B細胞をex vivoで増加させる方法は、対象動物から採集したB細胞をCD40アゴニスト及びB細胞生存促進因子(例えばBAFF(BlyS))を発現するフィーダー細胞とIL-4の存在下で接触させ、得られた細胞を続いてCD40アゴニスト及びB細胞生存促進因子(例えばBAFF(BlyS))を発現するフィーダー細胞とIL-21の存在下で接触させる工程を含み、前記工程は該増加B細胞の収集又は単離の前に実施される。該増加ポリクローン性B細胞はまた更なる選別に付すことができる。例えば、該増加ポリクローン性B細胞は、B10細胞の単離のために更なる選別に付すことができる。
自己免疫疾患、アレルギー性異常、炎症性異常又は免疫不全を有する対象動物を治療する方法もまた提供される。該方法は、本明細書に記載の増加B細胞又はB10細胞を含む組成物の治療的に有効な量を、自己免疫疾患、アレルギー性異常、炎症性異常又は免疫不全の治療を必要とする対象動物に投与する工程を含む。
器官、組織若しくは細胞移植物拒絶又は関連する移植片対宿主病を予防若しくは治療するために対象動物を処置する方法もまた提供される。該方法は、該B細胞及び/又はB10細胞組成物を含む組成物の治療的に有効な量を、移植物拒絶又は移植片対宿主病の治療を必要とする対象動物に投与する工程を含む。
B10細胞機能を対象動物で評価する方法もまた提供される。これらの方法では、B細胞は対象動物由来のサンプルから採集され、IL-21と少なくとも24時間接触される。該B細胞を続いて試験して該細胞がIL-10を産生できるか否かを決定するか、及び/又は正常なB細胞機能を有するコントロールと比較して産生されたIL-10の量及び/又はIL-10を産生できる培養中の細胞のパーセンテージが決定される。IL-10を発現するB細胞はB10細胞である。該B細胞は、CD40アゴニスト及びB細胞生存促進因子(例えばBAFF)を発現するフィーダー細胞とIL-4の存在下で接触させ、引き続いてIL-21と接触させることができ、前記接触は該細胞のIL-10産生能力の評価の前に実施される。
本明細書で提供されるB10細胞機能を評価する方法を用いて自己免疫又は炎症性疾患を診断することができ、さらに前記方法を用いて対象動物の疾患又は症状の病期を評価することができる。
B10細胞の免疫調節は抗原特異的であり、B細胞抗原受容体(BCR)特異性はB10細胞進化に多大な影響を与える。BCRシグナリングを正の方向に又は負の方向に調節する受容体又は経路もまた、B10細胞数をin vivoで調整することができる。例えば、CD19欠損(CD19-/-)マウスは本質的に調節性B10細胞を欠き、前記は、接触過敏時に及び多発性硬化症の実験的自己免疫脳脊髄炎(EAE)モデルで炎症及び疾患諸症候の悪化をもたらす。IL-10そのものはB10細胞の進化に必要とされない。なぜならば、IL-10受容体遺伝子を発現する能力を有するB細胞は通常IL-10-/-マウスでは進化しないからである。B10細胞数もまた、T細胞欠損ヌードマウス及び主要組織適合複合体クラスII(MHC-II)又はCD40分子(同族B細胞-T細胞相互作用に重要である)の発現不全を示すマウスで正常である。結果として、適切なBCRシグナルは、IL-10コンピテントB10細胞になるB細胞を選別すると考えられる。生来の病源体誘発シグナルもまた調節性B10細胞のin vivo進化に影響を及ぼす。B10細胞のIL-10産生がin vivoでどのように調節されるかについては、その他のことはほとんど明らかではなく、どのようにしてそのような希少B細胞がそのような強力なin vivo作用を発揮し、抗原特異的T細胞機能を炎症及び自己免疫時に選択的に阻害するのかは不明なままである。
さらに我々はまたヒトB細胞及びB10細胞はex vivoで増加させることができることを示す。B細胞を正常なヒト血液から採集し、マウスB細胞の増加に用いた方法と同じ方法を用いてex vivoで増加させた。実施例で述べるように、B細胞数は130倍増加したが、一方、B10細胞は5−6000倍増加した。したがって、これらの例は、該方法を用いて、応答性B細胞又はB10細胞の添加が治療方法となり得るヒトの疾患又は症状の自己由来治療に有用であり得るex vivo増加B細胞を作製できることを示している。
本明細書に記載されるものは、ポリクローン性B細胞(具体的にはB10細胞)をex vivoで増加させる方法である。該方法は、対象動物からB細胞を採集する工程及びそれらをIL-21とともにインキュベートする工程を含む。図1では、B10細胞は、IL-21(100ng/mL)とのex vivoインキュベーションの48時間後に2倍を超えて、さらに72時間インキュベーション後には3倍を越えて増加した。該方法はさらに、CD40リガンド(CD154)及びB細胞生存促進因子、例えばBAFF(BLyS(本明細書では互換的に用いられる))、又はCD40リガンド及び/又はBAFFを発現するフィーダー細胞とのインキュベーションを含み、B細胞及びB10細胞の更なる増加をもたらすことができる。実施例に示すように、B細胞は、IL-4との第一のインキュベーションと前記に続くIL-21を含む第二のインキュベーションによって更なる増加を生じることができる。これらの工程の一方又は両方がフィーダー細胞及び場合によってCD40リガンド及び/又はBAFFを含むことができる。本明細書に記載の方法を用いて全B細胞が増加されるが、B10細胞はこれらの方法を用いてより高頻度で増加された。さらに、該B10細胞は、LPS又は別の刺激性シグナルによる更なるex vivo刺激を必要とすることなくIL-10を産生することができた。適切には、該B細胞はヒトB細胞である。
