びMG/G指数を決定するための説明書を更に含む請求項22〜24のいずれか1つに記載のキット。
- 化学的又は免疫学的なMGのインビトロ測定方法を用いて、細胞外流体、細胞及び/又は組織の試料中におけるMG産生を測定及び分析する、代謝的に活性な癌の早期検出及び診断における使用のためのメチルグリオキサール(MG)。
- MALDI-TOF/TOF質量分析又は類似の手法を用いることを含む請求項26に記載の使用のためのメチルグリオキサール(MG)。