CD40アゴニストの非限定的な例には、CD40抗体及びそのフラグメント、CD40リガンド(CD154)及びそのポリペプチドフラグメント、小分子、合成薬、ペプチド(環状ペプチドを含む)、ポリペプチド、タンパク質、核酸、アプタマー、合成若しくは天然の無機分子、模倣薬剤、及び合成若しくは天然の有機分子が含まれる。ある種の実施態様では、CD40アゴニストはCD40抗体である。該CD40抗体は任意の形態であり得る。CD40に対する抗体は当業界では公知である(例えば以下を参照されたい:Buhtoiarov et al., 2005, J. Immunol. 174:6013-22;Francisco et al., 2000, Cancer Res. 60:3225-31;Schwulst et al., 2006, 177:557-65(前記文献は参照によりその全体が本明細書に組み入れられる))。該CD40アゴニストはCD40リガンドであり得る。さらに該CD40アゴニストは、フィーダー細胞の表面で発現されるか、又は可溶性であり得る。
実施例で用いられるフィーダー細胞は線維芽細胞であるが、他のフィーダー細胞を当該方法で用いてもよい。フィーダー細胞は、内皮細胞、上皮細胞、ケラチノサイト、メラニン細胞、又は他の間葉系若しくは間質細胞であり得る。該方法で用いられるインキュベーション又は培養期間は、当該方法の各工程について2から10日又は11日以上であり得る。適切には、インキュベーション時間は3から7日の間、適切には、前記時間は4から5日である。実施例に記載するように、フィーダー細胞は、当該方法で最適なB細胞増加を可能にするために、おそらく追加のシグナル(CD40アゴニスト及びBAFF以外のもの)を供給することを要求される。B細胞増加を最適にするためにフィーダー細胞によって供給される他の因子の予備的分析は、下記の実施例及び表1に含まれる。要約すれば、CD40アゴニスト及びBAFFに加えて、該フィーダー細胞は最小限VCAM-1及びCD44をCD44アゴニスト及びBAFFの他に発現する。フィーダー細胞によるCD24、インターロイキン-7(IL-7)、Mst1及びTslpの発現増加は、ex vivo増加中にB細胞数を増加させることができるフィーダー細胞と相関性を有する。同様に、フィーダー細胞におけるある種の分子のダウンレギュレーションは、B細胞増加を支援する能力の増加と相関性を有する。特に、フィーダー細胞におけるCD99、TGFBI、CXCR7、Dlk1、Jag1及びNotch1のダウンレギュレーションは、B細胞のex vivo増加をより良好に支援することができる細胞と相関性を有する。したがって、当業者は、B細胞のex vivo増加をより良好に支援できるフィーダー細胞を選別するか、又は遺伝子操作により作り出すことができる。当該情報もまた、最適なB細胞ex vivo増加のために生フィーダー細胞を必要としないB細胞増加手段の考案のために用いることができる。
本明細書に記載の方法を用いて作製された増加ポリクローン性B細胞を含む組成物もまた提供される。ある特徴では、該組成物は、50%、55%、60%、65%、70%、75%、80%、85%、90%、95%、98%、99%を超える、又は実質的に100%のB10若しくはB10pro細胞を含む。他の特徴では、該組成物は、抗体産生B細胞を含むように選別される。該組成物は、50%、55%、60%、65%、70%、75%、80%、85%、90%、95%、98%、99%を超える、又は実質的に100%の抗体産生B細胞を含むことができる。
該増加B細胞含有組成物を用いて医薬組成物を作製できる。上記に記載の増加B細胞及び医薬的に許容できる担体を含む医薬組成物が提供される。医薬的に許容できる担体は、細胞のin vivo投与に適切な任意の担体である。該組成物での使用に適切な医薬的に許容できる担体の例には、緩衝溶液、グルコース溶液、油系又は細胞培養系液体が含まれるが、ただしこれらに限定されない、該組成物に追加される成分は、適切には例えば賦形剤、例えば安定化剤、保存料、希釈剤、乳化剤及び滑沢剤を含み得る。医薬的に許容できる担体又は希釈剤の例には、安定化剤、例えば炭水化物(例えばソルビトール、マンニトール、デンプン、シュクロース、グルコース、デキストラン)、タンパク質(例えばアルブミン又はカゼイン)、タンパク質含有物質(例えばウシ血清又は脱脂乳)及び緩衝剤(例えばリン酸緩衝剤)が含まれる。

任意の疾患、異常又は症状に由来する任意の抗原を本発明の方法にしたがって用いることができる。例示的抗原には、細菌、ウイルス、寄生生物抗原、アレルゲン、自己抗原及び腫瘍関連抗原が含まれるが、ただしこれらに限定されない。DNA系ワクチンが用いられる場合、抗原は典型的には投与されるDNA構築物の配列によってコードされるであろう。また別には、抗原が複合物として投与される場合、該抗原は典型的には該投与複合物中に含まれるタンパク質であろう。特に抗原はタンパク質抗原、ペプチド、不活化全生物などを含むことができる。
別の実施態様では、B10細胞サブセットは将来の移植物ドナーに由来する抗原で感作され、したがって移植レシピエントのB10細胞によるIL-10産生レベルを増加させることができる。移植レシピエントのB10細胞サブセットによるIL-10産生増加は該移植レシピエントにおける移植物に対する免疫/炎症応答の低下をもたらす。移植におけるB10細胞の役割は下記でより完全に記載される。
該増加B細胞組成物を疾患又は症状を有する対象動物を治療する方法で用いることができる。B細胞及び特にB10細胞のそのような養子移入は、広範囲の多様な疾患の抑制に有効であり得る。前記疾患には、自己免疫疾患、炎症性疾患、又はB細胞若しくはB10細胞集団の対象動物の導入によって治療し得る一切の他の疾患が含まれる(ただし前記に限定されない)。B細胞又はB10細胞の養子移入はさらに、細胞及び/又は組織の移植に付随する免疫/炎症応答を最小限にするために利用することができる。
例示的な養子移入プロトコルでは、最初に標的ドナーから混合細胞集団が抽出される。適切にはB細胞が選別され、より適切にはB10細胞及びB10pro細胞が対象動物から選別される。ドナーから単離される細胞はドナー内のそれらが存在する任意の位置から単離でき、当該位置には、上記でより完全に記載されたドナーの血液、脾臓、リンパ節、及び/又は骨髄が含まれる(ただしこれらに限定されない)。当該適用に応じて、細胞は、健常なドナー、疾病罹患ドナー(緩解期又は活動病期にある)から、又は死亡ドナーの器官、血液若しくは組織から抽出できる。後者の事例では、ドナーは器官ドナーである。さらに別の実施態様では、細胞は該対象動物から入手され、増加及び/又は活性化され、該対象動物に戻され得る。
B10細胞集団は、当該疾患にふさわしい治療レジメンで、例えば病状の緩和のために1日又は数日にわたって1用量若しくは数用量を、又は疾患の進行を抑制しかつ疾患の再発を予防するために長期間にわたって周期的用量を投与することができる。例えば、該組成物は、4時間、6時間、8時間、12時間、1日、2日、3日、4日、1週間、2週間、又は3週間以上離して2回以上投与することができる。処方物として用いられるべき厳密な用量もまた、投与経路、疾患又は異常の重症度、及び疾患が性質として慢性であるか否かで左右され、医師の判断及び各患者の環境にしたがって決定されるべきである。
別の特徴では、ドナーから入手されたB細胞又はB10細胞は所望の位置でレシピエントに導入され、したがって該B細胞又はB10細胞集団の治療効果(すなわちIL-10産生又は抗体分泌)を特異的に目標とすることができる。そのような技術は、移植可能な免疫調整装置を用いて達成できる。前記装置は、例えばバーチャルリンパ節、例えば米国特許出願公開No.2003/0118630、国際出願N.WO1999/044583、米国特許6,645,500号(前記は参照によりその全体が本明細書に組み入れられる)に記載されたものである。この実施態様にしたがえば、B細胞又はB10細胞集団は上記に記載したようにドナーから単離され、移植可能免疫調整装置に添加され、続いて前記装置は、B細胞又はB10細胞集団の治療効果(すなわち抗体又はIL-10産生)が必要とされる位置でレシピエントに移植され得る。
本明細書で用いられるように、“十分量”又は具体的な結果の達成“のために十分な量”は、所望の効果を生じるために有効である本発明の多数のB、B10又はB10エフェクター細胞を指す(前記所望の効果は場合によって治療的効果である(すなわち治療的に有効な量の投与による))。例えば、“十分量”又は“のために十分な量”は、対象動物の症状の重症度を変化させるために有効な量であり得る。
本明細書で用いられる“治療的に有効な”量は、対象動物に何らかの改善又は利益を提供する量である。言い換えれば、“治療的に有効な”量は、少なくとも1つの臨床症候で何らかの緩和、軽減及び/又は低下を提供する量である。本発明の方法によって治療できる異常に付随する臨床的諸症候は当業者には周知である。さらにまた、該治療効果は、何らかの利益を対象動物に提供するかぎり完結的又は治癒的である必要がないことは当業者には理解されよう。個体を“治療する”ために要求される細胞の“治療的に有効な”数は、当業者に周知のように、B細胞の供給源、血液採集時の患者の免疫学的状態、治療時の個体の症状、及び治療時における免疫抑制薬剤若しくは物質による治療的処置レベルに左右されるであろう。
自己免疫疾患
IL-10レベルの下降に付随する疾患及び症状は本発明のこの特徴にしたがって治療できる。IL-10のレベル低下は、以下を含む(ただしこれらに限定されない)自己免疫疾患及び炎症性疾患で示されている:乾癬(Asadullah et al., 1998, J. Clin. Investig. 101:783-94;Nickoloff et al., 1994, Clin. Immunol. Immunopathol., 73:63-8;Mussi et al. 1994, J. Biol. Regul. 本発明のこの方法にしたがって任意のタイプの自己免疫疾患を治療できる。“自己免疫疾患又は異常”という用語は、対象動物自体の細胞、組織及び/又は器官に対する該対象動物の免疫反応によって引き起こされる細胞、組織及び/又は器官の損傷を特徴とする対象動物の症状を指す。“炎症性疾患”という用語は、炎症、好ましくは慢性炎症を特徴とする対象動物の症状を指すために“炎症性異常”という用語と互換的に用いられる。自己免疫異常は炎症を伴うことも伴わないこともある。さらにまた、炎症は自己免疫異常によって生じることも生じないこともある。したがって、ある種の異常は自己免疫及び炎症性異常の両方を特徴とし得る。
例えばアメリカリウマチ学会スコア(American College of Rheumatologists Score;ACRスコア)を用いて、患者又は患者集団の慢性関節リウマチの活動性を決定できる。この方法にしたがえば、患者は改善と相関性を有するスコアが与えられる。例えば、ACRが規定する因子で20%の改善を有する患者はACR20スコアが与えられるであろう。このスコア付与及び別のスコア付与を本明細書に記載するB10細胞機能を評価する方法と組み合わせて用いることができる。
本明細書に記載の方法にしたがって任意のタイプの炎症性疾患を治療することができる。炎症性疾患の非限定的な例には以下が含まれる(ただしこれらに限定されない):喘息、脳炎(encephilitis)、炎症性腸疾患、慢性閉塞性肺疾患(COPD)、アレルギー性異常、敗血症ショック、肺線維症、未分化型脊椎関節症、未分化型関節症、関節炎、炎症性骨溶解、及び慢性ウイルス性若しくは細菌性感染症に起因する慢性炎症。
本発明の方法は、ヘルパーT(Th)1媒介炎症応答関連疾患の治療を目的とするがTh2媒介炎症応答関連疾患の治療は目的としない治療方法を包含する。この実施態様のまた別の特徴では、本発明の方法は、Th1媒介炎症応答関連疾患ではなくTh2媒介炎症応答関連疾患の治療を目的とする治療方法を包含する。
IL-10は、虚血/再灌流損傷(Deng et al., 2001, Kidney Int. 60:2118-28)、移植片対宿主病、及び移植関連死(Baker et al., 1999, Bone Marrow Transplant 23:1123-9; Holler et al., 2000, Bone Marrow Transplant 25:237-41)を抑制できる。したがって、本発明のある実施態様は、その必要がある患者でIL-10のレベルを高めることによって移植関連疾患/症状の治療に密接に関係する。
ある実施態様では、内因性IL-10のレベルは、本明細書に記載の方法により作製したB10細胞セット(例えばB10細胞)を投与することによって、器官移植を受けた対象動物で高められる。B10細胞集団は当該患者自身から単離することができる(すなわち該対象動物がドナーである)。また別には、B10細胞は当該対象動物ではないドナーから単離され得る。B10細胞のドナーは当該器官のドナーと同じでもよく、又はB10細胞集団は数人のドナーからプールされてもよい。
本発明は、移植片対宿主病(GVHD)又は液性若しくは細胞性拒絶エピソードの発生率、重症度又は持続期間を減少させるために有効な組成物、治療処方物、方法及びレジメンを提供する。ある種の実施態様では、本発明の組成物及び方法は、固形組織又は器官移植片の虚血性再灌流損傷に対する宿主応答を減弱させるために有効である。好ましい実施態様では、本発明のB10エフェクター細胞組成物及び方法は、移植レシピエントにおける移植片の生存を長引かせるために有効である。
本発明のB10エフェクター細胞組成物及び方法を用いてGVHD又は液性若しくは細胞性拒絶を治療又は予防することができ、移植又は移植される個々の組織タイプに対する必要性を最初に生じさせた個々の適応症を考慮する必要はない。しかしながら、移植又は移植される組織タイプに対して必要性を生じさせた当該適応症はGVHD及び移植片拒絶の治療又は予防のための包括的な療養レジメンの基礎を提供でき、当該包括的レジメンは本発明のB10エフェクター細胞組成物及び方法を含む。
ある種の実施態様では、個々のレジメンは移植拒絶の段階にしたがって変動し、液性又は細胞性拒絶の後期段階にある患者のためにはよりアグレッシブな治療方法が指示される。液性拒絶の段階は当業界の知識及び技量にしたがって分類され得る。例えば、液性拒絶の段階は以下の基準にしたがってI期からIV期の1つに分類できる。I期潜伏性応答は循環性抗ドナーアロ抗体(特に抗HLA抗体)を特徴とし;II期サイレント反応は循環性抗ドナーアロ抗体(特に抗HLA抗体)及びC4d沈着を特徴とするが組織学的変化又は移植片機能不全は伴わず;III期準臨床的拒絶は循環性抗ドナーアロ抗体(特に抗HLA抗体)及びC4d沈着並びに組織病変を特徴とするが移植片機能不全は伴わず;IV期液性拒絶は循環性抗ドナーアロ抗体(特に抗HLA抗体)、C4d沈着、組織病変及び移植片機能不全を特徴とする。同様に、細胞性拒絶の基準は当業界の実務者に公知である。
本発明のB10エフェクター細胞組成物及び方法は、GVHD又は液性若しくは細胞性拒絶を治療又は予防するために、単独で又は他の治療薬剤若しくは治療レジメンと組み合わせて用いることができる。GVHD又は液性若しくは細胞性拒絶を治療又は予防するための他の療養レジメンは、例えば抗リンパ球療法、ステロイド療法、抗体枯渇療法、免疫抑制療法及び血漿交換の1つ以上を含むことができる。抗リンパ球療法は、抗胸線細胞グロブリン(サイモグロブリンとも称される)の移植レシピエントへの投与を含むことができる。抗リンパ球療法はまた、T細胞(及びB細胞表面抗原を含む)に対向する1つ以上のモノクローナル抗体の投与を含むことができる。そのような抗体の例には以下が含まれる(ただしこれらに限定されない):OKT3TM(ムロモナブ-CD3)、CAMPATHTM-1H(アレムツズマブ)、CAMPATHTTM-1G、CAMPATHTM-lM、SIMULECTTM(バシリキシマブ)、及びZENAPAXTM(ダクリズマブ)。
抗体枯渇療法は、例えば静脈内免疫グロブリンの移植レシピエントへの投与を含むことができる。抗体枯渇療法はまた、移植前にex vivoで適用される移植片の免疫吸着療法を含むことができる。免疫吸着は、任意の適切な技術(例えばタンパク質Aアフィニティー)又は抗体系アフィニティー技術(T細胞又はB細胞表面マーカーに対向する抗体(例えば抗CD3抗体)を用いる)を用いて達成できる。

ある実施態様では、ヒト移植レシピエントで液性又は細胞性拒絶を予防する組成物及び方法が提供され、ここで該移植レシピエントは、液性又は細胞性拒絶を発達させるリスクが高くなった患者又は患者集団と認定される。そのような患者はまた“感作されている”と称することができる。感作患者の認定基準は習熟医師には公知である。そのような基準は、例えばHLA抗原に対する循環性抗体(例えば抗HLAアロ抗体)の検出可能レベルを有する患者を含むことができる。そのような基準はまた、以前に移植、妊娠又は多数の輸血の経験がある患者を含むことができる。液性拒絶のリスクが高くなっている患者にはまた、ドナー-レシピエントHLAマッチングが不完全である者、及びABO不適合である移植物を有する者が含まれる。感作個体は、移植前の前処理又は条件付けのための好ましい候補対象者である。感作個体はまた、液性及び細胞性拒絶の予防のための移植後維持レジメンのための好ましい候補対象者である。
ある実施態様では、本発明のB10細胞組成物及び方法は、初期液性拒絶の治療のために療養レジメンを含むか、又は前記療養レジメンと併用される。具体的な実施態様では、該初期液性拒絶はI期、II期又はIII期拒絶である。初期液性拒絶の臨床指標は当業界の知識及び技量にしたがって決定され、例えば、患者における循環性ドナー特異的抗HLA抗体の発達、抗体活性における補体マーカーの存在(例えば移植片生検におけるC4d及びC3dの沈着)、及び移植片生検における抗HLA抗体の存在が含まれ得る。初期液性拒絶の他の指標は習熟医師には公知であり、例えば、抗内皮抗体(特に抗ビメンチン抗体)の発達、及び非古典的MHCクラスI関連鎖A(MICA)アロ抗体の発達が含まれ得る。ある実施態様では、該組成物及び方法は、部分的には移植片機能不全を特徴とする液性又は細胞性拒絶の治療のための療養レジメンを含むか、又は前記と併用される。具体的な実施態様では、液性又は細胞性拒絶の治療を必要とする患者又は患者集団は、移植片機能不全のための当業界で公知の基準にしたがって認定される。他の実施態様では、液性又は細胞性拒絶のための治療を必要とする患者又は患者集団は、組織移植片のタイプに固有の他の基準(例えば組織学的基準)にしたがって認定される。当業者はそのような基準を認識しているであろう。
組換え体、治療タンパク質又は異種タンパク質を投与される対象動物を治療する方法もまた提供される。当該方法は、本明細書に記載のB10細胞の治療的に有効な量を、遺伝的異常、移植、アレルギー、炎症、又は自己免疫異常のための治療を必要とする対象動物に投与する工程を含む。具体的には、B10細胞は、生物製剤又は他の治療薬(前記に対して対象動物が抗薬剤抗体を生じる可能性があるか又は既に生じている)とともに共投与され得る。
病院に到達する生物製剤療法の数は増加し続けているので、レシピエントがこれら薬剤に対して免疫応答をしばしば発達させるということはますます理解されるところである(免疫応答は薬剤の有効性に対して潜在的な臨床的衝撃を有する)。生物製剤に対する免疫応答は、一般的には、抗薬剤抗体(ADA)の検出及び特徴付け並びに薬剤曝露とADAの関係、ADAの有効性及び安全性の評価によってモニターされる。ADAの検出は必ずしも臨床的な結果が存在することを意味しない。しかしながら、ADAが薬剤有効性及び患者の安全性に対する挑戦であり得る事例の数は増加している。
したがって、ADAの産生を回避するために、B10細胞組成物を生物製剤療法と併用(生物製剤療法の前後に共投与)することができる。
対象動物でB10細胞機能を評価する方法もまた本明細書で提供される。当該方法は、ex vivoでB細胞を増加させる方法のために、上記で考察した対象動物からB細胞を採集する工程並びにCD40アゴニスト及びIL-21の存在下で該細胞を培養する工程を含む。本明細書に記載の任意のB細胞増加方法を利用して、B10細胞機能を対象動物で評価することができる。該細胞をIL-21と接触させた後、該細胞をアッセイして該細胞がIL-10を産生することができるか否かを決定する。IL-10を生成できる細胞のパーセンテージ及び/又は該細胞によって産生されるIL-10の量を決定できる。該決定は、細胞をIL-21と接触させた後、又は細胞を別の刺激性分子若しくは分子の組み合わせ(例えば抗原又はLPS)と接触させた後で実施できる。IL-10を産生する細胞のパーセンテージ、又は産生されるIL-10の量を当業者に利用可能な任意の方法によって決定できる。
IL-10の量又はIL-10を産生する細胞のパーセンテージを決定し、コントロールと比較することができる。適切には、コントロールは、該対象動物から入手した細胞と同様に処理した健康なドナー由来のB細胞を含む正常コントロールである。また別には、コントロールは、健康なドナー細胞が入ると予想される数値範囲によって示され得る。対象動物のB10細胞機能は、健康なドナーと比較して正常、活性過剰又は不全であり得る。工程(c)のIL-10を産生できるB10細胞のパーセンテージが、正常集団のIL-10を産生できるB10細胞の平均パーセンテージの2標準偏差内、適切には3標準偏差内であるならば、該B10細胞は正常である。工程(c)のIL-10を産生できるB10細胞のパーセンテージが、正常集団のIL-10を産生できるB10細胞の平均パーセンテージよりも2標準偏差を超えて、適切には3標準偏差を超えて低ければ、該B10細胞は不全である。工程(c)のIL-10を産生できるB10細胞のパーセンテージが、正常集団のIL-10を産生できるB10細胞の平均パーセンテージよりも2標準偏差を超えて、適切には3標準偏差を超えて高ければ、該B10細胞は活性過剰である。B10細胞機能が正常でないならば、当該方法を用いて、当該対象動物がB10細胞に影響を及ぼす疾患又は症状を有するか若しくは有する蓋然性が高いと診断するか又は指示することができる。
以下の実施例は例示することのみを意図し、本発明又は添付の特許請求の範囲を制限しようとするものではない。本明細書に引用した全ての参考文献は参照によりその全体が本明細書に組み入れられる。
in vitro増加CD5+B10細胞はそれらの調節機能を維持した。CD5+B10細胞の移入は、野生型マウスでEAE疾患の重症度を、疾患の諸症候の出現後に投与したときでさえ劇的に軽減させ、一方、CD5-B細胞は作用を示さなかった(図4D)。B10細胞のin vitro増加はIL-21及びCD40シグナルの両方を必要としたが、MHC-II発現は要求されなかった(図4E)。しかしながらin vitro増加MHC-II-/-CD5+B10細胞及びIL-10-/-CD5+B細胞はEAEの疾患重症度を調節せず(図4F)、さらにまたT細胞媒介疾患の調節におけるIL-10及び同族相互作用の必要性を立証付けた。B10細胞は、CD19-/-マウス又はMD4トランスジェニックマウス(卵のリゾチームに特異的な固定BCRを有する)由来のB細胞のin vitro培養中に増加せず、さらにまたB10細胞発生におけるBCR特異性及びシグナリングの重要性を強調した。そうでなければ、in vitro増加B10エフェクター細胞は、疾患の開始及び進行の両方における強力な調節因子であった。
我々のトランスフェクト実施間質細胞集団で、我々は、該細胞のわずかなサブセットだけが激しいB細胞接着及びクローン増加を支援できるということを決定した。頻度は、細胞の全てがCD154発現しBLySを分泌するにもかかわらず一般的には間質細胞の1%未満であった。これらの間質細胞培養は一般的には20倍を超えるB細胞増加を誘発できた。しかしながら、機械的単離によって、我々は、上記に記載したように25,000倍までの広い範囲に及ぶB細胞増加を均質的に支援する最適な細胞を単離できた(図4)。最大のB細胞及びB10細胞増殖のために最適化された間質細胞は、表現型的、形態学的及び増殖における特徴を基準にして機械的に単離することができる。例えば、B細胞接着及びロゼット形成を支援する個々の間質細胞を単離することによってB細胞増加を支援するより高い能力を有する間質細胞を濃縮することが可能であった。B細胞増加のために最適な間質細胞を単離するためのまた別の機械的手段には、単一細胞クローニング技術、細胞表面分子の発現又はそれらの喪失を基準にした細胞のフローサイトメトリー単離、及び/又は当業者に公知の他の技術が含まれ、当該技術に続いて最適なB細胞増加を支援する間質細胞の機能的な同定が実施される。
第三の例では、間質細胞のCXCR7転写物が、クローン#1 3T3-Balb/c細胞及びクローン#1 NIH-3T3細胞の両方でダウンレギュレートされる。CXCR7(以前にはRDC1)は、CXCL12(以前にはSDF-1)ケモカインのための受容体として機能する(前記ケモカインはB細胞と結合し、一連の正常プロセス及び病的プロセスを調節できる)。CXCR7はCXCL12のスカベンジャー受容体として機能し、CXCL12はまた両親株の3T3細胞株によって通常的に産生される。したがって、CXCR7発現の間質細胞における低下は、該培養系でCXCL12のB細胞結合を促進し得る。
さらにまた、辺縁帯B細胞発達には、デルタ様1とB細胞発現Notch-2との相互作用が絶対的に要求される。B細胞発達の他の局面は、Notch/デルタ様/ジャグド相互作用によって負の方向に調節される。例えば、デルタ様1を発現する3T3線維芽細胞は脂肪細胞分化のために間質細胞として作用するが、デルタ様発現が抑制されるか又はインターロイキン-7(IL-7)がまた培養に提供される場合には、初期B細胞分化のみを支援する。したがって、B細胞の発達及び機能におけるNotch/デルタ様/ジャグド連合の役割は複雑であるが、これらのタンパク質はB細胞及びB10細胞増加の決定的な調節因子であり得る。

Biol. 677, 99-111。簡単に記せば、単離白血球又は精製細胞を、完全培地(10% FCS、200μg/mLペニシリン、200 U/mLストレプトマイシン、4 mM L-グルタミン及び5x10-5M 2-メルカプトエタノール(いずれもGibco(Carlsbad, CA)より)を含むRPMI 1640培養液)に、LPS(10μg/mL、大腸菌血清型0111: B4, Sigma)、PMA(50ng/mL;Sigma)、イオノマイシン(500ng/mL;Sigma)及びモネンシン(2μM;eBioscience)とともに、48ウェルの平底プレートで5時間再懸濁させた。いくつかの実験では、細胞を48時間、アゴニスト作用をもつ抗マウスCD40 mAb(1μg/mL;HM40-3 mAb;BD Pharmingen)とともに記載にしたがってインキュベートした(Yanaba et al. 2009, J. Immunol. 182, 7459-7472)。IL-10の検出のためには、Fc受容体をマウスFc受容体mAb(2.4G2;BD PharMingen)で封鎖し、細胞表面染色の前にLIVE/DEAD(商標)フィクサブルヴァイオレット死細胞染色キット(LIVE/DEAD(商標)Fixable Violet Dead Cell Stain Kit)(Invitrogen-Molecular Probes)を用いて死細胞を検出した。染色細胞を固定し、さらにCytofix/Cytoperm kit (BD PharMingen)を製造業者の指示にしたがって用いて透過性にし、PE-結合マウス抗IL-10 mAbで染色した。IL-10-/-マウスの脾臓細胞が陰性コントロールとして供され特異性を明示し、さらにバックグラウンドIL-10染色レベルを確立させた。T細胞の細胞内サイトカイン染色のためには、リンパ球を染色前に5時間ブレフェルジンA(BFA、1μL/mL;eBioscience)の存在下でPMA(50ng/mL;Sigma, St. Louis, MO)及びイオノマイシン(1μg/mL;Sigma)でin vitro刺激した。リンパ球のフォワード及びサイド光散乱特性を有する生存細胞を、FACScanフローサイトメーター(Becton Dickinson)又はBD FACSCantoTMII(BD Biosciences)を用いて分析した。

Claims (49)
- IL-10を産生することができるB10細胞をex vivoで増加させる方法であって、対象動物から採集したB細胞をIL-21と接触させる工程及び該B10細胞を単離する工程を含む、前記方法。
- さらに細胞をCD40アゴニストと接触させる工程を含む、請求項1に記載の方法。
- さらにB細胞をCD40アゴニスト及びB細胞生存促進因子を発現するフィーダー細胞と接触させる工程を含む、請求項1に記載の方法。
- さらに最初にB細胞をCD40アゴニスト及びB細胞生存促進因子を発現するフィーダー細胞とIL-4の存在下で接触させる工程を含む、請求項3に記載の方法。
- B細胞生存促進因子がBAFF(BLyS)である、請求項3又は4に記載の方法。

a)対象動物から採集したB細胞をCD40アゴニスト及びB細胞生存促進因子を発現するフィーダー細胞とIL-4の存在下で接触させる工程;
b)工程(a)のB細胞をCD40アゴニスト及びB細胞生存促進因子を発現するフィーダー細胞とIL-21の存在下で接触させる工程;及び
c)工程(b)のB細胞を単離する工程。 - さらに工程(c)のB細胞からB10細胞を単離する工程を含む、請求項6に記載の方法。
- B10細胞が、CD1d、CD5、CD24又はCD27の細胞表面発現について選別することによって単離される、請求項1−5又は7に記載の方法。
- フィーダー細胞がさらにVCAM-1及びCD44を発現する、請求項3−8のいずれか1項に記載の方法。
- フィーダー細胞がさらにCD24、IL-7、MST1又はTs1pの少なくとも1つを発現する、請求項3−9のいずれか1項に記載の方法。
- フィーダー細胞が、コントロールフィーダー細胞よりも少なくとも2倍少ないCD99、CXCR7、Dlk1、Jag1、Notch1又はTGFBIを発現する、請求項3−10のいずれか1項に方法。
- B細胞が、対象動物の血液、脾臓、腹腔、リンパ節、骨髄、自己免疫疾患部位、炎症部位、又は移植物拒絶を示す組織から採集される、請求項1−11のいずれか1項に記載の方法。
- 請求項1−12のいずれか1項に記載の方法であって、B細胞が、該方法で使用する前に、非B細胞の除去、又は細胞表面IgM、IgD、IgG、IgA、IgE、CD19、CD20、CD21、CD22、CD24、CD40、CD72、CD79a、CD79b、CD1d、CD5、CD9、CD10、CD23、CD27、CD38、CD48、CD80、CD86、CD138、CD148、若しくはそれらの組み合わせについての選別によって単離される、前記方法。
- CD40アゴニストが、CD154、CD154のフラグメント、又はCD40と反応する抗体、アプタマー又はポリペプチド、又はそれらのフラグメントである、請求項2−13のいずれか1項に記載の方法。
- B細胞生存促進因子が、フィーダー細胞、BAFF(BLyS)、BAFFフラグメント、APRIL、CD22リガンド、CD22モノクローナル抗体、又はそれらのフラグメントの少なくとも1つから選択される、請求項3−14のいずれか1項に記載の方法。
- フィーダー細胞が、線維芽細胞、内皮細胞、上皮細胞、ケラチノサイト、メラニン細胞、又は他の間葉系若しくは間質細胞である、請求項3−15のいずれか1項に記載の方法。
- B細胞を3から10日間IL-4と接触させる、請求項4−16のいずれか1項に記載の方法。
- B細胞を4から8日間IL-21と接触させる、請求項1−17のいずれか1項に記載の方法。
- B10細胞がIL-10を産生しているか、又はIgM、IgD、IgG、IgA、IgE、CD19、CD20、CD21、CD22、CD24、CD40、CD72、CD79a、CD79b、CD1d、CD5、CD9、CD10、CD1d、CD23、CD27、CD38、CD48、CD80、CD86、CD138、CD148、若しくはそれらの組み合わせを含む細胞表面分子の弁別的発現を基準に単離される、請求項1−5又は7−18のいずれか1項に記載の方法。
- 請求項1−5又は7−19のいずれか1項に記載の方法であって、B10細胞が、該方法の後で単離された全B細胞の10%以上である、前記方法。
- 単離B10細胞の50%超が、IL-10産生能をもつB10細胞である、請求項1−5又は7−19のいずれか1項に記載の方法。
- 請求項1−21のいずれか1項に記載の方法によって製造される細胞を含む組成物。
- 前記細胞の90%超がB10細胞である、請求項22に記載の組成物。
- 請求項22又は23に記載の組成物及び医薬的に許容できる担体を含む医薬組成物。
- 自己免疫異常を有する対象動物を治療する方法であって、請求項22−24のいずれか1項に記載の組成物の治療的に有効な量を、自己免疫異常の治療を必要とする対象動物に投与する工程を含む、前記方法。
- 自己免疫疾患が、多発性硬化症、狼瘡、関節炎、炎症性腸疾患及び強皮症から選択される、請求項25に記載の方法。
- 器官、組織若しくは細胞の移植拒絶又は関連する慢性移植片対宿主病を予防若しくは治療するために対象動物を処置する方法であって、請求項22−24のいずれか1項に記載の組成物の治療的に有効な量を、移植拒絶又は移植片対宿主病の治療を必要とする対象動物に投与する工程を含む、前記方法。
- アレルギー性異常又は炎症性異常を有する対象動物を治療する方法であって、請求項22−24のいずれか1項に記載の組成物の治療的に有効な量を、アレルギー又は炎症の治療を必要とする対象動物に投与する工程を含む、前記方法。
- 炎症性異常が、喘息、脳炎、炎症性腸疾患、慢性閉塞性肺疾患(COPD)、アレルギー性異常、敗血症ショック、肺線維症、未分化型脊椎関節症、未分化型関節症、関節炎、炎症性骨溶解、及び慢性ウイルス性若しくは細菌性感染症に起因する慢性炎症から選択される、請求項28に記載の方法。
- 組換えタンパク質、治療薬タンパク質又は異種タンパク質を受容する対象動物を処置する方法であって、請求項22−24のいずれか1項に記載の組成物の治療的に有効な量を、遺伝性異常、移植、アレルギー、炎症又は自己免疫性異常の治療を必要とする対象動物に投与する工程を含む、前記方法。
- 組成物が自己B細胞を含む、請求項25−30のいずれか1項に記載の方法。
- 106から1010の間でB細胞が組成物で対象動物に投与される、請求項25−31のいずれか1項に記載の方法。
- 組成物が、対象動物で諸症候開始前、その最中、又はその開始後に投与される、請求項25−32のいずれか1項に記載の方法。
- 以下の工程を含む、B10細胞機能を対象動物で評価する方法:
a)対象動物由来のサンプルからB細胞を採集する工程;
b)B細胞をIL-21及びCD40アゴニストと接触させる工程;及び
c)B細胞がIL-10を産生できるか否かを決定する工程。 - IL-21とともに培養する前に、さらにB細胞をCD40アゴニスト及びB細胞生存促進因子を発現するフィーダー細胞とIL-4の存在下で接触させる工程を含む、請求項34に記載の方法。
- 細胞を、CD40アゴニスト及びB細胞生存促進因子を発現するフィーダー細胞とIL-21の存在下で接触させる、請求項34又は35に記載の方法。
- さらに、産生されたIL-10の量又はIL-10を産生できる細胞のパーセンテージを正常なB細胞機能を有するコントロールと比較する工程、及び対象動物でB10細胞機能が正常であるか又は異常であるかを決定する工程を含む、請求項34−36のいずれか1項に記載の方法。
- さらに、対象動物が正常B10細胞、不全B10細胞又は活性過剰B10細胞を有すると診断する工程を含む、請求項34−37のいずれか1項に記載の方法。
- 工程(c)のIL-10を産生できるB10細胞のパーセンテージが、正常集団のIL-10を産生できるB10細胞の平均パーセンテージの2標準偏差内にあれば、該B10細胞は正常であり、工程(c)のIL-10を産生できるB10細胞のパーセンテージが、正常集団のIL-10を産生できるB10細胞の平均パーセンテージよりも2標準偏差を超えて低ければ、該B10細胞は不全であり、さらに工程(c)のIL-10を産生できるB10細胞のパーセンテージが、正常集団のIL-10を産生できるB10細胞の平均パーセンテージよりも2標準偏差を超えて高ければ、該B10細胞は活性過剰である、請求項38に記載の方法。
- B細胞が、対象動物の血液、脾臓、腹腔、リンパ節、骨髄、自己免疫疾患部位、炎症部位、又は移植拒絶を示す組織から採集される、請求項34−39のいずれか1項に記載の方法。
- B細胞が、非B細胞の除去、又は細胞表面IgM、IgD、IgG、IgA、IgE、CD19、CD20、CD21、CD22、CD24、CD40、CD72、CD79a、CD79b、CD1d、CD5、CD9、CD10、CD23、CD27、CD38、CD48、CD80、CD86、CD138、CD148、若しくはそれらの組み合わせについての選別によって単離される、請求項34−40のいずれか1項に記載の方法。
- CD40アゴニストが、CD154、CD154のフラグメント、又はCD40と反応する抗体、アプタマー、ポリペプチド又はそれらのフラグメントである、請求項34−41のいずれか1項に記載の方法。
- B細胞生存促進因子が、フィーダー細胞、BAFF(BLyS)、BAFFフラグメント、APRIL、CD22リガンド、CD22モノクローナル抗体、又はそれらのフラグメントの少なくとも1つから選択される、請求項34−42のいずれか1項に記載の方法。
- フィーダー細胞が、線維芽細胞、内皮細胞、上皮細胞、ケラチノサイト、メラニン細胞、又は他の間葉系若しくは間質細胞である、請求項34−43のいずれか1項に記載の方法。
- B細胞を3から10日間IL-4と接触させる、請求項35−44のいずれか1項に記載の方法。
- B細胞を4から8日間IL-21と接触させる、請求項35−45のいずれか1項に記載の方法。
- さらに、前記比較を、対象動物がB10細胞機能に影響する疾患又は症状を有するか否かを診断するために使用することを含む、請求項38−46のいずれか1項に記載の方法。
- 疾患又は症状が、自己免疫疾患、移植片対宿主病、移植拒絶、抗薬剤抗体、又は炎症性異常である、請求項47に記載の方法。
- 本明細書に実質的に開示及び記載された方法及び組成物。
